BACKGROUND: Doege-Potter syndrome is a rare paraneoplastic phenomenon associated with solitary fibrous tumors of the pleura (SFTPs). It is characterized by the presence of severe, sustained, and treatment-refractory hypoglycemia. Hypoglycaemia, which may be the sole symptom at disease onset, is mediated by the secretion of high-molecular-weight insulin-like growth factor (IGF-2). Most tumors exhibit benign behavior, with a 100% survival rate at 5 years. However, 10% of these tumors may display aggressive behavior with local or metastatic recurrence. We present a clinical case of a patient with a benign solitary fibrous tumor of the pleura who presented with symptomatic hypoglycemia and required pulmonary and pleural surgical resection to control the paraneoplastic phenomenon. CASE PRESENTATION: A Hispanic 46-year-old man presented with a 15-day history of transient alterations in consciousness worsened by fasting. The relevant medical history included obstructive sleep apnea treated with continuous positive air pressure (CPAP) and previous smoking. In-hospital studies revealed noninsulinemic hypoglycemia and a benign SFTP. Complete surgical resection was performed while the patient received dextrose fluids and corticosteroids perioperatively for hypoglycemia. Subsequently, the hypoglycemia resolved, and the patient was followed-up without disease recurrence. CONCLUSION: Doege-Potter syndrome is challenging to recognize. However, effective treatment can be achieved with a high survival rate. Raising awareness among healthcare professionals about the recognition of this paraneoplasic syndrome patients will improve diagnostic suspicion, biochemical confirmation, the development of diagnostic and therapeutic guidelines, and the creation of predictive indices for aggressive presentations requiring closer monitoring.

OBJECTIVE: To assess the long-term outcome of renal oligohydramnios and risk factors for fetal, neonatal, and postneonatal death. STUDY DESIGN: This retrospective cohort study included fetuses with prenatally detected renal oligohydramnios between 2002 and 2023. Patients who were lost to follow-up were excluded. Fetal, neonatal, and long-term outcomes were evaluated, and their risk factors were analyzed. RESULTS: Of 131 fetuses with renal oligohydramnios, 46 (35%) underwent a termination of pregnancy, 11 (8%) had an intrauterine fetal death, 26 (20%) had a neonatal death, nine (7%) had a postneonatal death, and 39 (30%) survived. Logistic regression analyses showed that an earlier gestational age at onset (OR 1.16, 95% CI 1.01-1.37) was significantly associated with intrauterine fetal death; anhydramnios (OR 12.7, 95% CI 1.52-106.7) was significantly associated with neonatal death as a prenatal factor. Although neonatal survival rates for bilateral renal agenesis, bilateral multicystic dysplastic kidney (MCDK), and unilateral MCDK with contralateral renal agenesis were lower than for other kidney diseases, 1 case of bilateral renal agenesis and two of bilateral MCDK survived with fetal intervention. Kaplan-Meier overall survival rates were 57%, 55%, and 51% for 1, 3, and 5 years, respectively. In the Cox proportional hazards model, birth weight <2000 g (hazard ratio 7.33, 95% CI 1.48-36.1) and gastrointestinal comorbidity (hazard ratio 4.37, 95% CI 1.03-18.5) were significant risk factors for postneonatal death. CONCLUSION: Long-term survival following renal oligohydramnios is a feasible goal and its appropriate risk assessment is important.

SARS-CoV-2 is the virus responsible for a respiratory disease called COVID-19 that devastated global public health. Since 2020, there has been an intense effort by the scientific community to develop safe and effective prophylactic and therapeutic agents against this disease. In this context, peptides have emerged as an alternative for inhibiting the causative agent. However, designing peptides that bind efficiently is still an open challenge. Here, we show an algorithm for peptide engineering. Our strategy consists of starting with a peptide whose structure is similar to the interaction region of the human ACE2 protein with the SPIKE protein, which is important for SARS-COV-2 infection. Our methodology is based on a genetic algorithm performing systematic steps of random mutation, protein-peptide docking (using the PyRosetta library) and selecting the best-optimized peptides based on the contacts made at the peptide-protein interface. We performed three case studies to evaluate the tool parameters and compared our results with proposals presented in the literature. Additionally, we performed molecular dynamics (MD) simulations (three systems, 200 ns each) to probe whether our suggested peptides could interact with the spike protein. Our results suggest that our methodology could be a good strategy for designing peptides.

We present the case of a 71-year-old man with a history of type 2 diabetes mellitus manifesting abdominal pain, weight loss, dyspnea on small efforts, muscle weakness, and left subcostal swelling. Associated with this, he developed frequent episodes of syncope, diaphoresis, and night sweats, during which blood glucose levels below 50 mg/dL and improvement were documented after the administration of hypercaloric beverages or intravenous dextrose. Abdominal computed tomography revealed the presence of a tumor of approximately 20 cm dependent on the left adrenal gland. Histopathological diagnosis of a solitary retroperitoneal fibrous tumor was obtained, confirmed by immunohistochemistry.


Se presenta el caso de un varón de 71 años con antecedente de diabetes mellitus tipo 2 que manifiesta dolor abdominal, pérdida de peso, disnea de pequeños esfuerzos, debilidad muscular y aumento de volumen subcostal izquierdo. Asociado a esto desarrolló episodios frecuentes de síncope, diaforesis y sudoraciones nocturnas, durante los cuales se documentaron cifras de glucemia < 50 mg/dl y mejoría tras la administración de bebidas hipercalóricas o dextrosa intravenosa. La tomografía computarizada abdominal reveló la presencia de una tumoración de aproximadamente 20 cm dependiente de la glándula suprarrenal izquierda. Se obtuvo diagnóstico histopatológico de tumor fibroso solitario retroperitoneal confirmado por inmunohistoquímica.

INTRODUCTION AND IMPORTANCE: The Potter sequence is defined as a series of congenital defects related to severe oligohydramnios, associated with polycystic kidney disease, bilateral renal agenesis, pulmonary hypoplasia, obstructive uropathy and premature rupture of membrane, which compromises the life of the neonate sometime after birth. Within the evidence published so far, which is very little, no perforation of the gastrointestinal tract has been reported as a complication of this condition. CASE PRESENTATION: Male neonate born preterm with prenatal diagnosis of pulmonary hypoplasia, polycystic renal dysplasia and severe oligohydramnios (Potter sequence), presented acute respiratory distress syndrome 10 min after birth, requiring mechanical ventilation and admission to the intensive care unit. During her stay in intensive care, he developed abdominal distension and presence of biliary content in the nasogastric tube. An abdominal X-ray was performed and showed signs of pneumoperitoneum, evidencing gastric perforation on exploratory laparotomy. CLINICAL DISCUSSION: Gastric perforation in neonates is a condition that causes high health costs, morbidity, high risk of mortality and disability, regardless of the cause. The management of gastric perforation in Potter syndrome, as well as any other complication, represents a challenge due to the prognosis of these patients. Renal failure and acute respiratory distress syndrome are disorders that compromise the function of various structures and organs such as the heart and brain. CONCLUSION: Gastric perforation is a possible complication of the Potter sequence or syndrome. In addition, there is no literature describing the benefits or disadvantages of specific surgical techniques in the resolution of perforation.

Propósito/Contexto. Este artículo tiene como propósito establecer una relación de complementariedad entre la propuesta o intuición del bioquímico norteamericano Van Rensselaer Potter en torno a la Bioética Global y la teoría de la individuación del filósofo francés Gilbert Simondon, con el fin de entender el papel de los objetos técnicos en la individuación humana. Metodología/Enfoque. El enfoque parte de las "categorías" de concretización y de juicio técnico como puntos clave para mostrar que el proceso de concretización técnica es fundamental para el desarrollo de la intuición bioética potteriana. Es decir, la relación entre la intuición bioética y la teoría de la individuación nos puede posibilitar una nueva relación con la cultura técnica y con la cultura natural de cara a la "supervivencia aceptable" de la especie humana en el planeta. Resultados/Hallazgos. La concretización técnica y el juicio técnico permiten la comprensión de los sistemas a partir de su dinamismo interno y no desde categorías externas, de modo que el juicio técnico no juzga ni valora un sistema, sino que lo organiza para evitar su obsolescencia o, en palabras de V.R. Potter, da lugar a nuevas formas de relacionarnos con los sistemas bióticos y abióticos. Discusión/Conclusiones/Contribuciones. La contribución principal del artículo es destacar la relevancia de una adecuada relación con la técnica y el modo como esta contribuye a nuestra individuación y supervivencia.


Purpose/Context. This article aims to establish a complementary relationship between American biochemist Van Rensselaer Potter's global bioethics proposal or intuition and French philosopher Gilbert Simondon's theory of individuation to understand the role of technical objects in human individuation. Methods/Approach. The approach starts from the "categories" of concretization and technical judgment as critical points to show that the process of technical concretization is fundamental for the development of Potter's bioethical intuition. In other words, the relationship between bioethical intuition and the theory of individuation enables a new relationship with technical culture and natural culture vis-á-vis the "acceptable survival" of the human species on the planet. Results/Findings. Technical concretization and technical judgment allow understanding systems from their internal dynamism and not from external categories; therefore, the technical judgment does not judge or assess a system, but organizes it to avoid its obsolescence or, in Potter's words, gives rise to new ways of relating to biotic and abiotic systems. Discussion/Conclusions/Contributions. The article highlights the relevance of a good relationship with technique and its contribution to our individuation and survival.

Este artigo tem como propósito estabelecer uma relação de complementaridade entre a proposta ou intuição do bioquímico norte-americano Van Rensselaer Potter em torno da Bioética Global e a teoria da individuação do filósofo francés Gilbert Simondon, a fim de compreender o papel dos objetos técnicos na individualização humana. Metodologia/Abordagem. O enfoque parte das "categorias" de concretização e de juízo técnico como pontos-chave para mostrar que o processo de concretização técnica é fundamental para o desenvolvimento da intuição bioética potteriana. Ou seja, a relação entre a intuição bioética e a teoria da individuação pode possibilitar-nos uma nova relação com a cultura técnica e com a cultura natural de face á "sobrevivéncia aceitável" da espécie humana no planeta. Resultados/Descobertas. A concretização técnica e o juízo técnico permitem a compreensáo dos sistemas a partir do seu dinamismo interno e náo a partir de categorias externas, de modo que o juízo técnico náo julga nem valoriza um sistema, mas organiza-o para evitar a sua obsolescencia ou, nas palavras de V.R. Potter, dá lugar para novas formas de nos relacionarmos com os sistemas bióticos e abióticos. Discussão/Conclusões/Contribuições. A contribuição principal do artigo é destacar a relevancia de uma adequada relação com a técnica e o modo como esta contribui para a nossa individuação e sobrevivéncia.

Abstract The integration of field work with DNA barcoding is useful to disentangle lepidopteran interactions in nature. Adults of the little-known geometrid moth Cosmophyga cortesi Vargas, 2008 (Lepidoptera: Geometridae) were reared from folivorous larvae collected on the native tree Schinus areira L. (Anacardiaceae) at about 260 and 1500 m elevation in the transverse valleys of the Atacama Desert of northern Chile. Similarity of two DNA barcodes of C. cortesi was 99.7 and 100% with one sequence of an unidentified geometrid larva previously collected in a cell of the potter wasp Hypodynerus andeus (Packard, 1869) (Hymenoptera: Vespidae). This finding represents the first host plant record for C. cortesi, expands its previously documented altitudinal range by more than 1200 m and confirms that its larvae are captured by females of a potter wasp.

El síndrome paraneoplásico de Doege-Potter (SDP) se asocia a la presencia de tumores intratorácicos y diafragmáticos que generan un cuadro de hipoglucemia por excesivo consumo de glucosa por parte del tumor, y aumento de producción de IGF-2 (insulin growth factor 2) con mayor captación de glucosa periférica. Comunicamos el caso de una paciente femenina de 57 años, sin antecedentes de diabetes ni ingestión de fármacos hipoglucemiantes, que ingresó por un cuadro de hipoglucemia severa con glucemia de 38 mg/dL. Otros resultados: péptido C 1,05 ng/mL, insulinemia 10,5 uUI/mL, IGF-1 de 174 ng/mL, cortisol y prolactina normales. No disponíamos del dosaje de IGF-2. Ecografía abdominal sin patología evidente. Se inició alimentación rica en carbohidratos complejos. Continuó con hipoglucemias. A raíz de cervicalgia, se solicitó radiografía cervical y de tórax, evidenciándose opacidad en vértice izquierdo. La tomografía de tórax informó proceso pulmonar atípico. Por punción se diagnosticó carcinoma pulmonar pobremente diferenciado. Se inició quimioterapia, con reducción del tumor, y de la frecuencia y magnitud de las hipoglucemias. Concluimos que frente a cuadros de hipoglucemia en ausencia de factores causales debería sospecharse la presencia de un SD


Doege-Potter (SDP) is a paraneoplastic syndrome mostly associated with big intrathoracic or diaphragmatic tumors, causing hypoglycemia in non-diabetic patient due to an increased glucose consumption by the tumor cells and excessive production of IGF-2 (insulin growth factor 2). Case report: 57-year-old female patient without diabetes mellitus and not receiving hypoglycemic drugs who was assisted by severe hypoglycemia (38 mg/dL) with the following laboratory tests: C peptide 1.05 ng/mL, 10.5 uIU/mL insulinemia, 174 ng/mL IGF-1, normal cortisol and prolactin. IGF-2 was not available. Non-pathological abdominal ultrasound. In spite of a feeding rich in complex carbohydrate continued with hypoglycaemia. She started with neck pain and a cervical and chest X-ray was requested, which shows opacity in the left vertex. Chest tomography reported atypical lung process. The biopsy reported poorly differentiated carcinoma of the lung. After treatment with chemotherapy a reduction in tumor size and less episodes of hypoglycemia were observed. Conclusion: in the event of recurrent hypoglycaemia and in the absence of known etiologies we should consider the presence of SDP

El Síndrome de "Doege - Potter" es una entidad clínica rara con incidencia baja de difícil diagnóstico, poco conocida alrededor del mundo descrita en 1930, el cual consiste en un tumor intratorácico asociado a hipoglicemi asintomática. El objetivo del presente artículo es exponer un caso de "Síndrome de Doege-Potter", diagnosticado y tratado en el Centro de Especialidades Clínico - Quirúrgicas Jesús Obrero, en un paciente masculino de 55 años; tras la revisión sintomatológica, semiológica/topográfica, estudios imagenologicos, anatomopatologicos, y la revisión sistemática de la literatura internacional se llegó al diagnóstico final. Además se enfocara no solo conocimientos históricos, etiológicos y fisiopatológicos, sino medios diagnósticos estandarizados, que implica tomar en cuenta en esta patología una vez diagnosticada, para poder direccionar el tratamiento más adecuado según los hallazgos y el estado del paciente; siendo esta una manera de aportar en el levantamiento epidemiológico y casuístico de esta variedad rara y poco frecuente patología torácica a nivel mundial.


"Doege - Potter" syndrome is a rare clinical entity with low incidence of difficult diagnosis, little-known around the world described in 1930, which consists of an intrathoracic tumor associated with symptomatic hypoglycemia. The aim of this article is to expose a case of "syndrome of Doege-Potter", diagnosed and treated in the Centre of specialties Clinical - surgical Jesús Obrero, in a 55-year-old male patient; after reviewing symptomatology, / topographic, semiological studies imaging, pathological, and the systematic review of the international literature became the final diagnosis. In addition, focuses not only etiological, historical knowledge and physiopathological, but standardized diagnostic means, which implies taking into account in this disease diagnosed once, to be able to address the most appropriate treatment according to the findings and the patient's condition; this being a way to bring in the epidemiological and case lifting of this rare variety and frequent short thoracic pathology worldwide.

Doege-Potter syndrome with acromegaloid facial changes is extremely rare. Uncooked cornstarch along with glucocorticoids have been used as supportive care in patients with non-islet cell tumor hypoglycemia (NICTH). Preoperative embolization of hepatic solitary fibrous tumors (SFT) with NICTH has yielded unsatisfactory results. Herein we present the case of a 61-year-old man with a 3-month history of severe frequent hypoglycemic episodes and acromegaloid facial changes. During a spontaneous hypoglycemia (26 mg/dL), laboratory values showed a hypoinsulinemic pattern with low levels of GH, IGFPB3, and an IGF2/IGF1 ratio of 8.5:1. Cross-sectional imaging revealed a large (16 × 13 × 11 cm) hepatic tumor, and cytology was consistent with SFT. A preoperative right portal embolization was performed in an effort to induce normal remnant liver hypertrophy to allow for safe tumor resection. After the procedure, uncooked starch treatment followed by prednisone was started, achieving complete remission of hypoglycemic episodes in the preoperative setting. He subsequently underwent partial hepatectomy. The histologic diagnosis was compatible with a potentially malignant SFT. The patient had an excellent outcome with complete remission of hypoglycemia, improvement of facial acromegaloid changes, and no further evidence of disease. To our knowledge, this is the first case of a patient with Doege-Potter syndrome with acromegaloid facial changes induced by a potentially malignant liver SFT, treated successfully with a multimodal approach consisting of uncooked cornstarch, low-dose prednisone, preoperative embolization, and complete surgical resection. The use of cornstarch and low-dose glucocorticoids may be an adequate treatment in advance of undergoing surgery.

RESUMO

Zethus giseleae new species is described from Maranhão State, Northeastern Brazil. The species fits into the Z. hilarianus species-group which was partially revised only recently, and a set of features from both external morphology and male genitalia allows readily separation from closely related taxa. Notes on morphology and collecting records of Z. alessandroi are also provided, extending its range within the Southeast Amazon domain.

A Bioética surgiu em um momento em que a distância entre a ciência e as humanidades parecia intransponível. Em seu surgimento, a Bioética possuía um apelo para superar essa distância que se revelava fatal para o futuro desenvolvimento da humanidade e para a manutenção das condições de vida nesse planeta. É nesse contexto que V. R. Potter propôs seu conceito original de Bioética como uma integração entre a Biologia e os valores humanos - especialmente a filosofia moral - para orientar a sobrevivência humana. Nesse sentido, o presente texto procurará desenvolver o significado do conceito de Bioética para V. R. Potter e o problema das "duas culturas", isto é, da necessidade de construção de um diálogo entre as humanidades e a ciência, buscando uma convergência da relação ética de todos os saberes em torno do cuidado da vida.(AU)


The rise of Bioethics took place at a time when the gap between the sciences and the humanities seemed insurmountable. The appeal of Bioethics was in its mission of overcoming this distance which was proving to be critical for the future development of humanity and for maintaining the conditions for life on the planet. It was in this context that V. R. Potter put forward his original concept of Bioethics as an integration between Biology and human values - particularly moral philosophy - to guide human survival. In this sense, this paper will seek to develop and clarify the meaning of the concept of V. R. Potter's Bioethics and the problem of "two cultures", namely the need for building a dialogue between the humanities and the sciences, seeking a convergence of the ethical relationships of all knowledge around the idea of caring for life.(AU)


La Bioética surgió en un momento en el que la distancia entre la ciencia y las humanidades parecía insalvable. En su aparición, la Bioética presentaba un clamor para superar esa distancia que se revelaba fatal para el futuro desarrollo de la humanidad y para el mantenimiento de las condiciones de vida en ese planeta. Es en ese contexto que V. R. Potter propuso su concepto original de Bioética como una integración entre la Biología y los valores humanos - especialmente la filosofía moral - para orientar la supervivencia humana. En este sentido, este texto buscará desarrollar el significado del concepto de Bioética para V. R. This article aims to explore how literature can help to elaborate psychic experiences, not only for the one who writes it but also for the one who reads it. Based on jungian texts on analytical psychology and literature and focusing the creative process not the interpretation of the work, the present article evaluates how different types of texts (poetry, lyrics and books) from different periods can represent a conflict brought from the author's unconscious or act on the reader's individuation (AU)


Este artículo pretende explorar cómo la literatura puede ayudar a elaborar experiencias psíquicas, no sólo a quien la escribe sino también a quien la lee. En base a los textos junguianos sobre psicología analítica y literatura y con el enfoque principal en el proceso creativo en lugar de en la interpretación de la obra, se evalúa cómo diferentes tipos de textos (poesía, letras de canciones y libros) de distintos periodos pueden representar un conflicto surgido del inconsciente del autor o actuar sobre la individuación del lector (AU)

Este artigo tem como objetivo explorar como a literatura pode auxiliar na elaboração de vivências psíquicas, não apenas para quem escreve como também para quem lê. Com base nos textos de C. G. Jung sobre psicologia analítica e literatura, e mantendo o enfoque principal no processo criativo e não na interpretação da obra, o presente artigo avalia como diferentes tipos de textos (poemas, letras de músicas e livros) de épocas distintas podem representar conflitos surgidos do inconsciente do autor ou atuar na individuação do leitor.

RESUMO

Resumo A relação entre humano e ambiente torna-se problema ético no século XX, quando a aceleração do desenvolvimento econômico e científico acompanhou profundas alterações nos sistemas ecológicos globais. A ética ambiental reivindicou limites na relação dicotômica homem/natureza. Em 1970, Van Potter propôs a bioética como espaço interdisciplinar para o estudo da “sobrevivência humana”. Nos anos seguintes, a disciplina restringiu-se a conflitos clínicos e hospitalares, afastando-se das questões da ética ambiental. Este artigo analisa o conceito de “bioética ambiental” como perspectiva teórica que acompanha um processo de resgate histórico da abordagem de Van Potter da bioética, caracterizado pela perspectiva das dimensões interpessoais, socioeconômicas e políticas dos dilemas éticos ambientais.


Abstract The relationship between humans and the environment became an ethical problem in the twentieth century, when accelerated economic and scientific development was accompanied by profound alterations in global ecological systems. In response, environmental ethics called for limits in the dichotomous relationship between man and nature. In 1970, Van Potter proposed bioethics as the interdisciplinary study of “human survival.” Subsequently, the discipline focused on clinical and hospital conflicts. Environmental bioethics is analyzed in this article as a theoretical perspective that has historically drawn on Van Potter’s approach to bioethics, marked by the interpersonal, socioeconomic, and political dimensions of environmental ethical dilemmas.

Zethus wileyi Stange sp. nov. (Panama) is described and a cladistic analysis is carried in order to determine the relationship between the new species and the other species in the hilarianus species-group. The new species is assigned to its own new species-group due to evident differences in the morphology.

RESUMO

The first checklist of the Peruvian Hymenoptera listed 1169 species and subspecies of aculeate wasps, including 173 species of Pompilidae, seven of Scoliidae, 39 of Sphecidae and 403 of Vespidae. Herein are reported 32 species as new for Peru based mainly on the collection of the Natural History Museum, London. The loss of the endemic status of two species is also reported: Entypus peruvianus (Rohwer) (Pompilidae: Pepsinae) and Omicron ruficolle schunkei Giordani Soika (Vespidae: Eumeninae).
