RESUMO

Osteochondral lesions of the talus (OLTs) are the lesions that affect the articular cartilage and the subchondral bone of the talus. Symptoms develop between 6 and 12 months after the index trauma and are associated with degradation of quality of life. Two-thirds of the lesions (73%) are located on the medial part of the talus, 28% of the lesions are posteromedial, and 31% of the lesions are centromedial. Currently, OLT of up to 100 mm2 can behave in a more indolent condition, and above that area, the defect tends to transmit more shearing forces to adjacent cartilage and is more symptomatic.

Introduction: Legg-Calvé-Perthes disease (LCPD) is the idiopathic osteonecrosis of the capital femoral epiphysis in children. It is a self-healing condition, and the morphology of the hip may vary according to the severity of the disease, among several other factors. The treatment focuses on attempts to prevent femoral head collapse, obtain functional hip motion recovery, and reduce pain. Osteochondritis Dissecans (OCD) of the femoral head has been reported in 2% to 7% of patients diagnosed with healed LCPD. Although OCD may remain asymptomatic, the osteochondral fragment has the potential to become unstable, evolving into symptoms of pain, locking, catching, and snapping. Case report: We present a case report of a ten-year-old boy with an OCD lesion following LCPD who underwent effective osteochondral fixation through the surgical hip dislocation approach. The patient evolved to excellent functional recovery at 1 year post-operatively. Discussion: The surgical hip dislocation approach allows anatomical fixation of the OCD fragment, as well as improvement of hip biomechanics, decreasing pain, improving range of motion and joint congruency, and preserving the native articular cartilage. It also gives the surgeon the opportunity to assess hip stability, femoroacetabular impingement and labral tears, allowing a wide variety of options for the treatment of the healed LCPD. Introdução: A Doença de Legg-Calvé-Perthes (DLCP) é a osteonecrose idiopática da epífise femoral proximal em crianças. É uma condição auto resolutiva, porém a morfologia final do quadril pode variar de acordo com a gravidade da doença. O tratamento concentra-se na tentativa de prevenir o colapso da cabeça femoral, obtendo recuperação funcional do movimento do quadril e redução da dor. A osteocondrite dissecante (OCD) da cabeça femoral foi relatada em 2% a 7% dos pacientes diagnosticados com DLCP já curada. Embora a OCD possa permanecer assintomática, o fragmento osteocondral tem potencial para se tornar instável, evoluindo para sintomas de dor, bloqueio, impacto e estalido. Relato de caso: Apresentamos o relato de caso de um menino de 10 anos com OCD da cabeça femoral após DLCP, submetido à fixação osteocondral do fragmento por meio da abordagem cirúrgica de luxação do quadril. O paciente evoluiu com excelente recuperação funcional 1 ano após a cirurgia. Discussão: A abordagem cirúrgica da luxação do quadril permite a fixação anatômica do fragmento da OCD, bem como a melhora da biomecânica do quadril, diminuindo a dor, melhorando a amplitude de movimento e a congruência articular e preservando a cartilagem articular nativa. Também dá ao cirurgião a oportunidade de avaliar a estabilidade do quadril, impacto femoroacetabular e lesões labrais, permitindo uma ampla variedade de opções para o tratamento das sequelas da DLCP. PURPOSE: To perform a scoping review to identify the available evidence regarding osteochondritis dissecans (OCD) of the temporomandibular joint (TMJ). METHODS: An electronic search of the PubMed, Web of Science and Scopus databases was performed using the following terms: 'Temporomandibular Joint Disorders', 'Osteochondritis Dissecans', 'Joint Loose Bodies' and 'Temporomandibular Joint'. Full-text articles were obtained from the records after applying the inclusion/exclusion criteria. RESULTS: Ten articles were included in the analysis - six case reports, one case series, one retrospective study, one comparative study and one correlational study - with a total of 39 patients. The most frequently reported clinical presentation involved TMJ pain, locked jaw and articular noises (clicking and crepitus). The imaging methods used to identify OCD were radiographs, tomography, arthrography and magnetic resonance imaging. The reported imaging findings varied widely, but the most frequent were (single or multiple) calcified intra-articular loose bodies, signs of degenerative osseous changes, disc displacements, widening of the joint space and alterations in condylar morphology. Seven articles reported treatments (surgical or conservative), but the treatment outcome was not reported in all of the articles, which makes it difficult to make comparisons. CONCLUSION: OCD of the TMJ may present various non-specific clinical characteristics, and given the heterogeneous imaging findings, multiplanar images are required for an accurate diagnosis. Finally, the results do not allow recommending a standard treatment for OCD of the TMJ.

OBJECTIVE: To evaluate pathological changes in fossils from the Brazilian Intertropical Region (BIR), expanding the records of previously reported diseases for representatives of the Quaternary South American megafauna, including taxa not studied in previous works. MATERIALS AND METHODS: We carried out a thorough macroscopic analysis of fifteen unpublished specimens belonging to representatives of the Quaternary megafauna of BIR to identify evidence of pathological alterations. RESULTS: Alterations included: osteophytes in Toxodontidae, Megatheridae and E. laurillardi; rough subchondral bone, bone overgrowth and bone erosion in E. laurillardi; slit-shaped subchondral depressions in Equidae and E. laurillardi; and a triangular-shaped porous lesion in Mylodontidae. CONCLUSIONS: The alterations found allowed the recognition of the first cases of osteoarthritis for Toxodontidae and articular depressions for Equidae, and new cases of both diseases in Eremotherium laurillardi; a new case of osteochondritis dissecans for Mylodontidae; potential new cases of calcium pyrophosphate deposition and spondyloarthropathy for E. laurillardi SIGNIFICANCE: Our results provide additional evidence that calcium pyrophosphate deposition disease was widely spread among species of the South American megafauna and suggest that osteochondritis dissecans may have been relatively common among ground sloths. LIMITATIONS: The identification of calcium pyrophosphate deposition and spondyloarthropathy in E. laurillardi are quite tentative because the evidence found is ambiguous and the number of examined specimens is limited.

RESUMO

Reconstruction of unstable osteochondritis dissecans lesions of the capitellum using fresh osteochondral allograft transplantation from the capitellum has the advantages of restoring hyaline cartilage, matching the native radius of curvature, and avoiding the donor-site morbidity encountered with osteochondral autograft transfer. This technical note describes the indications and contraindications, pertinent anatomy, and surgical technique of open osteochondral allograft transplantation using fresh distal humerus allograft for the treatment of unstable osteochondritis dissecans lesions of the capitellum.

This study aimed to characterize the clinical and arthroscopic presentations of horses with osteochondral fragmentation at the metacarpophalangeal and metatarsophalangeal joints at the Veterinary Hospital of the University of São Paulo in a 10-year period (2010-2019). The hind limbs were affected similarly to the plantar and dorsal fragments from the first phalanx, while the thoracic limbs had a more frequent occurrence on the dorsal aspect of the joint. The Brazilian Sport Horses were the most frequently observed breed. The degree of lameness was variable and might be related to fragment localization, animal age, and physical activity. There was a significant statistical difference between the joint effusion and degree of lameness; therefore, it can be considered that the joint effusion is related to degenerative joint processes and might be triggered by the presence of the osteochondral fragment. Surgical correction, especially when performed in young animals using an arthroscopic procedure is probably the best treatment option for most cases, possibly acting as a minimizer of the occurrence of secondary joint degeneration. Prospective studies of specific breeds with a larger number of animals can expand this clinical characterization and enable a better understanding of the animal's recovery after surgical or conservative management.


O objetivo deste estudo foi caracterizar a apresentação clínica e artroscópica de equinos com fragmentação osteocondral em articulação metacarpo/metatarsofalangeana atendidos no Hospital Veterinário da Universidade de São Paulo, no período de 10 anos (2010-2019). Os membros pélvicos são mais acometidos com fragmentos plantaromediais da primeira falange, enquanto os membros torácicos apresentam uma caracterização de ocorrência de fragmentação dorsal da primeira falange, sendo o Brasileiro de Hipismo a raça de maior ocorrência nesta casuística. O grau de claudicação foi variável e pode relacionar-se com a localização do fragmento, a idade e o nível de atividade atlética desenvolvida. Houve relação estatística significativa entre a presença de efusão articular e o grau de claudicação, por isso suscita-se sobre a efusão relacionar-se com processos degenerativos articulares que podem ser desencadeados pela presença do fragmento osteocondral. A correção cirúrgica por meio de procedimento artroscópico provavelmente consiste na melhor opção de tratamento para a maioria dos casos, principalmente quando realizada em animais jovens, possivelmente atuando como minimizador da ocorrência de degeneração articular secundária. Estudos prospectivos de raças específicas e com um número maior de animais podem ampliar essa caracterização clínica e possibilitar um entendimento sobre a evolução após a opção ou não pelo tratamento cirúrgico.

BACKGROUND: Juvenile osteochondritis dissecans (JOCD) is an acquired joint disease of unknown etiology that affects skeletally immature patients and has the potential to progress to osteoarthritis. Recent studies have reported a high prevalence of vitaminD deficiency in patients with osteochondral lesions. The aim of our study was to determine serum vitaminD levels in patients diagnosed with JOCD. METHODS: Serum 25(OH)D levels of 31 patients (22 males) presenting 40 lesions (29 JOCD of the knee, and 11 of the ankle) were evaluated. The average age was 11.9±2.9years. HypovitaminosisD was defined as a value less than 30ng/mL and was divided into vitaminD insufficiency (20 to 30ng/mL) and vitaminD deficiency (<20ng/mL). RESULTS: HypovitaminosisD was present in 45.2% of the evaluated patients (32.2% insufficiency and 13% deficiency). No significant differences were found in the mean values and incidence of hypovitaminosis between those patients in which the sample was taken in warm or cold season (P=.267 and P=.875, respectively). Patients who required surgery had a higher incidence of hypovitaminosis than those treated conservatively (60% versus 31%, P=.054). There was no correlation in the incidence of hypovitaminosis with sex, location, stability of the lesion, or if the lesion was uni- or bilateral. CONCLUSION: In our series, almost half of the patients diagnosed with JOCD presented abnormal serum levels of vitaminD. A two-fold incidence of hypovitaminosis was observed in patients requiring surgical treatment compared to patients managed conservatively. The association found in this study does not imply causation, but it should be considered within the set of actions for the treatment of these injuries.

This study aimed to investigate the prevalence of juvenile osteochondral conditions (JOCCs) in yearling Thoroughbred (TB) racehorses in Southern Brazil, as well as to examine the same animals studied as weanlings by Bastos et al. (2017) and compare the radiographic findings for the two ages. Radiographs of 76 male and female TB yearlings from Paraná State, Southern Brazil, were investigated. The proximal interphalangeal, metacarpophalangeal/metatarsophalangeal (MC/MT), tarsal, and femorotibial (FT) joints were evaluated using 24 radiographic projections. The evaluation consisted of a severity index (rated as 0, 1, 2, 4, or 8), the sum of which resulted in an osteoarticular status (OAS) of good, intermediate, or poor for each animal. Radiographs of 92% of the investigated animals presented at least one finding consistent with JOCCs. The most affected region was the tarsal joint (72.9%), followed by the FT (50%), MT (25%), and MC (23%) joints. Thirty-three (43.4%) horses presented a decline in their OAS, whereas nine (11.8%) presented an improvement from the weanling to yearling age. The radiographic findings suggest that JOCCs are frequent in TB yearlings, but the meaning and relevance of these radiographic findings require further study.

Juvenile osteochondritis dissecans (JOCD) is a joint disorder of the subchondral bone and articular cartilage that affects skeletally immature patients.The aetiology of JOCD is unknown and the natural history is poorly characterized in part due to inconsistent and largely retrospective literature.Most OCD in children and adolescents presents as a stable lesion amenable to non-operative treatment or minimally invasive drilling. However, unstable forms can require a more aggressive approach.This article reviews the most recent literature available and focuses on the pathophysiology, diagnosis and treatment of JOCD of the knee. Osteochondritis dissecans (OCD) is a condition that corresponds to an idiopathic focal lesion affecting the subchondral bone with possible compromise of the stability of the adjacent cartilage. Treatment depends on the size of the lesion, cartilage stability, and the physeal status. The case reported is about an 18-year-old male patient who complained of suffering from knee pain for a period of ten months. Magnetic resonance imaging (MRI) revealed a lesion of 2 cm2 in the medial femoral condyle that compromised the subchondral bone, compatible with OCD. He underwent surgery that consisted of filling the subchonral defect with an iliac crest autograft and sealing the defect with a hyaluronic acid scaffold. At the 12-month follow-up, the MRI shows complete healing and the patient has resumed sports activities. Management with autologous iliac crest graft and hyaluronic acid scaffold represents an effective alternative treatment for OCD.

Objetivo: actualizar los conocimientos sobre el injerto osteocondral autólogo en la modalidad de mosaicoplastia. Métodos: la búsqueda de la información se realizó en el mes de octubre de 2016, a partir de la información obtenida se realizó una revisión bibliográfica de un total de 654 artículos publicados en las bases de datos PubMed, Hinari, SciELO y Medline mediante el gestor de búsqueda y administrador de referencias EndNote, de ellos se utilizaron 52 citas seleccionadas para realizar la revisión, 47 de ellas de los últimos cinco años, donde se incluyeron cuatro libros. Desarrollo: se describen los elementos necesarios para el diagnóstico clínico e imaginológico, en relación a estos últimos se hace énfasis en la imagen de resonancia magnética y la artroscopia. Se hace referencia a la clasificación de la sociedad internacional para la reparación de los cartílagos. Se describen las indicaciones quirúrgicas generales de las lesiones de cartílago y las específicas de la mosaicoplastia con sus principales ventajas y desventajas. Se plasman las principales complicaciones relacionadas con el procedimiento y los resultados de un grupo de investigaciones. Conclusiones: la mosaicoplastia es una variedad de injerto autólogo osteocondral empleada con buenos resultados en la actualidad, tiene ventajas, desventajas e indicaciones específicas.


Background: cartilage lesions are common nowadays. Their main symptoms and signs are pain and limited joint range of motion. There are many ways to treat them. Mosaicplasty is very used because of its several advantages. Objective: to update knowledge of autogenous osteochondral transplantation in mosaicplasty modality. Methods: a search in the databases PubMed, Hinari, SciELO and Medline was conducted through the information locator EndNote in October 2016. From the information obtained, a bibliographic review was carried out with 654 articles. Among them, 52 citations were used and 47 of them from the last five years, including four books. Development: fundamental elements for clinical and imaging diagnosis were described. With regard to the latter, it is emphasized on magnetic resonance imaging and arthroscopy. The international cartilage repair society classification system was pointed out. General surgical indications for cartilage lesions were described as well as specific ones for mosaicplasty and its main advantages and disadvantages. Complications were highlighted as well as the results of a group of researchers. Conclusions: mosaicplasty is a modality of autogenous osteochondral transplantation with good results because of its many advantages and disadvantages and specific indications to do it.

RESUMO

A osteocondrite dissecante (OCD) é um processo patológico que atinge o osso subcondral do joelho em criança e adolescente (OCDJ) e adultos jovens (OCDA) com efeitos secundários sobre a cartilagem articular com dor, edema, possível formação de corpos livres e sintomas mecânicos, inclusive bloqueio articular. A OCD pode levar a alterações degenerativas precoces da articulação, quando não tratada. Este artigo apresenta uma revisão e atualização sobre o problema com ênfase especial no diagnóstico e tratamento. Este pode incluir os métodos conservadores que mostram resultados mais satisfatórios para a OCDJ e os vários métodos cirúrgicos que incluem: técnicas reparativas como a remoção isolada do fragmento, as perfurações ósseas e a fixação do fragmento osteocondral e as técnicas restaurativas como as microfraturas, o transplante osteocondral autólogo (mosaicoplastia), o implante autólogo de condrócitos e o aloenxerto osteocondral fresco, considerando a lesão estável ou instável e sua viabilidade, bem como a maturidade esquelética e localização do processo. Estudos recentes para a avaliação dos resultados dos vários tipos de tratamento demonstram a falta de estudos com níveis de evidência confiáveis e sugere-se maior número de análises multicêntricas, prospectivas, randomizadas e controladas para estabelecer melhores diretrizes para o manuseio da doença.


Osteochondritis dissecans (OCD) is a pathological process affecting the subchondral bone of the knee in children and adolescents with open growth plates (juvenile OCD) and young adults with closed growth plates (adult OCD). It may lead to secondary effects on joint cartilage, such as pain, edema, possible formation of free bodies and mechanical symptoms, including joint locking. OCD may lead to degenerative changes may develop if left untreated. This article presents a review and update on this problem, with special emphasis on diagnosis and treatment. The latter may include either conservative methods, which show more predictable results for juvenile OCD, or various surgical methods, which include reparative techniques like isolated removal of the fragment, bone drilling and fixation of the osteochondral fragments, and restorative techniques like microfractures, autologous osteochondral transplantation (mosaicplasty), autologous chondrocyte implantation and fresh osteochondral allograft, depending on lesion stability, lesion viability, skeletal maturity and OCD process location. Recent assessments on the results from several types of treatment have shown that there is a lack of studies with reliable levels of evidence and have suggested that further multicenter prospective randomized and controlled studies on management of this disease should be conducted.
