Introducción: Los trabajadores de salud, especialmente enfermeras y enfermeros, laboratoristas y paramédicos tienen mayor riesgo de exposición a lesiones por pinchazo de aguja, en ocasiones, por un inadecuado manejo y almacenamiento de desechos hospitalarios. Objetivo: Describir las causas de lesión por pinchazo de aguja y las medidas de prevención tomadas por los trabajadores de salud. Métodos: Se realizó un estudio de corte transversal utilizando una encuesta en línea; participaron 74 trabajadores de salud del Hospital Nacional Almanzor Aguinaga Asenjo y el Hospital Regional Lambayeque; los datos están presentados como estadística descriptiva; para el análisis estadístico se utilizó InfoStat 2019. Resultados: La edad media de los encuestados fue de 28,9 años (rango de 17-48); la mayoría fueron mujeres (56,8 %), y el 43,2 %, varones. Del total de encuestados (42, 56,8 %), alguna vez tuvo una lesión por pinchazo de aguja, mientras, el 43,2 % no ha recibido lesiones. La severidad del pinchazo de aguja fue leve en 51,4 % de los casos, 6,8 % fue moderada y 41,9 % no tuvo lesión. Conclusiones: Las principales modalidades causantes de pinchazo de aguja encontradas en este estudio fueron: reencapuchar la aguja, extracción de sangre venosa, el movimiento repentino del paciente durante una inyección o una sutura. Una capacitación continua, el uso de equipos de protección personal y seguir las pautas de eliminación adecuada de los objetos cortopunzantes, ayudarían a prevenir este tipo de lesiones en trabajadores de salud y por ende, evitar la transmisión de infecciones a través de sangre percutánea.


Introduction: healthcare workers, especially nurses, laboratory workers and paramedics have a higher risk of exposure to needle-stick injuries, which is sometimes due to inadequate handling and hospital waste storage. Objective: to describe causes of needle-stick injury and preventive measures taken by healthcare workers. Methods: a cross-sectional study was conducted using an online survey; 74 healthcare workers from the Almanzor Aguinaga Asenjo National Hospital and the Lambayeque Regional Hospital participated; data are presented as descriptive statistics; InfoStat 2019 was used for the statistical analysis. Results: the mean age of the respondents was 28.9 years (range 17-48); the majority were female (56.8%), and 43.2%, male. The 56.8% from the total number of respondents (42) had ever had a needle-stick injury, while 43.2% had not received injuries. The severity of the needle stick was mild in 51.4% of the cases, 6.8% was moderate and 41.9% had no injury. Conclusions: needle recapping, venous blood withdrawal and patients' sudden movement during an injection or a suture were the main causes of needle stick found in this study. A continuous training, the use of personal protective equipment, and following the proper disposal guidelines for sharp objects would help health workers to prevent this type of injury, and thus to avoid the transmission of infections through percutaneous blood.

ABSTRACT Objective: To evaluate the prevalence of sharps accidents among dental students in southwest Goiás state, Brazil, and further survey their knowledge of biosafety and post-injury management. Material and Methods: A cross-sectional analytical observational study was carried out in 2018 following the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) guidelines. The study population included dental students in the 4th and 10th course semester. A pre-formulated self-administered questionnaire containing 14 objective questions was used for data collection. Mean and standard deviation values were calculated. Results: A total of 308 responses were obtained. Overall, 15.9% of the respondents reported having previously experienced accidents with sharps. Most dental students who claimed to know the biological risks to which they are exposed were in the 5th and 8th course semesters, and 67.2% of them reported knowing how to proceed in the event of a sharp accident. Conclusion: A low prevalence of sharps accidents has been reported, and dental students are considered to have a good knowledge of biosafety.

Mpox es una zoonosis vírica que causa síntomas similares a la viruela, aunque menos graves. La infección fue descrita inicialmente en África central y occidental. Luego del brote multinacional ocurrido el año 2022, ya no es considerada una emergencia de salud pública de importancia internacional. El mecanismo de transmisión es por contacto físico estrecho o directo con lesiones cutáneas de individuos infectados. Presentamos el caso clínico de una enfermera que se infectó por mpox tras un accidente cortopunzante durante la toma de muestra de una lesión por desteche con bisturí en un paciente con VIH. La transmisión percutánea tuvo un período de incubación corto, seguido de una lesión cutánea y síntomas sistémicos. Aunque infrecuente, se destaca el riesgo de transmisión ocupacional de mpox en la atención clínica. Es importante que el personal sanitario adhiera estrictamente a las medidas de prevención, como el uso de equipo de protección personal y la práctica segura en la toma de muestra.


Monkeypox is a viral zoonosis that causes symptoms similar to smallpox, but less severe. The infection was initially described primarily in central and western Africa. After multi-country outbreak in 2022; it is currently no longer a public health emergency of international concern. The main mode of transmission is through close or direct contact with the skin lesions of an infected individual. We report a case of a nurse was infected with mpox after a needlestick injury during a skin sample collection from an HIV-positive patient. Percutaneous transmission resulted in a short incubation period, followed by a skin lesion and systemic symptoms. This case highlights the risk of occupational transmission of mpox in healthcare settings. It is important for healthcare workers to take rigorous prevention measures, such as the use of appropriate personal protective equipment and safe sample collection practices.

NEED: Counting surgical sharps in surgeries mitigates the risk of retained surgical items in patients. Current procedures rely on manual counts throughout and at the end of each surgery. The manual count is lengthy, burdensome, and carries the risk of injuries. TECHNICAL SOLUTION: This paper presents the design and early evaluation of a technology aid to automatize the sharps' counting process; it is composed of a shell holding a set of optical sensors that count needles that pass through the shell's top slot. PROOF OF CONCEPT: The device was tested with needles originating from 20 surgeries. Users surveyed reported a decrease in counting time and perceived injury risk. The average count error was 4.4%, indicating the need to continue advancing the technology. NEXT STEPS: The counting technology needs to be refined to increase accuracy, and a user guide for the equipment must be prepared. Thereafter, a new set of trials must be performed. CONCLUSION: Our work provides a detailed requirements list for developing a technology aid to improve the needle count process and investigates one possible technological route to address this problem.

Objetivo: analisar o perfil dos acidentes ocorridos com materiais perfurocortantes entre a equipe de enfermagem notificados em dois hospitais. Métodos: estudo documental e descritivo realizado em um hospital de ensino e um filantrópico do interior paulista. Os dados foram coletados em fichas de notificação de acidentes com material biológico envolvendo profissionais da enfermagem entre 2016 e 2020, e analisados por meio de estatística descritiva. Protocolo de pesquisa aprovada pelo comitê de Ética em Pesquisa da instituição envolvida. Resultados: foram identificados 208 acidentes: 187 (89,9%) no hospital filantrópico e 21 (10,1%) no hospital de ensino. No hospital filantrópico e no hospital universitário foram notificados, respectivamente, 119 (63,64%) e 35 (18,71%) acidentes entre os técnicos de enfermagem; sendo 51 (27,27%) e 8 (38,1%) durante descarte de perfurocortante. Agulhas causaram 166 (79,8%) acidentes. Conclusão: os acidentes nos dois hospitais ocorreram majoritariamente por agulhas, no momento de descarte de perfurocortante, acometendo principalmente os técnicos em enfermagem.


Objective: to analyse the accidents with sharps profile among the nursing staff reported at two hospitals. Needles caused 166 (79.8%) accidents. Conclusion: at both hospitals, most accidents involved needles, at the time of sharps disposal, and mainly affected nursing technicians.

Resultados: se identificaron 208 accidentes: 187 (89,9%) en el hospital filantrópico y 21 (10,1%) en el hospital universitario. En el hospital filantrópico y en el hospital universitario fueron relatados 119 (63,64%) y 35 (18,71%) accidentes, respectivamente, entre técnicos de enfermería; siendo 51 (27,27%) y 8 (38,1%) durante la eliminación de objetos cortopunzantes. Las agujas provocaron 166 (79,8%) accidentes. Conclusión: los accidentes en ambos hospitales ocurrieron en su mayoría por agujas, en el momento de la eliminación de objetos cortopunzantes, afectando principalmente a los técnicos de enfermería.

RESUMO

We describe monkeypox virus (MPXV) transmission from a patient to a healthcare worker through needlestick injury. A lesion appeared at the inoculation site 5 days after injury. Blood tested MPXV-positive by PCR before symptoms worsened; blood remained MPXV-positive at discharge 19 days after symptom onset. Postexposure prophylaxis could prevent potential MPXV bloodborne transmission.

Abstract Objective: To identify factors related to tissue complications resulting from insulin therapy. Method: This is a descriptive, cross-sectional study carried out in a capital of northeastern Brazil. A semi-structured form and an observation guide were applied to assess the performance of insulin preparation and administration techniques. Descriptive statistics, association test, and multivariate logistic regression were used for data analysis. Results: Most participants were female (74.2%), aged between 51 and 70 years (50.0%), and had nine to eleven years of education (36.7%). The presence of some type of local complication in 73.5% of the participants and the failure to rotate the injection sites in 82.3% are highlighted. Being single/widowed and not rotating insulin application sites were related to local complications and increased the chances of their occurrence by 3.51 and 6.70 times, respectively. Conclusion: Marital status and nonrotation of injection site were related to the increased chances of tissue complications resulting from insulin therapy.


RESUMEN Objetivo: Identificar factores relacionados con la aparición de complicaciones tisulares resultantes de la terapia con insulina. Método: Estudio descriptivo, transversal, realizado en la capital del noreste de Brasil. Se aplicó un formulario semiestructurado y un guión de observación para evaluar el desempeño de las técnicas de preparación y administración de insulina. Para el análisis de los datos se utilizó estadística descriptiva, prueba de asociación y regresión logística multivariante. Resultados: La mayoría de los participantes eran mujeres (74,2%), con edades comprendidas entre 51 y 70 años (50,0%) y con nueve a once años de escolaridad (36,7%). Se destaca la presencia de algún tipo de complicación local en el 73,5% de los participantes y mala rotación de los sitios de inyección en el 82,3%. Ser soltero/viudo y no rotar los sitios de aplicación de insulina se relacionaron con complicaciones locales y aumentaron la posibilidade de su ocurrencia en 3,51 y 6,70 veces, respectivamente. Conclusión: Se demostró que el estado civil y la mala rotación de los sitios de aplicación estaban relacionados con el aumento de las posibilidades de complicaciones tisulares como resultado de la terapia con insulina.


RESUMO Objetivo: Identificar os fatores relacionados à ocorrência de complicações teciduais decorrentes da insulinoterapia. Método: Estudo descritivo, transversal, realizado em capital do nordeste brasileiro. Aplicaram-se formulário semiestruturado e roteiro de observação para avaliação do desempenho das técnicas de preparo e administração de insulina. Estatística descritiva, teste de associação e regressão logística multivariada foram utilizadas para análise dos dados. Resultados: A maioria dos participantes era do sexo feminino (74,2%), com idades entre 51 e 70 anos (50,0%), e nove a onze anos de estudo (36,7%). Destaca-se a presença de algum tipo de complicação local em 73,5% dos participantes e a não realização do rodízio dos locais de injeção em 82,3%. Estar solteiro/viúvo e não realizar rodízio dos locais de aplicação da insulina relacionaram-se às complicações locais e aumentaram as chances de ocorrência destas em 3,51 e 6,70 vezes, respectivamente. Conclusão: O estado civil e a não realização de rodízio dos locais de aplicação mostraram-se relacionados ao aumento das chances de complicações teciduais decorrentes da insulinoterapia.

ABSTRACT Objectives: to analyze the temporal trend of accidents due to percutaneous exposure in a public hospital in Brazil, between 2007 and 2019, according to sociodemographic and professional characteristics. Methods: analysis of time series of accidents due to percutaneous exposure that occurred in health workers. Sociodemographic and professional variables, accident profile, post-accident behavior and accident incidence rates were evaluated. The Prais Winsten regression was used for trend analysis and calculation of the annual percentage change, with a significance level of 5%. Results: 761 occupational accidents were recorded. There was a downward trend in the rate of percutaneous injuries among female workers (-0.012%; p=0.009), who had secondary education (-0.011%; p=0.035) and among all health professional categories (-0.010%; p =0.019). There was an increasing trend (0.018%; p= 0.050) among workers with ≥ 61 months of professional experience. Conclusions: the analysis showed a decreasing incidence of percutaneous accidents, which can be explained by multiple factors.


RESUMEN Objetivos: analizar tendencia temporal de accidentes por exposición percutánea en un hospital público brasileño, entre 2007 y 2019, segundo características sociodemográficas y profesionales. Métodos: análisis de series temporales de los accidentes por exposición percutánea ocurridos en trabajadores de salud. Fueron evaluadas las variables sociodemográficas, profesionales, perfil de los accidentes, conductas post-accidentes y tasas de incidencia de los accidentes. La regresión de Prais-Winsten fue empleada para análisis de tendencia y cálculo de la variación porcentual anual, con nivel de significación de 5%. Resultados: fueron registrados 761 accidentes ocupacionales. Hubo tendencia decreciente de la tasa de accidente percutáneo en trabajadores del sexo femenino (-0,012%; p=0,009), que poseían enseñanza media (-0,011%; p=0,035) y entre todas categorías profesionales de salud (-0,010%; p=0,019). Observado tendencia creciente (0,018%; p= 0,050) entre trabajadores con tiempo ≥ 61 meses de experiencia profesional. Conclusiones: el análisis evidenció incidencia decreciente de accidentes percutáneos, que puede ser explicada por múltiples factores.

Objetivo: Describir en un caso clínico una nueva técni- ca para la localización y la remoción de agujas fracturadas du- rante la anestesia odontológica mediante planificación virtual. Caso clínico: Una paciente de género femenino de 52 años de edad concurre a la Cátedra de Cirugía y Traumatolo- gía Bucomaxilofacial I de la Facultad de Odontología de la Universidad de Buenos Aires y relata que dos meses atrás, durante la atención odontológica, se produjo la fractura de la aguja durante la anestesia troncular mandibular. Se realiza diagnóstico y planificación virtual para conocer la ubicación exacta de la aguja y se confecciona un modelo estereolito- gráfico y una guía quirúrgica individualizada para removerla. El uso de una guía quirúrgica individualizada y confeccio- nada mediante planificación virtual permitió ubicar la aguja tridimensionalmente y con mayor precisión en espacios pro- fundos y disminuir tiempos operatorios (AU)


Aim: To describe in a clinical case a new virtual plan- ning technique for locating and removing a fractured dental anesthetic needle. Clinical case: A 52-year-old patient visited the De- partment of Oral and Maxillofacial Surgery I (School of Dentistry, University of Buenos Aires) with a retained den- tal needle in the pterygomandibular space. The needle had fractured during inferior alveolar nerve block two months previously. Virtual diagnosis and planning were performed to locate the needle and a stereolithographic model and a customized surgical guide were prepared. The use of cus- tomized surgical guides prepared by virtual planning ena- bled precise location of the dental needle in deep spaces and reduced operating times (AU)

BACKGROUND: Newly educated RNs are vulnerable to needle stick injuries (NSIs). Most needle stick injuries occur during practice and associated with nurses' practice and available resources. METHODS: This was a quantitative, descriptive study. Registered nurses (N = 120) with less than three years' experience. Data collected using a 26-item questionnaire among newly registered nurses. Data were analyzed descriptively and inferentially using (SPSS) version 20. RESULTS: Most of the nurses between the ages 20-30 years (49.7%), were female (73.4%), had 2 to <3 years' experience (44.2%) and experienced a needle stick injury (43.3%). Respondents knew about the policies on standard precautions but their practice was not compliant with safe practices. Adherence was associated with knowledge of policies on standard precautions (r. = 534, p < .01), having updated policies (r = .404, p < .01) and the appropriate use of resources (r. = 805, p < .01). CONCLUSION: There are a number of factors that contribute to NSIs among newly registered nurses which are personal as well as structural. Avoidable practices such as non-adherence to standard precautions while using hypodermic needles contributed to NSIs. Mandatory training and evaluation of nurses' knowledge and attitude regarding safety practices should be examined.

RESUMO

We report a fatal case of Rocky Mountain spotted fever (RMSF) in a man in Brazil without recent history of tick bites or environmental exposure. He received an accidental needlestick while working as a nurse. The nurse and his patient died. Both cases were confirmed as RMSF by molecular methods.

BACKGROUND: Clinical laboratory workers are at permanent risk of suffering an occupational accident due to exposure to biohazard. These accidents may cause severe diseases, such as HIV, HBV or HCV. OBJECTIVE: To describe the behavior of accidents due to occupational exposure to biohazard agents, and the characteristics of reports of occupational accidents in clinical laboratories in Yopal, Colombia. MATERIAL AND METHODS: Observational descriptive study with exploratory component and retrospective analysis of incident report forms of presumed occupational accidents. RESULTS: For the period between January 1, 2016, to December 31, 2018, an occupational accident percentage of 24.6 (95% confidence interval [95% CI], 16.7-32.5) was estimated, and an underreporting percentage of 39.3 (95% CI, 21.3-57.3). The main reasons for not reporting were a perceived lack of time (45.4%), and a diminished perception of risk exposure (36.4%). In contrast, factors associated with the probability of having less occupational accidents included being hired for service time (prevalence ratio [PR] = 0.19; 95% CI, 0.04-0.89; p = 0.034), and the permanent use of personal protective equipment (PR = 0.14; 95% IC, 0.02-0.88; p =0.036). CONCLUSION: Accident statistics in clinical laboratories prove the necessity to systematically reorganize risk management strategies that take into account human factor, organizational culture and control mechanisms.


INTRODUCCIÓN: las personas que trabajan en laboratorios clínicos están en permanente riesgo de sufrir accidentes laborales por la exposición a material biológico. Estos accidentes pueden causar enfermedades graves, como hepatitis B, hepatitis C o infección por VIH. OBJETIVO: describir el comportamiento de la accidentalidad por exposición ocupacional a agentes de riesgo biológico y las características del reporte de accidentes de trabajo en laboratorios clínicos de Yopal, Colombia. MÉTODOS: estudio observacional descriptivo con componente exploratorio y análisis retrospectivo de formatos de reporte de presuntos accidentes de trabajo. RESULTADOS: para el periodo comprendido entre el 1 de enero de 2016 y el 31 de diciembre de 2018, se estimó una accidentalidad laboral de 24.6% (intervalo de confianza al 95% [IC 95%] 16.7-32.5) y un subregistro de 39.3% (IC 95% 21.3-57.3). Las principales razones del subregistro incluyeron percibir limitaciones de tiempo (45.4%) y considerar irrelevante la exposición (36.4%). Por otro lado, estar contratado por prestación de servicio (razón de prevalencias [RP] = 0.19, IC 95% 0.04-0.89, p = 0.034) y usar siempre el equipo de protección personal (RP = 0.14, IC 95% 0.02-0.88, p = 0.036) se asociaron con la probabilidad de sufrir menor accidentalidad laboral. CONCLUSIÓN: las estadísticas de accidentalidad en laboratorios clínicos reflejan la necesidad de reorganizar de manera sistemática las estrategias de gestión del riesgo en función del factor humano, la cultura organizacional y los mecanismos de control.

ABSTRACT Objective: To identify in the literature the efficacy of needlesticks with safety devices to reduce the occurrence of occupational accidents with exposure to biological material among health workers. Method: Integrative literature review, structured in the stages: Guiding question, search, categorization of studies, evaluation, discussion and interpretation of results, and synthesis of knowledge. Search for original articles and systematic reviews on the main bases of the Health area, published from 2000 to 2016 in Portuguese, English and Spanish, with descriptors: needlesticks injuries, exposure to biological agents, needles, protective devices, occupational accidents, accident prevention and health personnel. Results: We selected eleven articles, most characterized the passive safety devices as more effective in reducing the occurrence of injuries by needlesticks. Conclusion: The use of needlesticks with safety devices reduces the occurrence of accidents, bringing greater solvency when combined with the training of workers.


RESUMEN Objetivo: Identificar en la literatura la eficacia del uso de agujas con dispositivos de seguridad para reducir la ocurrencia de accidentes del trabajo con exposición a material biológico entre trabajadores de la salud. Método: Revisión integrativa de la literatura, estructurada en las etapas: Cuestión orientadora, búsqueda, categorización de los estudios, evaluación, discusión e interpretación de los resultados, y síntesis del conocimiento. Busca artículos originales y revisiones sistemáticas en las principales bases del área de la salud, publicados desde 2000 hasta 2016 en Portugués, Inglés y Español, con descriptores: lesiones por pinchazo de agujas, exposición a agentes biológicos, agujas, equipos de seguridad, accidentes de trabajo, prevención de accidentes y personal de salud. Resultados: Se han seleccionado once artículos, la mayoría caracterizó los dispositivos de seguridad pasivos como más efectivos en la disminución de la ocurrencia de lesiones por agujas. Conclusión: La utilización de agujas con dispositivos de seguridad reduce la ocurrencia de los accidentes, trayendo mayor resolutividad cuando aliada a la capacitación de los trabajadores.

Método: Revisão integrativa da literatura, estruturada nas etapas: Questão norteadora, busca, categorização dos estudos, avaliação, discussão e interpretação dos resultados, e síntese do conhecimento. Busca a artigos originais e revisões sistemáticas nas principais bases da área da Saúde, publicados de 2000 a 2016 em português, inglês e espanhol, com descritores: ferimentos penetrantes produzidos por agulhas, exposição a agentes biológicos, agulhas, equipamentos de proteção, acidentes de trabalho, prevenção de acidentes e pessoal de saúde. Resultados: Foram selecionados onze artigos, a maioria caracterizou os dispositivos de segurança passivos como mais efetivos na diminuição da ocorrência de lesões por agulhas. Conclusão: A utilização de agulhas com dispositivos de segurança reduz a ocorrência dos acidentes, trazendo maior resolutividade quando aliada à capacitação dos trabalhadores.

BACKGROUND: Proper immunization and knowledge in infection prevention are key factors in protecting medical students. AIM OF THE STUDY: To describe the status on vaccination recommended for healthcare workers (HCW) and infection prevention knowledge. METHODS: We conducted a cross-sectional study on medical students at clinical years of medical school from a public University in Mexico. RESULTS: A total of 1,824 medical students responded the survey. One thousand ninety (59.8%) were women. Median age was 22 years. One thousand six hundred twenty-two (88.9%) knew their childhood immunization status. One thousand seventy-one (58.7%) were vaccinated against influenza for the 2016-2017 season; 1667 (91.4%) had been vaccinated at least once against hepatitis B, only 315 (18.9%) of vaccinated had received a full course with 3 doses. Most students were vaccinated against measles, mumps and rubella during childhood, 542 (29.7%) received an additional dosage during or after adolescence. Six hundred ninety-seven (38.2%) were concerned about vaccine's safety. A total of 1,431 (78.5%) properly identified situations were standard precautions are recommended, and 1540 (84.4%) had received some training on safe care delivery and personal protective equipment. Regarding needle-stick injuries, 1165 (63.9%) had been informed on the protocols to follow if an injury occurred. Three hundred forty-nine (19.1%) had suffered needle-stick injuries, only 125 (35.8%) received immediate medical attention at the point of care. CONCLUSIONS: Most medical students were not vaccinated as recommended, and they were not adequately instructed on safe practices for medical attention, nor advised or followed when a health-care related accident occurs. The results may be useful for implementation strategies on vaccination compliance and training on infection prevention.

Introduction: Systemic lupus erythematosus and infection by the human immunodeficiency virus are diseases that affect the immune system. There are few reported cases of the concomitance of both pathologies. Objective: To describe a clinical case of lupus who had a needlestick injury and had also HIV exposure. Method: Case report description. Results: A patient with lupus had a needlestick injury from an HIV positive patient. This person had several intolerances or side effects to retroviral prophylaxis, lupus flare was also observed; however, in the end of 24 weeks his serology was negative and viral load was also undetectable. Conclusion: This is the first case report of a patient diagnosed with SLE, using an immunosuppressive, exposed to HIV through a needlestick accident while working.


Introdução: O lúpus eritematoso sistêmico e a infecção pelo vírus da imunodeficiência humana são doenças que afetam o sistema imunológico. Existem poucos casos relatados da concomitância de ambas as patologias. Objetivo: Descrever um relato de caso de uma paciente com lúpus que teve um ferimento penetrante produzido por agulha e foi exposta ao vírus HIV. Método: relato de caso. Resultado: Um paciente com lúpus teve um ferimento penetrante produzido por agulha proveniente de um paciente com HIV positivo. Esta pessoa teve várias intolerâncias ou efeitos colaterais na profilaxia retroviral, também foi observado a atividade do lúpus; no entanto, no final de 24 semanas, sua sorologia foi negativa e a carga viral também não era detectável. Conclusão: Este relato é a primeira descrição de um acidente com lúpus que se contaminou acidentalmente por agulha com o vírus HIV.

ABSTRACT The rate of human immunodeficiency virus (HIV), hepatitis B virus (HBV), and hepatitis C virus (HCV) infections among health care workers that is caused by sharps injuries is higher in the Caribbean and Latin America than in other regions of the world. To respond to and reduce occupational exposures to bloodborne pathogens while also strengthening capacities in the Caribbean, needlestick injury prevention training programs for health care workers were implemented, beginning in 2011. The programs included lectures, workshops, policy reviews, evaluations of safety devices, and workplace assessment. During the training, baseline data from health care workers on their personal history of needlestick injuries and bloodborne pathogen exposure was collected. That baseline data showed that 40% of the participants had sustained sharps injuries during their professional career. In this capacity-building effort, 210 health care workers from five countries have been trained, six health care centers in the Caribbean have been evaluated. and occupational safety and health committees have been established in various countries to monitor and improve safety policies and practices.


RESUMEN La tasa de infecciones por los virus de la inmunodeficiencia humana (VIH), de la hepatitis B (VHB) y de la hepatitis C (VHC) en los trabajadores de atención de salud debidas a heridas por materiales punzocortantes es mayor en América Latina y el Caribe que en otras regiones del mundo. Con objeto de responder a las exposiciones laborales a los agentes patógenos de transmisión sanguínea y reducirlas, además de reforzar las capacidades en el Caribe, se instituyeron programas de capacitación para la prevención de las heridas por pinchazos accidentales en los trabajadores de salud a partir del 2011. Los programas comprendían conferencias, talleres, revisión de las políticas, análisis de los dispositivos de seguridad y evaluación de los lugares de trabajo. Durante la capacitación, se recopilaron datos iniciales suministrados por los trabajadores de atención de salud sobre sus antecedentes personales de heridas por pinchazos accidentales y exposición a agentes patógenos de transmisión sanguínea. Esos datos iniciales indicaron que 40% de los participantes habían sufrido alguna herida por materiales punzocortantes a lo largo de su carrera profesional. En esta iniciativa de formación de capacidades, se han capacitado 210 trabajadores de atención de salud de cinco países, se han evaluado seis centros de atención de salud en el Caribe y se han establecido comités de seguridad y salud ocupacional en diversos países para vigilar y mejorar las normas y prácticas de seguridad.


RESUMO As taxas de infecção pelo vírus da imunodeficiência humana (HIV), vírus da hepatite B (HBV) e vírus da hepatite C (HCV) em profissionais da saúde decorrentes de acidentes com materiais perfurocortantes são mais elevadas no Caribe e na América Latina que em outras regiões do mundo. Para combater e reduzir a exposição ocupacional a patógenos de transmissão sanguínea, igualmente reforçando as competências no Caribe, programas de capacitação em prevenção de acidentes com agulhas para profissionais da saúde foram implementados a partir de 2011. Os programas consistiram de palestras, seminários, análises de políticas, avaliações de dispositivos de segurança e avaliação dos locais de trabalho. Durante a capacitação, foram coletados dados de base dos profissionais da saúde relativos ao histórico pessoal de acidentes com agulhas e exposição a patógenos de transmissão sanguínea. Estes dados indicaram que 40% dos participantes haviam sofrido acidentes com materiais perfurocortantes durante a atividade profissional. Como parte desta iniciativa de capacitação, foi dado treinamento a 210 profissionais da saúde de cinco países, foram avaliados seis centros de saúde no Caribe e instituídos comitês de saúde e segurança ocupacional em diversos países para monitorar e melhorar as políticas e práticas de segurança.

The exposure was percutaneous in 515 (89%) and needles where involved in 406 (70%) of them. The sharps were present in the trash in 227 (39%) or in the environment in 167 (29%) of the reports. Professionals who work with frequent contact with domestic or urban waste were 196 (38%). The source was known in 112 (19%) of the exposures and blood was involved in 269 (46%). Only 101 (19%) of the injured subjects reported a complete course of vaccination for hepatitis B. Antiretroviral prophylaxis was prescribed for 392 (68%) of the exposed subjects. Conclusions: Sharps injuries occurring in the community are an important health problem. A great proportion would be avoided if practices on how to dispose needles and sharps used outside health units were implemented.

BACKGROUND: The studies enumerating the risk of HIV transmission to health care workers (HCWs) as 0.3% after percutaneous exposure to HIV-positive blood, and 0.09% after a mucous membrane exposure, are weakened by dated literature. Our study aims to demonstrate the seroconversion rate after exposure to HIV-contaminated body fluids in a major academic center in the United States. METHODS: A prospectively maintained database of reported occupational injuries occurring between 2002 and 2015 at an academic medical center was analyzed. Data collected included the type of injury, injured body part, type of fluid, contamination of sharps, involvement of resident physicians, use of postexposure prophylaxis, and patients' HIV, hepatitis B virus, and hepatitis C virus status. RESULTS: A total of 266 cases were included in the study. Most exposures were caused by percutaneous injuries (52.6%), followed by 43.2% mucocutaneous injuries. Of the injuries, 52.6% were to the hand and 33.5% to the face and neck. Blood exposure accounted for 64.3% of all cases. Of the patients, 21.1% received postexposure prophylaxis. None of the HCWs exposed to HIV-contaminated body fluids seroconverted (seroconversion rate, 0%). CONCLUSIONS: HIV does not seem to be as easily transmitted by needlestick, laceration, or splash injuries as previously surmised. Further large-scale and multicenter studies are needed for a more accurate estimation of the risk of transmission of HIV in U.S. health care workers.

Introduction: Dental students are often exposed to bloodborne pathogens during dental training. Several factors are involved in increased risk of human deficiency, hepatitis B and hepatitis C virus (HIV, HBV, and HCV) infection. However, there are few studies that address the risks and forms of prevention among dental students in Brazil. Methods: A cross-sectional study of occupational exposure to blood or body fluids among dental students of Universidade Federal do Rio Grande do Sul, Brazil, was performed. These students were referred to the Occupational Medicine Department of Hospital de Clínicas de Porto Alegre from January 2007 to April 2015. Analyzed data included type of exposure (needlestick injury, mucosal exposure, and exposure to non-intact skin); source patient status for HBV, HIV and HCV infection, accident during dental training, procedure performed, biological material involved, type of accident, and hepatitis B vaccination and serological protection status. The objective was to know the incidence rate and others characteristics of accidents in order to prevent them. Results: There were 312 accidents during the study period of 8 years and 4 months. Incidence rate was 87,42 exposures per 1000 students year. Source patient was known in 297 of the cases (95.2%), of which 3 were HBsAg reagent, 12 were HIV reagent, and 17 were HCV reagent. The majority of accidents occurred during procedure, but nearly as high as 40% occurred after procedure, of which 63% occurred during instrument cleaning, disinfecting or sterilizing. Most involved sharp instruments were anesthetic syringe needle and curette. Only 48% of dental students knew their antiHBs was > 10 mIU/mL. Conclusions: Dental students should be tested for hepatitis B immune status at the beginning of training, and vaccination should be available to all dental students before they start clinical practice. Work practice controls on sharp devices should be addressed at the beginning and strengthened during dental training. Dental training institutions should review instrumental cleaning process to minimize handling of loose contaminated instruments (AU)
