INTRODUCTION: A reduced platelet count (PLT) is a frequent post-operative finding in orthopedic surgery patients. Despite its prevalence, the characteristics of post-surgical thrombocytopenia have not been well described. METHODS: A retrospective chart review was conducted on patients who underwent a knee or hip replacement from 2012 to 2015. Patients who received heparin were excluded. RESULTS: A total of 56 patients were analyzed on post-operative days 0 to 4. By day 1, 90.9% of the patients experienced a reduction in their platelet counts. The lowest mean platelet count (nadir) occurred on day 2 (201.3 × 109/L). The average decrease in the platelet count from the baseline was 24% (95%CI: 20.6 - 27.2). The change in the platelet count from the baseline ranged from a 49.6% drop to a 14.2% increase. A substantial portion of patients experienced thrombocytopenia, with 28% occurring on day 2. Platelet counts less than 100 × 109/L occurred only once. The percent decrease in the platelet count from the baseline to any other time point was significantly larger in patients aged > 65 years, compared to patients aged ≤ 65 years (p = 0.007). Specifically, the average drop in the platelet count at the nadir (day 2) relative to the baseline was 27.8% in patients aged > 65 years, compared to 19.5% in patients aged ≤ 65 years. CONCLUSIONS: A reduction in the platelet count is a frequent post-operative finding in orthopedic surgery patients, even after removing confounding factors, such as heparin exposure, but clinical thrombocytopenia is uncommon. Alternative etiologies should be considered when the platelet count is less than 100 × 109/L. Vigilance should also be considered regarding elderly patients.

Abstract Introduction: Obstructive sleep apnea is characterized by a reduced airflow through the upper airways during sleep. Two forms of obstructive sleep apnea are described: the central form and the obstructive form. The obstructive form is related to many factors, such as the craniofacial morphology. Objective: To evaluate the correlation between the morphology of the cranial base, of the mandible and the maxilla, and obstructive sleep apnea severity. Methods: Eighty-four patients, mean age of 50.4 years old; 73 males and 11 females with obstructive sleep apnea were enrolled in the present study. Patients with high body mass index and comorbidities were excluded. Lateral cephalograms and polysomnography were collected for each patient to evaluate the correlation between craniofacial morphology and obstructive sleep apnea severity. A Spearman's rho correlation test between cephalometric measurements and obstructive sleep apnea indexes was computed. Statistical significance was set at p < 0.05. Results: Patients with a severe obstructive sleep apnea presented a reduction of sagittal growth of both effective mandibular length and cranio-basal length. The mandibular length was the only variable with a statistical correlation with apnea-hypopnea index. Vertical dimension showed a weak correlation with the severity of obstructive sleep apnea. No correlation with maxillary sagittal dimension was shown. Conclusion: Obstructive sleep apnea severity may be correlated to mandibular and cranial base growth. Facial vertical dimension had no correlation with obstructive sleep apnea severity.


Resumo Introdução: A apneia obstrutiva do sono é caracterizada por um fluxo de ar reduzido nas vias aéreas superiores durante o sono. Duas formas de apneia são descritas: a forma central e a forma obstrutiva. A forma obstrutiva tem sido relacionada a vários fatores, como a morfologia craniofacial. Objetivo: Avaliar a correlação entre a morfologia da base do crânio, da mandíbula e da maxila e a gravidade da apneia obstrutiva do sono. Método: Foram incluídos no presente estudo 84 pacientes com apneia obstrutiva do sono, com média de 50,4 anos; 73 homens e 11 mulheres. Pacientes com alto índice de massa corpórea e comorbidades foram excluídos. Foram coletados cefalogramas laterais e polissonografia para cada paciente para avaliar a correlação entre a morfologia craniofacial e a gravidade da apneia. Foi computado o coeficiente de correlação de postos de Spearman (rho) entre medidas cefalométricas e índices de apneia obstrutiva do sono. A significância estatística foi estabelecida em p < 0,05. Resultados: Pacientes com apneia obstrutiva do sono grave apresentaram redução do crescimento sagital do comprimento mandibular efetivo e do comprimento crânio-basal. O comprimento mandibular foi a única variável que apresentou correlação estatística com o índice de apneia-hipopneia. A dimensão vertical mostrou uma fraca correlação com a gravidade da apneia. Não foi demonstrada correlação com a dimensão maxilar sagital. Conclusão: A gravidade da apneia obstrutiva do sono pode estar correlacionada ao crescimento da base mandibular e craniana. A dimensão facial vertical não apresentou correlação com a gravidade da apneia.

INTRODUCTION: Prostate cancer (PCa) is the second most prevalent neoplasm among men in the world. Its treatment has a wide spectrum of alternatives and variables, ranging from active surveillance through radio and/or brachytherapy, to surgery. OBJECTIVE: The present work aimed to identify the predictive factors for biochemical recurrence and to evaluate the toxicity of the treatment using the association of external beam radiation therapy (EBRT) with high dose rate brachytherapy (HDR-BT) applied in the treatment of patients with prostate cancer. METHODS: Longitudinal retrospective study, using a prospectively collected database between 2005 and 2014 of 186 consecutive patients records with a diagnosis of low, intermediate, or high-risk prostate cancer treated with EBRT combined with HDR-BT, in a single medical institution located in the city of Campinas, SP, Brazil (Radium Institute). PSA increase over 2 ng/ml above the nadir PSA was considered as biochemical recurrence, following the definition of the Phoenix Consensus. Continuous and clinically relevant categorical variables (age, initial PSA, delivered dose in EBRT, number of implants, number of positive cores in transrectal biopsy, use of hormone blockade, Gleason score, TNM staging, post treatment PSA and PSA Nadir) were evaluated with absolute (n) and percentage (%) values using multiple logistic regression and validated our previously described optimal PSA nadir as predictor of biochemical recurrence. RESULTS: Post treatment PSA was the only independent predictor of biochemical recurrence, P<0.0001. The lower the PSA nadir the lower the biochemical recurrence risk (P=0.0009). PSA nadir >1 was the best cutoff (P=0.018) determinant of biochemical recurrence. The incidence of grade 3 late toxicity to the genitourinary tract was 0.6%, and there were no cases of severe complications to the gastrointestinal tract. CONCLUSION: External Beam Radiation Therapy conjugated to Brachytherapy in the treatment of Prostate Cancer has demonstrated low biochemical recurrence rates, mainly when PSA nadir <1, with low toxicity into both GU and GI tracts.

Objective To evaluate the relationship between two year PSA nadir (PSAn) after brachytherapy and biochemical recurrence rates in prostate cancer patients. Materials and Methods In the period from January 1998 to August 2007, 120 patients were treated with iodine-125 brachytherapy alone. The results analysis was based on the definition of biochemical recurrence according to the Phoenix Consensus. Results Biochemical control was observed in 86 patients (71.7%), and biochemical recurrence, in 34 (28.3%). Mean PSAn was 0.53 ng/ml. The mean follow-up was 98 months. The patients were divided into two groups: group 1, with two year PSAn < 0.5 ng/ml after brachytherapy (74 patients; 61.7%), and group 2, with two year PSAn ≥ 0.5 ng/ml after brachytherapy (46 patients; 38.3%). Group 1 presented biochemical recurrence in 15 patients (20.3%), and group 2, in 19 patients (43.2%) (p < 0.02). The analysis of biochemical disease-free survival at seven years, stratified by the two groups, showed values of 80% and 64% (p < 0.02), respectively. Conclusion Levels of two year PSAn ≥ 0.5 ng/ml after brachytherapy are strongly correlated with a poor prognosis. This fact may help to identify patients at risk for disease recurrence. .


Objetivo Avaliar os valores de PSA nadir (PSAn) dois anos após braquiterapia e sua relação com a taxa de recidiva bioquímica. Materiais e Métodos Cento e vinte pacientes foram tratados no período de janeiro de 1998 a agosto de 2007. Todos os pacientes foram submetidos a braquiterapia isolada. Os resultados foram analisados com base na definição de recidiva bioquímica do Consenso Phoenix. Resultados Oitenta e seis pacientes (71,7%) tiveram controle bioquímico e 34 (28,3%) apresentaram recidiva bioquímica. O PSAn médio foi 0,53 ng/ml. O seguimento médio foi 98 meses. Os pacientes foram divididos em dois grupos: grupo 1, com PSAn < 0,5 ng/ml dois anos após a braquiterapia (74 pacientes; 61,7%), e grupo 2, com PSAn ≥ 0,5 ng/ml dois anos após a braquiterapia (46 pacientes; 38, 3%). Este fato pode ajudar a identificar pacientes com risco de recidiva da doença. .

OBJECTIVE: To evaluate the 25-hydroxyvitamin D [25(OH)D] concentrations in individuals in the city of São Paulo belonging to different age groups and exhibiting specific behavioral characteristics and to correlate the 25(OH)D concentration with the level of UV radiation (UVR). PATIENTS AND METHODS: A total of 591 individuals were included, distributed as follows: 177 were living in institutions (NURSING, 76.2 ± 9.0 y old), 243 were part of the community elderly (COMMUNITY, 79.6 ± 5.3 y old), 99 were enrolled in a physical activity program targeting the elderly (ACTIVE, 67.6 ± 5.4 y old) and 72 were young (YOUNG, 23.9 ± 2.8 y old). Blood samples from all individuals were collected throughout the year. UVR measurements were taken by an official meteorology institution. RESULTS: The UVR values varied throughout the year, following a sinusoidal-like pattern. Because of the Earth's orbit, we hypothesized that there would be cyclic patterns for the 25(OH)D and UVR values that repeat every 12 mo. The general formula is represented by the equation P1+P2⋅sin(-2⋅π12⋅(t-P3)) The mean 25(OH)D concentration and the amplitude of the variation were significantly higher for the YOUNG and ACTIVE groups than for the COMMUNITY and NURSING groups. The nadir for UVR was in June, whereas the nadir for the 25(OH)D concentration was in the spring, corresponding to a delay of one season. CONCLUSIONS: There was seasonal variation in the 25(OH)D concentration for all the groups studied; however, the amplitude of the variation was higher for the groups of young and physically active people, possibly due to the higher level of sunlight exposure for these groups. The lowest 25(OH)D concentration was detected in the spring.

El propósito de este trabajo fue determinar la influencia del mérito genético para la producción de leche sobre el balance energético, los indicadores del metabolismo energético y la reactivación ovárica posparto. Para este estudio, se utilizaron 10 vacas Holstein puras de diferente mérito genético para la producción de leche, que se alimentan bajo un sistema de pastoreo rotacional con suplementación de concentrado. A estos animales se les realizó un seguimiento durante los 10.4 (± 2.5), 21.6 (± 4.3), 31.8(± 4.5), 42.3 (± 4.2), y 51.9 (± 3.9) días posparto, los cuales constituyeron los periodos de muestreo. En cada muestreo, los animales fueron pesados con cinta métrica, se les evaluó la con se estimó su balance de energía. Además, se tomaron muestras de suero sanguíneo para evaluar las concentraciones de glucosa y colesterol; y se efectuó un seguimiento de la reactivación ovárica por ultrasonografía. La mayoría de las vacas reiniciaron su actividad ovárica en la segunda semana posparto y más de la mitad presentaron su primera ovulación durante el periodo experimental. Se encontró una relación estadísticamente significativa (p<0.05) y con pendiente negativa entre el mérito genético para la producción de leche y la magnitud del nadir del BEN, sin embargo el mérito genético no tuvo influencia significativa sobre los días al nadir del BEN, dado que las vacas de mérito genético alto, como bajo incrementaron el consumo y la producción de leche siguiendo patrones similares, pero con magnitudes diferentes. De igual forma el merito genético no se relacionó significativamente con los días a la primera ovulación. Además, ninguna de las variables del balance de energía, ni los metabolitos sanguíneos afectaron los días a la primera ovulación o el número de folículos clasificados por tamaño.


The aim of this research was to establish the genetic merit for milk yield influence on energy balance, energy metabolic profiles and the postpartum ovarian resumption. In this research, 10 pure-Holstein Friesian dairy cows, varying in genetic merit values for milk yield and fed on a rotational-grazing system with a supplementary concentrate ration were used. A serial of examinations were performed on10.4 (± 2.5), 21.6 (± 4.3), 31.8 (± 4.5), 42.3 (± 4.2), and 51.9 (± 3.9) postpartum days. In every sampling day animals were weighed by tape measure, their body condition score were calculated and the energy balance was estimated for every cow. In addition, blood serum tests were performed to analyze glucose and cholesterol blood concentration and an ultrasonographic assessment was executed to determine ovarian resumption. Nearly all cows had an ovarian resumption on the second week postpartum and more than a half had shown their first postpartum ovulation on the trial period. The genetic merit values for milk yield were not related with production values, therefore, the dependent variables of milk yield did not fluctuated with genetic merit. There was a significant negative relationship (p<0.05) between genetic merit for milk production and the nadir extent of net energy balance (NEB). Although, the genetic merit did not have significant influence on the number of days between calving to the nadir-attaining day, because high genetic merit cows as low genetic merit cows increased the intake and milk yield following similar patterns, but with different extents. In the same way, the genetic merit did not have significant relationship with the interval of calving to first ovulation. Furthermore, no one of the energy balance variables or blood metabolites influenced the interval of calving to first ovulation or the amount of follicles classified by size.
