RESUMO

Microvasculature failure is expected in sepsis and at higher amine concentrations. Therefore, special attention focused individually on microcirculation is needed. Here, we present that methylene blue can prevent leukocytes from adhering to the endothelium in a rat model of lipopolysaccharide-induced endotoxemia. As hypothesis evidence, an intravital microscopy image is presented.

In this review, I summarise the circumstances leading to the collaboration between London and Örebro on the basic research performed to study potential mechanisms underlying the improved patency of saphenous veins harvested by the no-touch technique. Histological studies reveal various forms of vascular damage to saphenous vein grafts harvested in conventional coronary artery bypass grafting (CABG) whereas no-touch grafts retain a normal architecture. The perivascular fat that remains intact on no-touch saphenous vein grafts seems to play a particularly important role as the "protector" of all layers of the graft. In addition, the perivascular fat is a source of adipose cell-derived factors that may contribute to the success of the no-touch technique. While a number of trials have compared no-touch with conventional grafts following CABG, these have generally been limited to short follow-up periods, low patient numbers, and inadequate histological data. When handling no-touch saphenous vein at harvesting, there is no direct contact of the vein by surgical instruments, spasm does not occur, and high-pressure intraluminal distension is not required. While damage to both endothelial and vascular smooth muscle cells are evident at the microscopic and ultrastructural level in conventional saphenous vein grafts, their structure in no-touch grafts is preserved. Also, in no-touch veins, the vasa vasorum remains intact and transmural blood supply is maintained. This microvascular network is disrupted during conventional harvesting, a situation likely to stimulate processes involved in graft occlusion. The use of excess graft material for histology is to be encouraged for the assessment of vascular damage and even surgeon competence. If you don't look, you don't find.

Abstract Objective: The objective of this study was to evaluate the effect of Kangaroo Position (KP) in microcirculation (MC) of the flexor muscles of preterm newborns. Method: A controlled clinical trial was conducted in the city of Recife, Brazil, with 26 preterm children randomized in the Kangaroo Group (13) and in the Control Group (13). Assessments of blood flow, temperature, and tissue oxygen saturation (SO2) were made at two different times and in the biceps brachii muscle and hamstrings muscle group: before the KP and after 24 h of KP. In the Control Group, the registrations were performed at the times corresponding to those of the Kangaroo Group. The mean values among the times were analyzed by paired t-test for repeated measures. The clinical trial was recorded in Clinical Trials (NCT03611088). Results: In the Kangaroo Group there was an increase in tissue temperature and blood flow at the time evaluation periods (p < 0.05). In the control group, there was no statistical difference between the recording moments hamstring muscles group, but in the biceps brachii, there was a reduction in mean blood flow (p = 0.023). Conclusion: In conclusion, the KP has effects on the microcirculation of the flexor muscles of preterm newborns.

As these factors have been described as having constrictor or relaxant effects, they may be considered either beneficial or detrimental. The close proximity of PVAT to the adventitia provides an environment whereby adipose tissue-derived factors may affect the vasa vasorum, a microvascular network providing the vessel wall with oxygen and nutrients. Since medial ischaemia promotes aspects of graft occlusion the involvement of the PVAT/vasa vasorum axis in vein graft patency should be considered.

Amphetamine-induced neuroadaptations involve vascular damage, neuroinflammation, a hypo-functioning prefrontal cortex (PFC), and cognitive alterations. Brain angiotensin II, through angiotensin type 1 receptor (AT1 -R), mediates oxidative/inflammatory responses, promoting endothelial dysfunction, neuronal oxidative damage and glial reactivity. The present work aims to unmask the role of AT1 -R in the development of amphetamine-induced changes over glial and vascular components within PFC and hippocampus. Attention deficit was evaluated as a behavioral neuroadaptation induced by amphetamine. Brain microvessels were isolated to further evaluate vascular alterations after amphetamine exposure. Male Wistar rats were administered with AT1 -R antagonist, candesartan, followed by repeated amphetamine. After one week drug-off period, animals received a saline or amphetamine challenge and were evaluated in behavioral tests. Afterward, their brains were processed for cresyl violet staining, CD11b (microglia marker), GFAP (astrocyte marker) or von Willebrand factor (vascular marker) immunohistochemistry, and oxidative/cellular stress determinations in brain microvessels. Statistical analysis was performed by using factorial ANOVA followed by Bonferroni or Tukey tests. Repeated amphetamine administration increased astroglial and microglial markers immunoreactivity, increased apoptotic cells, and promoted vascular network rearrangement at the PFC concomitantly with an attention deficit. Although the amphetamine challenge improved the attentional performance, it triggers detrimental effects probably because of the exacerbated malondialdehyde levels and increased heat shock protein 70 expression in microvessels. All observed amphetamine-induced alterations were prevented by the AT1 -R blockade. Our results support the AT1 -R involvement in the development of oxidative/inflammatory conditions triggered by amphetamine exposure, affecting cortical areas and increasing vascular susceptibility to future challenges.

OBJETIVO: Analisar a reperfusão miocárdica após intervenção coronariana percutânea primária em diabéticos e não diabéticos. MÉTODOS: Estudo caso-controle prospectivo, realizado no período de janeiro de 2013 a março de 2014, incluindo pacientes com diagnóstico de infarto agudo do miocárdio com menos de 12 horas do início dos sintomas e submetidos à terapia de reperfusão com intervenção coronariana percutânea primária. Foi realizada avaliação do blush miocárdico, da resolução do supradesnivelamento de segmento ST >50% em 90 e em 180 minutos e do Thrombolysis in Myocardial Infarction (TIMI) frame count corrigido <40 quadros. RESULTADOS: Foram incluídos 201 pacientes no estudo, sendo 32 diabéticos. Levando em consideração os marcadores de reperfusão miocárdica estudados, indivíduos diabéticos tiveram menor resolução do segmento ST, menor blush miocárdico e maior TIMI frame count após angioplastia primária quando comparados aos não diabéticos. CONCLUSÕES: Diabéticos apresentaram piores índices de reperfusão miocárdica pós-angioplastia primária comparados aos não diabéticos. (AU)


OBJECTIVES: To analyze myocardial reperfusion following primary percutaneous coronary intervention in diabetic and non-diabetic patients. METHODS: This is a prospective casecontrol study, conducted from January 2013 to March 2014, including patients diagnosed with acute myocardial infarction less than 12 hours after the onset of symptoms, and undergoing reperfusion therapy with Primary percutaneous coronary intervention. The myocardial blush, ST-segment elevation >50% resolution in 90 and 180 minutes, and corrected Thrombolysis Myocardial Infarction (TIMI) frame count <40 frames were evaluated. RESULTS: A total of 201 patients were included in the study, of which 32 were diabetic. Taking the studied myocardial reperfusion markers into consideration, diabetic patients had lower ST segment resolution, lower myocardial blush, and higher TIMI frame count after primary angioplasty when compared to non-diabetics. CONCLUSIONS: Diabetic patients presented worse myocardial reperfusion rates after primary angioplasty when compared to non-diabetic patients. (AU)

Abstract Background: There are limited data on the prognosis of deferral of lesion treatment in patients with acute coronary syndrome (ACS) based on fractional flow reserve (FFR). Objectives: To provide a systematic review of the current evidence on the prognosis of deferred lesions in ACS patients compared with deferred lesions in non-ACS patients, on the basis of FFR. Methods: We searched Medline, EMBASE, and the Cochrane Library for studies published between January 2000 and September 2017 that compared prognosis of deferred revascularization of lesions on the basis of FFR in ACS patients compared with non-ACS patients. We conducted a pooled relative risk meta-analysis of four primary outcomes: mortality, cardiovascular (CV) mortality, myocardial infarction (MI) and target-vessel revascularization (TVR). Results: We identified 7 studies that included a total of 5,107 patients. A pooled meta-analysis showed no significant difference in mortality (relative risk [RR] = 1.44; 95% CI, 0.9-2.4), CV mortality (RR = 1.29; 95% CI = 0.4-4.3) and TVR (RR = 1.46; 95% CI = 0.9-2.3) after deferral of revascularization based on FFR between ACS and non-ACS patients. Such deferral was associated with significant additional risk of MI (RR = 1.83; 95% CI = 1.4-2.4) in ACS patients. Conclusion: The prognostic value of FFR in ACS setting is not as good as in stable patients. The results demonstrate an increased risk of MI but not of mortality, CV mortality, and TVR in ACS patients.


Resumo Fundamento: Existem dados limitados sobre o prognóstico do adiamento do tratamento das lesões em pacientes com síndrome coronária com base na reserva de fluxo fracionada (FFR). Objetivos: Realizar uma revisão sistemática da evidência atual sobre o prognóstico do adiamento do tratamento de lesões em pacientes com SCA com base na FFR, comparando-o com o prognóstico em pacientes sem SCA. Métodos: Pesquisamos as bases de dados do Medline, EMBASE, e Cochrane Library por estudos publicados entre janeiro de 2000 e setembro de 2017 que compararam o prognóstico do adiamento da revascularização das lesões com base na FFR em pacientes com SCA em comparação a pacientes sem SCA. Conduzimos uma metanálise do risco relativo de quatro desfechos primários: mortalidade, mortalidade cardiovascular, infarto do miocárdio (IM) e revascularização do vaso-alvo (TVR). Resultados: Identificamos sete estudos que incluíram um total de 5107 pacientes. A metanálise mostrou que não houve diferença quanto à mortalidade [risco relativo (RR) = 1,44; IC95%, 0,9-2,4), mortalidade cardiovascular (RR = 1,29; IC95% = 0,4-4,3) e TVR (RR = 1,46; IC95% = 0,9-2,3) após adiamento da revascularização com base na FFR entre pacientes com SCA e pacientes sem SCA. Tal adiamento foi associado com risco adicional de IM (RR = 1,83; IC95% = 1,4-2,4) em pacientes com SCA. Conclusão: O valor prognóstico da FFR na SCA não é tão bom como em pacientes estáveis. Os resultados mostram um risco aumentado de IM, mas não de mortalidade, mortalidade cardiovascular, e TVR em pacientes com SCA.

ABSTRACT BACKGROUND: Investigation of alterations to retinal microvasculature may contribute towards understanding the role of such changes in the pathophysiology of several chronic non-communicable diseases. The objective here was to evaluate the validity and reproducibility of retinal arteriole and venule diameter measurements made by Brazilian Longitudinal Study of Adult Health (ELSA-Brasil) graders. DESIGN AND SETTING: Cross-sectional study at six teaching and research institutions. METHODS: To evaluate validity, each of 25 retinal images from the University of Wisconsin (gold standard) was measured by five ELSA-Brasil graders. To evaluate reproducibility, 105 images across the spectrum of vessel diameters were selected from 12,257 retinal images that had been obtained between 2010 and 2012, and each image was reexamined by the same grader and by an independent grader. All measurements were made using the Interactive Vessel Analysis (IVAN) software. Bland-Altman plots, paired t tests and intraclass correlation coefficients (ICCs) were analyzed. RESULTS: Mean differences between ELSA-Brasil and gold-standard readings were 0.16 µm (95% CI -0.17-0.50; P = 0.31) for central retinal artery equivalent (CRAE), -0.21 µm (95% CI -0.56-0.14; P = 0.22) for central retinal vein equivalent (CRVE) and 0.0005 (95% CI -0.008-0.009; P = 0.55) for arteriole/venule ratio (AVR). Intragrader ICCs were 0.77 (95% CI 0.67-0.86) for CRAE, 0.90 (95% CI 0.780.96) for CRVE and 0.70 (0.55-0.83) for AVR. Intergrader ICCs were 0.75 (95% CI 0.64-0.85) for CRAE, 0.90 (95% CI 0.79-0.96) for CRVE and 0.68 (95% CI 0.55-0.82) for AVR. CONCLUSIONS: Retinal microvascular diameter measurements are valid and present moderate to high intra and intergrader reproducibility in ELSA-Brasil.

Numerous studies have shown that the venom of Phoneutria nigriventer (PNV) armed-spider causes excitotoxic signals and blood-brain barrier breakdown (BBBb) in rats. Nitric oxide (NO) is a signaling molecule which has a role in endothelium homeostasis and vascular health. The present study investigated the relevance of endothelial NO synthase (eNOS) uncoupling to clinical neurotoxic evolution induced by PNV. eNOS immunoblotting of cerebellum lysates processed through low-temperature SDS-PAGE revealed significant increased monomerization of the enzyme at critical periods of severe envenoming (1-2 h), whereas eNOS dimerization reversal paralleled to amelioration of animals condition (5-72 h). Moreover, eNOS uncoupling was accompanied by increased expression in calcium-sensing calmodulin protein and calcium-binding calbindin-D28 protein in cerebellar neurons. It is known that greater eNOS monomers than dimers implies the inability of eNOS to produce NO leading to superoxide production and endothelial/vascular barrier dysfunction. We suggest that transient eNOS deactivation and disturbances in calcium handling reduce NO production and enhance production of free radicals thus contributing to endothelial dysfunction in the cerebellum of envenomed rats. In addition, eNOS uncoupling compromises the enzyme capacity to respond to shear stress contributing to perivascular edema and it is one of the mechanisms involved in the BBBb promoted by PNV.

Angiogênese é um processo de surgimento de novos microvasos provenientes de vasos sanguíneos já existentes. O desenvolvimento tumoral e o processo de metástase são dependentes de angiogênese, pois o tumor em crescimento necessita de uma rede capilar que forneça nutrientes e oxigênio. A membrana corioalantóica de embrião de galinha (CAM) é um modelo experimental in vivo que oferece muitas vantagens, como a alta vascularização natural e alta taxa de reprodução, além de ser um modelo simples e de baixo custo. A CAM é composta por proteínas de matriz extracelular, que mimetizam o ambiente fisiológico de células cancerosas. A etapa de contagem do número total de vasos permite a determinação dos efeitos dos estímulos pró ou anti angiogênicos, portanto a padronização de um método eficaz é necessário. O presente estudo teve como objetivo geral avaliar o potencial angiogênico de células de uma linhagem de adenocarcinoma de cólon humano (HT29) e propor um método para quantificação da angiogênese induzida por células tumorais na CAM. Os embriões foram mantidos em sistema ex ovo. No oitavo dia, foram adicionados sobre a CAM, implantes de colágeno contendo células tumorais em diferentes concentrações. No décimo primeiro dia foi feito o registro fotográfico utilizando microscópio estereoscópico e foram determinados quatro scores para a quantificação e caracterização dos vasos, considerando-se se seccionavam o implante e também seu grau de ramificação. A contagem dos vasos, feita em uma área específica ao redor do implante, foi realizada após edição das imagens pelo programa Image Pro Plus. Os resultados mostraram aumento significativo do número de vasos que não seccionavam o implante para aqueles contendo 3 x 104 e 6 x 104 células. Pode-se concluir que a metodologia de contagem dos vasos, utilizando registros fotográficos e edições de imagens, é eficaz. Demonstrou-se que as células HT29 induzem a uma alteração no padrão de crescimento de novos vasos quando depositada sobre a CAM em implantes de colágeno e podem ser utilizadas como modelo experimental para se investigar o efeito de diferentes compostos sobre a angiogênese induzida por tumor.


Angiogenesis is a process of sprouting of new microvessels from existing blood vessels. The tumoral development and the metastasis process are angiogenesis dependent, because the growing tumor needs a capillaries network that provides nutrients and oxygen. The chicken chorioallantoic membrane (CAM) is an experimental in vivo model which offer many advantages, such as the high natural vascularization and high reproducibility, besides the simplicity and low cost. The CAM contains extracellular matrix proteins, which mimics the physiological cancer cell environment. The counting of the total number of vessels allows a determination of pro- and anti-angiogenic effects of different stimulus, therefore patterning an effective method is necessary. The general goal of the present study was to evaluate the angiogenic potential of a human colon adenocarcinoma cell line (HT29) and propose a method to quantify angiogenesis induced by cancer cells on the CAM. Embryos were cultivated in an ex ovo system. At the eighth day, collagen implants containing cancer cells in different concentrations were added on the top of CAM. At the eleventh day, the photographic records were made by using stereoscope microscope and were determined four scores for vessels quantification and characterization. Vessels counting were done in a specific area around the implant, and edition of the captured images were done using Image Pro Plus software. Our results showed a significant increase in vessels that do not section the implant. It was demonstrated that HT29 cells induce a change in the pattern of growth of new blood vessels when placed on CAM into collagen implants and can be used as an experimental model for investigating the effect of different compounds on tumor-induced angiogenesis.
