Introduction: In oral rehabilitation, the use of ceramic restorations is widely accepted due to its aesthetic capacity to mimic the naturalness of the dental tissue, provide longevity of the material, and present a greater marginal fit compared to crowns with a metal structure. Termination lines are biological preparations whose function is to minimize the cervical opening of the marginal seal. Consequently, analyzing the behavior of restorative materials under compressive forces decreases the risk of fracture and increases the success of the treatment. Objective: To compare the compressive stresses of lithium disilicate crowns with three different finishing lines. Methodology: In silico study of the simulation of a dental preparation on a lower right first molar with chamfer (0.6 mm), shoulder (0.5 mm) and deep chamfer (0.5 mm) finish lines. Using the SolidWorks®ï¸ version 2017 software, the maximum stresses, minimum stresses, and location of the compressive force were collected on the Megapascal (Mpa) measurement scale. Results: The chamfer type termination line (0.6mm) obtained a lower compressive stress compared to the other two shoulder type termination lines (0.5mm) and deep chamfer (0.5mm). Conclusions: It was shown that the chamfer type finishing line (0.6mm) presented a better force distribution, determining greater reliability in the selection of this finishing line with the use of a lithium disilicate crown in a unitary manner.

PURPOSE: Fixed restorations and dental enamel have different structures that produce different wear on opposing teeth, resulting in clinical problems. Therefore, it is necessary to determine the type of restoration that causes less wear on naturally opposing teeth to make recommendations. The objective of this study was to systematically analyze the evidence from observational studies and clinical trials on enamel wear in different ceramic restorations. STUDY SELECTION: The designs of the included studies were randomized clinical trials (RTCs), non-randomized clinical trials (non-RTCs), and observational studies (OS). The studies must answer the research question, be available in full text, be written in English or Spanish, and have had at least six months of follow-up. Protocol number: CRD42023397759. RESULTS: After screening 499 records, 20 RTCs were subjected to data extraction, 10 were excluded, 10 were included in the systematic review, and only 5 were included in the network meta-analysis. The risk of bias assessment reported moderate to high risk of bias, quality, and certainty of evidence was evaluated and rated as moderate. Network meta-analysis showed higher enamel wear was observed in natural dental enamel against metal-ceramic antagonists. CONCLUSIONS: Enamel wear occurs in all teeth, even when the antagonist is a natural tooth. The wear is larger on surfaces with the ceramic crown antagonists studied (metal-ceramic, glazed zirconia, and polished zirconia). It is necessary to conduct additional clinical trials with larger follow-up periods and sample sizes.

OBJECTIVE: To evaluate the effect of different hydrofluoric acid concentrations and etching times on the surface, chemical composition and microstructure of lithium disilicate. MATERIAL AND METHODS: Ninety specimens of pressed lithium disilicate (LDS) were obtained (IPS e.max Press, Rosetta SP and LiSi Press). The specimens of each material were divided in two groups according to the hydrofluoric acid concentration: 5% and 10% (n = 15/group), and subdivided according to the etching time: 20, 40 and 60 s (n = 5/group). Crystalline evaluations and chemical composition were performed through x-ray diffraction (XRD) and energy-dispersive x-ray spectroscopy (EDS), respectively. Microstructural analyses were performed by scanning electron microscope (SEM), surface roughness (Ra), and material thickness removal evaluation. Thickness removal and Ra data were analyzed by ANOVA and Tukey test (p < 0.05). RESULTS: XRD demonstrated characteristic peaks of lithium disilicate crystals, lithium phosphate and of a vitreous phase for all materials. EDS identified different compositions and SEM confirmed different surface responses to acid etching protocols. Material and etching time influenced Ra and material thickness removal (p < 0.05). CONCLUSION: Hydrofluoric acid concentration and etching time affect the surface characteristics of LDS differently. LiSi Press presented higher resistance to hydrofluoric acid etching compared to e.max Press and Rosetta SP. CLINICAL SIGNIFICANCE: Applying the appropriate etching protocol is pivotal to avoid excessive material removal and to prevent jeopardize the mechanical and optical properties of the material.

PURPOSE: To evaluate the reliability and failure modes of ultrathin (0.5 mm) lithium disilicate, translucent and ultra-translucent zirconia crowns for posterior teeth restorations. MATERIALS AND METHODS: Fifty-four mandibular first molar crowns of three ceramic materials: (1) Lithium disilicate (e.max CAD, Ivoclar Vivadent), (2) 3Y-TZP (Zirconn Translucent, Vipi), and (3) 5Y-PSZ (Cercon XT, Dentsply Sirona), with 0.5 mm of thickness were milled and cemented onto composite resin abutments. Eighteen samples of each group were tested under mouth-motion step-stress accelerated life testing in a humid environment using mild, moderate, and aggressive profiles. Data was subjected to Weibull statistics. Use level curves were plotted and reliability was calculated for a given mission of 100,000 cycles at 100, 200, and 300 N. Fractographic analyses of representative samples were performed in scanning electron microscope. RESULTS: Beta (ß) values suggest that failures were dictated by material's strength for lithium disilicate and by fatigue damage accumulation for both zirconias. No significant differences were detected in Weibull modulus and characteristic strength among groups. At a given mission of 100,000 cycles at 100 N, lithium disilicate presented higher reliability (98% CB: 95-99) regarding 3Y-TZP and 5Y-PSZ groups (84% CB: 65%-93% and 79% CB: 37&-94%, respectively). At 200 N, lithium disilicate reliability (82% CB: 66%-91%) was higher than 5Y-PSZ (20% CB: 4%-44%) and not significantly different from 3Y-TZP (54% CB: 32%-72%). Furthermore, at 300 N no significant differences in reliability were detected among groups, with a notable reduction in the reliability of all materials. Fractographic analyses showed that crack initiated at the interface between the composite core and the ceramic crowns due to tensile stress generated at the intaglio surface. CONCLUSIONS: Ultrathin lithium disilicate crowns demonstrated higher reliability relative to zirconia crowns at functional loads. Lithium disilicate and zirconia crown's reliability decreased significantly for missions at higher loads and similar failure modes were observed regardless of crown material. The indication of 0.5 mm thickness crowns in high-load bearing regions must be carefully evaluated. CLINICAL SIGNIFICANCE: Ultraconservative lithium disilicate and zirconia crowns of 0.5 mm thickness may be indicated in anterior restorations and pre-molars. Their clinical indication in high-load requirement regions must be carefully evaluated.

Objective: The aim of this study was to evaluate the marginal gap of frameworks produced using the CAD-CAM system, from zirconia and lithium disilicate blocks, adapted to a tooth preparation and a gypsum die. Material and Methods: For this study, a human first molar tooth was used as a master model with a full crown preparation. It was molded 20 times to obtain the gypsum die and randomly divided into 2 groups (n=10) for the fabrication of zirconia and lithium disilicate frameworks. The frameworks were made using pre-sintered zirconia blocks and lithium disilicate blocks, both CAD-CAM systems. The marginal gap was measured in µm at four points (buccal, palatal, mesial, and distal) using a comparator microscope with 30x magnification, with the framework seated on the master model (tooth), and on the gypsum die. Marginal gap data (µm) were evaluated using two-way analysis of variance and Tukey's test with a significance level of 5%. Results: The results showed that there was no statistically significant interaction between the factors studied (p=0.223) or isolated factors (ceramic factor p=0.886 and die factor p=0.786). Conclusion: Both ceramics produced using the CAD-CAM technique did not exhibit statistical differences in marginal adaptation on the two types of substrates, both on tooth preparation and on the gypsum die (AU)


Objetivo: O objetivo deste estudo foi avaliar o espaço marginal de estruturas produzidas usando o sistema CAD-CAM, a partir de blocos de zircônia e dissilicato de lítio, adaptadas a um preparo sobre dente e a um troquel de gesso. Material e Métodos: Para este estudo, um dente molar humano foi utilizado como modelo mestre com preparo para coroa total. Este foi moldado 20 vezes para obter o troquel de gesso e dividido aleatoriamente em 2 grupos (n=10) para a fabricação de estruturas de zircônia e dissilicato de lítio. As estruturas foram feitas usando blocos de zircônia pré-sinterizados e blocos de dissilicato de lítio, ambos sistemas para CAD-CAM. O espaço marginal foi medido em µm, em quatro pontos (bucal, palatal, mesial e distal), utilizando um microscópio comparador com ×30 de ampliação e com a estrutura assentada no modelo mestre (dente) e no troquel de gesso. Os dados de espaço marginal (µm) foram avaliados usando análise de variância bidirecional e teste de Tukey com um nível de significância de 5%. Resultados: Os resultados mostraram que não houve interação estatisticamente significativa entre os fatores estudados (p=0,223) ou isoladamente (fator cerâmica p=0,886 e fator troquel p=0,786). Conclusão: Ambas as cerâmicas produzidas usando a técnica CAD-CAM não apresentaram diferenças estatísticas em relação à adaptação marginal nos dois tipos de substratos, tanto na preparação dentária quanto no troquel de gesso(AU)

Abstract This study evaluated the effect of different firing protocols on the surface roughness and flexural strength of CAD/CAM lithium disilicate glass-ceramic (LD) after aging methods. Forty-two LD bars of 16 x 4 x 2 mm (IPS e-max CAD, Ivoclar) were randomly separated into two groups according to firing protocols: Single firing-Staining, glazing, and crystallization in a single step; Multiple firings-Crystallization+First staining+Firing+Second staining+Firing+Glazing+Firing. After protocols, initial surface roughness readings were taken (Surfcorder SE1700, Kosakalab). Samples were then randomly separated into three groups (n=7) according to the aging methods they were submitted: Thermomechanical cycling (TMC, ER System, Erios, 1,200,000 cycles, 0.3 MPa, 2 Hz and 5°C/37°C/55°C, 30 s swell time); Simulated toothbrushing (STB, Pepsodent, MAVTEC, 73,000 cycles), and Control (no aging). Final surface roughness readings were done, and samples were submitted to a three-point bending test (OM100, Odeme Dental Research) and fractographic analysis by scanning electron microscopy (EVO-MA10, ZEISS). Data were analyzed (2-way ANOVA, (α=.05). There was no difference (p>.05) in the flexural strength between the firing protocols, regardless of the aging method. STB decreased the flexural strength of samples submitted to multiple firings, different from control (p<.05). Without aging (Control), before TMC, and after STB, LD had lower surface roughness when submitted to multiple firings than to single firing (p<.05). The firing protocols did not affect the flexural strength or the surface roughness of the lithium disilicate glass-ceramic, even after aging. However, toothbrushing negatively affected the flexural strength and smoothed the surface of the ceramic submitted to multiple firings.


Resumo Este estudo avaliou o efeito de diferentes protocolos de queima na rugosidade de superfície e resistência à flexão de uma cerâmica vítrea de dissilicato de lítio (DL) para CAD/CAM, após métodos de envelhecimento. Quarenta e dois barras de DL (IPS e-max CAD, Ivoclar) de 16 x 4 x 2 mm foram separadas aleatoriamente em dois grupos de acordo com os protocolos de queima: Single firing-Pigmentação, glazeamento e cristalização em única etapa. Multiple firings-Cristalização+Primeira camada do pigmento+Queima+Segunda camada do pigmento+Queima+Glazeamento+Queima. Após os protocolos, foram realizadas leituras iniciais de rugosidade de superfície (Surfcorder SE1700, Kosakalab). As amostras foram então separadas aleatoriamente em três grupos (n=7) segundo o método de envelhecimento a que foram submetidas: Ciclagem termomecânica (CTM, Sistema ER, Erios, 1.200.000 ciclos, 0.3 MPa, 2 Hz e 5°C/37°C/55°C, 30 s de imersão), Escovação simulada (ES, Pepsodent, MAVTEC, 73.000 ciclos) e Controle (sem envelhecimento). Leituras finais de rugosidade foram realizadas, e as amostras foram submetidas ao ensaio de resistência à flexão em três pontos (OM100, Odeme Dental Research) e à análise fractográfica por microscopia eletrônica de varredura (EVO-MA10, ZEISS). Os dados foram analisados (ANOVA de dois fatores, (α=.05). Não houve diferença (p>.05) na resistência à flexão entre os protocolos de queima, independente do envelhecimento. ES diminuiu a resistência à flexão das amostras submetidas a multiple firings, diferente do controle (p<.05). Sem envelhecimento (Controle), antes da CTM e após ES, LD apresentou menor rugosidade de superfície quando submetido a multiple firings do que a single firing (p<.05). Os protocolos de queima não afetaram a resistência à flexão nem a rugosidade de superfície da cerâmica vítrea de dissilicato de lítio, mesmo após envelhecimento. No entanto, a escovação afetou negativamente a resistência à flexão e poliu a superfície da cerâmica submetida a multiple firings.

Amongst chairside CAD/CAM materials, the use of lithium-based silicate glass-ceramics (LSGC) for indirect restorations has recently been increasing. Flexural strength is one of the most important parameters to consider in the clinical selection of materials. The aim of this paper is to review the flexural strength of LSGC and the methods used to measure it. METHODS: The electronic search was completed within PubMed database from 2 June 2011 to 2 June 2022. English-language papers investigating the flexural strength of IPS e.max CAD, Celtra Duo, Suprinity PC, and n!ce CAD/CAM blocks were included in the search strategy. RESULTS: From 211 potential articles, a total of 26 were identified for a comprehensive analysis. Categorization per material was carried out as follows: IPS e.max CAD (n = 27), Suprinity PC (n = 8), Celtra Duo (n = 6), and n!ce (n = 1). The three-point bending test (3-PBT) was used in 18 articles, followed by biaxial flexural test (BFT) in 10 articles, with one of these using the four-point bending test (4-PBT) as well. The most common specimen dimension was 14 × 4 × 1.2 mm (plates) for the 3-PBT and 12 × 1.2 mm (discs) for BFT. The flexural strength values for LSGC materials varied widely between the studies. SIGNIFICANCE: As new LSGC materials are launched on the market, clinicians need to be aware of their flexural strength differences, which could influence the clinical performance of restorations.

OBJECTIVES: To assess the probability of survival of crowns made with a 3Y-TZP, a 5Y-TZP, and a lithium disilicate. MATERIALS AND METHODS: CAD-CAM premolar crowns with occlusal thicknesses of 1.0 mm or 1.5 mm and cemented on a metal dye. Step-stress accelerated life testing (SSALT) was performed to access the use level probability Weibull curves and reliability were calculated for the completion of a mission of 100,000 cycles at 300 to 1200 N. RESULTS: All ceramics showed a high probability of survival (87-99%) at 300 N, irrespective of thickness. 3Y-TZP shows no significant decrease in the probability of survival up to 1200 N (83-96%). Lithium disilicate presented lower reliability than zirconia under the 600 N mission. 5Y-TZP showed lower reliability than 3Y-TZP at 1200 N. There was no significant difference in the Weibull modulus (3.23-7.12). 3Y-TZP had the highest characteristic strength (2483-2972 N), followed by 5Y-TZP (1512-1547 N) and lithium disilicate (971-1154 N). CONCLUSION: Zirconia ceramics have a high probability of survival (up to 900 N-load), while lithium disilicate survives up to 300 N load only, irrespective of the thickness (1.0 mm or 1.5 mm). CLINICAL RELEVANCE: The probability of survival of posterior crowns made with zirconia ceramics resists extreme loads while glass ceramics resist normal chewing loads. In addition, crowns with thinner occlusal face showed sufficient mechanical behavior.

OBJECTIVE: The objective of this study was to assess the influence of immediate dentin sealing (IDS) on the fatigue behavior of laminate occlusal veneers fabricated with CAD/CAM lithium disilicate ceramic and resin composite. METHODS: Forty sound human molars were prepared and randomly divided into 4 groups (n = 10): RC-IDS+ (IDS and resin composite occlusal laminate veneer); RC-IDS- (resin composite occlusal laminate veneer without IDS); LD-IDS+ (IDS and lithium disilicate laminate veneer); LD-IDS- (lithium disilicate occlusal laminate veneer without IDS). The restorations were obtained using a digital workflow. After surface conditioning and bonding, thermocycling and accelerated fatigue tests (20 Hz, 5000 cycles with an initial load of 300 N, step-size of 100 N for 10,000 cycles, up to 1000 N, and then a step-size of 50 N until failure) were conducted. Fatigue data were recorded for both outcomes (crack or fracture) and statistically analyzed. Fractographic and adhesive interface analysis were conducted. RESULTS: The indirect resin composite groups showed better fatigue behavior compared to lithium disilicate. IDS only had a positive effect for the survival of resin composite restorations for the 'fracture' outcome. Evident presence of micro-gaps at the adhesive interface in the LD-IDS- group could be noted. SIGNIFICANCE: Immediate dentin sealing improved fatigue resistance behavior of resin composite occlusal veneers. However, this effect was not observed in lithium disilicate veneers.

OBJECTIVES: To evaluate the effect of the remaining tooth structure and different CAD/CAM materials on the fatigue performance and failure mode of endodontically treated premolars restored with endocrowns. MATERIALS AND METHODS: Ninety maxillary premolars were endodontically treated and assigned into 6 groups (n = 15) according to the number of remaining axial walls (four, three, and two) and restorative materials (ultra-translucent zirconia 5Y-PSZ [KATANA UTML] and lithium disilicate [IPS e.max-CAD]). The specimens were subjected to cyclic fatigue loading test (initial load 200 N; 20 Hz). An incremental step load of 100 N per 10,000 cycles was applied until failure. The fatigue failure load (FFL) and number of failure cycles (CFFs) data were statistically analyzed with two-way ANOVA and Kaplan-Meier test (α = 0.05). Failed specimens were examined under a stereomicroscope 25 × and failure modes were determined. RESULTS: FFL and CFF were significantly influenced by restorative material (p < 0.05). 5Y-PSZ endocrowns showed significantly higher FFL when compared with lithium disilicate. The number of remaining walls did not affect the fatigue behavior or failure mode of the specimens. Of the lithium disilicate restorations, 51% had repairable failures, while 95% 5Y-PSZ restorations had non-repairable failures. CONCLUSIONS: Zirconia endocrowns showed better FFL than lithium disilicate endocrowns, regardless of the number of remaining axis walls. Lithium disilicate and 5Y-PSZ endocrowns showed FFL higher than the normal masticatory loads. CLINICAL RELEVANCE: Restoring endodontically treated premolars with endocrown could be a promising treatment, regardless of the remaining axial walls. However, precaution should be taken in material selection since it affects the fatigue resistance and failure mode.

Mediante este caso se describen los criterios de selección del color en restauraciones indirectas con disilicato de litio en el sector anterior. Mujer de 34 años, atendida en la Facultad de Odontología de la Universidad Nacional de Concepción (FOUNC), Paraguay, por una preocupación estética del sector anterior, en especial la diferencia de color de las piezas dentarias. La rehabilitación consistió en la confección de coronas de disilicato de litio de las piezas 11, 12, 13, 14, 21, 22, 24, 25 y laminado para carillas de la pieza 23. Fueron aplicadas técnicas de selección de color individualizadas, pruebas de color y cementación antes del cementado final para obtener resultados altamente estéticos. Gracias a los resultados fue posible observar la conjunción de factores como la correcta selección del color del sustrato dentario, el cemento resinoso, el material de restauración y la comunicación eficiente con el laboratorio, fundamental para el éxito del tratamiento.


This case describes the color selection criteria for indirect lithium disilicate restorations in the anterior sector. A 34-year-old woman treated at the Faculty of Dentistry of the National University of Concepción (FOUNC), Paraguay, due to an aesthetic concern in the anterior sector, especially the difference in color of the teeth. The rehabilitation consisted of making lithium disilicate crowns for teeth 11, 12, 13, 14, 21, 22, 24, 25 and laminate for veneers for tooth 23. Individualized color selection techniques, color and cementation prior to final cementation for highly esthetic results. Thanks to the results, it was possible to observe the conjunction of factors such as the correct selection of the color of the dental substrate, the resinous cement, the restorative material, and efficient communication with the laboratory, essential for the success of the treatment.

O objetivo deste estudo foi avaliar a influência do sistema de cimentação, do tipo de material empregado para confecção do pilar intermediário e do tipo de material empregado para a confecção da coroa na força de união de próteses cimentadas sobre implantes. Para simular o desempenho do pilar noventa espécimes de zircônia (IPS E.MAX ZirCAD, Ivoclar Vivadent) e noventa espécimes de titânio (O'ring Capsule, Peclab) foram obtidos. A rugosidade superficial foi padronizada utilizando-se uma politriz circular com lixa de granulação 600 durante 1 minuto. Os espécimes que simularam as coroas foram confeccionados a partir de blocos de resina (Grandio Blocs, Voco), dissilicato de lítio (IPS E.MAX eCAD, Ivoclar Vivadent) e de zircônia translúcidos (Zirkom, OdontoMega) com 3mm de diâmetro e 4 mm de altura. Cada tipo de material de pilar recebeu a cimentação de todos os três tipos de materiais que simulam as coroas. Para cada combinação foram testados três diferentes cimentos, sendo eles Bifix Hybrid Abutment (Voco), Multilink Abutment (Ivoclar Vivadent) e Vita Adiva (VITA). Os espécimes foram submetidos a um ensaio de cisalhamento a uma velocidade de 1 mm/min. Os dados foram analisados usando ANOVA a três fatores (pilar x coroa x cimento) seguido pelo teste de Tukey (α = 5%). Os resultados mostraram que houve diferenças significativas para o tipo de pilar e de coroa (p<0.05) e para o tipo de cimento (p<0.05). Pode-se observar que o cimento Bifix propiciou uma resistência adesiva significativamente menor que o Vita, enquanto o Multilink resultou em valores intermediários. Observou-se também que a resistência de união ao titânio foi menor do que à zircônia para infraestrutura. A resistência de união entre a zircônia e a zircônia de infraestrutura obtiveram valores significativamente maiores. Podemos concluir que o tipo de sistema de cimentação, o tipo de pilar e o tipo de coroa influenciaram significativamente na força de união entre os espécimes de simularam as coroas e os espécimes que simularam o pilar intermediário. O cimento Bifix Abutment proporcionou os menores valores enquanto o Vita Adiva os maiores. A resistência de união ao pilar de titânio foi menor do que à zircônia para infraestrutura, enquanto a adesão à coroa de resina composta foi menor do que ao dissilicato de lítio, que por sua vez foi menor do que à zircônia translúcida.(AU)


The aim of this study was to evaluate the influence of the cementation system, the type of material used to make the intermediate abutment and the type of material used to make the crown on the bond strength of implant-cemented prostheses. To simulate the performance of the abutment, ninety zirconia specimens (IPS E.MAX ZirCAD, Ivoclar Vivadent) and ninety titanium specimens (O'ring Capsule, Peclab) were obtained. Surface roughness was standardized using a circular polisher with 600-grit sandpaper for 1 minute. The specimens that simulated the crowns were made from resin blocks (Grandio Blocs, Voco), lithium disilicate (IPS E.MAX eCAD, Ivoclar Vivadent) and translucent zirconia blocks (Zirkom, OdontoMega) with 3 mm in diameter and 4 mm height. Each type of abutment material was cemented from all three types of crown-simulating materials. For each combination, three different cements were tested, Bifix Hybrid Abutment (Voco), Multilink Abutment (Ivoclar Vivadent) and Vita Adiva (VITA). Specimens were secured to a shear test at a speed of 1 mm/min. Data were analyzed using three-way ANOVA (abutment x crown x cement) followed by Tukey's test (α = 5%). The results showed significant differences for the type of abutment and crown (p<0.05), and for the type of cement (p<0.05). It can be observed that Bifix cement provided significantly lower bond strength than Vita, while Multilink evolved in intermediate values. It was observed the tensile bond strength was lower than backbone zirconia. The bond strength between zirconia crown and zirconia infrastructure obtained significantly higher values. We can conclude that the type of cementation system, the type of abutment and the type of crown significantly influenced the bond strength between the crowns and the intermediate abutment. Bifix Abutment cement introduced the lowest values while Vita Adiva presented the highest values. The strength adhesion to the titanium abutment was lower than to the infrastructure zirconia, while the adhesion to the composite resin crown was lower than to the lithium disilicate, which in turn was lower than to the translucent zirconia (AU)

Objective: To evaluate the biaxial flexural strength (BFS) of lithium disilicate (L), cemented on different substrates (epoxy resin - E and metal - M) with dual-cure resin cement (Rc) and zinc phosphate cement (Zc), not aged, thermally aged (TC) or thermo-mechanical aged (TC/MC). Material and Methods: Disks of L, E, and M were fabricated, and the cementation was performed according to the following groups: ERc (L+E+Rc); MRc (L+M+Rc); MZc (L+M+Zc); EZc (L+E+Zc). Ten samples from each described group were tested in BFS, ten more samples were subjected to TC (1×104 cycles between 5 ºC and 55 ºC water), and the last 10 samples were subjected to TC/MC (MC: 1.2×106 cycles, 50 N, 3.8 Hz). The BFS test was performed and scanning electron microscopy (SEM) was performed to evaluate the failure mode. The effect of the cementation strategy (cement/substrate) was compared in each aging method and the effect of the aging method was evaluated for each cementation strategy by one-way ANOVA and Tukey post-hoc test (α=0.05). Results: The strength values were highest to M (237.8 ~ 463.9 MPa), in comparison to the E (41.03 ~ 66.76 MPa), despite aging and luting agent. Flexural strength data decreased after TC and TC/MC in groups cemented with Zc, but was stable when cemented with Rc. SEM analysis indicated that failure origins were located at the tensile surface of the L. Conclusion: Lithium disilicate discs cemented to the metallic substrate presented the highest biaxial flexural strength. The cementation with dual-cure resin cement did not decrease BFS after aging (AU)


Objetivo: Avaliar a resistência à flexão biaxial (BFS) do dissilicato de lítio (L), cimentado sobre diferentes substratos (resina epóxi - E e metal - M) com cimento resinoso dual (Rc) e cimento de fosfato de zinco (Zc), não envelhecido, submetido ao envelhecido térmico (TC) ou ao envelhecido térmico-mecânico (TC/MC). Material e Métodos: Foram confeccionados discos de L, E e M, e a cimentação foi realizada de acordo com os seguintes grupos: ERc (L+E+Rc); MRc (L+M+Rc); MZc (L+M+Zc); EZc (L+E+Zc). Dez amostras de cada grupo descrito foram testadas em BFS, mais dez amostras foram submetidas à TC (1×104 ciclos de imersão em água entre 5 ºC e 55 ºC), e as últimas 10 amostras foram submetidas à TC/MC (MC: 1.2 ×106 ciclos, 50 N, 3.8 Hz). Foram realizados os testes de BFS e a microscopia eletrônica de varredura (MEV) para avaliar o modo de falha. O efeito da estratégia de cimentação (cimento/substrato) foi comparado em cada método de envelhecimento e o efeito do método de envelhecimento foi avaliado para cada estratégia de cimentação por ANOVA a um fator e teste post-hoc de Tukey (α=0,05). Resultados: Os valores de resistência foram maiores para M (237.8 ~ 463.9 MPa), em comparação com E (41.03 ~ 66.76 MPa), independentemente do envelhecimento e do agente cimentante utilizado. Os dados de resistência à flexão diminuíram após TC e TC/MC nos grupos cimentados com Zc, mas se mantiveram estáveis quando cimentados com Rc. A análise MEV indicou que a origem das falhas estava localizada na superfície de tração do L. Conclusão: Os discos de dissilicato de lítio cimentados ao substrato metálico apresentaram maior resistência à flexão biaxial. A cimentação com cimento resinoso dual não diminuiu o BFS após o envelhecimento (AU)

Objective: The present study aims to evaluate the color stability and degree of conversion of amine-free dual cured resin cement compared to light cured and amine-containing dual cured resin cements used with two different translucencies of thin esthetic restorations. Material and Methods: A total of 120 specimens were prepared for color stability testing (n=60). The specimens were divided into three main groups according to the resin cement type. Group 1: amine-free dual cured, Group 2: light cured, Group 3: amine-containing dual cured. Each group was further subdivided according to the ceramic translucency into two subgroups: high and low translucency. Color stability was assessed by a spectrophotometer before and after thermal aging. For the degree of conversion assessment (n=60), Fourier transform infrared spectroscopy was used at three different time intervals. Statistical analysis was performed using multi-factorial ANOVA, followed by one-way ANOVA with Bonferroni correction. Results: Amine-containing resin cement showed significantly higher ΔEab and ΔE00in both translucencies (4.5±0.3, 3.5±0.3 respectively for high translucency ceramic and 3.8±0.4, 3.0±0.3 respectively for low translucency) than the other tested cements (p<0.001). The highest degree of conversion (DC) was shown after 2 weeks by the amine-free dual cured resin cement (86.27±0.74). Conclusion: Amine-free dual cured resin cement can be an alternative to light cured one for cementation of thin veneers since it showed comparable color stability and high degree of conversion (AU)


Objetivo: O presente estudo tem como objetivo avaliar a estabilidade de cor e o grau de conversão do cimento resinoso dual sem amina em comparação com cimentos resinosos fotopolimerizáveis contendo amina usados com duas translucidezes diferentes em restaurações estéticas definitivas. Material e Métodos: Um total de 120 espécimes foram preparados para teste de estabilidade de cor (n=60). Os espécimes foram divididos em três grupos principais de acordo com o tipo de cimento resinoso. Grupo 1: polimerização dupla sem amina, Grupo 2: fotopolimerização, Grupo 3: polimerização dupla contendo amina. Cada grupo foi ainda subdividido de acordo com a translucidez da cerâmica em dois subgrupos: alta e baixa translucidez. A estabilidade da cor foi avaliada por um espectrofotômetro antes e após o envelhecimento térmico. Para a avaliação do grau de conversão (n=60), a espectroscopia de infravermelho com transformada de Fourier foi usada em três intervalos de tempo diferentes. A análise estatística foi realizada usando ANOVA multifatorial, seguida de ANOVA um faot com correção de Bonferroni. Resultados: O cimento resinoso contendo amina apresentou ΔEab e ΔE00 significativamente maiores em ambas as translucidezes (4,5±0,3, 3,5±0,3 respectivamente para cerâmica de alta translucidez e 3,8±0,4, 3,0±0,3 respectivamente para baixa translucidez) do que os outros cimentos testados (p< 0,001). O maior grau de conversão (DC) foi mostrado após 2 semanas pelo cimento resinoso dual sem amina (86,27±0,74). Conclusão: O cimento resinoso dual sem amina pode ser uma alternativa ao cimento polimerizável na restauração de facetas finas, uma vez que apresentou estabilidade de cor comparável e alto grau de conversão. Objective: the aim of this study was to compare the mechanical behavior of different ceramics when used in thin vertical preparations versus traditional horizontal preparation. Material and Methods: two stainless-steel dies were milled to simulate a minimally invasive vertical preparation (VP) and a traditional horizontal preparation (HP) for an all-ceramic crown of a maxillary first premolar. The stainless-steel dies were duplicated using epoxy resin. Eighty monolithic crowns were milled and divided into 2 groups according to preparation design. Each design group was subdivided into 4 sub-groups according to material (n=10): IPS e.max CAD (lithium disilicate), Bruxzir shaded zirconia (full contour zirconia), CeraSmart (resin nanoceramic) and CEREC Tessera (advanced lithium disilicate). The crowns were cemented on their relevant epoxy resin dies using self-adhesive resin cement. All specimens were subjected to 15,000 thermocycles and then loaded to fracture in a universal testing machine. Data were analyzed using two-way ANOVA and Tukey pair wise comparison test. Results: the fracture resistance mean values of the VP (1344 + 118 N) was significantly lower than the HP design (1646 + 191 N). Ceramic crowns made of full contour zirconia had higher fracture resistance mean values (2842 + 380 N) than advanced lithium disilicate (1272 + 125 N) followed by lithium disilicate crowns (983 + 52 N) and resin nanoceramic (882 + 61 N). Conclusion: both vertical and horizontal preparations, regardless the different ceramic materials, showed clinically acceptable fracture resistance values. (AU)


Objetivo: o objetivo deste estudo foi comparar o comportamento mecânico de diferentes cerâmicas quando utilizadas em preparos verticais finos ou preparos horizontais tradicionais. Material e Métodos: dois modelos de aço inoxidável foram fresados para simular um preparo vertical minimamente invasivo (PV) e um preparo horizontal tradicional (PH) para uma coroa totalmente em cerâmica de um primeiro pré-molar superior. As matrizes de aço inoxidável foram duplicadas usando resina epóxi. Oitenta coroas monolíticas foram fresadas e divididas em 2 grupos de acordo com o desenho do preparo. Cada grupo foi subdividido em 4 subgrupos de acordo com o material (n=10): IPS e.max CAD (dissilicato de lítio), zircônia Bruxzir (zircônia de contorno total), CeraSmart (resina nanocerâmica) e CEREC Tessera (dissilicato de lítio avançado). As coroas foram cimentadas em suas respectivas matrizes de resina epóxi usando cimento resinoso autoadesivo. Todos os espécimes foram submetidos a 15.000 ciclos térmicos e então carregados até a fratura em uma máquina de teste universal. Os dados foram analisados usando ANOVA com dois fatores e teste de comparação por pares de Tukey. Resultados: os valores médios de resistência à fratura do PV (1344 + 118 N) foram significativamente menores do que PH (1646 + 191 N). As coroas de cerâmica feitas de zircônia de contorno total apresentaram maiores valores médios de resistência à fratura (2842 + 380 N) do que dissilicato de lítio avançado (1272 + 125 N), seguido por coroas de dissilicato de lítio (983 + 52 N) e resina nanocerâmica (882 + 61 N). Conclusão: preparos verticais e horizontais, independentemente dos diferentes materiais cerâmicos, apresentaram valores de resistência à fratura clinicamente aceitáveis. (AU)

Resumo Este estudo teve como objetivo avaliar a resistência à fractura do silicato de lítio reforçado com zircônio todo o material cerâmico "Celtra Press" em comparação com um "IPS e-max Press" em condições orais simuladas. Quatorze coroas cerâmicas foram fabricadas em moldes epóxi que foram duplicados a partir de moldes principais de aço inoxidável e divididos em dois grupos iguais (n=7) de acordo com o material de construção; Grupo A: Coroas fabricadas com material IPS e-max Press e Grupo B: Coroas fabricadas com material Celtra Press. As coroas foram então cimentadas nos seus moldes correspondentes com um cimento de resina auto-adesivo e submetidas a ciclos térmicos e cargas cíclicas. Em seguida, foram carregadas para fraturar numa máquina universal de ensaios. Os resultados foram tabulados e analisados estatisticamente utilizando os testes Kolmogorov-Smirnov e Shapiro-Wilk. O teste t de Student usado para comparar os valores médios. O nível de significância foi fixado em P ≤ 0,05 e intervalo de confiança de 95%. A análise estatística foi realizada utilizando o software Graph Pad Instat (Graph Pad, Inc.) para Windows. Os valores médios ± SD da resistência à fractura foram registados para o grupo Disilicate de lítio (1706,01 ±154,32 N) enquanto que os valores médios ± SD registados com o grupo celtra foram (1550,67±196,71 N). As coroas cerâmicas de silicato de lítio reforçado com zircónio produziram valores de resistência à fractura comparáveis aos das coroas cerâmicas de dissilicato de lítio e ambos os materiais testados estão dentro dos valores clinicamente aceitáveis na área posterior.


Abstract The aim of this study was conducted to assess the fracture resistance of zirconia reinforced lithium silicate all ceramic material "Celtra Press" compared to lithium disilicate one "IPS e-max Press" under simulated oral conditions. Fourteen ceramic crowns were fabricated on epoxy dies which were duplicated from stainless steel master die and divided into two equal groups (n=7) according to the material of construction; Group A: Crowns fabricated with IPS e-max Press material and Group B: Crowns fabricated with Celtra Press material. The crowns were then cemented onto their corresponding dies with a self-adhesive resin cement and subjected to thermocycling and cyclic loading. Then they were loaded to fractur in a universal testing machine. The results were tabulated and statistically analyzed using Kolmogorov-Smirnov and Shapiro-Wilk tests. Student t-test used to compare mean values. The significance level was set at P ≤ 0.05 and 95% Confidence interval. Statistical analysis was performed using Graph Pad Instat (Graph Pad, Inc.) software for windows. The mean ± SD values of fracture resistance were recorded for lithium Disilicate group (1706.01 ±154.32 N) meanwhile the mean ± SD value recorded with celtra group were (1550.67±196.71 N). Zirconia reinforced lithium silicate ceramic crowns produced comparable fracture resistance values to lithium disilicate ceramic crowns and both tested materials are within the clinically acceptable values in the posterior area.

Abstract To evaluate stress distribution when applying vertical and tangential forces to 1mm thick occlusal veneers with different finish lines preparations, using the finite element method. One extracted third molar was prepared for occlusal veneers, firstly without any bevel. It was scanned in order to design two groups of lithium disilicate (G1A, G2A) and composite resin (G1B, G2B) occlusal veneers. Then, the third molar preparation was modified, beveling the finish line and it was subsequently scanned again to design the occlusal veneer groups with bevel (LD: G3A, G4A and CR: G3B, G4B). The four groups were subjected to different forces (400 N vertical and 900 N tangential). At 400 N, the non-beveled veneers showed slightly higher Von Mises stress values (G1A: 783 MPa and G1B 736.5 MPa) than the beveled veneers (G3A: 685.7 MPa and G3B: 675.8 MPa). However, when 900 N tangential forces were applied, the beveled occlusal veneers showed higher Von Mises stress values (G4A: 4297 MPa and G4B: 4133 MPa) than the non-beveled occlusal veneers (G2A: 2581.1 MPa and G2B: 3519.1 MPa). Furthermore, it was observed that the tissue under the occlusal veneers with bevel showed higher Von Mises stress values than the models without any bevel. Beveled and non-beveled occlusal veneers of lithium disilicate and composite resin presented similar stress distribution values when vertical forces of 400 N were applied; whereas with tangential forces of 900 N applied near to the finish line, the beveled groups presented notably higher stress values than the non-beveled groups. However, both finish line preparations presented adequate values for possible clinical performance.


Resumen Evaluar la distribución de estrés al aplicar fuerzas verticales y tangenciales en carillas oclusales de 1mm de espesor con diferentes líneas de terminación marginal por el método de elementos finitos. Se preparó un tercer molar extraído para carilla oclusal, primero sin bisel. Se escaneó para diseñar dos grupos de carillas oclusales de disilicato de litio (G1A, G2A) y resina compuesta (G1B, G2B). Después, esta preparación molar fue modificada, biselando la línea de terminación y escaneándola nuevamente para diseñar los grupos de carillas oclusales con bisel (DL: G3A, G4A and RC: G3B, G4B). Los 4 grupos fueron sometidos a fuerzas diferentes (400 N vertical y 900 N tangencial). Con 400 N, las carillas sin bisel presentaron ligera mayor tensión de Von Mises (G1A: 783 MPa y G1B 736.5 MPa) que las carillas con bisel (G3A: 685.7 MPa y G3B: 675.8 MPa). De distinta forma, al aplicar fuerzas tangenciales de 900 N, las carillas oclusales con bisel presentaron mayor tensión de Von Mises (G4A: 4297 MPa y G4B: 4133 MPa) que las carillas oclusales sin bisel (G2A: 2581.1 MPa y G2B: 3519.1 MPa). Además, se observó que los tejidos subyacentes a las carillas oclusales con bisel, presentaron mayor tensión de Von Mises, frente a los modelos sin bisel. Las carillas oclusales con y sin bisel de disilicato de litio y resina compuesta presentaron una distribución de estrés similar con fuerzas verticales de 400 N, por otro lado, con fuerzas tangenciales de 900 N, los grupos con bisel presentaron notablemente mayor tensión que los grupos sin bisel. Sin embargo, ambos diseños de terminación marginal presentaron valores adecuados para un posible desempeño clínico.

ABSTRACT: Nowadays, maxillary midline diastema (MMD) can be treated with different multidisciplinary approaches. When restorative dentistry is needed, glass-matrix ceramic materials is one of the best choices, since they present good optical behavior and high survival rates in the anterior dentition. To obtain an adequate interphase, and color integration are one of the main restorative goals, and for that purpose, specific finishing and polishing procedures must be employed to avoid staining and ensure the restoration's color stability. In the case report presented in this article, a single lithium disilicate ceramic fragment was performed to close a MMD produced by the shape alteration of one of the maxillary central incisors. Also, the finishing and polishing procedure is discussed.

This study aimed to evaluate the effect of three hydrofluoric acid (HF) etching periods on the micro-tensile bond strength between two CAD-CAM ceramic systems [Vita Suprinity (VS) and feldspathic CEREC blocs (CB)] and a composite resin. The ceramics were categorized into six groups based on the surface conditioning protocol used, as follows: G1: CB-HF 5% for 20 s; G2: CB-HF 5% for 40 s; G3: CB-HF 5% for 60 s; G4: VS-HF 5% for 20 s; G5: VS-HF 5% for 40 s; G6: VS-HF 5% for 60 s. Scotchbond Universal was applied onto the pretreated ceramic surfaces and covered with Filtek Z350 XT composite resin. After 24 h, the specimens were cut into microbars (n = 16) and a micro-tensile bond strength test (µTBS) was carried out. An optical microscope was used to examine the fractured microbars. The results showed statistically significant differences between the factors tested (p < 0.01). Moreover, the mean MPa of G1(17.27), G2(13.03), G3(12.82), G4(15.83), G5(21.66), and G6(14.50) was seen to significantly differ. The predominant failure type observed was adhesive, and all three periods of HF etching produced satisfactory bonding between the composite resin and CB. An etching time of 40 s provided the highest µTBS value for VS.

The aim of this study was to evaluate the amount of radiating energy transmitted through lithium disilicate discs of different thicknesses and opacities with different curing units (CU). Discshaped specimens 8 mm in diameter and 1.5 mm or 0.7 mm thick were designed with Zbrush® and Cura 3D® software, milled from HT and MO IPS e-maxCAD blocks (Ivoclar Vivadent) with Ceramill Mikro 4 (Amann Girrbach, Austria) and processed according to manufacturer instructions. Thickness was checked with a Mitutoyo (México) caliper. Four groups (n=4) were formed: G1(HT/1.5 mm), G2(MO/1.5 mm), G3(HT/0.7 mm) y G4(MO/0.7 mm), and the effect of three CUs: Coltolux (Coltene- Whaledent), LED.C, (Woodpecker Medical Instrument Co.Ltd) and Deepcure L(3M-ESPE) was tested Measurements were taken with a halogen and LED radiometer (Woodpecker® LM-1-Guilin Woodpecker Medical Instrument Co.Ltd) after applying the guide of each CU directly on the reading window (d) and after interposing each of the specimens. The ratio of transmitted energy was determined in each case and data was analyzed with repeated measures ANOVA and Tukey test for multiple comparisons. LED.C: d:1600 mW/cm2, G1: 0.31(0.00), G2: 0.14(0.00) G3: 0.54(0.01), G4: 0.38(0.01); Deepcure L: d:1500 mW/cm2, G1: 0.34(0.01), G2: 0.20(0,00), G3: 0.56(0.01), G4: 0.41(0.02); Coltolux: d:1275 mW/cm2, G1: 0.44(0.01), G2: 0.24(0,00), G3: 0.65(0.01), G4: 0.47(0,00). Statistically significant differences were found among the curing units (P<0.001) and for the interaction CU-thickness (P<0.001) and CU-opacity (P=0.023). Within the conditions of this study, the ratio of light transmitted through lithium disilicate structures is related to their thickness and opacity, and to the curing unit employed.


Uno de los factores relacionados con éxito de las restauraciones y prótesis de cerámica de matriz vítrea es el logro de una fijación adhesiva para lo cual la correcta polimerización del medio de fijación es un requisito fundamental. El objetivo de este trabajo fue valorar la transmitancia de una cerámica a base de disilicato de litio de diferentes opacidades y espesores, con distintas unidades de curado. Probetas con forma de discos de 8mm de diámetro y dos diferentes espesores (1.5 mm y 0.7 mm) fueron diseñadas con Zbrush® y Cura 3D®, maquinadas a partir de bloques HT y MO de IPS e-maxCAD (Ivoclar Vivadent) con Ceramill Mikro 4 (Amann Girrbach, Austria) y procesadas según instrucciones del fabricante. Se obtuvieron cuatro grupos (n=4): G1(HT/1.5 mm), G2(MO/1.5 mm), G3(HT/0.7 mm) y G4(MO/0.7 mm),los espesores se controlaron con un calibre (Mitutoyo, México). A continuación, se registró la irradiancia con cada una de las unidades de curado (UC) Coltolux (Coltene- Whaledent) LED.C, (Woodpecker Medical Instrument Co.Ltd) y Deepcure L(3M-ESPE), aplicadas sobre la ventana de un radiómetro (Halogen and led radiometer Woodpecker® LM-1-Guilin Woodpecker Medical Instrument Co.Ltd) en forma directa (d), y luego de interponer los diferentes especímenes y se calculó la proporción de energía transmitida en cada caso. Los datos se analizaron con ANOVA de medidas repetidas y prueba de Tukey para las comparaciones múltiples. LED.C: d:1600 mw/ cm2, HT/0,7mm: 0,54(0,01), HT/1,5mm:0,31(0); MO/0,7mm: 0,38(0,01), MO/1,5mm: 0,14(0,00); Deepcure L: d:1500 mW/ cm2, HT/0,7 mm: 0,56(0,01), HT/1,5 mm: 0,34(0,01), MO/0,7 mm: 0,41(0,02), MO/1,5 mm: 0,20(0,00); Coltolux: d:1275 mw/ cm2, HT/0,7 mm:0,65 (0,01), HT/1,5 mm:0,44(0,01), MO/0,7 mm: 0,47(0,00), MO/1,5mm: 0,24(0,00). Se encontraron diferencias estadísticamente significativas entre las diferentes unidades de curado (P<0,001), y en la interacción unidades de curado-espesor (P<0,001) y unidades de curado-opacidad (P=0,023). La proporción de energía luminosa transmitida por una estructura de cerámica de disilicato de litio depende del espesor y opacidad de la restauración y de la unidad de curado utilizada.
