Background: The debate on percutaneous coronary intervention (PCI) of the unprotected left main coronary artery (LMCA) has been constant over time. Objective: To investigate the clinical and procedural characteristics and cardiovascular outcomes of PCI of unprotected LMCA. Material and methods: Observational study which included patients with unprotected LMCA disease undergoing PCI; patients with cardiogenic shock prior to the procedure were excluded. We describe the clinical and angiographic characteristics, as well as the major adverse cardiac and cerebrovascular events (MACCE) according to the year of the procedure. Results: We included 73 patients, with a SYNTAX I score of 31.2 ± 9.1, mostly with ST-elevation acute coronary syndrome (35%). There was a higher frequency of triple vessel coronary disease (63%) and distal LMCA lesions (35%). The provisional stent technique was the most used for distal lesions (58%) and the 2-stent technique for bifurcation lesions (78%), supported by intravascular ultrasound (IVUS) in 38%. During follow-up, 19 presented MACCE (26%), out of which cardiac death occurred in 13%, non-cardiovascular death in 5%, non-fatal acute myocardial infarction in 1%, cerebrovascular event in 2%, and revascularization of the treated vessel in 4%. Conclusions: It was observed a similar frequency to the one appearing in other studies of cardiovascular events, mainly in patients with intermediate risk, which supports the increasing use of percutaneous intervention in this population.


Introducción: el debate sobre la intervención coronaria percutánea (ICP) del tronco coronario izquierdo (TCI) no protegido ha sido constante a lo largo del tiempo. Objetivo: investigar las características clínicas, de procedimiento y los desenlaces cardiovasculares de la ICP del TCI no protegido. Material y métodos: estudio observacional que incluyó pacientes con enfermedad del TCI no protegido sometidos a ICP; se excluyeron pacientes con choque cardiogénico previo al procedimiento. Describimos las características clínicas y angiográficas, así como los eventos adversos cardiovasculares y cerebrales mayores (MACCE) según el año del procedimiento. Resultados: incluimos 73 pacientes, con puntuación de SYNTAX I de 31.2 ± 9.1, mayormente con síndrome coronario agudo con elevación del ST (35%). Hubo mayor frecuencia de enfermedad coronaria trivascular (63%) y lesión distal del TCI (35%). La técnica de stent provisional fue la más usada para lesiones distales (58%) y la técnica de 2 stents para las lesiones en bifurcación (78%), con apoyo del ultrasonido intravascular (IVUS) en el 38%. En el seguimiento se presentaron 19 MACCE (26%), de los cuales la muerte de causa cardiaca se presentó en el 13%, muerte no cardiovascular en 5%, infarto agudo al miocardio no fatal en 1%, evento vascular cerebral en 2% y nueva revascularización del vaso tratado en 4%. Conclusiones: se observó una frecuencia similar a la de otros estudios de eventos cardiovasculares, especialmente en pacientes con riesgo intermedio, lo cual apoya el uso creciente de la intervención percutánea en esta población.

Resumen: Introducción: el compromiso del tronco principal izquierdo no protegido (TPInp) en pacientes con enfermedad arterial coronaria (EAC) conlleva alto riesgo de eventos cardiovasculares. La cirugía de revascularización coronaria (CABG) ha sido el estándar de tratamiento; sin embargo, estudios recientes proponen la intervención coronaria percutánea (PCI) como alternativa. Objetivo: evaluar los desenlaces intrahospitalarios y durante el seguimiento del tratamiento de pacientes con compromiso del TPInp. Método: estudio observacional multicéntrico de cohorte retrospectivo, se evaluaron pacientes con EAC y afección del TPI. Muerte de cualquier causa, infarto de miocardio no fatal, accidente cerebrovascular, reintervención y/o readmisión hospitalaria fueron valorados como un desenlace compuesto e individualmente durante la hospitalizacioì n y el tiempo de seguimiento. Resultados: se incluyeron 82 pacientes, con edad promedio 71 ± 9.9 años; 35 recibieron PCI, 31 CABG y 16 manejo meìdico (MM). La mortalidad intrahospitalaria fue del 16%. En el seguimiento medio de 12.3 meses la tasa de mortalidad fue del 20%, la de reinfarto del 6% y la de readmisioìn hospitalaria del 4%. La tasa de incidencia de muerte para MM fue 5.19 por 10.000 personas/diìa, para PCI de 2.3 por 10.000 personas/diìa y para CABG de 1.06 por 10.000 personas/diìa; en el seguimiento la mortalidad fue mayor en el grupo de PCI (HR: 3.6; IC 95% 1.13-11.9; p = 0.02). La frecuencia cardiaca elevada al ingreso se asocioì con mayor mortalidad (0.05). Conclusión: el compromiso del TPI se asocia con alto riesgo de muerte y se presenta con anatomiìa coronaria compleja.


Abstract: Introduction: involvement of the unprotected left main coronary artery (TPInp) in patients with coronary artery disease (CAD) leads to a high risk of cardiovascular events. Coronary artery bypass grafting (CABG) has been the standard of management; however, recent studies propose percutaneous coronary intervention (PCI) as an alternative treatment. Objective: to evaluate the in-hospital and follow-up outcomes of patients with compromised TPInp. Method: retrospective multicenter observational cohort study, we evaluated patients with CAD and TPI involvement. Death from any cause, nonfatal myocardial infarction, stroke, reoperation, and/or hospital readmission were assessed as a composite endpoint and individually during hospitaliza- tion and follow-up time. Results: 82 patients were included, mean age 71 ± 9.9 years; 35 received PCI, 31 CABG, and 16 medical management (MM). In-hospital mortality was 16%. In the mean follow-up of 12.3 months, mortality was 20%, rein- farction 6% and hospital readmission 4%. The incidence rate of death was 5.19 per 10,000 people/day for MM, 2.3 per 10,000 people/day for PCI, and 1.06 per 10,000 people/day for CABG; at follow-up, mortality was higher in the PCI group (HR 3.6; 95%CI 1.13-11.9; p = 0.02). Elevated heart rate on admission was associated with higher mortality (p < 0.05). Conclusion: TPInp involvement is associated with a high risk of death and presents with complex coronary anatomy.

BACKGROUND: The efficacy and safety of percutaneous coronary interventions (PCI) relative to coronary artery bypass grafting (CABG) in patients with diabetes and unprotected left main coronary artery disease (LMCAD) are not well established. OBJECTIVES: To perform a meta-analysis evaluating the long-term outcomes after PCI with drug-eluting stents (DES), as compared with CABG, in patients with diabetes and unprotected LMCAD. METHODS: MEDLINE, Cochrane, and Embase were searched for randomized controlled trials (RCTs) that reported outcomes after PCI with DES versus CABG in unprotected LMCAD among patients with diabetes. To evaluate the long-term effects of these interventions, we restricted this analysis to studies with a minimum follow-up period of 3 years. Risk ratios (RRs) with 95% confidence intervals (CIs) were pooled with a random-effects model. Quality assessment and risk of bias were performed according to Cochrane recommendations. RESULTS: Four RCTs with a total of 1080 patients were included, 553 (51.2%) of whom underwent PCI. There was no difference for individual outcomes of all-cause mortality (RR: 1.21; 95% CI: 0.86-1.71; p = .27; I2 = 28%), cardiovascular death (RR 1.29; 95% CI: 0.76-2.18; p = .34; I2 = 0%), or myocardial infarction (MI) (RR: 0.94; 95% CI: 0.61-1.45; p = .79; I2 = 0%). However, the risk of stroke was reduced with PCI relative to CABG (RR: 0.41; 95% CI: 0.18-0.94; p = .04; I2 = 0%), whereas the risk of any repeat revascularization was higher in the PCI group (RR: 1.99; 95% CI: 1.44-2.75; p < .001; I2 = 0%). The risk of the composite outcome of all-cause mortality, MI, stroke, or repeat revascularization was higher after PCI compared with CABG (RR: 1.30; 95% CI: 1.09-1.56; p = .004; I2 = 0%). CONCLUSION: In this meta-analysis with more than 1000 patients with diabetes and unprotected LMCAD followed for a minimum of 3 years, the incidence of repeat revascularization was higher among those treated with PCI, whereas the risk of stroke was higher in patients treated with CABG.

RESUMEN Introducción: La cirugía de revascularización miocárdica (CRM) ha modificado la evolución natural de los pacientes con enfermedad de tronco de la arteria coronaria izquierda (TCI). En nuestro medio es escasa la información relacionada con el seguimiento a mediano y largo plazo de los pacientes intervenidos. Objetivo: Evaluar la implicancia de la enfermedad del TCI en la evolución alejada de los pacientes intervenidos con CRM, y conocer la mortalidad e incidencia de infarto de miocardio (IAM) y/o accidente cerebrovascular (ACV). Resultados: El seguimiento se completó en 438 pacientes (95,6%) con una mediana de 58 meses [Rango intercuartilo (RIC) 35-88 meses]. La sobrevida actuarial fue a 10 años del 91,8% para toda la población, sin diferencias significativas entre el grupo TCI (91,57%) vs. el grupo no TCI (91,86%), HR 1,008, IC95% 0,38-2,65, p=0,98. En el análisis multivariado se encontraron como predictores de mortalidad alejada la fracción de eyección ventricular izquierda preoperatoria (HR 0,95, IC 95% 0,93-0,97, p<0,001), la edad (HR 1,1, IC 95% 1,04-1,13, p< 0,001) y la prioridad no electiva de la cirugía (HR = 3,71; IC 95%: 1,3-10,35; p = 0,01). La sobrevida libre de IAM fue del 96,8% (TCI 94% vs. no TCI 97,4%, p= 0,8) y la libertad de ACV fue del 98% (TCI 97,8% vs. no TCI 98,1%, p= 0,8). Conclusión: En los pacientes sometidos a CRM, la presencia de enfermedad del TCI no incrementó la tasa de eventos duros (muerte, IAM y ACV) en el seguimiento alejado. Los resultados obtenidos en esta serie de pacientes son similares a los publicados en la bibliografía internacional utilizada para desarrollar las guías de revascularización miocárdica.


ABSTRACT Background: Coronary artery bypass grafting (CABG) has modified the natural evolution of patients with left main coronary artery (LMCA) disease. There is little information in our setting regarding the mid- and long-term follow-up of operated patients. Objective: The aim of this study was to evaluate the implication of LMCA disease in the long-term evolution of patients operated on with CABG, and to assess the mortality and incidence of myocardial infarction (AMI) and/or stroke. Results: Follow-up was completed in 438 patients (95.6%) with a median of 58 months [interquartile range (IQR) 35-88 months]. Actuarial survival at 10 years was 91.8% for the entire population, with no significant differences between the LMCA group (91.57%) vs. the non-LMCA group (91.86%), HR 1,008 95% CI 0.38-2.65, p=0.98. In multivariate analysis, preoperative left ventricular ejection fraction (HR = 0.95; 95% CI 0.93-0.97; p < 0.001), age (HR 1.1, 95% CI 1.04-1.13, p<0.001) and non-elective priority of surgery (HR=3.71; 95% CI 1.3-10.35; p=0.01) were independent predictors of long-term mortality. AMI-free survival was 96.8% (LMCA 94% vs. non-LMCA 97.4%, p=0.8) and freedom from stroke was 98% (LMCA 97.8% vs. non-LMCA 98.1 %, p=0.8). Conclusion: In patients undergoing CABG, the presence of LMCA disease did not increase the rate of hard events (death, AMI, and stroke) at the long-term follow-up. The results obtained in this series of patients are similar to those published in the international literature used to develop myocardial revascularization guidelines.

RESUMEN Introducción: La angioplastia de Tronco de la Coronaria Izquierda es una opción terapéutica en pacientes con anatomía favorable. Objetivos: Reportar nuestra experiencia en el tratamiento de estos pacientes, así como su evolución clínica y evaluar los predictores de eventos clínicos durante el seguimiento. Material y Métodos: Análisis retrospectivo de pacientes intervenidos entre 2011 y 2017 por obstrucción significativa del Tronco de la Coronaria Izquierda (Clínica Bazterrica y Clínica Santa Isabel). Se registró la incidencia de eventos clínicos al año y alejados (3 años como mínimo). Se realizó un análisis univariado y multivariado (modelo de riesgos proporcionales de Cox) para identificar aquellas variables asociadas a la ocurrencia de eventos (muerte e infarto). Resultados: Se incluyeron consecutivamente 95 pacientes, de los cuales en 39 el procedimiento fue no electivo (41,1%). La mortalidad global fue 9,3 % a los 12 meses y 13,6% alejada. La tasa de infarto de miocardio no fatal fue 7,2% al año y 14,5% alejada. La tasa de accidente cerebrovascular fue 2,1% y la de revascularización fue 10,4% a los 12 meses. El único predictor multivariado de eventos a 12 meses fue la indicación no electiva del procedimiento (p = 0,003). Cuando consideramos los eventos ocurridos luego del primer año, el único predictor multivariado fue la edad >70 años (p <0,0001). Conclusiones: Nuestros resultados de la angioplastia del Tronco de la Coronaria Izquierda corresponden a los reportados por otros autores. La ocurrencia de eventos al año y eventos alejados se relacionó con condiciones identificables como la indicación del procedimiento y la edad de los pacientes.


ABSTRACT Background: Left main percutaneous coronary intervention is a therapeutic option for patients with favorable anatomy. Objectives: The aim of this study was to report our experience in the treatment of these patients, their clinical evolution and the variables associated with clinical events during follow-up. Methods: This was a retrospective analysis of patients that underwent percutaneous coronary artery intervention for left main obstructive disease between 2011 and 2017 at Clínica Bazterrica and Clínica Santa Isabel. The incidence of clinical events was evaluated at one year and at long-term follow-up (at least 3 years after the intervention). An univariate and multivariate analysis (Cox proportional risk model) was performed to identify event-associated variables (death and infarction). Results: Among a total of 95 patients included in the study, 39 (41.1%) underwent a non-elective procedure. The rate of overall mortality was 9.3% at 12 months and 13.6% at long-term follow-up, and that of non-fatal myocardial infarction 7.2% and 14.5%, respectively. On the other hand, the rate of stroke and revascularization was 2.1% and 10.4% at 12 months, respectively. On multivariate analysis, the only independent predictors of adverse clinical events were non-elective intervention at 12 months (p = 0.003) and age >70 years after the first year (p <0.0001). Conclusions: Our results of left main percutaneous coronary intervention correspond with those reported by other authors. The incidence of one-year and long-term clinical events was associated with identifiable conditions, as procedure indication and age of the patients at the time of intervention

ST-segment elevation in aVR has traditionally been used for electrocardiographic identification of left main coronary artery (LM) myocardial infarction. We present two ST-segment elevation myocardial infarction (STEMI) cases with acute total occlusion of the LM without aVR ST-segment elevation. This report reviews the different electrocardiographic discriminators suggestive of unprotected LM STEMI. (Level of Difficulty: Intermediate.).

Coronary artery disease (CAD) is amongst the leading causes of death in human immunodeficiency virus (HIV)-infected persons. Severe left main disease (LMD) occurs in approximately five percent of HIV-infected patients, with chronic total occlusion (CTO) of this vessel being an even rarer phenomenon. We describe a non-adherent HIV-infected patient with a left main coronary artery (LMCA) CTO that presented with heart failure with mildly reduced ejection fraction (HFrEF) and ventricular tachycardia (VT).

Abstract Congenital pulmonary stenosis (PS) can be associated with pulmonary artery (PA) dilatation. In some cases, this can cause compression of nearby structures including the left main coronary artery (LMCA). This compression causes angina and is considered an indication for surgical treatment. We present the case of a patient with PS and angina secondary to LMCA compression by the right PA and review the main indications and options for surgical treatment.


Resumen La estenosis pulmonar congénita se asocia a dilatación de la arteria pulmonar. En algunos casos esto puede causar compresión de las estructuras adyacentes incluyendo el tronco de la coronaria izquierda. Esta compresión causa angina y es considerada una indicación para tratamiento quirúrgico. Presentamos el caso de un paciente con estenosis pulmonar y angina secundaria a compresión del tronco de la coronaria izquierda por la arteria pulmonar derecha y revisamos las indicaciones y opciones de tratamiento quirúrgico.

Presentamos el caso de un paciente con estenosis pulmonar y angina secundaria a compresión del tronco de la coronaria izquierda por la arteria pulmonar derecha y revisamos las indicaciones y opciones de tratamiento quirúrgico.

Resumen Antecedentes: El mejor método para el tratamiento de la revascularización de la lesión del tronco no protegido de la coronaria izquierda es un tema de creciente interés. Método: Se realizaron un total de 2,439 intervenciones coronarias percutáneas (ICP) durante 3 años; se incluyeron los pacientes con lesión del tronco no protegido de la coronaria izquierda (TCI; n = 48) y se compararon con pacientes a los que se realizó cirugía de revascularización coronaria (CRVC; n = 50). Se analizaron los eventos cardiovasculares en fase hospitalaria y extrahospitalaria. El promedio de seguimiento fue de 16 meses. Resultados: El riesgo cardiovascular fue mayor en pacientes con ICP; log EuroSCORE (16 ± 21 vs. 5 ± 6, p = 0.001), Syntax clínico (77 ± 74 vs. 53 ± 39, p = 0.04). Los pacientes del grupo de ICP presentaron al ingreso con más frecuencia infarto con elevación del ST (IMCEST) y con choque cardiogénico. Los eventos hospitalarios fueron similares en ICP y CRVC (14% vs. 18%, p = 0.64). El IMCEST fue menos frecuente en el grupo de ICP (0% vs. 10%, p = 0.03. Los eventos cardiovasculares fueron menores en el grupo de ICP (2.3% vs. 18%, p = 0.01) y hubo una disminución de la muerte general y cardiaca (2.3% vs. 12%, p = 0.08 y 2.3% vs. 8%, p = 0.24), al excluir los pacientes con choque cardiogénico como presentación. En la fase extrahospitalaria los eventos fueron similares en ICP y CRVC (15% vs. 12%, p = 0.46). La supervivencia sin eventos cardiovasculares, muerte general y cardiaca fueron comparables entre los grupos (log rank, p = 0.38, p = 0.44 y p = 0.16). Conclusión: Pese a que los pacientes de ICP mostraron un perfil de riesgo mayor que los de CRVC, la seguridad y eficacia intrahospitalaria y extrahospitalaria fueron similares. © 2016 Instituto Nacional de Cardiología Ignacio Chávez. Abstract Background: The best revascularisation method of the unprotected left main artery is a current and evolving topic. Methods: A total of 2439 percutaneous coronary interventions (PCI) were registered during a 3-year period. The study included all the patients with PCI of the unprotected left main coronary (n = 48) and matched with patients who underwent coronary artery bypass graft (CABG) (n = 50). Major adverse cerebral and cardiac events (MACCE) were assessed within the hospital and in outpatients during a 16 month follow up. Results: The cardiovascular risk was greater in the PCI group; logEuroSCORE 16 ± 21 vs. 5 ± 6, P = .001; clinical Syntax 77 ± 74 vs 53 ± 39, P = .04. On admission, the PCI group of patients had a higher frequency of ST segment elevation myocardial infarction (STEMI) and cardiogenic shock. The MACCE were similar in both groups (14% vs. 18%, P = .64). STEMI was less frequent in the PCI group (0% vs. 10%, P = .03). Cardiovascular events were lower in the PCI group (2.3% vs. 18%, P = .01), and there was a decrease in general and cardiac mortality (2.3% vs. 12%, P = .08 y 2.3% vs. 8%, P = .24), on excluding the patients with cardiogenic shock as a presentation. MACCE were similar in both groups in the out-patient phase (15% vs. 12%, P = .46). Survival without MACCE, general and cardiac death were comparable between groups (log rank, P = .38, P = .44 and P = .16, respectively). BACKGROUND: The best revascularisation method of the unprotected left main artery is a current and evolving topic. METHODS: A total of 2439 percutaneous coronary interventions (PCI) were registered during a 3-year period. The study included all the patients with PCI of the unprotected left main coronary (n=48) and matched with patients who underwent coronary artery bypass graft (CABG) (n=50). Major adverse cerebral and cardiac events (MACCE) were assessed within the hospital and in outpatients during a 16 month follow up. RESULTS: The cardiovascular risk was greater in the PCI group; logEuroSCORE 16±21 vs. 5±6, P=.001; clinical Syntax 77±74 vs 53±39, P=.04. On admission, the PCI group of patients had a higher frequency of ST segment elevation myocardial infarction (STEMI) and cardiogenic shock. The MACCE were similar in both groups (14% vs. 18%, P=.64). STEMI was less frequent in the PCI group (0% vs. 10%, P=.03). Cardiovascular events were lower in the PCI group (2.3% vs. 18%, P=.01), and there was a decrease in general and cardiac mortality (2.3% vs. 12%, P=.08 y 2.3% vs. 8%, P=.24), on excluding the patients with cardiogenic shock as a presentation. MACCE were similar in both groups in the out-patient phase (15% vs. 12%, P=.46). Survival without MACCE, general and cardiac death were comparable between groups (log rank, P=.38, P=.44 and P=.16, respectively). CONCLUSION: Even though the clinical and peri-procedural risk profile of the PCI patients were higher, the in-hospital and out-hospital efficacy and safety were comparable with CABG.

BACKGROUND: Currently available randomized data on the comparison between percutaneous coronary intervention (PCI) and coronary artery bypass graft (CABG) for the treatment of unprotected left main coronary disease (LMD) lacks statistical power due to low numbers of patients enrolled. OBJECTIVES: This study assessed long-term outcomes of PCI and CABG for the treatment of LMD in specific subgroups according to disease anatomic complexity. METHODS: We conducted a pooled analysis of individual patient-level data of the LMD patients included in the PRECOMBAT (Bypass Surgery Versus Angioplasty Using Sirolimus-Eluting Stent in Patients With Left Main Coronary Artery Disease) and SYNTAX (Synergy Between PCI With TAXUS and Cardiac Surgery) trials. Incidences of major adverse cardiac events were assessed at 5 years follow-up. RESULTS: Study population comprised 1,305 patients. The incidence of major adverse cardiac and cerebrovascular events at 5 years was 28.3% in the PCI group and 23.0% in the CABG group (hazard ratio [HR]: 1.23; 95% confidence interval [CI]: 1.01 to 1.55; p = 0.045). This difference is mainly driven by a higher rate of repeat revascularization associated with PCI (HR: 1.85; 95% CI: 1.38 to 2.47; p < 0.001). The 2 strategies showed similar rates of the safety composite endpoint of death, myocardial infarction, or stroke (p = 0.45). In patients with isolated LM or LM + 1-vessel disease, PCI was associated with a 60% reduction in all-cause mortality (HR: 0.40; 95% CI: 0.20 to 0.83; p = 0.029) and 67% reduction in cardiac mortality (HR: 0.33; 95% CI: 0.12 to 0.88; p = 0.025) when compared with CABG. CONCLUSIONS: In patients with unprotected LMD, CABG, and PCI result in similar rates of the safety composite endpoint of death, myocardial infarction, or stroke. In patients with isolated LM or LM + 1-vessel disease, PCI is associated with lower all-cause and cardiac mortality when compared to CABG.

INTRODUCTION: The left main coronary artery aneurysm is rare, with an incidence of 0.1%, being the atherosclerosis its main etiology. Angiography is the gold standard for diagnosis and treatment. Depending on the severity of coexisting coronary stenosis, patients with left main coronary artery aneurysms can be effectively managed either surgical or pharmacologically. CLINICAL CASE: We present a case of a 44 year-old male with a history of obesity, smoking and dyslipidemia, complaining of oppressive chest pain, dyspnea and diaphoresis. An electrocardiogram showed an ST-segment elevation on the anterior and lateral wall and positive enzymatic curve for infarction. He was initially treated with streptokinase with no reperfusion evidence after 3 hours of the onset of symptoms, so he underwent to rescue angioplasty. Angiography reported left main coronary artery aneurysm thrombosis. Afterwards, he presented cardiogenic shock and was revascularized with a coronary artery bypass graft of the mammary artery to the left anterior descending artery and the saphenous vein to the obtuse marginal, however he did not survive. Determination for 4G/5G PAI-1 polymorphism, glycoprotein IIIa PLA1/A2 gene and Glu298Asp polymorphism of the endothelial nitric oxide synthase gene was performed. CONCLUSIONS: Left main coronary artery aneurysms are rare, finding ONE in an acute myocardial infarction is a serious situation because of the challenging reperfusion techniques that are implied, such as in this case. The search for genetic factors related with hypofibrinolysis could guide stratification and therapy towards medical surgical or interventional management.

La angina de pecho es un síntoma frecuente en pacientes con hipertensión pulmonar (HP) de cualquier etiología. Aunque su fisiopatología no está aclarada, las causas propuestas son: la isquemia subendocárdica por aumento del estrés parietal del ventrículo derecho, la dilatación de la arteria pulmonar por incrementos transitorios de la presión pulmonar y la compresión extrínseca del tronco de la arteria coronaria izquierda (TCI) por la arteria pulmonar (AP) dilatada. Se presentan tres casos que muestran la relación entre la angina de pecho y la compresión del TCI en pacientes con HP asociada a cardiopatías congénitas, tratados mediante implante de stent coronario.


Chest pain is a frequent symptom in patients with pulmonary hypertension of any etiology. Its pathophysiology has not been clearly established, the proposed causes are ischemia due to increased right ventricle wall stress, transient increased pulmonary hypertension resulting in acute pulmonary artery dilatation and external compression of the left main coronary artery (LMCA) by a dilated pulmonary artery. We report and discuss here three cases where the association between chest pain and compression of the LMCA by a dilated pulmonary artery could be shown, and they were treated with coronary stenting.

BACKGROUND: A bicuspid aortic valve (BAV) is a common congenital heart disease, which affects 1-2% of the population. However, the relationship between BAVs and aortic dilation has not been sufficiently elucidated. METHODS: A total of 241 BAV patients who were referred to this hospital for cardiac surgery over a 4.75-year period were included in this study. In addition to the clinical characteristics of the included patients, the morphological features of the aortic valve and aorta, the length of the left main coronary artery, and the laboratory findings (the coagulation and hematological parameters as well as the total cholesterol concentration) were determined and compared with those of the tricuspid aortic valve (TAV) patients. RESULTS: The BAV patients were younger than the TAV patients for a valve surgery in the last 3 months of the study period. The BAV patients were predominantly male. Most of the BAVs that were surgically treated were stenotic, regurgitant, or combined, and only 19 (7.88%) were normally functioning valves. According to echocardiography or operative records, 148 (78.31%) were type A, 31 (16.40%) were type B, and 10 (5.29%) were type C. The left main coronary artery was much shorter in the BAV patients than it was in the TAV patients. There was no significant difference between BAV and TAV patients in the total cholesterol concentrations; whereas differences were noted between patients receiving lipid-lowering therapy and those not receiving lipid-lowering therapy. The dimensions of the aortic root, sinotubular junction, and ascending aorta were beyond normal limits, while they were significantly smaller in the BAV patients than in the TAV patients. They were also much smaller in patients receiving statin therapy than those not receiving statin therapy in both groups. Moreover, the aortic dilation in the BAV group was found to be significantly associated with patient age. CONCLUSIONS: The BAV patients developed aortic wall and aortic valve disorders at a younger age than the TAV patients and were predominantly male. Aortic dilation was observed in the aortic root, sinotubular junction, and ascending aortic segments in both the BAV and TAV patients undergoing surgical aortic valve replacement, although the BAV patients had a smaller degree of dilation than the TAV patients, and dilation was also significantly age-related in this group. The shorter left main coronary artery that the BAV patients possess may contribute to the progressive course of aortic dilation that these patients experience. Statin therapy did not affect the aortic annulus in either group, but did decrease the dimensions of the aortic root, sinotubular junction and ascending aorta. In general, statin therapy had a better effect on the aortas of the TAV patients than it did on those of the BAV patients.

Antecedentes La cirugía de revascularización miocárdica se considera hasta el momento el mejor tratamiento para los pacientes portadores de enfermedad del tronco de la coronaria izquierda (TCI); pese a ello existen pocos datos locales que comuniquen los resultados de esta cirugía. Objetivos Describir los resultados inmediatos y a los 5 años de una serie de cirugías coronarias en pacientes con lesiones del TCI. Material y métodos En el período 2003-2007 se estudiaron 174 cirugías coronarias del TCI realizadas en forma consecutiva en tres servicios quirúrgicos asociados. Se analizaron los resultados quirúrgicos a los 30 días y en el seguimiento hasta 5 años, en los que se tuvieron en cuenta la mortalidad de causa cardiovascular y por otras causas, los eventos cardiovasculares mayores, la necesidad de nueva revascularización y la supervivencia libre de síntomas. Las curvas se compararon con la supervivencia esperada para el mismo grupo de edad y sexo. Resultados El 90,8% de los pacientes tuvieron lesiones múltiples. La mortalidad a los 30 días fue del 4,0%, similar a la esperada según el EuroSCORE (6,4%, OR = 0,62, IC 95% 0,21-1,78; p = 0,333), y los eventos cardiovasculares mayores incluyeron 2,9% de infarto y 1,7% de accidente cerebrovascular (ACV). El seguimiento fue de 506 pacientes-años (12 a 60 meses) e involucró al 91% de los pacientes. Al considerar la mortalidad cardiovascular, la supervivencia a los 5 años fue del 83,7%, similar a la esperada en la población general. La supervivencia en el mismo plazo descendió al 77,6% cuando se computaron todas las causas de muerte (p = 0,436). La supervivencia a los 5 años libre de angina y de nueva revascularización fue del 72,2% y del 88,1%, respectivamente. Conclusiones En este estudio observacional de pacientes operados con lesión del TCI se demostró una mortalidad quirúrgica algo inferior a la calculada con el EuroSCORE y una tasa de ACV e infarto de entre el 1,7% y el 2,9%. El seguimiento a 5 años mostró una supervivencia similar a la esperada en la población general para la misma edad y tasas de supervivencia libre de angina y de nueva revascularización de entre el 72% y el 88%. Por último, el 90% de los TCI se asociaron con lesiones coronarias múltiples, poco proclives al tratamiento con angioplastia. Los resultados analizados en este estudio fueron consistentes con los hallados en la bibliografía internacional.


Background Coronary artery bypass graft surgery still remains as the treatment of choice of patients with left main coronary artery (LMCA) disease; however, there are few previous studies with local data on the outcomes of this type of surgery. Objectives To describe immediate and 5-year outcomes of a series of coronary surgeries in patients with LMCA disease. Material and Methods From 2003 to 2007, 174 consecutive coronary artery bypass graft surgeries to the LMCA were analyzed. The procedures had been performed by three Associated Surgical Centers. The following outcomes were assessed 30 days and 5 years after surgery: cardiovascular mortality, all-cause mortality, major cardiovascular events, need of a new revascularization and survival free of symptoms. Curves were compared with expected survival rates for the same age group and sex. Results Multiple lesions were present in 90.8% of patients. Mortality rate was 4.0% at 30 days, similar to the rate expected according to EuroSCORE (6.4%, OR = 0.62, I95% CI 0.21-1.78; p=0.333); major cardiovascular events included myocardial infarction (2.9%) and stroke (1.7%). Total follow-up was 506 patient-years (12 to 60 months) and involved 91% of patients. When cardiovascular mortality was considered, survival rate at 5 years was 83.7%, similar to the one expected in the general population. However, when all causemortality was considered, survival at 5 years decreased to 77.6% (p=0.436). During the same period, survival free of symptoms and of new revascularization was 72.2% and 88.1%, respectively. Conclusions In our series of patients subjected to coronary artery bypass graft surgery due to LMCA disease, operative mortality was somewhat lower than expected according to EuroSCORE, with rates of stroke and myocardial infarction between 1.7% and 2.9%. At 5-year follow-up, survival was similar to that expected in the general population for the same age group, and survival free of angina and revascularization was 72% and 88%. Finally, 90% of LMCA lesions were associated with multiple coronary obstructions, and these conditions are unlikely to be treated with angioplasty. The results analyzed in this study were consistent with the findings previously published in international bibliography.

Introducción El tratamiento de elección de la enfermedad del tronco de la coronaria izquierda (TCI) es la cirugía de revascularización miocárdica (CRM). Un número creciente de pacientes presenta comorbilidades y/o inestabilidad clínica que condicionan un alto riesgo quirúrgico. Objetivo Evaluar los resultados de la angioplastia (ATC) del TCI no protegido en pacientes con alto riesgo para CRM (EUROSCORE ≥ 6). Material y métodos De 59 pacientes con ATC de TCI no protegido se excluyeron 8 con infarto agudo de miocardio (IAM) en shock cardiogénico y 12 sin características de alto riesgo; de los restantes pacientes de alto riesgo fueron objeto de este estudio los 32 tratados con stents convencionales. Se comparó la mortalidad hospitalaria predicha por EUROSCORE logístico con la observada, así como la incidencia de complicaciones mayores y su evolución alejada. Resultados La mediana de edad fue de 76,5 años, el 41% tenía 80 años o más, el 22% eran mujeres, el 28% diabéticos, el 56% tenía disfunción ventricular moderada a grave, el 31% insuficiencia renal crónica, el 50% vasculopatía periférica, el 53% angina refractaria, el 22% IAM reciente, el 28% procedimientos de emergencia y la mediana de EUROSCORE fue de 10,5 puntos. El 41% de los pacientes presentaban compromiso del TCI distal. El éxito angiográfico fue del 94%. Se utilizaron inhibidores IIb/IIIa en el 47%, cutting balloon en el 28%, Rotablator® en el 3% y balón de contrapulsación en el 31%. En todos se implantó un stent y en el 50% se trataron otras obstrucciones. La mortalidad hospitalaria fue del 3,1% (intervalo de confianza del 95% 0,2%-14,5%, p = 0,003), en tanto que la predicha era del 23,8%. Ningún paciente presentó déficit neurológico, IAM transmural ni requirió diálisis. Un paciente debió ser sometido a CRM electiva por fracaso del procedimiento. La mediana de seguimiento fue de 15,5 meses, período en el que se registraron 6 muertes (2 cardiovasculares) y 4 reintervenciones. La incidencia acumulada de reintervención fue del 15,6% (IC 95% 6,0%-31,3%). Conclusiones La ATC del TCI no protegido en pacientes con alto riesgo quirúrgico debería considerarse una alternativa a la CRM.


Coronary artery by-pass graft surgery (CABG) is the treatment of choice for lesions of the left main coronary artery. Nevertheless, there are a great number of patients with high risk at surgery due to concomitant diseases and/or clinical instability. Objectives To assess the outcomes of percutaneous coronary intervention (PCI) in the unprotected left main coronary artery (LMCA) in high-risk patients for CABG (EUROSCORE ³ 6). Material and methods Fifty nine patients who had undergone PCI in the unprotected LMCA were assessed; 8 patients in cardiogenic shock secondary to acute myocardial infarction (AMI) and 12 patients not considered to have high risk at surgery were excluded. This study was performed among the remaining 32 high-risk patients treated with conventional stents. In-hospital mortality predicted by logistic EUROSCORE was compared to observed mortality, and the incidence of major adverse events and late follow-up outcomes were assessed. Results Median age was 76.5 years, 43% were 80 years old or older, 22% were women, 28% were diabetic, 56% had moderate to severe left ventricular dysfunction, 31% had chronic renal failure, 50% peripheral vascular disease, 53% had refractory angina, 22% recent AMI, 28% underwent emergency procedures, and median EUROSCORE was 10.5 points. Distal compromise of LMCA was present in 41% of patients. Angiographic success was achieved in 94% of the procedures. IIb/IIIa inhibitors were used in 47% of cases, cutting balloon in 28%, Rotablator® in 3% and counterpulsation balloon in 31%. A stent was implanted in all patients and other obstructions were treated in 50% of patients. In-hospital mortality rate was 3.1% (95% confidence interval, 0.2%-14.5%, p = 0.003), compared to a predicted mortality rate of 23.8%. Neurological deficit, transmural AMI or necessity for performing dialysis were not observed. The procedure failed in one patient who subsequently underwent elective CABG surgery. Median follow-up was 15.5 months; during this period 6 deaths (2 due to cardiovascular causes) and 4 reinterventions were reported. Cumulative incidence of reintervention was 15.6% (95% CI, 6.0%-31.3%). Conclusions PCI in the unprotected LMCA in patients with high risk at surgery should be considered an alternative to CABG surgery.
