AIM: To evaluate the influence of different levels of metal artifact reduction (MAR) tool and milliamperage (mA) on the diagnosis of fracture extension in endodontically treated teeth using cone beam CT (CBCT). MATERIALS AND METHODS: Ten maxillary premolars were endodontically treated and positioned in the empty sockets of a human maxilla covered with wax. CBCT acquisitions were performed using the Eagle Edge device (Dabi Atlante, Brazil) adjusted to 120 kVp, FOV of 4 × 6 cm, exposure time of 24 s and voxel size of 0.2 mm in 8 different conditions with different MAR (1, 2 and 3) and mA (3.2 and 6.3) levels. Crown-root fractures were simulated in the universal testing machine, and CBCT images were acquired again. Five radiologists evaluated the presence and extension of fractures with a 5-point scale. Statistical analysis was performed by analysis of variance, Tukey and Kappa test (α = 0.05). RESULTS: Although different mA levels did not significantly (p > 0.05) affect the diagnosis values for fracture presence and extension, when evaluated the different levels of MAR, AUC and sensitivity showed significantly higher values (p < 0.05) for MAR 0 using 6.3 mA and kappa agreement showed significantly higher values (p < 0.05) for MAR 0 and 2 using 6.3 mA. CONCLUSIONS: Although mA levels do not have a diagnostic effect when isolating the MAR level; in 6.3 mA, MAR 0 and 2 can positively influence the diagnosis of fracture extension in endodontically treated teeth using CBCT. CLINICAL RELEVANCE: The isolate evaluation of dental fracture presence can overlook diagnostics error of its extension.

This study aimed to evaluate radiographic and sonographic features, and histogram parameters based on grayscale ultrasound of the liver and spleen in healthy toco toucans. Fifteen adult toco toucans (Ramphastos toco), seven females and eight males, weighing approximately 650 g, were enrolled in the study. On the right lateral radiographic view, the liver was visualized in the midventral region of the coelomic cavity; ultrasonographically, the liver was located in the middle portion of the coelomic cavity in close relationship to the heart, and thoracic and abdominal air sacs. Two hepatic lobes were identified; the right lobe was larger than the left one. The spleen was visualized in 10 toco toucans on radiographs and only in eight toucans on ultrasound exams. The gallbladder was identified only on ultrasound. On the right lateral radiographic view, the spleen was visualized dorsal to the proventriculus and ventral to the lungs/air sacs as an oval shape. Ultrasonographically, the spleen was observed caudal to the liver, cranial to the proventriculus and craniodorsal to the ventriculus. There was no significant difference in spleen length for either radiographic or ultrasound measurements. The brightness intensity (BI) for mean gray level (G) and standard deviation of gray levels (SG ) in the liver were 101.315 (± 16.170) and 12.453 (± 2.616), respectively. Mean G and SG levels in the spleen were 63.940 (± 18.321) and 7.494 (± 3.595), respectively. In conclusion, the sonographic features and histogram parameters indicated that the liver is more echogenic and heterogeneous than the spleen, which must be considered for diagnosing alterations in these organs.

This study aimed to evaluate the application of radiography, ultrasonography, and computed tomography (CT) as auxiliaries in the determination of body fat and musculature in crab-eating foxes (Cerdocyon thous). Eight animals were evaluated and classified based on the body condition score (BCS) scale. The total cut volume, total visceral volume, total subcutaneous volume, visceral fat volume, subcutaneous fat volume, and total fat volume were measured. The correlation of the BCS with the other data was determined. In the ultrasound, there was a correlation of the vertebral epaxial musculature score (VEMS) with body weight, BCS, muscle mass score (MMS), animal's height, and height at the withers. In the radiographic analysis, the amount of subcutaneous fat obtained in L3 vertebrae did not present a significant correlation with BCS, as opposed to L6. In the tomographic analysis, TFV/L6, VFV/L6, and SFV/L6 correlated with BCS in L3, and only SFV /L6 in L6. When the ratios between fat volumes were correlated, the correlation with BCS occurred only in L6 in the TFV/TBV, SFV/SVC, and VFW variants. Radiography and CT were effective and had a good association with BCS. With the CT it was possible to demonstrate the distribution of total fat at the level of the L3 slice. The ratio of calculations considering the length of the L6 vertebra was more adequate, while at the level of L6 the ratio between the volumes of the compartments was more effective.

ABSTRACT Purpose: To longitudinally compare isolated structural parameters obtained using RTVue optical coherence tomography in patients with glaucoma and suspected glaucoma with stable visual fields. Methods: All patients were required to have a reliable SITA Standard 24-2 Humphrey Visual Field test. Visual field stability was defined as having <5 points with p<5% and/or having no points with p<1% and/or p<0.05% in the glaucoma progression analysis comparison graph. Furthermore, the glaucoma assessment strategy was used in optical coherence tomography. Results: The study included 75 eyes from 75 patients, 43 of which had glaucoma and 32 had suspected glaucoma. The mean visual field intervals were 29.57 ± 9.65 months between the first and third tests. No visual field parameter variations (mean deviation, pattern standard deviation, and visual field index) and no retinal nerve fiber layer or optic disk parameter variations between the first and third tests were observed (p>0.05 for all), and no retinal nerve fiber layer parameter variations throughout the study were observed, except for optic disk parameters presenting with cup volume changes (p=0.004). However, ganglion complex cells presented a progressively decreased average ganglion cell complex parameter, with a variability of -0.98% ± 3.71% (p=0.04) between the first and third tests. By contrast, the global loss volume progressively increased throughout the study, with a variability of 14.71% ± 44.52% (p=0.04) between the first and third tests. The inferior ganglion cell complex parameter was significantly decreased between the first and third tests (p=0.02). Conclusion: The present findings suggest that patients with glaucoma or suspected glaucoma with stable visual fields may present structural ganglion complex cell progression as assessed using RTVue optical coherence tomography.
RESUMO Objetivo: Comparar longitudinalmente os parâmetros estruturais isolados obtidos através da tomografia de coerência óptica RTVue em pacientes glaucomatosos e suspeitos de glaucoma com campos visuais estáveis. Métodos: Todos os incluídos deveriam ter Campimetria Computadorizada Humphrey Sita Standard 24-2 confiáveis. A estabilidade campimétrica foi definida se apresentassem menos de cinco pontos com p<5% e/ou nenhum ponto com p<1% e/ou p<0,05% no gráfico de comparação do Glaucoma Progression Analysis. Para a tomografia de coerência óptica, foi utilizado a estratégia de avaliação para glaucoma. Resultados: Foram incluídos 75 olhos de 75 pacientes: 43 com glaucoma e 32 suspeitos. A média dos intervalos do campo visual entre o 1o e 3o exame, foi de 29,57 ± 9,65 meses. Não houve variação para os parâmetros do campo visual (desvio médio, desvio padrão e índice da função visual) entre o primeiro e o último exame (p>0,05 para todos). Não houve variação dos parâmetros da camada de fibras nervosas da retina ao longo do estudo, enquanto que para os parâmetros do disco óptico, apenas cup volume apresentou mudança (p=0,004). Em relação à camada de células ganglionares da retina, notou-se uma redução progressiva na espessura média da Ganglionar Complex Cells com uma variabilidade entre o primeiro e último exame de -0,98 ± 3,71% (p=0,04). Quanto ao Global loss volume, houve um aumento progressivo ao longo do estudo com uma variabilidade entre o primeiro e último exame de 14,71 ± 44,52% (p=0,04). O parâmetro inferior do Ganglionar Complex Cells também reduziu significativamente entre o 1o e 3o exames (p=0,02). Os demais parâmetros da tomografia de coerência óptica RTVue se mantiveram estáveis entre o 1o e 3o exames. Conclusão: Os presentes achados sugerem que pacientes glaucomatosos ou com suspeita de glaucoma e com campos visuais estáveis, podem apresentar progressão estrutural na camada de células ganglionares da retina avaliada por meio da tomografia de coerência óptica RTVue.

Fundamento: El carcinoma basocelular es infrecuente en la piel cabelluda. Es un tumor de invasión local y crecimiento lento, puede ser agresivo, destruir tejidos vecinos, causar ulceración e invadir en profundidad cartílago y hueso. Objetivo: Evaluar los resultados de la aplicación del HeberFERON y el seguimiento con ecografía cutánea en pacientes con carcinoma basocelular en la piel cabelluda. Metodología: Se realizó un estudio observacional, descriptivo y longitudinal en una serie de casos con diagnóstico de carcinoma basocelular de la piel cabelluda en el Policlínico Centro de la ciudad Sancti Spíritus, durante el período de 10 de julio del 2018 a 29 de julio del 2022. Se incluyeron 6 casos. Las variables estudiadas fueron la respuesta al tratamiento mediante la clínica, la ecografía cutánea e histopatología y presencia de eventos adversos. Resultados: Predominó el sexo masculino, subtipo histológico sólido, subtipo clínico nódulo ulcerativo, tamaño del tumor mayor de 30 mm y tiempo de evolución de más de 12 meses; la respuesta al tratamiento en la mayoría de los casos fue parcial. Los eventos adversos fueron dolor y ardor en el sitio de inyección, fiebre, edema y eritema perilesional. Conclusiones: El HeberFERON resultó de utilidad en los pacientes con carcinoma basocelular del cuero cabelludo ya que redujo el tumor en unos casos y en otros lo eliminó. La ecografía permitió la evaluación en tiempo real de la neoplasia; los eventos adversos más frecuentes fueron la fiebre y el dolor en el sitio de inyección, a pesar de ello ningún paciente abandonó el tratamiento.
Background: Basal cell carcinoma is uncommon in the scalp. It is a slow-growing locally invasive tumor, it can be aggressive in destroying neighboring tissues, cause ulceration and invade deep into the cartilage and bone. Objective: To evaluate the results of HeberFERON application and follow-up with cutaneous echographical in patients with scalp basal cell carcinoma. Methodology: An observational, descriptive and longitudinal study was conducted in a series of cases diagnosed with scalp basal cell carcinoma at the Center Polyclinic in Sancti Spíritus city during the period from July 10, 2018 to July 29, 2022. Six clinical cases were included. The studied variables were the answer to the treatment by clinical, cutaneous echographical and histopathology and the presence of adverse events. Results: Male sex predominated, solid histologic subtype, clinical subtype ulcerative nodule, tumor size greater than 30 mm and evolution time of over 12 months; the treatment response in most cases was partial; adverse events were pain and burning at the injection site, fever, edema and perilesional erythema. Conclusion: It was observed that in patients with scalp basal cell carcinoma, the HeberFERON treatment reduced in some cases and eliminated the tumor in others. Echography allowed real-time evaluation of the neoplasm, fever and pain at the injection site were the most frequent adverse events. In spite of this, none of the patients abandoned the therapy.

La expansión de la pandemia de COVID-19 incrementó el interés en la utilidad y pertinencia del diagnóstico por imágenes en el manejo de pacientes sospechosos o confirmados. El trabajo pretende comunicar elementos para facilitar la interpretación de la información ofrecida por la tomografía computarizada y brindar una guía básica, a partir de los hallazgos observados en 1622 pacientes, para el análisis de las imágenes tomográficas. Se realizó una revisión, análisis crítico y sistematización de la información encontrada a partir de una búsqueda en las bases de datos y motores Medline, Epistemonikos, LibreOffice Base, Microsoft Academic, ClinicalKey, Science Research y MedNar con el criterio: imagen en SARS-CoV-2 (COVID-19), en inglés y español. En COVID-19 no todo es opacidad en vidrio esmerilado. Si bien son las lesiones más observadas, se resalta, como característico, la distribución de las mismas predominantemente periféricas, multifocales, bilaterales, posteriores y con una tendencia a la asimetría; más tarde, en la evolución, seguiría el patrón en empedrado. Este y otros hallazgos menos frecuentes pueden ser encontrados en alguna fase del curso de la enfermedad. En el trabajo se propone una guía de observación para la correcta interpretación de la tomografía del paciente con COVID-19.
The spread of COVID-19 pandemics increased interest in the usefulness and pertinence of imaging diagnosis in the management of suspected or confirmed patients. This work aims to communicate elements to facilitate the interpretation of the information offered by computed tomography and provide a basic guide, based on the findings observed in 1622 patients, for the analysis of tomographic images. A review, critical analysis and systematization of the information―found from a search in Medline, Epistemonikos, LibreOffice Base, Microsoft Academic, ClinicalKey, Science Research y MedNar databases and engines―was carried out, with the criterion: image in SARS-CoV-2 (COVID-19), in English and Spanish. In COVID-19 not everything is opacity in ground glass. Although they are the most observed lesions, their distribution predominantly peripheral, multifocal, bilateral; posterior and with a tendency to asymmetry is highlighted as characteristic; later, in the evolution, they will follow the crazy paving pattern. This and other less frequent patterns may be found at some stage in the course of the disease. An observation guide for the correct interpretation of the tomographic imaging of patients with COVID-19 is proposed in the work.

Introducción: El crecimiento y el desarrollo son fenómenos biológicos que implican aumento de masa celular y diferenciación de forma o función, respectivamente. En beneficio del diagnóstico, se utilizan las bondades del ultrasonido en la evolución biométrica del crecimiento fetal. Objetivo: Determinar mensuraciones biométricas fetales más asociadas a la restricción del crecimiento fetal en infantes que sufrieron restricción del crecimiento intrauterino. Métodos: Se realizó un estudio longitudinal descriptivo y retrospectivo de gestantes captadas en dos áreas de salud del municipio Santa Clara, que terminaron su embarazo entre septiembre del 2013 y octubre del 2018 y cuyos recién nacidos experimentaron restricción del crecimiento. La muestra se clasificó al nacimiento en pequeños y adecuados, según condición trófica y en cada grupo se estudió relación con valores percentilares de variables biométricas en los dos últimos trimestres. Resultados: Predominaron en todas las biometrías y en los dos trimestres los valores por debajo del décimo percentil. En la totalidad de los infantes y durante todo el período fetal, la circunferencia abdominal estuvo por debajo del percentil 10. En las variables largo del fémur y circunferencia cefálica los percentiles más altos se encontraron en el tercer trimestre y desde el segundo en la variable diámetro biparietal, posiblemente relacionado con restricciones asimétricas del crecimiento. Conclusiones: La variable biométrica circunferencia abdominal desde el segundo trimestre es fiable en la detección de restricciones del crecimiento intrauterino y su existencia por debajo del décimo percentil debe servir como certeza de su existencia, aun cuando la condición trófica del recién nacido parezca revelar otra realidad(AU)
Introduction: Growth and development are biological phenomenons involving cell mass increase and differentiation of form or function, respectively. For diagnostic purposes, the benefits of ultrasound are used for the biometric evolution of fetal growth. Objective: To determine fetal biometric measurements mostly associated with fetal growth restriction in infants who suffered intrauterine growth restriction. Methods: A descriptive and retrospective longitudinal study was carried out with pregnant women from two health areas of Santa Clara Municipality, who finished their pregnancy between September 2013 and October 2018 and whose newborns experienced growth restriction. The sample was classified at birth into small and adequate, according to trophic condition; and, in each group, the relationship with percentile values of biometric variables in the last two trimesters was studied. Results: Values below the tenth percentile predominated in all biometrics and in the two trimesters. In all infants and during the whole fetal period, abdominal circumference was below the tenth percentile. In the variables femur length and cephalic circumference, the highest percentiles were found in the third trimester; while, from the second trimester on, the same occurred in the variable biparietal diameter, possibly related to asymmetric growth restrictions. Conclusions: The biometric variable abdominal circumference is, from the second trimester on, reliable in the detection of intrauterine growth restrictions; its existence below the tenth percentile should serve as certainty of its existence, even when the trophic condition of the newborn seems to reveal another reality(AU)

A articulação temporomandibular (ATM) é considerada uma das mais complexas articulações que compõem o sistema musculoesquelético. Aspectos anatômicos da sua localização impõe dificuldades de acesso e abordagem clínica das disfunções temporomandibulares (DTM), sendo o método tomográfico computadorizado por feixe cônico (TCFC) uma modalidade que possibilita identificação dos componentes ósseos da ATM e processos patológicos como a doença articular degenerativa (DAD). Este estudo teve como objetivo caracterizar o osso medular de cabeças de mandíbula acometidas por aplainamento, osteófito e erosão. Foram realizadas análise de textura (AT) em 80 imagens de TCFC de cabeças de mandíbula, com três regiões de interesse (ROIs) correspondentes aos sítios ósseos específicos. Os exames foram exportados para o software MaZda, onde as ROIs foram delimitados seguindo os contornos previamente demarcados e valores da matriz de coocorrência (MCO) foram calculados para os parâmetros selecionados da AT. Posteriormente os dados foram submetidos a análise estatística com o teste de Kruskal-Wallis com correção do p-valor pelo método FDR e nível de significância de 5%. O grupo erosão apresentou um comportamento significativamente diferente dos demais grupos de estudo em todos os parâmetros analisados. Pôde-se concluir que as medulares ósseas condilares de ATM acometidas por erosão apresentaram comportamentos distintos, indicando que já haja processos de alterações degenerativas instalados possibilitando assim a avaliação da condição medular em TCFC através da AT(AU)
The temporomandibular joint (TMJ) is considered one of the most complex joints that make up the musculoskeletal system. Anatomical aspects of its location impose difficulties in accessing and clinically approaching temporomandibular disorders (TMD), with the cone beam computed tomographic method (CBCT) being a modality that allows the identification of the bone components of the TMJ and pathological processes such as degenerative joint disease (DAD). This study aimed to characterize the medullary bone of mandible heads affected by flattening, and erosion. Texture analysis (TA) was performed on 80 CBCT images of condyle heads, with three regions of interest (ROIs) corresponding to specific bone sites. The exams were exported to the MaZda software, where the ROIs were delimited following previously marked contours and values of the co-occurrence matrix (MCO) were calculated for the selected TA parameters. Subsequently, the data were submitted to statistical analysis with the Kruskal-Wallis test with correction of the p-value by the FDR method and a significance level of 5%. The erosion group showed a significantly different behavior from the other study groups in all analyzed parameters. It could be concluded that the TMJ condylar bone marrow affected by showed different behaviors, indicating that there are already installed of degenerative alterations, thus enabling the assessment of the spinal cord condition in CBCT through TA.(AU)

Introduction: The role of objective parameters in terms of improvement of the accuracy of high-resolution computed tomography (HRCT) of the temporal bone in the diagnosis of otosclerosis remains unclear. Objectives: To investigate the relationship between the density of the fissula antefenestram (FAF) and of the width of the transversal section of the basal turn of the cochlea (BTC), and the diagnosis of otosclerosis. Methods: This is a retrospective study in which preoperative HRCT data from ears of patients submitted to stapedotomy due to otosclerosis (case group) were evaluated. For the control group, normal hearing ears having undergone HRCT for other purposes were included. Case and control HRCT images were objectively assessed by an experienced blinded radiologist. During this evaluation, measurements of the relative radiological density of the FAF and of the transversal section of the BTC were obtained. The results were compared between the groups. Also, a receiver operating characteristic curve was created and the area under the curve (AUC) was calculated for each variable. Significance level was set at .05. Results: 40 ears were included in each group. Case ears presented reduced values for the relative radiological density on the FAF (p-value<0.0001). Moreover, ears with otosclerosis (p-value: 0.022) presented lower transversal section of the BTC. The AUC for these variables reached 0.929 and 0.646, respectively. Conclusions: Otosclerotic ears present reduced radiological density on the FAF and narrower BTC. The relative density of the FAF also shows a great diagnostic power in the context of this disease.

RESUMEN La osteogénesis imperfecta (OI), también conocida como enfermedad de los huesos de cristal, es una enfermedad rara, causada principalmente por mutaciones en los genes COL1A1 y COL1A2. Aunque el 85-90% de los eventos son causados por mutaciones en el propio colágeno, las formas recesivas pueden ser el resultado de otros defectos. Dado que pueden ocurrir hallazgos similares entre la OI y otras enfermedades, es necesario un diagnóstico diferencial para una adopción más rápida de los tratamientos apropiados. Debido a la necesidad constante de exámenes, estos pacientes tienen más probabilidades de tener complicaciones por la radiación ionizante, por lo que es muy importante cumplir estrictamente con todos los requisitos de protección radiológica. OBJETIVO Verificar la existencia de protocolos de imagen utilizados en el diagnóstico de la OI y describir las técnicas radiográficas involucradas en el proceso. METODOLOGIA Se trata de un estudio cualitativo en el que se utilizaron las revistas PubMed, BIREME, CAPES y ScienceDirect, con el objetivo de verificar la presencia de investigaciones dirigidas a la creación de protocolos de imagen para auxiliar el diagnóstico de la OI. CONSIDERACIONES FINALES: Si bien ha demostrado ser de gran utilidad, debido a los riesgos a los que están expuestos los pacientes con OI, el uso de herramientas que liberan radiaciones ionizantes debe ser monitoreado constantemente. Por lo tanto, los protocolos deben revisarse para que, incluso con una reducción de la dosis, no se pierda la resolución y el detalle de la imagen.
ABSTRACT: The osteogenesis imperfecta (OI), also known as brittle bone disease, is a rare disease, mainly caused by mutations in genes COL1A1 and COL1A2. Although 85-90% of events are caused by mutations in collagen itself, the recessive forms may be the result of other defects. Since similar findings may occur between OI and other diseases, a differential diagnosis is required for faster adoption of appropriate treatments. Due to the constant need for tests, these patients are more likely to have complications due to ionizing radiation, so it is very important to strictly comply with all radiological protection requirements. OBJETIVO To verify the existence of imaging protocols used in the diagnosis of OI and to describe the radiographic techniques involved in the process. METHODOLOGY This is a qualitative study in which PubMed, BIREME, CAPES y ScienceDirect databases were used, with the objective of verifying the presence of research aimed at the creation of imaging protocols to assist in the diagnosis of OI. FINAL CONSIDERATIONS Although it has proved very useful, because of the risks to which patients with OI are exposed, the use of tools that release ionizing radiation should be monitored constantly. With this, the protocols should be reviewed so that even with a dose reduction, the resolution and detail of the image are not lost

Abstract We present the case of a 56-year-old black female patient from a rural area in the Morón municipality, Ciego de Ávila province, Cuba. She was admitted with symptoms of dysentery with several days of evolution and a later episode of high fever, compromised general status, and abdominal pain located in the right hypochondrium. Analytical studies reported leukocytosis with a predominance of polymorphonuclear cells, Entamoeba histolytica was found in the stool study. Abdominal ultrasound reported a mixed image of 110 x 84 mm in the upper right lobe of the liver, as confirmed by computed tomography. This image was interpreted as a possible liver abscess. The patient received antimicrobial treatment for four weeks without a good response, thus requiring surgical intervention. She evolved favorably and was discharged after 21 days.
Resumen Se presenta el caso de una paciente de raza negra de 56 años procedente de área rural de Morón, provincia Ciego de Ávila (Cuba), quien ingresa por cuadro clínico de disentería de varios días de evolución acompañado de fiebre, compromiso de su estado general y dolor abdominal en el hipocondrio derecho. Los estudios analíticos de laboratorio mostraron leucocitosis con predominio de neutrófilos y presencia de trofozoitos de Entamoeba histolytica en la materia fecal. La ecografía de abdomen reporto una imagen mixta de 110 x 84 mm en el lóbulo derecho del hígado y la tomografía confirmó la lesión que se interpretó como un posible absceso hepático. Se inició tratamiento antimicrobiano por un periodo de 4 semanas sin adecuada respuesta por lo que requirió tratamiento quirúrgico. Su evolución fue favorable con egreso a los 21 días.

Introducción: El traumatismo abdominal cerrado puede provocar lesiones orgánicas graves con hemorragias que demandan un tratamiento quirúrgico emergente y es la principal causa de muertes evitables en todos los grupos de edad. Objetivo: Evaluar las características clínico quirúrgicas de los pacientes con trauma cerrado de abdomen. Métodos: Se realizó un estudio observacional descriptivo y retrospectivo para evaluar el comportamiento del trauma cerrado de abdomen en 81 pacientes atendidos en el Hospital General Provincial Docente "Roberto Rodríguez Fernández" de Morón desde enero del 2014 hasta diciembre de 2019. Resultados: El adulto joven fue el grupo de edad predominante (34,6 por ciento) del sexo masculino (19,8 por ciento), el accidente de tránsito (48,1 por ciento) fue la causa principal. El cuadro hemorrágico (39,5 por ciento) seguidos del cuadro doloroso abdominal (38,3 por ciento) representaron los síntomas clínicos más relevantes. La positividad de los exámenes auxiliares estuvo representada por la ecografía abdominal (74,1 por ciento) seguida de la punción abdominal (9,9 por ciento. Las lesiones hepáticas (27,0 por ciento) seguidas de las esplénicas (19,0 percent) fueron las vísceras macizas más afectadas. El 77,8 por ciento fue intervenido quirúrgicamente y las técnicas quirúrgicas hepáticas fueron las más empleadas para un total de 17 casos (26,9 por ciento). El choque hipovolémico (12,7 por ciento) fue la complicación posoperatoria más encontrada. Se reportó un 12,3 por ciento de fallecidos. Conclusiones: El paciente con trauma cerrado de abdomen debe ser considerado siempre politraumatizado cuyo tratamiento inicial se dirige a la estabilización rápida e identificación de lesiones que amenacen la vida(AU)
Introduction: Blunt abdominal trauma can cause severe organ injury with hemorrhage demanding emergent surgical treatment. It is the leading cause of preventable death among all age groups. Objective: To assess the clinical-surgical characteristics of patients with blunt abdominal trauma. Methods: A descriptive and retrospective observational study was carried out to assess the characteristics of blunt abdominal trauma in 81 patients treated at Roberto Rodríguez Fernández General Provincial Teaching Hospital of Morón from January 2014 to December 2019. Results: Young adults represented the predominant age group (34.6 percent), together with the male sex (19.8 percent). Traffic accident (48.1 percent) was the main cause. Hemorrhagic symptoms (39.5 percent) was the most relevant clinical symptoms, followed by abdominal pain (38.3 percent). Positive results in complementary tests were represented by abdominal ultrasound (74.1 percent), followed by abdominal puncture (9.9 percent). Hepatic lesions (27.0 percent) was the most affected solid viscera, followed by splenic lesions (19.0 percent). 77.8 percent of cases were operated on and hepatic surgical techniques were the most frequently used, accounting for 17 cases (26.9 percent). Hypovolemic shock (12.7 percent) was the most frequent postoperative complication. A death rate of 12.3 percent was reported. Conclusions: The patient with blunt abdominal trauma should always be considered polytraumatized, in which case the initial treatment is aimed at rapid stabilization and identification of life-threatening injuries(AU)

This study used contrast radiography to evaluate gastrointestinal transit times in cockatiels (Nymphicus hollandicus) and investigated the sedative effects of intranasal midazolam in this species and its usefulness in facilitating the manual restraint required for radiographic studies. Twelve healthy adult cockatiels received intranasal midazolam at dose of 2 mg/kg, and iohexol at 15 ml/kg by crop gavage. Radiographic images were obtained before contrast administration, 3 minutes after and then each 10 minutes for 90 minutes. Sedation quality of the bird was evaluated during the radiographic study and assessed according to an adapted visual sedation scale. Three minutes after iohexol administration, the cervical oesophagus and the crop were filled in all birds. At the same time, the contrast medium reached the thoracic oesophagus, proventriculus, isthmus and ventriculus in most birds. In all cockatiels, median (range) transit times were 3 (3-10) minutes for proventriculus and ventriculus, 10 (10-40) minutes for small intestine and 45 (30-70) minutes for large intestine. The overall gastrointestinal transit time was 50 (30-90) minutes.Crop remained filled with iohexol throughout the study, while oesophagus and isthmus presented a pattern of contrast progression different from the other gastrointestinal segments. According to the visual sedation scale, cockatiels presented a moderate to intense muscular relaxation, and intranasal midazolam seems to be an appropriate sedation protocol for radiographic study. All cockatiels remained healthy after the study and presented clear and watery stools at least 12 hours after, due to gastrointestinal emptying.
O presente estudo objetivou determinar os tempos de trânsito gastrintestinal de calopsitas (Nymphicus hollandicus) por meio do estudo radiográfico contrastado e investigar os efeitos sedativos do midazolam intranasal nesta espécie, bem como a viabilidade do uso deste fármaco para facilitar a contenção manual durante o exame radiográfico. Doze calopsitas, adultas e saudáveis, receberam midazolam intranasal, na dose de 2 mg/kg, e 15 ml/kg de iohexol por gavagem. Imagens radiográficas foram obtidas antes da administração do meio de contraste, três minutos depois da administração e a cada 10 minutos, até 90 minutos de estudo. A qualidade da sedação foi avaliada durante todo o estudo radiográfico por meio de escala visual adaptada. Três minutos após a administração do iohexol, esôfago cervical e inglúvio foram preenchidos em todas as aves. Ao mesmo tempo, o meio de contraste alcançou esôfago torácico, proventrículo, istmo e ventrículo na maioria dos animais. Em todas as aves, a mediana e intervalo dos tempos de trânsito foram três (3-10) minutos em proventrículo e ventrículo, 10 (10-40) minutos em intestino delgado e 45 (30-70) minutos em intestino grosso, sendo que o tempo total de trânsito gastrintestinal foi 50 (30-90) minutos. Inglúvio permaneceu preenchido por meio de contraste durante todo o estudo radiográfico, enquanto esôfago e istmo apresentaram padrão de trânsito diferente dos demais segmentos avaliados. As aves apresentaram moderado a intenso relaxamento muscular durante o estudo e a administração de midazolam intranasal mostrou-se como protocolo sedativo apropriado em calopsitas. Todas as aves permaneceram saudáveis e apresentaram fezes com aspecto aquoso e esbranquiçado no mínimo 12 horas após o estudo radiográfico, devido ao esvaziamento gastrintestinal.

Background: Diagnostic ultrasound has revolutionized obstetric practice. Doppler techniques provided in vivo studies of the fetus-placental and uterus-placental circulations, in an increasingly precise manner. The assessment of fetal well-being is essential in obstetric practice, however, in veterinary medicine, there is a deficit of ultrasound parameters related to fetal distress. The main of this research was to determine the hemodynamic characteristics and behavior, through Doppler ultrasonography, of maternal-fetal vessels during the final third of gestation in bitches with fetuses under stress, as well as correlating them with each other and with fetal distress. Materials, Methods & Results: An observational, prospective study was carried out, in which 30 healthy bitches were examined, during the last third of pregnancy, with fetuses under fetal distress. Fetuses were evaluated in 2 stages: (1st): 50-54 days and (2nd): 55-60 days of gestation. All fetuses were calmly assessed, one by one, and fetal heart rate (FHR) was measured 5 times, for at least 3 min, to identify and confirm fetal distress. At least 3 fetuses were evaluated in each bitch and each measurement was performed in triplicate, setting a hemodynamic mean for each value evaluated. The flow pattern in the middle cerebral artery (MCA), internal carotid artery (ICA) and umbilical artery (UA) was evaluated. The analyzed vessel was initially visualized in B mode, followed by evaluation by color and spectral Doppler. The variables observed were: peak systolic velocity (PSV), end diastolic velocity (EDV) and the resistivity (RI) and pulsatility (PI) index, as well as the systole/diastole ratio (S/D). The analysis of RI and PI of the umbilical artery showed a statistically significant difference between the mean values found for the times studied, increasing from time 1 to time 2. For middle cerebral and internal carotid arteries, PI, RI and the relationship systole/diastole (S/D), showed a statistically significant difference between the mean values found for the times studied, decreasing from time 1 to time 2. Discussion: Fetal heart rate (HR) findings are considered normal above 200 beats per minute (bpm), only if preceded by acceleration and deceleration. This fact confirms the presence of fetal stress, by this parameter, for the fetuses in the present study. Corroborating the picture of fetal distress, another study concluded that HR tends to increase up to 20 days before delivery, a fact that did not occur in the study in question, where animals in the same gestational period were evaluated. Recent research has concluded that an elevated umbilical artery PI at 28 weeks of gestation, in the absence of fetal growth restriction or prematurity, is associated with some adverse cognitive findings in 12-year-olds children. In the present study, a progressive increase in this index was observed throughout the final third of pregnancy. Although the pups were not monitored after birth, this data serves as a warning for veterinarians, and can be useful in the assessment and diagnosis of possible postnatal nervous alterations. Like what was found in the present study, a study reported that from the 4th week to birth, the PSV of the umbilical artery (UA) increased almost linearly, with high significance. For the middle cerebral arteries, the PI showed a statistically significant difference between the mean values found for the times studied, however, decreasing throughout the final third of pregnancy.

ABSTRACT Objective: to discuss the social representations about peripheral venipuncture and the use of contrast media of people submitted to radiological examinations, referring to stressor concept. Method: this is qualitative research, outlined in the procedural approach of the Social Representation Theory, carried out with 57 users undergoing computed tomography and magnetic resonance imaging at a university hospital in Juiz de Fora, Minas Gerais, Brazil. In-depth individual interviews triggered by guiding questions were conducted in January 2019. The contents of the interviews were fully transcribed and content analysis was carried out in three stages (pre-analysis, material exploration and treatment/interpretation of results). We established thematic analysis based on the dimensions and representational origins, which made it possible to discuss the findings anchored in the concepts of stressors, allowing the identification of three categories based on intrapersonal, interpersonal and transpersonal stressors. Results: puncture and examination were represented by individual, group, professional and therapeutic environment experiences, classified into the following categories: itinerary and conceptions on puncture and contrasted examinations based on intrapersonal stressors; shared relationships on puncture and examination, based on interpersonal stressors; and experiences in the therapeutic environment of a diagnostic imaging service, based on transpersonal stressors. Conclusion: the social representations were signified by examination, results and impacts on life, portraying stressors based on images/feelings of doubt and positive behaviors rationally justified, which explain human responses to reified content, enabling the restructuring of health care and nursing.
RESUMEN Objetivo: discutir las representaciones sociales sobre la venopunción periférica y el uso de medios de contraste en personas sometidas a exámenes radiológicos, refiriéndose al concepto de estresores. Método: investigación cualitativa, enmarcada en el enfoque procedimental de la Teoría de las Representaciones Sociales, realizada con 57 usuarios sometidos a tomografía computarizada y resonancia magnética en un hospital universitario de Juiz de Fora, Minas Gerais, Brasil. Se realizaron entrevistas individuales en profundidad desencadenadas por preguntas orientadoras en enero de 2019. Los contenidos de las entrevistas se transcribieron íntegramente y se realizó un análisis de contenido en tres etapas (preanálisis, exploración del material y tratamiento/interpretación de los resultados). El análisis de contenido temático-categoría establecido a partir de las dimensiones y orígenes representacionales permitió discutir los hallazgos anclados en los conceptos de estresores, permitiendo la identificación de tres categorías a partir de estresores intrapersonales, interpersonales y transpersonales. Resultados: la punción y el examen fueron representados por las experiencias individuales y grupales, con los profesionales y el ambiente terapéutico, clasificados en las categorías: itinerario y concepciones sobre punción y exámenes contrastados, a partir de estresores intrapersonales; relaciones compartidas sobre punción y examen, basadas en estresores interpersonales; y experiencias en el ambiente terapéutico de un servicio de diagnóstico por imagen, basado en estresores transpersonales. Conclusión: las representaciones sociales fueron significadas por el examen, los resultados y los impactos en la vida, retratando estresores a partir de imágenes/sentimientos de duda y comportamientos positivos justificados racionalmente, que explican las respuestas humanas a los contenidos cosificados, posibilitando la reestructuración de los cuidados de salud y enfermería.

Método: pesquisa qualitativa, delineada na abordagem processual da Teoria das Representações Sociais, realizada com 57 usuários submetidos a exames de Tomografia Computadorizada e Ressonância Magnética em um Hospital Universitário de Juiz de Fora, Minas Gerais, Brasil. Realizaram-se entrevistas individuais em profundidade desencadeadas por questões norteadoras em janeiro de 2019. Os conteúdos das entrevistas foram transcritos na íntegra e realizou-se análise de conteúdo em três etapas (pré-análise, exploração do material e tratamento/interpretação dos resultados). A análise temático-categorial estabelecida a partir das dimensões e origens representacionais possibilitou a discussão dos achados ancorada nos conceitos de estressores, permitindo a identificação de três categorias baseadas nos estressores intrapessoais, interpessoais e transpessoais. Resultados: a punção e o exame foram representados pelas vivências individuais, grupais, com profissionais e ambiente terapêutico, classificados nas categorias: itinerário e concepções sobre punção e exames contrastados, com base em estressores intrapessoais; relações compartilhadas sobre a punção e o exame, fundamentadas em estressores interpessoais e vivências no ambiente terapêutico de um serviço de diagnóstico por imagem, a partir dos estressores transpessoais. Conclusão: as representações sociais foram significadas por exame, resultado e impactos na vida, retratando estressores alicerçados em imagens/sentimentos de dúvida e comportamentos positivos justificados racionalmente, que explicitam respostas humanas a conteúdos reificados, possibilitando a reestruturação do cuidado em saúde e em enfermagem.
RESUMO
The aim of this study was to evaluate the anatomical structures of the skulls of peccaries to establish the basis for their clinical study and future preclinical research. Ten skulls of adult peccaries were subjected to tomographic examination. The data obtained were processed via three-dimensional image reconstruction software (3D images). The reconstructions obtained from the neurocranium of the studied specimens allowed the identification and description of the following structures: nasal bone, frontal bone, parietal bones, incisor bone, maxillary bone, zygomatic bone, temporal bone, palatal bone, occipital bone, vomer bone, pterygoid bone, sphenoid bone, paranasal sinuses and orbit. Computed tomography proved to be an important diagnostic tool in the investigation of the skull of this species, allowing the acquisition of anatomical values not yet documented for the species in the literature.(AU)
O objetivo deste estudo foi avaliar as estruturas anatômicas dos crânios de catetos, a fim de se estabelecerem as bases para seu estudo clínico e futuras pesquisas pré-clínicas. Dez crânios de catetos adultos foram submetidos a exame tomográfico. Os dados obtidos foram introduzidos em um software de reconstrução de imagens tridimensionais (imagens em 3D). As reconstruções obtidas do neurocrânio dos espécimes estudados permitiram a identificação e a descrição das seguintes estruturas: osso nasal, osso frontal, ossos parietais, osso incisivo, osso maxilar, osso zigomático, osso temporal, osso palatino, osso occipital, osso vômer, osso pterigoide, osso esfenoide, seios paranasais e órbita. A tomografia computadorizada mostrou-se como uma ferramenta diagnóstica importante na investigação do crânio dessa espécie, permitindo a aquisição de valores anatômicos ainda não documentados para a espécie na literatura.(AU)

Introducción: El método clínico por sí solo no es definitivo para diagnosticar una enfermedad cerebrovascular, por lo que es importante el uso de medios de diagnóstico imagenológicos como la tomografía axial computarizada, que permite investigar los aspectos morfológicos y funcionales del cerebro. Objetivo: Describir las características clínicas y tomográficas de pacientes con infarto cerebral. Métodos: Se efectuó un estudio descriptivo y transversal de 269 pacientes con infarto cerebral, atendidos en el Hospital Provincial Docente Clinicoquirúrgico Saturnino Lora Torres de Santiago de Cuba, desde enero hasta diciembre de 2019. Resultados: Predominaron el sexo masculino (54,6 %), el grupo etario de 70-79 años, las afectaciones motoras (76,5 %), la alteración de la conciencia (51,3 %) y el trastorno del lenguaje (50,5 %). En 38 pacientes los resultados de la tomografía fueron negativos antes de las 24 horas de evolución y 64,0 % se consideraron mejorados con secuelas. Conclusiones: La tomografía antes de las 24 horas demuestra una baja positividad y los hallazgos evolucionan con el tiempo, aunque es posible reconocer varios signos precoces de ictus agudo en menos de 4-6 horas.
Introduction: The clinical method by itself is not definitive to diagnose a cerebrovascular disease, reason why the use of imaging diagnostic means, as the computerized axial tomography is important, as it allows to investigate the morphological and functional aspects of the brain. Objective: To describe the clinical and tomographic characteristics of patients with complete stroke. Methods: A descriptive and cross-sectional study of 269 patients with complete stroke, assisted in Saturnino Lora Torres Teaching Clinical Surgical Provincial Hospital was carried out in Santiago de Cuba, from January to December, 2019. Results: There was a prevalence of the male sex (54.6 %), 70-79 age group, motor disorders (76.5 %), alteration of the conscience (51.3 %) and language disorder (50.5 %). In 38 patients the results of the tomography were negative before the 24 hours of clinical course and 64.0 % were better with aftereffects of the disease. Conclusions: Tomography before the 24 hours demonstrates a low positivity and findings have a favorable clinical course as time goes by, although it is possible to recognize several early signs of acute ictus in less than 4-6 hours.

O megaesofago adquirido é uma causa pouco frequente de regurgitação em caninos. O diagnóstico é confirmado por radiografias simples ou contrastadas o caso de uma cadela de 9 anos com histórico de regurgitação por seis meses e endoscopia, tomografia, cintilografia ou ressonância magnética. Este trabalho relata Qualittas exames radiográficos simples e contrastados sem evidência de megaesofago. A esofagografia foi repetida por meio do fornecimento de contraste de bário misturado confirmar o megaesofago. Apesar de essa técnica ser pouco realizada, é um método a alimento comercial seco para cães, que permitiu evidenciar a dilatação esofágica e alternativo eficaz no diagnóstico de megaesofago canino, principalmente quando as demais abordagens radiográficas forem inconclusivas.(AU)
Diagnosis is confirmed by simple or contrast radiographs, endoscopy, tomography. dogs scintigraphy, or magnetic resonance imaging. Esophagography with barium sulfate contrast is the most commonly used method, however, it may be inconclusive dilation marking does not occur. This paper reports the case of a 9-year-old female dog, with a history of regurgitation over six months, simple and contrast radiographic exams showing no evidence of megaesophagus. The esophagography exam was repeated with the addition of barium contrast mixed with commercial dry pet food which verified esophageal dilatation and confirmed megaesophagus. Although this aluno de graduação technique is not widely used, it is an effective alternative method for diagnosis of canine.(AU)
El megaesofago adquirido es una causa poco frecuente de regurgitación en caninos. El diagnóstico se realiza por medio de radiografías simples y contrastadas. endoscopia, tomografía, centellografía o resonancia magnética. Este trabajo relata el caso de una perra de 9 años, con histórico de regurgitación de seis meses y exámenes radiográficos simples e contrastados sin evidencia de megaesofago. La esofagografia fue repetida utilizando contraste de bario mezclado con un alimento comercial seco para perros, que permitió diagnosticar la distensión esofágica, confirmando asi el megaesofago. A pesar de ser una técnica poco utilizada, la misma resultó ser un método alternativo eficiente en el diagnóstico de megaesófago adquirido canino, principalmente cuando las otras técnicas radiográficas no son conclusivas.(AU)

ABSTRACT Objective: to know the contributions of nursing in the implementation of the quality management principle of the accreditation program in imaging diagnosis. Methods: a single, qualitative case study carried out in an accredited radiology and imaging diagnosis service. The data collection took place through semi-structured interviews, direct observation, and documentary analysis with the support of software in organizing the data for analysis. Results: a total of four thematic units emerged: the accreditation process in imaging services, the implementation of the program, the role of nursing in imaging services and patient safety and the management of non-conformities in imaging services. Of the other data sources, the word risk was highlighted and a non-conformity was evidenced in the external audit. Final considerations: nursing contributed mainly to the management of the risks involved in the performance of imaging and patient safety tests, requirements of the quality management principle of the accreditation program.
RESUMEN Objetivo: conocer los aportes de la enfermería en la implementación del principio de gestión de calidad del programa de acreditación del diagnóstico por imagen. Métodos: estudio de caso único, cualitativo, en servicio acreditado de radiología e imagen diagnóstica. La recolección se realizó a través de entrevistas semiestructuradas, observación directa y análisis documental con apoyo de software para el análisis. Resultados: se obtuvieron cuatro clases temáticas: el proceso de acreditación en los servicios de imagenología, la implementación del programa, el rol de la enfermería en los servicios de imagenología y seguridad del paciente y la gestión de no conformidades en los servicios de imagenología. De los otros datos, se destacó la palabra riesgo y se detectó una no conformidad en la auditoría externa. Consideraciones finales: la enfermería contribuyó principalmente en la gestión de los riesgos involucrados en los exámenes de imagen y la seguridad del paciente, requisitos del programa de acreditación.

A coleta se deu através de entrevista semiestruturada, observação direta e análise documental com apoio de software na organização dos dados para análise. Resultados: obteve-se quatro classes temáticas: o processo de acreditação nos serviços de imagem, a implementação do programa, o papel da enfermagem nos serviços de imagem e na segurança do paciente e a gestão de não conformidades nos serviços de imagem. Das demais fontes de dados, destacou-se a palavra risco e foi evidenciada uma não conformidade na auditoria externa. Considerações finais: a enfermagem contribuiu principalmente na gestão dos riscos envolvidos na realização dos exames de imagem e segurança do paciente, requisitos do princípio gestão da qualidade do programa de acreditação.

Este trabalho relata Qualittas exames radiográficos simples e contrastados sem evidência de megaesofago. A esofagografia foi repetida por meio do fornecimento de contraste de bário misturado confirmar o megaesofago. Apesar de essa técnica ser pouco realizada, é um método a alimento comercial seco para cães, que permitiu evidenciar a dilatação esofágica e alternativo eficaz no diagnóstico de megaesofago canino, principalmente quando as demais abordagens radiográficas forem inconclusivas.
Diagnosis is confirmed by simple or contrast radiographs, endoscopy, tomography. dogs scintigraphy, or magnetic resonance imaging. Esophagography with barium sulfate contrast is the most commonly used method, however, it may be inconclusive dilation marking does not occur. This paper reports the case of a 9-year-old female dog, with a history of regurgitation over six months, simple and contrast radiographic exams showing no evidence of megaesophagus. The esophagography exam was repeated with the addition of barium contrast mixed with commercial dry pet food which verified esophageal dilatation and confirmed megaesophagus. Although this aluno de graduação technique is not widely used, it is an effective alternative method for diagnosis of canine.
El megaesofago adquirido es una causa poco frecuente de regurgitación en caninos. El diagnóstico se realiza por medio de radiografías simples y contrastadas. endoscopia, tomografía, centellografía o resonancia magnética. Este trabajo relata el caso de una perra de 9 años, con histórico de regurgitación de seis meses y exámenes radiográficos simples e contrastados sin evidencia de megaesofago. La esofagografia fue repetida utilizando contraste de bario mezclado con un alimento comercial seco para perros, que permitió diagnosticar la distensión esofágica, confirmando asi el megaesofago. A pesar de ser una técnica poco utilizada, la misma resultó ser un método alternativo eficiente en el diagnóstico de megaesófago adquirido canino, principalmente cuando las otras técnicas radiográficas no son conclusivas.
