RESUMO

Tracing methods of non-European EVOOs commercialized worldwide are becoming crucial for effective authenticity controls. Limited analytical studies of these oils are available on a global scale, similar to those of European EVOOs. We report for the first time the fatty acid concentrations, bulk-oil 2H/1H, 13C/12C, and 18O/16O ratios and fatty acid 13C/12C ratios of 43 authentic monovarietal EVOOs from different geographical regions in Argentina and Uruguay. The samples were obtained from a wide range of latitudes and altitudes along an E-W profile, from lowlands near the Atlantic Ocean to the pre-Andean highlands near the Pacific Ocean. Principal component scores were used to cluster EVOOs into three groups- central-western Argentina, central Argentina, and Uruguay-based on nine stable isotope ratios and the oleic-linoleic acid concentration ratio. The bulk 2H/1H and 18O/16O values and 13C/12C of palmitoleic and linoleic acids provide good tools for differentiating these oils via linear discriminant analysis.

The production and diversity of the supply of food products has grown exponentially in recent decades, along with the increase in cardiovascular diseases associated with poor diet. Among the factors, one of the most important is the lack of information for the consumer when purchasing. The present work evaluates the impact of implementing the traffic light labelling of foods in reducing deaths due to Noncommunicable Diseases (NCD). Econometric techniques of two-way effects panel data from 21 Ecuadorian provinces taken from official sources for the analysis period 2010-2019. For Ecuador, a favourable impact was found in the reduction of the NCD mortality rate, especially in the last three years; in 2017, it was reduced by 0.129%, in 2018 by 0.305%, and in 2019 by 0.289%. An implication of economic policy is the advertising regulation of products that may affect health, strengthening surveillance and sanctioning companies that fail to comply with the regulation.

AIMS: Fracture liaison services (FLS) use a multidisciplinary approach to treat patients who have experienced an osteoporotic fracture to reduce the risk of subsequent fractures. To date, there has been minimal FLS implementation in Latin America where fractures continue to be undertreated. This study aims to estimate the number of fractures averted, bed days avoided, and costs saved resulting from universal FLS implementation in Brazil, Mexico, Colombia, and Argentina. MATERIALS AND METHODS: A calculator was developed to estimate the annual benefits of FLS programs in Brazil, Mexico, Colombia, and Argentina from a public hospital perspective. It was assumed all patients with a hip, vertebral, or wrist fracture were referred to an FLS program. Country-specific data were obtained from a previous systematic review and interviews with osteoporosis experts. Hospitalization and post-hospitalization costs were expressed in 2019 USD without discounting. Costs of FLS implementation were not considered. RESULTS: In 2019, the number of FLS patients prevented from having a subsequent hip, vertebral, or wrist fracture was estimated as 15,607 in Brazil, 8,168 in Mexico, 5,190 in Argentina, and 2,435 in Colombia with total bed days saved of 142,378 in Brazil, 75,877 in Mexico, 52,301 in Argentina, and 21,725 in Colombia. The annual cost savings in 2019 were highest in Argentina (28.1 million USD), followed by Mexico (19.6 million USD), Brazil (7.64 million USD) and Colombia (3.04 million USD). Over five years (2019-2023) the cumulative cost savings were 145 million USD in Argentina, 106 million USD in Mexico, 40.5 million USD in Brazil, and 16.1 million USD in Colombia. CONCLUSION: Universal FLS implementation in Brazil, Mexico, Colombia, and Argentina was predicted to prevent 31,400 fractures, avoid 292,281 bed days, and save 58.4 million USD in 2019, though caution is warranted in the interpretation of these results due to high uncertainty. Increased implementation of FLS programs in Latin American countries may help to realize these benefits.

Aim: To elicit patients' preferences for HIV treatment of the rural population in Colombia.Methods: A discrete choice experiment (DCE), conducted in a HIV clinic in Bogotá, was used to examine the trade-off between five HIV treatment attributes: effect on life expectancy, effect on physical activity, risk of moderate side-effects, accessibility to clinic, and economic costs to access controls. Attributes selection was based on literature review, expert consultation and a focus group with six patients. An efficient experimental design was used to define two versions of the questionnaire with each of 12 choice sets and a dominance task was added to check reliability. A mixed logit model was then used to analyse the data and sub-group analyses were conducted on the basis of age, gender, education, and sexual preference.Results: A total of 129 HIV patients were included for analysis. For all treatment attributes, significant differences between at least two levels were observed, meaning that all attributes were significant predictors of choice. Patients valued the effect on physical activity (conditional relative importance of 27.5%) and the effect on life expectancy (26.0%) the most. Sub-group analyses regard age and education showed significant differences: younger patients and high educated patients valued the effect on physical activity the most important, whereas older patients mostly valued the effect on life expectancy and low educated patients mostly valued the accessibility to clinic.Limitations: One potential limitation is selection bias, as only patients from one HIV clinic were reached. Additionally, questionnaires were partly administered in the waiting rooms, which potentially led to noise in the data.Conclusions: This study suggests that all HIV treatment characteristics included in this DCE were important and that HIV patients from rural Colombia valued short-term efficacy (i.e. effect on physical activity) and long-term efficacy (i.e. effect on life expectancy) the most.

Aim: This study aimed to assess patients' preferences for HIV treatment in an urban Colombian population.Methods: A Discrete Choice Experiment (DCE) was conducted. Urban Colombian HIV patients were asked to repetitively choose between two hypothetical treatments that differ in regard to five attributes 'effect on life expectancy', 'effect on physical activity', 'risk of moderate side effects, 'accessibility to clinic' and 'economic cost to access controls'. Twelve choice sets were made using an efficient design. A Mixed Logit Panel Model was used for the analysis and subgroup analyses were performed according to age, gender, education level and sexual preference.Results: A total of 224 HIV patients were included. All attributes were significant, indicating that there were differences between at least two levels of each attribute. Patients preferred to be able to perform all physical activity without difficulty, to have large positive effects on life expectancy, to travel less than 2 h, to have lower risk of side-effects and to have subsidized travel costs. The attributes 'effect on physical activity' and 'effects on life expectancy' were deemed the most important. Sub-analyses showed that higher educated patients placed more importance on the large positive effects of HIV treatment, and a more negative preference for subsidized travel cost (5% level).Limitations: A potential limitation is selection bias as it is difficult to make a systematic urban/rural division of respondents. Additional, questionnaires were partly administered in the waiting rooms, which potentially led to some noise in the data.Conclusions: Findings suggests that short-term efficacy (i.e. effect on physical activity) and long-term efficacy (i.e. effect on life expectancy) are the most important treatment characteristics for HIV urban patients in Colombia. Preference data could provide relevant information for clinical and policy decision-making to optimize HIV care.

This study investigated the relationship between 18O and 2H isotopes in samples of Mexican hair and drinking water. The purpose of this study was twofold: to quantify the relationship between isotopes in Mexican hair and tap water, in order to understand the impact of water stress and differing socioeconomic status on accurate predictions of drinking water; and to determine whether currently existing semimechanistic models can accurately represent the relationship between hair and tap water. This study used a subset of paired samples of human hair (n = 62) and tap water (n = 76). Isotope values in tap water ranged from -11.4‰ to -4.3‰ and -79.1‰ to -22.5‰, and in hair from +9.5‰ to +16.1‰ and -90.8‰ to -53.7‰, for δ18O and δ2H, respectively. The most depleted δ18O and δ2H hair values came from individuals in the state of Morelos. For modern Mexican populations, positive correlations between isotopes in hair and water were not significant, with correlation coefficients r = 0.61 (p = 0.05) and r = 0.60 (p = 0.06) for 18O and 2H, respectively. Error-in-variables regression yielded linear fits that were somewhat better for 2H relative to 18O: δ18Oh = 0.183 [±0.132] δ18Otw + 15.7 [±0.9]‰ (r2 = 0.23); δ2Hh = 0.181 [±0.076] δ2Htw - 64.0 [±3.0]‰ (r2 = 0.34). In short, data from this Mexican population did not exhibit the strong relationships between isotope values of 18O and 2H in tap water and hair that have been characteristic of other populations studied to date. Given the economic stratification of this region and the poor correlation between hair and water samples, the authors considered the possibility that l, the fraction of the diet derived from local sources, and fs, the fraction of nonexchangeable H in keratin that was fixed in vivo, are local rather than global parameters for this population. The authors estimated different values of l and fs for each location. Given the anticipated importance of the nonlocal dietary contribution, they treated the isotopic content of nonlocal food and the offset parameters for predicting isotopes in locally derived food as tuning parameters and compared the results with parameters based on the American supermarket diet. They found that, although O and H isotopes in water and hair maintained similar geographic distributions, O and H isotopes in tap water explained only a small part of the variation observed in hair samples. Compared to the standard American supermarket diet, the Mexican estimates for nonlocal diet and local diet offsets predicted regional distributions of l and fs that cleanly segregated urban areas from rural towns.

RESUMO

This paper analyzes a randomized experiment that uses novel strategies to promote handwashing with soap at critical points in time in Peru. It evaluates a large-scale comprehensive initiative that involved both community and school activities in addition to communication campaigns. The analysis indicates that the initiative was successful in reaching the target audience and in increasing the treated population's knowledge about appropriate handwashing behavior. These improvements translated into higher self-reported and observed handwashing with soap at critical junctures. However, no significant improvements in the health of children under the age of 5 years were observed. Copyright © 2015 John Wiley & Sons, Ltd.

Stable oxygen isotopes are increasingly used in ecological research. Here, I present oxygen isotope (δ(18)O) values for bone carbonate and collagen from howler monkeys (Alouatta palliata), spider monkeys (Ateles geoffroyi) and capuchins (Cebus capucinus) from three localities in Costa Rica. There are apparent differences in δ(18)Ocarbonate and δ(18)Ocollagen among species. Monkeys from moist forest have significantly lower isotope values than those from drier localities. Because patterns are similar for both substrates, discrimination (Δ) between δ(18)Ocarbonate and δ(18)Ocollagen is relatively consistent among species and localities (17.6 ± 0.9‰). Although this value is larger than that previously obtained for laboratory rats, consistency among species and localities suggests it can be used to compare δ(18)Ocarbonate and δ(18)Ocollagen for monkeys, and potentially other medium-bodied mammals. Establishing discrimination for oxygen between these substrates for wild monkeys provides a foundation for future environmental and ecological research on modern and ancient organisms.

Canopy CO2 concentrations in a tropical rainforest in French Guiana were measured continuously for 5 days during the 1994 dry season and the 1995 wet season. Carbon dioxide concentrations ([CO2]) throughout the canopy (0.02-38 m) showed a distinct daily pattern, were well-stratified and decreased with increasing height into the canopy. During both seasons, daytime [CO2] in the upper and middle canopy decreased on average 7-10 µmol mol-1 below tropospheric baseline values measured at Barbados. Within the main part of the canopy (≥ 0.7 m), [CO2] did not differ between the wet and dry seasons. In contrast, [CO2] below 0.7 m were generally higher during the dry season, resulting in larger [CO2] gradients. Supporting this observation, soil CO2 efflux was on average higher during the dry season than during the wet season, either due to diffusive limitations and/or to oxygen deficiency of root and microbial respiration. Soil respiration rates decreased by 40% after strong rain events, resulting in a rapid decrease in canopy [CO2] immediately above the forest floor of about 50␣µmol mol-1. Temporal and spatial variations in [CO2]canopy were reflected in changes of δ13Ccanopy and δ18Ocanopy values. Tight relationships were observed between δ13C and δ18O of canopy CO2 during both seasons (r 2 > 0.86). The most depleted δ13Ccanopy and δ18Ocanopy values were measured immediately above the forest floor (δ13C = -16.4‰; δ18O = 39.1‰ SMOW). Gradients in the isotope ratios of CO2 between the top of the canopy and the forest floor ranged between 2.0‰ and 6.3‰ for δ13C, and between 1.0‰ and 3.5‰ for δ18O. The δ13Cleaf and calculated c i/c a of foliage at three different positions were similar for the dry and wet seasons indicating that the canopy maintained a constant ratio of photosynthesis to stomatal conductance. About 20% of the differences in δ13Cleaf within the canopy was accounted for by source air effects, the remaining 80% must be due to changes in c i/c a. Plotting 1/[CO2] vs. the corresponding δ13C ratios resulted in very tight, linear relationships (r 2 = 0.99), with no significant differences between the two seasons, suggesting negligible seasonal variability in turbulent mixing relative to ecosystem gas exchange. The intercepts of these relationships that should be indicative of the δ13C of respired sources were close to the measured δ13C of soil respired CO2 and to the δ13C of litter and soil organic matter. Estimates of carbon isotope discrimination of the entire ecosystem, Δe, were calculated as 20.3‰ during the dry season and as 20.5‰ during the wet season.
