RESUMO

Background: Refractive errors, including myopia, hyperopia, and astigmatism, are the leading causes of visual impairment in school-aged children and can significantly impact their academic performance and quality of life. This study aimed to assess the prevalence of refractive errors among school children from economically disadvantaged areas in Northwest México, using a consistent methodology to facilitate comparison with global data. Methods: We adopted the Refractive Error Study in Children (RESC) protocol by the World Health Organization to examine the prevalence of myopia, hyperopia, and astigmatism. The study comprised a systematic sampling of children aged 6 to 18 years from diverse schools in Northwest México. Trained optometrists conducted visual acuity testing and autorefraction, while ophthalmologists performed cycloplegic refraction to ensure accuracy. Results: The study found a myopia (SE ≤-1.50 D at least one eye) prevalence of 14.55% (95% CI: 13.27-15.91), with a higher incidence in females (6.92%) compared to males (6.00%) in at least one eye. Hyperopia (SE ≥ +1.00 D at least one eye) was less common, at 3.23% (95% CI: 2.61-3.95), with a slightly higher occurrence in males in at least one eye. Astigmatism (Cylinder ≥ 0.75 D at least one eye) was present in 18.63% (95% CI: 17.21-20.12) of the students in at least one eye, with no significant difference between genders. These findings are consistent with other studies in regions such as Puerto Rico and Iran, indicating widespread refractive error issues among schoolchildren. Conclusions: The high prevalence of refractive errors, particularly myopia and astigmatism, highlights the critical need for regular vision screenings in schools and the implementation of public health interventions to provide corrective eyewear. Our study confirms the importance of utilizing standardized methodologies like the RESC protocol to compare refractive error prevalence across different geographical and socio-economic contexts, thereby informing global public health strategies.

Objectives: The present study analyzed the reciprocal relationships between four common pediatric ophthalmic diseases (i.e., hyperopia, myopia, astigmatism, and strabismus) and attention‐deficit/hyperactivity disorder (ADHD) in children. Methods: This study enrolled 86,028 children with ADHD and 1,798,673 children without ADHD in the Taiwan Maternal and Child Health Database who were born at any time from 2004 to 2017. Cox proportional hazards regression models were used to estimate the bidirectional relationships of the four ophthalmic diseases with ADHD in children after adjusting for age, sex, and gestational age at birth. Survival curves for time-to-event variables were estimated using the Kaplan-Meier method, and the log-rank test was used to compare the curves. Results: The results indicated that ADHD significantly predicted the occurrence of hyperopia, myopia, astigmatism, and strabismus. Furthermore, hyperopia, myopia, astigmatism, and strabismus significantly predicted the occurrence of ADHD. The time between enrollment and ADHD diagnosis was shorter for patients with ophthalmic diseases than for the control group, and the time between enrollment and ophthalmic disease diagnosis was also shorter for ADHD patients than for the control group. Sex differences were found in the associations between ADHD and ophthalmic diseases. Conclusion: Clinicians should monitor children with ADHD for hyperopia, myopia, astigmatism, and strabismus to ensure appropriate treatment, and vice versa.

AIM: To determine the prevalence of refractive error in 5- to 17-year-old schoolchildren in Puerto Rico. METHODS: A quantitative descriptive study of 2867 children aged 5 to 17y from all seven educational regions of Puerto Rico was conducted from 2016-2019. Refractive error was determined via static and subjective refraction. Children with distance acuity ≤20/40 or near visual acuity ≤20/32 had a cycloplegic refraction. Data analysis included descriptive statistics, correlation coefficient, Kruskal-Wallis, Chi-square, and t test calculations. RESULTS: Twenty percent of the children had a spherical equivalent refractive error ≤ -0.50 D, 3.2% had a spherical equivalent ≥ +2.00 D, and 10.4% had astigmatism ≥1 D. There was a statistically (but non-clinically) significant myopic change in spherical equivalent refractive error with age (P<0.001). The prevalence of myopia increased with age (P<0.001) but not hyperopia (P=0.59) or astigmatism (P=0.51). Males had a significantly higher hyperopic spherical equivalent than females (P<0.001). Females had a higher prevalence of myopia (P<0.001) than males, but there was no difference in the hyperopia (P=0.74) or astigmatism prevalence (P=0.87). CONCLUSION: The prevalence of a spherical equivalent equal to or less than -0.50 D (myopia, 20.7%) is one of the highest among similar-aged children worldwide. Further studies should explore the rate of myopia progression in children in Puerto Rico. Individual children must be monitored to examine the need for treatment of myopia progression.

Introducción: La cirugía actual de la catarata no comprende solo restablecer la visión en su mayor cantidad posible, sino que incluye además la psicofísica de la visión. Objetivo: Evaluar los resultados de la cantidad, calidad visual y de vida en la cirugía refractiva bilateral simultánea del cristalino en pacientes hipermétropes. Métodos: Se realizó un estudio descriptivo longitudinal prospectivo de serie de casos en 108 ojos de 54 pacientes con catarata bilateral e hipermetropía operados por cirugía bilateral simultánea en el Instituto Cubano de Oftalmología "Ramón Pando Ferrer" desde noviembre 2018 hasta abril 2022. Se emplearon los porcentajes y los números absolutos para resumir las variables cualitativas. En el caso de las cuantitativas se usó la media con su desviación estándar (DE) y el intervalo de confianza del 95 por ciento. Resultados: Fue más frecuente el sexo femenino con una edad media de 69,1 ± 8,1 años. El 55,11 por ciento de los pacientes tuvo dureza NO3. La agudeza visual monocular mejoró en el 97,2 por ciento y el 100 por ciento la mejoró de forma bilateral. Hubo una pérdida celular endotelial de 10,19 por ciento. La visión de colores y la sensibilidad al contraste mostraron de igual manera mejoría significativa. Conclusiones: La cirugía bilateral simultánea del cristalino en pacientes hipermétropes contribuye de manera significativa a la mejoría de la visión en cantidad y calidad, por lo que permite recuperar la calidad de vida(AU)


Introduction: Current cataract surgery does not comprise only restoring vision to its greatest possible quantity, but also includes the psychophysics of vision. Objective: To evaluate the results of quantity, visual quality and quality of life in simultaneous bilateral refractive surgery of the crystalline lens in hyperopic patients. Methods: A prospective longitudinal descriptive case series prospective study was performed in 108 eyes of 54 patients with bilateral cataract and hyperopia operated by simultaneous bilateral surgery at the Cuban Institute of Ophthalmology "Ramón Pando Ferrer" from November 2018 to April 2022. Quality of life was also taken into account by the FV-14 survey. Percentages and absolute numbers were used to summarize the qualitative variables. In the case of quantitative variables, the mean with standard deviation (SD) and 95 percent confidence interval were used. Results: The female gender was more frequent with an average age of 69.1±8.1 years. 55.11 percent of patients had NO3 hardness. Monocular visual acuity improved in 97.2 percent and 100 percent improved bilaterally. There was an endothelial cell loss of 10.19 percent. Color vision and contrast sensitivity likewise showed significant improvement. Conclusions: Simultaneous bilateral lens surgery in hyperopic patients, contributes significantly to the improvement of vision in quantity and quality, thus allowing recovery of quality of life(AU)

Objetivo: Analizar la presión intraocular posterior a la cirugía bilateral simultánea del cristalino en pacientes hipermétropes. Métodos: Se realizó un estudio prospectivo y analítico de serie de casos, con 108 ojos de 54 pacientes con catarata bilateral e hipermetropía, operados por cirugía bilateral simultánea, en el Instituto Cubano de Oftalmología "Ramón Pando Ferrer", desde noviembre de 2018 hasta abril de 2022. Se analizaron las variables edad, sexo, dureza nuclear, mejor agudeza visual con y sin corrección, monocular y binocular, profundidad de cámara anterior y longitud axil, todo esto relacionado a la presión intraocular. Resultados: Se emplearon los porcentajes y los números absolutos para resumir las variables cualitativas; en el caso de las cuantitativas se usó, la media con su desviación estándar y el intervalo de confianza del 95 por ciento. El sexo femenino fue el más frecuente (57,4 por ciento), la edad media fue de 69,1 ± 8,1 años. El 55,11 por ciento de los pacientes tuvo dureza NO3. La mejor agudeza visual corregida monocular y bilateral mejoraron de manera significativa, hubo una pérdida celular endotelial del 10,19 por ciento. No se observó asociación significativa de la presión intraocular con la longitud axil, la profundidad de la cámara anterior, esfera refractiva, ni la dureza del cristalino, la cual disminuyó después de la cirugía. Conclusipor cientoones: La cirugía bilateral simultánea del cristalino en pacientes hipermétropes, contribuye de manera significativa a la mejoría de la visión y a la disminución de la presión intraocular posoperatoria(AU)

There was no significant association of intraocular pressure with axillary length, anterior chamber depth, refractive sphere, nor lens hardness, which decreased after surgery. Conclusions: Simultaneous bilateral lens surgery in hyperopic patients significantly contributes to improved vision and decreased postoperative intraocular pressure(AU)

RESUMO

A 16-year-old boy with elevated hyperopia presented to the office with a 24-h history of bilateral blurred vision, mainly of the left eye, and bilateral central serous chorioretinopathy. He showed a clinically recognizable bacillary layer detachment in one eye and excellent multimodal diagnostic image correlation, with the best-corrected visual acuity as 20/400. He had bilateral serous retinal detachment, as confirmed by optical coherence tomography. Laser photocoagulation was performed with good results, and reestablishment of the foveal anatomical structure was documented 16 days after treatment.

El nacimiento prematuro favorece la aparición de alteraciones visuales donde los defectos refractivos altos prevalecen, pues se afecta el proceso de emetropización. La revisión tuvo como objetivo describir la miopía del prematuro como resultado de una alteración en el desarrollo del segmento anterior. La hipermetropía se observa de mayor cuantía en estos pacientes, así como el astigmatismo significativo. Entre los factores de mayor peso se encuentran, además de la prematuridad, el bajo peso al nacer, la presencia de retinopatía del prematuro y su tratamiento con láser. Las evaluaciones oftalmológicas sistemáticas permiten detectar y corregir a tiempo estas alteraciones y favorecen una función visual óptima con menor riesgo de ambliopía. Los artículos que se consultaron son fundamentalmente de los últimos cinco años, en idiomas español e inglés, disponibles en textos completos y resúmenes en algunas bases de datos como PubMed, Ebsco, Google Académico y Scielo(AU)


Preterm birth may affect the emmetropization process, leading to the appearance of visual alterations characterized by a high prevalence of refractive defects. A case is described of myopia of prematurity resulting from an alteration in the development of the anterior segment. Hyperopia and astigmatism are significantly frequent in these patients. Other causative factors besides prematurity are low birth weight, the presence of retinopathy of prematurity and its treatment with laser therapy. Systematic ophthalmologic evaluation allows early detection and correction of these alterations, leading to optimal visual function and a lower risk for amblyopia. The articles consulted are mainly from the last five years, written in Spanish or English, and available as full texts and abstracts in databases like PubMed, EBSCO, Google Scholar and SciELO(AU)

Abstract Provisions for migration underlie asymmetric relations between sending and receiving countries, such as the cases of Mexico and the United States of America, although studies focus on stigma. The objective of this study was to establish the reliability and construct validity of an instrument that measures the phenomenon. A transversal and exploratory work was carried out with a selection of 300 students, considering their affiliation to a public university in a strategic alliance with multinationals for vocational training. A structure of four factors related to risk, utility, hyperopia and identity was observed, although the design of the research limited the results to the research scenario, suggesting the extension of the work towards negative dispositions such as exclusion, discrimination and the stigma.


Resumen Las disposiciones para la migración subyacen en las relaciones asimétricas entre los países de origen y destino, como los casos de México y los Estados Unidos de América, aunque los estudios se centran en el estigma. El objetivo de este estudio fue establecer la fiabilidad y la validez de constructo de un instrumento que mide el fenómeno. Se realizó un trabajo transversal y exploratorio con una selección de 300 estudiantes, considerando su afiliación a una universidad pública en una alianza estratégica con multinacionales para la formación profesional. Se observó una estructura de cuatro factores relacionados con el riesgo, la utilidad, la hipermetropía y la identidad, aunque el diseño de la investigación limitó los resultados al escenario de la investigación, lo que sugiere la extensión del trabajo hacia disposiciones negativas como la exclusión, la discriminación y el estigma. AIM: To determine the distribution of refractive errors in a school-age population in Quintana Roo (Mexico) in the framework of an international cooperation campaign for the prevention of blindness. METHODS: A sample of 2647 school-age children (ranging from 5 to 14 years old) with a mean age of 9.1±1.9 years old were tested by trained volunteers for distance visual acuity (VA) and refractive errors. The first screening examination included uncorrected distance visual acuity (UDVA) and VA with a +2.00 D lens. Inclusion criteria for a second complete cycloplegic eye examination performed by an optometrist were UDVA <20/25 (0.10 logMAR or 0.8 decimal) and/or VA with +2.00 D ≥20/25. RESULTS: A total of 633 (23.9%) children underwent the second complete eye examination. Mean logMAR UDVA was 0.035±0.094 (range 1.00 to 0.00 logMAR) for the right eyes and 0.036±0.160 (range 1.00 to 0.00 logMAR) for the left eyes. Bilateral amblyopia was found in 17 children (2.7% of refracted eyes; 0.64% of the total). The main reason for visual impairment (VI) in the sample analyzed was found to be refractive errors. In 12 children (1.9% of refracted eyes; 0.45% of the total) the VI was bilateral and 9 (1.4% of refracted eyes; 0.34% of the total) achieved a corrected distance visual acuity of 20/25 or better in both eyes. Mean magnitude of sphere and refractive cylinder was +0.20±0.96 D and -0.43±0.85 D in right eyes, and +0.24±1.08 and -0.43±0.83 D in left eyes. The proportion of myopic eyes [standard equivalent (SE) ≤-0.50 D] was 4.6% of the whole sample (5290 eyes). The mean magnitude of myopia was -0.84±3.44 D for the right eyes and -0.82±5.21 D for the left eyes. The proportion of hyperopic patients (SE≥+2.00 D) was 2.4% (15/633), which corresponded to 0.60% of the whole sample (32/5290 eyes). No statistically significant correlation of age to manifest sphere or cylinder was found. CONCLUSION: VI due to uncorrected refractive errors can be easily corrected with glasses but it is still a burden to be treated. Myopia is prevalent in this sample. More efforts towards correcting uncorrected refractive errors are needed.

ABSTRACT The following report describes a case of apical leucoma syndrome after hyperopic photorefractive keratectomy following hyperopic laser in situ keratomileusis and the subsequent treatment of this complication with focal phototherapeutic keratectomy. The patient underwent focal transepithelial phototherapeutic keratectomy of 70 mm ablation and 1.5 mm diameter after pupil and ablation offset correction. After 1 year, significant improvement in vision was observed and only slight residual opacity remained on biomicroscopy and optical coherence tomography. Focal phototherapeutic keratectomy was effective and safe for the treatment of this complication.


RESUMO O relato a seguir descreve um caso da síndrome do Leucoma Apical após cirurgia ceratorrefrativa hipermetrópica depois de Laser hiperópico in situ keratomileusis, e o subsequente tratamento desta complicação com ceratectomia fototerapêutica focal. O paciente foi submetido à ceratectomia fototerapêutica focal transepitelial com ablação de 70 mm e diâmetro de 1,5 mm, após correção de offset pupilar. Depois de um ano, foi observada uma melhora significativa da acuidade visual permanecendo apenas leve opacidade residual na biomicroscopia e tomografia de coerência óptica. A ceratectomia fototerapêutica focal foi efetiva e segura para o tratamento desta complicação.

ABSTRACT Purpose: The aim of this study was to establish the frequency of refractive errors in children and adolescents aged between 8 and 17 years old, living in the metropolitan area of Bucaramanga (Colombia). Methods: This study was a secondary analysis of two descriptive cross-sectional studies that applied sociodemographic surveys and assessed visual acuity and refraction. Ametropias were classified as myopic errors, hyperopic errors, and mixed astigmatism. Eyes were considered emmetropic if none of these classifications were made. The data were collated using free software and analyzed with STATA/IC 11.2. Results: One thousand two hundred twenty-eight individuals were included in this study. Girls showed a higher rate of ametropia than boys. Hyperopic refractive errors were present in 23.1% of the subjects, and myopic errors in 11.2%. Only 0.2% of the eyes had high myopia (≤-6.00 D). Mixed astigmatism and anisometropia were uncommon, and myopia frequency increased with age. There were statistically significant steeper keratometric readings in myopic compared to hyperopic eyes. Conclusions: The frequency of refractive errors that we found of 36.7% is moderate compared to the global data. The rates and parameters statistically differed by sex and age groups. Our findings are useful for establishing refractive error rate benchmarks in low-middle-income countries and as a baseline for following their variation by sociodemographic factors.


RESUMO Objetivo: O objetivo deste estudo foi estabelecer a frequência de erros refrativos em crianças e adolescentes com idade entre 8 e 17 anos, residentes na região metropolitana de Bucaramanga (Colômbia). Métodos: Este estudo foi uma análise secundária de dois estudos descritivos transversais que aplicaram levantamentos sociodemográficos e avaliaram a acuidade e a refração visuais. As ametropias foram classificadas como erros miopicos, erros hipermetrópicos e astigmatismo misto. Os olhos eram considerados emétropes se nenhuma dessas classificações fosse feita. Os dados foram coletados usando software livre e analisados com STATA/IC 11.2. Resultados: Mil e duzentos e vinte e oito indivíduos foram incluídos neste estudo. As meninas mostraram uma maior taxa de ametropia do que os meninos. Erros refrativos hipermetrópicos estavam presentes em 23,1% dos indivíduos e erros miópicos em 11,2%. Apenas 0,2% dos olhos apresentavam miopia alta (≤-6,00 D). O astigmatismo misto e a anisometropia eram incomuns e a frequência de miopia aumentava com a idade. Houve leituras queratométricas mais acentuadas estatisticamente significativas em míopes em comparação com os olhos hipermétropes. Conclusões: A frequência de erros de refração que encontramos em 36,7% é moderada em comparação com os dados globais. As taxas e os parâmetros diferiram estatisticamente por sexo e grupos etários. Nossas descobertas são úteis para estabelecer padrões de referência de erro de refração em países de baixa renda média e como base para seguir sua variação por fatores sociodemográficos.

Resumo Objetivo: Comparar as alterações da refração e da biometria ocular na população infantil hipermetrópica com e sem correção óptica total. Métodos: Realizou-se estudo prospectivo longitudinal não randomizado em 41 pacientes com hipermetropia, entre 3 e 6 dioptrias ou/e com esotropia acomodativa pura nos ambulatórios do Hospital Geral Universitário e Oftalmocenter Santa Rosa, com idade inicial entre 4 e 6 anos. Os pacientes foram divididos em dois grupos, em que o Grupo 1 compôs-se pelos pacientes hipermétropes que não necessitavam usar sua correção óptica ou poderiam usá-la parcialmente, e o Grupo 2 por pacientes com esotropia acomodativa pura e pelos hipermétropes que necessitavam usar toda sua correção óptica. Os pacientes submeteram-se a exame oftalmológico completo, incluindo refração objetiva em autorrefrator com cicloplegia, biometria óptica e topografia corneana em uma medida inicial e outra 3 anos mais tarde. Comparou-se a refração e parâmetros biométricos com teste T student. Resultados: A média da idade inicial foi de 5,23 ± 0,81 e 5,36 ± 0,74 anos, a refração inicial foi +3,99 ± 0,92 e +4,27 ± 0,85 D, o diâmetro anteroposterior do globo ocular foi de 21,42 ± 0,84 e 21,22 ± 0,86 mm, e a ceratometria foi de 42,55 ± 1,24 e 42,39 ± 1,22 D, para os Grupos 1 e 2, respectivamente. Em relação à refração, houve redução significativa do poder esférico no Grupo 1, em 3 anos; e não houve no Grupo 2 (p<0,05). Com relação ao diâmetro anteroposterior do globo ocular, ocorreu aumento significativo no Grupo 1 e não houve no Grupo 2 (p<0,05 ). Não se verificou diferença significativa na comparação das ceratometrias em 3 anos nos Grupos 1 e 2. Conclusão: Estes dados permitiram concluir que a correção total da hipermetropia pode prejudicar a emetropização natural em crianças.


Abstract Objective: To compare changes in refraction and ocular biometric parameters in hyperopic children with and without full optical correction. Methods: Non-randomized prospecting study with 41 subjects (21 males and 20 females) aged 4 to 6 years with accommodative esotropia and or hyperopia between 3 to 6 diopters, select in Hospital Geral Universitário and Oftalmocenter Santa Rosa. The patients were divided in two groups: group 1 for hyperopic patients that did not need to use optical correction or could use partial correction, and group 2 for patients with accommodative esotropia or hyperopia who needed to use full optical correction all the time. The patients were examined to a complete ophthalmological examination, including objective cycloplegic refraction with auto refractometer, optical biometry and corneal topography, in baseline measurements and 3 years after that. Refraction and ocular biometric parameters were compared using T student test. Results: The mean initial age was 5.23 ± 0.81 and 5.36 ± 0.74 years; the initial refractive error in average was +3.99 ± 0.92 e +4.27 ± 0.85 D, the initial axial length was 21.42 ± 0.84 and 21.22 ± 0.86 mm, and initial keratometry was 42.55 ± 1.24 e 42.39 ± 1.22 D for group 1 and 2, respectively. In relation to refractive error, there was a significant decrease in group 1 and there was not in group 2 (p < 0.05). In relation to axial length, there was significant increase in group1 and there was not in group 2 (p<0.05). The 3-year comparison showed no statistically significant differences in keratometry for both groups. Conclusion: This study suggests that full optical correction of hyperopia may inhibit natural emmetropization during early and late childhood.

Desde el primer reporte de pliegues coroideos, en el año 1884, el conocimiento sobre su etiología, el mecanismo de producción y su manejo se ha incrementado notablemente. Las causas de los pliegues coroideos abarcan un extenso número de condiciones muy variadas; entre ellas se encuentran los idiopáticos, la hipertensión intracraneana, algunas drogas como el topiramato, las infiltraciones difusas de la coroides por linfomas e hiperplasia linfoide, la hipotonía por contracción de tejido fibrovascular, los tumores coroideos y orbitarios (intraconales y extraconales), entre otras. Se presenta una paciente femenina de 57 años de edad quien acudió por vez primera a la Consulta de Oftalmología en el año 2008 por disminución lenta y progresiva de la agudeza visual de ambos ojos. El examen de fondo de ojo mostró discos ópticos con bordes definidos, buena coloración y presencia de pliegues coroideos radiales al disco en polo posterior de ambos ojos. La refracción arrojó una hipermetropía que fue en aumento en las consultas de seguimiento hasta el presente sin variar el aspecto del fondo de ojo. Se concluye que el caso presenta síndrome de hipermetropía adquirida con pliegues coroideos(AU)


Since the first reported case of choroidal folds in 1884, the understanding of their mechanisms, aetiologies, and management has expanded signally. With ophthalmoscopy, choroidal folds can be appreciated by the light and dark bands observed deep to the retina. While choroidal folds are visible on ophthalmoscopic examination, they are more easily identified using fluorescein angiography. A-scan ultrasound may reveal a shortened axial length. Common B-scan ultrasonographic findings include thickening of the choroid or flattening of the posterior aspect of the globe. Causes of choroidal folds are many, among them are idiophatic, increased intracranial pressure, diffusely infiltrative conditions (lymphomas and benign lymphoid hiperplasia), hypotony, drug induced (an unusual complication of certain medications such as Topiramate), contraction of fibrovascular tissue, choroidal neoplasms and orbital mass lesions (intraconal and extraconal tumours). We report a 57 years old woman who came to ophthalmogy consultation first in 2008 complaning of slowly progressive loss of vision. Fundoscopy showed well defined optic disc with radial choroidal folds of the posterior pole in both eyes. Manifest refraction showed hyperopia that increased in the following visits until the present. Fundoscopy showed not changes. Definitive diagnosis was syndrome of acquired hyperopia with choroidal folds(AU)

AIM: To assess the proportion of refractive errors in the Mexican population that visited primary care optometry clinics in fourteen states of Mexico. METHODS: Refractive data from 676 856 patients aged 6 to 90y were collected from optometry clinics in fourteen states of Mexico between 2014 and 2015. The refractive errors were classified by the spherical equivalent (SE), as follows: sphere+½ cylinder. Myopia (SE>-0.50 D), hyperopia (SE>+0.50 D), emmetropia (-0.50≤SE≤+0.50), and astigmatism alone (cylinder≥-0.25 D). A negative cylinder was selected as a notation. RESULTS: The proportion (95% confidence interval) among all of the subjects was hyperopia 21.0% (20.9-21.0), emmetropia 40.7% (40.5-40.8), myopia 24.8% (24.7-24.9) and astigmatism alone 13.5% (13.4-13.5). Myopia was the most common refractive error and frequency seemed to increase among the young population (10 to 29 years old), however, hyperopia increased among the aging population (40 to 79 years old), and astigmatism alone showed a decreasing trend with age (6 to 90y; from 19.7% to 10.8%). There was a relationship between age and all refractive errors (approximately 60%, aged 50 and older). The proportion of any clinically important refractive error was higher in males (61.2%) than in females (58.3%; P<0.0001). From fourteen states that collected information, the proportion of refractive error showed variability in different geographical areas of Mexico. CONCLUSION: Myopia is the most common refractive error in the population studied. This study provides the first data on refractive error in Mexico. Further programs and studies must be developed to address the refractive errors needs of the Mexican population.

AIM: To evaluate the distribution of refractive error in young subjects in a rural area of Paraguay in the context of an international cooperation campaign for the prevention of blindness. METHODS: A sample of 1466 young subjects (ranging from 3 to 22 years old), with a mean age of 11.21±3.63 years old, were examined to assess their distance visual acuity (VA) and refractive error. The first screening examination performed by trained volunteers, included visual acuity testing, autokeratometry and non-cycloplegic autorefraction. Inclusion criteria for a second complete cycloplegic eye examination by an optometrist were VA <20/25 (0.10 logMAR or 0.8 decimal) and/or corneal astigmatism ≥1.50 D. RESULTS: An uncorrected distance VA of 0 logMAR (1.0 decimal) was found in 89.2% of children. VA <20/25 and/or corneal astigmatism ≥1.50 D was found in 3.9% of children (n=57), with a prevalence of hyperopia of 5.2% (0.2% of the total) in this specific group. Furthermore, myopia (spherical equivalent ≤-0.5 D) was found in 37.7% of the refracted children (0.5% of the total). The prevalence of refractive astigmatism (cylinder ≤-1.50 D) was 15.8% (0.6% of the total). Visual impairment (VI) (0.05≤VA≤0.3) was found in 12/114 (0.4%) of the refracted eyes. Main causes for VI were refractive error (58%), retinal problems (17%, 2/12), albinism (17%, 2/12) and unknown (8%, 1/12). CONCLUSION: A low prevalence of refractive error has been found in this rural area of Paraguay, with higher prevalence of myopia than of hyperopia.

PURPOSE: To describe ocular component values and refraction in 6-17-year-old school children in Campinas, Brazil. METHODS: To obtain a representative sample, a total of 1100 6-17-year-old public school students in Campinas, Brazil, were randomly selected and divided into groups according to median ages 6 years (range 5-7 years), 10 years (range 9-11 years), 14 years (range 13-15 years), and 17 years (range 16-18 years). Cycloplegic autorefraction and ocular biometry were performed on each child. RESULTS: Parental consent for participation was obtained for 778 of the 1100 students (70.7%). Of these, 440 (56.6%) were female and 330 (44.4%) male. Sex distributions were similar for each median age group. Median age groups 6, 10, 14, and 17 years displayed significant trends of decreasing mean spherical equivalent refraction (SER; 0.96, 0.89, 0.57, and 0.23 diopters, D, respectively), mean lens thickness (3.50, 3.42, 3.41, and 3.45 mm, respectively), and mean lens power (23.6, 22.7, 21.8, and 21.2D, respectively), but significant trends of increasing mean anterior chamber depth (ACD; 3.00, 3.12, 3.16, and 3.15 mm, respectively), mean vitreous chamber depth (VCD; 16.0, 16.4, 16.6, and 16.8 mm, respectively), and mean axial length (22.5, 23.0, 23.2, 23.4 mm, respectively; all p for trend <0.001). CONCLUSIONS: Significant trends towards lower refractive power, increased prevalence of myopia, increased ACD, VCD, and axial length, and decreased lens power were associated with increasing age in these children. There were no significant changes in corneal thickness or corneal curvature.

RESUMO Objetivo: Investigar a associação da hipermetropia com ambliopia, estrabismo, anisometropia e astigmatismo. Métodos: A hiperopia foi classificada em Grupo 1: maior ou igual a +5.00D; Grupo 2: maior que +3.25D e menor que +5.00D, com diferença de equivalente esférico maior ou igual a 0.50D; Grupo 3: maior que +3.25D e menor que +5.00D, com diferença de equivalente esférico menor que 0.50D e Grupo 4: com equivalente esférico maior e igual a +2.00D. O Grupo controle pertencente ao equivalente esférico menor que +2.00D. Resultados: A presença de hipermetropia maior e igual a SE+2.00D foi significativamente associada à maior proporção de crianças com ambliopia (27,2 vs. 14,8%, OR = 2,150, p<0,001) e estrabismo (70,8 vs. 39,3%, OR = 3,758, p<0,0001. A presença de hipermetropia também foi significativamente associada à maior proporção de anisometropia nos grupos com hipermetropia maior e igual a SE+2.00 (29,1 vs. 9,9%, OR = 3,708, p<0,0001) e astigmatismo (24 vs. 9,9%, OR = 2,859 p<0,0001). Conclusão: A presença e magnitude da hipermetropia entre crianças foram associadas à maior proporção de erros refrativos e visuais, como estrabismo, ambliopia, astigmatismo e anisometropia.


RESUMO Objective: Evaluate ocular trauma cases related to falling in elderly patients e compare the prevalence and severity of the cases. Methods: A series of cases was made with 52 patients aging 60 or more within the period of 36 months presenting ocular trauma related to falling, whereas the prevalence between the gender, the need for hospitalization or surgery and subsequent visual deficit were evaluated, as well as the severity of the cases. Results: Thirty-three (63.5%) of 52 patients were from the female gender, over which 30.3% had need for surgery and 18.2% developed visual deficit and 19 (36.5%) were from the male gender where 42.1% needed surgery and 26.3% developed visual deficit. Conclusions: The study has shown a higher prevalence of cases in the female gender, although the severity was higher in the male gender.

PURPOSE: This study sought to determine the prevalence of myopia in schoolchildren of a rural population in Mexico. METHODS: A cross-sectional study was conducted in 317 children between 6 and 12 years old. A complete refractive examination was performed, including static retinoscopy without cycloplegic agents. All procedures were conducted according to the Declaration of Helsinki. RESULTS: In total, 9.7% (95% CI: 13.07-6.52) of the examined children were myopic (spherical equivalent ≤ -0.50 D), 4.4% (95% CI: 6.66-2.14) presented astigmatism (cylinder ≤ -1.50 D), and 5.4% (95% CI: 7.89-2.91) presented hyperopia (spherical equivalent ≥ +0.50 D). CONCLUSION: Additional research is required to assess the prevalence of refractive errors in rural areas in Mexico, to analyze the associated risk factors, and to implement appropriate eye care plans for this population.
