Introducción: El tratamiento de las Hernias Hiatales (HH) tipo III y IV es quirúrgico. Hay controversia sobre el refuerzo con malla. Nuestro objetivo fue comparar los resultados a largo plazo entre el uso o no de refuerzos protésicos. Materiales y Métodos: Cohorte prospectiva de 95 pacientes con HH tipo III y IV, entre los años 1997 y 2015 en el Hospital Clínico de la Universidad de Chile. Se evaluaron las características radiológicas, endoscópicas y funcionales pre y postoperatorias. Recidiva definida como recurrencia mayor a 3 cm. Análisis estadístico con chi2 y Test U-Mam-Whitney. P-value a 10 años) de HH tipo III y IV reparadas quirúrgicamente, no hay diferencias en la recidiva clínica con o sin el uso de mallas.


Introduction: The treatment of Hiatal Hernias (HH) type III and IV is surgical. There is controversy about reinforcement with mesh. Our objective was to compare the long-term results between the use or not of prosthetic reinforcements. Materials and Methods: Prospective cohort of 95 patients with HH type III and IV, between 1997 and 2015 at the Clinical Hospital of the University of Chile. Pre and postoperative radiological, endoscopic and functional characteristics were evaluated. Recurrence defined as a recurrence greater than 3 cm. Statistical analysis with chi2 and U-Mann-Whitney test. p-value 10 years) of surgically repaired type III and IV HH, there are no differences in clinical recurrence with or without the use of mesh.

La hernia hiatal gigante consiste en el desplazamiento hacia el tórax de más del 30 % del estómago, lo que origina una protrusión anormal por encima de la pinza diafragmática. Esto puede ocurrir por varios mecanismos como la aparición de alteraciones a nivel de la unión gastroesofágica o por atrofia diafragmática, que puede darse por varias razones como cambios involutivos, traumatismos extensos o daños ocasionados al nervio frénico. La mayoría de casos suelen ser asintomáticos, sin embargo, cuando se presentan manifestaciones clínicas, estas varían dependiendo del tamaño que presente la herniación, y van desde dolor torácico (también epigástrico), náuseas, vómitos o distensión abdominal. En caso de presentar complicaciones como vólvulo gástrico o úlceras de Cameron, los síntomas incluyen la tríada de Borchardt, que consta de dolor y distensión abdominal, vómitos violentos y dificultad para pasar la sonda nasogástrica. Se presenta el caso de un paciente masculino de 82 años que ingresó por emergencia por presentar dolor abdominal intenso, hematemesis abundante y trastorno del sensorio. En el examen físico se observa una facies pálida y diaforesis; a la palpación, el indicativo principal es el dolor abdominal en la zona epigástrica. Se solicita endoscopia de emergencia, que revela una úlcera profunda con bordes congestivos y sangrado activo en la porción inferior del esófago, además de observar también que parte del fondo gástrico y el cardias se hernian hacia el tórax. Esto confirmó el diagnóstico de una hernia hiatal gigante, sin embargo, el tratamiento es interrumpido por el paciente al pedir el alta voluntaria tras negarse a una intervención quirúrgica.


Giant hiatal hernia is a condition in which more than 30 % of the stomach is displaced upward toward the thorax, thus producing an abnormal protrusion above the diaphragmatic clamp. This can occur by various mechanisms such as alterations at the gastroesophageal junction or diaphragmatic atrophy that can take place due to a number of reasons, including involutional changes, extensive trauma or damage to the phrenic nerve. Most cases are usually asymptomatic; however, when clinical manifestations occur, they vary depending on the size of the herniation and range from chest pain (also epigastric pain), nausea, vomiting to abdominal distension. In case of presenting complications such as gastric volvulus or Cameron lesions, the symptoms include Borchardt's triad, which consists of abdominal pain and distension, violent vomiting and difficulty passing a nasogastric tube. We present the case of an 82-year-old male patient who was admitted to the emergency room for severe abdominal pain, severe hematemesis and sensory disorder. On physical examination, pale facies and diaphoresis were observed; on palpation, the main indication was abdominal pain in the epigastric area. Emergency endoscopy was requested, finding a deep ulcer with congestive edges and active bleeding in the lower portion of the esophagus, in addition to observing that part of the gastric fundus and the cardias were herniated toward the thorax. This confirmed the diagnosis of a giant hiatal hernia; however, the treatment was interrupted by the patient when he asked for voluntary discharge after refusing to undergo surgery.

La tasa de recidiva radiológica luego de la cirugía de las hernias hiatales grandes tipo IV es cercana al 50%. La mayoría de las veces la recidiva supone un ascenso parcial de la funduplicatura al tórax (telescopaje), y si no tiene expresión clínica puede optarse por un manejo conservador (solo 3-6% requieren cirugía de revisión). Sin embargo, en ocasiones hay otro tipo de recidivas (paraesofágicas) que deben valorarse mediante estudios funcionales, endoscópicos y radiológicos, que dan disfagia con mayor frecuencia. Un tránsito contrastado digestivo superior permite en este caso identificar el tipo de recidiva herniaria, con migración parcial del fundus gástrico hacia el tórax paraesofágico izquierdo, y una funduplicatura continente pero realizada técnicamente en por debajo de lo deseable (cuerpo alto) (figura). La resolución del cuadro implica la reducción de la hernia a la cavidad abdominal, con cierre del defecto diafragmático, desmontar la funduplicatura anterior anatomizando la zona y la reconfección de una nueva funduplicatura.

ABSTRACT BACKGROUND: The influence of body mass index on perioperative complications of hiatal hernia surgery is controversial in the surgical literature. AIMS: The aim of this study was to evaluate the influence of body mass index on perioperative complications and associated risk factors for its occurrence. METHODS: Two groups were compared on the basis of body mass index: group A with body mass index <32 kg/m2 and group B with body mass index ³32 kg/m2. A multivariate analysis was carried out to identify independent predictors for complications. Complications were classified based on the Clavien-Dindo score. RESULTS: A total of 49 patients were included in this study, with 30 in group A and 19 in group B. The groups were compared based on factors, such as age, Charlson Comorbidity Index, surgical techniques used, type and location of hiatal hernia, and length of stay. Findings showed that 70% of patients had complex hiatal hernia. In addition, 14 complications also occurred: 7 pleuropulmonary and 7 requiring reoperation. From the seven reoperated, there were three recurrences, two gastrointestinal fistulas, one diaphragmatic hernia, and one incisional hernia. Complications were similar in both the groups, with type IV hiatal hernia being the only independent predictor. CONCLUSIONS: Body mass index does not affect perioperative complications in anti-reflux surgery and type IV hiatal hernia is an independent predictor of its occurrence.


RESUMO RACIONAL: O impacto do índice de massa corpórea nos resultados da cirurgia de hérnia de hiato é controverso na literatura. OBJETIVOS: avaliar o impacto do índice de massa corpórea nas complicações perioperatórias em pacientes submetidos a cirurgia de hérnia hiatal, e seus possíveis preditores. MÉTODOS: análise retrospectiva 49 pacientes submetidos a tratamento cirúrgico de hérnias hiatais complexas por videolaparoscopia, divididos em dois grupos pelo índice de massa corpórea (grupo A<32kg/m2 - 30 pacientes e grupo B ³32 kg/m2 — 19 pacientes) e comparados quanto suas características e complicações. A análise multivariada foi aplicada para avaliar as variáveis preditoras independentes de complicações. As complicações foram classificadas conforme Clavien Dindo. RESULTADOS: Os grupos foram similares conforme a idade, índice de comorbidade de Charlson, técnica operatória empregada, tipo de hérnia de hiato, área do hiato esofageano, e tempo de internação pós-operatória. Setenta por cento dos pacientes possuíam hérnias de hiato complexas (gigantes ou recidivadas). Catorze complicações foram observadas: 7 pleuro pulmonares e 7 necessitando reoperação, sendo destas 3 recidivas, 2 fístulas digestivas, 1 hérnia diafragmática e 1 hérnia incisional. As complicações foram semelhantes em ambos os grupos, e a hérnia de hiato tipo IV foi o único preditor independente. CONCLUSÕES: O índice de massa corpórea não influencia nos resultados perioperatórios e a hérnia de hiato tipo IV é o único preditor independente de complicações.

ABSTRACT Hiatal hernias are at high risk of recurrence. Mesh reinforcement after primary approximation of the hiatal crura has been advocated to reduce this risk of recurrence, analogous to mesh repair of abdominal wall hernias. However, the results of such repairs have been mixed, at best. In addition, repairs using some type of mesh have led to significant complications, such as erosion and esophageal stricture. At present, there is no consensus as to (1) whether mesh should be used, (2) indications for use, (3) the type of mesh, and (4) in what configuration. This lack of consensus is likely secondary to the notion that recurrence occurs at the site of crural approximation. We have explored the theory that many, if not most, "recurrences" occur in the anterior and left lateral aspects of the hiatus, normally where the mesh is not placed. We theorized that "recurrence" actually represents progression of the hernia, rather than a true recurrence. This has led to our development of a new mesh configuration to enhance the tensile strength of the hiatus and counteract continued stresses from intra-abdominal pressure.


RESUMO As hérnias hiatais têm alto risco de recidiva. O reforço com tela após a aproximação primária dos pilares hiatais tem sido defendido, para reduzir esse risco de recidiva, análogo ao reparo com tela de hérnias da parede abdominal. No entanto, os resultados de tais reparos foram variados, na melhor das hipóteses. Além disso, os reparos com algum tipo de tela levaram a complicações importantes, como erosão e estenose esofágica. Atualmente, não há consenso sobre 1) se a tela deve ser usada, 2) indicações para uso, 3) o tipo de tela e 4) em qual configuração. Essa falta de consenso provavelmente é secundária à noção de que a recidiva ocorre no local da aproximação crural. Exploramos a teoria de que muitas, se não a maioria, das recidivas ocorrem nas faces anterior e lateral esquerda do hiato, normalmente onde a tela não é colocada. Nós teorizamos que a "recidiva" na verdade representa a progressão da hérnia, em vez de uma verdadeira recidiva, levando ao desenvolvimento de uma nova configuração da tela, para aumentar a resistência à tração do hiato e neutralizar as tensões contínuas da pressão intra-abdominal.

Introducción: el reflujo laringofaríngeo (RLF) se origina por el flujo retrógrado de contenido gástrico hacia la faringe, pero es una entidad diferente de la enfermedad por reflujo gastroesofágico (ERGE). El objetivo del estudio fue determinar la correlación entre los signos endoscópicos de la fibrolaringoscopia y la videoendoscopia digestiva alta (VEDA). Material y métodos: estudio observacional, retrospectivo y analítico. Se incluyeron pacientes que consultaron por sintomatología de RLF y ERGE. Los hallazgos visualizados por fibrolaringoscopia flexible, VEDA y biopsia de mucosa gástrica de cada paciente se compararon con la prueba de chi-cuadrado (χ²). Se consideró significativo un valor de p ≤ 0,05. Resultados: se incluyeron 318 pacientes entre 18 y 84 años. Se encontró que el 100 % de los pacientes con esofagitis tenía laringitis (p = 0,001); el 100 % de los pacientes con hernia hiatal tenían RLF (p = 0,001); el 97 % de los pacientes con Helicobacter pylori en la mucosa gástrica tenían RLF (p = 0.001). El 71 % de los pacientes con hernia hiatal tenían esofagitis (p = 0,001). Se encontró una asociación lineal entre la edad y la hernia hiatal con la edad y el RLF (p = 0,03). Conclusiones: en este estudio, los signos encontrados en la fibrolaringoscopia tuvieron una asociación estadística con la VEDA. Aproximadamente el 90 % de los pacientes con signos de laringitis tuvo una correlación con algún grado de esofagitis, esófago de Barrett, hernia hiatal y Helicobacter pylori. También se encontró que la laringitis por RLF y la hernia hiatal se relacionaron directamente con el incremento de la edad.


Introduction: Laryngopharyngeal reflux (LPR) is caused by the retrograde flow of gastric contents towards the pharynx, but it is a different entity from gastroesophageal reflux disease (GERD). The objective of the study was to determine the relationship between the endoscopic signs of fiber laryngoscopy and upper gastrointestinal video endoscopy. Material and methods: Observational, retrospective and analytical study. Patients who consulted for LPR and GERD symptoms were included. The findings visualized by flexible fiber laryngoscopy, upper gastrointestinal video endoscopy and gastric mucosal biopsy of each patient were compared with the Chi-square (χ²) test. A value of p ≤ 0.05 is estimated significantly. Results: 318 patients between 18 and 84 years old were included. A relationship was found in 100% of the patients with esophagitis had laryngitis (p = 0.001); 100% of the patients with hiatal hernia had LPR (p = 0.001); 97% of patients with Helicobacter pylori in the gastric mucosal have LPR (p= 0.001); 71% of patients with hiatal hernia had esophagitis (p = 0.001). A linear association was found between age and hiatal hernia with age and LPR. (p = 0.03). Conclusions: In this study, the signs found in fiber laryngoscopy had a statistical association with the upper gastrointestinal video endoscopy. Approximately 90% of patients with signs of laryngitis had correlation with some degree of esophagitis, Barrett's esophagus, hiatal hernia and Helicobacter pylori. It was also found that LPR and hiatal hernia were directly related to increasing age

Introducción. En las hernias paraesofágicas tipo IV se produce la herniación del estómago junto a otros órganos abdominales. La herniación del páncreas es muy infrecuente.Caso clínico. Varón de 57 años que acude por dolor torácico, disnea e intolerancia al decúbito. En la tomografía computarizada toracoabdominal se observa hernia diafragmática que contiene colon transverso, intestino delgado y páncreas, con reticulación de la grasa alrededor del mismo, compatible con pancreatitis aguda. Conclusión. La asociación de hernia hiatal con páncreas herniado y pancreatitis es extremadamente infrecuente. El diagnóstico se estableció mediante tomografía computarizada y el tratamiento fue conservador, con cirugía diferida de la hernia de hiato.


Introduction. In type IV paraesophageal hernias, the stomach is herniated along with other abdominal organs. Herniation of the pancreas is very rare. Clinical case. A 57-year-old man presented with chest pain, dyspnea, and intolerance to decubitus. The thoracoabdominal computed tomography shows a diaphragmatic hernia containing the transverse colon, small intestine and pancreas, with reticulation of fat around it, compatible with acute pancreatitis. Conclusion. The association of hiatal hernia with herniated pancreas and pancreatitis is extremely rare. The diagnosis was established by computerized tomography and the treatment was conservative, with delayed surgery for the hiatal hernia.

Resumen El reparo de la hernia hiatal es un tema de debate debido a las posibles complicaciones asociadas que han cambiado a través de los años. En la literatura se reportan complicaciones asociadas al procedimiento hasta en un 30 % de los casos. Las complicaciones diferentes a la recurrencia y a largo plazo son infrecuentes, reportadas en menos del 9 % de los casos. La inclusión de la malla protésica en el esófago es una rara complicación y solo se han reportado pocos casos sobre esta. Entre los factores asociados a este desenlace se encuentran descritos: el material protésico, la técnica quirúrgica y la tensión de la malla sobre el tejido intervenido; sin embargo, es difícil establecer asociaciones directas de cada factor dado que la literatura actual solo cuenta con reportes de casos. A continuación, se muestra el caso clínico de un paciente, quien, después de una reparación de hernia hiatal con malla, presenta la inclusión de material protésico en el esófago; se aborda el diagnóstico y el manejo de la misma.


Abstract Hiatal hernia repair has been a subject of debate due to the possible associated complications that have changed over the years. The literature reports up to 30% of cases with complications associated with the procedure. Complications other than recurrence and long-term complications are rare and reported in less than 9% of cases. The migration of the prosthetic mesh into the esophagus is a rare complication and only a few cases have been reported. The factors associated with this outcome include prosthetic material, surgical technique, and mesh tension on the intervened tissue. However, it is difficult to establish direct associations of each factor since the current literature has only case reports. The following is a clinical case of a patient in whom the prosthetic material migrated into the esophagus after a hiatal hernia repair with mesh. The diagnosis and treatment offered are discussed.

La hernia hiatal es una entidad relativamente frecuente y puede ser un hallazgo incidental en un estudio ecocardiográfico. Describimos el caso de una paciente con diagnóstico de masa en la aurícula izquierda (AI) sin clara etiología, a quien se le realiza el diagnóstico de hernia hiatal por resonancia magnética cardíaca (RMC) y luego se demuestra mediante ecocardiografía de contraste el contenido gástrico de la masa tras la ingestión de una bebida carbonatada, lo que permite de forma rápida y sencilla aclarar el diagnóstico.


Hiatal hernia is a relatively common entity, and may be an incidental finding in an echocardiographic study. We describe the case of a patient with a diagnosis of a mass in the left atrium with no clear etiology, in whom the diagnosis of hiatal hernia is made by cardiac magnetic resonance imaging and then demonstrated by contrast echocardiography the gastric content of the mass after the ingestion of a carbonated drink, which allows quickly and easily to clarify the diagnosis.


A hérnia de hiato é uma entidade relativamente comum, e pode ser um achado incidental em um estudo ecocardiográfico. Descrevemos o caso de um paciente com diagnóstico de massa em átrio esquerdo sem etiologia definida, em que o diagnóstico de hérnia de hiato é feito por ressonância magnética cardíaca e posteriormente demonstrado por ecocardiografia contrastada com uma bebida gaseificada, permitindo esclarecer de forma rápida e fácil o diagnóstico.

Resumen Introducción: La hernia hiatal (HH) de tipo I por deslizamiento es el tipo más frecuente, siendo difícil de definir objetivamente, por lo que el principal foco de controversia es su diagnóstico. El objetivo del presente trabajo es reportar los resultados respecto de la precisión diagnóstica del estudio preoperatorio y confirmarlo con el diagnóstico laparoscópico de este tipo de HH. Materiales y Método: Estudio prospectivo descriptivo de serie que incluyen pacientes con síntomas típicos de enfermedad por reflujo gastroesofágico, los cuales se sometieron a estudio con esófago-gastro-duodenoscopía, estudio manométrico y radiológico de esófago, estómago y duodeno con bario. Se incluyen sólo los pacientes en los cuales la endoscopía revela la existencia de HH por deslizamiento ≪ 5 cm. Estos pacientes se sometieron a tratamiento quirúrgico confirmándose o no la existencia de HH al momento de la exploración laparoscópica. Resultados: El valor predictivo positivo y sensibilidad para manometría fue de un 51,2% y 70%, para la radiología 91,7% y 80,5% y para endoscopia 80,3% y 70,7% respectivamente. Conclusión: Para el diagnóstico confiable de HH antes del tratamiento, las tres investigaciones mencionadas deben ser obligatorias antes de la cirugía.


Introduction: Being type I hiatal hernia (HH) the most frequent, is difficult to define objectively and therefore, the main focus of controversy is the diagnosis. The aim of this paper is to report the results regarding the diagnostic accuracy of the preoperative study and to confirm it with the laparoscopic diagnosis of hiatal hernia. Materials and Method: This descriptive and prospective study includes patients with typical symptoms of gastroesophageal reflux disease who underwent esophageal-gastro-duodenoscopy, manometry and radiological study of esophagus with barium swallow. Only patients in whom endoscopy reveals the existence of HH by sliding ≪ 5 cm are included. These patients underwent surgical treatment confirming or not the existence of HH at the time of laparoscopic exploration. Results: The positive pre- dictive value and sensibility for manometry was 51.2% and 70%, for radiology 91.7% and 80.5%, and for endoscopy 85.3% and 70.7% respectively. Conclusion: For the reliable diagnosis of HH before treatment, the three mentioned investigations must be mandatory before the surgery.

Resumen El hipo crónico es un síntoma que puede provocar una invalidez significativa y a menudo revela una enfermedad subyacente. A continuación, se presenta el caso de un varón de 68 años que ingresó con hipo de más de 3 meses de duración que se asociaba con epigastralgia, vómitos posprandiales y pérdida ponderal. Había sido intervenido en 2 ocasiones debido a una enfermedad por reflujo gastroesofágico y hernia hiatal, una primera en la que se realizó una fundoplicatura y, posteriormente, una reintervención consistente en el cierre de los pilares diafragmáticos y re-Nissen laparoscópico. La clínica se debía a una obstrucción hiatal por acodamiento de la fundoplicatura previa y fue resuelta mediante la reposición hiatal a los parámetros anatómicos y desmontaje del Nissen previo.


Abstract Chronic hiccups is a rare symptom that can lead to significant disability and often reveals an underlying disease. The following is the case of a 68-year-old man who was admitted due to hiccups that had lasted more than 3 months associated with epigastric pain, postprandial vomiting, and weight loss. He had undergone surgery twice due to gastroesophageal reflux disease and hiatal hernia. During the first procedure, a fundoplication was performed, and then, he underwent a reoperation consisting of diaphragmatic pillars closure and laparoscopic Nissen. The symptoms were caused by a hiatal obstruction due to the kinking of the previous fundoplication and were resolved by repositioning the hiatus to anatomical parameters and dismantling the previous Nissen.

Reconstruction of the digestive tract is a surgical challenge; we propose a novel and successful alternative using a large vascularized pedicled jejunum anastomosed to the cervical esophagus in an emergency situation. A 65-year-old female patient in follow-up due to a gigantic type IV paraesophageal hernia, whom underwent surgical hernial defect correction, had friable gastric tissue and esophageal lesions that forced an urgent esophagojejunostomy. Immediate post-operative recovery had no incidents and during the outpatient follow-up no complications were reported. This case report represents a paradigm change in the impossibility of raising the vascularized pedicled jejunum to the neck in an emergency situation.


La reconstrucción del tracto digestivo es un reto quirúrgico. Proponemos una alternativa novedosa y exitosa utilizando un gran yeyuno pediculado vascularizado anastomosado al esófago cervical en situación de emergencia. Una paciente de 65 años en seguimiento por una hernia paraesofágica tipo IV requirió corrección quirúrgica del defecto herniario; tenía tejido gástrico friable y lesiones esofágicas que forzaron una esofagoyeyunostomía urgente. La recuperación posoperatoria inmediata no tuvo incidentes y el seguimiento ambulatorio no mostró complicaciones. Este reporte de caso rompe el paradigma de la imposibilidad de ascender el yeyuno pediculado vascularizado al cuello en una situación de emergencia.

RESUMEN Fundamento: la hernia hiatal es la anormalidad más frecuente del tracto digestivo superior, se clasifica en cuatro tipos, estas hernias pueden ser por deslizamiento y paraesofágicas, estas últimas de presentación mucho menos frecuente en la práctica clínica como es el caso que se presentó tipo IV; la mayoría son asintomáticas son descubiertas incidentalmente en radiografías de tórax o en tomografía computarizada multicorte y cuando son sintomáticas muchas veces están asociadas a complicación. Objetivo: presentar un paciente con hernia hiatal tipo IV diagnosticada por tomografía computarizada multicorte. Presentación del caso: paciente masculino, de 58 años de edad, con antecedentes de hipertensión arterial, que refiere presentar una hernia diafragmática diagnosticada hace 15 años, que presenta dolor abdominal a tipo cólico de moderada intensidad acompañado de distensión abdominal eructos, vómitos y disnea. Al examen físico, abdomen globuloso que sigue los movimientos respiratorios, doloroso a la palpación superficial y profunda. En el tórax se auscultan ruidos hidroaéreos en el lado derecho. La tomografía computarizada multicorte demuestra la herniación de estómago, colon transverso y epiplón en hemitórax derecho a través del hiato esofágico. Conclusiones: las hernias hiatales son las anomalías más frecuentes del tracto digestivo superior pero las gigantes tipo IV con herniación de otras estructuras abdominales, como colon, omento, intestino delgado, hígado y páncreas son una forma infrecuente de presentación de las mismas. La tomografía computarizada multicorte y el estudio baritado de esófago-estómago-duodeno, resultan fundamentales para el diagnóstico y caracterización de estas lesiones.


ABSTRACT Background: the hiatal hernia is the most frequent abnormality of the upper digestive tract, which are classified into four types, these hernias can be by sliding and paraesophageal, the latter being much less frequent presentation in clinical practice as is the case presented type IV; most of them are asymptomatic and are discovered incidentally on chest x-rays or multi-slice CT and when they are symptomatic they are often associated with complications. Objective: to describe a patient with hiatal hernia type IV diagnosed by multi-slice CT. Case report: a 58-year-old male patient with a history of arterial hypertension, who reported having a diaphragmatic hernia diagnosed 15 years ago, who presents with abdominal pain with a moderate intensity of colic accompanied by abdominal distension, belching, vomiting, and dyspnea. At physical examination, globular abdomen that follows the respiratory movements, painful to superficial and deep palpation. Hydro-aerial noises are heard in the right side of the thorax. Multi-slice CT shows herniation of the stomach, transverse colon and omentum in the right chest cavity through the esophageal hiatus. Conclusions: hiatal hernias are the most frequent anomalies of the upper digestive tract but type IV giants with herniation of other abdominal structures, such as colon, omentum, small intestine, liver and pancreas are an infrequent form of presentation of them. Multi-slice CT and barium study of the esophagus, stomach and duodenum are fundamental for the diagnosis and characterization of these lesions.

Introducción. La reparación laparoscópica es el estándar de tratamiento en hernia hiatal gigante. Sin embargo, a pesar de su baja morbilidad, la tasa de recurrencia sigue siendo alta. Nuestro objetivo fue describir los resultados de la reparación laparoscópica de hernia hiatal gigante, independientemente de la técnica de cruroplastia empleada.Métodos. Se llevó a cabo un estudio retrospectivo de pacientes llevados a reparación laparoscópica de hernia hiatal gigante en el periodo 2009-2017. Se analizaron los datos demográficos, la técnica quirúrgica, las complicaciones y la estancia hospitalaria. Se revisaron los resultados de la endoscopia, la radiografía de vías digestivas altas y la escala de síntomas GERD-HRQOL, obtenidos luego de un año de cirugía. Resultados. Se incluyeron 44 pacientes con un tamaño promedio de la hernia de 7 cm. Se practicó cruroplastia con sutura simple en 36,4 %, sutura más refuerzo con politetrafluoroetileno (PTFE) o dacrón, en 59,1 %, y se usó malla en 4,5 %. Hubo 12 complicaciones, la estancia hospitalaria promedio fue de 3,5 días y no hubo mortalidad. Se encontró recurrencia endoscópica o radiológica en 6/20 pacientes, todas pequeñas y asintomáticas. En 23 pacientes, la escala GERD-HRQOL reportó un valor promedio de 7,7 y 78 % de satisfacción. Solo un paciente requirió cirugía de revisión. Conclusión. El método preferido de reparación laparoscópica de la hernia hiatal gigante es la cruroplastia sin malla, técnica asociada a baja morbilidad y adecuado control de los síntomas. La tasa de recurrencia es similar a la reportada en la literatura. Se requieren estudios prospectivos con seguimiento completo a largo plazo para validar estos resultados


Introduction: Laparoscopic repair is the standard treatment for giant hiatal hernia. However, despite its low morbidity, recurrence rate remains high. Our goal was to describe the results of laparoscopic repair of giant hiatal hernia, regardless of the technique used for cruroplasty.Methods: A retrospective study of patients undergoing laparoscopic repair of giant hiatal hernia was carried out from 2009 to 2017. Demographic data, surgical technique, complications, and hospital stay were analyzed. The results of endoscopy, radiography of upper digestive tract, and GERD-HRQOL symptoms scale, obtained after one year of surgery, were reviewed.Results: Forty-four patients with an average hernia size of 7 cm were included. Simple suture cruroplasty was performed in 36.4%, suture plus reinforcement with polytetrafluoroethylene (PTFE) or Dacron in 59.1%, and mesh repair in 4.5%. There were 12 complications, the average hospital stay was 3.5 days, and there was no mortality. Endoscopic or radiological recurrence was found in 6/20 patients, all small and asymptomatic. In 23 patients, the GERD-HRQOL scale reported an average value of 7.7 and 78% patient satisfaction. Only one patient required revision surgery.Conclusion: The preferred method of laparoscopic repair of giant hiatal hernia is meshless cruroplasty, a technique associated with low morbidity and adequate symptom control. The recurrence rate is similar to that reported in the literature. Prospective studies with long-term follow-up are required to validate these results

ABSTRACT BACKGROUND: The gastroesophageal reflux disease (GERD) is the most common esophageal disease in medical practice, and it is suspected according to patients' symptoms. GERD can be classified in erosive esophagitis (EE) according to the presence of upper gastrointestinal endoscopy findings. OBJECTIVE: To evaluate endoscopic findings in patients with symptoms suggestive of GERD comparing epicemiological and risk factors. METHODS: Upper endoscopy reports were examined retrospectively from patients with symptoms of GERD such as heartburn, regurgitation, cough, throat clearing, globus and chest pain. EE was determined based on Los Angeles classification. Comparisons between risk factors in EE and non-EE groups were done with statistical analysis. RESULTS: A total of 984 endoscopic reports were examined and 676 selected for analysis (281 with EE and 395 with non-EE form). Most were female 381 (56.36%) with a mean age of 44.01±15.40 years. Hiatal hernia was present in 47(6.96%) and smoking in 41(6.07%). Univariate logistic regression showed that male (OR=2.24, CI 95%, 1.63-3.06) and hiatal hernia (OR=4.52, CI 95%, 2.30-8.89) were independent predictors of erosions in the EE group. The presence of hiatal hernia (OR=12.04, CI 95%, 3.57-40.62), smoking (OR=8.46, CI 95%, 3.28-31.32) and aged patients (OR=8.01, CI 95%, 2.42-26.49) were also indicated as a risk factor for severe EE (grades C and D of Los Angeles). CONCLUSION Male gender and hiatal hernia were associated with EE. Aged patients, smoking and hiatal hernia were related to severe EE. It is suggested that the risk factors for EE and non-EE types are different. Cohort studies are necessary to identify the exact mechanisms involved in each disease form.


RESUMO CONTEXTO: A doença do refluxo gastroesofágico (DRGE) é uma das doenças digestivas mais comuns na prática médica e deve ser suspeitada de acordo com os seus sintomas clínicos, podendo ser classificada em esofagite erosiva (EE) de acordo com os achados de endoscopia. OBJETIVO: Avaliar os achados endoscópicos em pacientes com sintomas sugestivos de DGRE comparando fatores de risco e epidemiológicos. MÉTODOS: Resultados de endoscopias digestiva foram examinados retrospectivamente de pacientes com sintomas relacionados com DRGE como pirose, regurgitação, tosse, pigarro, globus e dor torácica. EE foi determinada de acordo com a classificação de Los Angeles. Comparação de fatores de risco entre os grupos EE e não-EE foram feitos com análise estatística. RESULTADOS: Um total de 984 endoscopias foram examinadas e 676 endoscopias selecionadas para análise (281 com EE e 395 sem EE). A maioria dos pacientes era do sexo feminino 381 (56,36%) com uma idade média de 44,01±15,40 anos. Hérnia hiatal esteve presente em 47 (6,96%) e tabagismo em 41 (6,07%). Regressão logística uni variada mostrou que sexo masculino (OR=2,24 - IC 95%: 1,63-3,06) e hérnia hiatal (OR=4,52 - CI 95%: 2,30-8,89) foram fatores de risco independentes de EE. A presença de hérnia hiatal (OR=12,04 - CI 95%: 3,57-40,62), tabagismo (OR=8,46 - CI 95%: 3,28-31,32) e pacientes idosos (OR=8,01 - CI 95%, 2,42-26,49) foram fatores de risco no grupo de EE grave (classes C e D de Los Angeles). CONCLUSÃO: Sexo masculino e hérnia hiatal foram associados com EE. Idade avançada, tabagismo e hérnia hiatal foram relacionados à forma grave de EE. É sugerido que os fatores de risco de pacientes com e sem EE sejam diferentes. Estudos de coorte são necessários para identificar os mecanismos exatos envolvidos em cada forma da doença.

El esófago corto es una complicación infrecuente del reflujo gastroesofágico, polémico para algunos autores. Su diagnóstico se hace de manera intraoperatoria, ante la imposibilidad de conservar un esófago intrabdominal mayor a tres centímetros desde el hiato diafragmático. Algunos expertos niegan la existencia de esta entidad, argumentando que se debe a una mala disección del esófago. En este artículo, se presenta el caso de un paciente sometido a herniorrafia hiatal con colocación de malla, con gran dificultad para conservar una adecuada longitud del esófago abdominal, y que no requirió gastroplastia (AU)


Short esophagus is an uncommon complication of gastroesophageal reflux, controversial for some authors. The diagnosis is made intraoperatively when , the impossibility of leaving an intra-abdominal esophagus longer than 3 cm becomes patent. Some experts, however, doubt about the existence of this entity, arguing that it is due to a bad dissection of the esophagus. In this article, we present the case of a patient where a hiatal herniorraphy was performed with great difficulty to conserve an adequate length of the abdominal esophagus, without requiring gastroplasty (AU)

Resumen Se presenta un caso de hernia paraesofágica en una mujer con diagnóstico inicial de enfermedad ácidopéptica. El diagnóstico preciso requirió radiografía de tórax, radiografía de vías digestivas altas y tomografia axial computarizada. Fue tratada mediante abordaje por videolaparascopía; seis meses después estaba asintomática y sin complicaciones. El médico general debe considerar la probabilidad de hernia paraesofágica en pacientes añosos con síntomas del tracto digestivo superior inexplicados, especialmente cuando los síntomas son crónicos y sin respuesta adecuada al tratamiento con inhibidores de la bomba de protones.


Abstract A case of paraesophageal hernia in a woman with an initial diagnosis of peptic acid disorders. Precise diagnosis required chest radiography, upper gastrointestinal tract x-ray and computed tomography. She was treated by videolaparoscopic approach; six months later she was asymptomatic and suffered no complications. The General Practitioner should consider the likelihood of paraesophageal hernia in elderly patients with unexplained upper digestive tract symptoms, especially when the symptoms are chronic and unresponsive to treatment with proton pump inhibitors.

This case study describes a 71-year-old man with signet-ring cell gastric adenocarcinoma and malignant sigmoidal polyp; and typical features of Saint's triad and Heyde syndrome. He had digestive bleeding, two types of hernia, diverticulosis, arterial hypertension, malignant polyp, and antecedent of smoking, lung tuberculosis, and surgical correction of aortic valve stenosis. There is a hypothetical inverse relationship between herniosis and development of malignancy; however, the patient herein described presented gastric and sigmoidal cancers. Gastrointestinal malignancies are sometimes associated with paraneoplastic entities, isolated or manifested as syndromes, but neither Saint's triad or Heyde syndrome have been included. This patient persisted clinically stable during the preoperative period, but suddenly died; Trousseau's syndrome would be the most probable mechanism of sudden death in this setting. Case reports can stimulate further studies to get additional knowledge about unusual entities.


Este estudio de caso describe un hombre de 71 años de edad, con adenocarcinoma gástrico con células en anillo de sello y un pólipo maligno sigmoideo; y características típicas de la tríada de Saint y del síndrome de Heyde. Tuvo una hemorragia digestiva, dos tipos de hernias, divertículos, hipertensión arterial, y pólipo maligno; con antecedente de tabaquismo, tuberculosis pulmonar, y corrección quirúrgica de estenosis de la válvula aórtica. Hay una hipotética relación inversa entre hernioses y el desarrollo de malignidades; sin embargo, el paciente que se describe en el presente documento presentó cánceres gástrico y sigmoideo. Neoplasias gastrointestinales se asocian a veces con entidades para neoplásicas aisladas o manifiestan síndromes, pero ni la tríada de Saint ni el síndrome de Heyde se ha incluido. Este paciente persistió clínicamente estable durante el período preoperatorio, pero de repente murió; síndrome de Trousseau sería el mecanismo más probable de muerte súbita en esta situación. Los informes de casos pueden estimular más estudios para obtener un conocimiento adicional sobre esas entidades inusuales.

Objective: To evaluate salivary microbial flora of patients with hiatal hernia and compare it with that of healthy subjects. Material and Methods: In this cross-sectional study, 50 patients with hiatal hernia measuring >1 cm and 50 healthy subjects (as the controls) were selected using simple random technique. One mL of salivary sample was taken from each patient, transferred into 50-mL Falcon tubes and immediately carried to the microbiology Laboratory of Tabriz Faculty of Medicine. The salivary samples were cultured on specific Streptococcus viridans (S. mitis, S. mutans, S. salivarius and S. sanguis), Enterococcus spp. and Lactobacillus culture media. Then the samples were incubated at 37°C for 7 days, followed by evaluation of the bacterial colonies. Statistical significance was defined at p<0.05. Results: A total of 34% of subjects with hiatal hernia and 26% healthy subjects exhibited Lactobacillus gasseri in their salivary samples; 16% of subjects with hernia and 6% of healthy subjects exhibited Enterococci spp. in their salivary samples. In addition, 82% of subjects with hernia and 72% of healthy subjects exhibited S. mutans in their salivary samples; 74% and 4% of subjects with hernia and 76% and 4% of healthy subjects exhibited gram-positive and gram-negative bacilli in their salivary samples, respectively. Furthermore, 98% of subjects with hernia and 86% of healthy subjects exhibited gram-positive cocci in their salivary samples, however without significant difference between the groups (p>0.05). Conclusion: No significant differences in the counts of Lactobacillus spp., Enterococcus spp., Streptococcus viridans and gram-positive and gram-negative bacterial species between healthy controls and subjects with hiatal hernia.
