Abstract Introduction: Upper endoscopy is the standard method for diagnosing early-stage gastric cancer. However, according to estimates, up to 20% of tumors are not detected, and their accuracy may be affected by the variability in their performance. In Colombia, most diagnoses take place in advanced stages, which aggravates the problem. Protocols have been proposed to ensure the complete observation of areas prone to premalignant lesions to address variability. Objective: To build and validate an automatic audit system for endoscopies using artificial intelligence techniques. Methodology: In this study, 96 patients from a teaching hospital underwent video-documented endoscopies, spanning 22 stations rearranged to minimize overlaps and improve the identification of 13 key gastric regions. An advanced convolutional network was used to process the images, extracting visual characteristics, which facilitated the training of artificial intelligence in the classification of these areas. Results: the model, called Gastro UNAL, was trained and validated with images of 67 patients (70% of cases) and tested with 29 different patients (30% of cases), which reached an average sensitivity of 85,5% and a specificity of 98,8% in detecting the 13 gastric regions. Conclusions: The effectiveness of the model suggests its potential to ensure the quality and accuracy of endoscopies. This approach could confirm the regions evaluated, alerting less experienced or trained endoscopists about blind spots in the examinations, thus, increasing the quality of these procedures.


Resumen Introducción: La endoscopia digestiva alta es el método estándar para diagnosticar el cáncer gástrico en etapas tempranas. Sin embargo, su precisión puede verse afectada por la variabilidad en su realización, y se estiman hasta 20% de tumores no detectados. En Colombia, la mayoría de los diagnósticos se realizan en etapas avanzadas, lo que agrava el problema. Para abordar la variabilidad, se han propuesto protocolos con el fin de asegurar la observación completa de áreas propensas a lesiones premalignas. Objetivo: Construir y validar un sistema de auditoría automática para endoscopias usando técnicas de inteligencia artificial. Metodología: En este estudio, 96 pacientes de un hospital universitario se sometieron a endoscopias documentadas en video, abarcando 22 estaciones reorganizadas para minimizar solapamientos y mejorar la identificación de 13 regiones gástricas clave. Se utilizó una red convolucional avanzada para procesar las imágenes, extrayendo características visuales, lo que facilitó el entrenamiento de la inteligencia artificial en la clasificación de estas áreas. Resultados: El modelo, llamado Gastro UNAL, fue entrenado y validado con imágenes de 67 pacientes (70% de los casos) y probado con 29 pacientes distintos (30% de los casos), con lo que alcanzó una sensibilidad promedio del 85,5% y una especificidad del 98,8% en la detección de las 13 regiones gástricas. Conclusiones: La eficacia del modelo sugiere su potencial para asegurar la calidad y precisión de las endoscopias. Este enfoque podría confirmar las regiones evaluadas, alertando puntos ciegos en la exploración a los endoscopistas con menos experiencia o en entrenamiento, de tal forma que se aumente la calidad de estos procedimientos.

Abstract Esophagogastroduodenoscopy (EGD) is a diagnostic and therapeutic study for various upper gastrointestinal tract diseases. It is a fundamental part of the education and training fellows receive in gastroenterology programs, focusing on developing skills to perform it safely, effectively, and efficiently and master the technique. Historically, value has been attached to the number of procedures required to acquire skills; there is a discrepancy in the recommendations by scientific societies regarding the "ideal" number to meet the minimum requirements and learn the necessary skills. However, each student has different innate and developed abilities that make this process asymmetric. Adopting a combined training approach (observation, clinical practice, and simulation) is the best method to learn diagnostic EGD. We are facing a generation with forms of learning different from those traditionally implemented, which implies an ongoing process of training and updating for endoscopists dedicated to teaching.


Resumen La esofagogastroduodenoscopia (EGD) es un estudio diagnóstico y terapéutico para diferentes enfermedades del tracto gastrointestinal superior, que hace parte fundamental de la formación y el entrenamiento que reciben los fellows en los programas de gastroenterología, y se centra en el desarrollo de capacidades para llevarla a cabo de manera segura, eficaz, eficiente y con un claro dominio de la técnica. Históricamente, se le ha dado un valor fundamental a la cantidad de procedimientos requeridos para la adquisición de las habilidades y existe discrepancia en la recomendación dada por las diferentes sociedades científicas respecto a la cantidad "ideal" para cumplir con las exigencias mínimas y lograr las destrezas necesarias; sin embargo, cada estudiante tiene unas habilidades innatas y desarrolladas diferentes que hacen que ese proceso sea asimétrico en cada caso. La adopción de un enfoque de entrenamiento combinado (observación, práctica clínica y simulación) es el mejor método para aprender EGD diagnóstica, dado que estamos frente a una generación que tiene formas de aprendizaje diferentes a las implementadas tradicionalmente, lo cual implica un proceso de entrenamiento y actualización constante de los endoscopistas que se dedican a la docencia.

STUDY OBJECTIVE: Semaglutide is a long-acting glucagon-like peptide-1 receptor agonist used for management of type 2 diabetes and/or obesity. To test the hypothesis that perioperative semaglutide use is associated with delayed gastric emptying and increased residual gastric content (RGC) despite adequate preoperative fasting, we compared the RGC of patients who had and had not taken semaglutide prior to elective esophagogastroduodenoscopy. The primary outcome was the presence of increased RGC. DESIGN: Single-center retrospective electronic chart review. SETTING: Tertiary hospital. PATIENTS: Patients undergoing esophagogastroduodenoscopy under deep sedation/general anesthesia between July/2021-March/2022. INTERVENTIONS: Patients were divided into two (SG = semaglutide, NSG = non-semaglutide) groups, according to whether they had received semaglutide within 30 days prior to the esophagogastroduodenoscopy. MEASUREMENTS: Increased RGC was defined as any amount of solid content, or > 0.8 mL/Kg (measured from the aspiration/suction canister) of fluid content. MAIN RESULTS: Of the 886 esophagogastroduodenoscopies performed, 404 (33 in the SG and 371 in the NSG) were included in the final analysis. Increased RGC was observed in 27 (6.7%) patients, being 8 (24.2%) in the SG and 19 (5.1%) in the NSG (p < 0.001). Semaglutide use [5.15 (95%CI 1.92-12.92)] and the presence of preoperative digestive symptoms (nausea/vomiting, dyspepsia, abdominal distension) [3.56 (95%CI 2.2-5.78)] were associated with increased RGC in the propensity weighted analysis. Conversely, a protective [0.25 (95%CI 0.16-0.39)] effect against increased RGC was observed in patients undergoing esophagogastroduodenoscopy combined with colonoscopy. In the SG, the mean time of preoperative semaglutide interruption in patients with and without increased RGC was 10.5 ± 5.5 and 10.2 ± 5.6 days, respectively (p = 0.54). There was no relationship between semaglutide use and the amount/volume of RGC found on esophagogastroduodenoscopy (p = 0.99). Only one case (in the SG) of pulmonary aspiration was reported. CONCLUSIONS: Semaglutide was associated with increased RGC in patients undergoing elective esophagogastroduodenoscopy. Digestive symptoms prior to esophagogastroduodenoscopy were also predictive of increased RGC.

Cirrhosis is the end-stage of chronic liver disease and constitutes a leading cause of potential years of working life lost, especially in the Americas and Europe. Its natural history is characterized by an asymptomatic phase called compensated cirrhosis, followed by a rapidly progressive phase characterized by liver-related complications termed decompensated cirrhosis. Complications could be related to portal hypertension and/or liver dysfunction, including ascites, portal hypertensive gastrointestinal bleeding, encephalopathy, and jaundice. This review will discuss some of the most important precipitants of hepatic decompensation, including acute variceal bleeding, spontaneous bacterial peritonitis, and hepatic encephalopathy.

INTRODUCTION AND AIMS: Endoscopy is the most effective method for identifying gastric adenocarcinoma (GAC). Interval gastric cancer (IGC) is GAC that is diagnosed 2-3 years after a normal endoscopy. Its characteristics are unknown in the Colombian environment. The clinical, histopathologic, and endoscopic characteristics were evaluated, along with the presentation rate, proton pump inhibitor (PPI) use, and IGC survival rate, and compared with other types of GAC. METHODS: A retrospective, analytic study was conducted on a prospective cohort. It evaluated 513 patients with GAC treated at our institution, within the time frame of January 2012 and June 2018. The patients had endoscopic diagnosis of GAC and endoscopy within the past three years that was negative for tumor. RESULTS: A total of 513 patients diagnosed with GAC were evaluated. Forty-two of the patients had IGC (8.2%): 9 early lesions and 33 advanced lesions (79%). The IGCs were smaller (31 vs. 41 mm; P < .01), as well as flatter and more depressed (P < .01). There was no association with PPI use, but there was an association with a history of gastrectomy and anastomosis (P = .02), as well as the absence of red flags (P < .003). The most frequent locations were the gastric body (52%) and the antrum (26%). Overall two-year survival was similar between IGC and GAC (37.1 vs. 39.3%, P = .72). CONCLUSION: A total of 8.2% of recently diagnosed GAC were cases of IGC. The presence of anastomosis and the absence of red flags were related to IGC. Overall survival was poor and there were no differences from the other types of GAC detected.

Resumen Los parámetros de calidad para endoscopia digestiva alta han introducido indicadores intraprocedimiento, dentro de los cuales la adecuada visibilidad de la mucosa, libre de saliva, moco o burbujas, puede aumentar la posibilidad de detección de lesiones en fase temprana. Sin embargo, el uso de mucolíticos y antiburbujas ha mostrado gran variabilidad de eficiencia según las soluciones, concentraciones, tiempos de exposición y escala de visibilidad aplicados. Objetivos: determinar la efectividad de diferentes soluciones de premedicación para la limpieza de la mucosa digestiva; validar, mediante una prueba de concordancia interobservador, una nueva escala de adecuada visualización de la mucosa (TVMS) para el esófago, estómago y duodeno; y reportar eventos adversos o complicaciones relacionadas con las soluciones utilizadas y los procedimientos realizados. Material y métodos: estudio de cohortes prospectivas comparativas. Se incluyeron 412 pacientes adultos, ASA I y ASA II, para endoscopia diagnóstica bajo sedación consciente, distribuidos en 6 cohortes similares, divididas en dos grupos: no premedicación, 2 cohortes C1 (ayuno de 6 a 8 horas)y C2 (agua 100 mL); premedicación, 4 cohortes C3 a C6 (C3: agua 100 m L + simeticona 1000 mg; C4: agua 100 mL + simeticona 200 mg + N-acetilcisteína 600 mg; C5: agua 100 mL + simeticona 200 mg + N-acetilcisteína 1000 mg; C6: agua 100 mL + simeticona 200 mg + Hedera helix 70 mg). Se ingirió la solución 15 a 30 minutos antes del paso por cricofaríngeo. Se realizó la prueba de Kappa para medir la concordancia interobservador de la escala TVMS. Resultados: De 412 pacientes, 58% fueron de sexo femenino; 23% (136) fue de cohortes C1 y C2 y 67% (276) fue de cohortes C3 a C6. El tiempo medio de exposición a cada solución fue de 24,4 minutos. El volumen de lavado para lograr una adecuada visualización fue significativamente diferente entre ambos grupos: en los pacientes con premedicación se utilizaron 75,6 mL, mientras que en los pacientes sin premedicación se utilizaron 124 mL (p = 0,000), con una calidad de TVMS excelente de 88,7% frente al 41,4%, respectivamente. La cohorte C4 (agua 100 mL + simeticona 200 mg + N-acetilcisteína 600 mg) mostró ser la más efectiva con una diferencia significativa (p = 0,001) frente a C1 (ayuno) y C2 (placebo con agua 100 mL), y también tuvo una eficiencia superior frente a C3, C5 y C6 en su orden. No se presentaron eventos adversos o complicaciones en relación con la endoscopia, la sedación y los productos usados en la premedicación. Conclusiones: la solución más efectiva como premedicación para lograr una excelente visibilidad de la mucosa digestiva correspondió a la cohorte C4 (SIM 200 + NAC 600 + H2O 100 mL). La escala TVMS propuesta es una herramienta muy completa y fácil de aplicar por más de un observador. La premedicación ingerida, con antiburbuja, mucolítico y agua hasta 100 mL, entre 15 y 30 minutos previos a endoscopia, es segura en las condiciones descritas en este estudio.


Abstract Quality parameters for upper gastrointestinal endoscopy have introduced intraprocedural indicators, including adequate mucosal visualization free of saliva, mucus, or bubbles, which may increase the possibility of early-stage injury detection. The use of mucolytics and anti-foaming agents has shown great efficiency variability depending on the type of solution, concentrations, exposure times and visibility scale applied. Objectives: To determine the effectiveness of different premedication solutions for cleaning the digestive mucosa; to validate, by means of an interobserver concordance test, a new scale for the adequate visualization of the mucosa (TVMS) for the esophagus, stomach, and duodenum; and to report adverse events or complications associated with the solutions used and the procedures performed. Material and methods: Prospective, comparative cohort study. 412 adult patients, ASA I and ASA II, were included for diagnostic endoscopy under conscious sedation. They were distributed in 6 similar cohorts and divided into two groups: non-premedication, 2 in C1 (fasting 6 to 8 hours) and C2 (water 100 mL) cohorts; premedication, 4 C3 to C6 cohorts (C3: water 100 mL + simethicone 1000 mg; C4: water 100 ml + simethicone 200 mg + N-acetylcysteine 600 mg; C5: water 100 ml + simethicone 200 mg + N-acetylcysteine 1000 mg; C6: water 100 ml + simethicone 200 mg + Hedera helix 70 mg). The solution was swallowed 15 to 30 minutes passing through the cricopharyngeus muscle. The Kappa test was performed to measure interobserver concordance of the TVMS scale. Results: Of 412 patients, 58% were female; 23% (136) were included in the C1 and C2 cohorts; and 67% (276) were in the C3 to C6 cohorts. The average exposure time to each solution was 24.4 minutes. The wash volume for proper visualization was significantly different between the two groups. In premedicated patients, 75.6 mL of solution were used, while in patients without premedication, 124 mL were used (p = 0.000), with an excellent quality of TVMS of 88.7% versus 41.4%, respectively. The C4 cohort (water 100 mL + simethicone 200 mg + N-acetylcysteine 600 mg) was the most effective with a significant difference (p= 0.001) compared with the C1 (fasting) and C2 (placebo with water 100 mL) cohorts. It also had better efficiency compared to the C3, C5 and C6 cohorts in that order. There were no adverse events or complications associated with endoscopy, sedation, or premedication products. Conclusions: The most effective solution as a premedication to achieve excellent visibility of the digestive mucosa was that used in the C4 cohort (SIM 200 + NAC 600 + H2OR 100 mL). The proposed TVMS scale is a very complete and easy tool to apply by more than one observer. Premedication ingested, with anti-foam, mucolytic and water up to 100 mL, between 15 and 30 minutes before endoscopy, is safe under the conditions described in this study.

Objective: To present and discuss the endoscopic and histological results, as well as the incidence of Helicobacter pylori and other diseases, indications and characteristics of upper digestive endoscopies performed in children. Material and methods: Twenty-five endoscopies were performed in children aged six months to 11 years (mean 7.69 years), from February 2013 to January 2016. In 200 patients, endoscopies were diagnostic and serial biopsies were performed (esophagus, stomach and duodenum), in 120 of them. Results: The indication of endoscopy was diagnosed in 88.89% of the patients, and in 26 patients, a therapeutic procedure was performed. The most frequent endoscopic findings were esophagitis in 49 patients, gastritis in 84 and duodenitis in 16 patients. Four duodenal ulcers were diagnosed. In the therapeutic endoscopies, six gastrostomies were performed, 14 foreign body withdrawals, five nasoenteral tube passages and esophageal dilatation. The H. pylori survey was performed by anatomopathological method and was positive in 26 (13%) of the 200 patients in whom it was searched. Conclusion: pediatric endoscopy is an important niche of the digestive endoscopy, where it is important to emphasize the relevance of the institutional structure that performs these procedures, in order to conduct them safely, being able to treat possible and feasible complications


Objetivo: Presentar y discutir los hallazgos endoscópicos e histológicos, así como la incidencia de Helicobacter pylori y otras enfermedades, indicaciones y características de endoscopia digestiva alta realizada en niños. Material y métodos: Fueron realizadas 225 endoscopias en niños de seis meses a 11 años (media de 7,69 años) a partir de febrero de 2013 hasta enero de 2016. En 200 pacientes, en las endoscopias diagnósticas se llevan a cabo biopsias seriadas (esófago, estómago y duodeno) en 120 de ellos. Resultados: La indicación de endoscopia fue diagnóstica en el 88,89% de los pacientes y en 26 pacientes se realizaron un procedimiento terapéutico. Los hallazgos endoscópicos más frecuentes fueron esofagitis en 49 pacientes, gastritis y duodenitis 84 y en 16 pacientes se diagnosticaron cuatro úlceras duodenales. En endoscopias terapéuticas fueron realizadas seis gastrostomías, catorce extracciones de cuerpos extraños, cinco pasajes de sonda nasogástrica y una dilatación esofágica. El estudio de H. pylori se realizó por el método histopatológico y fué positivo en 26 (13%) de 200 pacientes en los que se han buscado. Conclusión: La endoscopía pediátrica es un nicho importante de la endoscopía digestiva donde es importante enfatizar la relevancia de la estructura institucional que realiza estos procedimientos para conducirlos con seguridad y ser capaces de tratar las complicaciones posibles

La hemorragia de vías digestivas altas (HVDA) es una emergencia habitual; su etiología más común es laúlcera péptica. La restauración del volumen intravascular y la presión arterial son la prioridad del manejo previo al identificar la causa del sangrado. La esofagogastroduodenoscopia (EGD) debe realizarse luego de lareanimación inicial, así como lograrse la estabilización hemodinámica, para identifi car la causa del sangradoy dar el tratamiento necesario. Se realiza un estudio sobre el evento en un hospital de referencia de tercernivel, en Cundinamarca, Colombia.Materiales y métodos: Estudio descriptivo retrospectivo; datos obtenidos de la historia clínica electrónica de pacientes adultos que consultaron por urgencias en el Hospital Universitario de la Samaritana (HUS) por HVDA, diagnosticada por hematemesis, melenas, rectorragia o anemia, y a quienes se les realizó EGDdurante el periodo abril de 2010-abril de 2011. Resultados: Se atendió a 385 pacientes a lo largo del período de estudio; se excluyó a 100 de ellos,debido a hemorragia secundaria a várices esofágicas, historia clínica incompleta y hemorragia digestivabaja, para un total de 285 pacientes analizados. Conclusiones: La mayoría de la población atendida en el Hospital Universitario de La Samaritana (HUS)por HVDA son adultos mayores de 60 años. La úlcera péptica sigue siendo el diagnóstico más común asociadoal uso de AINE y ASA. La mortalidad es comparable a la de las regencias internacionales.


Upper gastrointestinal tract bleeding is a common emergency whose most common etiology is a peptic ulcer.Restoration of intravascular volume and blood pressure management are priorities before identifyingthe cause of bleeding. After initial resuscitation and after hemodynamic stabilization has been achieved, anesophagogastroduodenoscopy (EGD) should be performed to identify the cause of bleeding and determinethe treatment needed. This is a study performed at a third level referral hospital in Cundinamarca, Colombia.Materials and Methods: This is a retrospective study of data from electronic medical records of adult patients admitted to the emergency room of the Hospital Universitario de la Samaritana (HUS) because of uppergastrointestinal tract bleeding which ahd been diagnosed because of hematemesis, melena, rectal bleedingand/or anemia. Patients all underwent EGD between April 2010 and April 2011.Results: 385 patients with upper gastrointestinal tract bleeding were seen during the study period, but 100were excluded because of bleeding secondary esophageal varices, incomplete clinical histories and lowergastrointestinal bleeding. A total of 285 patients were included. 69.1 % were older than 60 years, 73.3 % hadhypertension, 55.1 % reported use of infl ammatory drugs (NSAIDs) and aspirin (ASA), 19.6 % reported previous bleeding episodes, and 17.9 % had hemodynamic instability. 63 patients (22.1 %) required endoscopichemostasis, and 32 (11.2 %) experienced rebleeding. Overall mortality reported was 13.1 % of which 55.3 %were men. Mortality attributable to gastrointestinal bleeding was 3.1 %.Conclusions: The majority of patients served by the HUS with upper GI bleeding are adults over 60 years.Peptic ulcers remain the most common diagnosis associated with the use of NSAIDs and ASA. The mortalityrate is comparable to international standards.

Fundamento: el cáncer gástrico es una de las principales causas de muerte. En Cuba la mayoría son diagnosticados en fases avanzadas ensombreciendo el pronóstico. La linitis plástica es una forma de crecimiento submucoso del adenocarcinoma. Objetivo: ofrecer lección a médicos sobre los signos endoscópicos de sospecha y complementarios a utilizar para diagnosticarlo rápidamente. Presentación de caso: paciente que refería epigastralgia, vómitos y pérdida de peso. La radiografía contrastada de estómago evidenció rigidez y defecto de lleno en curvatura mayor y menor. La esofagogastroduodenoscopía mostró ausencia de peristalsis y estenosis de la luz del estómago. En el examen laparoscópico se observó exteriorización del tumor. El diagnóstico definitivo se realizó por macrobiopsia después de la cirugía. Conclusiones: se ha de sospechar la entidad ante un paciente con la clínica sugestiva de tumor y hallazgos endoscópicos como la disminución de la peristalsis y la reducción de la luz del órgano.


Background: gastric cancer is one of the main causes of death. In Cuba most of the cases are diagnosed in advanced stages clouding the outlook. The plastic linitis is a form of submucosal growth adenocarcinoma. Objective: to provide medical lesson on endoscopic signs of suspicion and complementary use to diagnose it quickly. Case Presentation: a patient referred epigastralgia, vomiting and weight loss. The stomach contrasted radiography showed stiffness and squarely defect in major and minor curvature. The esophagogastroduodenoscopy showed absence of peristalsis and stenosis of the light of the stomach. In the laparoscopic exam the tumor externalization was observed. The final diagnosis was carried out by macrobiopsy after surgery. Conclusions: doctors have to suspect the disease in a patient with clinical suggestive features of tumor and endoscopic findings as decreased peristalsis and reduced organ light.

RESUMO

RACIONAL: O correto posicionamento da sonda nasogástrica para nutrição enteral tem sido alvo de várias investigações, demonstrando as controvérsias e a complexidade do procedimento. OBJETIVO: Prever as medidas antropométricas para realizar a inserção de sonda para nutrição enteral até o estômago empregando a esofagogastroduodenoscopia para calcular-se o comprimento necessário até o estômago. MÉTODO: Medidas externas foram obtidas entre a ponta do nariz vs lobo da orelha vs apêndice xifóide vs umbigo e altura correlacionadas com a medida padrão obtida em pacientes submetidos à esofagogastroduodenoscopia diagnóstica. RESULTADOS: Encontrou-se correlação estatisticamente significativa entre a transição esofagogástrica identificada durante a endoscopia, acrescida da distância da rima labial à arcada dentária superior, com a distância compreendida entre os pontos anatômicos lóbulo da orelha e apêndice xifóide (r=0.75). CONCLUSÃO: A distância entre o lóbulo da orelha ao apêndice xifóide (0.75) e a distância entre o lóbulo da orelha ao apêndice xifóide até o ponto médio da cicatriz umbilical, subtraindo a distância da ponta do nariz ao lóbulo da orelha, foram parâmetros anatômicos seguros para estimar-se a distância da transição esofagogástrica. A altura na posição em pé (r = 0,72) também pode ser utilizada como um indicador da distância necessária para inserir o tubo no estômago.


BACKGROUND:The correct placement of a nasogastric tube for enteral nutrition is subject of several investigations, demonstrating the controversy of the procedure. AIM: To establish an external measure that can correspond to the internal measurement which determines the insertion length of nasogastric feeding tube up to the stomach. METHODS: External measures were obtained between points: nose tip vs earlobe vs xiphoid appendix vs umbilicus and height correlated with the standard measures obtained from patients undergoing diagnostic esophagogastroduodenoscopy. RESULTS: It was found a significative statistical correlation between esophagogastric junction, identified during the esophagogastroduodenoscopy, with the distance measured between the anatomic points of the earlobe and xiphoid appendix (r= 0.75) and from this line with the orthostatic height (r=0.72). CONCLUSION: The distance between the earlobe to the xiphoid appendix (0.75) and the distance between the earlobe to the xiphoid appendix to the midpoint of the umbilicus, subtracting the distance from tip of nose to earlobe, were safe anatomical parameters to reach the esophagogastric junction. The height in the standing position (r= 0.72) also can be used as an indicator of the length necessary to insert the tube into the stomach. RESUMO

Determinar la prevalencia del H. pylori en pacientes procedentes de nivéleles socioeconómicos medio y alto con gastritis crónica y ulcera. Se evaluó mediante esofagogastroduodenoscopía (EGDC) a 776 pacientes, los cuales fueron divididos en 4 grupos de acuerdo a los hallazgos endoscópicos; para establecer su nivel socioeconómico se interrogó sobre lugar y zona de residencia, nivel educativo, ocupación, nivel de ingresos por mes, tipo de de salud. Se encontró una prevalencia de infección por H. pylori del 62.9%, predominante en el sexo masculino (65%) en relación al sexo femenino (62.7%) y en el grupo etáreo de 14 a 29 años (57.2%); según el nivel socioeconómico, se encontró mayor prevalencia de infección en el nivel socioeconómico medio bajo (65.4%), con muy poca diferencia en relación al nivel alto (65%). La prevalencia de infección por H. pylori en pacientes de estratos socioeconómicos medio y alto con gastritis crónica es significativamente alta y similar a prevalencias obtenidas en anteriores estudios.


The objetive was to determine the prevalence of Helicobacter pylori in patients from middle and upper socioeconomic level with chronic gastritis and ulcer. We evaluated by esophagogastroduodenoscopy (EGDC) to 776 patients, whom were divided in 4 groups according to endoscopic findings, to establish their socioeconomic level they were interrogated about the place and area of residence, educational level, occupation, level of monthly income, type of housing, water supply and drainage, household and access to health services. We found a prevalence of helicobacter pylori infection of62.9%, predominantly in males (65%) in relation to women (62.7%) and in the etareus group between 14 to 29 years old (57.2%); about the socioeconomic level, the highest prevalence of infection was found in the lower middle socioeconomic level (65.4%), with little difference in relation to high socioeconomic level (65%). The prevalence of helicobacter pylori infection in patients of middle and upper socioeconomic levels with chronic gastritis are significantly higher and similar compared to prevalences obtained in previous studies.

Objetivo: Describir la población con ingesta de cuerpos extraños en vía digestiva y aérea, e identificar los métodos diagnósticos y tratamientos efectuados. Materiales y métodos: Se presenta un estudio descriptivo, retrospectivo de revisión de historias clínicas de pacientes pediátricos que ingresan por ingesta de cuerpo extraño tratados por el Servicio de Cirugía Pediátrica del Hospital Universitario Erasmo Meoz de Cúcuta, atendidos entre enero de 2000 y diciembre de 2007. Resultados: Se recopilan 158 eventos. Con mayor frecuencia de ingesta de cuerpos extraños en los hombres (70.9%). Se encontró el cuerpo extraño en el 97.5%, 72.8% en la vía digestiva, 23.4% en la vía aérea y 3.2% en cavidad bucal El sitio más frecuente en la vía digestiva fue el esófago proximal y en la vía aérea, el bronquio fuente derecho. Los cuerpos extraños más comúnmente encontrado fueron las monedas. Fueron sintomáticos el 93.7% de los pacientes, con sialorrea y vómito. Se utilizaron como métodos diagnósticos las radiografías de cuello, tórax y abdomen. Para la extracción se realizó endoscopia de vías digestivas altas en 54.7% de los pacientes; en los demás, según el caso se realizó broncoscopia o laringoscopia y extracción con pinza de MacGill. Conclusiones: La ingesta de cuerpos extraños en los niños es un evento común, por lo cual se debe tener conocimiento en cuanto al diagnóstico oportuno, manejo y prevención. Objective: To describe eth infant and children population with foreign body in airway or digestive tract, and their diagnosis and treatment. Materials and methods: A descriptive, retrospective clinical record review was done, including all pediatric patients attended by foreign body ingestion history; all were treated in Pediatric Surgery Service, Hospital Universitario Erasmo Meoz, Cúcuta (Colombia), from January 2000 to December 2007. Results: 158 foreign body ingestion cases were detected; 70.9% were presented in male. In 97.5% a foreign body were found, 72.8% in digestive tract, 23.4% in airway, and 3.2% in mouth. The place most frequently affected were upper esophagus and right bronchus. Most saw foreign bodies were coins; 93.7% patients develop symptoms such as sialorrhea and vomiting. Neck, chest, and abdominal X-ray were diagnoses methods used, for treatment an extraction of foreign bodies, esophagogastroduodenoscopy, was performed in 54.7% of the patients; bronchoscopy or laringoscopy and extraction with MacGill forceps according to each case were also performed. Conclusions: Foreign body ingestion in is a very often event; it bind to have the knowledge about the opportune diagnose, treatment and prevention.
