PURPOSE: In this study, we examined the facial, dental, periodontal, and tomographic features associated with excessive gingival display (EGD) when smiling in young adults self-reporting a "gummy smile," categorized by potential etiology. METHODS: The study included 25 healthy adults (18-42 years old; 23 women and 2 men) who self-reported EGD. Participants completed a health questionnaire and underwent a periodontal examination assessing probing depth, clinical attachment level, keratinized gingival width, and gingival thickness (GT). Extraoral and intraoral photographs were taken for smile analysis and to determine facial and dental characteristics. Cone-beam computed tomography (CBCT), performed with a lip retractor in place, was used to measure the distance from the gingival margin (GM) to the cementoenamel junction (CEJ), the distance from the CEJ to the alveolar crest, buccal bone thickness, and GT. The extent of EGD when smiling was quantified as the distance from the GM at the upper central incisor to the upper lip edge when smiling fully. The smile was categorized into 4 types based on gingival exposure characteristics observed during full smile. RESULTS: Most participants were female (92%), with a mean age of 28.77±6.56 years. The average EGD was 4.2±2.44 mm, extending bilaterally from the anterior to the posterior maxilla. Two primary etiological factors were identified, alone or in combination: vertical maxillary excess (VME), predominantly indicated by an anterior maxillary height greater than 29 mm and a large interlabial gap; and altered passive/active eruption (APE), primarily characterized by square teeth (64%), upper central incisor width-to-height ratio (CIW:CIH) exceeding 87.5%, and GM-CEJ distance on CBCT exceeding 2 mm. CONCLUSIONS: These findings suggest a multifactorial etiology of EGD, primarily associated with VME and APE. Clinical periodontal examination, CBCT conducted with a lip retractor, CIW:CIH, and soft tissue facial cephalometric analysis may aid in identifying the etiological factors of EGD.

El avance de la tecnología permite realizar cambios en la rehabilitación y estética, reduce los tiempos de trabajo y disminuye los riesgos que se presentan al momento de utilizar materiales convencionales para la toma de impresiones. Existen dos técnicas que se pueden emplear, la analógica y la digital; la primera presenta mayores riesgos que el operador no puede controlar, así como tiempos más prolongados. Este reporte describe la rehabilitación bucal en dos pacientes con desgaste dental, se optó por usar ambas técnicas, la analógica y la digital, como tratamiento; en la primera se empleó el encerado convencional y en la segunda el software Exocad para el encerado digital. Finalmente en ambos casos se aplicó (AU)


The advance of technology has allowed changes in rehabilitation and esthetics, providing shorter working times and reducing the risks that occur when using conventional materials for taking impressions. There are two techniques that can be used, analog and digital, the first one presents greater risks that the operator cannot control, as well as more time. This report describes the oral rehabilitation in patients with dental wear. The treatment was carried out using the analogical and digital techniques, the former using conventional waxing and the latter using Exocad software for the digital waxing; finally, in both cases the injection technique with high-load Flow resin was used (AU)

BACKGROUND: The inherently technique-sensitive nature of periodontal plastic procedures demands a significant level of skill and expertise. The incorporation of three-dimensional (3D) printing technologies emerges as a potential strategy to optimize and simplify surgical procedures. This case report describes the digital workflow and presents the clinical outcomes achieved using a guided coronally advanced flap for the treatment of a single gingival recession (GR). METHODS AND RESULTS: A female patient with a gingival recession type 1 (RT1 B-) defect on the mandibular second left premolar underwent successful treatment using a guided coronally advanced flap (g-CAF) and de-epithelized connective tissue graft (CTG). The digital planning included intraoral scanning of the mandible and hard palate using an intraoral scanner, with resulting polygon format (PLY) files exported for virtual model creation. The CAF guide was meticulously designed to orient horizontal and vertical incisions at the papillae base adjacent to the GR defect. For the donor site, a guide was specifically created, positioning the graft area 2 mm apically to the premolars' gingival margins. The delineation of this area involved two horizontal and vertical incisions, meticulously based on the dimensions of the GR. The digitally designed guides were then 3D-printed using a surgical guide-specific resin, contributing to the precise execution of the innovative surgical approach. Complete root coverage was achieved. CONCLUSION: This case report demonstrates that g-CAF can be a promising approach for the treatment of single GR. HIGHLIGHTS: Why is this case new information? To the best of the authors' knowledge, this is the first manuscript to report a guided procedure for the treatment of gingival recession. This report provides the digital workflow for the fabrication of a guide to perform the coronally advanced flap for single recession defects. What are the keys to successfully manage this case? It is necessary to adequately scan the recession defect area and palate. Properly not only design the guide using specific software but also print it. The guide has to be stable when in position for the surgical procedure. What are the primary limitations of this technique? This guide was designed to help surgeons during the incisions. However, it does not provide aid to split and release the flap and suture.

OBJECTIVES: To evaluate the stability of periodontal tissues 3 (T3), 6 (T6), and 12 (T12) months after esthetic crown lengthening (ACL) and the possible correlations between changes in those structures. MATERIALS AND METHODS: Twenty individuals were evaluated through clinical assessment, photography, and tomography. Measurements included gingival margin (GM), clinical crown length (CCL), interdental papilla height (PH) and width (PW), gingival thickness (GT), bone thickness (BT), probing depth (PD), distance between alveolar crest and GM, distance between alveolar crest and cementoenamel junction. Nonparametric and correlation statistics were performed (p < 0.05). RESULTS: CCL at T0 was 7.42 ± 0.70 mm and increased to 9.48 ± 0.49 mm immediately after ACL, but it decreased to 8.93 ± 0.65 mm at T12. PD decreased 0.60 mm from T0 to T6, and it increased 0.39 mm from T6 to T12. BT decreased 0.20 mm, while GT increased 0.29 mm from T0 to T12. Both PW and PH showed enlargement in T12. A positive moderate correlation was found between CCL/T0 and CCL/T12, GT/T0 and AC-GM/T12, BT/T0 and GT/T12. A few negative moderate correlations were PD/T0 and CCL/T12, PD/T0 and PH/T0, PD/T0 and BT/T12. CONCLUSIONS: ACL procedure was effective. Although some rebound occurred, that was not clinically important. PD tended to reestablish its original length, partially due to a migration of GM during the healing period. Besides, a thickening of supracrestal soft tissues was observed. CLINICAL RELEVANCE: The present study centers on the factors influencing the stability of periodontal tissues after esthetic crown lengthening, underscoring the procedure's influence on esthetics and biology and the need for careful treatment planning.

OBJECTIVE: Demonstrate the ability of 0.5 mm thickness lithium disilicate laminates associated with resin cement to mask different substrate saturations. METHODS: 60 specimens (n = 5) were prepared with CAD/CAM lithium disilicate glass-ceramic IPS e.max CAD, 0.5 mm thick, in three degrees of translucency (HT, MT and LT); cemented by Variolink Esthetic LC Light+ (L) and Neutral (N) on composite resin substrates A1 and A4 shades. Color measurements were performed by a spectrophotometer (CM-3700d -Konica Minolta). The L*, a*, b* parameters of the tri-layer structure (laminate/ resin cement/ substrate) were used to calculate color difference ( ∆ E 00 ) by CIEDE2000 formula and Translucency Parameter ( TP 00 ). This study adopted ∆ E 00 = 0.8, as perceptibility threshold (PT), and ∆ E 00 = 1.8, as acceptability threshold (AT). Statistical analyses were performed by one-way ANOVA and Tukey tests (a = 0.05). RESULTS: Color difference between resin cement shades showed statistical differences (p < 0.0001) and ∆ E 00 > 1.8 for all groups. Color difference between substrates saturations showed a significant statistical difference (p < 0.0001) and ∆ E 00 values above AT for all groups. The TP 00 demonstrated significant statistical differences (p < 0.0001). The LTL combination, the opaquest set, showed the smallest ∆ E 00 values between substrate shades differences, however, above AT. CONCLUSION: The translucency of the lithium disilicate, the substrate saturation and the resin cement opacity influenced final color of restoration. The ceramic translucency impact on final color was reduced by opaque resin cement. Furthermore, the LT lithium disilicate and the Light+ resin cement reduced the translucency of the restoration, contributing to mask saturated substrate. CLINICAL RELEVANCE: This study shows the influence of conservative lithium disilicate laminates with different degrees of translucency associated with resin cement on final color of saturated substrate restorations. A minimally invasive esthetic protocol for masking substrates is demonstrated by associating low translucency ceramic laminates and opaque resin cement.

Introducción: Los rasgos oclusales en un grupo étnico pueden estar asociados con factores genéticos y socioculturales que contribuyen a la morfología de los rasgos oclusales y faciales en grupos indígenas. Objetivo: Determinar la necesidad de tratamiento ortodóntico según el índice estético dental y el sexo en la etnia Kichwa Saraguro, Ecuador. Métodos : Estudio observacional, descriptivo, transversal, realizado en el año 2018 en 465 adolescentes de 12 a 16 años, pertenecientes a la etnia Kichwa Saraguro y residentes en Loja, Ecuador. Se empleó la estadística descriptiva, se obtuvo la frecuencia absoluta y el porcentaje para las variables cualitativas; la media, la desviación estándar y la mediana para variables cuantitativas. Para la determinación de normalidad de los datos se usó la prueba Kolmogorov Smirnov y para la relación del sexo con los componentes del índice estético dental y la necesidad de tratamiento ortodóntico se usó la prueba de U de Mann-Whitney y ji al cuadrado. Un valor de α = 0,05 se consideró como estadísticamente significativo. Resultados: Del total de la población, en el 61,06 por ciento se encontró una maloclusión leve con tratamiento innecesario; en el 18,06 por ciento maloclusión manifiesta con tratamiento optativo, en el 9,89 por ciento maloclusión grave con tratamiento deseable y en el 10,97 por ciento maloclusión muy grave con tratamiento obligatorio. Se encontraron diferencias significativas de p = 0,028 en dientes incisivos, caninos y premolares visibles perdidos del maxilar y la mandíbula. Conclusión: Más de la mitad de los participantes de la etnia Kichwa Saraguro presentaron maloclusión leve con tratamiento innecesario y, de acuerdo con el sexo, más de un cuarto de los participantes femeninos mostraron maloclusión manifiesta con tratamiento optativo y maloclusión grave con tratamiento sumamente deseable(AU)


Introduction: Occlusal features in an ethnic group may be associated with genetic and sociocultural factors that contribute to the morphology of occlusal and facial features in indigenous groups. Objective: To determine the need for orthodontic treatment according to the dental esthetic index and sex in the Kichwa Saraguro ethnic group, Ecuador. Methods: Observational, descriptive, cross-sectional study that was carried out in 2018 in 465 adolescents aged 12 to 16 years, belonging to the Kichwa Saraguro ethnic group and residing in Loja, Ecuador. Descriptive statistics were used. Absolute frequency and percentage were obtained for qualitative variables, and mean, standard deviation and median for quantitative variables. The Kolmogorov Smirnov test was used to determine the normality of the data, and the Mann-Whitney U test and chi-squared test were used to determine the relationship between sex and the components of the dental esthetic index and the need for orthodontic treatment. A value of α = 0.05 was considered statistically significant. Results: Of the total population, 61.06 percent were found to have a mild malocclusion with unnecessary treatment. In 18.06 percent manifest malocclusion with optional treatment was found. Severe malocclusion with desirable treatment was found in 9.89 percent and very severe malocclusion with mandatory treatment in 10.97 percent. Significant differences of p = 0.028 were found in visible missing incisors, canines and premolars of the maxilla and mandible. Conclusion: More than half of the participants from the Kichwa Saraguro ethnic group presented mild malocclusion with unnecessary treatment and, according to sex, more than a quarter of the female participants showed manifest malocclusion with optional treatment and severe malocclusion with highly desirable treatment(AU)

Aim: The main purpose of this study was to conduct a narrative review investigating the possible relationship between permanent maxillary anterior teeth and anthropometric facial parameters in different populations. Methods: Searches were performed in the PubMed, BVS (Biblioteca Virtual em Saúde) and SciELO databases to identify relevant scientific articles using the following search terms: "maxillary anterior teeth", "facial measurements" and "anthropometry", in such a way that 218 publications were found. After application of inclusion and exclusion criteria, 13 publications remained for full-text reading. Results: All studies involved male and female samples and it was notorious that male measurements were unanimously higher than those obtained in the opposite sex. The age of the analyzed participants ranged from 17 to 60 years; however, a mean age of 18 to 25 years was the most investigated in literature. In addition, facial measurements including the bizygomatic width, interpupillary distance, intercanthal distance, interalar width and intercommissural width have been proposed to help determine the correct size of anterior teeth. Conclusion: It was concluded that despite the limited number of studies, some factors that influence dental and facial dimensions, such as sex and age, can be identified. However, there is no standardization of the facial or dental parameters used in the studies, a fact that makes it difficult to establish a universal ratio for clinical dental practice

BACKGROUND: Connective tissue graft substitutes have been used widely to overcome autogenous graft limitations. Nevertheless, they do not provide comparable results in the treatment of periodontal and peri-implant soft tissue defects. Based on the principles of tissue-engineered materials, injectable platelet-rich fibrin (i-PRF) has been combined with collagen matrices (CMs) to enhance their clinical efficacy. To the best of our knowledge, this is the first case series demonstrating the use of i-PRF for the biofunctionalization of a volume-stable collagen matrix (VCMX) as an adjunct to coronally advanced flap (CAF) to treat single gingival recession (GR) defects. METHODS & RESULTS: The study included 10 patients. Bleeding on probing, probing depth, GR height, clinical attachment level, esthetics, and dentin hypersensitivity were evaluated. After 6 months, a significant GR reduction (RecRed: 2.15 ± 0.7 mm; p = 0.005) and percentage of root coverage (% RC) of 81.13% were observed. Additionally, 40% of the sites showed complete root coverage. Gingival thickness increased 0.64 mm. Patient-centered evaluations demonstrated dentin hypersensitivity and esthetics improvements by the end of follow-up. CONCLUSION: VCMX biofunctionalized with i-PRF associated with CAF technique showed promising clinical outcomes in the treatment of single RT1 GR defects.

Introdução:Em busca da estética e da função mastigatória,é cada vez mais crescente aprocurapor reabilitações implantossuportadas. O guia multifuncional surgepara orientar a disponibilidade óssea e contribuir no planejamento da instalação tridimensional dos implantes, seguindo os princípios do planejamento reverso.Objetivo:Descrever, por meio de um caso clínico, a possibilidade de obtenção de resultados de excelente previsibilidade em coroas unitárias implantossuportadas, por meio do uso de guias multifuncionais, no planejamento do início ao fim de tratamento.Relato decaso:Paciente A.M.F, 44 anos, sexo feminino, compareceu à clínica de Prótese dentária do Departamento de Odontologia/UFRN com queixa de insatisfação da sua prótese removível e harmonia do sorriso. Ao exame clínico e radiográfico, observou-se ausência do elemento 12, apresentando um espaço interoclusal de 5mm e distância médio-distal de 7mm. Foi confeccionado o guia em resina acrílica, o qual foi preenchido o espaço desdentado com um dente de estoque. Em seguida, o dente deste guia teve seu centro perfurado com uma broca esférica para peça reta na região de cíngulo e com isso, preenchida com guta percha em bastão. Após isso, o paciente foi encaminhado para realizar uma tomografia computadorizada cone beam com o guia multifuncional em posição.As imagens obtidas permitiram o planejamento para instalação do implante, como inclinação e posicionamento favorável, bem como ausência de disponibilidade óssea na região. Além disso, o guia funcionou em outras etapas do tratamento da paciente, como na fase provisória.Conclusão:Os guias auxiliam em diversas fases do tratamento e permitem maior previsibilidade dos resultados em reabilitações protéticas unitárias implantossuportadas, apresentando-se como um dispositivo promissorpara ocorreto posicionamento do implante (AU).


Introduction:In search of esthetics and improved masticatory function, the demand for implant-supported rehabilitation is increasing. Multifunctional guides emerge to assess bone availability and help plan the three-dimensional installation of implants, following the principles of reverse planning. Objective:To describe, through a clinical case, the possibility of obtaining excellent predictability in implant-supported single crowns, through the use of multifunctional guides, inthe planning of a treatment from beginning to end. Case report:Patient A.M.F, 44 years old, female, came to the Prosthodontics clinic at the Department of Dentistry/UFRN complaining of dissatisfaction with her removable prosthesis and the harmony of her smile. Clinical and radiographic examination revealed the absence of element 12, with an interocclusal space of 5mm and a mid-distal distance of 7mm. The acrylic resin guide was made and the edentulous space was filled with a stock tooth. The center of theguide tooth was then drilled with a spherical straight-bit burr in the cingulum region and filled with gutta-percha stick. The patient was then referred for a cone beam computed tomography with the multifunctional guide in position. The images obtained allowed planning for implant installation, such as favorable inclination and positioning, as well as the absence of bone availability in the region. In addition, the guide was effective during other stages of the patient's treatment, such as the provisional phase.Conclusion:The guides assist in various phases of treatment and allow greater predictability of results in implant-supported single prosthetic rehabilitations, presenting themselves as a promising device for correct implant positioning (AU).

La guía multifuncional hasurgido para orientar la disponibilidad ósea y ayudar a planificar la instalación tridimensional de implantes, siguiendo los principios de la planificación inversa. Objetivo: Describir, a través de un caso clínico, la posibilidad de obtener una excelente predictibilidad en coronas unitarias implantosoportadas, mediante el uso de guías multifuncionales, en la planificación desde el início hasta el final del tratamiento. Informe de caso: Paciente A.M.F, 44 años, sexo femenino, compareció a la clínica de Prostodoncia del Departamento de Odontología/UFRN quejándose estar insatisfecha con su prótesis removible y con la armonía de su sonrisa. El examen clínico y radiográfico reveló la ausencia del elemento 12, con un espacio interoclusal de 5 mm y una distancia medio-distal de 7 mm. Se confeccionó una guía de resina acrílica y se rellenó el espacio edéntulo con un diente provisorio. A continuación, se perforó el centro del diente guía con una broca recta esférica en la región del cíngulo y se le rellenó con gutapercha en barra. Posteriormente, el paciente fue remitido a una tomografía computarizada cone beamcon la guía multifuncional en posición. Las imágenes obtenidas permitieron planificar la instalación del implante, como inclinación y posicionamiento favorables, así como la ausencia de disponibilidad ósea en la región. La guía también funcionó en otras fases del tratamiento del paciente, como en la fase provisional. Conclusión:Las guías ayudan en varias fases del tratamiento y permiten una mayor previsibilidadde los resultados en rehabilitaciones protésicas unitarias implantosoportadas, presentándose como un dispositivo prometedor para el correcto posicionamiento de los implantes (AU).

PURPOSE: To evaluate the effects of 1 versus 2 glaze firings on the color and mechanical properties of an extrinsically characterized lithium disilicate ceramic after thermal cycling, brushing, or both. MATERIALS AND METHODS: Eighty specimens were divided into 2 groups: 1 glaze firing (GL1) and 2 glaze firings (GL2). Each group was subdivided into 4 groups (n = 10), according to the experimental conditions: thermal-cycling, brushing, thermal-cycling + brushing, and immersion in distilled water (control). Color variation, surface roughness, and Vickers microhardness were analyzed before each designated experiment and after the simulated periods of 2.5, 5, and 10 years. Three-way mixed ANOVA was used for all outcomes, followed by 1-way ANOVA, repeated measures 1-way ANOVA, Bonferroni post hoc test, and t-test to check for statistical differences (α = 0.05). RESULTS: Thermal cycling generated greater color changes in the GL1 group at 2.5 and 5 years (p < 0.001; p = 0.013). Brushing generated color changes in GL1 at 5 years (p = 0.003) and in GL2 at 10 years (p = 0.017). Regarding surface roughness, the GL1 group suffered alterations in thermal cycling + brushing at 5 years. In the control group, the GL1 group exhibited higher roughness values than GL2 (p < 0.05). Most of the groups experienced an increase in microhardness at 2.5 years (p < 0.05). In the GL1 group, thermal-cycling increased the microhardness at 5 years (p = 0.006); at 5 and 10 years, the GL1 group had a higher microhardness than the GL2 in thermal-cycling + brushing (p < 0.05). CONCLUSION: Ceramics with 1 glaze firing showed greater color, roughness, and microhardness changes compared to those submitted to 2 firings.

A faceta dental é um procedimento executado há muito tempo e com os avanços dos materiais e técnicas apresenta-se com frequência como uma opção para alterar a estética dentária, refletindo na aparência do sorriso. Apareceram novas terminologias ligadas às facetas dentais aumentando a oferta e a procura por tais procedimentos. Na sequência, as indicações das facetas dentais aumentaram, bem como as sobre-indicações e os problemas éticos e legais. Enquanto as facetas dentais podem ser uma opção válida e interessante quando bem indicadas e conduzidas, é importante que os profissionais da odontologia estejam cientes de que, por mais avançados que sejam os materiais utilizados, eles não podem substituir os tecidos dentais que são desgastados no preparo do dente. Assim, é essencial que o dentista seja capaz de indicar e contraindicar o procedimento de facetas dentais com base em critérios técnicos e éticos, em vez de se deixar levar por modismos ou vontades do paciente. Uma vez que um profissional decida realizar um tratamento com facetas dentais, ele é o responsável pelo procedimento, mesmo que tenha sido feito exclusivamente a pedido do paciente. O Termo de Consentimento Livre e Esclarecido deve ser personalizado para o caso com o claro registro do entendimento do paciente, bem como a autorização para o tratamento. Deve-se buscar a filosofia da odontologia minimamente invasiva, respeitando os requisitos técnicos e legais na execução das facetas dentais e considerando sempre o bem-estar do paciente no longo prazo


The use of dental veneers has increased in recent years due to advancements in materials and techniques, which have made it a frequent option for enhancing dental esthetics and improving smiles. However, the rise in the number of veneer procedures has also resulted in an increase in overtreatment and ethical/legal concerns. While dental veneers can be an effective solution for improving the appearance of teeth, it is important for dental professionals to be aware that they are not as good as the dental tissue that may need to be removed during preparation. Dentists must use their technical and ethical expertise to determine whether veneers are appropriate for a patient, based on their individual needs and expectations. Patient desires and trends should not be the sole determining factors. If a dental professional decides to proceed with a veneer treatment, they are responsible for ensuring that it is conducted using minimally invasive dentistry techniques and in accordance with technical and ethical principles. A customized Informed Consent Term is mandatory for each situation. The long-term wellbeing of the patient should be the top priority throughout the process

Introdução:A reabilitação protética implantosuportada de espaços edêntulos na região do sorriso é um desafio para o cirurgião-dentista. Para obtenção da estética em próteses unitárias sobre implante é necessário considerar aspectos como o correto posicionamento do implante e sua harmonia com os tecidos moles e duros. Objetivo:relatar o resultado estético e funcional de um tratamento com auxílio de coroa provisória associada ao condicionamento gengival na reabilitação final com coroa unitária implantossuportada. Relato de caso clínico: Paciente MJFA, 36 anos, sexo feminino, compareceu à clínica de Prótese Dentária do Departamento de Odontologia/UFRN queixando-se de trauma dentário com perda do elemento dentário 15 e necessidade de "ficar com sorriso mais bonito". Após instalação de implante com conexão cônica e período de osseointegração, foi realizada a confecção da coroa provisória sobre implante e iniciada sessões de condicionamento gengival por meio de acréscimos com resina acrílica, utilizando a técnica de pressão gradual sob a margem gengival. Observou-se uma melhora no tecido periimplantar e um perfil de emergência adequado. O caso possui proservação de 3 anos. Conclusões:a realização de condicionamento gengival previamente a prótese final é uma etapa importante para alcançar umareabilitação com característicasestéticas e funcionais semelhantes à de dentes naturais (AU).


Introduction:Implant-supported prosthetic rehabilitation of edentulous spaces in the smile areais a challenge for dental surgeons. To achieve pleasing esthetics in single implant prostheses it is necessary to consider aspects such as the correct positioning of the implant and its harmony with the soft and hard tissues.Objective:to report the esthetic and functional results of a treatment with the aid of a provisional crown associated with gingival conditioning in the final rehabilitation with a single implant-supported crown.Clinical case report:Patient MJFA, 36 years old, female, attended the Prosthodonticsclinic of the Department of Dentistry/UFRN complaining of dental trauma with loss of tooth 15 and the need to "havea more beautiful smile". Afterinstalling an implant with a conical connection and a period of osseointegration, a temporary crown was made on the implant and gingival conditioning sessions were initiatedby means of acrylic resin augmentations, using the gradual pressure technique under the gingival margin. An improvement in the peri-implant tissue and an adequate emergenceprofile were observed. The case has a 3-year follow-up period.Conclusions:performing gingival conditioning prior to the final prosthesis is an important step in achieving rehabilitation with esthetic and functional characteristics similar to those of natural teeth (AU).

Después de la colocación de un implante con conexión cónica y de un período de osteointegración, se realizó una corona provisional sobre el implante y se iniciaron sesiones de acondicionamiento gingival con aumentos de resina acrílica, utilizando la técnica de presión gradual bajo el margen gingival. Se observó una mejora del tejido periimplantario y un perfil de emergencia adecuado. El caso tiene un seguimiento de 3 años. Conclusiones: el acondicionamiento gingival previo a la prótesis definitiva es una etapaimportante para conseguiruna rehabilitación con características estéticas y funcionales similares a las de los dientes naturales (AU).

An esthetically pleasing smile is a valuable aspect of physical appearance and plays a significant role in social interaction. Achieving the perfect balance between extraoral and intraoral tissues is essential for a harmonious and attractive smile. However, certain intraoral deficiencies, such as non-carious cervical lesions and gingival recession, can severely compromise the overall aesthetics, particularly in the anterior zone. Addressing such conditions requires careful planning and meticulous execution of both surgical and restorative procedures. This interdisciplinary clinical report presents a complex case of a patient with esthetic complaints related to asymmetric anterior gingival architecture and severely discolored and eroded maxillary anterior teeth. The patient was treated using a combination of minimally invasive ceramic veneers and plastic mucogingival surgery, resulting in a successful outcome. The report emphasizes the potential of this approach in achieving optimal esthetic results in challenging cases, highlighting the importance of an interdisciplinary team approach in achieving a harmonious balance between dental and soft tissue aesthetics.

Tooth reduction guides allow clinicians to obtain the ideal space required for ceramic restorations. This case report describes a novel design (CAD) for an additive computer-aided manufactured (a-CAM) tooth reduction guide with channels that permitted access for the preparation and evaluation of the reduction with the same guide. The guide features innovative vertical and horizontal channels that permit comprehensive access for preparation and evaluation of the reduction with a periodontal probe, ensuring uniform tooth reduction and avoiding overpreparation. This approach was successfully applied to a female patient with non-carious lesions and white spot lesions, resulting in minimally invasive tooth preparations and hand-crafted laminate veneer restorations that met the patient's aesthetic demands while preserving tooth structure. Compared to traditional silicone reduction guides, this novel design offers greater flexibility, enabling clinicians to evaluate tooth reduction in all directions and providing a more comprehensive assessment. Overall, this 3D printed tooth reduction guide represents a significant advancement in dental restoration technology, offering clinicians a useful tool for achieving optimal outcomes with minimal tooth reduction. Future work is warranted to compare tooth reductions and preparation time for this guide to other 3D printed guides.

La exposición gingival excesiva es una condición conocida como sonrisa gingival. Esta alteración genera en muchos casos insatisfacción y solicitud de tratamiento por parte de los pacientes. Este exceso de exposición gingival podría obedecer a diferentes etiologías como un exceso maxilar en sentido vertical, un labio superior corto e hipermóvil, la erupción pasiva alterada, la extrusión dentoalveolar en el sector anterior o la combinación de estas causas. El enfoque de tratamiento va a depender del diagnóstico, de la complejidad del caso y de las expectativas de cada paciente. Por lo tanto, las rutas terapéuticas principales son el alargamiento de corona clínica, el reposicionamiento labial, la cirugía ortognática o la aplicación de toxina botulínica. Para ello, se debe considerar las ventajas y limitaciones de cada tratamiento estético, para garantizar predictibilidad y éxito.


Excessive gingival exposure is a condition known as a gummy smile. This generates in many cases dissatisfaction and request for treatment by patients. This excess of gingival exposure could be due to different etiologies such as vertical maxillary excess, a short and mobile upper lip, altered passive eruption, dentoalveolar extrusion in the anterior sector or a combination of these causes. The treatment approach will depend on the diagnosis, the complexity of the case and the expectations of each patient. Therefore, the main therapeutic routes are clinical crown lengthening, lip repositioning, orthognathic surgery, or the application of botulinum toxin. For this, the advantages and limitations of each aesthetic treatment must be considered, to guarantee predictability and success.

O período da adolescência é relevante no tratamento ortodôntico, pois nessa fase de desenvolvimento dentofacial ocorrem alterações que modificam demasiadamente a dentição e a face. Geralmente é nessa faixa etária que se inicia o tratamento ortodôntico e sua busca é desencadeada predominantemente por uma demanda estética. Durante o tratamento ortodôntico, o perfil facial pode sofrer grandes alterações. Sendo assim, o planejamento do tratamento ortodôntico baseado em evidência cientifica requer também a participação do paciente no relato da autopercepção sobre seu perfil facial. A autopercepção de pacientes no ciclo de vida da adolescência pode ser influenciada por fatores como a idade, sexo e autoestima. Diante disso, o objetivo desse estudo foi avaliar a influência da autoestima, do bemestar e das características do indivíduo na autopercepção do perfil facial em adolescentes. Foi realizado um estudo observacional transversal com uma amostra de 158 adolescentes de ambos os sexos, com idade entre 10 a 19 anos, pacientes e alunos de Graduação da Faculdade de Odontologia da Universidade Federal de Minas Gerais. A autopercepção dos perfis faciais foi avaliada pela apresentação de um template contendo 10 fotografias de perfil facial, sendo nove fotografias padronizadas e uma fotografia de perfil do participante do estudo. O participante foi solicitado a identificar o seu perfil facial dentre os 10 apresentados. As fotografias de perfil facial foram realizadas por uma pesquisadora treinada. A autoestima dos adolescentes foi medida por meio da versão brasileira validada da Escala de Rosenberg. No caso de adolescentes menores de 18 anos, seus pais/responsáveis responderam a um questionário sobre as condições socioeconômicas e demográficas de suas famílias, e sobre o quanto o bem-estar do adolescente era afetado pelas condições bucais. Os adolescentes de 18 anos ou mais responderam à mesmas questões. Análises descritivas e regressão de Poisson com variância robusta, não ajustada e ajustada, foram realizadas (p<0,05). O modelo de análise ajustada demonstrou que a autopercepção do perfil facial era maior em adolescentes mais velhos (Razão de Prevalência [RP]=1,79; 95%Intervalo de Confiança [IC]:1,34-2,40; p<0,001) e cujo bem-estar geral não era afetado pelas condições bucais (RP=1,41; 95%IC:1,10-1,81; p=0,007). Os adolescentes que relatam se sentirem fracassados (RP=0,63; 95%IC:0,46-0,85; p=0,003) e aqueles que apresentam uma atitude positiva consigo mesmo (RP=1,46; 95%IC: 1,00-2,14; p=0,050) têm maior probabilidade de reconhecer o seu perfil facial do que aqueles que não possuem esses sentimentos. Conclui-se que adolescentes mais velhos, que não tiveram seu bem-estar geral afetado pela condição bucal, que relataram sentirem-se fracassados e possuírem uma atitude positiva sobre si mesmos apresentaram maior probabilidade de reconhecer o seu perfil facial.


The period of adolescence is relevant in orthodontic treatment, as changes occur in this phase of dentofacial development that greatly modify the dentition and the face. It is usually in this age group that orthodontic treatment begins and its search is predominantly triggered by an aesthetic demand. Facial profile can undergo major changes during orthodontic treatment, according to the type of treatment. Therefore, orthodontic treatment planning based on scientific evidence also requires the patient's participation in reporting self-perception of their facial profile. The self-perception of patients in the adolescence life cycle can be influenced by factors such as age, sex and self-esteem. Therefore, the objective of this study was to evaluate the influence of self-esteem, well-being and individual characteristics on the self-perception of the oral facial profile in adolescents. A cross-sectional observational study was carried out with a sample of 158 adolescents of both sexes, aged between 10 and 19 years, patients and undergraduate students of the Faculty of Dentistry of Universidade Federal de Minas Gerais. The self-perception of facial profiles was assessed by presenting a template containing 10 profile photographs, nine of which were standardized photographs and one profile photograph of the study participant. The participant was asked to identify his/her soft tissue profile among the 10 photographs. Soft tissue profile photographs were taken by a trained researcher. The adolescents' self-esteem was measured using the validated Brazilian version of the Rosenberg Scale. For adolescents under 18 years of age, their parents/guardians completed a questionnaire about the socioeconomic and demographic conditions of the adolescents and their families, and about how much the adolescent's well-being was affected by oral conditions. Adolescents aged 18 years and over answered the same questions. Descriptive statistics and unadjusted and adjusted Poisson regression with robust variance were used for data analysis (p<0.05). The adjusted analysis model showed that self-perception of the facial profile was higher in older adolescents (PR=1.79; 95%CI:1.34-2.40; p<0.001) and whose general well-being was not affected by oral conditions (PR=1.41; 95%CI:0.91-1.81; p=0.007). Adolescents who report feeling like failures (PR=0.63; 95%CI:0.46-0.85; p=0.003) and those who have a positive attitude towards themselves (PR=1.46; 95% CI: 1.00-2.14; p=0.050) are more likely to recognize their facial profile than those who do not have these feelings. It is concluded that older adolescents, who did not have their general well-being affected by their oral condition, who reported feeling like failures and having a positive attitude about themselves, were more likely to recognize their facial profile.

ABSTRACT Objective: This study aimed to conduct a literature review on self-perception of oral aesthetics in individuals undergoing cosmetic dental treatment and its impact on quality of life. Methods: 834 articles were found, of these, 31 articles were selected and accessed from the scientific productions indexed in the PubMed, LILACS and SciELO electronic databases. To rescue the sample, the following descriptors were used in Portuguese: estética dentária, odontologia cosmética, autoimagem and autopercepção; and in English: dental esthetics, cosmetic dentistry, self-image and self-perception, from 2016 to 2021. Results: The articles highlighted the relationship between aesthetic self-perception and the need for treatment. It was found that the aesthetic and psychosocial self-perception had a positive impact, improving the self-esteem and quality of life of individuals after undergoing aesthetic dental treatments. Conclusions: The research considered that new studies should be carried out on this theme, and that dentists should seek more knowledge related to oral and general health conditions, as well as on the self-perception of oral aesthetics, in order to prepare them for the performance. of interventions and planning of curative actions.


RESUMO Objetivo: Este estudo teve como objetivo realizar um levantamento bibliográfico na literatura sobre a autopercepção da estética bucal em indivíduos submetidos a tratamentos estéticos dentários e o seu impacto sobre a qualidade de vida. Métodos: Foram encontrados 834 artigos, destes, 31 artigos foram selecionados e acessados a partir das produções científicas indexadas nas bases eletrônicas da PubMed, LILACS e SciELO. Para o resgate da amostra utilizaram-se os seguintes descritores em português: estética dentária, odontologia cosmética, autoimagem e autopercepção; e em inglês: esthetics dental, cosmetic dentistry, self image e self-perception, entre os anos de 2016 a 2021. Resultados: Os artigos evidenciaram a relação entre a autopercepção estética e a necessidade de tratamento. Verificou-se que a autopercepção estética e psicossocial teve impacto positivo, melhorando a autoestima e qualidade de vida dos indivíduos após serem submetidos à tratamentos estéticos odontológicos. Conclusão: A pesquisa considerou que novos estudos devem ser realizados nesta temática, e que cirurgião-dentista deve buscar mais conhecimentos relacionados às condições de saúde oral e geral, bem como, sobre a autopercepção da estética bucal, a fim, de prepará-los para a realização de intervenções e planejamento de ações curativas.

Substitutos de enxerto de tecido conjuntivo têm sido amplamente utilizados para superar as limitações dos enxertos autógenos no tratamento de defeitos dos tecidos moles periodontais e peri-implantares. No entanto, o desempenho clínico desses biomateriais ainda é inferior. A biofuncionalização de matrizes colágenas usando fibrina rica em plaquetas injetável (i-PRF) foi proposta como uma estratégia para aprimorar a bioatividade e, portanto, a eficácia clínica desses substitutos mucosos. Desta forma, o objetivo deste estudo foi avaliar a eficácia do uso da matriz colágena estável em volume (FG) biofuncionalizada com i-PRF no tratamento de recessões gengivais unitárias (RGs) do ponto de vista clínico, estético e de parâmetros centrados no paciente. Para tal, foram selecionados 66 pacientes portadores de RGs unitárias RT1, os quais foram alocados aleatoriamente em um dos seguintes grupos: grupo CAF (n=22), retalho posicionado coronariamente (CAF); grupo CAF+FG (n=22), CAF associado à FG; e grupo CAF+FG+i-PRF (n=22), CAF associado à FG biofuncionalizada com i-PRF. Após 6 meses, os três grupos apresentaram taxas de recobrimento radicular significativas [CAF: 69,1% (2,02 ± 1,06 mm); CAF+FG: 67,44% (1,7 ± 0,81 mm) e CAF+FG+i-PRF: 64,92% (1,64 ± 0,80 mm), sem diferença entre os grupos (p=0,33). Os grupos que receberam os biomateriais forneceram um maior ganho em espessura de tecido queratinizado (ETQ) (CAF: 0,12 ± 0,2 mm; CAF+FG: 0,43 ± 0,24 mm; CAF+FG+i-PRF: 0,48 ± 0,25 mm; p=0,000). Não foram observadas diferenças significativas em termos de altura de tecido queratinizado em nenhum dos grupos e tempos avaliados (p>0,05). Todos os grupos apresentaram redução significativa da hipersensibilidade dentinária e melhorias nas condições estéticas (p>0,05). Também não foram observadas diferenças em termos de dor e morbidade pósoperatórias (p>0,05). Dentro das limitações do presente estudo, conclui-se que as três abordagens forneceram resultados semelhantes e satisfatórios após 6 meses de acompanhamento. A adição da FG, biofuncionalizada ou não com i-PRF, proporcionou benefícios adicionais em termos de ganho de ETQ. (AU)


Soft tissue graft substitutes have been widely used to overcome the limitations of autogenous grafts in the treatment of periodontal and peri-implant soft tissue defects. However, the clinical performance of these biomaterials is still inferior. The biofunctionalization of collagen matrices using injectable platelet-rich fibrin (i-PRF) has been proposed as a strategy to enhance the bioactivity and, therefore, the clinical efficacy of these biomaterials. Thus, the aim of this study was to evaluate the effectiveness of using biofunctionalized volume-stable collagen matrix (VCMX) with i-PRF in the treatment of single gingival recessions (GRs) from clinical, esthetic, and patient-centered parameters. For this purpose, 66 patients with single RT GRs were selected and randomly allocated to one of the following groups: CAF group (n=22), coronally advanced flap (CAF); CAF+VCMX group (n=22), CAF combined with VCMX; and CAF+ VCMX +iPRF group (n=22), CAF combined with biofunctionalized VCMX with i-PRF. After 6 months, all three groups exhibited significant root coverage rates [CAF: 69.1% (2.02 ± 1.06 mm); CAF+FG: 67.44% (1.7 ± 0.81 mm); and CAF+FG+iPRF: 64.92% (1.64 ± 0.80 mm), with no difference between the groups (p=0.33). The groups that received the biomaterials showed a greater gain in keratinized tissue thickness (KTT) (CAF: 0.12 ± 0.2 mm; CAF+FG: 0.43 ± 0.24 mm; CAF+FG+i-PRF: 0.48 ± 0.25 mm; p=0.000). No significant differences were observed in terms of keratinized tissue height in any of the groups and assessed time points (p>0.05). All groups showed a significant reduction in dentin hypersensitivity and improvements in esthetic conditions (p>0.05). No differences were also observed in terms of post-operative pain and morbidity (p>0.05). Within the limitations of this study, it is concluded that all three approaches provided similar and satisfactory results after 6 months of follow-up. The addition of VCMX, whether biofunctionalized or not with i-PRF, provided additional benefits in terms of keratinized tissue thickness gain. (AU)

El perfil facial de un paciente con prótesis total removible podría no ser óptimo cuando el paciente tiene rebordes óseos conservados tridimensionalmente, planteando la necesidad de una prótesis dentaria y no dentogingival. El objetivo de este reporte es describir la transición y tratamiento de una paciente desdentada maxilar, usuaria de prótesis removible con excesivo soporte labial y disponibilidad ósea favorable. En la planificación se prueban parámetros estéticos mediante una ordenación dentaria sin flanco y en base a esta se planifica una "Cirugía de Implantes Protésicamente Guiada" para rehabilitación fija de arco completo sobre implantes en posiciones 1.6, 1.3, 1.1, 2.1, 2.3 y 2.6. Se concluye que con la técnica descrita se puede realizar una evaluación clínica veraz de la propuesta protésica, lo que se traspasa a la guía radiográfica y quirúrgica. Lo anterior permite consistencia entre planificación, cirugía, provisonalización y prótesis definitiva.


The facial profile of a patient with a complete removable denture may not be optimal when the patient has a three-dimensionally preserved bone ridge, raising the need for a dental rather than dentogingival prosthesis. The objective of this report is to describe the transition and treatment of a maxillary edentulous patient, user of a removable prosthesis with excessive lip support and favorable bone availability. In the planning phase, aesthetic parameters are tested using a dental set-up without flank. Based on this, a "Prosthetically Driven Implant Surgery" is planned for a fixed full-arch implant-supported prosthesis with implants located in 1.6, 1.3, 1.1, 2.1, 2.3 and 2.6. It is concluded that, with the described technique, a truthful clinical evaluation of the prosthetic proposal can be performed, and then transferred to the radiographic and surgical guide. This allows consistency between planning, surgery, provisionalization and final prosthesis.

A crescente busca pelo fator estético por meio de procedimentos odontológicos, junto ao impacto das redes sociais na atuação do profissional e no processo de tomada de decisão do paciente, provoca questionamentos sobre a conduta ética e legal do cirurgião-dentista. Dessa maneira, o objetivo deste trabalho foi analisar os aspectos éticos e legais relacionados à conduta profissional na abordagem e proposta de tratamento odontológico com facetas estéticas e lentes de contato dental divulgados por meio da ferramenta digital YouTube®. O estudo foi realizado em duas etapas: a primeira, a partir da análise de 10 vídeos de relatos de caso de pacientes que realizaram tratamento com facetas estéticas e lentes de contato dental, e, na segunda etapa, foram analisados 10 vídeos com o tema relacionado à apresentação que os cirurgiões-dentistas (CD) fazem sobre os tratamentos estéticos. Em estudo quantitativo-qualitativo, procedeu-se a análise das percepções do profissional (que oferta serviços) e do paciente (consumidor), confrontadas em seu conteúdo, tendo como referência aspectos éticos e legais vigentes. Como resultado, tem-se que nos vídeos de CD, 3 (30%) não abordam limitações e riscos, e nenhum trata sobre Termo de Consentimento Livre e Esclarecido (TCLE) e contrato. Apesar disto, nos vídeos de pacientes foram observadas poucas reclamações sobre as informações prestadas (10%) e nenhuma menção a processos. Logo, percebe-se, a partir deste estudo, que, os vídeos analisados frequentemente ferem normas éticas e legais da categoria profissional, sendo de realização arriscada, ainda que aparentemente os pacientes não estejam queixando-se de prática abusiva, os conselhos profissionais podem agir de ofício e instaurar processos éticos contra os profissionais


The crescent search for the aesthetic factor through dental procedures, coupled with the impact of social media on professional activities and the patient's decision making process, raises questions about the dentists ethical and legal conduct. The objective of this study was analyzing the ethical and legal aspects related to the professional's conduct in their approach and proposal of dental treatment with aesthetic dental facets and contact lenses in the digital means of YouTube®. The study was performed in two stages: the first one through the analysis of 10 YouTube® videos, related to case studies from the perspective of patients who had been subject to treatment with aesthetic dental facets and contact lenses; and the second stage analyzing 10 videos related to how dentists present the aesthetic treatments through this media. In a quantitative-qualitative study the two perspectives were analyzed, the professional as treatment offered and the patient as consumer, confronted through their content, referring back to current ethical and legal aspects, observed or disregarded, and the implied risks to the professional, serving as reading reference to critical, alert, competent and responsible practice. The results show that, in the dentists videos, 3 of them (30%) do not tackle the limitations and risks, and none mention informed consent forms (ICF). Despite this, the patient videos show few complaints about the information given beforehand (10%) and no mention of lawsuits. Therefore it is noted through this study that, although publicity in health services has its risks, the professionals apparently are not incurring in abusive practices
