BACKGROUND: The surgical approach to pathologies of the Ascending Thoracic Aorta (ATA) that compromise aortic root and the aortic arch is currently one of the most complex interventions in the spectrum of cardiac surgery, where circulatory arrest with cerebral perfusion plays an important role for Success postoperative and patient survival. CASE PRESENTATION: We present the case of a 57-year-old patient with the only history of arterial hypertension and an ATA Aneurysm that compromised segment of the aortic root up to segment 2 of the aortic arch. A successful Bentall surgery was performed, debranching supra-aortic vessels with Total Circulatory Arrest with Deep Hypothermic Cerebral Perfusion-Antegrade Bilateral. CONCLUSIONS: With the advent of new anesthetic and neuroprotection techniques, perioperative imaging protocols, advanced hemodynamic monitoring, and invaluable advances in perfusion and Extracorporeal Circulation with circulatory arrest, they have made this surgical challenge a valuable tool for today's cardiovascular surgeon.

Intravascular leiomyoma is an uncommon disease and depending of vascular involvement and anesthetic challenge. We review a case of a 53-year-old woman who underwent vena cava leiomyoma resection under cardiopulmonary bypass using deep hypothermic circulatory arrest (DHCA). Invasive hemodynamic and neurologic monitoring, transesophageal echocardiography, and viscoelastic coagulation test were used during the procedure. Total surgical resection was accomplished with no complications and the patient was extubated 2 days after surgery without cardiac or neurologic deficit. Although uncommon, level IV intravascular leiomyoma surgery is a challenge because the total resection needs DHCA, prolonged cardiopulmonary bypass and aortic cross-clamp times. These conditions expose the patient to the risk of coagulopathy, low cardiac output syndrome, and neurologic deficit.

There is currently a paradigm shift in the conduct of adult aortic arch repair. Although deep hypothermic circulatory arrest has been the classic perfusion platform for adult aortic arch repair, recent developments have challenged this aortic arch paradigm. There has been a gradual clinical drift towards moderate, and even mild, hypothermic circulatory arrest combined with antegrade cerebral perfusion. This paradigm shift appears to be associated with equivalent clinical outcomes, and in certain settings, with improved outcomes. The advent of endovascular therapy has challenged even further the concept that circulatory arrest is required for adult aortic arch repair. These dramatic advances have resulted in the emergence of an international aortic arch surgery study group that aims to advance this dynamic field through consensus statements, meta-analysis, clinical database analysis, prospective registries, and randomized controlled trials.

Apesar dos grandes avanços terapêuticos, a parada circulatória total em hipotermia profunda pode ser necessária para a correção cirúrgica de algumas cardiopatias congênitas. O manejo adequado da hipotermia e a parada circulatória são essenciais para menor morbidade, principalmente neurológica. Hematócrito entre 25 e 30%; resfriamento lento a 17-18ºC; e períodos de parada circulatória com menos de 20 minutos são algumas das recomendações a serem seguidas para melhor sobrevida do paciente.(AU)


Despite great therapeutic progress, deep hypothermic circulatory arrest may be necessary to surgically correct some congenital cardiopathies. Conducting hypothermia and circulatory arrest adequately is crucial to assure less morbidity, especially neurological morbidity. Hematocrit between 25 and 30 %; slow cooling to 17-18º C and periods of circulatory arrest for no longer than 20 minutes are some of the recommendations to be followed to assure patients' survival.(AU)
