The aim of this study was to describe the behavior of the indicator of the proportion of pregnant women with dental care performed in all Brazilian states before and after the implementation of the Prevent Brazil Program and correlate the findings with variables. This is an ecological, analytical study with secondary data, whose units of analysis are 26 of the federative units of Brazil and the Federal District. The variables proportion of pregnant women with dental care performed between the years 2018 and 2022 (per four months), number of Family Health Strategy teams, FHS coverage and Primary Care (PHC) coverage in 2020, number of oral health family health teams (eSB) in 2021, estimated population size for 2021 and Human Development Index for 2010, were collected from the Primary Care Health Information System (SISAB), the Brazilian Institute of Geography and Statistics (IBGE) and the e-Manager System of the Ministry of Health. The results obtained show that during the years analyzed the indicator had growth in all federative units and in the Federal District, however, some states had growth of the indicator above the national average: Amazonas, Tocantins, Alagoas, Ceará, Paraíba, Pernambuco, Rio Grande do Norte, Sergipe, Mato Grosso do Sul Paraná and Santa Catarina. The indicator had a negative correlation with HDI and a positive correlation with the coverage of family health and oral health teams. There was an increase in the proportion of dental care to pregnant women in PHC between the years studied, however, this indicator does not allow the evaluation of the quality of this care nor the impact on maternal and infant morbidity and mortality.

BACKGROUND: The acceptability and preference for clear aligner therapy (CAT) has been increasing among orthodontists, but there is still a lack of consensus regarding CAT best practices. Consequently, this study aimed to investigate CAT practices among orthodontists practicing in Canada. METHODS: The survey was conducted among orthodontists practicing in Canada using a modified previously published survey. Sixty orthodontists participated (6.1% response rate). It consisted of 11 sections with open and closed questions related to demographic information and particularities about using or not using CAT. The survey responses were exported from REDCap to a Microsoft Excel (Microsoft, Redmond, Wash) spreadsheet, then statistically analyzed using SPSS software (SPSS for Windows, version 21.0; IBM Inc., Armonk, NY, USA). The comments were categorized under themes and subthemes. Data were organized in descriptive statistics, expressing frequencies and percentages. RESULTS: Almost 30% of the orthodontist's annual caseload was treated with CAT, most frequently prescribed to adult patients. Case complexity and patient cooperation were the factors that most influenced the decision to prescribe CAT. Almost half of orthodontists reported sometimes combining CAT with adjunctive fixed appliances. CONCLUSIONS: Most orthodontists prescribe CAT, and its use is based on the malocclusion's complexity. Orthodontists who do not prescribe CAT believe that fixed appliance therapy has superior treatment outcomes.

RESUMO

INTRODUCTION: Oral diseases affect a significant proportion of the world's population, yet international comparisons involving oral health outcomes have often been limited due to differences in the way country-level primary data are collected. In response to this, the World Dental Federation (FDI) Oral Health Observatory project was launched with the goal of collecting and producing standardised international data on oral health across countries. The aim of this descriptive cross-sectional study was to examine associations between self-reported general health and a range of factors (sociodemographics, oral health-related behaviours, oral impacts, clinical variables) using these standardised international datasets. METHODS: Dentists within FDI member National Dental Associations who chose to take part in the project were selected using a multistage sampling method. The number of dentists in each cluster was set according to the proportion of the national population living in the area, and 50 patients per dentist were systematically approached to take part. Patients and dentists completed 2 separate questionnaires on a mobile app. Ordinal logistic regression (conducted in December 2022) was used to analyse the linked patient and dentist data from 6 countries: China (n = 2242); Colombia (n = 1029); India (n = 999); Italy (n = 711); Japan (n = 1271); and Lebanon (n = 798). Self-reported general health was the dependent variable, with age, sex, education, self-reported oral health-related behaviours, self-reported oral impacts, and clinical variables acting as the independent variables. RESULTS: The results demonstrated a different pattern of associations in the different countries. Better self-reported general health was associated with degree-level education in all 6 countries and with reporting no oral impact and no sensitive teeth in 4 countries. Several country-specific patterns were also found, including the importance of tooth brushing in Colombia, periodontal health in Italy, and differing associations with sugary drinks consumption in India and Japan. CONCLUSIONS: These descriptive findings provide a basis for further research and, importantly, for advocacy in identifying patient oral health care needs according to both person-reported and clinical aspects. This can facilitate optimisation of service provision and potentially influence policy and investments.

RESUMO

BACKGROUND: Surveys are a widely used research method in dentistry in different specialities. The study aimed to determine the quality of survey-based research reports published in dentistry journals from 2015 to 2019. METHODS: A cross-sectional descriptive research study was conducted. The report quality assessment was carried out through the SURGE guideline modified by Turk et al. Four journals indexed in the Web of Science were selected: BMC Oral Health, American Journal of Orthodontics and Dentofacial Orthopedics, Journal of Dental Education, and Journal of Applied Oral Science. The selection of articles was made using the PubMed database considering the following search words: questionnaire OR survey, two trained reviewers applied the guideline to the selected articles, and the controversies were solved by discussion and consensus. RESULTS: A total of 881 articles were identified, of which 99 met the selection criteria and were included in the study. The best-reported items (n = 99) were four: the two that described the introduction of a study, the results reflecting and concerning the study objectives, and the review by an ethics committee. Five items were poorly reported: to declare the incentives to study participants (n = 93), three items on the description of statistical analyses (n = 99, 99, and 94), and information on how nonrespondents differed from respondents (n = 92). CONCLUSIONS: There is a moderate quality of reporting of all aspects that should be considered in survey-based studies in dentistry journals. Poorly reported criteria were found mainly in the statistical analysis.

Abstract Objective: to describe three oral health indicators-the proportions of tooth loss, dental visits, and toothbrushing frequency-according to job market insertion in 2019. Methods: a descriptive study with data from the 2019 Brazilian National Health Survey (PNS). We included individuals aged from 18 years or older, irrespective of employment status. We calculated the oral health indicators, as well as their respective 95% confidence intervals (95%CI). Results: the sample was composed by 54,343 workers, 29,889 (53.9%) being male. The proportion of 13 or more teeth loss was higher in the informal employment sector [10.5% (95%CI: 9.8;11.2)]. The proportion of people that did not consult a dentist in the previous 12 months was higher in the informal sector [51.9% (95%CI: 50.8;53.0)] and among unemployed [52.4% (95%CI: 49.6;55.2)]. Lower toothbrushing frequency was observed in the informal workers' group [5.2% (95%CI: 4.7;5.6)] and among male workers [7.5% (95%CI: 6.8;8.2)]. Conclusion: worse oral health indicators were found among informal and unemployed workers, in addition to gender inequalities.

RESUMO

ABSTRACT Objective: To evaluate the oral health and related factors among people with visual impairment in a municipality in southeastern Brazil. Methods: A cross-sectional and descriptive household study was carried out on a representative sample of people with visual impairments in a medium-sized city in Brazil. A clinical-dental examination was performed (evaluating tooth loss, caries, periodontal health, dental plaque use and need of prosthesis) and a semistructured questionnaire was applied, investigating the socioeconomic conditions (sex, age, marital status, skin color and income) and the use of dental services (reason, time since, and type of service used [private/public] in last dental visit). Descriptive and bivariate analysis were performed to asses the factors related to oral health. Results: The mean age of participants was 65 years (±15.05), with the majority women (70.9%). 53.1% of the interviewees were edentulous, but 58.2% had a good perception of oral health. The blind ones use less public services (p=0.038) and with a lower frequency (p=0.014) than subjects with low vision. Those with less schooling had higher tooth loss and lower periodontal problems (p <0.05). There was a reduction of dental plaque six months after the educational action (30.7%; p=0.01), especially among the low vision participants (p=0.01). Conclusion: Severe tooth loss and unsatisfactory hygiene contrast with self-perception of oral health. Visual acuity and schooling were related to access and oral health. Actions for promotion and equity are necessary to overcome the inequalities encountered.

Abstract Background Patients with chronic renal failure may have their oral health impaired as a result of the disease itself, its treatment, and its associated lifestyle alterations. Objective To assess the factors associated with poor self-rated oral health among adults with chronic renal failure treated by hemodialysis. Method This is a cross-sectional study with 243 adults undergoing hemodialysis in a hospital in Minas Gerais, Brazil in 2013-2014. The outcome was assessed by the self-report of oral health categorized into good (excellent/good) and bad (fair/bad / very bad). The independent variables included sociodemographic conditions, general health, oral health, and the use of dental services were collected through a structured questionnaire. The association between the outcome and the independent variables was tested using multiple logistic models with hierarchical inclusion of variables. Results The prevalence of poor self-rated oral health was 35.4%. The youngest (p = 0.015), those who have undergone hemodialysis in the shortest time (p = 0.016), have halitosis (p <0.001), need dental treatment (p <0.001), and had their last dental appointment not for pain (p = 0.027) expressed higher odds of poor self-rated oral health, independently of sociodemographic and health conditions. Conclusion Sociodemographic conditions, time on hemodialysis, oral impairments, and use of dental services affected the oral health self-assessment among adults undergoing hemodialysis.

Introduction: the search for dental care is an important parameter to determine how individuals understand the oral health. Objective: to evaluate the use of dental services and associated factors in cancer patients in Minas Gerais, Brazil. Material and method: a cross-sectional study carried out with a sample of 441 adult patients attending a voluntary association to support cancer patients in the State of Minas Gerais, Brazil. The outcome was assessed through self-report of dental appointments in the last year. Independent variables included socioeconomic status, general and cancer-related health, oral health, and dental care. The associations between the outcome and the independent variables were tested using bivariate and multiple logistic regression, and the results were expressed as odds ratios (OR) and 95% confidence intervals (95%CI). Result: the prevalence of dental appointments in the last year was 33.3%. Individuals who have between four and seven (OR: 1.80; 95%CI: 1.02-3.18) and those who have eight or more years of schooling (OR: 3.20; 95%CI: 1.07-9 .60) had greater, whereas, the edentulous (OR: 0.44; 95%CI: 0.21-0.92) had less odds to have had a dental appointment in the last year, independently of socioeconomic status, general health and oral health conditions. Conclusion: recent dental care was uncommon among cancer patients. Furthermore, there were inequalities related to schooling and the number of remaining teeth in the use of services. The identification and prioritization of groups with unfavorable access to services are fundamental in planning the provision of dental care to this population.

Exposure to chemicals in the work environment has been associated with oral conditions. The aim of this study was to examine the association between handling chemical products and self-rated oral health in a sample of Brazilian workers. A cross-sectional study used data from the National Health Survey (PNS), carried out in 2013, in Brazil. The sample consisted of 36,442 workers of both sexes and aged ≥ 18 years. Socioeconomic, demographic, occupational, lifestyle and health status characteristics were described. For the perception of oral health, the outcome variable was considered. For occupational exposure to chemicals, the explanatory variable. Bivariate and multivariate analyzes were developed using the Stata version 12.0 statistical program. Age, race/color and region of residence were significantly associated with a higher prevalence of negative self- rated oral health. Higher prevalence of negative self-assessment of oral health among individuals exposed to chemical products (PR=1.15[1.10-1.21] for males and PR=1.23[1.16-1.31] for the women). These associations remained significant in the multivariate model for both sexes. In conclusion, there is a higher prevalence of negative perception of oral health among workers exposed to handling chemical products in the workplace.

RESUMO

AIMS: To investigate the factors associated with poor oral health-related quality of life (OHRQoL) in a sample of Brazilian older adults. METHODS AND RESULTS: A cross-sectional study was conducted with 535 non-institutionalized elders aged 60 years or older from Piracicaba, São Paulo, Brazil. OHRQoL was measured using the Geriatric Oral Health Assessment Index (GOHAI). Data on sociodemographic characteristics, self-perceived general health status, and health-related behaviors were obtained through a structured questionnaire. Data on chronic diseases were obtained from health records. Associations between exploratory factors and low OHRQoL (% GOHAI score <30) were evaluated using multivariate Poisson regression models to estimate adjusted prevalence ratios (PRs) and confidence intervals. The mean OHRQoL score was 30 (± 4.4). In bivariate analysis, being not married, smoking, and self-rated "fair/poor" general health status were associated with lower OHRQoL. In the adjusted model, self-rated "fair/poor" general health (PR: 1.25; 95% CI: 1.05-1.48), presence of chronic diseases (PR: 1.88; 95% CI: 1.37-2.58), smoking (PR: 1.25; 95% CI: 1.02-1.53), and reason for last dental appointment (PR: 1.34; 95% CI: 1.13-1.59) were associated with poor OHRQoL. CONCLUSION: Non-institutionalized older adults with a history of chronic diseases, who smoke, have a negative perception of their general health, and had the last dental appointment motivated by pain present significantly higher prevalence rates of poor OHRQoL.

RESUMO

INTRODUCTION: The coronavirus disease 2019 (COVID-19) pandemic has created a new scenario for dental practice, and teledentistry (TD) is a field of relevance for dentists. OBJECTIVE: To evaluate the impact of the COVID-19 pandemic on the knowledge, practices, and expectations of dentists toward TD and its association with sociodemographic variables. METHODS: A cross-sectional survey was administered to dentists in Colombia as part of global research on the impact of COVID-19 on dentistry. The core questionnaire included 5 additional questions on TD that were added by the Colombian research team in 5 domains about TD: Q1, knowledge; Q2, practices; Q3, expectations; Q4, usefulness; and Q5, reimbursement. Sociodemographic variables were also measured. The questionnaire was validated in a pilot test and administered via digital media. Descriptive and bivariate analysis for sociodemographic and TD variables were done. For the evaluation of the chances of a determined answer, an ordinal multinomial logistic regression (Q1 and Q2) and binomial logistic regression (Q3 and Q5) analyses were made. RESULTS: In total, 5,370 dentists answered the survey (women: 3,878; mean age: 45 y). The response rate of the survey was 16.84%. Knowledge and practice of TD before the COVID-19 pandemic were both low (25.75% and 12.42%, respectively); however, since the beginning of the current pandemic, knowledge increased by 62.72% and practice by 42.53%. Some 93.1% of respondents considered that TD could be useful in their clinical practice, and 59.55% would consider practicing it after the pandemic. A high number of dentists (88.58%) did not charge for these services. Logistic regressions showed significant differences according to sociodemographic variables. CONCLUSIONS: The COVID-19 pandemic has had a great impact on TD, increasing knowledge and practice significantly. Dentists expect to continue practicing TD in the future. However, some geographical, technological, and attitudinal barriers need to be broken down to increase daily use of TD. KNOWLEDGE TRANSFER STATEMENT: This article provides an overview about the impact of COVID-19 pandemic in the knowledge and attitudes about teledentistry (TD) in general dentists and both clinical and nonclinical specialists. Although some barriers to the use of TD continue, this pandemic brought with it a new scenario for dental practice, which must be implemented through training for professionals to increase its use.

RESUMO

OBJECTIVES: The aim of this study was to assess the structural validity of the Oral Health Impact Profile-14 (OHIP-14) instrument in Indigenous and non-Indigenous populations from Australia and Brazil using a network analysis approach. METHODS: Cross-sectional data collected using OHIP-14 were obtained for Indigenous and non-Indigenous populations from Australia and Brazil. Networks were estimated using the Gaussian graphical model. Items of the OHIP-14 instrument are represented as nodes and the partial correlations between items as edges. Dimensionality was assessed using exploratory graph analysis. Structural consistency and item stability were computed using a bootstrap sampling method. Standardized node strength across each dimension was also calculated. RESULTS: Four dimensions were identified across all samples, although the item arrangement of most dimensions presented variation. Similarities with the theoretical domains of the instrument were found. Items from the conceptually derived OHIP-14 domains formed separated clusters or blended with other items in a single dimension. Most dimensions across all samples showed an acceptable structural consistency. Item stability revealed some discrepancies among items of dimensions of both Indigenous networks. CONCLUSION: The psychometric network perspective adopted in this study provides validation of the OHIP-14 structure in Indigenous and non-Indigenous populations. The structural consistency and item stability analyses showed that both Indigenous networks present a higher number of cross-domain items and less defined boundaries between dimensions. These findings indicate that OHIP-14 does not measure attributes in the same extent among different cultures. KNOWLEDGE TRANSFER STATEMENT: This study demonstrates a new analytical framework from which to conceptualize and interpret the construct oral health-related quality of life using the Oral Health Impact Profile (OHIP-14). Network graphs facilitate knowledge translation of findings to professionals with no expertise in psychometric methods. OHIP-14 is a valuable tool to oral health research and clinical practice. Differences in conceptions of health may influence the extent that the instrument measures oral health impacts. Consequently, dimension scores do not always provide appropriate measures and should be avoided in research reports and assessments of treatment outcomes.

The study aimed to analyze strategies for the implementation of the Brazilian National Oral Health Policy (PNSB) and its influence on oral health conditions in state capitals in the first decade of the 21st century. This is a policy assessment study. Data on oral health conditions were obtained from the databases of the Brazilian National Oral Health Surveys in 2003 and 2010 (SBBrasil 2003 and SBBrasil 2010), calculating the annual percent change in variables pertaining to oral health conditions as dependent variables for Brazilian state capitals. The PNSB was analyzed according to its conceptual foundations: oral healthcare model, organization of the supply of oral health services, implementation of oral health promotion strategies, and existence of fluoridation of the public water supply. Data were obtained from the national health databases and face-to-face interviews with oral health coordinators in 13 state capitals. The analyses were controlled and adjusted by the target population's socioeconomic conditions. An association was observed between the characteristics of oral health promotion, services supply, and model of care, indicating that the public services' dynamic is dependent on the management priorities and the policy's conduction. In conclusion, the PNSB is implemented differently in Brazil's state capitals according to the ways its guidelines are conducted in each capital and whether it favors best practices and strategies in oral healthcare. However, the influence of the PNSB is still not clear on the modification of oral health indicators from 2003 to 2010.

Abstract: The objective was to analyze the periodontal condition severity and the occurrence of pathogenic microorganisms in the oral cavity of an adult population of an Afrodescent Community of northeastern Brazil. This is an observational and cross- sectional study performed through an oral clinical examination, using a standardized clinical record. For the subjects with periodontal disease, the bacterial biofilm was collected in a Petri dish containing 0.9% physiological solution to detect the presence of microorganisms Entamoeba gingivalis and Trichomonas tenax, and later observed under an optical microscope. Statistical analysis was performed by calculating the prevalence of periodontal disease and the frequency of the protozoa in the bacterial biofilm. Statistical significance of the relationships researched was verified by Fisher's exact test. It was evaluated 29 subjects pertaining to the Quilombola Patioba community, aged 35 to 44 years. The results showed that among the adults of the community, there was a high prevalence of periodontal disease (75.86%), being higher in the 1st and 6th sextants of the Community Periodontal Index (CPI). E. gingivalis positivity occurred in most sextants affected by gingivitis, while in the condition of periodontitis, this microorganism was not present in the 3rd, 4th and 6th sextants. In all sextants affected by periodontal disease, T. Tenax was observed when associated with gingivitis. It is worth mentioning the begging of the elaboration of health policies, social and professional commitment that foster a greater promotion of oral health and quality of life for the quilombolas of northeastern Brazil.

This study analyzes associated factors to self-perceived oral health and use of oral health services in native and immigrant adults from the PELFI cohort in Spain. A cross-sectional analysis was conducted (401 adults ≥ 18 years, from Spain, Ecuador, Colombia and Morocco). Frequencies for sociodemographic, self-perceived general and oral health variables were calculated. The association between oral health/oral health services use and origin country was estimated by logistic regression (adjusted odds ratio-aOR-; 95% confidence intervals -95%CI-). Ecuadorian men were more likely to report dental caries (aPR 2.75; 95%CI 1.30-5.80) and Moroccan women were more likely to report gingival bleeding (aPR 3.61; 95%CI 1.83-7.15) and the use of oral health services ≥ 1 year/never (aPR 1.69; 95%CI 1.06-2.69). Colombian women were less likely to report missing teeth (aPR 0.73; 95%CI 0.56-0.95). Poor self-perceived oral health indicators were observed in immigrants and were modified for sociodemographic and general health variables.

RESUMO

Abstract Objective: To determine the prevalence and socio-behavioural risk factors for dental caries among children at selected LGAs in Lagos State. Material and Methods: This was a descriptive study of 592 school children in four Local Government Areas of Lagos, Nigeria. The presence of caries was recorded using the World Health Organization criteria. Descriptive statistics were reported for analysis of comparative DMFT and SiC scores in relation to age, gender, and other socio-demographic variables. Logistic regression analysis was used to analyze the differential impact of the variables on the probability of being in the high caries prevalence group. Results: The prevalence of dental caries was 16.0% with mean dmft for age 6 being 1.3 ± 1.57 while the mean DMFT for age 12 was 0.15 ± 0.67. The mean Sic for age 6 was 1.5 ± 0.53 while the mean SiC for age 12 was 1.09 ± 0.29. The mean SiC values was significantly higher in the primary and permanent dentition among those who had never visited the dentist, female students, those who don't use fluoridated toothpaste and those who eat sweets and candy several times a day. After logistic regression analysis, those with no previous dental visit (OR=3.05; CI: 1.72-4.67) and females (OR=1.55; CI: 1.16-1.62) still had significantly higher SiC Values. Conclusion: The prevalence of caries was low in the study population. Being female, non-use of fluoride-containing toothpaste and not visiting the dentist were significant predictors of dental caries among children attending private schools.

RESUMO

Abstract Objective: To determine whether psychosocial status is associated with orthodontic treatment needs in high school students using the Psychosocial Impact of the Dental Aesthetic Questionnaire (PIDAQ) and the Index of Orthodontic Need (IOTN). Material and Methods: This cross-sectional study comprised 95 adolescents, both genders, aged 15-17 years old. PIDAQ was used to assess psychosocial status. The IOTN Aesthetic Component (IOTN-AC) and the IOTN Dental Health Component (DHC) was used to assess treatment needs. Examination of the oral cavity was carried out with the help of the IOTN ruler by observing the MOCDO components: missing teeth, overjet, crossbite, displacement, and overbite. The Chi-square test was used to determine the association of psychosocial status and orthodontic treatment need. The reliability test was carried out by using Cronbach's Alpha test (0.978). Results: There was an association between psychosocial status and orthodontic treatment need based on the IOTN-AC (p=0.001) and between psychosocial status and orthodontic treatment need based on the IOTN-DHC (p=0.140). Conclusion: There is an association between psychosocial status based on the PIDAQ and subjective orthodontic treatment need based on the IOTN-AC, but there is no association between psychosocial status based on the PIDAQ and objective orthodontic treatment need based on the IOTN-DHC in students of SMAN 27 Jakarta.

RESUMO

Abstract Relevant public policies in oral health have been implemented in Brazil since 2004. Changes in the epidemiological status of dental caries are expected, mainly in the child population. This study aimed to assess the dental caries experience and associated factors among 12-year-old children in the state of São Paulo, Brazil. A cross-sectional population-based study was conducted with data obtained from 415 cities, including a total of 26,325 schoolchildren who were included by the double-stage cluster technique, by lot and by systematic sampling. The statistical model included data from the São Paulo Oral Health Survey (SBSP 2015), the Human Development Index (HDI) and the Social Vulnerability Index (SVI). A theoretical-conceptual model categorized the variables into three blocks, namely, contextual (HDI, SVI, region of residence and fluoridation of water), individual (sex and ethnicity) and periodontal conditions (gingival bleeding, dental calculus and the presence of periodontal pockets), for association with the experience of caries (DMFT). Statistically significant associations were verified by hierarchical multivariate logistic (L) and Poisson (P) regression analyses (p < 0.05). The results showed that 57.7% of 12-year-old children had caries experience. Factors that determined a greater prevalence of dental caries in both models were nonwhite ethnicity (ORL = 1.113, ORP = 1.154) and the presence of gingival bleeding (ORL = 1.204, ORP = 1.255). Male children (ORL = 0.920 ORP = 0.859) and higher HDI (ORL = 0.022), ORP = 0.040) were associated with a lower prevalence of dental caries experience. Water fluoridation was associated with a lower DMFT index (ORP = 0.766). Dental caries experience is still associated with social inequalities at different levels. Policymakers should direct interventions towards reducing inequalities and the prevalence of dental caries among 12-year-old children.
