The effectiveness of a visceral leishmaniasis (VL) control strategy based on the application of 4 % deltamethrin impregnated collars (DIC) exclusively in seropositive dogs was assessed between 2018 and 2019, through a prospective study. The effectiveness of DIC-collaring was evaluated by comparing the incidence rate of anti-leishmanial antibodies among dogs from two endemic districts in Brazil. In one of the areas, the conventional control measure which is based on the non-compulsory euthanasia of LV seropositive dogs, was practiced by the official healthy service as a regular procedure, whereas strategic collaring, conceived in this study, was carried out in the other. Results of serological tests applied to serum samples collected from all domiciled dogs were evaluated in three consecutive times, spaced by around 200 days. Incidence rates of VL seroreactivity were compared between districts in the same period of time as well as within the same district, in consecutive periods. Based on the results, the risk of infection in the population under conventional control measure was up to four times higher than the risk of infection where DIC-collaring was used. The strategic use of collar proposed here emerged as a promising measure for VL control in dogs from endemic areas. Strategic collaring does not rely on the euthanasia of infected animals, an extremely controversial procedure, and instead of being used in all dogs, as collaring is normally recommended; only seropositive dogs are intervened. Strategic use of DIC has the potential to drastically reduce costs, if compared to mass collaring canine population.

This work is focused on developing a self-calibration algorithm for an orientation estimation of cattle movements based on a quaternion Kalman filter. The accelerometer signals in the earth's frame provide more information to confirm that the cow is performing a jump to mount another cow. To obtain the measurements in the earth's frame, we propose a self-calibration method based on a strapdown inertial navigation system (SINS), which does not require intervention by the user once deployed in the field. The self-calibration algorithm uses a quaternion-based Kalman filter to predict the angular orientation with bias correction, and update it based on the measurements of accelerometers and magnetometers. The paper also depicts an alternate update to adjust the inclination using only the accelerometer measurements. We conducted experiments to compare the accuracy of the orientation estimation when the body moves similarly to cow mount movements. The comparison is between the proposed self-calibration algorithm with the IvenSense MPU9250 and Bosch BNO055 and the quaternion attitude estimation provided in the BNO055. The auto-calibrating algorithm presents a mean error of 0.149 rads with a mean consumption of 308.5 mW, and the Bosch algorithm shows an average error of 0.139 rads with a mean consumption of 307.5 mW. When we executed this algorithm in an MPU9250, the average error was 0.077 rads, and the mean consumption was 277.7 mW.

Botrytis cinerea is a necrotrophic fungus characterized mainly by its wide host range of infected plants. The deletion of the white-collar-1 gene (bcwcl1), which encodes for a blue-light receptor/transcription factor, causes a decrease in virulence, particularly when assays are conducted in the presence of light or photocycles. However, despite ample characterization, the extent of the light-modulated transcriptional responses regulated by BcWCL1 remains unknown. In this study, pathogen and pathogen:host RNA-seq analyses, conducted during non-infective in vitro plate growth and when infecting Arabidopsis thaliana leaves, respectively, informed on the global gene expression patterns after a 60 min light pulse on the wild-type B05.10 or ∆bcwcl1 B. cinerea strains. The results revealed a complex fungal photobiology, where the mutant did not react to the light pulse during its interaction with the plant. Indeed, when infecting Arabidopsis, no photoreceptor-encoding genes were upregulated upon the light pulse in the ∆bcwcl1 mutant. Differentially expressed genes (DEGs) in B. cinerea under non-infecting conditions were predominantly related to decreased energy production in response to the light pulse. In contrast, DEGs during infection significantly differ in the B05.10 strain and the ∆bcwcl1 mutant. Upon illumination at 24 h post-infection in planta, a decrease in the B. cinerea virulence-associated transcripts was observed. Accordingly, after a light pulse, biological functions associated with plant defense appear enriched among light-repressed genes in fungus-infected plants. Taken together, our results show the main transcriptomic differences between wild-type B. cinerea B05.10 and ∆bcwcl1 after a 60 min light pulse when growing saprophytically on a Petri dish and necrotrophically over A. thaliana.

Rodent self-grooming is a stereotyped behavior that can rise due to stressors such as novelty. In the present study, the occurrence of a rebound effect was investigated by means of manipulation of contextual novelty and of the possibility of self-grooming (with an Elizabethan collar which blocked head-body contact). Fourty-six male rats were submitted to an experiment latter replicated with 43 females. Half of the animals were submitted to habituation sessions (30 min) to the test box in three days. The other half was similarly handled but habituated to small cages. On the fourth day, rats from both conditions were assigned to Elizabethan or sham collar groups. Each animal was observed for 15 min with its respective collar and for other 15 min with no collars. Increased locomotion was observed in the rats not habituated to the test box. Such habituation did not affect any grooming parameter. On the other hand, the animals wearing Elizabethan collars during the first half of the test, as compared to those which wore sham collars, showed increased self-grooming during the second half of the test (i.e., with no collars). Females showed pretty similar results. Present results, thus, evidence a rebound effect in self-grooming.

The parasitic protozoan Giardia intestinalis, the causative agent of giardiasis, presents a stable and elaborated cytoskeleton, which shapes and supports several intracellular structures, including the ventral disc, the median body, the funis, and four pairs of flagella. Giardia trophozoite is the motile form that inhabits the host small intestine and attaches to epithelial cells, leading to infection. The ventral disc is considered one important element of adhesion to the intestinal cells. It is adjacent to the plasma membrane in the ventral region of the cell and consists of a spiral layer of microtubules and microribbons. In this work, we studied the organization of the cytoskeleton in the ventral disc of G. intestinalis trophozoites using high-resolution scanning electron microscopy or helium ion microscopy in plasma membrane-extracted cells. Here, we show novel morphological details about the arrangement of cross-bridges in different regions of the ventral disc. Results showed that the disc is a non-uniformly organized structure that presents specific domains, such as the margin and the ventral groove region. High-resolution scanning electron microscopy allowed observation of the labeling pattern for several anti-tubulin antibodies using secondary gold particle-labeled antibodies. Labeling in the region of the emergence of the microtubules and supernumerary microtubules using an anti-acetylated tubulin antibody was observed. Ultrastructural analysis and immunogold labeling for gamma-tubulin suggest that disc microtubules originate from a region bounded by the bands of the banded collar and merge with microtubules formed at the perinuclear region. Actin-like filaments and microtubules of the disc are associated, showing an interconnection between elements of the cytoskeleton of the trophozoite.

Objective. The maintenance of good work ability has proven to have a positive effect on health, quality of work and productivity. There is little evidence regarding the association between musculoskeletal complaints, individual and work-related factors, and work ability in blue-collar and white-collar workers. Methods. A cross-sectional study of hospital and university workers (n = 360) was performed to determine the association between musculoskeletal complaints, individual and work-related factors, and work ability; two multiple linear regression models were created. Results. In white-collar workers, musculoskeletal complaints of the dorsal/lumbar region (ß = -0.259) and the wrist/hand segment (ß = -0.151) were significantly associated with reduced work ability. A higher body mass index (ß = -0.227) was also negatively associated with the dependent variable. In blue-collar workers, musculoskeletal complaints of the dorsal/lumbar region (ß = -0.317) and the elbow/forearm segment (ß = -0.171) were significantly associated with lower work ability. Increased age (ß = -0.204) was associated with reduced work ability in this group. No significant correlations were found between work-related factors and work ability. Conclusions. Musculoskeletal complaints and individual factors, but not work-related factors, are associated with work ability in Chilean blue-collar and white-collar workers. Nevertheless, the variables that explain the change in work ability are different between these groups.

BACKGROUND: This research work establishes the relationship between job strain and being overweight among Mexican managers. Recently in Mexico, there has been a sharp increase in work-related diseases and mental health disorders. Furthermore, evidence shows that Mexicans rank top among employees who suffer from stress, yet research on the impact of job strain on the phenomena of obesity and being overweight among such vulnerable job positions in the industrial field is scarce. METHODS: The sample included 170 overweight middle and senior managers from six companies in the Mexican Manufacturing Industry. Cedillo's Spanish version of the Job Content Questionnaire by Karasek was used, and the Body Mass Index (BMI) was used to characterize an overweight condition. Structural Equations Modelling studied the relationships among variables. RESULTS: Even though, the model shows a power of explanation of 6%(R2â=â0.06), the variable showing the greatest direct effect on the overweight variable is social support, with 21%(pâ<â0.01, ß=â-0.21). Regarding the total effects, only two of the four variables studied contributed directly to the overweight variation: the social support variable and the job demand variable. CONCLUSIONS: The results of the model hold a relatively low explanatory power; however, they do show a relationship between the studied variables. Also, the importance of the supervisor and co-workers' support should be considered when developing organizational strategies for the prevention of work stress and an overweight condition.

Dogs play a major role in public health because of potential transmission of zoonotic diseases, such as rabies. Dog roaming behavior has been studied worldwide, including countries in Asia, Latin America, and Oceania, while studies on dog roaming behavior are lacking in Africa. Many of those studies investigated potential drivers for roaming, which could be used to refine disease control measures. However, it appears that results are often contradictory between countries, which could be caused by differences in study design or the influence of context-specific factors. Comparative studies on dog roaming behavior are needed to better understand domestic dog roaming behavior and address these discrepancies. The aim of this study was to investigate dog demography, management, and roaming behavior across four countries: Chad, Guatemala, Indonesia, and Uganda. We equipped 773 dogs with georeferenced contact sensors (106 in Chad, 303 in Guatemala, 217 in Indonesia, and 149 in Uganda) and interviewed the owners to collect information about the dog [e.g., sex, age, body condition score (BCS)] and its management (e.g., role of the dog, origin of the dog, owner-mediated transportation, confinement, vaccination, and feeding practices). Dog home range was computed using the biased random bridge method, and the core and extended home range sizes were considered. Using an AIC-based approach to select variables, country-specific linear models were developed to identify potential predictors for roaming. We highlighted similarities and differences in term of demography, dog management, and roaming behavior between countries. The median of the core home range size was 0.30 ha (95% range: 0.17-0.92 ha) in Chad, 0.33 ha (0.17-1.1 ha) in Guatemala, 0.30 ha (0.20-0.61 ha) in Indonesia, and 0.25 ha (0.15-0.72 ha) in Uganda. The median of the extended home range size was 7.7 ha (95% range: 1.1-103 ha) in Chad, 5.7 ha (1.5-27.5 ha) in Guatemala, 5.6 ha (1.6-26.5 ha) in Indonesia, and 5.7 ha (1.3-19.1 ha) in Uganda. Factors having a significant impact on the home range size in some of the countries included being male dog (positively), being younger than one year (negatively), being older than 6 years (negatively), having a low or a high BCS (negatively), being a hunting dog (positively), being a shepherd dog (positively), and time when the dog was not supervised or restricted (positively). However, the same outcome could have an impact in a country and no impact in another. We suggest that dog roaming behavior is complex and is closely related to the owner's socioeconomic context and transportation habits and the local environment. Free-roaming domestic dogs are not completely under human control but, contrary to wildlife, they strongly depend upon humans. This particular dog-human bound has to be better understood to explain their behavior and deal with free-roaming domestic dogs related issues.

The effect of employing collars impregnated with deltamethrin 4% (DM4) to control canine visceral leishmaniasis (CVL) was evaluated. as were the individual factors associated with this infection. A cohort study that included household dogs was conducted between 2002 and 2006. The presence of pathognomonic signals, peridomiciliary sleep habits and breed were the main factors associated with the infection. The use of DM4 collars contributed to the reduction of CVL with an effectiveness of 66%, and the dogs' survival rate was greater than 90% at 50 months. In conclusion, the adoption of DM4 collars reduced the number of euthanized canines and in the incidence of CVL, and this reduction was sustained for one year after discontinuing the use of the collar.

The largest feline in the Americas and the third largest in the world, the jaguar is an apex predator in the food chain and a key species in the ecosystems where it occurs, developing important ecological functions in maintaining ecosystem balance. In Brazil, the Pantanal is considered an important refuge for the species, and protected areas such as the Taiamã Ecological Station (TES) are relevant for conservation of pristine ecosystem where the species persist. Thus, considering that this area is located in one of the regions with the highest concentration of this large cat in the Pantanal, the objective of this study was to evaluate the effectiveness of the protection given by the TES for the jaguar population. The station is a flooded island in the middle of the Paraguay river and surrounded by extensive wetlands. We monitored ten jaguars using GPS collar at the TES and its surroundings. The samples were separated into high-water season (January to June) and low-water season (July to December), and the estimated home ranges were grouped as: 1) residents only and 2) all monitored individuals. The stabilization of the home ranges of eight jaguars, considered residents, was determined through variograms. When all jaguars were analysed together there was 55% overlap between the clustered areas of the two analysed seasons. In the analysis excluding non-resident individuals there was 72% overlap between the clustered areas. The type of land cover inside these areas was very similar between these periods. The range of this protected area is not sufficient to effectively protect these jaguar populations, since the grouped home ranges of the resident animals studied are 3.5(wet)/2.5(dry) times larger than the area of the TES. ANTECEDENTES: Las unidades de Cardiología intervencional han evidenciado un número creciente de procedimientos, cada vez más variados y complejos, lo cual podría eventualmente generar daños a los profesionales ocupacionalmente expuestos a radiaciones ionizantes (POEs) de no contar con los adecuados elementos de radioprotección y un uso correcto de ellos. OBJETIVO: Caracterizar la disponibilidad y utilización de los elementos de radioprotección y dosimetría de unidades de cardiología intervencionista de centros Sudamericanos. MATERIAL Y MÉTODOS: Se realizó una encuesta autoaplicada a 139 POEs, de ambos sexos de 7 países, a través de una plataforma on-line, se les consultó sobre características demográficas, dosimétricas y de radioprotección. RESULTADOS: Los elementos de radioprotección más tradicionales; delantales y cuellos plomados se utilizaron un 99,5 % y 98,4 % respectivamente, aquellos elementos más recientes como gafas, gorros y paños plomados solo alcanzaron un 36,8 %, 6,8 % y 34,2%, de utilización respectivamente, en cuanto a la utilización de los dosímetros, solo un 7,9 % lo hace apegado a las normas de la Organización Internacional de Energía Atómica (OIEA). CONCLUSIÓN: Se constató en la muestra analizada una falta de elementos de radioprotección y un uso inadecuado de ellos, urge realizar intervenciones educativas y técnicas para mejorar estos datos.
BACKGROUND: Due to the increasing number of interventional cardiology procedures currently performed, health professionals (POE) are exposed to ionizing radiation unless adequate protective elements are used. AIM: to describe the use of radioprotection elements and dosimetry in interventional cardiology laboratories in South America. METHODS: A self-administered survey was performed on 139 POE of both sexes and 7 countries using an online platform. Demographic data, dosimetry and characteristics of radioprotection analyzed. RESULTS: Commonly used radioprotective elements (lead aprons and collars) were used in 99.5% and 98% respectively. Recently introduced protection elements like lead goggles, caps and drapery were used in 36.8, 6.8 and 34.2% , respectively. Dosimetry according to the International Atomic Energy Commission (OIAE) was performed in only 7.9% of the procedures. CONCLUSION: there is a severe lack of adequate radioprotection during interventional cardiology procedures. Urgent measures, including technical implementation and educational interventions are needed to improve radioprotection in interventional cardiology.

However, the surroundings of the TES have low human impact and no deforestation and, in this way, the landscape context of the station may have a significant effect on the persistence of a large carnivore like the jaguar. However, it is necessary to create more protected areas in order to prevent possible future environmental changes in the areas surrounding TES.

We have described that formation of basidiocarps by Ustilago maydis requires illumination. In the current research, we have proceeded to analyze what kind of light receptors are involved in this phenomenon. Accordingly, we investigated whether the homologues of the White Collar (WC), and the phytochrome (PHY) genes played a role in this process. Mutants deficient in either one of the three U. maydis WC homologue genes (WCO1a, WCO1b, WCO2), or the phytochrome-encoding the PHY gene were obtained. Phenotypic analysis of the mutants showed that ∆wco1a mutants formed similar numbers of basidiocarps than wild-type strain, whereas ∆wco1b mutants were severely affected in basidiocarp formation when illuminated with white, blue or red light. ∆wco2 and ∆phy1 mutants did not form basidiocarps under any illumination condition. These data indicate that Wco1a is the main blue light receptor, and Wco1b may operate as a secondary blue light receptor; Phy1 is the red light receptor, and Wco2 the transcription factor that controls the photo stimulation of the genes involved in the formation of fruiting bodies. It is suggested that effectiveness of the light receptors depends on the whole structure of the complex, possibly, because their association is necessary to maintain their functional structure.

BACKGROUND: Spinal cord injury (SCI) is present in around 2-4% of trauma victims. More than half of this injuries are located at the cervical region. Twenty percent of victims with cervical spinal trauma and 5% of patients with severe traumatic brain injury (TBI) will have an SCI. Cervical immobilization with rigid or semirigid collars is routinely used as prophylactic or definitive treatment intervention in general trauma care. An important adverse effect of cervical collars application is the increase in intracranial pressure (ICP) values. This systematic review and meta-analysis aim to estimate the overall magnitude of ICP changes after cervical collar application. METHODS: Major electronic databases (Ovid/Medline, Embase and Cochrane Library) were systematically searched for prospective studies that assessed ICP changes after cervical collar applications. Study level characteristics and ICP values before, during and after cervical collar application, were extracted. The meta-analysis was performed using random-effects model. RESULTS: Five studies comprising 86 patients were included in the systematic review and the quantitative synthesis. The overall increase in ICP after collar application was statistically significant (weighted mean difference [WMD] = 4.43; 95%CI 1.70, 7.17; P < 0.01), meaning an overall ICP increase of approximately 4.4 mmHg. The decrease in ICP values after collar removal reached statistical significance (WMD = - 2.99; 95%CI - 5.45, - 0.52; P = 0.02), meaning an overall ICP decrease of approximately 3 mmHg after collar removal. ICP values before and after cervical collar application were not statistically significant (WMD = 0.49; 95%CI - 1.61, 2.59; P = 0.65), meaning no ICP change. CONCLUSIONS: Heterogeneous studies of application of cervical collars as a partial motion restriction strategy after injuries have demonstrated increases in ICP in TBI patients. Increases in ICP can induce complications in TBI patients. Appropriate selection criteria for cervical motion restriction in TBI patients need to be considered.

Background: The blood pressure responses to baroreflex perturbations can be assessed only using the variable-pressure neck chamber technique. However, the application of this approach in hospital environments is limited owing to the loud noise emitted during its operation. This study was aimed at developing a noiseless neck suction chamber device (NCD) that could stimulate the baroreceptors located in the carotid sinus in humans. Methods: A non-invasive device was developed to pressurize the carotid arteries externally. A microcontroller with a computer interface and neck chamber (3D-printed) was used. The anatomical neck chamber was fitted on six healthy, young, asymptomatic participants (five men; 32 ± 6 year), who were normotensive, nonsmoking, in sinus rhythm, free of known cardiovascular or metabolic diseases, and not consuming any acute or chronic medications. A suction of -60 mmHg was applied for 5 s, and the corresponding data were recorded. Before each study visit, the participants were instructed to abstain from caffeine, alcohol, and strenuous exercise for 12-24 h. Results: In all the trials, a significant reflex bradycardia (-10 ± 2 bpm) and depressor response (-15 ± 4 mmHg) to neck suction were observed, consistent with the results in the literature. The neck chamber device operated noiselessly [sound pressure level (SPL) of 34.3 dB] compared to a regular vacuum-cleaner-based system (74.6 dB). Conclusion: Using the proposed approach, consistent blood pressure and heart rate responses to carotid baroreflex hypertensive stimuli could be recorded, as in previous studies conducted using neck collar devices. Furthermore, the neck chamber device operated noiselessly and can thus be applied in hospital environments.

ABSTRACT Introduction: Low back pain is a prevalent condition with health, social and occupational impact. The aim of this study is to assess the influence of social and occupational factors on worker patients with low back pain. Method: A descriptive study was conducted on 349 subjects with low back pain. The main independent variable was being a manual or non-manual worker. Other social-occupational and lifestyle variables were also considered. The level of disability was established according to the Oswestry low back pain disability questionnaire, and the association between the type of job and the test was evaluated using multiple linear regressions. Results: Manual workers are heavier smokers (47%), experience more pain (74.3%), have a lower economic status (89.3%), use more drugs (57.7%), have limitations in walking (17.5%) and standing (25.6%), used carrying protocols (85.5%), make repetitive movements (77.4%). They made trunk (52.6%) and upper limb movements (24.8%), and received less prevention training (51.7%), but used low back protection (19.6%). The non-manual workers had greater limitation in handling loads (37.4%) and sitting (43.5%), and computer screen user protocols were applied (94.8%), and had a lower social class classification (57.1 low-middle class). Performing manual tasks at work is significantly associated with an increase of 2 points in the Oswestry test compared to non-manual workers, when adjusted for age and gender (coefficient β: 2, 95% CI: 0.6-3.36). Conclusion: Low back pain is a prevalent condition, with an unfavorable prognosis that has an individual, social, and occupational impact. Performing manual tasks is associated with an increase in the disability scale, regardless of age and gender.
RESUMEN Introducción: El dolor lumbar es una dolencia prevalente con repercusión sanitaria, social y laboral. Es objetivo de este trabajo valorar la influencia de las variables sociolaborales en pacientes con lumbalgia que trabajan. Método: Estudio descriptivo en 349 sujetos con lumbalgia. Es variable independiente principal ser trabajador manual y no manual; también son recogidas otras variables sociolaborales y de estilo de vida. La incapacidad se obtuvo mediante la escala del cuestionario de Oswestry y la asociación entre el tipo de tarea y el test se evaluó con regresión lineal múltiple. Resultados: Los trabajadores manuales consumen más tabaco (47%), tienen más dolor (74,3%), peor situación económica (89,3%), consumen más fármacos (57,7%), tienen limitación en deambulación (17,5%) y bipedestación (25,6%), se les aplican protocolos de cargas (85,5%) y movimientos repetidos (77,4%), realizan movimientos de tronco (52,6%) y miembros superiores (24,8%) y reciben menor formación preventiva (51,7%), pero utilizan protección lumbar (19,6%). Los trabajadores no manuales tienen mayor limitación en cargas (37,4%) y sedestación (43,5%) se les aplican protocolos de usuarios de pantallas (94,8%) y tienen peor tipificación en clase social (57,1 clase media-baja). Realizar tareas manuales en el trabajo se asocia significativamente con un aumento de 2 puntos en el test de Oswestry respecto a los no manuales, ajustado por edad y género (coeficiente β: 2, IC 95%: 0,65-3,36). Conclusión: La lumbalgia es una dolencia prevalente, de curso desfavorable, que implica impacto individual, social y laboral. Realizar tareas manuales se asocia con un aumento en la escala de incapacidad, independientemente de la edad y el género.

Since the early 1970s, initial management of patients with suspected spinal injuries has involved the use of a cervical collar and long spine board for full immobilization, which was thought to prevent additional injury to the cervical spine. Despite a growing body of literature demonstrating the detrimental effects and questionable efficacy of spinal immobilization, the practice continued until 2013, when the National Association of EMS Physicians issued a position statement calling for a reduction in the use of spinal immobilization and a shift to spinal-motion restriction. This article examines the literature that prompted the change in spinal-injury management and the virtual elimination of the long spine board as a tool for transport.

INTRODUCCIÓN: La diabetes mellitus es un importante problema de salud pública, según la Organización Mundial de la Salud, 422 millones de adultos en todo el mundo en 2014. En 2012 provocó 1.5 millones de muertes a nivel mundial; es la primera causa de amputación no traumática en miembros inferiores y las infecciones del pie ocurren con alta frecuencia en pacientes mal controlados. Este caso muestra la reconstrucción de defectos de partes blandas en el tercio inferior pierna, talón, maléolos y pie a través de la descripción del colgajo sural reverso. En este estudio se discuten las ventajas y desventajas de la utilización de esta técnica. CASO CLÍNICO: Paciente de sexo femenino 56 años de edad con antecedentes de Diabetes Mellitus tipo 2 mal controlada, que sufrió una fractura de tobillo izquierdo tratada con osteosíntesis; durante el postoperatorio acudió a emergencia, por presentar dolor de gran intensidad en tobillo izquierdo, fiebre, dehiscencia de herida quirúrgica, acompañada de secreción serosa. EVOLUCIÓN: La paciente luego de varias limpiezas quirúrgicas y cambios de terapia de cierre asistido por presión negativa, se consigue controlar la infección, con persistencia del defecto de cobertura ósea y del material de osteosíntesis. Se presentaron complicaciones adicionales como la rigidez articular, atrofia muscular; se decidió realizar reconstrucción del tercio distal del pie usando colgajo fasciocutáneo sural de flujo reverso, que evolucionó satisfactoriamente, consiguiendo cubrir el defecto con tejido biológico. CONCLUSIÓN: El colgajo sural de flujo reverso, es una técnica reproducible, que permite cubrir defectos de cobertura en tercio distal de pierna y tobillo; acortar los tiempos de hospitalización, especialmente cuando hay exposición ósea o de material de osteosíntesis.
BACKGROUND: Diabetes mellitus is a major public health problem, according to the World Health Organization, 422 million adults worldwide in 2014. In 2012, it caused 1.5 million deaths worldwide. Diabetes is the leading cause of non-traumatic amputation in lower limbs and foot infections occur with high frequency in poorly controlled patients. This case shows the reconstruction of soft tissue defects in the lower third leg, heel, malleoli and foot through the description of the reverse sural flap. In this study the advantages and disadvantages of the use of this technique are discussed. CASE REPORT: A 56-year-old female patient with a poorly controlled history of Diabetes Mellitus type 2, who suffered a fracture of the left ankle treated with osteosynthesis, and who during the postoperative period attended emergency orthopedics and traumatology, due to severe pain in the left ankle, fever, dehiscence of surgical wound, accompanied by serous secretion. EVOLUTION: The patient, after several surgical cleanings and changes in closure therapy assisted by negative pressure, manages to control the infection, with persistence of the bone coverage defect and the osteosynthesis material. There were additional complications such as joint stiffness, muscle atrophy; it was decided to reconstruct the distal third of the foot using a reverse flow sural fasciocutaneous flap, which evolved satisfactorily, managing to cover the defect with biological tissue. CONCLUSIONS: The sural flap of reverse flow is a reproducible technique that allows to cover coverage defects in the distal third of the leg and ankle; shorten hospitalization times, especially when there is bone exposure or osteosynthesis material.
