OBJECTIVES: To systematically evaluate the efficacy and superiority of Flunarizine Hydrochloride when combined with Traditional Chinese Medicine (TCM) Decoctions in treating migraine headaches. METHOD: The authors conducted a comprehensive search for clinical Randomized Controlled Trials (RCTs) investigating the combination of Flunarizine Hydrochloride with Chinese herbal decoctions in treating migraines. The databases searched included CNKI, VIP, Wanfang, PubMed, WOI, Cochrane Library, and Embase, covering the period from January 1, 2019, to November 10, 2023. Two independent researchers meticulously screened, extracted, and assessed the relevant data, employing the Revman 5.3 software for meta-analysis. RESULTS: The meta-analysis revealed that, in comparison to Flunarizine Hydrochloride used in isolation, the combination with Chinese herbal decoctions markedly enhanced the effective rate (RR = 1.26, 95 % CI [1.18, 1.34], p < 0.0001). Moreover, significant improvements were observed in the TCM symptom score (MD = 4.97, 95 % CI [-6.74, -3.19], p < 0.00001). The observation group demonstrated a statistically significant improvement in endothelin levels compared to the control group (I2 = 85 %, MD = -13.66, 95 % CI [-17.87, -9.45], p = 0.0001). The observation group showed a significant reduction in NRS scores compared to the control group, indicating better outcomes (I2 = 95 %, MD = -2.11, 95 % CI [-3.09, -1.12], p < 0.0001). The observation group was superior to the control group in terms of the reduction in the number of episodes (I2 = 63 %, MD = -1.16, 95 % CI [-1.45, -0.87], p = 0.007). CONCLUSIONS: The confluence of Flunarizine Hydrochloride with traditional Chinese medicine decoctions in treating migraine patients demonstrated substantial clinical efficacy and improvement in TCM symptom score over the use of Flunarizine Hydrochloride alone.

OBJECTIVE: To investigate the mechanism of induction of ferroptosis by brazilin in breast cancer cells. METHODS: Breast cancer 4T1 cells were divided into 6 groups: control, brazilin 1/2 half maximal inhibitory concentration (IC50), IC50, 2×IC50, erastin (10 µg/mL) and capecitabine (10 µg/mL) groups. The effect of brazilin on the proliferation of 4T1 cells was detected by cell counting kit-8 assay, and the treatment dose of brazilin was screened. The effect of brazilin on the mitochondrial morphology of 4T1 cells, and the mitochondrial damage was evaluated under electron microscopy. The levels of Fe2+, reactive oxygen species (ROS), malondialdehyde (MDA), glutathione (GSH) and glutathione peroxidase 4 (GPX4) were estimated using various detection kits. The invasion and migration abilities of 4T1 cells were detected by scratch assay and transwell assay. The expressions levels of tumor protein p53, solute carrier family 7 member 11 (SLC7A11), GPX4 and acyl-CoA synthetase long-chain family member 4 (ACSL4) proteins were quantified by Western blot assay. RESULTS: Compared to the control group, the 10 (1/2 IC50), 20 (IC50) and 40 (2×IC50) µg/mL brazilin, erastin, and capecitabine groups showed a significant decrease in the cell survival rate, invasion and migration abilities, GSH, SLC7A11 and GPX4 protein expression levels, and mitochondrial volume and ridge (P<0.05), and a significant increase in the mitochondria membrane density, Fe2+, ROS and MDA levels, and p53 and ACSL4 protein expression levels (P<0.05). CONCLUSIONS: Brazilin actuated ferroptosis in breast cancer cells, and the underlying mechanism is mainly associated with the p53/SLC7A11/GPX4 signaling pathway.

INTRODUCTION: Cupping therapy is a widely used technique in Brazilian physical therapy for the treatment of musculoskeletal disorders. However, there is limited scientific evidence to support its effectiveness. OBJECTIVE: To investigate the profile, training, clinical practice, and scientific updates of Brazilian Physical Therapists who use cupping therapy as a therapeutic resource for musculoskeletal disorders. METHODS: A cross-sectional study was conducted through an online questionnaire, including 646 Physical Therapists who use cupping therapy in their practice. All data were analysed descriptively. RESULTS: Cupping therapy is a technique that has been widely adopted in clinical practice by Physical Therapists, particularly among young, female professionals who have recently graduated from private universities. The primary reason for interest in this technique among these Physical Therapists is the high demand from patients. Additionally, it is often used in conjunction with other manual therapeutic techniques. They identified easy access, low cost, and ease of use as the key factors that make cupping therapy an attractive option. However, a lack of high-quality scientific evidence, as described in the literature, was identified as a major barrier to its use. CONCLUSION: The Physical Therapists included in this study use cupping therapy in their clinical practice, relying heavily on their own experience and the preferences of their patients, rather than utilizing the third pillar of evidence-based practice, which is to rely on the best available evidence. This study suggests that these Physical Therapists are currently implementing a technique without current scientific recommendations for its use in the treatment of musculoskeletal disorders.

Background: Sickle-cell diseases (SCD) are a group of hereditary disorders in which a specific mutation in the gene that encodes the hemoglobin ß chain leads to formation of an anomalous hemoglobin molecule (HbS) with high polymerization power. This leads to sickling of erythrocytes in situations of low oxygen tension, such as in microcirculation, resulting in peripheral microvasculature occlusion, chronic hemolysis, inflammation, and damage to several target organs. Malleolar ulcers are among the most-debilitating complications of the disease, as they are associated with significant pain, secondary infections, and social impact due to their aesthetic impairment. There are no completely satisfactory therapeutic options for this complication; local healing agents, antibiotics, and dressings are used, with high rates of recurrence and complications, such as osteomyelitis and even limb amputation. Case: This case study evaluated the effect of Traditional Chinese Medicine techniques on chronic malleolar ulcers in a 49-year-old male patient. Ten sessions of systemic acupuncture (combinations involving Source, Master, Energetic Action, and Extraordinary Vessels points), auriculotherapy, and dressing with magnets were conducted. Results: Although the primary outcome sought was not reached (decrease in ulcer diameters), this patient had great reduction of local pain, a decrease in limb edema, and important reduction of his inflammatory condition, reflected in his decreasing blood levels of C-reactive protein. Conclusions: These results show that acupuncture should be considered as an important auxiliary treatment for SCD complications.

Löfgren syndrome (LS) is a unique acute manifestation of sarcoidosis and characterized by erythema nodosum, bilateral hilar lymphadenectasis, and/or bilateral ankle arthritis or periarthritis. A 37 - year - old female patient with LS presented with fever accompanied by multiple joint swelling and pain, nodular skin erythema, and bilateral hilar lymphadenectasis. The patient had received treatment involving non - steroidal anti - inflammatory drugs and glucocorticoids in other hospitals, but the effects were poor, and the conditions reemerged. The LS duration has lasted for more than 3 months. Following traditional Chinese medicine (TCM) treatment, syndrome differentiation as well as giving patients oral Chinese medicine decoction, the symptoms of the patient were rapidly relieved within one week and did not recur during a six - month follow - up period. This case is the first clinical report of acute sarcoidosis LS treated using T CM and reflects the significant advantages of this form of therapy in emergency treatment


El síndrome de Löfgren (LS) es una manifest ación única y aguda de sarcoidosis, caracterizada por eritrema nodoso, linfadenectasis hilar bilateral, y/o a r tritis de tobillo bilateral o periartritis. Una paciente de 37 años de sexo femenino con LS se presentó con fiebre, acompañada de inflamación y do lor múltiple de articulaciones, eritrema nodular cutáneo, y linfadenectasis hilar bilateral. La paciente recibió un tratamiento que consistió en antiinflamatorios no esteroidales y glucocorticoides en otros hospitales, pero los efectos fueron leves y las c ondiciones reemergieron. El LS ha durado más de tres meses. Siguiendo el tratamiento de medicina tradicional china (MTC), la diferenciación de síndrome, así como darles a los pacientes una decocción de medicina china por vía oral, los síntomas de la pacien te rápidamente fueron aliviados en el curso de una semana y no recidivaron durante los seis meses de un seguimiento. El caso es el primer reporte clínico de tratamiento de sarcoidosis aguda asociada a LS usando TCM y refleja las significativas ventajas de esta forma de terapia en el tratamiento de emergencia.

ETHNOPHARMACOLOGICAL RELEVANCE: Cervical cancer is one of the most common malignancies in women that continues to be a public health problem worldwide. Human papillomavirus (HPV) infection is closely related as the causative agent of almost all cases of cervical cancer. Currently, there is no effective treatment for the persistence of HPV. Although vaccines have shown promising results in recent years, they are still a costly strategy for developing countries and have no therapeutic effect on existing infections, which is why the need arises to search for new strategies that can be used in treatment, suppressing oncogenic HPV and disease progression. Extracts of Schisandra Chinensis and Pueraria lobata have been used in traditional medicine, and it has been shown in recent years that some of their bioactive compounds have pharmacological, antioxidant, antitumor, apoptotic, and proliferation effects in HPV-positive cells. However, its mechanism of action has yet to be fully explored. AIM OF THE STUDY: The following study aimed to determine the chemical composition, antioxidant activity, and potential antiproliferative and viral oncogene effects of natural extracts of S. chinensis and P. lobata on HPV-18 positive cervical cancer cells. MATERIALS AND METHODS: The HPV-18-positive HeLa cells were treated for 24 and 48 h with the ethanolic extracts of S chinensis and P. lobata. Subsequently, cell viability was evaluated using the resazurin method, the effect on the cell cycle of the extracts (1.0, 10, and 100 µg/mL) was measured by flow cytometry, the gene of expression of the E6/E7, P53, BCL-2, and E2F-1 were determined by RT-PCR and the protein expression of p53, Ki-67, x|and Bcl-2 by immunohistochemistry. Additionally, the chemical characterization of the two extracts was carried out using LC-MS, and the total phenolics content (TPC), Total flavonoid content (TFC), and DPPH radical scavenging capacity were determined. Data were analyzed using the Mann-Whitney and Kruskal Wallis U test with GraphPad Prism 6 software. RESULTS: The natural extracts of Schisandra chinensis and Pueraria lobata induced down-regulation of E6 HPV oncogene (p<0.05) and a strong up-regulation of P53 (p<0.05), E2F-1 (p<0.05), and Bcl-2 (p<0.05) gene expression. Simultaneously, the natural extracts tend to increase the p53 protein levels and arrest the cell cycle of HeLa in the G1/S phase (p<0.05). Investigated extracts were characterized by the occurrence of bioactive lignans and isoflavones in S. chinensis and P. lobata, respectively. CONCLUSION: The extracts of S. chinensis and P. lobata within their chemical characterization mainly present lignan and isoflavone-type compounds, which are probably responsible for inhibiting the expression of the HPV E6 oncogene and inducing an increase in the expression of p53, Bcl -2 and E2F-1 producing cell cycle detection in S phase in HeLa cells. Therefore, these extracts are good candidates to continue studying their antiviral and antiproliferative potential in cells transformed by HPV.

Resumo O Fórum para a Cooperação Econômica e Comercial entre a China e os Países de Língua Portuguesa, mais conhecido como Fórum de Macau, fundado em 2003, atua há 20 anos como elo entre a China e os países lusófonos. Desde 2016, o Secretariado Permanente do Fórum de Macau publica um anuário destacando as principais atividades realizadas pelo mesmo no ano anterior. Este artigo tem como foco analisar o papel do Fórum de Macau como organizador e mediador de iniciativas de Saúde Global através da análise de todos os artigos publicados em seus anuários entre 2016 e 2023. Foram encontradas 34 ocorrências de artigos tratando de algum tema relacionado à área da saúde, com quase dois terços relacionados à medicina tradicional chinesa, relatos de organização frequente de atividades que fomentam a cooperação internacional em saúde, além de uma ativa participação durante o primeiro ano da pandemia de Covid-19. O Fórum de Macau é uma plataforma multilateral de diálogo entre a China, Macau e o mundo de fala portuguesa, tendo a pauta da saúde e a divulgação da medicina tradicional chinesa ocupado um espaço de destaque em seu anuário.


Abstract The Forum for Economic and Trade Cooperation between China and Portuguese-speaking Countries, better known as Forum Macao, founded in 2003, has been acting as a link between China and Portuguese-speaking countries for 20 years. Since 2016, the Permanent Secretariat of Forum Macao has published a yearbook highlighting the main activities it carried out in the previous year. This article focuses on analyzing Forum Macao's role as an organizer and mediator of Global Health initiatives by analyzing all the articles published in its yearbooks between 2016 and 2023. We found 34 articles dealing with a health-related topic, with almost two thirds related to traditional Chinese medicine, reports of frequent organization of activities that foster international cooperation in health, as well as active participation during the first year of the Covid-19 pandemic. Forum Macao is a multilateral platform for dialogue between China, Macao and the Portuguese-speaking world, with the health agenda and the dissemination of traditional Chinese medicine occupying a prominent place in its yearbook.

The present study was conducted to ascertain the estrogenic effect of Zhuang Medicated Thread Moxibustion (ZMTM) and explore its time - sensitive impact on estradiol (E2) in female perimenopausal rats. 40 female rats were randomized into four gr oups: the control, model, ZMTM, and acupuncture groups. The perimenopausal syndrome was induced in the last three groups with a daily subcutaneous dose of 80 mg/kg of 4 - vinylcyclohexene diepoxide for 15 days. Afterward, rats in the model and control group s were fed routinely, while rats in the ZMTM and acupuncture groups were treated with six ZMTM and acupuncture courses, respectively. Results of the study suggested that following the six courses of treatment, the E2 level in the model group was significan tly the lowest, while the regular group was the highest (P < 0.05). There was also a gradual increase in the E2 level of the ZMTM group compared to the model and acupuncture groups, e.g. after the 5th and 6th courses of treatment, their E2 level was signif icantly higher than the model and acupuncture groups. The ZMTM group was better than the model and acupuncture groups. In summary, ZMTM can improve perimenopausal induced rats' estrogen level.


El presen te estudio se llevó a cabo para determinar el efecto estrogénico de la moxibustión con hilo medicado Zhuang (ZMTM) y explorar su impacto sensible al tiempo en el estradiol (E2) en ratas hembras perimenopáusicas. Se dividió al azar una muestra de 40 ratas h embras en cuatro grupos: control, modelo, ZMTM y acupuntura. El síndrome perimenopáusico se indujo en los últimos tres grupos con una dosis subcutánea diaria de 80 mg/kg de diepóxido de 4 - vinilciclohexeno durante 15 días. Después, las ratas en los grupos m odelo y control fueron alimentadas rutinariamente, mientras que las ratas en los grupos ZMTM y acupuntura recibieron seis cursos de ZMTM y acupuntura, respectivamente. Los resultados del estudio sugieren que después de los seis cursos de tratamiento, el ni vel de E2 en el grupo modelo fue significativamente más bajo, mientras que el grupo regular fue más alto ( p < 0,05). También hubo un aumento gradual en el nivel de E2 del grupo ZMTM en comparación con los grupos modelo y acupuntura, por ejemplo, desp ués del quinto y sexto cursos de tratamiento, su nivel de E2 fue significativamente más alto que los grupos modelo y acupuntura. El grupo ZMTM fue mejor que los grupos modelo y acupuntura. En resumen, el ZMTM puede mejorar el nivel de estrógeno de las rata s inducidas por la perimenopausia.

Since the advent of Covid-19, several natural products have been investigated regarding their in silico interactions with SARS-CoV-2 proteases - 3CLpro and PLpro, two of the most important pharmacological targets for antiviral development. Phenylethanoid glycosides (PG) are a class of natural products present in important medicinal plants and a drug containing this group of active ingredients has been successfully used in the treatment of Covid-19 in China. Thus, a dataset with 567 derivatives of this class was built from reviews published between 1994 and 2020, and their interaction against both SARS-CoV-2 proteases was investigated. The virtual screening was performed by filtering the PGs through the evaluation of scores based on the AutoDock Vina, GOLD/ChemPLP, and GOLD/GoldScore evaluation functions. The bRO5 pharmacokinetic parameters of the PGs ranked in the previous step were analyzed and their interaction with key amino acid residues of the 3CLpro and PLpro enzymes was evaluated. Ninety-eight compounds were identified by computational approaches against PLpro and 80 PGs against 3CLpro. Of these, four interacted with key catalytic residues of PLpro, which is an indicative of inhibitory activity, and three compounds interacted with catalytic key residues of 3CLpro. Of these, five PGs occur in plants of the Traditional Chinese Medicine (TCM), while two are components of plants/formulations currently used in the Covid-19 protocols in China. The data presented here show the potential of PGs as selective inhibitors of SARS-CoV-2 3CLpro and PLpro.

Resumo O Tai Chi Chuan (TCC) está entre as Práticas Integrativas e Complementares (PICS) mais utilizadas e apresenta, segundo a literatura científica, evidências clínicas para tratamento de diversos problemas de saúde. Entretanto, pouco se sabe sobre o desenvolvimento dessa modalidade no contexto do Sistema Único de Saúde (SUS). Neste sentido, o objetivo do artigo foi compreender a prática terapêutica do TCC em um serviço de referência em PICS no SUS, a partir de um estudo de caso qualitativo com observação participante e entrevistas semiestruturadas com usuários e terapeutas. Observou-se dessemelhanças entre a prática terapêutica do Tai Chi Chuan e as práticas corporais biomédicas, assim como a presença de diretrizes norteadoras de cuidado do SUS, como o acolhimento, a escuta qualificada, o vínculo terapêutico e a integralidade da atenção. Também foi identificada a prevalência de sofrimento mental como motivo de procura pela prática, necessidade de maior apoio financeiro ao serviço e resistência ao uso da Medicina Tradicional Chinesa por parte da rede de apoio de algumas usuárias.


Abstract Tai Chi Chuan (TCC) is among the most used Integrative and Complementary Health Practices (PICS) and, according to the scientific literature, presents clinical evidence for the treatment of various health problems. However, little is known about the development of this practice in the context of the Brazilian National Health System (SUS). In this sense, the objective of the article was to understand the therapeutic practice of TCC in a reference service in PICS in SUS, from a qualitative case study with participant observation and semi-structured interviews with users and therapists. Dissimilarities were observed between the therapeutic practice of TCC and biomedical body practices, as well as the presence of guidelines for SUS care, such as welcoming, qualified listening, therapeutic connection, and comprehensive care. The prevalence of mental distress was also identified as a reason for seeking the practice, as well as the need for greater financial support for the service, and the resistance to the use of Traditional Chinese Medicine by some female users' support network.

Background: The growth of exotic pet medicine is leading to fast developments in clinical investigations on birds. Acupuncture, specifically pharmacopuncture, offers safe chemical restraint options. Objectives: To investigate pharmacopuncture at acupoint GV20 in blue-fronted Amazon parrots (Amazona aestiva) using ketamine and midazolam. Methods: Sixteen healthy birds were distributed into four groups (C: intramuscular control; 1/2 C: 1/2 dose intramuscular control; 1/2 GV20: 1/2 dose at acupoint GV20; 1/5 GV20: 1/5 dose at acupoint GV20). Degree of sedation, latency, recuperation time, heart and respiratory rate, and body temperature were measured. Quantitative data were analyzed by a Student's t-test. Results: The C, 1/2 C, and 1/2 GV20 groups showed the same degree of sedation. The 1/2 GV20 group showed longer latency times (6 ± 2.1) than the 1/2 C (2.5 ± 0.5) group. Sedation time did not differ between the C (28 ± 9.8), 1/2 C (30.5 ± 8.6), and 1/2 GV20 (41 ± 22.24) groups. The 1/2 GV20 group recuperated faster (13.7 ± 3.7) than the C group (64.2 ± 3.5). The C and 1/2 C groups showed tremors and slow and unstable recovery. Two animals in the C group showed mild hypothermia (38°C). Conclusion: The use of 1/2 GV20 was effective and safe to sedate blue-fronted Amazon parrots without side effects, providing easy, stable, and fast recovery. The use of 1/5 GV20 had a shorter sedation time. These findings show that the combination of acupuncture and drugs provides new possibilities for efficient anesthetic protocols with fewer side effects in birds.

Embora muitas das técnicas da medicina tradicional chinesa sejam praticadas atualmente nos serviços do Sistema Único de Saúde, a regulamentação da acupuntura no Brasil é conflituosa e marcada por disputas judiciais. Envolvidas nas disputas em torno do exercício profissional dessas práticas estão autarquias federais e entidades profissionais da saúde. O presente artigo analisou a constituição dos discursos de atores envolvidos no processo de regulamentação da acupuntura no Brasil em um recorte temporal histórico, referente ao período de 2000 a 2012. Foram pesquisados 228 textos nos sites institucionais de 39 entidades ligadas à medicina tradicional chinesa e, particularmente, à acupuntura. Os textos foram submetidos à análise de discurso conforme a abordagem de Dominique Maingueneau. As formações discursivas da acupuntura médica e da acupuntura multiprofissional configuram entendimentos distintos sobre a institucionalização dessa prática. A ausência de um consenso sobre a delimitação do campo de atuação do acupunturista é um obstáculo considerável para a regulamentação da prática. As questões ético-legais das delimitações do campo de atuação, a criação de diretrizes para ensino e pesquisa e o estabelecimento de parâmetros de segurança, qualidade e eficácia são desafios para a implementação da acupuntura nos serviços de saúde.


Although many of the techniques of traditional Chinese medicine are currently practiced in the Brazilian Public Health System health services, the regulation of acupuncture in Brazil is conflicting and marked by legal disputes between federal authorities and health professionals about the professional practice of this health practice. The presente article aimed to analyse the constitution of the different discourses of actors involved in the process of acupuncture regulation in Brazil in a historical timeframe of this trajectory, from 2000 to 2012. 228 texts were collected from the institutional websites of 39 entities related to traditional Chinese medicine and acupuncture. Texts were submit ted to the discourse analysis of Dominique Maingueneau. The discursive formations of medical acupuncture and multidisciplinary acupuncture constitute different understandings about the institutionalization of acupuncture. The absence of a consensus on the delimitation of the acupuncturist's field of action is a considerable obstacle to the regulation of the practice. The ethical-legal dilemmas of the field boundaries, the creation of guidelines for teaching and research and the establishment of safety, quality and efficacy parameters are challenges for the implementation of acupuncture in health services.

Medicinal plants are the primary sources of healthcare among the people of developing countries in villages and local towns. Documenting and reporting the traditional knowledge of medicinal plants may contribute to pharmaceutical research development. For this reason, we present our findings on ethnomedicinal plants from Lingchuan County, Shanxi, China, an unexplored area rich in medicinal plant resources. Information of ethnomedicinal plants were collected through questionnaire/semi-structured interviews from 180 informants, including traditional healers. Field surveys were conducted in 53 villages of Lingchuan County from 2017 to 2018. Informed consent was obtained from each participant before conducting the interview process. Quantitative analysis was performed for each recorded species, such as Relative Frequency Citation (RFC), Use Value (UV), and Factor of Informant Consensus (FIC). Diseases were categorized into twelve groups. A total 138 species of medicinal plants were recorded, belonging to 123 genera of 58 families. Asteraceae was the dominant plant family with 19 species, followed by Rosaceae and Fabaceae. Herbs were dominant among plant life-forms with 96 species, followed by shrubs and trees (15 species each). Roots were the most commonly used plant parts with 58 species, followed by whole plants and fruits (28 species each). Most plant species were reported non-toxic (84, 60%), followed by unknown toxicity (35, 25%), poisonous, and less toxic (19, 14%). Quantitative analysis revealed that Forsythia suspensa was with higher (0.33) RFC value, and Scutellaria baicalensis was recorded with a higher (0.91) UV. Treated diseases were categorized in 12 groups and evaluated by their FIC value, in which gynecological diseases have higher (0.93) FIC value followed by urinary system diseases. Most medicinal plants are used to clear away heat and relieve the surface. The present study revealed that local people of Lingchuan County confidently use ethnomedicinal plants for their healthcare needs. The higher indices value of a plant species resulted from quantitative analysis warrants further investigation, which may possess valuable phytochemical compounds that may result in new drugs for treating various human disorders.


As plantas medicinais são as principais fontes de cuidados de saúde entre as pessoas dos países em desenvolvimento, nas aldeias e cidades locais. É importante documentar e relatar o conhecimento tradicional de plantas medicinais, dado que pode contribuir para o desenvolvimento da pesquisa farmacêutica. Por esta razão, apresentamos neste estudo nossas descobertas sobre plantas etnomedicinais do condado de Lingchuan, Shanxi, China, uma área inexplorada e rica em recursos de plantas medicinais. As informações sobre as plantas etnomedicinais foram coletadas por meio de questionário/entrevista semiestruturada com 180 participantes, incluindo curandeiros. As pesquisas de campo foram realizadas em 53 aldeias do condado de Lingchuan, entre 2017 e 2018. O consentimento informado foi obtido de cada participante antes de conduzir o processo de entrevista. Foi realizada uma análise quantitativa de cada espécie registrada, através da Frequência Relativa de Citação (RFC), Valor de Uso (UV) e Fator de Consenso dos Informantes (FCI). As doenças foram categorizadas em doze grupos. Foram registradas 138 espécies de plantas medicinais, pertencentes a 123 gêneros de 58 famílias. Asteraceae foi a família de plantas dominante, totalizando 19 espécies, seguida pelas famílias Rosaceae e Fabaceae. As ervas foram consideradas como as principais formas de vida vegetal usadas, com 96 espécies, seguidas por arbustos e árvores (15 espécies cada). As raízes foram as partes de plantas mais utilizadas com 58 espécies, seguidas de plantas inteiras e frutos (28 espécies cada). A maioria das espécies relatadas foram estabelecidas como não tóxicas (84, 60%), seguida de plantas com toxicidade desconhecida (35, 25%), venenosas e menos tóxicas (19, 14%). A análise quantitativa revelou que a espécie Forsythia suspensa alcançou o maior (0,33) valor de RFC, e a Scutellaria baicalensis obteve o maior (0,91) valor de UV registrado. As doenças tratadas foram categorizadas em 12 grupos e avaliadas pelos seus respectivos valores de FCI, onde as doenças ginecológicas apresentaram maior (0,93) valor de FCI, seguido pelas doenças do sistema urinário. Foi concluído que a maioria das plantas medicinais são usadas para amenizar calores e alívio tópico. O presente estudo revelou que a população local do condado de Lingchuan confia no uso de plantas etnomedicinais para lidar com seus problemas de saúde. Seria válido aprofundar as investigações relativas aos índices mais altos obtidos nas análises quantitativas das espécies analisadas, visto que podem possuir compostos fitoquímicos valiosos que podem resultar em novos medicamentos para o tratamento de diversas doenças humanas.

Abstract Inflammatory bowel disease (IBD) is a chronic inflammatory disorder of the intestine, demonstrating an increasing incidence every year. TongXieYaoFang (TXYF) has been used widely in China as a complementary therapy to relieve the symptoms of IBD for hundreds of years. In the present research, a network pharmacology-based approach was used to systematically explore the intrinsic mechanisms of TXYF in IBD at the molecular level. Network pharmacology-based methods, which mainly included database mining, screening of bioactive compounds, target prediction, collection of IBD-related targets, gene enrichment analysis, network construction, and molecular docking, were employed in the present study. Network analysis revealed a total of 108 potential targets derived from 22 component compounds of TXYF, among which 34 targets were common with the IBD-related targets. In the protein-protein interaction (PPI) network, 10 key targets were identified. The gene enrichment analysis suggested that anti-inflammatory processes, such as NF-kappa B signaling pathway and Toll-like receptor signaling pathway, could be the core processes involved in the action of TXYF in IBD. Molecular docking results revealed that three compounds present in TXYF exhibited strong binding affinity for PTGS2. The present study provides novel insights into the molecular mechanisms and network approaches of TXYF action in IBD from a systemic perspective. The potential targets and pathways identified in the present study would assist in further research on the clinical application of TXYF in IBD therapy.

Introduction: Traditional Chinese Medicine (TCM) represents one of the first holistic approaches in the world to treat and prevent disease. Herbal medicine is one of the major therapeutic remedy in TCM. It often involves multi-herb therapies instead of single herb preparations. Parallel to western medicine, hundreds of herbal formulas have been made available as finished products. Currently, the use of herbal products is popular as treatment option or to complement western medicine. Indications of the herbal formulas were established by TCM terms such as heat-clearing and/or detoxifying which lack modern pharmacological meanings. It is difficult for people without relevant background to understand such terms and their implications for treatments. Furthermore, due to the quality control issues of herbal medicines which contain multiple constituents, consumers may be confronted with the risk of using unstandardized products. Hence, in this thesis, the modernization of TCM is discussed through employing scientific pharmaceutical approaches to a traditional formula, called Erding formula (EF). The aim was to investigate if a new indication, hyperuricemia, can be assigned to a heat-clearing and detoxifying formula. Our hypothesis was: Can Erding formula be used for hyperuricemia treatment and is esculetin a bioactive marker for this new indication? Methods: A hypoxanthine and potassium oxonateinduced hyperuricemic mouse model, a xyleneinduced inflammatory mouse model, and an acetic acidinduced pain model were used to investigate EF and its constituent herbs. The quantity of esculetin was measured by high-performance liquid chromatography. The therapeutic effect of esculetin was assessed using potassium oxonate induced hyperuricemic mouse model, and esculetin and its metabolites were characterized in serum via ultra-performance liquid chromatographyquadrupole time-of-flight mass spectrometry. To develop a modern dosage form, a laboratory-scale wet bead milling approach was employed to prepare esculetin nanocrystals. The formulation was further optimized by design of experiment, and an optimized formulation was then characterized for its saturation solubility and short-term stability. Results: The study showed that EF and Viola yedoensis Makino (Viola) lowered uric acid (UA) levels, while EF and all four individual herbs had antiinflammatory and analgesic activities. These findings revealed that EF was able to treat hyperuricemia and suggested that Viola was the main herb in EF on reducing UA levels. The study showed that esculetin significantly reduced UA levels and six metabolites of esculetin were identified in serum. This confirms that esculetin was absorbed and is a suitable bioactive and quality control marker for EF in hyperuricemia treatment. An esculetin-Povacoat nanocrystal formulation with a 200 nm particle size was successfully prepared. The formulation presented up to a 1.5-fold increase in saturation solubility compared to the bulk esculetin and it was stable for 180 days. Conclusion: The studies proved that Erding formula can be used for hyperuricemia treatment with esculetin as bioactive quality control marker. As well, a new nano-sized formulation of the bioactive marker, esculetin, was created. This presented the possibility to develop an innovative nanotechnological product of the active substances derived from herbal medicine. The findings facilitated a better understanding of TCM terms and concept through mechanistic scientific experiments. This study revealed a potential pathway and an idea to modernize TCM without setting aside its unique concepts. This might increase the global acceptance of TCM products. Furthermore, the TCM concept might be useful in the development of multi-component drug products


Medicina Tradicional Chinesa (MTC) representa uma das primeiras abordagens holísticas em âmbito global para tratar e prevenir doenças. A fitoterapia consiste na principal terapia na MTC. Frequentemente, envolve terapias com múltiplas ervas em vez de preparações individuais. Paralelamente à medicina ocidental, centenas de fórmulas herbais foram disponibilizadas como produtos acabados. Atualmente, o uso de produtos fitoterápicos é popular como opção de tratamento ou para complementar a medicina ocidental. As indicações das fórmulas fitoterápicas foram estabelecidas pelos termos da MTC, tais como "limpeza pelo calor e / ou desintoxicante", que não têm significados farmacológicos modernos. É difícil para a população em geral e mesmo para profissionais sem histórico relevante na área entender tais termos e suas implicações para os tratamentos. Além disso, devido às questões de controle de qualidade dos medicamentos fitoterápicos que contêm múltiplos constituintes, os pacientes podem ser confrontados com o risco de usar produtos não padronizados. Assim, nessa tese, a modernização da MTC é discutida por meio da utilização de abordagens farmacêuticas científicas para uma fórmula tradicional, denominada fórmula de Erding (FE). O objetivo foi o de investigar se uma nova indicação, a hiperuricemia, pode ser atribuída a uma fórmula desintoxicante e de compensação de calor. Nossa hipótese foi: a fórmula de Erding pode ser usada para tratamento de hiperuricemia e a esculetina é um marcador bioativo para essa nova indicação? Foi empregado modelo de camundongo hiperuricêmico induzido por hipoxantina e oxonato de potássio, outro modelo de camundongo inflamatório induzido por xileno e, adicionalmente, modelo de dor induzida por ácido acético. Esses modelos foram usados para investigar a FE e suas ervas constituintes. A quantidade de esculetina foi determinada por cromatografia líquida de alta eficiência. O efeito terapêutico da esculetina foi avaliado utilizando modelo de camundongo hiperuricêmico induzido por oxonato de potássio, e a esculetina e seus metabólitos foram caracterizados no soro por cromatografia líquida de alto desempenho - espectrometria de massa. Para desenvolver forma farmacêutica moderna, uma abordagem de moagem em escala úmida reduzida foi empregada tendo em vista a preparação de nanocristais de esculetina. A formulação foi ainda otimizada empregado planejamento experimental. Essa fórmula foi caracterizada quanto à sua solubilidade de saturação e estabilidade a curto prazo. O estudo mostrou que a FE e a Viola yedoensis Makino (Viola) reduziram os níveis de ácido úrico (AU), enquanto a FE e as quatro plantas individuais apresentaram atividades antiinflamatória e analgésica. Esses resultados revelaram que a FE foi capaz de tratar a hiperuricemia e sugeriu que a viola foi a principal erva da FE na redução dos níveis de AU. O estudo mostrou também que a esculetina reduziu significativamente os níveis de AU e os seis metabólitos da esculetina foram identificados no soro. Tal resultado confirma que a esculetina foi absorvida e pode ser usada como marcador de controle bioativo e de qualidade para FE, no tratamento da hiperuricemia. A formulação de nanocristais de esculetin-povacoat® apresentou tamanho de partícula de 200 nm. A formulação apresentou aumento de 1,5 vezes na solubilidade de saturação em comparação com a esculetina em escala micrométrica e manteve-se estável durante 180 dias. Os estudos comprovaram que a fórmula de Erding pode ser utilizada no tratamento da hiperuricemia empregando a esculetina como marcador bioativo de controle de qualidade. Além disso, foi desenvolvida formulação inovadora, em escala nanométrica, do marcador bioativo, a esculetina. Esse resultado permitiu desenvolver produto com base nanotecnológica das substâncias ativas derivadas do fitoterápico, assim comol permitiram melhor compreensão dos termos e dos conceitos da MTC por meio de experimentos científicos mecanicistas. Esse estudo revelou potencial para a modernização da MTC sem excluir seus conceitos únicos. Isso pode aumentar a aceitação global dos produtos MTC. Além disso, o conceito de MTC pode ser útil no desenvolvimento de medicamentos de múltiplos componentes

Abstract TCMSP platform of systematic pharmacology of traditional Chinese medicine This study aimed to investigate the molecular mechanism of Fructus Ligustri Lucidi (NZZ, Chinese abbreviation) against osteoporosis (OP) by means of network pharmacology.ChemDraw Professional 15.1 software and Molinspiration Smiles database were used to draw the chemical formulas of the components. The active ingredients and related target proteins of NZZ were searched in platform of systematic pharmacology of traditional Chinese medicine database, Drugbank, Therapeutic Target Database, SymMap and other databases. Gene Ontology(GO) enrichment and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis were carried out on the selected target through Enrichr and KEGG Automatic Annotation databases, and their mechanism was studied. A total of 29 compounds and 140 corresponding targets, including 14 key targets and 14 protein factors in protein-protein interaction core network were obtained. The key targets were tumor necrosis factor(TNF), interleukin(IL)-6R and sestrogen receptor alpha. The number of GO items was 466 (P<0.05), including 399 items of biological process (BP), 54 items of cell composition (MF) and 13 items of molecular function (CC). KEGG pathway enrichment screened 85 signaling pathways (P<0.05), including the IL-17 signaling pathway, TNF signaling pathway, advanced glycation end products and their receptors signaling pathway and cAMP signaling pathway. The active ingredients of NZZ. exert their anti-OP effects through multi-components, multi-targets and multi-pathways, which can provide new evidence for further study of their anti-OP mechanism.

A acupuntura é a prática integrativa e complementar mais utilizada nos países ocidentais e com maior institucionalização nos sistemas públicos de saúde. Considerando que a medicina chinesa/acupuntura foi absorvida pela prática biomédica na diáspora de seu conhecimento para o Ocidente, questiona-se se suas características autóctones de cuidado continuam presentes na sua aplicação clínica na Atenção Primária à Saúde (APS). Investigou-se qual referencial de cuidado orientava todos os profissionais com formação em Acupuntura (n = 21) da rede básica de saúde de Campinas/SP, Brasil. Todos os entrevistados foram formados nos métodos tradicionais da Acupuntura, atuavam por meio desse referencial, diferenciavam-no da Biomedicina. A acupuntura foi considerada eficaz para atender à integralidade das demandas de cuidado e para a diminuição do uso de fármacos. Porém, constataram-se significativas dificuldades estruturais na oferta da prática da acupuntura.(AU)


Acupuncture is the most used integrative and complementary practice in western countries and is widely incorporated into public health systems. Considering that Chinese medicine/acupuncture was absorbed by biomedical practice during the diaspora of this type of knowledge in the West, we question whether its autochthonous care characteristics remain present in its clinical application in primary health care. We investigated which care approach guides professionals with training in acupuncture (n = 21) working in primary care services in Campinas/SP, Brazil. All the interviewees trained in traditional methods of acupuncture adopted this approach, differentiating it from biomedicine. Acupuncture was considered effective in meeting the demands of comprehensive care and reducing the use of pharmaceuticals. La acupuntura es la práctica integradora y complementaria más utilizada en los países occidentales y con mayor institucionalización en los sistemas públicos de salud. Considerando que la medicina china/acupuntura fue absorbida por la práctica biomédica en la diáspora de su conocimiento hacia el occidente, se cuestiona si sus características autóctonas de cuidado continúan presentes en su aplicación clínica en la atención primaria de la salud (APS). Se investigó cuál era el referente de cuidado que orientaba a todos los profesionales con formación en acupuntura (n = 21) de la red básica de salud de Campinas/SP, Brasil. Todos los entrevistados se habían formado en los métodos tradicionales de la acupuntura, actuaban por medio de este referente y lo diferenciaban de la biomedicina. La acupuntura se consideró eficaz para atender la integralidad de las demandas de cuidado y para la disminución del uso de fármacos. Sin embargo, se constataron significativas dificultades estructurales en la oferta de la práctica de la acupuntura.(AU)

Background: Chronic pruritus is a prevalent, multifactorial and debilitating condition that is often underestimated. This article reviews current evidence to evaluate the efficacy of acupuncture for the treatment of itch. Results: Although many researchers have conducted observational studies, clinical trials, and systematic reviews on the subject, the lack of more-robust and well-designed studies to prove the effectiveness of acupuncture in the management of itch is evident. Many published studies are of poor quality, with no clear description of randomization methods, and an absence of control groups and measurable clinical outcomes. In addition, a lack of standardization in methods for assessing pruritus and the acupuncture treatment protocols hinders more-comprehensive and higher-quality pooled data analysis. Conclusions: Current evidence cannot fully support acupuncture for the treatment of itch yet.

QUESTION: What are the effects of dry cupping on pain intensity, physical function, functional mobility, trunk range of motion, perceived overall effect, quality of life, psychological symptoms and medication use in individuals with chronic non-specific low back pain? DESIGN: Randomised controlled trial with concealed allocation, intention-to-treat analysis and blinding of participants and assessors. PARTICIPANTS: Ninety participants with chronic non-specific low back pain. INTERVENTIONS: The experimental group (n = 45) received dry cupping therapy, with cups bilaterally positioned parallel to the L1 to L5 vertebrae. The control group (n = 45) received sham cupping therapy. The interventions were applied once a week for 8 weeks. OUTCOME MEASURES: Participants were assessed before and after the first treatment session, and after 4 and 8 weeks of intervention. The primary outcome was pain intensity, measured with the numerical pain scale at rest, during fast walking and during trunk flexion. Secondary outcomes were physical function, functional mobility, trunk range of motion, perceived overall effect, quality of life, psychological symptoms and medication use. RESULTS: On a 0-to-10 scale, the between-group difference in pain severity at rest was negligible: MD 0.0 (95% CI -0.9 to 1.0) immediately after the first treatment, 0.4 (95% CI -0.5 to 1.5) at 4 weeks and 0.6 (95% CI -0.4 to 1.6) at 8 weeks. Similar negligible effects were observed on pain severity during fast walking or trunk flexion. Negligible effects were also found on physical function, functional mobility and perceived overall effect, where mean estimates and their confidence intervals all excluded worthwhile effects. No worthwhile benefits could be confirmed for any of the remaining secondary outcomes. CONCLUSION: Dry cupping therapy was not superior to sham cupping for improving pain, physical function, mobility, quality of life, psychological symptoms or medication use in people with non-specific chronic low back pain.
