Objective: The objective is to expose the cardiovascular alterations in patients diagnosed with pediatric inflammatory multisystem syndrome (PIMS) associated with COVID-19 during the SARS-CoV-2 pandemic, in order to understand the disease, its evolution, and optimal management upon diagnosis. Method: Retrospective, observational, cross-sectional analytical study of patients diagnosed with PIMS according to the criteria of the World Health Organization at the National Institute of Pediatrics, from March 2020 to December 2021. Results: During the study period, 77 patients with PIMS were diagnosed. The results showed correlation between the shock state and alteration of laboratory markers (platelets 144217.29 ± 139321.6 µL [p < 0.001], procalcitonin 27.37 ± 38.37 ng/ml [p = 0.05] and ferritin 1937.87 ± 2562.63 [p < 0.001]). The ventricular function in patients with shock was significantly lower compared to those without shock (49.6 ± 9.1% vs. 58.1 ± 8.4 %; t-Student p < 0.001), as well as injury to the left coronary artery (p = 0.02). There is a correlation between NT-proBNP and ventricular dysfunction (Kruskal-Wallis p = 0.007). Statistical significance was found in the association between death, elevation of inflammatory markers and ventricular dysfunction (p < 0.001). Conclusions: The cardiovascular alterations observed, in order of frequency, were pericardial effusion (25.7%), myocarditis (15%), mild ventricular dysfunction (13.5%) and small coronary aneurysm with predominance of the left coronary artery and the anterior descending one.


Objetivo: Exponer las alteraciones cardiovasculares en los pacientes diagnosticados con síndrome inflamatorio multisistémico pediátrico (PIMS) asociado a COVID-19 durante la pandemia por SARS-CoV-2 con el fin de comprender la enfermedad, su evolución y el manejo óptimo al diagnóstico. Método: Estudio retrospectivo, observacional, transversal y analítico de pacientes con diagnóstico de PIMS de acuerdo con los criterios de la Organización Mundial de la Salud en el Instituto Nacional de Pediatría, de marzo de 2020 a diciembre de 2021. Resultados: Durante el periodo de estudio se diagnosticaron 77 pacientes con PIMS. Los resultados demostraron una correlación entre el estado de choque y la alteración de los marcadores de laboratorio (plaquetas 144217.29 ± 139321.6 µl [p < 0.001], procalcitonina 27.37 ± 38.37 ng/ml [p = 0.05] y ferritina 1937.87 ± 2562.63 [p < 0.001]). La función ventricular en los pacientes con choque se registró significativamente menor en comparación con aquellos sin choque (49.6 ± 9.1 % vs. 58.1 ± 8.4 %; t de Student p < 0.001), así como lesión en la arteria coronaria izquierda (p = 0.02). Existe una correlación entre el NT-proBNP y la disfunción ventricular (Kruskal-Wallis p = 0.007). Se encontró significancia estadística en la asociación entre fallecimiento, elevación de los marcadores inflamatorios y disfunción ventricular (p < 0.001). Conclusiones: Las alteraciones cardiovasculares observadas fueron, en orden de frecuencia, derrame pericárdico (25.7%), miocarditis (15%), disfunción ventricular leve (13.5%) y aneurisma pequeño coronario con predominio de la arteria coronaria izquierda y la descendente anterior.

Several psychosocial, sleep/circadian, and cardiometabolic disorders have intricately interconnected pathologies involving melatonin disruption. Therefore, we hypothesize that melatonin could be a therapeutic target for treating potential comorbid diseases associated with this triad of psychosocial-sleep/circadian-cardiometabolic disorders. We investigated melatonin's target prediction and tractability for this triad of disorders. The melatonin's target prediction for the proposed psychosocial-sleep/circadian-cardiometabolic disorder triad was investigated using databases from Europe PMC, ChEMBL, Open Targets Genetics, Phenodigm, and PheWAS. The association scores for melatonin receptors MT1 and MT2 with this disorder triad were explored for evidence of target-disease predictions. The potential of melatonin as a tractable target in managing the disorder triad was investigated using supervised machine learning to identify melatonin activities in cardiovascular, neuronal, and metabolic assays at the cell, tissue, and organism levels in a curated ChEMBL database. Target-disease visualization was done by graphs created using "igraph" library-based scripts and displayed using the Gephi ForceAtlas algorithm. The combined Europe PMC (data type: text mining), ChEMBL (data type: drugs), Open Targets Genetics Portal (data type: genetic associations), PhenoDigm (data type: animal models), and PheWAS (data type: genetic associations) databases yielded types and varying levels of evidence for melatonin-disease triad correlations. Of the investigated databases, 235 association scores of melatonin receptors with the targeted diseases were greater than 0.2; to classify the evidence per disease class: 37% listed psychosocial disorders, 9% sleep/circadian disorders, and 54% cardiometabolic disorders. Using supervised machine learning, 546 cardiovascular, neuronal, or metabolic experimental assays with predicted or measured melatonin activity scores were identified in the ChEMBL curated database. Of 248 registered trials, 144 phase I to IV trials for melatonin or agonists have been completed, of which 33.3% were for psychosocial disorders, 59.7% were for sleep/circadian disorders, and 6.9% were for cardiometabolic disorders. Melatonin's druggability was evidenced by evaluating target prediction and tractability for the triad of psychosocial-sleep/circadian-cardiometabolic disorders. While melatonin research and development in sleep/circadian and psychosocial disorders is more advanced, as evidenced by melatonin association scores, substantial evidence on melatonin discovery in cardiovascular and metabolic disorders supports continued R&D in cardiometabolic disorders, as evidenced by melatonin activity scores. A multiplatform analysis provided an integrative assessment of the target-disease investigations that may justify further translational research.

Abstract Introduction Hypertension is a cardiovascular risk factor of wide magnitude among people of African descent, especially those living in Quilombos. However, little is known about the factors associated with cardiovascular risk in residents of the urban Quilombola community. Objetive To analyze the cardiovascular risk and health-related factors in the family context of hypertensive Afro-descendants living in an urban Quilombola. Materials and methods It is a cross-sectional and community-based study carried out from November 2017 to March 2018. The study's population consisted of 303 hypertensive patients enrolled in the family health unit, with ages ranging from 35 to 79 years old, of both genders; and using antihypertensive drugs. The instruments used for data production were: The Primary Arterial Hypertension Questionnaire and the Framingham Risk Score . Results There was a significant association between cardiovascular risk and cardiovascular disease (CVD) family history (p<0.011), type II diabetes (p<0.001) and overweight and obesity (p<0.010). Conclusion Research has shown that hypertensive Quilombola people have consistent cardiovascular risk outcomes, especially with CVD family history, type II diabetes, overweight and obesity, schooling and gender with significant associations.


Resumen Introducción La hipertensión es un factor de riesgo cardiovascular de gran magnitud entre las personas de ascendencia africana, especialmente las que viven en quilombos. Sin embargo, se sabe poco sobre los factores asociados con el riesgo cardiovascular en los residentes de la comunidad urbana Quilombola. Objetivo Analizar el riesgo cardiovascular y los factores asociados con la salud en el contexto familiar de los descendientes africanos hipertensos que viven en una comunidad urbana de Quilombolas. Material y métodos Este es un estudio transversal y basado en la comunidad, realizado desde noviembre de 2017 hasta marzo de 2018. La población de estudio consistió en 303 pacientes hipertensos inscritos en la unidad de salud familiar, de edad de 35 a 79 años, ambos sexos; y en el uso medicamentos antihipertensivos. Los instrumentos utilizados para producir los datos fueron: el Cuestionario de Hipertensión en Atención Primaria, la puntuación de riesgo de Framingham . Resultados Se observó una asociación significativa entre el riesgo cardiovascular y los antecedentes familiares de enfermedad cardiovascular (ECV) (p <0.011), diabetes tipo II (p <0.001) y sobrepeso y obesidad (p <0.010). Conclusión La investigación mostró que las personas Quilombolas hipertensos tienen resultados consistentes con respecto al riesgo cardiovascular, especialmente con la inclusión de antecedentes familiares de ECV, diabetes tipo II, sobrepeso y obesidad, educación y sexo con asociaciones significativas.

Material e métodos Trata-se de um estudo censitário de base comunitária, realizado de novembro de 2017 a março de 2018. A população do estudo foi composta por 303 pacientes hipertensos matriculados na unidade de saúde da família, com idades entre 35 e 79 anos, ambos os sexos; e em uso de medicamentos anti-hipertensivos. Os instrumentos utilizados para produzir os dados foram: o Questionário de Hipertensão na Atenção Primária, e o escore de risco de Framingham . Resultados Foi observada associação significativa entre risco cardiovascular e histórico familiar de doença cardiovascular (DCV) (p <0,011), diabetes tipo II (p <0,001) e sobrepeso e obesidade (p <0,010). Conclusão A pesquisa mostrou que pessoas hipertensas quilombolas apresentam resultados consistentes em relação ao risco cardiovascular, principalmente com a inclusão de histórico familiar de DCV, diabetes tipo II, sobrepeso e obesidade, educação e sexo com associações significativas.

Chili pepper is a prominent cultivated horticultural crop that is traditionally used for food seasoning and is applied for the treatment and prevention of multiple diseases. Its beneficial health properties are due to its abundance and variety of bioactive components, such as carotenoids, capsaicinoids, and vitamins. In particular, carotenoids have important nutraceutical properties, and several studies have focused on their potential in the prevention and treatment of human diseases. In this article, we reviewed the state of knowledge of general aspects of chili pepper carotenoids (biosynthesis pathway, types and content in Capsicum spp., and the effects of processing on carotenoid content) and recent findings on the effects of carotenoid nutraceuticals, such as antioxidant, cancer preventive, anti-inflammatory, cardiovascular disorder preventive, and anti-obesity effects.

According to investigations in phytomedicine and ethnopharmacology, the therapeutic properties of garlic (Allium sativum) have been described by ancestral cultures. Notwithstanding, it is of particular concern to elucidate the molecular mechanisms underlying this millenary empirical knowledge. Allicin (S-allyl prop-2-ene-1-sulfinothioate), a thioester of sulfenic acid, is one of the main bioactive compounds present in garlic, and it is responsible for the particular aroma of the spice. The pharmacological attributes of allicin integrate a broad spectrum of properties (e.g., anti-inflammatory, immunomodulatory, antibiotic, antifungal, antiparasitic, antioxidant, nephroprotective, neuroprotective, cardioprotective, and anti-tumoral activities, among others). The primary goal of the present article is to review and clarify the common molecular mechanisms by which allicin and its derivates molecules may perform its therapeutic effects on cardiovascular diseases and neuroinflammatory processes. The intricate interface connecting the cardiovascular and nervous systems suggests that the impairment of one organ could contribute to the dysfunction of the other. Allicin might target the cornerstone of the pathological processes underlying cardiovascular and neuroinflammatory disorders, like inflammation, renin-angiotensin-aldosterone system (RAAS) hyperactivation, oxidative stress, and mitochondrial dysfunction. Indeed, the current evidence suggests that allicin improves mitochondrial function by enhancing the expression of HSP70 and NRF2, decreasing RAAS activation, and promoting mitochondrial fusion processes. Finally, allicin represents an attractive therapeutic alternative targeting the complex interaction between cardiovascular and neuroinflammatory disorders.

OBJECTIVE: To establish the impact that timing of diagnosis and place of birth have on neonatal outcomes in those with readily treatable critical congenital heart disease. STUDY DESIGN: This was a population-based study with a complete national cohort of live-born infants with transposition of the great arteries and aortic arch obstruction in New Zealand between 2006 and 2014. Timing of diagnosis, place of birth, survival to surgery, in-hospital events, and neonatal mortality were reviewed. Live births with a gestation of ≥35 weeks and without associated major extracardiac anomalies were included for analysis. RESULTS: A total of 166 live-born infants with transposition of the great arteries and 87 with aortic arch obstruction were included. Antenatal detection increased from 32% in the first 3 years to 47% in the last 3 years (P = .05). During the same period, neonatal mortality decreased from 9% to 1% (P = .02). No deaths occurred after surgical intervention. An antenatal diagnosis was associated with decreased mortality (1/97 [1%] vs 11/156 [7%]; P = .03) and birth outside the surgical center was associated with increased risk of mortality (11/147 [7%] vs 1/106 [1%]; P = .02). Those with an antenatal diagnosis required fewer hours of mechanical ventilation (P = .02) and had shorter durations of hospital stay (P = .05) compared with those diagnosed >48 hours after birth. CONCLUSIONS: The mortality risk for transposition of the great arteries and critical aortic arch obstruction is greatest before cardiac surgery. Improved antenatal detection allowing delivery at a surgical center is associated with reduced mortality.

BACKGROUND: Antidepressants have been widely prescribed for depression, anxiety, sleep disorders, and in the management of behavioural symptoms of adult-old patients. Although generally safe, newer generation antidepressants are not devoid of the risk of inducing clinically relevant adverse events. OBJECTIVES: To investigate the association between newer generation antidepressants and the occurrence of cardiovascular adverse events and electrocardiogram (ECG) abnormalities. METHOD: Studies were included in the review according to the following criteria: a) clinical trials (placebo-controlled or not) or case reports; b) short- or long-term interventions with antidepressants; c) prescription of newer generation antidepressants as first-line treatment; d) samples of adult or adult-old patients. From a total of 301 articles addressing the association between antidepressants and cardiovascular adverse events as primary or secondary outcomes, we selected 30 controlled clinical trials and 10 case reports. RESULTS: In most clinical studies, the effects of antidepressants on cardiac function are usually computed as secondary outcome variables, however with limited information. Conversely, case reports tend to present more comprehensive sets of clinical and laboratorial parameters, but the generalization of such data is limited by the small number of observations. The occurrence of QTc prolongation (with increased risk of torsade de pointes) has been reported. Aging, higher dosages of antidepressants, drug interaction, and pre-existing cardiovascular comorbidities were found as risk factors for the aforementioned cardiovascular and ECG abnormalities. CONCLUSION: Prescribing antidepressants requires caution given their potential impact on cardiac function, and the clinician should carefully monitor cardiovascular and ECG parameters particularly in cases with underlying heart disease.

SUMMARY Polycystic ovary syndrome (PCOS) is a heterogeneous endocrine disorder with variable prevalence, affecting about one in every 15 women worldwide. The diagnosis of polycystic ovary syndrome requires at least two of the following criteria: oligoovulation and/or anovulation, clinical and/or biochemical evidence of hyperandrogenism and morphology of polycystic ovaries. Women with PCOS appear to have a higher risk of developing metabolic disorders, hypertension and cardiovascular disorders. The aim of this article was to present a review of the literature by searching the databases Pubmed and Scielo, focusing on publications related to polycystic ovaries, including its pathogenesis, clinical manifestations, diagnosis and therapeutic aspects, as well as its association with cardiovascular and arterial hypertensive disorders.


RESUMO A síndrome dos ovários policísticos (SOP) é uma desordem endócrina heterogênea com prevalência variável, que afeta cerca de uma em cada 15 mulheres no mundo. O diagnóstico da SOP requer, pelo menos, dois dos seguintes critérios: oligo-ovulação e/ou anovulação, evidência clínica e/ou bioquímica de hiperandrogenemia e morfologia dos ovários policísticos. As mulheres com SOP parecem ter um risco mais elevado de desenvolver distúrbios metabólicos, hipertensão e doenças cardiovasculares. O objetivo deste artigo foi apresentar uma revisão da literatura por meio de pesquisa nas bases de dados PubMed e Scielo, focada em publicações relacionadas com ovários policísticos, incluindo patogênese, manifestações clínicas, diagnóstico e aspectos terapêuticos, bem como associação com doenças cardiovasculares e hipertensão arterial.

Numerosos estudios han encontrado una asociación robusta entre los síntomas depresivos o la depresión mayor y un incremento en la morbilidad y la mortalidad cardiaca posterior a un síndrome coronario agudo (SCA); sin embargo, también se ha reportado evidencia en contra. Debido a esta divergencia, la depresión aún no ha sido aceptada por la Asociación Americana del Corazón como un factor de riesgo para un mal pronóstico en pacientes con enfermedades cardiovasculares. En este artículo se aborda el tema de la depresión en pacientes que han tenido un SCA y los factores que pueden influir en la divergencia de los resultados mencionada. Se concluye que la depresión se relaciona con un peor pronóstico en estos pacientes, específicamente cuando su presencia es posterior al evento o se trata de un cuadro resistente al tratamiento. Los síntomas cognitivos y somáticos son indicadores de mal pronóstico, así como la presencia de ansiedad comórbida.


Studies have found a strong association between depressive symptoms and major depression and cardiac morbidity and mortality increased after acute coronary syndrome (ACS); however it has also been reported evidence against this association. This difference in results may explain why depression has not yet been accepted by the American Heart Association as a risk factor for poor prognosis in patients with CVD. In this article depression is discussed as a risk factor of poor prognosis in patients who have had ACS and the factors that may influence the divergence of results in the studies. Our conclusions show that depression is related with a worse prognosis in ACS patients specifically when the symptomatology occurred after the event or when it is a treatment-resistant disorder. Cognitive and somatic symptoms and also comorbid anxiety are bad prognosis indicators.

BACKGROUND: In the last few decades, a large number of studies have produced compelling evidence that patients with schizophrenia are at increased risk for developing several medical conditions and diseases, including obesity, metabolic disturbances, and cardiovascular diseases. Several protocols have been designed with the aim of reducing such risk. OBJECTIVE: To investigate current physical health status in a population of outpatients with schizophrenia. METHODS: A cross-sectional study was conducted in our outpatient clinic, selecting subjects who met DSM-IV diagnosis criteria for schizophrenia. Data were collected regarding clinical characteristics, lifestyle, medication in use, and biometric and laboratory parameters. RESULTS: A total of 261 patients were included. We found a high prevalence of elevated body mass index (BMI . 25) (70%), dyslipidemia (73.2%), and metabolic syndrome (28.7%). Patients' ages were associated with worsened lipid profiles, but other variables, such as disorder duration or type of antipsychotic in use, were not associated with any metabolic disturbance. Despite the increased prevalence of these conditions, only a small portion of the sample was under regular medical treatment. CONCLUSION: Outpatients with schizophrenia show signs of poor physical health conditions. These findings reinforce the need for an intensive and appropriate approach to assure that these patients receive adequate clinical referral and treatment.

It is well known that the kidney plays an important role in the development of cardiovascular diseases such as hypertension. The normal aging process leads to changes in kidney morphology, hemodynamics and function, which increase the incidence of cardiovascular events in the elderly population. These disturbances are influenced by several factors, including gender. In general, females are protected by the effects of estrogens on the cardiorenal system. Several studies have demonstrated the beneficial effects of estrogens on renal function in the elderly; however, the relationships between androgens and kidney health during one’s lifetime are not well understood. Sex steroids have many complex actions, and the decline in their levels during aging clearly influences kidney function, decreases the renal reserve and facilitates the development of cardiovascular disorders. Therefore, in this review, we discuss the cellular, biochemical, and molecular mechanisms by which sex hormones may influence renal function during the aging process.

Objetivo: Analizar la incidencia del Síndrome de quemarse por el trabajo (SQT) (burnout) en una muestra de profesionales estudiantes de "prevención de riesgos laborales" y su relación con problemas de deterioro cardiovascular. Métodos: La muestra son 312 participantes. Las variables del estudio fueron: SQT, evaluado con el Maslach Burnout Inventory-General Survey (MBI-GS), y síntomas cardiovasculares percibidos por el individuo, que se evaluaron mediante cinco ítems. Los datos fueron recogidos en el transcurso de cursos de formación. Resultados: Se obtuvo una relación positiva y significativa entre el SQT y problemas cardiovasculares. Además, altos niveles de agotamiento y cinismo, y bajos niveles de eficacia profesional se relacionaron positiva y significativamente con esas alteraciones, lo que ponen de manifiesto problemas para la salud y para la calidad de vida del individuo. Conclusión: Es importante prevenir el SQT en técnicos de prevención de riesgos laborales, como población de riesgo, para disminuir los problemas de salud en el trabajo.(AU)


Objective: To analyse the prevalence of burnout in a sample of subjects studying degrees related to labour risks prevention, and the relationship between burnout and cardiovascular complaints. Methods: The sample consisted of 312 students of labour risks prevention, professionals from diverse occupational sectors. The variables were burnout, estimated by the Maslach Burnout Inventory-General Survey (MBI-GS), and the frequency of cardiovascular complaints, evaluated by five items. Data were gathered during a training period. Results: Burnout shown a positive and significant relationship with cardiovascular complaints. Moreover, Exhaustion and Cynicism were positively and significantly related with cardiovascular complaints. A negative and significant relationship was found between Professional efficacy and cardiovascular complaints. Some of these alterations have arise when subject are working Conclusion: Burnout must be prevented in order to improve the quality of working life among technicians of prevention of labour risks.(AU)
