Introduction: Periodontitis is an inflammatory disease that affects the supporting tissues of teeth, the effects of excess of nitric oxide, may contribute to the symptoms of periodontitis. Objective: To determine the serum nitric oxide concentration in generalized chronic and aggressive periodontitis patients and to compare it with a healthy subject group from the Mexican population. Materials and methods: A case and control study was performed. Sixty-nine individuals were recruited from the Clínica de Posgrado de Periodoncia of the Centro Universitario de Ciencias de la Salud, Universidad de Guadalajara, México. Patients with clinical features of generalized chronic periodontitis (GCP group, n=19), generalized aggressive periodontitis (GAP group, n=11), and a group of healthy subjects (HS group, n=39) were included in the study. Informed consent was obtained from each subject, and serum nitric oxide concentration was measured by an enzyme-linked immunosorbent assay. Results: Nitric oxide concentration in the study groups was greater in the GCP group (462.57 ± 16.57 µmol/L) than in the GAP group (433.84 ± 18.61 µmol/L) and the HS group (422.46 ± 12.07 µmol/L). A comparison using Student's t-test (one-tailed) between healthy subjects and generalized chronic periodontitis showed borderline significance (p<0.04), whereas no significant differences were observed in HS and GAP groups, with a p-value of 0.64, and the GAP vs. GCP p-value was 0.33. Conclusion: The serum nitric oxide concentration observed in the present study suggests that nitric oxide plays a major role in the inflammatory process, which cannot necessarily be linked to the severity of the disease and periodontal tissue destruction.


Introducción. La periodontitis es una enfermedad inflamatoria que afecta los tejidos de soporte dental; los efectos del exceso de óxido nítrico pueden contribuir a los síntomas de la periodontitis. Objetivo. Determinar la concentración de óxido nítrico en el suero de los pacientes con periodontitis agresiva y crónica generalizada, y compararla con la de individuos sanos de población mexicana. Materiales y métodos. Se trata de un estudio de casos y controles. Se incluyeron 69 individuos de la Clínica de Posgrado de Periodoncia del Centro Universitario de Ciencias de la Salud de la Universidad de Guadalajara. Se dividieron en tres grupos: pacientes con periodontitis crónica generalizada (GCP, n=19), pacientes con periodontitis agresiva generalizada (GAP, n=11) e individuos sanos periodontalmente (HS, n=39). La comparación entre HS y GCP mediante la prueba estadística t de Student (una cola), mostró diferencias significativas (p<0,04), y no se observaron diferencias entre los grupos HS y GAP (p=0,64), ni entre GAP y GCP (p=0,33). Conclusiones. La concentración de óxido nítrico en suero, observada en el presente estudio, sugiere que el óxido nítrico desempeña un importante papel en el proceso inflamatorio, lo que no necesariamente está ligado a la gravedad de la enfermedad ni a la destrucción del tejido periodontal.

Introdução: A doença periodontal, de origem infecciosa, constitui diferentes quadros clínicos de caráter multifatorial e inflamatório. A fotobiomodulação é uma técnica não invasiva que tem demonstrado ser capaz de diminuir a inflamação e proporcionar alívio da dor. Estudos também demonstraram que a adoção da fotobiomodulação adjuvante à raspagem e alisamento radicular tem sido capaz de reduzir a carga bacteriana proveniente da periodontite crônica. Objetivos: Analisar os efeitos da fotobiomodulação como terapia adjuvante à raspagem e ao alisamento radicular no tratamento da periodontite crônica publicados nos últimos cinco anos. Métodos: Trata-se de uma revisão integrativa da literatura, realizada no período de setembro a novembro de 2021, mediante a utilização dos seguintes descritores: "Periodontitis", "Photobiomodulation" e "Therapy, selecionando artigos publicados em inglês durante o período entre 2016 a 2021. Resultados: Foram identificados 47 trabalhos. Destes, foram excluídos 39 estudos que se dispersaram do tema e 2 que se encontravam indisponíveis para o acesso, resultando em 6 artigos que foram consultados integralmente. Foi consenso entre os artigos consultados que a prática da fotobiomodulação tornou-se um potencial agente terapêutico no tratamento da periodontite crônica contribuindo para a redução da contagem de periodontopatógenos e que atua de maneira coadjuvante às raspagens radiculares. Conclusão: A fotobiomodulação mostrou-se uma ferramenta promissora na área odontológica. Entretanto, a grande variedade nos parâmetros de tratamentos e protocolos utilizados na fotobiomodulação impossibilita uma comparação e uma análise mais crítica e rigorosa dos resultados obtidos nos trabalhos analisados.


Introduction: Periodontal disease, which has an infectious origin, constitutes a multifactorial and inflammatory different clinical condition of multifactorial, inflammatory, and infectious origin. Photobiomodulation is a non-invasive technique that has been shown to decrease inflammation and provide pain relief. Studies also have shown that the choosing of photobiomodulation as adjuvant therapy to scaling and root planing has been able to reduce the bacterial load from chronic periodontitis. Objectives: To analyze the effects of photobiomodulation as an adjuvant therapy to scaling and root planing in the treatment of chronic periodontitis in the studies published in the last five years. Methods: This is an integrative literature review, carried out from September to November 2021, using the following descriptors: "Periodontitis", "Photobiomodulation" and "Therapy, from selected articles published in English during the period between 2016 to 2021. Results: From the research and selection of studies to compose this integrative literature review, 47 studies were initially identified based on the descriptors. From those, 39 papers that were outside from the topic and 2 papers that were unavailable for access were excluded from the present review. Then remained, 6 articles that were fully consulted. The practice of photobiomodulation has become a potential therapeutic agent in the treatment of chronic periodontitis reducing the count of periodontopathogens and as an adjunct therapy to root scaling. Conclusion: Photobiomodulation therapy has become a promising tool in the dental field, however, the great variety in the treatment parameters and protocols used for photobiomodulation makes impossible to compare and perform a more critical and rigorous analysis of the results collected in the present work.

AIM: To investigate the confluence of caries and periodontitis indicators from adolescence to elderhood among Americans. MATERIALS AND METHODS: This cross-sectional study explored the grouping among a set of caries and periodontitis indicators (the proportion of sites with bleeding on probing, moderate probing pocket depth [PPD, 4-5 mm], severe PPD [≥6 mm], moderate clinical attachment level [CAL, 3-4 mm], severe CAL [≥5 mm], number of teeth with furcation involvement, number of decayed teeth, number of teeth with pulp involvement, and the number of missing teeth) in 14,421 Americans from the NHANES III study. Exploratory factorial analysis was used to determine the constructs between those indicators (factorial loading ≥0.3). These analyses were stratified by age and confirmed with a confirmatory factorial analysis. We also performed a sensitivity analysis using the NHANES 2011-2014. RESULTS: Two constructs were extracted. The first, Chronic Oral Diseases Burden, grouped caries indicators with moderate PPD and moderate CAL for the youngest subjects (13-39 years old), while for the subjects over 50 years, the Chronic Oral Disease Burden grouped caries indicators with severe CAL and PPD and furcation involvement. The second construct, Periodontal Destruction, grouped only periodontitis indicators. CONCLUSIONS: Caries and periodontitis indicators grouped consistently across the different age ranges in lapse times of 25 years.

Background: Probing of the periodontal pocket is an essential part of the diagnosis of periodontal disease and 15-77% of untreated periodontal patients experience pain during probing. Therefore, the objective of this study is to evaluate the pain perceived by patients with dental plaque-induced gingivitis and chronic periodontitis during periodontal probing and the main objective includes the evaluation of the relationship between pain perceived during periodontal probing and gingival inflammatory parameters. Material and Methods: A total of 475 participants were recruited into the study. The patients were divided into two groups: Group-A (Gingivitis Group - 275 patients) and Group-B (Chronic Periodontitis Group - 200 patients). Clinical parameters included analysis of bleeding on probing, simplified gingival index, pocket depth on probing, and clinical attachment level. Pain score was recorded using the HP VAS scale and all patients participated in the study after a detailed explanation of the study protocol. Results: A significant difference in pain perception was noted between groups, highlighting the role of the degree of inflammation in the examination of periodontal parameters. Conclusion: Within the limitations of the present study, we can conclude that pain perception is directly correlated with the degree of inflammation in periodontitis rather than plaque-induced gingivitis during periodontal probing. Therefore, some form of adjuvant topical anesthesia may be considered in order to reduce pain levels in severely inflamed patients, to encourage continued acceptance of supportive periodontal therapy.


Antecedentes: El sondaje de la bolsa periodontal es una parte esencial en el diagnóstico de la enfermedad periodontal. 15-77% de los pacientes periodontales no tratados experimentan dolor durante el sondaje. Por lo tanto, el objetivo de este estudio es evaluar el dolor percibido por pacientes con gingivitis inducida por placa dental y periodontitis crónica durante el sondaje periodontal y el objetivo principal incluye la evaluación de la relación entre el dolor percibido durante el sondaje periodontal con parámetros inflamatorios gingivales. Material y Métodos: Un total de 475 sujetos fueron reclutados en el estudio. Los sujetos se dividieron en 2 grupos: Grupo - A (Grupo de gingivitis - 275 pacientes) y Grupo - B (Grupo de periodontitis crónica - 200 pacientes). Los parámetros clínicos incluyeron el análisis del sangrado al sondaje, el índice gingival simplificado, la profundidad de la bolsa al sondaje y el nivel de inserción clínica. La puntuación del dolor se registró utilizando la escala HP VAS y todos los pacientes participaron en el estudio después de una explicación detallada del protocolo del estudio. Resultados: Se notó una diferencia significativa en la percepción del dolor en el grupo B que en el grupo A, lo que significa el papel del grado de inflamación en el examen de los parámetros periodontales. Conclusión: Dentro de las limitaciones del presente estudio, podemos concluir que la percepción del dolor se correlaciona directamente con el grado de inflamación que se observa en la periodontitis más que con la gingivitis inducida por la placa dental durante el sondaje periodontal. Por lo tanto, se puede considerar alguna forma de anestesia tópica adyuvante para reducir los niveles de dolor en pacientes gravemente inflamados para fomentar la aceptación continua de la terapia periodontal de apoyo.

Objetivo: estudos sugerem uma associação entre a doença periodontal e a doença pulmonar obstrutiva crônica (DPOC). O objetivo deste estudo foi avaliar a condição periodontal em pacientes com DPOC. Metodologia: Este estudo transversal incluiu 33 pacientes com DPOC e 30 pacientes sem DPOC (grupo controle). Todos os pacientes realizaram espirometria e foram examinados por um dentista. A condição periodontal foi avaliada através dos seguintes índices: número de dentes, índice de placa (IP), índice de sangramento gengival (IG), profundidade de sondagem (PS), PS dos sítios doentes (PSD), nível de inserção clínica (NIC), NIC dos sítios doentes (NICD) e percentual de sítios doentes (NICDP). Para análise estatística foram utilizados o teste de qui-quadrado de Pearson, o testet de Student, análise de regressão logística e cálculo do odds ratio. Resultados: O grupo DPOC apresentou maiores IP (p=0,01), NIC (p=0,001) e NICDP (p<0,001), com odds ratio de 1,2 (95%IC: 1,023­1,408). Conclusões: Os pacientes com DPOC apresentaram pior condição periodontal. Mais estudos são necessários para esclarecer o papel da terapia periodontal no protocolo de acompanhamento dos pacientes com DPOC


Objective: evidence suggests an association between periodontal disease and chronic obstructive pulmonary disease (COPD). The aim of this study was to evaluate the periodontal status in patients with COPD. Methods: this cross-sectional study included 33 patients with COPD and 30 patients without COPD (control group). All patients underwent spirometry and were examined by a dentist. Periodontal status was evaluated by the following indexes: number of teeth, plaque index (PI), gingival bleeding index (GBI), probing depth (PD), PD of diseased sites (PDD), clinical attachment level (CAL), CAL of diseased sites (CALD) and percentage of diseased sites (CALDP). The findings were analyzed under Pearson's chi-square test, independent samples t test and logistic regression with odds ratio. Results: the COPD group had higher PI (p = 0,01), CAL (p = 0,001) and CALDP (p <0,001), and the odds ratio for patients with COPD was 1.2 (95% CI: 1,023-1,408). Conclusions: patients with COPD present worse periodontal status. More studies are needed in order to clarify the role of periodontal therapy in the management of COPD.

Background: Probing of periodontal pockets is an essential part in the diagnosis of periodontal disease. Fifteen to seventy seven percent of untreated periodontal patients experience pain during probing. Hence the aim of this study is to evaluate the pain perceived by patients with gingivitis and periodontitis during periodontal probing. The goals of this study were to compare the patients' pain perception when using a conventional UNC15 probe and a manual pressure sensitive periodontal probe, and to relate the clinical features of gingivitis and periodontitis to the discomfort associated with periodontal probing. Material and Methods: A total of 475 subjects were recruited into the study. The subjects were initially divided into two groups ­ Group ­ A (Gingivitis group - 275 patients) and Group ­ B (Chronic Periodontitis group -200 patients) according to the AAP 1999 Classification. These two groups were further subdivided into two groups each (Gingivitis ­ Conventional Probe ­ GCP, Gingivitis ­ Manual Pressure Sensitive Probe ­ GMPS, Periodontitis - Conventional Probe ­ PCP, Periodontitis ­ Manual Pressure Sensitive Probe ­ PMPS) using a computer generated program of random numbers. Results: A significant difference was noted in pain perception when pressure sensitive probe was used compared to conventional UNC-15 probe. Reduced Bleeding on Probing and Pain scores were noted in Chronic periodontitis subjects with use of pressure sensitive probe, which was statistically significant (p<0.001). Conclusion: Dentistry has changed its focus towards painless dentistry. In this context, the present study presents data towards use of manual pressure sensitive probes, which offers an advantage of low cost when compared to more advanced computerized systems with reduced pain during periodontal examination. It could result in a positive attitude of the patients towards continuous supportive periodontal therapy thereby monitoring periodontal health.


Antecedentes: El sondaje de los sacos periodontales es una parte esencial en el diagnóstico de la enfermedad periodontal. Del quince al setenta y siete por ciento de los pacientes periodontales no tratados experimentan dolor durante el sondaje. De ahí que el objetivo de este estudio fué evaluar el dolor percibido por pacientes con gingivitis y periodontitis durante el sondaje periodontal. Los objetivos de este estudio fueron comparar la percepción del dolor de los pacientes al usar una sonda UNC15 convencional y una sonda periodontal sensible a la presión manual, y relacionar las características clínicas de la gingivitis y la periodontitis con la incomodidad asociada con el sondaje periodontal. Material y Métodos: Un total de 475 sujetos fueron reclutados en el estudio. Los sujetos se dividieron inicialmente en dos grupos - Grupo - A (grupo de Gingivitis - 275 pacientes) y Grupo - B (grupo de Periodontitis Crónica - 200 pacientes) de acuerdo con la Clasificación AAP 1999. Estos dos grupos se subdividieron en dos grupos cada uno (Gingivitis - Sonda convencional - GCP, Gingivitis - Sonda manual sensible a la presión - GMPS, Periodontitis - Sonda convencional - PCP, Periodontitis - Sonda manual sensible a la presión - PMPS) usando un programa generado por computadora de datos aleatorios. números. Resultados: Se notó una diferencia significativa en la percepción del dolor cuando se usó una sonda sensible a la presión en comparación con la sonda UNC-15 convencional (p<0,001). Conclusion: La odontología ha cambiado su enfoque hacia una odontología sin dolor. En este contexto, el presente estudio presenta datos hacia el uso de sondas manuales sensibles a la presión, que ofrece una ventaja de bajo costo en comparación con sistemas computarizados más avanzados con reducción del dolor durante el examen periodontal. Podría resultar en una actitud positiva de los pacientes hacia la terapia periodontal de apoyo continuo, monitoreando así la salud periodontal.

Rheumatoid arthritis (RA) was significantly associated with increased overall risk of periodontitis, both chronic, inflammatory pathologies leading to connective tissue breakdown and bone destruction. To identify clinical and/or serological variables routinely evaluated during follow-up of people with RA which are associated with the severity of their periodontal disease. An observational, cross-sectional study was carried out, which included RA patients according to ACR/EULAR 2010 criteria having chronic periodontal disease. RA clinical parameters (disease duration, erythrocyte sedimentation rate, serum C-reactive protein, disease activity (DAS28) and rheumatoid factor, presence of bone erosions and rheumatic nodules) and also corticosteroid therapy were considered. Periodontitis was evaluated according to the American Academy of Periodontology (1999) and chronic periodontitis was assessed by full mouth periapical radiographic examination, periodontal probing depth, clinical attachment level and bleeding index. A total of 110 subjects with RA and chronic periodontitis were included. The female/male relation was 5.1, and no significant differences between genres were found in rheumatic or oral variables. RA patients with longer disease duration, higher disease activity and with rheumatic nodules had significantly greater periodontitis severity. Multivariate analysis confirmed that severe periodontitis was associated with DAS283 4.1 (OR 51.4, CI 95% 9.4-281.5) and the presence of rheumatic nodules (OR 6.4, CI 95% 1.3-31.6). Disease activity and rheumatic nodules were strongly associated with severe periodontitis. Based on these findings it would be desirable to include interdisciplinary management at an early stage of RA to ensure comprehensive treatment of both pathologies.

RESUMEN Introducción: La diabetes promueve alteración en la función de neutrófilos y la deposición en los tejidos periodontales de los productos finales derivados de la glicolización avanzada. Por ello, los diabéticos son vulnerables a la enfermedad periodontal inflamatoria crónica. Estudiar su comportamiento es indispensable para acometer acciones integrales y efectivas para su control y tratamiento. Objetivo: Determinar el comportamiento de la enfermedad periodontal inflamatoria crónica en pacientes de la Consulta de Atención Integral al Paciente Diabético en el Municipio Santa Cruz del Sur. Métodos: Se realizó un estudio descriptivo transversal, desde septiembre 2018 hasta mayo 2019, en el que se incluyeron 60 pacientes que acudieron durante ese período que presentaban más de 10 dientes en boca, afección periodontal inflamatoria crónica y dieron su consentimiento para participar. Se realizó examen clínico periodontal y examen radiográfico periapical. Resultados: El mayor número de pacientes con enfermedad periodontal inflamatoria crónica fue de 41-60 años de edad, predominó el sexo femenino, en ambas variables prevaleció la periodontitis crónica avanzada, afección preponderante también en los pacientes de más de 10 años de padecimiento de la diabetes mellitus. Conclusiones: En los pacientes con control metabólico inadecuado imperó la periodontitis crónica avanzada seguida de la moderada.


ABSTRACT Introduction: Diabetes promotes impaired neutrophil function and deposition in periodontal tissues of end products derived from advanced glycolization. The refore, diabetics are vulnerable to chronic inflammatory periodontal disease. Studying their behavior is essential to undertake comprehensive and effective actions for their control and treatment. Objective: To determine the behavior of chronic inflammatory periodontal disease in patients of the Comprehensive Care Consultation for Diabetic Patients in the Municipality of Santa Cruz del Sur. Methods: A cross-sectional descriptive study was carried out, in the period from September 2018 to May 2019, in which 60 patients who were attended during this period were included, they had more than 10 teeth in the mouth with chronic inflammatory periodontal disease and gave their consent to participate. A periodontal clinical examination and periapical radiographic examination were performed. Results: The largest number of patients with periodontal disease were 41-60 years old, female sex predominated, in both variables advanced chronic periodontitis prevailed, also preponderant in patients with more than 10 years of suffering from diabetes mellitus. Conclusions: Advanced chronic periodontitis prevailed in patients with inadequate metabolic control, followed by moderate.

ABSTRACT Objective: The purpose of this study was to clinically evaluate the effect of one-stage full-mouth ultrasonic debridement in the treatment of diabetic subjects with severe chronic periodontitis. Methods: Sixteen patients with uncontrolled diabetes (glycated hemoglobin ≥ 7%) and severe chronic periodontitis (at least 8 teeth with a probing depth and bleeding on probing were selected and randomly assigned to 2 treatment groups: control group (n=7) - quadrant-wise scaling and root planning; test group (n=9) - full-mouth ultrasonic debridement in one session of 45 minutes. The following outcomes were assessed: plaque index, gingival index, bleeding on probing, probing depth, clinical attachment level and gingival margin position. All parameters were assessed at baseline, 1 and 3 months after treatment. Probing depth, clinical attachment level and gingival margin position were analyzed using ANOVA and Tukey test, and the other clinical parameters analysed with the Friedman and Mann-Whitney tests. Results: An improvement in probing depth, clinical attachment level was observed after treatment in both groups, but without statistically significant differences between them. After 3 months, in initially deep pockets, the PD decreased 1.6 mm and 1.7 mm in test and control group, respectively. Clinical attachment level gain ≥ 2 mm was statistically significant at sites in control (10.1%) and test (13,4%) groups in 3 months. Conclusions: One-stage full-mouth ultrasonic debridement promoted similar clinical outcomes to those obtained with the scaling and root planning in the treatment of diabetic subjects with severe chronic periodontitis.


RESUMO Objetivo: Avaliar clinicamente o efeito do debridamento ultrassônico de boca toda no tratamento da periodontite crônica severa em pacientes diabéticos. Métodos: Dezesseis pacientes diabéticos descompensados (Hemoglobina Glicada ≥ 7%) com periodontite crônica severa (mínimo de 8 dentes com profundidade de sondagem ≥ 5 mm e sangramento à sondagem) separados aleatoriamente em 2 grupos: Controle (7): raspagem e alisamento radicular por quadrante e Teste (9): debridamento ultrassônico sessão única de 45 minutos. Os parâmetros clínicos avaliados foram: Índice de placa, Índice gengival, Sangramento à Sondagem, Profundidade de Sondagem, Nível de inserção clínico e Posição da Margem Gengival. As variáveis foram avaliadas no início, 1 e 3 meses após o tratamento. Para análise das variáveis quantitativas (Profundidade de sondagem, Nível de inserção e Posição da margem gengival de bolsas moderadas) foram realizados ANOVA e teste de Tukey e para as demais foram utilizados os testes de Friedman e Mann-Whitney. Resultados: Observou-se melhora na profundidade de sondagem e nível de inserção clínica após o tratamento em ambos os grupos, sem diferenças estatisticamente significativas entre eles. Após 3 meses, nas bolsas inicialmente profundas, a profundidade de sondagem reduziu 1,6 e 1,7 mm nos grupos teste e controle, respectivamente. Houve um ganho estatisticamente significante no nível de inserção ≥ 2 mm nos grupos controle (10,1%) e teste (13,4%), aos 3 meses. Conclusão: O debridamento ultrassônico de boca toda promoveu resultados clínicos semelhantes aos obtidos com a raspagem e alisamento radicular no tratamento da periodontite crônica severa em indivíduos diabéticos.

RESUMEN Fundamento: La enfermedad periodontal inmunoinflamatoria crónica se considera un problema de salud bucal a escala mundial; los deportistas también son susceptibles de padecerla. Objetivo: Identificar el estado periodontal de adolescentes de la Escuela de Iniciación Deportiva Provincial Lino Salabarría de Sancti Spíritus. Metodología: Se realizó un estudio observacional descriptivo de corte transversal durante el curso 2020-2021. La población se constituyó por 810 adolescentes de 7.mo a 12.mo grados de la Escuela de Iniciación Deportiva Provincial Lino Salabarría de Sancti Spíritus; se seleccionó la muestra de 36 mediante muestreo probabilístico, aleatorio simple estratificado. Se estudiaron las variables estado periodontal y factores de riesgo. Se determinó el estado periodontal mediante el índice de Rusell y la higiene bucal por el de higiene bucal simplificado. Resultados: Se constató que el 94.4 % de los adolescentes presentó gingivitis y solo el 5.6 % periodontitis. El 100 % presentó higiene bucal deficiente, el 94.2 % apiñamiento dentario y el 91.7 % cálculo dental. Conclusiones: La mayoría de los adolescentes presentó gingivitis y entre los factores de riesgo se constató la totalidad con higiene bucal deficiente y casi todos con apiñamiento dentario y cálculo dental.


ABSTRACT Background: Chronic immune-inflammatory periodontal disease is considered a worldwide oral health problem; athletes are also susceptible to it. Objective: To identify the periodontal state of adolescents from the Sancti Spíritus Provincial Sports Initiation School Lino Salabarría. Methodology: A cross-sectional descriptive observational study was conducted during the 2020-2021 school year. The population consisted of 810 adolescents from 7th to 12th grades from the Sancti Spíritus Provincial Sports Initiation School Lino Salabarría; the sample of 36 was selected by stratified simple random probability sampling. The variables periodontal state and risk factors were studied. Periodontal status was determined by Rusell index and oral hygiene by the simplified oral hygiene index. Results: It was found that 94.4 % of adolescents had gingivitis and only 5.6 % had periodontitis. 100 % presented poor oral hygiene, 94.2 % dental crowding and 91.7 % dental calculus. Conclusions: Most adolescents presented gingivitis and among the risk factors, all had poor oral hygiene also almost all had dental crowding and dental calculus.

Con el objetivo de identificar los factores de riesgo asociados a la periodontitis crónica en pacientes adultos, se realizó un estudio analítico, de casos y controles en el Policlínico David Moreno Domínguez de Santa Rita, Jiguaní, Granma, de octubre 2017 - agosto 2019. De un universo de 989pacientes atendidos en consulta de Periodoncia, se seleccionó una muestra probabilística de 126 pacientes, entre 35 y 64 años (42 casos y 84 controles). Se realizó examen bucal e interrogatorio. Se aplicó el índice periodontal de Rusell y el IAHB de Love. Para el análisis univariado se empleó la razón de productos cruzados Odds Ratio y el Chi-Cuadrado, con una confiabilidad del 95 % y una probabilidad < 0,05. Mostraron asociación a la periodontitis crónica: el tabaquismo (OR= 9,5372; p= 0,0000), la diabetes mellitus (OR= 6,4339; p= 0,0001), la higiene bucal deficiente (OR=5,693; p=0,0000) y el sexo masculino (OR=2,5294, p= 0,0281). El factor de riesgo que presentó mayor valor de asociación a la periodontitis crónica fue el tabaquismo. La edad no constituyó un riesgo a tener en cuenta para la aparición de la enfermedad.


ABSTRACT Periodontitis is the most common chronic inflammatory disease observed in humans, with the objective of identify the risk factors associated with chronic periodontitis in adult patients. An analytical, case-control study was carried out at the David Moreno Domínguez polyclinic in Santa Rita, Jiguaní, Granma, from October 2017 - August 2019. From a universe of 989 patients seen in the Periodontics clinic, a probabilistic sample was selected of 126 patients between 35 and 64 years old (42 cases and 84 controls). Oral examination and interview were performed. Russell's periodontal index and Love's IAHB were applied. For the univariate analysis, the ratio of crossed products Odds Ratio and Chi-Square were used, with a reliability of 95% and a probability <0.05. Smoking (OR = 9.5372; p = 0.0000), diabetes mellitus (OR = 6.4339; p = 0.0001), poor oral hygiene (OR = 5.693; p = 0.0000) and male sex (OR = 2.5294, p = 0.0281) showed an association with chronic periodontitis. The risk factor that presented the highest value of association with chronic periodontitis was smoking. Age was not a risk to take into account for the onset of the disease.

Como objetivo de identificar os fatores de risco associados à periodontite crônica em pacientes adultos, foi realizado um estudo analítico caso-controle na Policlínica David Moreno Domínguez em Santa Rita, Jiguaní, Granma, de outubro de 2017 a agosto de 2019. De um universo de 989 pacientes atendidos no consultório de Periodontia, foi selecionada uma amostra probabilística de 126 pacientes, com idades entre 35 e 64 anos (42 casos e 84 controles). Exame oral e questionamento foram realizados. O índice periodontal de Russell e o IAHB de Love foram aplicados. Para a anúlese univariada, utilizo u-se a raseo dos produtos cruzados Odds Ratio e o Qui-quadrado, com confiabilidade de 95% e probabilidade <0,05. Eles mostraram associação com periodontite crônica: tabagismo (OR = 9,5372; p = 0,0000), diabetes mellitus (OR = 6,4339; p = 0,0001), higiene bucal precária (OR = 5,693; p = 0,0000) e sexo masculino (OR = 2,5294, p = 0,0281). O fator de risco que apresentou maior valor de associação à periodontite crônica foi o tabagismo. A idade não foi um risco a ser levado em consideração para o início da doença.

ABSTRACT: Despite the reported effects of smokeless tobacco (ST) on the periodontium and high prevalence of ST use in rural populations and in males studies on this specific topic are limited. The purpose of this cross-sectional investigation was to measure lipid peroxidation (as an end product of oxidative stress) end product i.e. Malondialdehyde (MDA) in saliva of patients with gingivitis, chronic periodontitis and to assess the influence of smokeless tobacco on Salivary Malondialdehyde (S-MDA). Total 30 patients with gingivitis, 30 with chronic periodontitis and 30 Smokeless Tobacco Chewers with Chronic Periodontitis and 30 periodontally healthy subjects were included in the study. Plaque Index (PI), Gingival Index (GI), Probing Pocket Depth (PD), and Clinical Attachment Loss (CAL) were recorded followed by stimulated Saliva sample collection. Salivary MDA Levels were assessed by UV Spectrophotometry. There was a statistically significant increase in the salivary MDA levels in gingivitis, chronic periodontitis and in smokeless tobacco chewers with chronic periodontitis when compared with healthy group. Higher salivary MDA levels in gingivitis group, chronic periodontitis, and smokeless tobacco chewers with chronic periodontitis reflects increasedoxygen radical activity during periodontal inflammation.


RESUMEN: A pesar de los efectos reportados del tabaco sin humo (TS) sobre el periodonto y la alta prevalencia del uso de TS en poblaciones rurales y en hombres, los estudios sobre este tema específico son limitados. El propósito de esta investigación transversal fue medir el producto final de la peroxidación lipídica (como producto final del estrés oxidativo), es decir, malondialdehído (MDA) en la saliva de pacientes con gingivitis, periodontitis crónica y evaluar la influencia del tabaco sin humo en el malondialdehído salival (S-MDA). Se incluyeron en el estudio un total de 30 pacientes con gingivitis, 30 con periodontitis crónica y 30 masticadores de tabaco sin humo con periodontitis crónica y 30 sujetos periodontalmente sanos. Se registraron el índice de placa (PI), el índice gingival (GI), la profundidad de la bolsa de sondeo (PD) y la pérdida de adherencia clínica (CAL), seguidos de la recogida de muestras de saliva estimuladas. Los niveles de MDA en saliva se evaluaron mediante espectrofotometría UV. Hubo un aumento estadísticamente significativo en los niveles de MDA en saliva en gingivitis, periodontitis crónica y en masticadores de tabaco sin humo con periodontitis crónica en comparación con el grupo sano. Los niveles más altos de MDA en saliva en el grupo de gingivitis, periodontitis crónica y masticadores de tabaco sin humo con periodontitis crónica reflejan un aumento de la actividad de los radicales de oxígeno durante la inflamación periodontal.

RESUMEN La periodontitis es una afección crónica que afecta los tejidos de soporte del diente lo que conlleva en ocasiones a la pérdida del mismo. El objetivo fue evaluar la eficacia y seguridad del plasma rico en plaquetas en el tratamiento de la periodontitis crónica. Se realizó un ensayo clínico en fase II b, controlado, aleatorizado, con evaluación a ciegas por terceros. La muestra abarcó 70 pacientes portadores de periodontitis crónica que acudieron a consulta de Periodoncia de la Clínica Universitaria de Especialidades Estomatológicas "Manuel Cedeño" entre enero 2017 a diciembre del 2019. Los individuos quedaron distribuidos aleatoriamente en: grupo A o experimental tratado con plasma rico en plaquetas y grupo B o control con tratamiento convencional. Las variables fueron: edad, sexo, inflamación de la encía, profundidad de las bolsas periodontales, pérdida de inserción periodontal, tiempo de evolución, efecto terapéutico y eventos adversos. Los resultados mostraron que a los 12 meses del tratamiento curaron 34 pacientes del grupo experimental (97,14 %) y 33 pacientes del grupo control (94,39 %). No se detectaron eventos adversos. Se llegó a la conclusión que el tratamiento con plasma rico en plaquetas en la periodontitis crónica demostró eficacia terapéutica y seguridad sin diferencias significativas con respecto al tratamiento convencional.


ABSTRACT Periodontitis is a chronic condition that affects the supporting tissues of the tooth, which sometimes leads to tooth loss. The objective was to evaluate the efficacy and safety of platelet-rich plasma in the treatment of chronic periodontitis. A randomized, controlled, phase II b clinical trial was conducted with blind third-party evaluation. The sample comprised 70 patients with chronic periodontitis who attended the Periodontics consultation of the "Manuel Cedeño" University Clinic of Stomatological Specialties between January 2017 and December 2019. The individuals were randomly distributed into: group A or experimental group treated with plasma rich in platelets and group B or control with conventional treatment. The variables were: age, sex, inflammation of the gingiva, depth of periodontal pockets, loss of periodontal attachment, time of evolution, therapeutic effect and adverse events. The results showed that at 12 months of treatment 34 patients in the experimental group (97,14 %) and 33 patients in the control group (94,39 %) cured. No adverse events were detected. It was concluded that treatment with platelet-rich plasma in chronic periodontitis demonstrated therapeutic efficacy and safety without significant differences with respect to conventional treatment.

Um ensaio clínico randomizado, controlado, fase II b foi conduzido com avaliação cega por terceiros. A amostra foi composta por 70 pacientes com periodontite crônica que compareceram à consulta de Periodontia da Clínica Universitária de Especialidades Estomatológicas "Manuel Cedeño" entre janeiro de 2017 e dezembro de 2019. Os indivíduos foram distribuídos aleatoriamente em: grupo A ou grupo experimental tratado com plasma rico em plaquetas e grupo B ou controle com tratamento convencional. As variáveis ​​foram: idade, sexo, inflamação da gengiva, profundidade das bolsas periodontais, perda da inserção periodontal, tempo de evolução, efeito terapêutico e eventos adversos. Os resultados mostraram que, aos 12 meses de tratamento, 34 pacientes do grupo experimental (97,14%) e 33 pacientes do grupo controle (94,39%) curaram. Nenhum evento adverso foi detectado. Concluiu-se que o tratamento com plasma rico em plaquetas na periodontite crônica demonstrou eficácia terapêutica e segurança sem diferenças significativas em relação ao tratamento convencional.

PURPOSE: Periodontitis is associated with a dysbiosis of periodontopathic bacteria, which stimulate the interleukin (IL)-23/IL-17 axis that plays an essential role in the immunopathogenesis of this disease, leading to alveolar bone destruction through receptor activator of nuclear factor κB ligand (RANKL). IL-23 receptor mRNA (IL-23R) has been identified in periodontitis, and IL-17 receptor A mRNA (IL-17RA) and its protein have not yet been evaluated in patients with periodontitis. In this study was measure IL-23R and IL-17RA in gingival tissue (GT) from patients with generalized chronic periodontitis (GCP) and generalized aggressive periodontitis (GAP) and to explore correlations with clinical parameters. METHODS: We included 16 healthy subjects (HS), 18 patients with GCP, and 14 with GAP. GT samples were collected during periodontal surgery. Both IL-23R and IL-17RA were detected by enzyme-linked immunosorbent assay. RESULTS: The results were analyzed with Mann-Whitney U test and Spearman' rank correlation coefficients using SPSS version 25.0. We found lower IL-23R levels in patients with GCP and GAP than in HS. Contrarily, we observed higher IL-17RA levels in GCP and GAP patients than in HS. Moreover, we found negative correlations between IL-23R in GT and probing depth and clinical attachment loss (CAL). Likewise, a positive correlation of IL-17RA in GT with CAL was found. CONCLUSIONS: The results of these findings suggest that the reverse behavior between IL-23R and IL-17RA in periodontitis patients may also be involved with the activation of RANKL, which promotes alveolar bone loss.

RESUMEN Introducción: El tratamiento periodontal no quirúrgico corresponde a la terapia estándar para la periodontitis crónica. En este contexto, se ha propuesto que el uso de probióticos orales como complemento al tratamiento estándar podría mejorar los resultados obtenidos clínicamente, sin embargo no existe claridad respecto a su efectividad. Métodos: Se realizó una búsqueda en Epistemonikos: base de datos de revisiones sistemáticas en salud mantenida mediante el cribado de múltiples fuentes de información, la cuál incluye MEDLINE, EMBASE, Cochrane, entre otras. Se extrajeron los datos desde las revisiones identificadas y un análisis de los datos de los estudios primarios. Se realizó un metanálisis y se preparó una tabla de resumen de los resultados utilizando el método GRADE. Resultados y conclusiones: Se Identificaron seis revisiones sistemáticas que en conjunto incluyeron 14 estudios primarios, todos correspondientes a ensayos aleatorizados. De los resultados analizados se concluye que el uso de probióticos orales como complemento al tratamiento no quirúrgico de periodontitis crónica probablemente resulta en nula o poca diferencia en la reducción de la profundidad de sondaje, el sangrado al sondaje y una ganancia de nivel de inserción clínico en los sitios afectados. Por otro lado, no se encontraron estudios que evaluaran el riesgo de presentar halitosis en pacientes sometidos a terapia periodontal no quirúrgica con adición de probióticos.


ABSTRACT: Introduction: Non-surgical periodontal treatment is the standard treatment to chronic periodontitis. It has been proposed that the use of oral probiotics as an adjunctive therapy may improve clinical outcomes, however there is no clarity regarding its effectiveness. Methods: We searched in Epistemonikos, the largest database of systematic reviews in health, which is maintained by screening multiple information sources, including MEDLINE, EMBASE, Cochrane, among others. We extracted data from the systematic reviews, reanalyzed data of primary studies, conducted a meta-analysis and generated a summary of findings table using the GRADE approach. Results and conclusions: We identified six systematic reviews including 14 studies overall, of which all were randomized trials. We concluded that the addition of probiotics to non-surgical periodontal treatment probably results in little or no difference in probing depth reduction, clinical attachment level gain and reduction in bleeding on probing. No studies were found evaluating halitosis in patients undergoing non-surgical periodontal treatment with the addition of probiotics.

Tener una clasificación de enfermedades periodontales es de gran utilidad para los clínicos al brindar una guía sistematizada de las distintas manifestaciones de los problemas periodontales. La nueva clasificación de enfermedades y condiciones periodontales de 2017 tiene como una de sus características principales la agrupación de la periodontitis con estadios y grados, como manifestación de enfermedades sistémicas y enfermedades periodontales necrosantes; así como la introducción de un apartado que habla de salud y enfermedad peri-implantaria. En el caso de la periodontitis, el estadio se refiere a la severidad de la enfermedad y la complejidad del tratamiento; mientras que el grado se refiere al ritmo del progreso y riesgo de recidiva de la enfermedad, en este trabajo se ofrece una ruta de diagnóstico didáctica y se desarrollan las principales diferencias entre las últimas dos clasificaciones, al tiempo que compartimos la experiencia al implementar esta nueva clasificación en nuestro instituto.


Having a classification of periodontal conditions and diseases is a very useful tool for clinicians by providing a systematic guide to different manifestations of periodontal issues. The 2017 classification of periodontal diseases and conditions has as one of the main characteristics the division of the Periodontitis group classified by stage, which assigns the severity and complexity of the treatment, and by grade, which refers to the rhythm of progression and the risk of future relapse; periodontitis as a manifestation of systemic conditions and necrotizing periodontal diseases. As well as the featuring of peri-implant health and diseases. A more didactic route of diagnostic will be given in this publication and the main differences between the last two classifications will be explained, while we share the experience of implementing this new classification in our institute.

Introducción: La periodontitis es una enfermedad infecciosa que afecta a los tejidos de sostén de los dientes, su prevalencia se estima entre el 35 y 45% de la población adulta. Las enfermedades cardiovasculares tienen relación directa con la periodontitis crónica, las bacterias periodontales pueden pasar la barrera epitelial de los tejidos periodontales y lograr la propagación sistémica a través de los vasos sanguíneos, causando la inflamación del endocardio. Objetivo: Determinar prevalencia de enfermedad periodontal como factor de riesgo cardiovascular en adultos de 25 a 60 años en Nacajuca, Tabasco. Material y métodos: Se realizó un estudió observacional, prospectivo, transversal y analítico, con muestra de 40 participantes entre 25 y 60 años, el 52% (21) son hombres y el 49% (19) mujeres, se empleó el sistema no probabilístico por conveniencia. Las variables fueron edad, género, grado de conocimiento sobre enfermedad periodontal, nivel de alimentación, grado de tabaquismo, grado de higiene bucal y grado de periodontitis. Resultado: La prevalencia de enfermedad periodontal como factor de riesgo cardiovascular es 48% (19 personas) con periodontitis crónica moderada y avanzada generalizada. Conclusión: Con base en la revisión bibliográfica, las personas con el grado de periodontitis crónica moderada y avanzada generalizada son propensas a desencadenar una enfermedad cardiovascular (infarto agudo de miocardio). Sin embargo, son necesarios más estudios de experimentación longitudinal, con base en el vínculo que tiene la enfermedad periodontal y cardiovascular (AU)


Introduction: Periodontitis is an infectious disease that affects the tissue of the teeth, its prevalence is estimated between 35 and 45% of the adult population. Cardiovascular diseases are directly related to chronic periodontitis, periodontal bacteria can pass the epithelial barrier of periodontal tissues and achieve systemic propagation through the blood vessels causing Inflammation of the endocardio. Objective: To determine the prevalence of periodontal disease as a cardiovascular risk factor in adults from 25 to 60 years Nacajuca, Tabasco. Material and methods: An observational, prospective, transverse and analytical study was carried out, with a sample of 40 participants between 25 and 60 years, 52% (21) corresponds to the masculine genus and 49% (19) represents the female genus, the non-probabilistic system was employed by Convenience. The variables were age, gender, degree of knowledge on periodontal disease, feeding level, degree of smoking, degree of oral hygiene and degree of periodontitis. Result: the prevalence of periodontal disease as a cardiovascular risk factor is 48% (19 people) with chronically moderate and advanced generalized periodontitis. Conclusion: Based on the bibliographical review people with the degree of chronic periodontitis moderate and advanced generalized are prone to trigger a cardiovascular disease (acute myocardial infarction). However, more studies of longitudinal experimentation are necessary, based on the link which has the periodontal and cardiovascular disease (AU)

Although the standard treatment for periodontal disease is based on scaling and root planing (SRP), the use of antimicrobial photodynamic therapy (aPDT) has been studied as a complement to obtain better clinical results. The purpose of this study was to evaluate the effect of aPDT as adjuncts to SRP, compared with SRP alone, on clinical parameters of chronic periodontal patients. Only randomized controlled trials with at least 3-month follow-ups, of SRP alone and in association with aPDT, were included. The MEDLINE (PubMed), Google Scholar, and LILACS databases were searched for articles published up to July 2020. Random-effects meta-analyses were conducted for clinical attachment level (CAL) and probing pocket depth (PPD) change after treatment. Of 141 potentially relevant papers, 22 were included. The association between SRP and aPDT promoted a significant CAL gain and PPD reduction. Periodontal treatment was partially improved by aPDT, and a favorable effect of indocyanine green-mediated aPDT was observed, and high concentrations of phenothiazine chloride presented clinical improvement as well.

BACKGROUND: The aim of this study was to evaluate the clinical, radiographic and patient-centered results of enamel matrix derivative (EMD) therapy in intrabony defects in aggressive periodontitis (AgP) patients and compare them with those in chronic periodontitis (CP) patients. METHODS: Sixty intrabony defects in AgP and CP patients associated with ≥ 6 mm residual probing pocket depth (PPD) were included and randomly assigned to one of three groups: AgP+CS (conservative surgery) (n = 20); AgP+CS/EMD (n = 20); CP+CS/EMD (n  =  20). Clinical parameters were measured at baseline and after 6 and 12 months. Defect resolution (DR) and bone filling (BF) were used for radiographic analysis. The quality of life was recorded at baseline and 6 months using OHIP-14 and VAS scale in the early post-therapy period. RESULTS: PPD and relative clinical attachment level (rCAL) improved for all groups during follow-up (P ≤ 0.05), and AgP+CS/EMD presented a higher rCAL gain (2.4 ± 1.0 mm) when compared to AgP control patients (1.6 ± 1.6 mm, P ≤ 0.05) after 12 months. No difference was observed between AgP+CS/EMD and CP+CS/EMD groups (P > 0.05). No radiographic differences were observed among groups at any time point (P > 0.05). All the groups reported a positive impact on OHIP-14 total score, without differences among them. CONCLUSIONS: EMD therapy of intrabony defects promotes additional benefits in AgP patients, presenting a similar regeneration rate compared to CP patients, and has proven to be a viable therapy for the treatment of individuals with AgP.
