RESUMO

Background: Cardiac diseases are among the leading causes of death worldwide, including sudden cardiac arrest in particular. Nursing professionals are often the first to encounter these scenarios in various settings. Adequate preparation and competent knowledge among nurses significantly impact survival rates positively. Aim: To describe the state of knowledge about Basic and Advanced Life Support guidelines among Ecuadorian nursing professionals. Methodology: A nationwide, descriptive, cross-sectional study was conducted from February to April 2023 among Ecuadorian nursing professionals. Participants were invited through official social media groups such as WhatsApp and Facebook. The study utilized a self-administered online questionnaire to evaluate theoretical knowledge of Basic Life Support (BLS) and Advanced Life Support (ALS). Knowledge scores were assigned based on the number of correct answers on the tests. T-tests and one-way ANOVA were used to examine relationships between knowledge scores and demographic and academic training variables. Results: A total of 217 nursing professionals participated in the study. The majority of the participants were female (77.4%) and held a university degree (79.9%). Among them, only 44.7% claimed to have obtained a BLS training certificate at least once, and 19.4% had ALS certification. The overall BLS knowledge score (4.8/10 ± 1.8 points) was higher than the ALS score (4.3/10 ± 1.8 points). Participants who had obtained BLS certification and those who used evidence-based summaries as a source of extracurricular training achieved higher BLS and ALS knowledge scores. Conclusion: Ecuadorian nursing professionals in this study exhibited a significant deficiency in theoretical knowledge of BLS and ALS. Formal training and preparation positively impact life support knowledge. Support and inclusion of Ecuadorian nurses in training and academic preparation programs beginning at the undergraduate level are essential for promoting life support knowledge and improving outcomes.

RESUMO

BACKGROUND: Education mediated by simulation is a widely used method for teaching basic life support (BLS). The American Heart Association recommends protocols based on scientific evidence to reduce sequelae and mortality. We aimed to assess learning and retention of knowledge of BLS in students of the first semester of the medical course using teaching methods of dialogic expository class (group 1), expository and demonstrative class (group 2), and the two previous methodologies associated with simulated practice (group 3), and after 3 months, memory retention. MATERIALS AND METHODS: This was an experimental, prospective, randomized study. Participants were assessed in terms of performance in theoretical and simulated practical tests, satisfaction with training (Likert scale), and knowledge retention. RESULTS: The practical test results were analyzed by two experienced observers. Students had 20% progression in knowledge and 80% retention of knowledge after 3 months of exposure comparing the theoretical pre- and posttest. The students in group 3 performed better than the others (P = 0.007) in the posttest. With the simulated practice, the knowledge acquired was maintained after 3 months with a mean performance of 90%, but in the test of the infant age group, there was a loss of learning retention by 10%. There was no difference of the results between the two evaluators (P < 0.001). The training was positively assessed by the participants. CONCLUSION: The use of different methodologies promoted knowledge progression, with emphasis on simulated practice. Learning retention was maintained after 3 months. In order to teach BLS to infants, it may be necessary to improve teaching techniques.

RESUMO

Whilst CPR training is widely recommended, quality of performance is infrequently explored. We evaluated whether a checklist can be an adequate tool for chest compression quality assessment in schoolchildren, compared with a real-time software. This observational study (March 2019-2020) included 104 schoolchildren with no previous CPR training (11-17 years old, 66 girls, 84 primary schoolchildren, 20 high schoolchildren). Simultaneous evaluations of CPR quality were performed using an observational checklist and real-time software. High-quality CPR was determined as a combination of 70% correct maneuvers in compression rate (100-120/min), depth (5-6 cm), and complete release, using a real-time software and three positive performance in skills using a checklist. We adjusted a multivariate logistic regression model for age, sex, and BMI. We found moderate to high agreement percentages in quality of CPR performance (rate: 68.3%, depth: 79.8%, and complete release: 91.3%) between a checklist and real-time software. Only 38.5% of schoolchildren (~14 years-old, ~54.4 kg, and ~22.1 kg/m2) showed high-quality CPR. High-quality CPR was more often performed by older schoolchildren (OR = 1.43, 95%IC:1.09-1.86), and sex was not an independent factor (OR = 1.26, 95%IC:0.52-3.07). For high-quality CPR in schoolchildren, a checklist showed moderate to high agreement with real-time software. Better performance was associated with age regardless of sex and BMI.

OBJECTIVE: To assess the competence of schools and physical education teachers in the prevention of sudden death (SD) and basic life support meassures, including availability and use of the automatic external defibrillator (AED). METHODS: A descriptive cross-sectional study included physical education teachers from different Schools (public, subsidized and private) in Santiago, Chile. An online questionnaire was applied including demographic data of the teacher, comentence and willingness to perform cardiopulmonary resuscitation (CPR), presence and use of AED and school preparation in case of a SD. RESULTS: From a total of 97 teachers surveyed do not feel capable of performing CPR, in spite of the fact that 70% of them completed a CPR course. Among those who took a course, their willingness to perform resuscitation was 99%, compared to 83% en those not having taken the course (p = 0.003). There was a marked difference in DEA availability according to class of school (private 52,4%, public 29,6 subsidized 15%, p=0,001), but the knowledge on how to use de DEA was uniformly insufficient (42,9 %, 40,7% and 25%, p=0,43) Most schools do not have a protocol to face SD. CONCLUSIONS: We observed that schools and physical education teachers have an insufficient and outdated preparation in sudden death prevention and basic life support. The availability of AED differed according to the class of establishment.

RESUMO

The sudden cardiac arrest (CA) and death of athletes are dramatic and emotionally impacting events for health professionals, family, and society. Although the practice of sport participation improves general health, physical fitness, and quality of life, intense physical exercise can be a trigger for CA and sudden death occasionally in the presence of known or unknown cardiac disorders (mainly hypertrophic cardiomyopathy) and risk factors (environment, health style, family, and genetic). The present review found that sudden death associated with CA was not such a common event in competitive athletes, but it might be an underestimated event in recreational athletes. Thus, considering the exponential increase in sport participation, both in a recreational or competitive way, and the rate of sudden CA, knowledge of implementing prevention and treatment strategies is crucial. This includes preparation of health professionals and lay people in basic life support (BLS); screening and pre-participation assessment in sport programs and health education; and promotion for the recognition of CA and early completion of BLS and rapid access to automatic external defibrillator to improve the victim survival/prognosis. Thus, the purpose of this review is to provide for health professionals and lay people the most updated information, based on current guidelines, of how to proceed in an emergency situation associated with sudden CA of young adult athletes.

RESUMO

BACKGROUND: Cardiopulmonary resuscitation is usually taught in universities through theoretical lectures and simulations on mannequins with low retention of knowledge and skills. New teaching methodologies have been used to improve the learning, placing the student at the center of the process. Likewise, the outside community knows next to nothing about cardiopulmonary resuscitation. Patients who have an out-of-hospital cardiac arrest will die if the effective maneuvers are not promptly done. Learning by teaching could be a way to answer both requirements. It was therefore decided to evaluate whether the medical students' cardiopulmonary resuscitation performance would improve when they teach other people, and if those people could learn with them effectively. METHODS: A non-randomized controlled trial was designed to assess whether teaching Basic Life Support would increase students' learning. Socially engaged, seeking to disseminate knowledge, 92 medical students were trained in Basic Life Support and who subsequently trained 240 community health professionals. The students performed theoretical and practical pre- and post-tests whereas the health professionals performed theoretical pre- and post-tests and one practical test. In order to assess the impact of teaching on students' learning, they were divided into two groups: a case group, with 53 students, reassessed after teaching health professionals, and a control group, with 39 students, reassessed before teaching. RESULTS: The practical students' performance of the case group went from 13.3 ± 2.1 to 15.3 ± 1.2 (maximum = 17, p < 0.001) and theoretical from 10.1 ± 3.0 to 16.4 ± 1.7 (maximum = 20, p < 0.001) while the performance of the control group went from 14.4 ± 1.6 to 14.4 ± 1.4 (p = 0.877) and from 11.2 ± 2.6 to 15.0 ± 2.3 (p < 0.001), respectively. The theoretical performance of the health professionals changed from 7.9 ± 3.6 to 13.3 ± 3.2 (p < 0.001) and the practical performance was 11.7 ± 3.2. CONCLUSIONS: The students who passed through the teaching activity had a theoretical and practical performance superior to that of the control group. The community was able to learn from the students. The study demonstrated that the didactic activity can be an effective methodology of learning, besides allowing the dissemination of knowledge. The University, going beyond its academic boundaries, performs its social responsibility.

RESUMO

AIM: The study evaluated the theoretical knowledge and practical ability of students in paediatric dentistry concerning basic life support (BLS) and cardiopulmonary resuscitation (CPR) in children and babies. METHODS: Seventy paediatric dentistry students answered a questionnaire and also performed a simulation of the manoeuvres of BLS and CPR on baby and child manikins. RESULTS: The results showed that 41 (58%) students had never received BLS training. When questioned about the correct ratio of compression and ventilation during CPR, most students answered incorrectly. For the CPR of babies in the presence of a first responder only 19 (27.1%) answered correctly (30 × 2), and for babies with two rescuers, 23 (32.8%) answered correctly (15 × 2); in relation to the correct rhythm of chest compressions, 38 (54.4%) answered incorrectly; when asked if they felt prepared to deal with a medical emergency in their dental surgeries, only 12 (17.1%) stated "yes". In the practice evaluation, 51 (73%) students who had been assessed in CPR manoeuvres for children and 55 (78%) in the manoeuvres for babies scored inadequately. CONCLUSIONS: The evaluated students did not have adequate knowledge about CPR in children and babies.

The teaching-learning process in the contemporary world must be cause for concern for the advancement of technology and the profile of digital natives. The use of Digital Educational Technologies (DET) should be inserted in the teaching of Cardiopulmonary Resuscitation (CPR). The American Heart Association (AHA) encourages the teaching of CPR in different modalities. Objective: To develop DET - videotape, simulation video, and evaluation instruments - theoretical and practical, on adult CPR in Basic Life Support (BLS), using the Automatic External Defibrillator (AED) in the hospital setting. Methodology: This is an applied research and technological production developed at the Ribeirão Preto Nursing School of the University of São Paulo (EERP / USP). The population was composed of 16 experts in Urgency and Emergency (UE). For the evaluation and validation, through adapted instruments, of the script/screenplay and storyboard of a videotape, script/screenplay and storyboard of a simulation video, questionnaire and Objective and Structured Clinical Examination (OSCE) on adult CPR in BLS with the use of AED in a hospital environment. For selection of the expertises, a minimum score of five points was adopted according to Fehring (1987) criteria. The model proposed by Fleming, Reynolds and Wallace (2009), which consists of Phase I: Pre-production, Phase II: Production and Phase III: Post-production, was adopted. For the preparation of the questionnaire the basic rules for the elaboration of the Multiple Choice Question (MCQ) of the manual of the National Council of Medical Examiners were applied. The OSCE has been developed in accordance with the guidelines of the Medical Council of Canada. The AHA guidelines published in 2015 were the first adopted literature. The research was approved by the Research Ethics Committee (REC), according to Resolution 466/2012. For the evaluation of the data, descriptive statistics and interrater concordance analysis by Gwet's AC1 were applied. Categorization of agreement according to Landis and Koch (1997). RESUMO

OBJECTIVES: To describe learning outcomes of undergraduate nursing students following an online basic life support course (BLS). METHODS: An online BLS course was developed and administered to 94 nursing students. Pre- and post-tests were used to assess theoretical learning. Checklist simulations and feedback devices were used to assess the cardiopulmonary resuscitation (CPR) skills of the 62 students who completed the course. RESULTS: A paired t-test revealed a significant increase in learning [pre-test (6.4 ± 1.61), post-test (9.3 ± 0.82), p < 0.001]. The increase in the average grade after taking the online course was significant (p<0.001). No learning differences (p=0.475) had been observed between 1st and 2nd year (9.20 ± 1.60), and between 3rd and 4th year (9.67 ± 0.61) students. A CPR simulation was performed after completing the course: students checked for a response (90%), exposed the chest (98%), checked for breathing (97%), called emergency services (76%), requested for a defibrillator (92%), checked for a pulse (77%), positioned their hands properly (87%), performed 30 compressions/cycle (95%), performed compressions of at least 5 cm depth (89%), released the chest (90%), applied two breaths (97%), used the automated external defibrillator (97%), and positioned the pads (100%). CONCLUSIONS: The online course was an effective method for teaching and learning key BLS skills wherein students were able to accurately apply BLS procedures during the CPR simulation. This short-term online training, which likely improves learning and self-efficacy in BLS providers, can be used for the continuing education of health professionals.

The new developments in pediatric cardiopulmonary resuscitation (CPR) (AHA 2015) and basic and advanced life support are reviewed schematically in cases of cardiopulmonary arrest in children. In this article we summarize the key aspects and major changes made to the previous update (2010). It is suggested that a Pediatric Emergency Team and a Rapid Response Team to provide fast and effective care should be incorporated in hospitals. A scoring system (PEWS) is described that will allow for the identification and prediction of patients at risk of decompensation due to different etiologies. Initial treatment of severe sepsis and septic shock with a fluid bolus of 20 ml/kg in infants and children with crystalloid or colloid fluids is indicated. The C-A-B sequence from the 2010 guides for pediatric CPR is maintained (although the recommendations of ILCOR 2015 attach the same importance to both "A-B-C" and "C-A-B" sequences). The depth of chest compression should be at least one third of the anteroposterior diameter, or approximately 4 cm in infants. In children and adults compressions are equivalent to depressing no more than 5 to 6 cm, with a compression frequency of 100 to 120 CPM. If lay rescuers do not want, or cannot, provide sufficient ventilation, we recommend that chest compressions should be performed. For treatment of non-pulsed VF and VT, an initial shock (2 to 4 J/kg) with mono or biphasic defibrillator should be applied, followed by chest compressions; subsequent doses of ≥ 4 J/kg are safe and effective.

RESUMO

Abstract: Introduction: Basic life support (BLS) are a group of maneuvers that constitute a fundamental tool to save lives. Therefore, knowledge about BLS has to be part of the curriculum in medical schools. Objective: Evaluate the knowledge level about BLS and evaluate its associated socio-educational factors, in medical students from nine Peruvian universities. Material and methods: We carried out a cross-sectional multicenter study in 2013 and 2014. We included undergraduate medical students from nine universities in eight regions or Peru. We measured the knowledge about BLS using a validated questionnaire, which was based in American Heart Association Guidelines (2010); adequate knowledge was defined as a minimum proportion of 50% of correct answers. We estimate adjusted prevalence ratios using generalized linear models. Results: We included 1,564 students, 13% of the students had an adequate knowledge, and average score was 6.3 ± 3.2 (range: 0-16). Adequate knowledge was associated with attending semesters that belong to academic stage of clinical sciences (p = 0.02; aPR: 1.82; CI: 95%: 1.11-2.98) and having received a previous BLS course (p < 0.01; aPR: 2.96; CI: 95%: 2.18-4.01); adjusted by age, sex, having received a previous injections and first aids courses, and to manifest the desire to be trained in practical BLS. Conclusion: A low proportion of students have an adequate knowledge level about BLS in this population.

Resultados: 86 estudantes eram do sexo feminino e 18 do sexo masculino, a idade média 23,32±6,66 anos.

Cardiac arrest is the leading cause of death in developed and developing countries, and mainly occurs in non-hospital environments. With this global scenario, which is considered a present public health problem, training laypeople in Basic Life Support (BLS) with clinical simulation has an important role in improving survival rate of the cardiac arrest victims. This present study aims to evaluate the knowledge (theoretical) and skills (practical) of laypeople before and after the BLS training underwent to teaching and learning strategies, expository and dialogue class and practical activities in skills lab or simulation class in CPR/CPA. This study has an almost experimental, quantitative and time-series approach. The population study is composed of students from the first to fourth semester of Nursing at Federal University of Mato Grosso (UFMT) and the sample of 104 participants in BLS for laypeople in two steps. In Step 1, all the students had to undergo to the theoretical pretest, expository and dialogue class followed by practical activity in the skills lab with a mid-fidelity manikin and using automated external defibrillator (AED), clinical simulation in the lab using the same manikin and AED and immediate theoretical posttest. Step 2 occurred from the 15th to 20th days after the first one and all the students had to undergo to clinical simulation in the lab using the same mid-fidelity manikin and AED followed by immediate theoretical posttest. It was elaborated theoretical evaluation tools, simulation scenario and OSCE - objective structured clinical examination in the clinical scenario with checklist, validated in aspect and content by a judges committee. The data collection strategy was the BLS for laypeople. It was evaluated the theoretical tests and OSCE (objective structured clinical examination) - clinical examination structured in Simulation Lab, using as tool the mid-fidelity manikin and AED. The activity was filmed and analysed by three evaluators. The outcomes of Step 1 and 2 of BLS for laypeople were analysed: theoretical performance on theoretical and practical tests in OSCE, total execution time, each OSCE realm and the quality of external chest compression (ECC) such as frequency and deepness in 120 seconds of cardiopulmonary resuscitation (CPR). Outcomes: 86 female and 18 male students, median age 23.32±6.66. Median score in pretest was 3.52±2.03 and the average 3.0. At immediate posttest (post 1) the median was 8.01±1.19 (average = 8.0) and at immediate posttest (post 2) 7.41±2.68 (average = 8.5). Comparing the pretest averages and posttests 1 and 2, there is a statistically significant difference (p<0.001) of knowledge retention. Comparing theoretical posttests 1 and 2 revealed the statistically significant difference (p=0.019) with knowledge loss at posttest 2. Step 1 OSCE average (posttest 1) was 3.63±0.30 and Step 2 (posttest 2) 3.63±0.31; when compared, these scores demonstrated that there is not a statistically significant difference (p=0.966). The OSCE execution time average at posttest 1 was 156,96±8,16 seconds and at posttest 2 138.68±43.58 seconds, which demonstrated statistically significant difference (p<0.001). OSCE intervention realm "Arrival on the scene" showed posttest 1 20,78±6,20s and posttest 2 16,46±4,56s averages, the realm "Check Responsiveness" had averages of 6,38±3,48s in posttest 1 and 5,13±2,36s in posttest 2, the realm "Using DEA" averages of 77,30±14,45s in posttest 1 and posttest 2 of 66,80±11,44s, all of them with statistically significant difference (p<0,001). Evaluating the quality of ECT the reached deepness average in millimeters (mm) resulted 36.26±9.62 (average 35.00) in posttest 1 and 39.36±10.96 (average 40.00) in posttest 2. The number of ECC per minute showed 116.01±21.72 (average 116.00) in posttest 1 and 98.94±19.52 (average 100.00) in posttest 2, the total of compressions (ECC) in 120 seconds showed 227.88±41.81 (average 226.00) and 197.31±30.42 (average 195,50) in posttest 2, the number of surface ECC 132.20±89.94 (average 146.50) and 74.96±75.76 (average 53,00) in posttest 2, all of them with statistically significant difference (p<0,001). In the incomplete ECC showed 25.09±56.95 (average 0,00) in posttest 1 and 14.51±35.45 (average 0,00) in posttest 2 with statistically significant difference (p=0,012). In this present study, besides the different teaching strategies approaches for laypeople, the outcomes demonstrate that they were effective and the learning goals reached, since the posttests had higher scores in previous knowledge and skills of steps 1 and 2 of OSCE. However it showed a loss of knowledge and skills retention in 15 to 20 days after the BLS training for laypeople, what demonstrates the need of permanent training

RESUMO

We aimed to evaluate knowledge of first aid among new undergraduates and whether it is affected by their chosen course. A questionnaire was developed to assess knowledge of how to activate the Mobile Emergency Attendance Service - MEAS (Serviço de Atendimento Móvel de Urgência; SAMU), recognize a pre-hospital emergency situation and the first aid required for cardiac arrest. The students were also asked about enrolling in a first aid course. Responses were received from 1038 of 1365 (76.04%) new undergraduates. The questionnaires were completed in a 2-week period 1 month after the beginning of classes. Of the 1038 respondents (59.5% studying biological sciences, 11.6% physical sciences, and 28.6% humanities), 58.5% knew how to activate the MEAS/SAMU (54.3% non-biological vs 61.4% biological, P=0.02), with an odds ratio (OR)=1.39 (95%CI=1.07-1.81) regardless of age, sex, origin, having a previous degree or having a relative with cardiac disease. The majority could distinguish emergency from non-emergency situations. When faced with a possible cardiac arrest, 17.7% of the students would perform chest compressions (15.5% non-biological vs 19.1% biological first-year university students, P=0.16) and 65.2% would enroll in a first aid course (51.1% non-biological vs 74.7% biological, P<0.01), with an OR=2.61 (95%CI=1.98-3.44) adjusted for the same confounders. Even though a high percentage of the students recognized emergency situations, a significant proportion did not know the MEAS/SAMU number and only a minority had sufficient basic life support skills to help with cardiac arrest. A significant proportion would not enroll in a first aid course. Biological first-year university students were more prone to enroll in a basic life support course.

RESUMO

OBJECTIVES: This study was designed to assess cardiopulmonary resuscitation quality and rescuer fatigue when rescuers perform one or two minutes of continuous chest compressions. METHODS: This prospective crossover study included 148 lay rescuers who were continuously trained in a cardiopulmonary resuscitation course. The subjects underwent a 120-min training program comprising continuous chest compressions. After the course, half of the volunteers performed one minute of continuous chest compressions, and the others performed two minutes, both on a manikin model. After 30 minutes, the volunteers who had previously performed one minute now performed two minutes on the same manikin and vice versa. RESULTS: A comparison of continuous chest compressions performed for one and two minutes, respectively, showed that there were significant differences in the average rate of compressions per minute (121 vs. 124), the percentage of compressions of appropriate depth (76% vs. 54%), the average depth (53 vs. 47 mm), and the number of compressions with no errors (62 vs. 47%). No parameters were significantly different when comparing participants who performed regular physical activity with those who did not and participants who had a normal body mass index with overweight/obese participants. CONCLUSION: The quality of continuous chest compressions by lay rescuers is superior when it is performed for one minute rather than for two minutes, independent of the body mass index or regular physical activity, even if they are continuously trained in cardiopulmonary resuscitation. It is beneficial to rotate rescuers every minute when performing continuous chest compressions to provide higher quality and to achieve greater success in assisting a victim of cardiac arrest. .
