Background: Portosystemic shunt (PSS), an alteration commonly found in toy dogs, is caused by an anastomosis between the systemic and portal circulation, interfering with the metabolism of several toxins. It can be of congenital or acquired origin and is classified as intra- or extrahepatic. Clinical signs include the gastrointestinal tract, nervous system, and urinary system according to the fraction of the shunt. It is diagnosed by several imaging tests and exploratory laparotomy. Therapy involves drug therapy and/or surgical correction of the anomalous vessels. Thus, the aim is to present an unusual case of extrahepatic cPSS originating from the left gastric vein and insertion into the azygos vein. Case: A 2-year-old female toy poodle, spayed, weighing 2.7 kg was treated with a history of recurrent cystitis and neurological signs such as focal seizures, ataxia, tremors, blindness, lethargy, head pressing, and compulsive gait. Complementary tests revealed normochromic microcytic anemia, neutrophilia-induced leukocytosis, monocytosis, and lymphopenia. Biochemical analysis revealed hypoproteinemia due to hypoglobulinemia, an increase in alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase, and gamma-glutamyl transferase, and a decrease in urea. In the urinalysis, ammonium biurate crystals were detected, and Doppler ultrasound revealed microhepathy and the presence of an anomalous gastrosplenic vein inserted into the azygos vein, a finding compatible with the congenital extrahepatic PSS. Abdominal tomography confirmed vascular deviation with a sinuous path originating from the left gastric and splenic veins, inserting into the azygos vein, measuring approximately 5.95 cm in length. Cranial tomography revealed changes consistent with hepatic encephalopathy. Drug therapy was performed with hydration, liver chow, lactulose, probiotics, metronidazole, S-adenosyl-L-methionine, and ursodeoxycholic acid, and after 15 days, surgery was performed to place a 3.5 mm ameroid constrictor ring for gradual occlusion of the anomalous vessel. The animal recovered well, and a control abdominal ultrasound was repeated 30 days after the procedure, noting that the constrictor had not yet fully occluded the deviation. Doppler imaging revealed a favorable evolution with an increase in the diameter of the portal vein in the hepatopetal direction. The patient was followed-up for a year and had a normal and healthy life. Discussion: Extrahepatic PSS is frequently diagnosed in purebred and toy dogs, commonly occurring between the portal vein and one of its tributaries, with a lower frequency of anomalous vessels between the azygos veins, as in the present report. The patient's age and clinical signs were compatible with the disease, in addition to ammonia biurate crystals and hematological and biochemical alterations. The neurological clinical signs observed were compatible with hepatic encephalopathy secondary to congenital PSS. The imaging examinations facilitated the identification of the extrahepatic vascular anomaly, with the tomography being more accurate and helping in proper surgical planning. Clinical treatment should be performed for presurgical stabilization, and occlusion can be performed by placing cellophane bands or an ameroid constrictor, which is the technique of choice for congenital PSS, as it allows for slow constriction to avoid acute portal hypertension, as in this case, emphasizing that anesthesia in animals with portosystemic shunts must be performed with care.

Less than 25% of patients with a thoracic vascular injury arrive alive to the hospital and more than 50% die within the first 24 hours. Thoracic trauma with the involvement of the great vessels is a surgical challenge due to the complex and restricted anatomy of these structures and its association with adjacent organ damage. This article aims to delineate the experience obtained in the surgical management of thoracic vascular injuries via the creation of a practical algorithm that includes basic principles of damage control surgery. We have been able to show that the early application of a resuscitative median sternotomy together with a zone 1 resuscitative endovascular balloon occlusion of the aorta (REBOA) in hemodynamically unstable patients with thoracic outlet vascular injuries improves survival by providing rapid stabilization of central aortic pressure and serving as a bridge to hemorrhage control. Damage control surgery principles should also be implemented when indicated, followed by definitive repair once the correction of the lethal diamond has been achieved. To this end, we have developed a six-step management algorithm that illustrates the surgical care of patients with thoracic outlet vascular injuries according to the American Association of the Surgery of Trauma (AAST) classification.

El trauma torácico penetrante con compromiso de los grandes vasos es un problema quirúrgico dado a su severidad y la asociación con lesiones a órganos adyacentes. El objetivo de este artículo es presentar la experiencia en el manejo quirúrgico de las lesiones del opérculo torácico con la creación de un algoritmo de manejo quirúrgico en seis pasos prácticos de seguir basados en la clasificación de la AAST. que incluye los principios básicos del control de daños. La esternotomía mediana de resucitación junto con la colocación de un balón de resucitación de oclusión aortica (Resuscitative Endovascular Balloon Occlusion of the Aorta - REBOA) en zona 1 permiten un control primario de la hemorragia y mejoran la sobrevida de los pacientes con trauma del opérculo torácico e inestabilidad hemodinámica.

RESUMO
This report describes two cases of multiple anomalies associated with the azygos venous system. In Case 1, the hemiazygos vein drained the 9th to 11th left posterior intercostal veins. At T9 vertebral level, the interazygos vein passed obliquely and anterior to the aorta to drain into the azygos vein on the right. In Case 2, the common venous trunk formed by the accessory hemiazygos and hemiazygos veins passed anterior to the aorta to drain into the azygos vein at T9 vertebral level. These findings represent pre-aortic inter-azygos veins, which is a rare variation of the azygos venous system. Pre-aortic inter-azygos veins can mimic pathologies such as enlarged lymph nodes, tumors and aneurysms leading to misinterpretation of radiographs, computerized tomography and magnetic resonance scans. Surgeons need to be aware of this type of variation so as to avoid injury of any anomalous passing venous vessels.
Este informe describe dos casos de múltiples anomalías asociadas con el sistema venoso ácigos. En el caso 1, la vena ácigos drena las 9 y 11 venas intercostales posteriores izquierdas. A nivel de la 9 vértebra torácica, la vena interácigos pasó oblicuamente y anterior a la aorta para drenar en la vena ácigos del lado derecho. En el caso 2, el tronco venoso común formado por las venas hemiácigos accesoria y hemiácigos pasaron anterior a la aorta para drenar en la vena ácigos a nivel de la 9 vértebra torácica. Estos resultados representan venas interácigos pre-aórticas, que es una rara variación del sistema venoso ácigos. Las venas interácigos pre-aórticas pueden imitar patologías, como agrandamiento de nódulos linfáticos, tumores y aneurismas que conducen a una mala interpretación de las exploraciones por radiografías, tomografía computarizada y resonancia magnética. Los cirujanos deben ser conscientes de estos tipos de variaciones para evitar lesionarlos.

ABSTRACT BACKGROUND AND OBJECTIVES: Inadvertent venous catheterizations occur in approximately 9% of lumbar epidural anesthetic procedures with catheter placement and, if not promptly recognized, can result in fatal consequences. The objective of this report is to describe a case of accidental catheterization of epidural venous plexus and its recording by computed tomography with contrast injection through the catheter. CASE REPORT: A female patient in her sixties, physical status II (ASA), underwent conventional cholecystectomy under balanced general anesthesia and an epidural with catheter for postoperative analgesia. During surgery, there was clinical suspicion of accidental catheterization of epidural venous plexus because of blood backflow through the catheter, confirmed by the administration of a test dose through the catheter. After the surgery, a CT scan was obtained after contrast injection through the catheter. Contrast was observed all the way from the skin to the azygos vein, passing through anterior and posterior epidural venous plexuses and intervertebral vein. CONCLUSION: It is possible to identify the actual placement of the epidural catheter, as well as to register an accidental catheterization of the epidural venous plexus, using computed tomography with contrast injection through the epidural catheter.
RESUMO JUSTIFICATIVA E OBJETIVOS: A cateterização venosa inadvertida ocorre em aproximadamente 9% das anestesias peridurais lombares com introdução de cateter e caso não seja prontamente reconhecida pode trazer consequências fatais. O objetivo deste relato é descrever um caso de cateterização acidental do plexo venoso peridural e o seu registro por tomografia computadorizada com injeção de contraste pelo cateter. RELATO DE CASO: Paciente feminina, sexagenária, estado físico II (ASA), submetida à colecistectomia convencional sob anestesia geral balanceada e peridural com cateter para analgesia pós-operatória. Durante cirurgia houve suspeição clínica de cateterização acidental do plexo venoso peridural, por refluxo de sangue pelo cateter, fato confirmado pela administração de dose-teste pelo cateter. Feita tomografia computadorizada com injeção de contraste pelo cateter, após o termino da cirurgia. Observado todo o trajeto do contraste desde a pele até a veia ázigo, passando pelo plexo venoso peridural anterior, posterior e veia intervertebral. CONCLUSÃO: É possível a identificação do real posicionamento do cateter peridural, bem como o registro da cateterização acidental do plexo venoso peridural, por meio de tomografia computadorizada com injeção de contraste pelo cateter peridural.

RESUMO
A avaliação do limiar de desfibrilação (DFT) durante o implante do cardioversor-desfibrilador (CDI) é uma etapa relevante do procedimento, uma vez que, em até 16% dos pacientes, podemos encontrar elevados DFT. Relatamos o caso de um paciente portador de cardiomiopatia dilatada (CMPD) idiopática submetido a implante de CDI biventricular. Durante o procedimento, apresentou elevado DFT e se mostrou resistente às modalidades terapêuticas usuais. Optamos pelo implante de eletrodo de desfibrilação em veia ázigos, com resolução do quadro.
The evaluation of the defibrillation threshold (DFT) during the implantation of a cardioverter-defibrillator (ICD) is an important stage of the procedure, as a high DFT can be found in up to 16% of patients. We report a patient with idiopathic dilated cardiomyopathy (DCM) submitted to a biventricular ICD implantation. During the procedure, the patient showed a high DFT and showed to be resistant to usual therapeutic modalities. We opted for the azygos vein defibrillation lead implantation, with good resolution.

Formed by the azygos, hemiazygos and accessory hemiazygos veins, this system is responsible for the venous drainage of the thorax and partially of the abdomen. Abnormalities of this system are not uncommon, and many are referred to in literature. The complete agenesis of the accessory hemyazigos vein is associated with complete regression of the left posterior cardinal vein or atrophy of the left subcardinal vein. The reported case consists of the complete absence of the accessory hemiazygos vein, culminating in the drainage of the 4th, 5th, 6th, 7th and 8th left posterior intercostal veins directly into the azygos vein of a male cadaver. The importance of the theme is due to the necessity of recognizing patterns and frequencies of these variations when facing procedures addressing the mediastinum or the major vessels.

La evaluación del umbral de desfibrilación (UDF) es una práctica habitual en la mayoría de los implantes de cardiodesfibriladores implantables (CDI). La práctica estándar busca obtener una desfibrilación exitosa de la fibrilación ventricular. En pacientes con umbrales de desfibrilación elevados en los cuales esto no resulta posible se da inicio a una serie de maniobras tendientes a corregir la situación. En esta presentación se describe un caso en el que se recurrió a una estrategia de uso poco frecuente para mejorar resultados durante la falla del UDF. Se trata de un paciente de 78 años, de sexo masculino, con antecedentes de miocardiopatía dilatada, que fue derivado al laboratorio de electrofisiología para el implante de un CDI para prevención primaria. Durante el procedimiento, el CDI falló en desfibrilar al paciente aun luego de haber posicionado el cable en diferentes áreas del ventrículo derecho y de haber optimizado la onda de choque. El paciente finalmente recibió el implante de un electrodo de desfibrilador en la vena ácigos, lo que permitió la reorientación del vector de desfibrilación hacia el eje posterior, con el resultado de una desfibrilación exitosa de la fibrilación ventricular por el CDI.
The evaluation of the defibrillation threshold (DFT) is common after the implantation of implantable cardioverter defibrillator (ICD) devices. The goal of this standard of care is to achieve successful defibrillation of ventricular fibrillation. In patients with elevated defibrillation thresholds, alternative techniques are required to correct the situation. We describe a case in which an uncommon strategy was used to improve DFT after failed defibrillation. A 78-year old man with a history of dilated cardiomyopathy was referred to the electrophysiology laboratory to have an ICD implanted as primary prevention strategy. During the procedure, the ICD failed to defibrillate the patient even after the lead was placed in different areas of the right ventricle and after optimizing the shock wave. A defibrillation lead was implanted in the azygos vein, and the shock vector was directed towards the posterior axis; a successful defibrillation was thus achieved.

The aim of the present study was to determine the most common origin of the azygos vein. Thirty cadavers male and female, white and non-white adult individuals of different ages fixed in 10 percent formaldehyde and dissected. All cadavers had an undisclosed clinical death and were donated to the Universidade Estadual de Ciências da Saúde de Alagoa s, Brazil. Eleven different formations were found. The right subcostal vein was was only observed in 13 cases (43.33 percent); the azygos vein was formed by the confluence of the right subcostal and right ascending lumbar vein in three cases (10 percent); by the right subcostal vein with a contribution from the inferior vena cava (IVC) in three cases (10 percent); by the right subcostal with contribution from IVC and right ascending lumbar vein in three cases (10 percent); by the right and left subcostal veins in two cases (6.66 percent); by the right and left subcostal veins and contribution from the IVC in one case (3.33 percent); by the right and left subcostal veins and left accessory renal vein in one case (3.33 percent); by the left renal vein in one case (3.33 percent); by the right subcostal and left gonadal veins with contribution from the IVC in one case (3.33 percent); by the right subcostal and left renal veins in one case (3.33 percent); and composed by the continuation of the 11th posterior intercostal vein in one case (3.33 percent). Based on the results, the right subcostal vein was the only structure with a significant presence in the formation of the azygos vein.
El objetivo del estudio fue verificar cual es la disposición más frecuente del origen de la vena ácigos. Fueron disecados 30 cadáveres de individuos adultos, de ambos sexos, de diferentes grupos étnicos, fijados en formaldehído al 10 por ciento, donados a la Universidade Estadual de Ciencias da Saúde de Alagoas. Se encontraron 11 formaciones diferentes. En 13 casos (43,33 por ciento) se observó sólo la vena subcostal derecha; en 3 casos (10 por ciento) la vena ácigos estaba formada por la confluencia de las venas subcostal derecha y lumbar ascendente derecha; en 3 casos (10 por ciento) formado por las venas subcostal derecha y una contribución de la vena cava inferior VCI; en 3 casos (10 por ciento) por las venas subcostal derecha y contribución de la VCI y lumbar ascendente derecha; 2 casos (6,66 por ciento) por las venas subcostales derecha e izquierda; en 1 caso (3,33 por ciento) por las venas subcostal derecha, izquierda y contribución de la VCI; en 1 caso (3,33 por ciento) por las venas subcostal derecha e izquierda y renal accesoria izquierda; en 1 caso (3,33 por ciento) por la vena renal izquierda; en1 caso (3,33 por ciento) por las venas subcostal derecha, gonadal izquierda y contribución de la VCI; en 1 caso (3,33 por ciento) por las venas subcostal derecha y renal izquierda y en 1 caso (3,33 por ciento) por la continuación de la 11 vena intercostal posterior. Con base en los resultados podemos concluir que la vena subcostal derecha fue la única estructura con presencia significativa en la formación de la vena ácigos.

A ausência congênita de veia cava inferior é achado incomum e ocasional durante exames complementares ou cirurgias do abdome. Em razão de a maioria dos pacientes sem outras malformações serem assintomáticos, há dificuldade em estimar a prevalência dessas anomalias. O objetivo deste trabalho foi relatar o caso de um paciente de 28 anos, sexo masculino, referido para radiografia de tórax devido a quadro de febre alta (39ºC) não-responsiva à medicação. Radiografia de tórax sugeriu dilatação de veia ázigos. Em inquérito dirigido para sintomas cardiovasculares, referiu dor torácica (durante atividade física e em repouso) e dispneia ocasionais durante o sono associada ao período em que intensificou a prática de exercícios. Realizada tomografia computadorizada sem contraste e, posteriormente, contrastada. As mesmas não esclareceram o caso, fazendo-se necessário angiotomografia, a qual evidenciou ausência de veia cava inferior torácica.
The congenital absence of the inferior vena cava is a rare and occasional finding at complementary exams or abdominal surgeries. As the majority of patients without other malformations are asymptomatic, it is hard to estimate the prevalence of these anomalies. Our goal was to report a case of a 28-year-old patient, male, referred to thorax X-ray due to high fever state (39ºC), nonresponder to medication. The X-ray suggested azygos vein dilatation. The interview about cardiovascular symptoms revealed thoracic pain during both physical exercises and at rest, and occasional dyspnea during sleep related to the period of intensification of physical exercises. Computadorized tomography with and without contrast did not solve the case, what explain the request of an angiotomography, that showed absence of thoracic inferior vena cava.

CONTEXTO: A complicação mais frequente após a desconexão ázigo-portal e esplenectomia em doentes com esquistossomose mansônica hepatoesplênica é a trombose da veia porta. OBJETIVOS:Avaliar a incidência, os fatores preditivos dessa complicação, assim como, a evolução clínica, laboratorial, endoscópica e ultrassonográfica desses pacientes. MÉTODOS: Foram analisados retrospectivamente os prontuários de 155 doentes esquistossomóticos submetidos a desconexão ázigo-portal e esplenectomia. RESULTADOS: Trombose de veia porta foi observada em 52,3 por cento dos pacientes, sendo 6,5 por cento de trombose total e 45,8 por cento de trombose parcial. Os pacientes que evoluíram com trombose de veia porta apresentaram mais frequentemente diarreia no pós-operatório. Febre foi evento habitual que ocorreu em 70 por cento dos casos, mais frequente, entretanto, nos doentes com trombose total da veia porta (100 por cento). Trombose de veia mesentérica superior ocorreu em quatro doentes (2,6 por cento), sendo mais frequente entre os com trombose total da veia porta. Não se encontrou diferença estatística quanto aos parâmetros clínicos, laboratoriais, endoscópicos e recidiva hemorrágica no pós-operatório tardio, quando comparados os pacientes com e sem trombose portal. CONCLUSÕES: A trombose de veia porta no pós-operatório da desconexão ázigo-portal e esplenectomia é evento frequente, sem nenhum fator preditivo para sua ocorrência; na maioria dos casos a trombose é parcial e apresenta evolução benigna, com baixa morbidade; trombose total da veia porta está mais frequentemente associada à trombose da veia mesentérica superior, com elevada morbidade; a trombose da veia porta, parcial ou total, não acarretou complicações no período pós-operatório tardio.
CONTEXT: Portal vein thrombosis is the most frequent complication after esophagogastric devascularization and splenectomy for hepatosplenic schistosomosis. OBJECTIVE: To evaluate portal vein thrombosis in 155 patients with schistosomal portal hypertension submitted to esophagogastric devascularization and splenectomy. METHODS: We retrospectively analyzed not only the incidence and predictive factors of this complication, but also clinical, laboratorial, endoscopic and Doppler sonography outcome of these patients. RESULTS: Postoperative portal thrombosis was observed in 52.3 percent of the patients (partial in 45.8 percent and total in 6.5 percent). Postoperative diarrhea was more frequent in patients with portal vein thrombosis. Fever was a frequent postoperative symptom (70 percent) but occurred in a higher percentage when total portal vein thrombosis was present (100 percent). Superior mesenteric vein thrombosis occurred in four patients (2.6 percent) and was associated with total thrombosis of the portal vein. There was no statistical difference between patients with and without portal vein thrombosis according to clinical and endoscopic parameters during late follow-up. It was not possible to identify any predictive factor for the occurrence of this complication. CONCLUSIONS: Portal vein thrombosis is an early and frequent event after esophagogastric devascularization and splenectomy, usually partial with benign outcome and low morbidity. Total portal vein thrombosis is more frequently associated with a high morbidity complication, the superior mesenteric vein thrombosis. Long-term survival was not influenced by either partial or total portal thrombosis.
