Background: The anconeus epitrochlearis muscle is an anatomical variant prevalent in amphibians but unusual in humans. In favorable cases, this muscle provides protection to the cubital nerve but can result in neuropathy due to compression of the cubital nerve. Case Description: We present two cases with different clinical manifestations but both did not respond to conservative treatment. We opted for a surgical decompression where the anconeus epitrochlearis muscle was found intraoperatively, and the muscle fibers were dissected. Conclusion: Considering the presence of the muscle variant is the key point to intraoperatively achieve a complete dissection of the muscle fibers of the anconeus epitrochlearis muscle variant and obtain the decompression of the cubital nerve with satisfactory postoperative results.
RESUMO
BACKGROUND: Persistent sciatic artery (PSA) is a congenital malformation due to incomplete involution during the embryonic period. Its etiology is unknown, with an estimated incidence of 0.02 - 0.04% in the whole population and a mean age of 60-65 years. Its presentation can be bilateral. It is asymptomatic in most cases and is usually detected accidentally; however, some symptoms may appear, such as claudication, sciatic neuralgia, and pain in the affected limb. It can also manifest as an aneurysmal dilatation or thrombosis that can generate distal embolism with ischemia. CASE REPORT: In this case study, a patient in her 90s with a suspected peripheral arterial disease, which required an angiotomography of the lower limb, showing a superficial femoral artery running to the middle and distal third of the thigh and a vascular structure running in the sciatic neurovascular bundle corresponding to a persistent sciatic artery presenting atherosclerotic changes and extensive occlusion, was presented. The patient's treatment was clinical with dual antiplatelet therapy and prophylactic anticoagulation. This was due to comorbidities and age. Moreover, the PSA occlusion was not critical and did not significantly impact the patient's quality of life. CONCLUSION: Most patients with this anatomical variant are asymptomatic all their lives, but some of them may present symptoms with serious consequences. It is important to suspect it by clinical presentation and perform diagnostic confirmation by angiotomography. Treatment has yielded excellent results with endovascular techniques. Certain patients only require conservative treatment with anticoagulants and antiplatelet agents.

Introducción: la presencia de un conducto colédoco doble es una variante anatómica infrecuente con menos de 200 casos publicados en la literatura hasta la fecha, siendo fundamental los estudios preoperatorios que se le deben realizar a los pacientes, a fin de tener la sospecha o el diagnóstico confirmado, no sólo de la variante anatómica, sino de la patología asociada, siendo estas la coledocolitiasis y las patologías malignas del árbol biliar las más frecuentes, aunque gran parte de los casos son hallazgos incidentales e intraoperatorios.Caso clínico : se presenta el caso de masculino de 81 años de edad, con antecedente de colecistectomía 20 años previos, con diagnóstico de síndrome ictérico obstructivo, y estudios de colangiorresonancia y CPRE que reportan coledocolitiasis. Se decide llevar a acto quirúrgico y se diagnóstica la presencia de conducto colédoco doble como hallazgo. Se realiza derivación biliodigestiva de tipo hepaticoyeyunoanastomosis más ligadura de conducto colédoco accesorio con evolución satisfactoria, con alta médica a los 5 días posteriores, y resultado de biopsia negativo para malignidad.Conclusión : el doble colédoco debe ser adecuadamente diagnosticado y estudiado, así no se confirme el diagnóstico preoperatorio los pacientes deben acudir con estos estudios realizados al acto quirúrgico, ya que, dicho tratamiento dependerá fundamentalmente de la patología asociada(AU)
Introduction: the presence of a double common bile duct is an infrequent anatomical variant with less than 200 cases published in the literature to the date, and preoperative studies that must be performed on patients are essential in order to have the suspicion or diagnosis confirmed, not only of the anatomical variant, but also of the associated pathology, these being choledocholithiasis and malignant pathologies of the biliary tract the most frequent, although most of the cases are incidental and intraoperative findings.Clinical case : the case of an 81-year-old male is presented, with a history of cholecystectomy 20 years prior, with a diagnosis of obstructive icteric syndrome, and magnetic resonance cholangiography and ERCP studies that reported choledocholithiasis. It was decided to carry out surgery and the presence of double common bile duct was diagnosed as a finding. Biliodigestive derivation hepaticojejunoanastomosis type and accessory common bile duct ligation was performed with satisfactory evolution, with medical discharge 5 days later, and biopsy result negative for malignancy.Conclusion : the double common bile duct should be properly diagnosed and studied, even if the preoperative diagnosis is not confirmed, patients should attend the surgical procedure with these studies, since said treatment will depend fundamentally on the associated pathology(AU)

The patellar tendon is an integral part of the knee extensor mechanism and has been historically described as a single tendon. A doubled patellar tendon is an exceedingly rare finding. We present a case of a crossed doubled patellar tendon in a 70-year-old male with a history of right knee pain, which to our knowledge has only been reported once before in the literature. The presence of a doubled patellar tendon has a potential influence on surgical planning and in the etiology of anterior knee pain.

Resumen Introducción: el cerebro es un órgano altamente irrigado, y esta irrigación es suministrada por el círculo arterial cerebral: una red arterial anastomótica con frecuentes variaciones anatómicas, algunas asociadas con patologías. El objetivo es describir las características antropométricas y variaciones anatómicas del círculo arterial cerebral en una muestra de especímenes cadavéricos humanos. Materiales y métodos: se revisaron 50 encéfalos, de los cuales 24 cumplieron con los criterios de inclusión. Se realizaron medidas de diámetro y longitud y también se describieron variaciones anatómicas y variaciones morfométricas. Resultados: siete especímenes evidenciaron variaciones anatómicas (29.1 %); 3 (12.5 %), arteria cerebral anterior (ACA) ácigos; uno (4.1 %), doble ACA en el segmento A1; uno (4.1 %), triple ACA en el segmento A2 y doble arteria comunicante anterior (AcomA); uno (4.1 %), agenesia de la arteria comunicante posterior (AComP) derecha; uno (4.1 %), de la AComP izquierda. El 29.1 % presentó variaciones antropométricas; el 12.5 %, hipoplasia de la AComP derecha; el 12.5 %>, de la AComP izquierda, y el 4.1 %>, hipoplasia bilateral de la AComP. Conclusión: la presencia de variaciones anatómicas fue inferior a la hallada en otros estudios, con predominio de variantes de la circulación anterior. Se describe la presencia de triple ACA en el segmento A2 y doble arteria comunicante anterior.
Abstract Introduction: The brain is a highly irrigated organ; this irrigation is supplied by the cerebral arterial circle: an anastomotic arterial network with frequent anatomical variations, some of which are associated with pathologies. This study aimed to describe the anthropometric characteristics and anatomical variations of the cerebral arterial circle in a sample of human cadaveric specimens. Materials and Methods: A total of 50 brains were examined, 24 of which met the inclusion criteria. Diameter and length measurements were obtained, and anatomical and morphometric variations were described. Results: Seven specimens (29.1%) had anatomical variations: 3 (12.5%) had azygos anterior cerebral artery (ACA), 1 (4.1%) had double ACA in segment A1, 1 (4.1%) had triple ACA in segment A2 and double anterior communicating artery (AcomA), 1 (4.1%) had right posterior communicating artery (AComP) agenesis, and 1 (4.1%) had left AComP agenesis. Seven specimens (29.1%) had anthropometric variations: 3 (12.5%) had hypoplasia of right AComP and 3 (12.5%) of left AComP and 1 (4.1%) had bilateral hypoplasia of AComP. Conclusion: The presence of ana-tomical variations was lower than that reported in other studies, with a predominance of variations of the anterior circulation. The presence of triple ACA in segment A2 and double AcomA was described.
Resumo Introdução: o cérebro é um órgão altamente irrigado; esta irrigação é fornecida pelo círculo arterial cerebral: uma rede arterial anastomótica com frequentes variações anatómicas, algumas associadas a patologias. O objetivo é escrever as características antropométricas e variações anatômicas do círculo arterial cerebral em uma amostra de espécimenes cadavéricos humanos. Materiais e métodos: se revisaram 50 encéfalos dos quais 24 cumpriram os critérios de inclusão; realizaram-se medidas de diâmetro e longitude, descreveram-se variações anatômicas e variações morfométricas. Resultados: o 29,1% dos espécimenes apresentaram variações anatómicas (n = 7 ), 3 (12,5%) apresentaram artéria cerebral anterior (ACA) ázigos, um (4,1%) apresentou dupla ACA no segmento A1, um (4,1%) apresentou triple ACA no segmento A2 e dupla artéria comunicante anterior (AcomA), um (4,1%) apresentou agenesia da artéria comunicante posterior (AComP) direita, um (4,1%) da AComP esquerda. 29,1% apresentaram variações antropométricas, três (12,5%) apresentaram hipoplasia de AComP direita, três (12,5%) da AComP esquerda e um (4,1%) hipoplasia bilateral da AComP. Conclusão: a presença de variações anatómicas foi inferior à reportada em outros estudos, com predomínio de variantes da circulação anterior, se descreve a presença de triple ACA no segmento A2 e dupla artéria comunicante anterior.

One of the most common causes of stroke is rupture of aneurysms whose approach requires knowledge of anatomical variants. The aim of this study was to determine the prevalence of anatomical variants of the anterior cerebral artery (ACA) and the anterior communicating artery (AComA) by 3D computed tomography angiography (3D CTA) in Mexican individuals. A retrospective, observational, cross-sectional descriptive study of 283 patients, independent of sex or age, in which morphometric measurements of cerebral vessels were evaluated using contrasted 3D CTA from a period of two years was performed. We found at least one "atypical" variant in a third of the study population (33.93 %). The most common "atypical" variant was the absence of the AComA (14.1 %). A significant association between the hypoplastic variant of the right A1 segment and hypoplasia of the left A1 and the right A2 was found, while hypoplasia of the left A1 was associated with hypoplasia of the right A2. There is a difference in the prevalence of anatomical variants of the ACA-AComA complex in Mexican population in relation to other populations. The typical variant is the most prevalent form in the study population. However, the presence of atypical variants represents an important number that should be taken into account in clinical and surgical procedures.
Una de las causas más frecuentes de accidente cerebrovascular es la ruptura de aneurismas cuyo abordaje requiere el conocimiento de las variantes anatómicas. El presente estudio tuvo como objetivo determinar la prevalencia de variantes anatómicas de la Arteria Cerebral Anterior (ACA) y la Arteria Comunicante Anterior (AComA) mediante angiotomografías computarizadas 3D (angioTAC 3D) de individuos mexicanos. Se realizó un estudio retrospectivo, observacional, transversal y descriptivo en el que se evaluaron angioTAC contrastados con reconstrucción 3D de 283 pacientes, sin considerar género ni edad, obtenidas durante un periodo de dos años a los que se les realizaron mediciones morfométricas en los vasos de interés. Se encontró al menos una variante "atípica" en un tercio de la población estudiada (33,93 %). La variante "atípica" más común fue la ausencia de AComA (14,1 %). Se encontró asociación significativa entre la variante hipoplásica del segmento A1 derecha y la hipoplasia de A1 izquierda y A2 derecha; mientras que la hipoplasia de A1 izquierda se asoció a la variante hipoplasia de A2 derecha, encontrándose mayor tendencia de aparición de aneurismas en función del menor diámetro del segmento A2 derecho de la ACA. Existe diferencia en la prevalencia de variantes anatómicas del complejo ACA-AComA en población mexicana con respecto a otras poblaciones. La variante típica constituye la forma más prevalente en la población estudiada. Sin embargo, la presencia de variantes atípicas representa una cifra importante que deberá tomarse en cuenta en procedimientos clínicos y quirúrgicos.

The persistent sciatic artery is a rare anatomical variant, representing the persistence of the sciatic artery in adult life that is responsible for the major blood supply to the lower limb in early embryologic development. Such persistence may be bilateral and can remain asymptomatic for many years. However, aneurysmal degeneration has been described as a complication of the persistent sciatic artery, which may cause critical limb ischemia resulting from thrombosis or embolization of aneurysm thrombus. Digital subtraction angiography, Doppler ultrasound, computed tomography angiography and magnetic resonance angiography are the most frequently used diagnostic tools to detect, classify and determine the presence of complications of a PSA. Early detection of this vascular abnormality on imaging studies can avoid life-threatening complications. We describe 4 patients with PSA that were diagnosed as an incidental finding in magnetic resonance imaging of the hip and demonstrate its characteristic imaging appearance.

Resumen:Este trabajo describe la presencia de la variante anatómica de la arteria axilar en un caso incidental de disección del Departamento de ciencias morfológicas de la Escuela de Medicina de la Universidad de Ciencias Médicas, dicha variante se presenta en menos de un 0.2% de la población. Así mismo se revisa el tema y su importancia clínica.
Abstract:This work describes the presence of anatomical variant of the axillary artery in an incidental case dissecting the Department of Morphological Sciences , School of Medicine, Universidad de Ciencias Médicas, said variant occurs in less than 0.2 % of the population. Also the subject and its clinical significance is reviewed.

Introducción: el drenaje venoso de los riñones se produce a partir de las venas renales que clásicamente se describen como troncos únicos que drenan en las paredes laterales de la vena cava inferior. Objetivo: determinar las características morfológicas de las venas renales. Materiales y métodos: se realizó un estudio cuantitativo, longitudinal, prospectivo, observacional y descriptivo, en 47 bloques, provenientes de cadáveres sin cirugías arteriovenosas aortorenales, enfermedad aórtica aneurismática, ni malformaciones congénitas renoureterales demostrables, en el Hospital Provincial Clínico Quirúrgico Docente Celia Sánchez Manduley, de Manzanillo, provincia Granma. Los bloques fueron lavados, fijados y disecados por el método macroscópico directo. Resultados: Las venas renales fueron únicas en el 61,70 % (18 bloques) de los bloques, siendo más constantes en el lado izquierdo (87,23 %). El recorrido fue prearterial en el 88,18 % de las venas, siendo este recorrido más frecuente en el lado izquierdo (66,03 %). La unión a la vena cava inferior fue en la cara lateral en el 95,46 % de las venas, siendo esta unión más constante en el lado izquierdo (98,11 %). Conclusiones: las venas renales presentan un patrón de variabilidad anatómica bajo. Las variantes a la norma anatómica fueron más frecuentes en el lado derecho, siendo su conocimiento importante en el planeamiento de la cirugía exerética y reconstructiva nefrourológica.

The variants of the anatomical norm were more frequent in the right side, being its knowledge important in the process of planning the exeretic and nephro-urologic reconstructive surgery.

Although the accessories muscles are part of our nature, to recognize them allows us to avoid confusion with other pathologies. Even some accessories wrist muscles may be symptomatic and require surgical intervention. In this paper we present the anatomy and imaging aspects of ultrasound and magnetic resonance imaging of these muscles, so that the radiologist can become familiar with them, avoiding unnecessary interventions or diagnostic errors.
Si bien los músculos accesorios son parte de nuestra naturaleza, el saber reconocerlos nos permite evitar confusiones con otras entidades patológicas. Incluso algunos músculos accesorios en la muñeca pueden ser sintomáticos y requerir intervención quirúrgica. En este trabajo exponemos la anatomía y aspectos imaginológicos en ultrasonido y resonancia magnética de dichos músculos, para que el radiólogo esté familiarizado con ellos, evitando errores diagnóstico o intervenciones innecesarias.

In the present case study, there is an anomalous pattern of superficial branch of radial nerve (SBRN) that can be recognized as an anatomical variant of the left upper limb of a 60 years old donated embalmed male cadaver in the Department of Human Anatomy, Universiti Putra Malaysia, in which the superficial branch of radial nerve supplies the extensor carpi radialis longus and extensor carpi radialis brevis muscle. The superficial branch of radial nerve gave off branch to supply the proximal fusion part of extensor carpi radialis longus and extensor carpi radialis brevis muscles immediately after bifurcation of the radial nerve into the superficial branch of radial nerve and deep branch of radial nerve. This variation is very important to the surgeon as the presence of the variant will make them more careful in dealing with nerve entrapment surgery, reconstructive surgery and in the pain management services, the variation giving indications for treatment.
En el presente estudio se observó un caso de un patrón anómalo del ramo superficial del nervio radial (RSNR) reconocido como una variante anatómica del miembro superior izquierdo en un cadáver de sexo masculino de 60 años perteneciente al Departamento de Anatomía Humana de la Universidad Putra, Malasia. En este caso el ramo superficial del nervio radial inervaba a los músculos extensores radial largo y corto del carpo. El ramo superficial del nervio radial otorgaba un ramo a nivel del origen fusionado de los músculos extensores radiales del carpo, inmediatamente después de la división en ramos superficial y profundo del nervio radial. Esta variación es de importancia para el cirujano debido a que requiere un mayor cuidado en el tratamiento quirúrgico de atrapamiento del nervio o cirugía reconstructiva para el manejo del dolor.
