RESUMO

This study has a quantitative cross-sectional design that aims to investigate the relationships between gender, age, status (migrant or Chilean-born), educational satisfaction, and overall life satisfaction among adolescent students in 7th and 8th grades of the Chilean educational system. The sample includes 406 students from four municipal public educational centers located in the Santiago district of the Metropolitan Region of Chile, with at least 20% migrant enrollment. The data were analyzed using quantitative methods using the R language, with descriptive analysis, cross-tabulation analysis, and independence tests. The packages used were: ggplot2, tidyverse and ggstatplot. The study found that age has an impact on the level of satisfaction with education and relationships formed with peers. Specifically, younger students expressed higher levels of satisfaction in these areas compared to older students. Moreover, foreign students showed a statistically significant difference in relation to age range, suggesting that as these students grow older, they tend to become less satisfied with the relationships they form with their classmates. In contrast, no statistically significant difference was found among Chilean students, indicating that the findings discussed here may not be generalizable to this specific population. The analysis also indicates a significant difference in the entire sample, suggesting a correlation between age range and level of satisfaction with schooling.

ABSTRACT Purpose: The regulatory effect of microRNA on diseases has been confirmed. This study aimed to evaluate the expression of microRNA-210-3p in age-related cataracts and assess the effect of abnormal miR-210-3p expressions on H2O2-induced SAR01/04 cells. Methods: Reverse-transcription quantitative polymerase chain reaction method was performed to assess the levels of miR-210-3p in aqueous humor samples. Receiver operating characteristic analysis was employed to assess the discrimination ability of miR-210-3p between patients with age-related cataracts and healthy people, and Pearson correlation analysis was used to identify the correlation between miR-210-3p and oxidative stress indices such as superoxide dismutase, glutathione peroxidase, malonaldehyde. Cell counting kit-8 assay and Transwell assay were used to estimate the biological function of H2O2-induced age-related cataract cell model. The levels of oxidative stress indices such as superoxide dismutase, glutathione peroxidase, and malonaldehyde were measured to evaluate the degree of oxidative stress damage in the age-related cataract cell model. The relationship between miR-210-3p and its target gene was verified by luciferase reporter gene analysis. Results: The miR-210-3p expression was elevated in the aqueous humor of patients with age-related cataracts. A high miR-210-3p expression showed a high diagnostic value for age-related cataracts and was significantly associated with the level of oxidative stress markers in patients with age-related cataracts. The inhibition of miR-210-3p can reverse oxidative stress stimulation and adverse effects on H2O2-induced cell function. Conclusions: The results suggested that miR-210-3p could promote cell viability, cell migration, and oxidative stress by targeting autophagy-related gene 7 in in vitro age-related cataract cell model.


RESUMO Objetivo: O efeito regulador do microRNA em doenças tem sido confirmado, e este artigo tentou avaliar a expressão do microRNA-210-3p na catarata relacionada à idade e avaliar o efeito da expressão anormal do miR-210-3p em células SAR01/04 induzidas por H2O2. Métodos: O método de transcrição reversa seguida de reação em cadeia da polimerase (RT-PCR) quantitativa foi realizado para avaliar os níveis de miR-210-3p em amostras de humor aquoso. Análise de características operacionais do receptor foi feita para avaliar a capacidade de discriminação do miR-210-3p entre pacientes com catarata relacionada à idade e pessoas saudáveis. A análise de correlação de Pearson identificou a correlação do miR-210-3p e índices de estresse oxidativo, como superóxido dismutase, glutationa peroxidase, malonaldeído. O ensaio de contagem de células kit-8 (cck-8) e o ensaio no sistema Transwell foram utilizados para estimar a função biológica do formato de células de catarata relacionada com a idade induzida por H2O2. Os níveis de índices de estresse oxidativo como superóxido dismutase, glutationa peroxidase e malonaldeído foram detectados para avaliar o grau de dano do estresse oxidativo em formato de células de catarata relacionada à idade. A relação entre miR-210-3p e seu gene alvo foi verificada por análise do gene repórter luciferase. Resultados: A expressão miR-210-3p foi elevada no humor aquoso de pacientes com catarata relacionada à idade. A expressão miR-210-3p altamente expressiva mostrou alto valor diagnóstico para catarata relacionada à idade e foi significativamente associado ao nível de marcadores de estresse oxidativo em pacientes com catarata relacionada à idade. A inibição de miR-210-3p pode reverter a estimulação do estresse oxidativo e os efeitos adversos da função celular induzida por H2O2. Conclusões: Esses dados sugeriram que a expressão miR-210-3p poderia promover a viabilidade celular, migração celular e estresse oxidativo ao direcionar genes ATG 7 relacionados à autofagia em modelo in vitro de células de catarata relacionadas à idade.

Objetivos: analisar o efeito da idade-período-coorte nas taxas de mortalidade por câncer do colo do útero no Centro-Oeste do Brasil. Métodos: estudo ecológico de séries temporais, de 1980 a 2019, cujas fontes de dados foram o Sistema de Informação sobre Mortalidade e as estimativas populacionais do Instituto Brasileiro de Geografia e Estatística. Modelos de efeito idade-período-corte com distribuição de Poisson foram usados. Resultados: observaram-se 19.260 óbitos, correspondente à taxa de mortalidade padronizada média de 17,23/100 mil mulheres. O efeito da idade indicou aumento progressivo das taxas com o avançar da idade. O efeito do período evidenciou redução do risco de morte no Distrito Federal (2015-2019), Mato Grosso do Sul (2010-2014) e Mato Grosso (2010-2019), além de aumento em Goiás (2015-2019). O efeito da coorte revelou aumento do risco de morte para as nascidas antes de 1950-1954 e redução nas gerações a partir de 1955-1959. Conclusão: há evidências de efeito da idade-período-coorte na mortalidade por câncer do colo do útero no Centro-Oeste brasileiro, o que demanda fortalecimento de ações para sua prevenção e controle voltadas para mulheres de coortes e idades sob maior risco de morrer por essa causa.


Objectives: to analyze the effect of age-period-cohort on cervical cancer mortality rates in the Center-West of Brazil. Methods: ecological time series study from 1980 to 2019, utilizing data from the Mortality Information System and population estimates from the Brazilian Institute of Geography and Statistics. Age-period-cohort effect models with Poisson distribution were employed. Results: 19,260 deaths were observed, corresponding to an average standardized mortality rate of 17.23/100,000 women. The age effect indicated a progressive increase in rates as age advanced. The period effect showed a reduction in the risk of death in the Federal District (2015-2019), Mato Grosso do Sul (2010-2014), and Mato Grosso (2010-2019), as well as an increase in Goiás (2015-2019). The cohort effect showed an increase in the risk of death for those born before 1950-1954 and a reduction in the generations from 1955-1959 onwards. Conclusion: there is evidence of an age-period-cohort effect on mortality from cervical cancer in the Brazilian Center-West, which calls for the strengthening of actions for its prevention and control aimed at women of cohorts and ages at greater risk of dying from this cause.

Métodos:estudio de series temporales ecológicas, de 1980 a 2019, cuyas fuentes de datos fueron el Sistema de Información de Mortalidad y las estimaciones de población del Instituto Brasileño de Geografía y Estadística. Se utilizaron modelos de efecto edad-período-corte con distribución de Poisson. Resultados: se registraron 19.260 fallecimientos, lo que corresponde a una tasa media de mortalidad estandarizada de 17,23/100.000 mujeres. El efecto edad indicó un aumento progresivo de las tasas con el avance de la edad. El efecto período mostró una reducción del riesgo de muerte en el Distrito Federal (2015-2019), Mato Grosso do Sul (2010-2014) y Mato Grosso (2010-2019), así como un aumento en Goiás (2015-2019). El efecto cohorte mostró un aumento del riesgo de muerte para los nacidos antes de 1950-1954 y una reducción en las generaciones a partir de 1955-1959. Conclusión: hay evidencias de un efecto edad-período-cohorte en la mortalidad por cáncer de cuello uterino en el Centro-Oeste brasileño, lo que exige el refuerzo de las acciones para su prevención y control dirigidas a las mujeres de cohortes y edades con mayor riesgo de morir por esta causa.

The aim of this study was to assess the erosive tooth wear (ETW) at early and mature adulthood in subjects with natural normal occlusion. The sample consisted of 23 untreated subjects with normal occlusion. Dental models were taken longitudinally from the same subjects at 13 (T0), 17 (T1) and 60.9 years of age (T2) at a university. Evaluation of ETW was performed using a modified Basic Erosive Wear Examination (BEWE) index. Interphase changes were evaluated using Friedman and Dunn's test. Ordinal logistic regression was used to assess the influence of sex, dental arch, tooth and dental surfaces on the erosive tooth wear. Linear regression was used to evaluate whether the ETW degree at T1 could discriminate the degree of ETW at T2. The significance level adopted was 5%. ETW showed a significant increase with aging. The median ETW index at T0, T1 and T2 was 2, 4 and 7, respectively. ETW was greater in males in the incisors and canines and on the incisal/occlusal and lingual tooth surfaces. No significant differences were found between the maxillary and mandibular arches. Subjects with severe ETW at mature adulthood had greater tooth wear at age 17. In conclusion, ETW significantly increased during aging in subjects with normal occlusion. The greater the degree of tooth wear at early adulthood, the greater the tooth wear at mature adulthood. Preventive care should be recommended during early adulthood in patients demonstrating erosive tooth wear in order to avoid worsening with aging.

Las mujeres posponen su maternidad por el deseo de superación personal y profesional. Se conoce que la cantidad y calidad de los óvulos por ciclo dependen de la edad de la paciente. Las tasas de éxito en tratamientos de reproducción asistida disminuyen con la edad, especialmente después de los 40 años. Se observan tasas más altas de nacidos vivos en mujeres más jóvenes y las tasas disminuyen significativamente en mujeres mayores debido a la disminución de la fertilidad y el aumento de abortos espontáneos. Por ello, la edad es crucial al evaluar la posibilidad de un embarazo exitoso mediante tratamientos de reproducción asistida (TRA). Las indicaciones para realizar fertilización in vitro (FIV) con óvulos propios en mujeres mayores de 40 años incluyen iniciar lo más pronto procedimientos de alta complejidad, buena evaluación de la reserva ovárica con análisis de la hormona antimülleriana y conteo de folículos antrales para realizar asesoramiento genético, proponer FIV-inyección intracitoplasmática de espermatozoides (ICSI) antes de los 44 años, generar expectativas realistas y realizar consentimiento informado, con estadisticas propias. En la REDLARA, de todos los procedimientos de FIV-ICSI, el 34% de las pacientes tienen más de 40 años; se prefiere transferir blastocistos con prueba genética preimplantacional de aneuploidías (PGT-A) para seleccionar embriones euploides. Las tasas de éxito son bajas, inclusive cuando son tasas de embarazo por transferencia de un embrión en el grupo de mujeres ≥ 40 años (18,2% sin PGT, 42,7% con PGT en el IMRCRP). Se recomienda acumular óvulos o embriones realizando múltiples estimulaciones ováricas. Se debe optar por transferir un solo embrión para evitar complicaciones obstétricas con embarazos múltiples en pacientes ≥ 40 años, por el alto riesgo debido a la edad.


Women postpone motherhood because of their desire for personal and professional improvement. It is known that the quantity and quality of oocytes per cycle depends on the patient's age. Success rates in assisted reproduction treatments decrease with age, especially after 40 years of age. Higher live birth rates are observed in younger women, and rates decrease significantly in older women due to decreased fertility and increased miscarriages. Therefore, age is crucial when assessing the possibility of a successful pregnancy through assisted reproductive treatments (ART). The indications to perform in vitro fertilization (IVF) with own ovules in women older than 40 years include starting as soon as possible highly complex procedures, good evaluation of ovarian reserve with antimüllerian hormone analysis (AMH) and antral follicle count (AFC) for genetic counseling, proposing IVF-intracytoplasmatic sperm injection (ICSI) before the age of 44 years, generating realistic expectations and informed consent, with own statistics. At REDLARA, of all IVF-ICSI procedures, 34% of patients are over 40 years old; preference is given to transfer blastocysts with preimplantation genetic testing for aneuploidy (PGT-A) to select euploid embryos. Success rates are low, even when they are pregnancy rates per embryo transfer in the group of women ≥ 40 years (18.2% without PGT, 42.7% with PGT in IMRCRP). It is recommended to bank ovules or embryos by performing multiple ovarian stimulations. A single embryo transfer should be chosen to avoid obstetric complications with multiple pregnancies in patients ≤ 40 years, because of the high risk due to age.

The burden of herpes zoster disease is significant worldwide, with millions affected and an increasing incidence. Increased age and immunosuppression due to disease or drugs have been related to its recurrence. The aim of this work was to determine the pharmacological management of herpes zoster and identify factors associated with recurrence, representing a longitudinal retrospective study identifying the pharmacological management of patients with herpes zoster and the factors related to the first recurrence using a population database. Follow-up was carried out for up to 2 years, and descriptive analysis and Cox proportional hazards regression were performed. A total of 2978 patients with herpes zoster were identified, with a median age of 58.9 years and 65.2% being women. The treatment mainly involved acyclovir (98.3%), acetaminophen (36.0%), and non-steroidal anti-inflammatory drugs (33.9%). A total of 2.3% of patients had a first recurrence. Corticosteroids were used in a greater proportion for recurrence than for the initial herpes episode (18.8% vs. 9.8%, respectively). Being female (HR:2.68;95%CI:1.39-5.17), age ≥60 years (HR:1.74;95%CI:1.02-2.96), having liver cirrhosis (HR:7.10;95%CI:1.69-29.80), and having hypothyroidism (HR:1.99;95%CI:1.16-3.40) were associated with greater probability of a first recurrence. The vast majority of patients were managed with acyclovir, and the use of acetaminophen or non-steroidal anti-inflammatory drugs for pain management was frequent. Several conditions were found that increased the probability of presenting a first recurrence of herpes zoster, such as age over 60 years, being a woman, suffering from hypothyroidism, and liver cirrhosis.

ABSTRACT Objective The World Health Organization and Centers for Disease Control and Prevention recommend the use of face masks in public. This study aimed to evaluate the effects of face masks on pulse rate and partial blood oxygen saturation in patients without cardiorespiratory disorders. Methods A total of 150 volunteers of both sexes were divided into three groups (n=50) according to age (children, young adults, and older adults). The partial blood oxygen saturation and pulse rate were measured for each volunteer using a digital oximeter while wearing a facial mask and remaining at rest. The masks were removed for two minutes, and partial blood oxygen saturation and pulse rate were remeasured. The materials and types of masks used were recorded. The t -test for paired samples was used to compare the mean values obtained before and after removing the masks. Results The most frequently used mask was a two-layered cloth (64.7%). A decrease in pulse rate was observed after removing the face mask in males, particularly in children (p=0.006) and young adults (p=0.034). Partial blood oxygen saturation levels increased in young adult males after mask removal (p=0.01). Conclusion The two-layer cotton tissue face masks are associated with a higher pulse rate and reduced arterial blood oxygen saturation without associated clinical disorders, mainly in adult men with a lower tolerance to breathing and ear discomfort.

Resumo Objetivo Avaliar a qualidade de vida relacionada à deglutição em idosos hospitalizados. Métodos Estudo transversal analítico e observacional. Participaram 52 idosos internados em clínica médica de um hospital público no Distrito Federal. Foram aplicados os instrumentos Eating Assessment Tool e o Quality of Life in Swallowing Disordens, além de coleta de dados sociodemográficos e condições de saúde. Resultados Dos idosos participantes 30,8% apresentaram risco de disfagia autorrelatada. Os idosos com risco de disfagia apresentaram menor pontuação no domínio "tempo de se alimentar" e maior pontuação no domínio de "saúde mental". O único domínio que não houve diferença estatística no padrão de resposta dos participantes que apresentaram ou não risco de disfagia foi o domínio sono. Entre as variáveis dos 11 domínios do Quality of Life in Swallowing Disordens foi possível observar correlações positivas em sua maioria com diferentes graus. Conclusão A qualidade de vida relacionada à deglutição de idosos hospitalizados está diretamente manifestada com a diminuição do convívio social, aumento do tempo para se alimentar, medo e fardo.


Resumen Objetivo Evaluar la calidad de vida relacionada con la deglución en adultos mayores hospitalizados. Métodos Estudio transversal analítico y observacional. Participaron 52 adultos mayores internados en clínica médica de un hospital público en el Distrito Federal. Se aplicaron los instrumentos Eating Assessment Tool y Quality of Life in Swallowing Disordens, además de la recopilación de datos sociodemográficos y condiciones de salud. Resultados De los adultos mayores que participaron, el 30,8 % presentó riesgo de disfagia autoinformada. Los adultos mayores con riesgo de disfagia presentaron un menor puntaje en el dominio "tiempo para alimentarse" y un puntaje más alto en el dominio "salud mental". El único dominio en el que no hubo una diferencia estadística en el tipo de respuesta de los participantes que presentaron o no riesgo de disfagia fue el del dominio sueño. Entre las variables de los 11 dominios del Quality of Life in Swallowing Disordens se observaron mayormente correlaciones positivas en diferentes niveles. Conclusión La calidad de vida relacionada con la deglución de adultos mayores hospitalizados está directamente manifestada con la reducción de la convivencia social, el aumento de tiempo para alimentarse, el miedo y la carga.


Abstract Objective To evaluate swallowing-related quality of life in hospitalized elderly patients. Methods Cross-sectional analytical and observational approach was used in our study. Fifty-two elderly patients in a medical clinic of a public hospital in Distrito Federal [Federal District] participated. In addition to collecting sociodemographic and health condition data, the Eating Assessment Tool and the swallowing-related quality of life Questionnaire were applied. Results Of the elderly participants, 30.8% had a self-reported risk of dysphagia. The elderly at risk for dysphagia had lower scores in the "eating duration" domain and higher scores in the "mental health" domain. The only domain in which there was no statistical difference in the response pattern of the participants who were, or were not at risk for dysphagia was the "sleep domain." Among the variables of the 11 domains of the Swallowing Quality of Life Questionnaire, mostly positive correlations were found, with different degrees. Conclusion The swallowing-related quality of life of hospitalized elderly patients is directly manifested as decreased social interaction, increased eating time, fear to eat, and swallowing as a burden.

BACKGROUND: Guidelines on managing arterial blood pressure have begun to include lower targets, despite the risk for overmedication-induced hypotension. Because there is no consensus as to what constitutes overmedication in the treatment of hypertension, we conducted a study to develop relevant diagnostic criteria. METHODS: We used ambulatory monitoring during a 24-hour period to measure blood pressure in 567 patients with medication-controlled hypertension. We then divided this cohort into 3 groups. Group A (n = 413 [72.8%]) comprised patients in whom less than 10% of systolic blood pressure measurements were less than 100 mm Hg (no or mild hypotension). Group B (n = 154 [27.2%]) comprised patients in whom 10% or more of the measurements were less than 100 mm Hg, signifying overmedication. Group C comprised those patients in Group B in whom 20% or more of the measurements were less than 100 mm Hg (n = 74 [48.1%]; attributed to "accentuated" overmedication). Group C patients were prescribed significantly more antihypertensive drugs than were group A patients. RESULTS: Our findings support a 2-part definition of overmedication in hypertension treatment: (1) mild overmedication, in which 10% or more but less than 20% of systolic blood pressure measurements are less than 100 mm Hg in the awake state and less than 90 mm Hg during sleep; and (2) accentuated overmedication, characterized by 20% or more of valid systolic blood pressure measurements less than 100 mm Hg in the awake state and less than 90 mm Hg during sleep, during a 24-hour period. CONCLUSION: We think this guidance will help to avoid overmedicating and consequences such as fainting and falls when systolic blood pressure is too low.

The objective was to know the behavior of fractures in Mexican children and adolescents. According to our study, fractures in Mexican male children and adolescents seem to be decreasing; however, we still need more national studies to know the possible causes of these fractures. PURPOSE: To describe the trends of fractures in Mexican children and adolescents across a 12-year period (2007 to 2019), and to analyze if these trends have changed over time between sexes and age groups. METHODS: We identified all fracture cases registered in children and adolescents (0 to 19 years) at the emergency rooms and surgical departments of the Mexican Institute of Social Security between January 2007 and December 2019. We used ICD-10 to classify the fractures. The population was divided into two age groups: children (0 to 9 years) and adolescents (10 to 19 years). Additional information regarding sex and age was gathered up as well. We calculated annual incidence; incidence rates are presented per 10,000 population at risk. Changes in fracture trends were calculated using the average annual percentage change (AAPC). RESULTS: Over 12 years, 1,400,443 fractures were registered. The most frequent site of fracture was forearm in 37.1% followed by shoulder (18.1%). The overall rates of fractures have remained similar over 12 years (86.5, IQR 81.0-94.2); however, a significant decrease in fractures was observed the last 3 years (2017-2019). According to the AAPC, only in men, in both age groups, a significant decrease in fractures was observed. CONCLUSION: This is the first study in Mexico to follow the behavior of fractures in the pediatric population over 12 years. Fractures seem to be decreasing in children and adolescents. An epidemiological follow-up of childhood fractures is necessary to understand the causes of fractures to generate better prevention and treatment strategies.

ABSTRACT Objectives: to investigate the impact of age, motor dysfunction and neuropsychiatric symptoms on the quality of life of people with multiple sclerosis in comparison to healthy peers. Methods: a total of 141 participants were tested in a single session. The assessments were composed by general questionnaires applied in both groups and by specific instruments restricted to multiple sclerosis. Multiple regression models were applied to assess relationships between predictors and outcome. Results: age, motor dysfunction and neuropsychiatric symptoms explained 56.6% of quality of life of the multiple sclerosis group. Age and neuropsychiatric symptoms explained 36.6% of quality of life in the control group. Age impacted more the multiple sclerosis group than the control group. Neuropsychiatric symptoms affected both groups similarly. Motor dysfunction impacted 21.9% of the quality of life in multiple sclerosis. Conclusions: the predictors explained considerable variance of quality of life in multiple sclerosis, which should guide public health policies.


RESUMO Objetivos: investigar o impacto da idade, da disfunção motora e dos sintomas neuropsiquiátricos sobre a qualidade de vida de pessoas com esclerose múltipla na comparação com controles saudáveis. Métodos: 141 participantes foram testados em uma única sessão. As avaliações foram compostas por questionários gerais aplicados em ambos os grupos e por instrumentos específicos à esclerose múltipla. Modelos de regressão múltipla foram usados para avaliar relações entre preditores e desfecho. Resultados: idade, disfunção motora e sintomas neuropsiquiátricos explicaram 56,6% da qualidade de vida do grupo esclerose múltipla. Idade e sintomas neuropsiquiátricos corresponderam a 36,6% da qualidade de vida do grupo-controle. Idade impactou mais o grupo esclerose múltipla do que o grupo-controle. Sintomas neuropsiquiátricos afetaram os grupos semelhantemente. A disfunção motora impactou 21,9% da qualidade de vida na esclerose múltipla. Conclusões: os preditores explicaram considerável variação da qualidade de vida na esclerose múltipla, o que deve nortear políticas públicas de saúde.


RESUMEN Objetivos: investigar el impacto de edad, disfunción motora y síntomas neuropsiquiátricos en la calidad de vida de personas con esclerosis múltiple en comparación con controles saludables. Métodos: 141 participantes fueron evaluados en una sesión. Las evaluaciones fueron compuestas por cuestionarios generales aplicados en ambos grupos y por instrumentos específicos a la esclerosis múltiple. Modelos de regresión múltiple fueron utilizados para evaluar relaciones entre predictores y resultado. Resultados: edad, disfunción motora y síntomas neuropsiquiátricos explicaron 56,6% de la calidad de vida en la esclerosis múltiple. Edad y síntomas neuropsiquiátricos explicaron 36,6% de la calidad de vida del grupo control. Edad afectó más la esclerosis múltiple que al grupo de control. Síntomas neuropsiquiátricos afectaron los grupos similarmente. La disfunción motora impactó 21,9% de la calidad de vida en la esclerosis múltiple. Conclusiones: los predictores explicaron una considerable variación de la calidad de vida en la esclerosis múltiple, lo que debe guiar políticas de salud pública.

ABSTRACT Objective: To identify the factors associated with the use of the N95 respirator in Nursing and Medical students in the daily life of the covid-19 pandemic. Methods: Descriptive-analytical cross-sectional study carried out in 2020. A total of 830 students from three universities in Peru participated in the study. Associations were evaluated using Pearson's Chi-Square and multivariate Poisson modeling with log linkage. Results: Statistically significant differences were found between the preference to use the N95 respirator in relation to masks according to the activity they perform (p=0.001) and where they live (p=0.005). The multivariate analysis reported that the associated characteristics were age, activity performed, perception and fear of being infected by covid-19. Conclusion: The choice of N95 respirator is influenced by individual factors and perceptions. Spaces are needed to discuss daily life, the way of living, caring and educating, considering the socioeconomic dimensions and beliefs.


RESUMO Objetivo: Identificar os fatores associados ao uso do respirador N95 em estudantes de Enfermagem e Medicina no cotidiano da pandemia de COVID-19. Métodos: Estudo descritivo-analítico transversal, realizado em 2020. Participaram 830 estudantes de três universidades do Peru. As associações foram avaliadas por meio do Qui-Quadrado de Pearson e modelagem multivariada de Poisson com link de log. Resultados: Foram encontradas diferenças estatisticamente significativas entre a preferência de uso do respirador N95 em relação às máscaras, de acordo com a atividade exercida (p=0,001) e o local de residência (p=0,005). A análise multivariada relatou que as características associadas foram a idade, a atividade que desempenha, a percepção e o medo de ser infectado pela COVID-19. Conclusão: Na preferência de escolha do respirador N95, intervêm fatores e percepções individuais. São necessários espaços para discutir o cotidiano, o modo de viver, cuidar e educar, considerando as dimensões socioeconômicas e as crenças.

El análisis multivariado reportó que las características asociadas fueron la edad, la actividad que desempeña, percepción y miedo de que podrían infectarse con COVID-19. Conclusión: En la preferencia de elegir el respirador N95, intervienen factores individuales y las percepciones. Se necesitan espacios para discutir lo cotidiano, la manera de vivir, cuidar y educar; considerando dimensiones socioeconómicas y creencias.

SUMMARY OBJECTIVE: Individuals aged ≥65 years are more susceptible to COVID-19 disease and admission to intensive care is most notable. The scoring systems (national early warning score, quick sequential organ failure assessment, shock index) are recommended for rapid assessment of patients in emergency room conditions. The goal of our study is to evaluate scoring systems in conjunction with predictive factors of need for admission to intensive care of patients ≥65 years old with a diagnosis of COVID-19 who applied to the emergency room. METHODS: Patients were divided into two groups according to evolution in the emergency room, being those who needed or not intensive care. National Early Warning Score, quick sequential organ failure assessment, shock index scores and serum biochemistry, blood count and blood gas values were evaluated from hospital information management system records. RESULTS: Of the patients included in the study, 80.8% were admitted to the ward and 14.5% to the unit of intensive care. Lymphocyte count, base deficit and bicarbonate levels were lower, and the levels of C-reactive protein, lactate, D-dimer, urea and lactate dehydrogenase were higher in patients who needed intensive care. Quick sequential organ failure assessment and shock index were considered significant in the group admitted to the intensive care unit. CONCLUSIONS: We recommend that quick sequential organ failure assessment and shock index be used quickly, practically and easily in predicting the need for intensive care unit in patients aged ≥65 years in emergency department diagnosed with COVID-19.

BACKGROUND: Polypharmacy has become an increasingly public health issue as population age and novel drugs are developed. Yet, evidence on low- and middle-income countries (LMIC) is still scarce. OBJECTIVE: This work aims to estimate the prevalence of polypharmacy among Brazilians aged 50 and over, and investigate associated factors. METHODS: A cross-sectional study was conducted using data from the baseline assessment of the Brazilian Longitudinal Study of Aging (ELSI-Brazil), a nationally representative study of persons aged 50 years and older (n=9,412). Univariate and bivariate analyses described the sample. Robust Poisson regression was used to estimate prevalence ratios and predict probabilities of polypharmacy. RESULTS: Prevalence of polypharmacy was estimated at 13.5% among older adults in Brazil. Important disparities were observed in regard to gender (16.1% among women and 10.5% among men), race (16.0% among whites and 10.1% among blacks) and geographic region (ranging from 5.1% in the North to 18.7% in the South). The multivariate analysis showed that polypharmacy is associated with various sociodemographic/individual factors (age, gender, race, education, region, health status, body mass index) as well as with several variables of healthcare access/utilization (number of visits, same physician, provider's knowledge of patient's medications, gate-keeper, and difficulty managing own medication). Overall, the more utilization of health services, the higher the probability of polypharmacy, after adjusting for all other model covariates. CONCLUSIONS: Polypharmacy prevalence is relatively low in Brazil, compared to European countries. After controlling for variables of healthcare need and demographic characteristics, there is still substantial residual variance in polypharmacy prevalence. Policies to identify inappropriate prescribing and reduce regional discrepancies are necessary.

Abstract Objective To assess craniofacial changes from early adulthood to the seventh decade of life in individuals with normal occlusion. Methodology The sample comprised lateral cephalograms of 21 subjects with normal occlusion (11 male, 10 female), taken at 17 (T1) and 61 years of age (T2). Anteroposterior and vertical maxillomandibular relationships, and dentoalveolar and soft tissue changes were analyzed. Interphase comparisons were performed using paired t-tests. Differences between sexes, and subgroups with and without tooth loss were evaluated using t-tests (p<0.05). Results Maxillary and mandibular anterior displacement, and facial and ramus height increased from T1 to T2. Maxillary molars showed significant mesial angulation. Maxillary and mandibular molars, and mandibular incisors developed vertically during the evaluation period. Soft tissue changes included a decrease of the nasolabial angle, upper and lower lip retrusion, decrease of upper lip thickness and increase of the lower lip and soft chin thickness. Maxillary incisor exposure by the upper lip decreased 3.6 mm in 40 years. Males presented counterclockwise rotation of the mandible, whereas females showed mandibular clockwise rotation and backward displacement of the chin. The group with tooth loss showed a greater increase of the posterior facial height and ramus height. Conclusion We observed aging changes in dentoskeletal structures and soft tissue, as well as sexual differences for craniofacial changes during the maturational process. Subjects with multiple tooth losses showed a greater increase in mandibular ramus height.

Objetivo: Avaliar a incidência e a evolução da Covid-19 em indivíduos com diagnóstico prévio de doença renal crônica estágio 5D e também comparar diabéticos e não diabéticos. Métodos: Foi analisada a mortalidade por Covid-19 em pacientes com doença renal crônica estágio 5D sem diabetes e no subgrupo de pacientes diabéticos. A comparação da idade média entre diabéticos e não diabéticos e pacientes que morreram e sobreviventes foi realizada por meio do teste t de Student para amostras independentes. A proporção de óbitos entre diabéticos e não diabéticos foi calculada pelo teste do qui-quadrado de Pearson. Resultados: A pro- porção de óbitos entre pacientes diabéticos e não diabéticos com doença renal crônica estágio 5D foi significativamente diferente (p=0,0245; razão de chance de 5,03; IC95% 1,26-21,84). A idade média foi de 55,0±15,6, e 15 pacientes (35,7%) eram diabéticos e significativamente mais velhos que os não diabéticos (p=0,0233). Não foi observada diferença na proporção de diabéticos entre homens e mulheres (p=0,5758) e nem na idade média entre diabéticos e não diabéticos (p=0,1829). Conclusão: A mortalidade por Covid-19 em pacientes com doença renal crônica estágio 5D é significativamente maior do que na população em geral, e este estudo mostrou diabetes mellitus e idade como fatores de risco adicionais para mortalidade nesta população.


Objective: To evaluate the incidence and progression of Covid-19 in individuals previously diagnosed with stage 5 chronic kidney disease and compare diabetic and nondiabetic indi- viduals. Methods: Covid-19 mortality in nondiabetic 5 chronic kidney disease patients and in the subgroup of diabetic patients was analyzed. The average age comparison between diabetic and nondiabetic patients and those who died and survived was performed using Student t test for independent samples. The proportion of deaths between diabetics and nondiabetics was calculated using Pearson's Chi-squared test. Results: The proportion of deaths between diabetic and nondiabetic 5 chronic kid- ney disease patients was significantly different (p=0.0245), odds ratio of 5.03 (95%CI= 1.26-21.84). The average age was 55.0±15.6, and 15 patients (35.7%) were diabetic and significantly older than nondiabetic (p=0.0233). No difference was observed in the proportion of diabetics between men and women (p=0.5758) or in the average age between diabetics and nondiabetics (p=0.1829). Conclusion: Covid-19 mortality in 5 chronic kidney disease patients is significantly higher than in general popula- tion and this study has shown diabetes mellitus and age as ad- ditional risk factors for mortality in this population.

OBJECTIVE: this study aims to evaluate the relationship between body mass index (BMI), age at which knee joint arthroplasty is performed and complications. The hypothesis is that the higher the BMI, the greater likelihood that the patient will require surgery at an earlier age.Methods: this is a cohort study with all patients who underwent a primary knee arthroplasty, between August 2013 and February 2019, in a tertiary level university hospital. Association between BMI, age and complications were analyzed. Quality of life of patients was also evaluated with the Oxford Knee Score (OKS). RESULTS: 565 primary total knee replacements (TKR) were performed. A cut-off point was found in BMI of 30; 348 patients had a BMI ≤30 and 173 patients had a BMI >30. When comparing the two groups, a statistically significant difference (p = 0.0186) was found in the age at which the TKR was performed. There was a significant improvement for both groups in functional score (Oxford knee score). Additionally, intra and post-operative complications showed no statistically significant difference. CONCLUSION: patients with BMI greater than 30 required primary knee arthroplasty at a younger age (average: 3.5 years), compared to patients with a lower BMI. Obesity does not appear to confer and independent risk for surgery in the short and mid-term. Knee arthroplasty improves significantly quality of life in the short and mid-term, regardless of their BMI, as measured with the OKS.

Las enfermedades cardiovasculares se han constituido en la primera causa de muerte a nivel mundial. La investigación que se presenta tuvo el fin de determinar el riesgo cardiovascular en las mujeres de edades comprendidas entre 30 y 49 años, que acuden a recibir atención primaria en el Centro de Salud N°1 Ibarra en el 2018; para lo que, se aplicó un diseño cuantitativo de tipo no experimental descriptivo de corte transversal y documental. Se trabajó con la totalidad de la población en estudio, conformada por 372 individuos de sexo femenino. Los factores de riesgo con mayor incidencia entre las participantes fueron el colesterol total (45,3%) y el índice de masa corporal con 37,6%. Entre la población estudiada, las investigadoras observaron afectaciones en la totalidad de los factores de riesgo considerados en el score de Framingham. Al analizar, el nivel de riesgo cardiovascular, se apreció que la mayoría de la población investigada se ubicó en las categorías sin riesgo y riesgo bajo. Aunque entre aquellos integrantes con más de un factor predisponente el rango de diferencia entre las edades biológica y cardiovascular osciló entre 11,47 y 43 años.


Cardiovascular diseases have become the leading cause of death worldwide. This research aimed to determine the cardiovascular risk in women aged between 30 and 49, who attend primary care at Ibarra Health Center No. 1 in 2018. A quantitative design of descriptive non-experimental cross-sectional and documentary type was applied. The entire population of 372 female individuals was considered. The risk factors with the highest incidence among participants were total cholesterol (45.3%) and body mass index with 37.6%. Among the population studied, the researchers observed effects on all the risk factors considered in the Framingham score. The level of cardiovascular risk was analyzed in order to show that most of the population was placed in the categories without risk and low risk. However, among those members with more than one predisposing factor the range of difference between the biological and cardiovascular ages ranged between 11.47 and 43 years of age.

ABSTRACT BACKGROUND: It is widely assumed that gender, age, gastritis and Helicobacter pylori , all have some degree of correlation and, therefore, can synergistically lead to the development of gastric cancer. OBJECTIVE: In this cross-sectional study, we expected to observe the above mentioned correlation in the analysis of medical records of 67 patients of both sexes (female, n=44), mean age ± standard deviation: 41±12 years old, all from Belém (capital of Pará State, Brazilian Amazon), a city historically known as one with the highest gastric cancer prevalence in this country. METHODS: All patients were submitted to upper gastrointestinal endoscopy for gastric biopsy histopathological analysis and rapid urease test. All diagnoses of gastritis were recorded considering its topography, category and the degree of inflammatory activity, being associated or not associated with H. pylori infection. RESULTS: The results show that no statistically relevant associations were found among the prevalences of the observed variables. CONCLUSION: The authors hypothesize that observed risk factors associated to gastric cancer might be lesser synergistic than is usually expected.


RESUMO CONTEXTO: É amplamente assumido que gênero, idade, gastrite e Helicobacter pylori , todos têm algum grau de correlação e, portanto, podem sinergicamente levar ao desenvolvimento de câncer gástrico. OBJETIVO: Neste estudo transversal, esperamos observar a correlação acima mencionada na análise de prontuários de 67 pacientes de ambos os sexos (sexo feminino, n=44), média de idade ± desvio padrão: 41±12 anos, todos de Belém (capital do Estado do Pará, Amazônia Brasileira), uma cidade historicamente conhecida como sendo uma das que apresenta maior prevalência de câncer gástrico no país. MÉTODOS: Todos os pacientes foram submetidos à endoscopia digestiva alta para análise histopatológica da biópsia gástrica e teste rápido da urease. Todos os diagnósticos de gastrite foram registrados considerando sua topografia, categoria e grau de atividade inflamatória, sendo associada ou não associada à infecção por H. pylori . RESULTADOS: Os resultados mostram que não foram encontradas associações estatisticamente relevantes entre as prevalências das variáveis observadas. CONCLUSÃO: Os autores levantam a hipótese de que os fatores de risco associados ao câncer gástrico podem ser menos sinérgicos do que o esperado.
