Introducción: El yoga es una actividad que trata de un ejercicio el cual contiene una intensidad baja a moderada, la cual no se centra exclusivamente en el entrenamiento físico, sino que también en el desarrollo de la mente y el espíritu de uno mismo. El yoga puede obtener un mayor impacto en el equilibrio y en la ganancia de fuerza de la parte superior del cuerpo, además, demuestra mejora en la aptitud cardiorrespiratoria y flexibilidad. Metodología: El enfoque de desarrollo fue de tipo cuantitativo en donde se realizó una revisión sistemática como metodología de búsqueda de información, relacionada al yoga como una terapia complementaria y los beneficios que éste aportaba al bienestar de las personas mayores. Resultados: En cada uno de los ensayos controlados aleatorizados que fueron recabados para fines de esta revisión sistemática. Se destaca la importancia y los beneficios del yoga en la movilidad, fuerza, flexibilidad y espiritualidad de los usuarios que practican esta terapia. Discusión: Los artículos analizados pertenecen a ensayos clínicos o estudios aleatorizados, los cuales permitieron responder de manera efectiva a nuestra pregunta de investigación, la cual consiste en reconocer si el yoga es efectivo para disminuir el riesgo de dependencia funcional y eliminar hábitos que no son saludables para las personas mayores, además de mejorar la calidad de vida actual. Gracias a ello se pudo evidenciar que esta terapia en adultos mayores genera cambios positivos respecto a estado y condición física, la ejecución de esta práctica mejora la calidad de vida en un 80% Conclusión: La yoga como terapia complementaria si entrega beneficios en la calidad de vida de la población adulta mayor, dado que, que hubo una mejora tanto en la movilidad, calidad de vida y autovalencia de los adultos mayores[AU]


Introduction: Yoga is an activity that deals with a low to moderate intensity exercise, which is not exclusively focused on physical training, but also on the development of the mind and spirit itself. Yoga may have a greater impact on balance and upper body strength gains, and have shown improvements in cardiorespiratory fitness and flexibility. Methodology:the development approach was of a quantitative type where a systematic review was carried out as a methodology for searching for information related to yoga as a complementary therapy and the benefits that it brought to the well-being of the elderly. Results:The importance and benefits of yoga on the mobility, strength, flexibility and spirituality of users who practice this therapy are highlighted in each of the randomized controlled trials that were collected for the purposes of this systematic review. Discussion: The articles analyzed belong to clinical trials or randomized studies, which allowed us to effectively answer our research question. The activity of yoga in older adults generates positive changes regarding state and physical condition, the execution of this practice improves the quality of life by 80% Conclusion: Yoga as a complementary therapy delivers benefits in the quality of life of the adult population elderly, it can be said that there was an improvement in mobility, quality of life and self-valence of the elderly[AU]


Introdução: O Yoga é uma atividade que trata de um exercício de intensidade baixa a moderada, que não se foca exclusivamente no treino físico, mas também no desenvolvimento da mente e do espí-rito. A ioga pode ter um impacto maior no equilíbrio e nos ganhos de força da parte superior do corpo e mostrou melhorias na apti-dão cardiorrespiratória e flexibilidade. Metodologia: a abordagem de desenvolvimento foi do tipo quantitativo onde foi realizada uma revisão sistemática como metodologia de busca de informações re-lacionadas ao yoga como terapia complementar e os benefícios que trouxe para o bem-estar dos idosos. Resultados: A importância e os benefícios do yoga na mobilidade, força, flexibilidade e espiri-tualidade dos usuários que praticam esta terapia são destacados em cada um dos ensaios clínicos randomizados que foram coletados para fins desta revisão sistemática. Discussão: Os artigos analisa-dos pertencem a ensaios clínicos ou estudos randomizados, o que nos permitiu responder de forma eficaz à nossa questão de inves-tigação. A atividade de yoga em idosos gera mudanças positivas quanto ao estado e condição física, a execução desta prática mel-hora a qualidade de vida em 80% Conclusão: o yoga como terapia complementar traz benefícios na qualidade de vida da população adulta idosa, pode-se dizer que houve melhora na mobilidade, qualidade de vida e autovalência dos idosos[AU]

Introducción: El desarrollo positivo es un estadio de ajuste psicosocial que acentúa las potencialidades y los recursos entre jóvenes y sus contextos próximos. En Australia, se desarrolló el primer modelo de desarrollo positivo para adultos emergentes, el cual consta de cinco componentes (competencia social, satisfacción con la vida, confianza y tolerancia con los otros, confianza hacia autoridades e instituciones, acción y compromiso cívico), pero no ha sido investigado en Chile. Objetivo: Analizar la evidencia disponible en la literatura científica sobre los cinco componentes del modelo de desarrollo positivo en adultos emergentes universitarios de Chile. Materiales y métodos: Revisión narrativa de la literatura publicada entre 2013 y 2023, en bases de datos Web of Science, Scopus, SciELO, Redalyc y Dialnet. Resultados: Se obtuvieron 83 artículos, de los cuales 53 cumplieron los requisitos de inclusión; en su mayor parte fueron estudios cuantitativos (81,13%) y en español (75,47%). Se evidenció una disparidad en la cantidad de estudios por temática; se encuentra mayor investigación en satisfacción con la vida y acción y compromiso cívico. Conclusiones: Existen brechas en la literatura científica chilena en el estudio del desarrollo positivo; además de que se discuten particularidades idiosincrásicas y desafíos asociados a la etapa de adultez emergente.


Introduction: Positive development is a stage of psychosocial adjustment that accentuates the potentials and resources of young people and their close contexts. The first positive development model for emerging adulthood was developed in Australia, which includes five components: social; life satisfaction; trust and tolerance of others; trust in the authorities and institutions; and action and civic engagement. However, this model has not been investigated in Chile. Objective: To analyze the evidence available in the scientific literature on the five components of the positive development model in emerging adults universities. Materials and methods: A narrative review of the literature published between 2013 and 2023 in the Web of Science, Scopus, SciELO, Redalyc, and Dialnet databases was conducted. Results: 83 articles were selected, of which 53 met the inclusion criteria; most of them were qualitative studies (81.13%) and in Spanish (75.47%). There is an evident disparity in terms of the number of studies and topics, e.g., there is more research on life satisfaction and civic action and commitment. Conclusions: There are gaps in the Chilean scientific literature in the study of positive development. In addition, idiosyncratic particularities and challenges associated with the stages of emerging adulthood are discussed.


Introdução: O desenvolvimento positivo é uma fase de ajustamento psicossocial que acentua o potencial e os recursos dos jovens e dos seus contextos imediatos. Na Austrália, foi desenvolvido o primeiro modelo de desenvolvimento positivo para adultos emergentes, que consiste em cinco componentes (social, satisfação com a vida, confiança e tolerância com os outros, confiança nas autoridades e instituições, ação e envolvimento cívico), mas ainda não foi investigado no Chile. Objetivo: Analisar as evidências disponíveis na literatura científica sobre os cinco componentes do modelo de desenvolvimento positivo em adultos universitários emergentes no Chile. Resumo O cuidador informal presta cuidados permanentes ou regulares a pessoas idosas em situação de dependência, sem remuneração. Objetivou-se identificar as percepções de cuidadores informais sobre motivações, necessidades e benefícios do cuidado ao idoso dependente. Estudo qualitativo realizado com 10 cuidadoras informais portuguesas, a partir de um instrumento com questões sobre o cuidado prestado ao idoso e suas percepções sobre o exercício dessa função. Os resultados revelaram como motivações para o cuidado: relação de proximidade e confiança, dever de cuidar, familiar mais disponível, proximidade da residência, inexistência de vagas e elevado custo das instituições de acolhimento e desejo do idoso permanecer na sua habitação. Os principais cuidados aos idosos são: hidratação, higiene, alimentação, administração terapêutica, companhia, apoio emocional, conforto, entretenimento, promoção da autonomia e dignidade. As necessidades identificadas pelas cuidadoras foram: apoio domiciliário, da segurança social e da entidade empregadora do cuidador, ajuda financeira, suporte psicológico e capacitação para cuidar do idoso. Os benefícios do cuidado informal para o idoso dependente apontados foram: celeridade do apoio familiar, segurança física e emocional, afeto e companheirismo. Este estudo dá voz a cidadãos cruciais.


Abstract The informal caregiver provides non-remunerated permanent or regular care to dependent older adults. This qualitative study aimed to identify the perceptions of informal caregivers about motivations, needs, and benefits of caring for dependent older adults. It was conducted with ten Portuguese informal caregivers, based on an instrument with questions about the care provided to older adults and their perceptions about performing this role. The results revealed the following motivations for care: proximity and trust relationship, duty of care, more available family members, home proximity, lack of vacancies, high cost of shelter institutions, and older adults' desire to remain in their homes. The primary care activities for older adults are hydration, hygiene, food, therapeutic administration, companionship, emotional support, comfort, entertainment, and promoting autonomy and dignity. The needs identified by the caregivers were home, social security, and the caregiver's employer support, financial help, psychological support, and training to care for the older adults. The benefits of informal care for dependent older adults were prompt family support, physical and emotional security, affection, and companionship. This study gives voice to crucial citizens.

Objetivo: Determinar las conductas sexuales protectoras en estudiantes de una universidad de Chillán, Chile. Métodos: Estudio cuantitativo, descriptivo con un muestreo probabilístico por conveniencia de 234 estudiantes universitarios, para los criterios de inclusión destaca haber iniciado actividad sexual. La recolección de datos fue a través de Google forms. El instrumento utilizado fue "Cuestionario de conducta sexual segura". Resultados: Del total de los alumnos encuestados el 64,1 % son mujeres. En cuanto al estado civil, el 93,5 % se identifica soltero. Solo el 47,4 % detiene la actividad sexual para utilizar condón. El 80,8 % evita relaciones sexuales en la primera cita, un 81,6 % nunca tienen relaciones anales sin condón, y el 99,6 % nunca ha tenido sexo con personas que utilizan drogas. Se encontró que la media para conducta sexual protectora es de 70,2 %. Conclusión: Los encuestados presentan conductas sexuales protectoras(AU)


Objective: To determine protective sexual behaviors in students at a university in Chillán, Chile. Methods: Quantitative, descriptive study with a probability sampling by convenience of 234 university students, for the inclusion criteria it stands out to have initiated sexual activity. Data collection was through Google form. The instrument used was the "Safe sexual behavior questionnaire". Results: Of the total students surveyed, 64.1% are women. Regarding marital status, 93.5% identify themselves as single. Only 47.4% stop sexual activity to use a condom. 80.8% avoid sexual relations on the first date, 81.6% never have anal relations without a condom, and 99.6% have never had sex with people who use drugs. It was found that the mean for protective sexual behavior is 70.2 %. Conclusion: The respondents present protective sexual behaviors(AU)

Introdução:O processo do cuidado integral à saúde é missão básica do Sistema Único de Saúde e da atenção primária à saúde. Objetivo:Relatar a experiência de ação interventiva na população idosa acerca da prevenção de doenças crônicas prevalentes nessa faixa etária, no território de uma unidade de saúde da família no município de Parnamirim. Metodologia:Trata-se de um estudo descritivo do tipo relato de experiência que foi desenvolvido a partir de um projeto de intervenção em estágio desaúde coletiva, com ênfase na educação em saúde para a pessoa idosa e portadora de diabetes mellitus e hipertensão arterial sistêmica, em uma unidade de saúde da família de Parnamirim, em abril de 2023. Resultados:A partir da ação, que abarcou estagiários de Medicina e a equipe multiprofissional da atenção primária à saúde, foi possível explanar a importância das ações no âmbito da saúde coletiva e do seguimento terapêutico, farmacológico e não farmacológico, para gerar autonomia e autogestão de disfunções crônicas e diminuir a probabilidade de complicações inerentes, como infarto agudo do miocárdio, nefropatia, retinopatia e polineuropatia diabética. Conclusões:A experiência foi de grande valia para todos os participantes, ao contar com elementos proeminentes na implementação da saúde no cenário da atenção básica em nosso país, especialmente com a pretensão de rastreio e controle de patologias crônicas de alta prevalência, e que são uma das principais causas de morbimortalidade no Brasil (AU).


Introduction:The process of comprehensive health care is the basic mission of the Unified Health System and primary health care. Objective:To report the experience of an intervention in a population of older adults regarding the prevention of chronic diseases prevalent in this age group in the territory of a family health unit in the city of Parnamirim. Methodology:This is a descriptive study of the experience report typethat was developed from an intervention project in a public health internship with an emphasis on health education for older adults with diabetes mellitus and systemic hypertension arterial at a family health unit in Parnamirim in april 2023. Results:Based on the action, which included medical interns and the multidisciplinary primary health care team, it was possible to explain the importance of actions within the scope of public health and therapeutic, pharmacological and non-pharmacological monitoring to generate autonomy and self-management of chronic dysfunctions and reduce the likelihood of inherent complications, such as acute myocardial infarction, nephropathy, retinopathy and diabetic polyneuropathy. Conclusions:The experience was of great value to all participants, as it included prominent elements in the implementation of health in the primary care scenario in our country, especially with the aim of screening and controlling high-prevalence chronic pathologies which are among the main causes of morbidity and mortality in Brazil (AU).


Introducción: El proceso de atención integral en salud es la misión básica del Sistema Único de Salud y de la atención primaria de salud. Objetivo: Relatar la experiencia de intervención en población de adultos mayores en relación a la prevención de enfermedades crónicas prevalentes en ese grupo etario, en el territorio de una unidad de salud de la familia de la ciudad de Parnamirim.Metodología: Se trata de un estudio descriptivo del tipo relato de experiencia que se desarrolló a partir de un proyecto de intervención en una pasantía de salud pública, con énfasis en educación en salud para adultos mayores y personas con diabetes mellitus e hipertensión arterial sistémica, en una unidad de salud de la familia de Parnamirim, en abril de 2023. Resultados: A partir de la acción, que incluyó a médicos pasantes y al equipo multidisciplinario de atención primaria a la salud, fue posible explicar la importancia de las acciones en el ámbito de la salud pública y seguimiento terapéutico, farmacológico y no farmacológico, para generar autonomía y autocontrol de las disfunciones crónicas y reducir la probabilidad de complicaciones inherentes, como infarto agudo del miocardio, nefropatía, retinopatía y polineuropatía diabética.Conclusiones: La experiencia fue de gran valor para todos los participantes, ya que incluyó elementos destacados en la implementación de la salud en el escenario de la atención primaria en nuestro país, especialmente con el objetivo de cribar y controlar patologías crónicas de alta prevalencia, que son una de las principales causas de morbilidad y mortalidad en Brasil (AU).

INTRODUCTION: The dynamic arterial elastance (EaDyn), calculated as pulse pressure variation divided by stroke volume variation, has been studied as a predictor of vasopressor weaning. However, its potential as a haemodynamic tool for tapering off vasopressors in patients with sepsis remains unexplored. Therefore, our study aimed to assess whether using EaDyn for weaning vasopressor support could reduce the duration of vasopressor support in patients with sepsis. METHODS AND ANALYSIS: This pragmatic single-centre controlled clinical trial will take place at Fundación Santa Fe de Bogotá, Colombia. Adult patients diagnosed with septic shock according to the sepsis-3 criteria and a Sequential Organ Failure Assessment score ≥4 will be included. A total of 114 patients (57 per group) will undergo conventional critical care monitoring, and the weaning of vasopressor support will be initiated based on the EaDyn or mean arterial pressure (MAP), depending on the assigned group. EaDyn will be estimated based on the measurements obtained from a PiCCO device connected to a PulsioFlex Monitoring Platform (PULSION Medical Systems SE, Feldkirchen, Germany). Our primary outcome is the difference in vasopressor support duration between the EaDyn and MAP groups.Participants and statisticians performing the statistical analysis will be blinded to the group allocation. Dependent and independent variables will be analysed through univariate and multivariate statistical tests. Since we will perform three repeated measurements for analysis, we will implement a Bonferroni post hoc correction. Additionally, Cox regression and Kaplan-Meier analyses will be conducted to address objectives related to time. ETHICS AND DISSEMINATION: This study was approved by the Ethics Committee at Fundación Santa Fe de Bogotá (CCEI-16026-2024). Written informed consent will be obtained from all participants. The results will be disseminated through publication in peer-reviewed journals and presentations at national and international events. TRIAL REGISTRATION NUMBER: NCT06118775.

OBJECTIVE: The purpose of this study was to conduct a literature review on transition programs from pediatric to adult care and the role of neurosurgery as individuals with spina bifida (SB) transition, and to provide a framework for neurosurgical providers to assist in the transition to adult-centered care. METHODS: A comprehensive literature review was conducted according to the PRISMA statement, with a search in Medline and Embase to identify US clinical programs reporting on their experiences establishing a transition program for adolescents and young adults with SB. Data were collected for authors, year, transition clinic location, model of care for transition clinic, ages served, and specialty clinical team. RESULTS: The literature search yielded 698 articles, 5 of which met the inclusion criteria. These 5 studies included 4 transition programs for which models of care and approach to transition, clinical services involved, establishment of goals, and age of initiation and transition were identified. All programs described setting transition goals, ranging from community services, to self-management, to health care navigation, to patient-driven goals, with 1 program reporting a quality-of-life measurement component to their model. CONCLUSIONS: Robust SB transition programs can be established by applying the expanded chronic care model, reviewing lessons learned by other programs, advocating at the institutional level, and seeking support via professional organizations. While the comprehensive role of neurosurgical providers in these programs is still being defined, a shared vision of enhancing the health and quality of life for individuals with SB and their families is needed by all subspecialists involved.

Background: Tuberous sclerosis complex is a genetic neurocutaneous autosomal dominant syndrome, characterized by the development of multiple benign tumors (hamartomas) affecting various systems. Heart-benign tumors that result from the complex are called cardiac rhabdomyomas. Unlike hamartomas that occur in other organs, cardiac rhabdomyomas are most prevalent in infants and very young children with tuberous sclerosis complex. We present a case of a young adult with tuberous sclerosis who had an unusually late diagnosis of cardiac rhabdomyomas. Case report: A 22-year-old male patient of Afro-descendant, diagnosed with tuberous sclerosis complex in childhood, presented with refractory epilepsy and was treated only with lacosamide. The patient came to medical consultation due to a recent history of episodic, persistent chest pain in the sternal region, associated with physical effort. Echocardiography revealed a non-dilated left ventricle, with several rounded masses of high echogenicity without pedicles at the apical level, the largest measuring 14 × 11 mm, consistent with cardiac rhabdomyomas. Conclusion: Cardiac rhabdomyomas rarely develop in adulthood for individuals with tuberous sclerosis. These late-onset cases can exhibit various symptoms, from simple to complex presentations. Regular clinical checkups are essential for adults with tuberous sclerosis complex.

Rationale & Objective: Atypical hemolytic uremic syndrome (aHUS) is a rare form of thrombotic microangiopathy (TMA) caused by complement dysregulation. Ravulizumab is a C5i approved for the treatment of aHUS. This analysis assessed long-term outcomes of ravulizumab in adults and pediatric patients with aHUS. Study Design: This analysis reports 2-year data from 2 phase 3, single-arm studies. Setting & Participants: One study included C5i-naïve adults (NCT02949128), and the other included 2 cohorts of pediatric patients (C5i-naïve and those who switched to ravulizumab from eculizumab [pediatric switch patients]; NCT03131219). Exposure: Patients received intravenous ravulizumab every 4-8 weeks, with the dose depending on body weight. Outcomes: The primary endpoint in the studies of C5i-naïve patients was complete TMA response, which consisted of platelet count normalization, lactate dehydrogenase normalization, and ≥25% improvement in serum creatinine concentrations from baseline, at 2 consecutive assessments ≥4 weeks apart. Analytical Approach: All analyses used descriptive statistics. No formal statistical comparisons were performed. Results: In total, 86 and 92 patients were included in efficacy and safety analyses, respectively. Complete TMA response rates over 2 years were 61% and 90% in C5i-naïve adults and pediatric patients, respectively. The median increase in estimated glomerular filtration rate from baseline was maintained over 2 years in C5i-naïve adults (35 mL/min/1.73 m2) and pediatric patients (82.5 mL/min/1.73 m2). Most adverse events and serious adverse events occurred during the first 26 weeks. No meningococcal infections were reported. Improvement in the Functional Assessment of Chronic Illness Therapy - Fatigue score achieved by 26 weeks was maintained over 2 years. Limitations: Limitations were the small sample of pediatric switch patients and limited availability of genetic data. Conclusions: Long-term treatment with ravulizumab is well tolerated and associated with improved hematologic and renal parameters and quality of life in adults and pediatric patients with aHUS.


This research tested a drug called ravulizumab for the treatment of atypical hemolytic uremic syndrome (aHUS). aHUS is a rare disease that causes clots in tiny blood vessels. This can damage the kidneys and other organs. We analyzed data from 2 clinical trials in which children and adults with aHUS received ravulizumab through a tube placed in a vein (intravenous line). They received ravulizumab every 4-8 weeks depending on their weight. We found that treating patients for 2 years with ravulizumab was associated with improved blood health, kidney function, and quality of life and was well tolerated. These results support ravulizumab as a long-term treatment for people with aHUS.

Objective. To determine the effect of self-distancing, self-transcendence, and family functioning on self-care agency in Mexican older adults. Methods. Correlational-explanatory design, with a sample of 253 elderly, collecting data through a simple random sampling. A personal data questionnaire was applied, the scale of: self-transcendence, the self-distancing subscale, the family APGAR and the ability to self-care in Mexican population from different demographic groups. Descriptive and inferential statistics were applied (Mann-Whitney U and a structural equation model) and the study was approved by a registered ethics committee. Results.The study had participation from 253 elderly, with a mean age of 68.02 years, with prevalence of the female sex (60.1%); the level of education was primary school or lower (51.4%). It was observed that the group of chronic diseases had lower self-distancing (U = 4.449.5, p = 0.038) and greater self-transcendence (U = 4177.0, p = 0.008), and selfcare (U = 4365.5, p = 0.024) than the group without chronic diseases. It was also found that self-transcendence, self-distancing, and family functionality produce a positive effect of 37% on selfcare. Conclusion. Self-distancing, self-transcendence, and family functionality explain an important proportion of selfcare in the elderly. Said knowledge permits understanding the care behavior of the elderly and, thus, propose future educational interventions by nursing to prevent or avoid functional, cognitive loss and social effects.


Objetivo. Determinar el efecto del autodistanciamiento, autotrascendencia y funcionamiento familiar sobre la agencia de autocuidado en adultos mayores mexicanos. Métodos. Diseño correlacional-explicativo, con una muestra de 253 adultos mayores, recolectado mediante un muestreo aleatorio simple. Se aplicó un cuestionario de datos personales, la escala de: autotrascendencia, la subescala de autodistanciamiento, el APGAR familiar y la capacidad de autocuidado en población mexicana de diferentes grupos demográficos. Se aplicó estadística descriptiva e inferencial (U de Mann-Whitney y un modelo de ecuación estructural) y contó con la aprobación de un comité de ética registrado. Resultados. Participaron 253 adultos mayores, con una media de 68.02 años, predominó el sexo femenino (60.1 %) y el grado de estudio de primaria o menor (51.4 %). Se observó que el grupo de enfermedades crónicas tuvo menor autodistanciamiento (U = 4.449,5, p = 0.038) y mayor autotrascendencia (U = 4.177,0, p = 0,008) y autocuidado (U = 4.365,5, p = 0.024) que el grupo sin enfermedades crónicas. También se encontró que la autotrascendencia, autodistanciamiento y funcionalidad familiar producen un efecto positivo de un 37 % sobre el autocuidado. Conclusión. El autodistanciamiento, autotrascendencia y la funcionalidad familiar explican una importante proporción del autocuidado en los adultos mayores. Dicho conocimiento permite entender la conducta del cuidado del adulto mayor y de esta manera proponer a futuro intervenciones educativas por enfermería con miras a prevenir o evitar la pérdida funcional, cognitiva y afectaciones sociales.

Estatísticas descritivas e inferenciais (U de Mann-Whitney e modelo de equações estruturais) foram aplicadas e tiveram aprovação de um comitê de ética registrado. Resultados. Participaram 253 idosos, com idade média de 68.02 anos, predominou o sexo feminino (60.1%) e o nível de escolaridade era fundamental ou inferior (51.4%). Observou-se que o grupo com doenças crônicas apresentou menor autodistanciamento (U=4.449.5, p=0.038) e maior autotranscendência (U=4177.0, p=0.008) e autocuidado (U=4365.5, p= 0.024) que o grupo sem doenças Crônicas. Verificou-se também que a autotranscendência, o autodistanciamento e a funcionalidade familiar produzem um efeito positivo de 37% no autocuidado. Conclusão. O autodistanciamento, a autotranscendência e a funcionalidade familiar explicam uma proporção importante do autocuidado em idosos. Esse conhecimento permite compreender o comportamento de cuidar do idoso e desta forma propor futuras intervenções educativas pela enfermagem com vistas a prevenir ou evitar perdas funcionais, cognitivas e efeitos sociais.

INTRODUCTION: Stroke in young individuals is becoming increasingly prevalent worldwide. Its causes can vary widely, so a thorough investigation by a multidisciplinary team is needed. Pinpointing the precise underlying pathology responsible for the stroke yields benefits for patients, particularly in recurrent events. CASE PRESENTATION: A 38-year-old man presented to the emergency department with symptoms suggestive of stroke, including right hemiparesis, dysarthria, ataxic gait, and right central facial palsy. The brain magnetic resonance image revealed an ischemic lesion located in the left basal ganglia and near the corona radiata. Following an extensive workup, a diagnosis of nephrotic was reached. Histopathology and the exclusion of secondary causes confirmed primary membranous nephropathy as the underlying condition. The patient underwent treatment tailored to address the specific glomerulopathy, along with anticoagulation therapy and immunosuppression as per current guidelines. Subsequent assessments showed stabilization of renal function, resolution of the edema, and the absence of new thromboembolic events during follow-up. CONCLUSION: The nephrotic syndrome should be recognized as a potential underlying cause of stroke in young patients and, therefore, it should be included in the differential diagnosis during the evaluation of patients with coagulopathies. Nephrotic syndrome screening may be done by conducting a simple urinalysis readily available in most healthcare facilities. This underlines the importance of considering renal pathology in the assessment of stroke etiologies, especially when coagulation abnormalities are present.


Introducción. Los eventos cerebrovasculares en los jóvenes son un problema creciente en todo el mundo. Su etiología puede ser variada y requieren un trabajo riguroso de un equipo multidisciplinario. La identificación de la enfermedad específica que conduce al ictus tiene un impacto beneficioso en los pacientes, especialmente en aquellos con eventos recurrentes. Presentación del caso. Se presenta el caso de un hombre de 38 años que acudió al servicio de urgencias con hemiparesia derecha, disartria, ataxia y parálisis facial central derecha. La resonancia magnética cerebral reveló una lesión isquémica localizada en los ganglios basales izquierdos, cerca de la corona radiada. Después de un estudio exhaustivo, se estableció el diagnóstico de síndrome nefrótico. No obstante, al analizar las características histopatológicas y descartar otras causas secundarias, el diagnóstico final fue una nefropatía membranosa primaria. El paciente recibió tratamiento específico para su glomerulopatía, anticoagulación e inmunosupresión según las guías vigentes. Durante el seguimiento, se encontró estabilización de la función renal, el edema se resolvió y no se identificó ningún nuevo evento tromboembólico. Conclusión. El síndrome nefrótico debe considerarse entre las posibles causas del ictus en pacientes jóvenes y debería tenerse en cuenta en los estudios de coagulopatías. El tamizaje de esta enfermedad requiere únicamente un uroanálisis, el cual está disponible en la mayoría de los centros de atención de salud.

Less than half of U.S. veterans meet physical activity guidelines. Even though changing physical activity can be challenging, prior studies have demonstrated that it is possible. Older adults are using technology to aid in such behavior change. However, research that explores the mechanisms of how technology can aid in behavior change is lacking, especially among older veterans. Thus, the purpose of this secondary, convergent mixed methods study was to explore how older veterans engaged with technologies that were used during a multicomponent telerehabilitation program. The study included veterans aged ≥60 years with ≥3 chronic medical conditions and physical function limitation. Quantitative data were collected during the primary randomized controlled trial, and qualitative data were collected via individual interviews following completion of the telerehabilitation program. Data were merged and then analyzed by high vs. low technology engagement groups. Key similarities and differences between groups were identified in five domains: satisfaction with the virtual environment, coping self-efficacy, perceptions of Annie (automated text messaging platform), experiences using the activity monitor, and self-management skills. Findings can help inform the successful integration of similar technologies into physical rehabilitation programs. Further study is warranted to understand additional factors and mechanisms that influence technology engagement in telerehabilitation.

BACKGROUND AND OBJECTIVES: The objective is to describe the demographic, clinical, functional characteristics and outcomes of older adult patients hospitalized in the acute unit of the San Ignacio University Hospital (HUSI). METHODS: Descriptive, cross-sectional observational study, based on the review of the medical records of patients hospitalized in the Geriatrics Unit of the HUSI during the period 2019-2021. VARIABLES: Demographics, comorbidities, baseline situation, main cause of entry and outcomes. The diagnosis of geriatric syndromes was made through the Barthel index, the Lawton and Brody scale, FRAIL scale, mini nutritional assessment short form and Confusion Assessment Method criteria. RESULTS: A total of 4601 patients were analyzed, whose average age was 83years (56.2% women). 72.4% had some degree of dependency for basic activities of daily living, 90.8% had some degree of dependency for instrumental activities of daily living, 32.2% had malnutrition, 15. 7% falls, 9.9% oropharyngeal dysphagia, 32.2% frailty, 28.1% delirium, 54.1% previous dementia. The main comorbidities presented were arterial hypertension, chronic obstructive pulmonary disease and diabetes. 2.9% had some complication during their hospitalization, 10.8% died, and the hospital stay was 5days. CONCLUSION: Older adult patients admitted to the acute unit of the HUSI have a high frequency of dependency, dementia and nutritional disturbances.

Ovarian cancer (OC) is the leading cause of death in women with gynecological cancers. Its diagnosis is more likely in advanced ages, with the older population being the most seen in consultations. Poly(ADP-ribose) inhibitors (PARPi) have changed OC clinical practice and evolution, showing great benefit. However, there is a lack of evidence of PARPi in elderly population that can impact the therapeutic decision and the safety/efficacy. It is necessary to avoid age as limiting factor in PARPis prescription. We conducted a review of the most relevant randomized phase III trials of maintenance PARPi after first-line treatment of advanced OC. We observed the lack of a single criterion for considering older patients, varying among trials. There is a benefit of PARPis in different populations. However, PARPi effect on quality of life is not reported, something of great relevance considering their vulnerability. Measures are needed to benefit older patients to better adapt PARPi treatment.

RESUMO O objetivo deste estudo foi compreender as representações sociais de jovens que vivem com o HIV. Estudo qualitativo, ancorado na Teoria das Representações Sociais, realizado com 16 jovens em acompanhamento ambulatorial. Os dados foram coletados entre maio e outubro de 2022, por meio de entrevista semiestruturada. Para o tratamento dos dados, empregou-se a técnica de análise lexical, operacionalizada com o auxílio do software Iramuteq. A representação fortemente associada ao diagnóstico de HIV está relacionada ao desconhecimento, preconceito e aceitação, apresentando, também, elementos positivos como recomeço. Classes: Diagnóstico positivo para o HIV: o 'eu' de agora; HIV em face do desconhecimento e do estigma; e Relações interpessoais: diálogo, acolhimento e respeito necessários aos jovens. Compreendeu-se que as representações sociais dos jovens com HIV refletem a sua condição na sociedade. Os dados deste estudo mostram que existem formas distintas de vivenciar o HIV, muitas vezes representadas pela fragilidade, pela resignação, e outras, pelo fortalecimento. A fim de minimizar a vulnerabilidade do jovem à exposição ao HIV e aos riscos de conviver com o estigma e preconceito social, fazem-se necessárias a adoção de estratégias de conscientização da sociedade sobre a doença e a ampliação da divulgação de políticas públicas que abarcam essa temática.


ABSTRACT This study was aimed at understanding the social representations of young people living with HIV. This is a qualitative study, based on the Social Representations Theory, carried out with 16 young people in outpatient follow-up. Data were collected between May and October 2022, through semi-structured interviews. For data processing, the lexical analysis technique was used, operationalized with the aid of the IRAMUTEQ software. The representation strongly associated with the diagnosis of HIV is related to lack of knowledge, prejudice and acceptance, also presenting positive elements such as a new beginning. Classes: Positive diagnosis for HIV: the new 'me'; HIV in the face of ignorance and stigma; and Interpersonal relationships: dialogue, acceptance and respect necessary for young people. It was understood that the social representations of young people with HIV reflect their condition in society. The data show that there are different ways of experiencing HIV, often represented by fragility, resignation, and other times by strengthening. In order to minimize the vulnerability of young people to exposure to HIV and the risks of living with stigma and social prejudice, it is necessary to adopt strategies to raise awareness about the disease and to expand the dissemination of public policies that cover this topic.

El 25% de los pacientes con Enfermedades Inflamatorias Intestinales (EII) se diagnostican antes de los 20 años. En la mayor parte de los centros del país se lleva a cabo la "transferencia" del paciente desde un centro de atención pediátrico a uno de adultos. La "transición" es un criterio de calidad con beneficios en el control de la EII reduciendo el número de recaídas, de hospitalizaciones y de cirugías. Por tal motivo hemos desarrollado un Programa Interdisciplinario de Transición entre dos hospitales de referencia nacional e internacional en EII. Materiales y métodos: Entre 1/2021 y 12/ 2022 se incorporaron 24 pacientes que ingresaron en 3 fases: Fase 1 Pacientes entre 14 y 16 años asistidos en el Hospital Garrahan (HG) con un abordaje interdisciplinario. Fase 2. A partir de los 17 años se realizaron 2 (dos) encuentros en el HG en conjunto con gastroenterólogos de adultos evaluando adherencia y autonomía y la Fase 3 llevada a cabo en el Hospital B. Udaondo (HBU) sólo con el equipo de adultos luego de 6 meses de realizada la transferencia evaluando adherencia al tratamiento, consultas a emergencias, internación y/o cirugías Resultados: Al inicio del Programa el 66% de los pacientes presentaban una actividad moderada a severa vs el 8% al finalizar la fase 3. Luego de la transferencia el 12,5% necesito ingreso a guardia e internación y un 8% tratamiento quirúrgico. El 83% de los pacientes continúan en seguimiento luego de 6 meses de haber sido transferidos (AU)


Twenty-five percent of patients with inflammatory bowel diseases (IBD) are diagnosed before the age of 20 years. In most centers in the country, the "transfer" of the patient from a pediatric to an adult care center is done. However, "transition" is a quality criterion with benefits in the control of IBD by reducing the number of relapses, hospitalizations, and surgeries. For this reason, we developed an Interdisciplinary Transition Program between two national and international reference hospitals in IBD. Materials and Methods: Between January 2021 and December 2022, we incorporated 24 patients into a three-phase program. Phase 1 involved patients between 14 and 16 years of age seen at Garrahan Hospital (HG) with an interdisciplinary approach. Phase 2 began from 17 years of age, with two meetings held at HG involving adult gastroenterologists to evaluate adherence and autonomy. Phase 3 was conducted at Hospital B. Udaondo (HBU) only with the adult team, six months after the transfer, evaluating adherence to treatment, emergency consultations, hospitalizations, and/or surgeries. Results: At the beginning of the program, 66% of the patients presented with moderate to severe disease activity, compared to 8% at the end of Phase 3. After the transfer, 12.5% of the patients required emergency department visits and hospitalization, and 8% required surgical treatment. Eighty-three percent of the patients continue in the program and are still being followed up six months after the transfer (AU)

ResumoObjetivo: Avaliar a dificuldade na visibilidade e palpação da rede venosa periférica em adultos e idosos admitidos em uma unidade de intervenção hemodinâmica. Método:Trata-se de um estudo observacional e prospectivo com abordagem quantitativa. Realizado com 105 participantes, dentre adultos e idosos, admitidos em unidade de intervenção hemodinâmica, em um hospital geral público e de ensino. Os dados foram coletados por meio de instrumento estruturado contendo variáveis demográficas e clínicas, com observação da punção intravenosa periférica. Realizou-se a análise descritiva, calculando as proporções. Resultados: A dificuldade durante a palpação e a visualização da veia foram de 11,4% e 16,2%, respectivamente. Entre os participantes que apresentaram dificuldade para visualização da veia, 60% tiveram insucesso na primeira tentativa de punção. Conclusão: A punção periférica em veias que possuem maior dificuldade para visibilidade e palpação contribuem para o insucesso na primeira tentativa.


AbstractObjective: To evaluate the difficulty in visibility and palpation of the peripheral venous network in adults and older adults admitted to a hemodynamic intervention unit. Method: An observational prospective study was conducted with 105 adults and older adults admitted to a hemodynamic intervention unit, in a public general teaching hospital. Data were collected by means of a structured instrument consisting of demographic and clinical variables, with observation of peripheral intravenous puncture from October to December 2022. Descriptive analysis was performed, calculating proportions. Results: Results show a difficulty during palpation and visualization of the vein of 11.4% and 16.2%, respectively. Among those who had difficulty visualizing the vein, 60% were unsuccessful in the first puncture attempt. Conclusion: Peripheral puncture in veins with greater visibility and palpation difficulty contribute to failure in the first attempt


ResumenObjetivo: Evaluar la dificultad de visibilidad y de palpación de la red venosa periférica en adultos y ancianos ingresados en una unidad de intervención hemodinámica. Método: Se trata de un estudio observacional y prospectivo con enfoque cuantitativo. Participaron 105 adultos y ancianos ingresados en una unidad de intervención hemodinámica, en un hospital general público y universitario. Los datos se recolectaron mediante un instrumento estructurado, que contiene variables demográficas y clínicas, con observación de punción intravenosa periférica, durante los meses de octubre a diciembre de 2022. Se realizó un análisis descriptivo en el cual se calculó las proporciones. Resultados: La dificultad reportada durante la palpación y la visualización de la vena fue del 11,4% y el 16,2%, respectivamente. Entre los participantes que tuvieron dificultades en la visualización de la vena, el 60% falló en el primer intento de punción. Conclusión:La punción de venas periféricas que presentan mayores dificultades en la visibilidad y la palpación tiene tendencia a fallar en el primer intento

SUMMARY: The average volumes of normal heart chambers in computed tomography (CT) are used not only as clinical criterions for heart disease diagnosis, but also as references in cardiology. With the development of artificial intelligence (AI), numerous CT data can be analyzed and segmented automatically. This study aimed to determine the average volumes of the four chambers in healthy adult hearts and present surface models with the average volume. Coronary CT angiographs of 508 Korean individuals (330 men and 178 women, 20 - 39 years old) were obtained. An automatic segmentation module for 3D Slicer was developed using machine learning in Anatomage KoreaTM. Using the module, the four chambers and heart valves in the CT were segmented and reconstructed into surface models. Surface models of the four chambers of identical hearts in the CT were produced using SimplewareTM. The volumes of structures were measured using Sim4life Light and statistically analyzed. After determining the average volumes of the four chambers, surface models of the average volumes were constructed. In both software measurements, the atrial volumes of females increased with age, and the ventricular volumes of males decreased significantly with age. The atrial and ventricular volumes of Simpleware were larger and smaller than those of Anatomage, respectively, because of errors in the Simpleware. Regarding the volume measurement, our module developed in this study was more accurate than the Simpleware. The average volume and three-dimensional models used in this study can be used not only for clinical purposes, but also for educational or industrial purposes.


Los volúmenes medios de las cámaras cardíacas normales en la tomografía computarizada (TC) se utilizan no sólo como criterios clínicos para el diagnóstico de enfermedades cardíacas, sino también como referencia en cardiología. Con el desarrollo de la inteligencia artificial (IA), numerosos datos de TC se pueden analizar y segmentar automáticamente. Este estudio tuvo como objetivo determinar los volúmenes promedio de las cuatro cámaras en corazones adultos sanos y presentar modelos de superficie con el volumen promedio. Se obtuvieron angiografías coronarias por TC de 508 individuos coreanos (330 hombres y 178 mujeres, de 20 a 39 años). Se desarrolló un módulo de segmentación automática para 3D Slicer utilizando aprendizaje automático en Anatomage KoreaTM. Utilizando el módulo, las cuatro cámaras y valvas cardíacas de la TC se segmentaron y reconstruyeron en modelos de superficie. Se produjeron modelos de superficie de las cuatro cámaras de corazones idénticos en la TC utilizando SimplewareTM. Los volúmenes de las estructuras se midieron utilizando Sim4life Light y se analizaron estadísticamente. Después de determinar los volúmenes promedio de las cuatro cámaras, se construyeron modelos de superficie de los volúmenes promedio. En ambas mediciones de software, los volúmenes atriales de las mujeres aumentaron con la edad y los volúmenes ventriculares de los hombres disminuyeron significativamente con la edad. Los volúmenes atrial y ventricular de Simpleware eran mayores y menores que los de Anatomage, respectivamente, debido a errores en Simpleware. En cuanto a la medición de volumen, nuestro módulo desarrollado en este estudio fue más preciso que el Simpleware. Los modelos tridimensionales y de volumen medio utilizados en este estudio se pueden utilizar no solo con fines clínicos, sino también con fines educativos o industriales.
