Introducción: Las enfermedades alérgicas son muy comunes en la población pediátrica. Entre las causas frecuentes se encuentran los aeroalérgenos del ambiente, y la identificación de estos es de gran ayuda tanto para el diagnóstico como para el tratamiento. Objetivos: Determinar la frecuencia de aeroalérgenos, por medio de la determinación de Inmunoglobulina E (IgE) específica a alérgenos comunes por la prueba de sensibilidad cutánea en pacientes pediátricos con síntomas de asma y rinitis alérgica. Materiales y Métodos: Diseño observacional, descriptivo, prospectivo, transversal, población de pacientes de 4 a 17 años con síntomas compatibles con asma y rinitis alérgica que acudieron a un centro asistencial pediátrico en el periodo de estudio. Se realizó por medio de las Pruebas de punción cutánea (PCP). Resultados: La edad media de los pacientes fue de 8 años, 57% pacientes de sexo masculino y 43% de sexo femenino. El 53% de los pacientes presenta rinitis y asma, 45% solo rinitis y el 2% solo asma. El 79% de los pacientes presentó reacción positiva de sensibilización alérgica por medio de la PCP. Teniendo en cuenta la sensibilización por tipo de aeroalérgenos se tuvo que el 64% de los pacientes tuvo reacción positiva a ácaros, 19% a animales 18% a cucarachas, 8% a pólenes y 6% a hongos. Conclusión: La gran mayoría de pacientes con asma padecía rinitis alérgica concomitante y los ácaros del polvo fueron los aeroalérgenos más frecuentemente determinados en las pruebas cutáneas de alergia en niños con asma y rinitis de nuestra población.


Introduction: Allergic diseases are very common in the pediatric population. Among the frequent causes are aeroallergens from the environment and the identification of these is a great help for diagnosis and treatment. Objectives: To determine the frequency of aeroallergens, through the determination of Immunoglobulin E (IgE) specific to common allergens by the skin sensitivity test in pediatric patients with symptoms of asthma and allergic rhinitis. Materials and Methods: Observational, descriptive, prospective, cross-sectional design, population of patients from 4 to 17 years old with symptoms compatible with asthma and allergic rhinitis who attended a pediatric care center during the study period. It was carried out by means of Skin Puncture Tests (PCP). Results: The mean age of the patients was 8 years, 57% male patients and 43% female. 53% of the patients presented Rhinitis and Asthma, 45% only Rhinitis and 2% only Asthma. 79% of the patients presented a positive allergic sensitization reaction through PCP. Taking into account the sensitization by type of aeroallergens, 64% of the patients had a positive reaction to mites, 19 % to animals 18% to cockroaches, 8% to pollens and 6% to fungi. Conclusion: The vast majority of patients with asthma suffered from concomitant allergic rhinitis and dust mites were the most frequently determined aeroallergens in allergic skin tests in children with asthma and rhinitis in our population.

OBJECTIVE: Environmental control includes measures to prevent exposure to common aeroallergens in an individual's home. Questionnaires are part of the clinical practice of health assessment, and are also widely used in research. Our aim was to develop and validate a questionnaire to identify possible sources of aeroallergens present in the indoor environment. METHODS: This study describes the development, validation and application of a questionnaire. For content validation the Content Validation Index and Ordinal Cronbach's Alpha Index have been used; Polychoric Correlations for the agreement between judges; and an Exploratory Factor Analysis for the structure of the questionnaire, while for reliability assessment, Intraclass Correlation Coefficient has been applied. RESULTS: Twenty-one doctors participated as judges to validate the questionnaire, which 204 patients answered. The Content Validity Index for all the questions on the "Clarity" aspect was 0.846 ± 0.152 and on the "Relevance" aspect, 0.954 ± 0.080. Cronbach's alpha coefficient for the "Clarity" aspect was 0.88 with a 95% confidence intervals (CI) and the "Relevance" aspect, 0.94 with a 95% CI. The average Intraclass Correlation Coefficient was 0.94 and all the F tests were highly significant. CONCLUSIONS: The questionnaire developed by our group was considered valid and reliable, and is capable of portraying the home environment without the need for a personal visit to the patient's home. This questionnaire would be a good tool to use in research or during patient consultations to assess the patient's home environment, as this latter assessment is essential for the management of patients with respiratory allergies.

This study aims to evaluate the sensitization distribution of aeroallergens in patients with allergic conjunctivitis (AC) living in a temperate subhumid region and to describe the prevalence in a Mexican cohort. A total of 761 patient records were revisited, including the results of the skin prick test (SPT) for 45 aeroallergens. We found 292 patients with AC and a positive SPT in a 5-year period. The most frequent aeroallergens include dust mites (69.2% for Dermatophagoides pteronyssinus and 47.6% for Dermatophagoides farinae), trees (36% for cypress and 22.6% for ash), animals (33.9% for dogs and 26.7% for cats), and grasses (21.2% for Paspalum notalum and 19.9% for Poa pratensis). Among the studied population, a prevalence of 38.4% for the disease was calculated. House dust mites were responsible for most sensitizations found in the studied population. There is a high prevalence of AC in the selected cohort during the period studied.

Extensive flooding caused by Hurricane María in Puerto Rico (PR) created favorable conditions for indoor growth of filamentous fungi. These conditions represent a public health concern as contamination by environmental fungi is associated with a higher prevalence of inflammatory respiratory conditions. This work compares culturable fungal spore communities present in homes that sustained water damage after Hurricane María to those present in dry, non-flooded homes. We collected air samples from 50 houses in a neighborhood in San Juan, PR, 12 and 22 months after Hurricane María. Self-reported data was used to classify the homes as flooded, water-damage or dry non-flooded. Fungi abundances, composition and diversity were analyzed by culturing on two media. Our results showed no significant differences in indoor fungal concentrations (CFU/m3) one year after the Hurricane in both culture media studied (MEA and G25N). During the second sampling period fungal levels were 2.7 times higher in previously flooded homes (Median = 758) when compared to dry homes (Median = 283), (p-value < 0.005). Fungal profiles showed enrichment of Aspergillus species inside flooded homes compared to outdoor samples during the first sampling period (FDR-adjusted p-value = 0.05). In contrast, 22 months after the storm, indoor fungal composition consisted primarily of non-sporulated fungi, most likely basidiospores, which are characteristic of the outdoor air in PR. Together, this data highlights that homes that suffered water damage not only have higher indoor proliferation of filamentous fungi, but their indoor fungal populations change over time following the Hurricane. Ultimately, after nearly two years, indoor and outdoor fungal communities converged in this sample of naturally ventilated homes.

BACKGROUND: Allergy skin prick tests are important tools for the diagnosis of respiratory allergic diseases. Cockroach antigens have been identified as the cause of rhinitis, asthma, and other allergic diseases. OBJECTIVE: To show that the cockroach antigen participates in the cause of allergic diseases. METHODS: A retrolective and cross-sectional study was carried out, for which clinical records and history of atopy were reviewed; the results were obtained from blood biometry, nasal cytology, total IgE, and coproparasitoscopic serial tests. After prior informed consent, skin tests were applied in 3-74 year-old patients. RESULTS: 1,837 patients were studied; the prevalence of cutaneous reactivity to the cockroach antigen was of 17.90%; 56% of the patients had a diagnosis of allergic rhinitis, and only 6% had been diagnosed with asthma and rhinitis. CONCLUSION: The application of the cockroach antigen in skin tests must be considered in the practice of all allergists.


Antecedentes: Las pruebas cutáneas por técnica de punción son herramientas importantes para el diagnóstico de la alergia respiratoria. Se han identificado los antígenos de las cucarachas como responsables del desarrollo de rinitis, asma y otras enfermedades alérgicas. Objetivo: Demostrar la participación del antígeno de cucaracha en enfermedades alérgicas. Métodos: Se llevó a cabo un estudio retrolectivo y transversal, para el cual se revisaron historias clínicas y los antecedentes de atopia; se recolectaron los resultados de biometría hemática, citología nasal, IgE total y estudio coproparasitoscópico seriado. Previo consentimiento informado se aplicaron pruebas cutáneas en pacientes de tres a 74 años. Resultados: Se estudiaron 1837 pacientes, la prevalencia de reactividad cutánea al antígeno de cucaracha fue de 17.90 %; 56 % de los pacientes tenía diagnóstico de rinitis alérgica y solo 6 %, de asma y rinitis. Conclusión: Se debe considerar la aplicación del antígeno de cucaracha en las pruebas cutáneas en la práctica del alergólogo.

Pollinosis is sub-diagnosed and rarely studied in tropical countries. Cashew tree pollen has been reported as an allergen source although the knowledge of its immunoglobulin E (IgE)-reactive molecules is lacking. Therefore, this work aimed to identify IgE-reactive molecules and provide a proteomic profile of this pollen. From the 830 proteins identified by shotgun analysis, 163 were annotated to gene ontology, and a list of 39 proteins filtered for high confidence was submitted to the Allfam database where nine were assigned to allergenic families. Thus, 12 patients from the northeast of Brazil with persistent allergic rhinitis and aggravation of symptoms during cashew flowering season were selected. Using a 2D-based approach, we identified 20 IgE-reactive proteins, four already recognized as allergens, including a homolog of the birch isoflavone-reductase (Bet v 6). IgE-reactivity against the extract in native form was confirmed for five patients in ELISA, with three being positive for Bet v 6. Herein, we present a group of patients with rhinitis exposed to cashew tree pollen with the first description of IgE-binding proteins and a proteomic profile of the whole pollen. Cashew tree pollen is considered an important trigger of rhinitis symptoms in clinical practice in the northeast of Brazil, and the elucidation of its allergenic molecules can improve the diagnostics and treatment for allergic patients.

Las enfermedades alérgicas han aumentado en las últimas décadas en todo el mundo. El asma y la rinitis alérgica podrían representar un espectro de la misma enfermedad, cuya patogénesis puede explicarse, entre otros factores, por la sensibilización a aeroalergenos. Los aeroalergenos más frecuentemente involucrados como sensibilizantes se hallan en el polvo del interior del hogar o lugar de trabajo (indoor). Dentro de los aeroalergenos indoor, los dermatophagoides son los más prevalentes. Este estudio demuestra que la prevalencia de sensibilidad a aeroalergenos indoor, en pacientes con rinitis y asma, es coincidente con la bibliografía. Es un área a investigar en el futuro, la prevalencia de sensibilidad a aeroalergenos outdoor


Allergic diseases have increased in recent decades worldwide. Asthma and allergic rhinitis could represent a spectrum of the same disease, whose pathogenesis can be explained, among other factors, by sensitization to aeroallergens. The aeroallergens most frequently involved as sensitizers are found in the dust inside the home or workplace (indoor). Within indoor aeroallergens, dermatophagoides are the most prevalent. This study demonstrates that the prevalence of sensitivity to indoor Aeroallergens, in patients with rhinitis and asthma, is consistent with the literature. It is an area to investigate in the future, the prevalence of sensitivity to outdoor aeroallergens.

BACKGROUND: Symptoms of allergic diseases are a common cause of consultation during childhood. OBJECTIVE: To describe the profile of a population of Colombian preschool children seen in an allergology department and assess possible risk factors. METHODS: Descriptive, cross-sectional study of patients younger than 5 years who were assessed for one year. A complete medical history was obtained and allergology workup was carried out with skin prick tests as appropriate for each case. Risk factors were assessed by means of multivariate analysis. RESULTS: A total of 674 patients were included, out of which 382 (52.7%) were males. Median age was 34 months. The reasons for consultation were cough or recurrent wheezing (54.3%), rhinitis (48.7%), and eczema (32.2%). Skin prick tests were performed with aeroallergens in 299 patients (48%) and with food in 170 (27%), with positivity being found in 114 (38%) and 16 (9.4%), respectively. Main sensitizers were dust mites and egg. A direct association was found between eczema and prematurity (OR = 0.496; 95 % CI = 0.289-0.823)], between recurrent cough/wheezing and family history of allergy (OR = 1.837; 95 % CI = 1,306-2,586), and between recurrent cough/wheezing and history of bronchiolitis (OR = 2.646; 95 % CI = 1.812-3.886). CONCLUSION: Respiratory symptoms represented the main cause of consultation. Dust mites were the most commonly identified allergens. Family history of allergy and bronchiolitis.


Antecedentes: Los síntomas de enfermedad alérgica constituyen una causa frecuente de consulta en la infancia. Objetivo: Describir el perfil de una población de preescolares colombianos atendidos en un servicio de alergología y evaluar los posibles factores de riesgo. Métodos: Estudio descriptivo trasversal de pacientes menores de cinco años evaluados durante un año. Se realizó historia clínica completa y estudio alergológico con pruebas cutáneas según el caso. Se analizaron factores de riesgo mediante análisis multivariado. Resultados: Se incluyeron 674 pacientes, 382 (52.7 %) hombres. La mediana de edad fue 34 meses. Los motivos de consulta fueron tos o sibilancias recurrentes (54.3 %), rinitis (48.7 %) y eccema (32.2 %). Se realizaron pruebas cutáneas con aeroalérgenos a 299 pacientes (48 %) y con alimentos a 170 (27 %), encontrando positividad en 114 (38 %) y 16 (9.4 %), respectivamente. Los principales sensibilizadores fueron los ácaros y el huevo. Se encontró asociación directa entre eccema y prematurez (OR = 0.496; IC 95 % = 0.289-0.823), tos/sibilancias recurrentes e historia familiar de alergia (OR = 1.837; IC 95 % = 1.306-2.586), y tos/sibilancias recurrentes y antecedente de bronquiolitis (OR = 2.646; IC 95 % = 1.812-3.886). Conclusión: Los síntomas respiratorios representaron la principal causa de consulta. Los ácaros fueron los alérgenos más identificados. La historia familiar de alergia y la bronquiolitis parecen ser factores de riesgo.

INTRODUCCIÓN: La rinitis alérgica (RA) es una enfermedad inflamatoria de la mucosa nasal mediada por IgE. Existen controversias en los diferentes estudios acerca de la relación entre la gravedad de los síntomas de RA y la magnitud de la sensibilización medidas por pruebas cutáneas con alergenos. OBJETIVOS: Relacionar la magnitud de sensibilización alérgica con los grados clínicos de la RA. POBLACIÓN Y MÉTODO: Estudio Observacional, analítico y transversal, que incluyó pacientes de ambos sexos, entre 6 y 18 años con diagnóstico de RA. Se clasificaron según el grado clínico establecido por Allergic rhinitis and its impact on asthma (ARIA). La sensibilización alérgica se midió a través de pruebas cutáneas por puntura con aeroalergenos y se determinó su magnitud según: PC1 (número de alérgenos positivos), PC2 (sumatoria de milímetros de pápulas) e Índice de atopía (relación entre PC2/PC1). Se dividieron según la presencia de monosensibilización y/o polisensibilización. ANÁLISIS ESTADÍSTICO: Se realizó estadística descriptiva y analítica utilizando prueba de Wilcoxon Mann Withney y prueba de Kruskal Wallis. Se consideró un valor significativo a una p<0,05. RESULTADOS: Se incluyeron 156 pacientes con una mediana de edad de 11,18 años (5-18,66 años). No se encontró diferencia en los valores de PC1, PC2 ni en el índice de atopía entre los cuatro grados clínicos de rinitis (p=0,3370, p=0,2222 y p=0,8774 respectivamente). Tampoco se observó diferencia en el índice de atopia entre los pacientes monosensibilizados y polisensibilizados (p=0,5493). Al comparar la rinitis según la frecuencia no se encontró diferencia en los valores de PC1, PC2 ni en el índice de atopía entre los grupos intermitente y persistente (p=0,0810; p=0,0587 y p=0,5705). Con respecto a la gravedad de la rinitis no se encontró diferencia en los valores de PC1, PC2 ni en el índice de atopía entre los grupos leve y moderado/grave (p=0,5693; p=0,1653 y p=0,4613). CONCLUSIONES: Las pruebas cutáneas por puntura de lectura inmediata no son un método diagnóstico capaz de diferenciar los grados clínicos de rinitis. (AU)


INTRODUCTION: Allergic rhinitis (RA) is an inflammatory disease of nasal mucosal mediated by IgE. There are controversies in the different studies about the relationship between the severity of RA symptoms and the magnitude of sensitization measured by skin tests with allergens. OBJECTIVE: Relate the magnitude of allergic sensitization to the clinical grades of AR. PATIENTS AND METHODS: In a observational, analytical and crosssectional study, we included patients aged 6 to 18 years with diagnosis of allergic rhinitis (RA). Were classified according to the clinical grade established by Allergic Rhinitis and its Impact on Asthma (ARIA) guide. Allergic sensitization was measured through skin prick test with aeroallergens and their magnitude was determined according to: PC1 (Number of positive allergens), PC2 (sum of millimeters of papules) and index of atopy (relation between PC2/PC1). The patients were divided in two groups according to the presence of monosensitization and/or polysensitization. RESULTS: We included 156 patients with a median age of 11,18 years (5-18,66 years). There were no difference in the values of PC1, PC2 or in the index of atopy between the clinical grades of rhinitis (p=0,337; p=0,2222 and p=0,8774 respectively). There was no difference in the index of atopy between monosensitized and polysensitized patients (p=0,5493). When comparing rhinitis according to the frequency, no difference was found in the values of PC1, PC2 or the index of atopy between the intermittent and persistent groups (p=0,0810; p= 0,0587 and p= 0,5705). Regarding the severity of rhinitis, no difference was found in the values of PC1, PC2 or in the index of atopy between the mild and moderate/severe groups (p=0,5693; p=0,1653 and p= 0,4613). CONCLUSIONS: Skin prick tests with aeroallergens are not a diagnostic method capable of differentiating the clinical grades of rhinitis. (AU)

BACKGROUND: Allergic diseases diagnosis must be based on adequate allergological anamnesis and an immunological sensitization test; the most sensitive and specific is the skin prick test. OBJECTIVE: To determine the frequency of skin reactivity to aeroallergens, by age groups, in patients of the Department of Allergy and Clinical Immunology of the Hospital Universitario de Puebla, in Mexico. METHODS: A cross-sectional study was conducted that included patients aged 2 to 64 years with symptoms suggestive of allergic disease, in which skin prick tests with aeroallergens were performed; the diagnostic criteria were those of international guidelines. Frequencies, percentages and dispersion measures were calculated. RESULTS: Of 173 patients, 63 % were females. Mean age was 22.3 years. The frequency of skin reactivity for Quercus sp. was 12.72 %, for Periplaneta americana, 9.83 %, for Dermatophagoides farinae, 9.25 %, for Cynodon dactylon, 8.09 %, for Blatella germanica, 8.09 %, for Holcus halepensis, 6.94 %, for Dermatophagoides pteronyssinus, 6.36 %, for Schinus molle, 5.78 %, for Fraxinus uhdei, 5.20 %, for Lolium perenne, 5.20 %, for Ambrosia eliator, 5.20 % and for Artemisa tridentata, 4.62 %. CONCLUSION: Although Dermatophagoides are the most frequently reported aeroallergens, the most common aeroallergen in this study was pollen, probably owing to geographical and environmental factors, although this was not observed in the analysis by age groups.


Antecedentes: El diagnóstico de las enfermedades alérgicas debe basarse en la historia clínica alergológica adecuada y en una prueba inmunológica de sensibilización; la de mayor sensibilidad y especificidad es la prueba cutánea por punción. Objetivo: Determinar la frecuencia de la reactividad cutánea hacia aeroalérgenos, por grupos etarios, en pacientes del Servicio de Alergia e Inmunología Clínica del Hospital Universitario de Puebla, México. Métodos: Se realizó estudio transversal que incluyó a pacientes de 2 a 64 años de edad, con síntomas sugestivos de enfermedad alérgica, en quienes se realizaron pruebas cutáneas con aeroalérgenos; los criterios diagnósticos fueron los de las guías internacionales. Se calcularon frecuencias, porcentajes y medidas de dispersión. Resultados: De 173 pacientes, 63 % fue del sexo femenino. La edad media fue de 22.3 años. La frecuencia de la reactividad cutánea para Quercus sp. fue 12.72 %, Periplaneta americana 9.83 %, Dermatophagoides farinae 9.25 %, Cynodon dactylon 8.09 %, Blatella germanica 8.09 %, Holcus halepensis 6.94 %, Dermatophagoides pteronyssinus 6.36 %, Schinus molle 5.78 %, Fraxinus uhdei 5.20 %, Lolium perenne 5.20 %, Ambrosia eliator 5.20 % y Artemisa tridentata 4.62 %. Conclusión: Los Dermatophagoides son los aeroalérgenos más identificados, pero en el presente estudio fue más común un polen, probablemente debido a factores geográficos-medioambientales, aunque no fue así en el análisis por grupos etarios.

Objetivos: A IL-10 pode imunomodular as respostas alérgicas no hospedeiro. A respeito dos geohelmintos, trabalhos divergem quanto à alteração do quadro clínico e/ou testes cutâneos de hipersensibilidade. Pouco é conhecido sobre a produção de IL-10 frente a aeroalérgenos durante as geohelmintíases. Neste estudo, verificamos a produção de IL-10 por células mononucleares de sangue periférico de pacientes com ou sem geohelmíntiases, de zona urbana, em resposta ao Dermatophagoides pteronyssinus ou Blomia tropicalis. Métodos: Estudo experimental, com 21 pacientes de zona urbana, em que foram realizadas 84 determinações de IL-10. Após exames coprológicos, em triplicata, dois grupos foram formados, crianças enteroparasitadas e não-parasitadas. Amostras de sangue foram coleta das para purificação de células mononucleares e cultivo com antígenos de Ascaris sp (Asc), mitógeno concanavalina A (Con A), O. pteronyssinus (Dep) e B. tropicalis (Bt). A IL-10 do sobrenadante celular foi dosada (ELISA). Resultados: Idade dos pacientes variou de 9-12 anos e sem diferença quanto ao sexo. Nos indivíduos parasitados a carga parasitária foi baixa. Frente ao Asc, Con A ou Dep não houve diferença na síntese de IL-10 pelas células do grupo parasitado e não parasitado. Contudo, uma produção significativamente maior de IL-10, frente ao estímulo com Bt, nos não-parasitados foi observada. Conclusões: Pacientes de zona urbana parasitados produzem menos IL-10 frente à B. tropicalis, alérgeno mais prevalente em Recife, podendo levar a predisposição às manifestações alérgicas. Estes dados corroboram a hipótese que baixa carga parasitária de geohelmintos, principalmente Ascaris lumbricoides, pode predispor à asma/sibilância.


Objectives: IL-10 can immunomodulate allergic reactions in the host. Concerning of the geohelminths, the studies diverge about the alteration of clinical allergic disease and/or skin prick test reactivity. IL-10 production in response to aeroallergens during the geohelminths infection is poorly known. In this study, we verified the IL-10 syntheses by peripheral blood mononuclear cells from low parasitic load infected or non-infected patients coming from urban areas, when stimulated with Dermatophagoides pteronyssinus (Dep) or Blomia tropicalis (Bt). Methods: Experimental analysis of 21 patients from urban areas, which was performed 84 IL-10 determinations. After the parasitological exams (Hofmann/Kato-Katz), in triplicate, two groups were obtained, geohelminths infected and non-infected children. Blood sample were harvested for purification of mononuclear cells and culture with Ascaris sp antigen (Asc), concanavalina A mitogen (Con A), Dep and Bt. IL-10 content in cellular supernatant was measured (ELISA). Results: Patients age was among 9-12 years old and without difference in gender. In parasite infected individual the parasite load was low. In response to Asc, Con A or Dep there was not difference in the IL-10 synthesis. However, in response to Bt stimulus, high production of IL-10 by non-parasite infected group was observed. Conclusions: Parasite infected patients from urban areas product less IL-10 to Bt, allergen more prevalent in Recife, could lead to predisposition to allergic manifestation. These finding corroborate the hypotheses that low parasite load of geohelminths, especially Ascaris lumbricoides, can predispose to asth ma/wheezing.
