RESUMO

Recently we identified a P408S, P415L allele of beta-glucuronidase in several Mexican patients with mucopolysaccharidosis type VII (Sly syndrome) and presented evidence that both mutations are required to produce the MPS VII allele (Islam et al., 1996). In an attempt to determine whether either of these mutations exists as a benign polymorphism among Mexicans, we developed a PCR method to screen simultaneously for both mutations and used it to screen a population sample of 187 Mexican individuals in the Guadalajara area, all from the north-western states of Mexico. Neither mutation was present in 374 alleles studied. In addition, we had the opportunity to carry out prenatal diagnosis in a fetus at risk for homozygosity for this MPS VII allele at the 15th week of gestation using enzymatic assays as well as by analysis of genomic DNA isolated from cultured amniotic fluid cells. The fetus was found to be heterozygous for the P408S, P415L allele. The newborn's heterozygosity was confirmed after birth by enzyme assays on plasma and leukocytes, and by analysis of DNA from leukocytes.

To determine the value of computed tomography and electrophysiologic studies in predicting neurologic outcome, we prospectively studied 41 children with acute bacterial meningitis, using clinical examination, computed tomography of the head, electroencephalography, brain-stem auditory evoked response, and visual evoked potential mapping during the acute illness. Two children died; 32 of the remaining 39 children were reviewed clinically, electrophysiologically, and with computed tomography between 5 and 38 months after the illness. The electrophysiologic data obtained during the illness were not found to alter the acute-stage management. Focal or generalized suppression, demonstrated on the electroencephalogram, was associated with a poor outcome. Cerebral infarction and edema, demonstrated by computed tomography of the head, were predictive of a poor outcome, but enlarged ventricular and subarachnoid spaces and increased subdural effusions were of no predictive value. Neither computed tomographic scans nor electrophysiologic data were better indicators of neurologic prognosis than the clinical examination.

This study deals with the development of a method to identify typical food dishes, successfully applied in the State of Sonora, Mexico. The State was divided into six regions considering the size of the territory and existence of different physical, economic and sociocultural conditions, and how they influence the consumption patterns. A list of 66 regional food dishes was prepared by interviewing housewives from different parts of Sonora who now lived in the city of Hermosillo. From this list, 47 dishes were identified as regionally consumed, and a State-wide survey was performed to determine their frequency of consumption. Based on the categories of "very frequently", "frequently" and "less frequently", a group of 15 dishes (2 per region, plus 3 dishes throughout the State) was selected. A questionnaire was used to obtain the different ways they had of preparing them. The "typical recipe" for each dish was also determined, using the mode as statistical parameter. Samples of these food dishes were prepared at the laboratory following the selected home recipes for proximal analysis and determination of their vitamin A, thiamine, riboflavin, niacin, calcium, phosphorus and iron content. The results showed that a portion of "gallina pinta" contributed more than 25% of the daily recommended intake established by the National Institute of Nutrition "Salvador Zubirán" (INNSZ) for an adult man for energy, protein, iron, niacin and 24.3% thiamine and 21.0% calcium. The "chivichangas de queso", for example, contributed 45.2% of the calcium requirement, and the "tamales de carne", 36% iron.(ABSTRACT TRUNCATED AT 250 WORDS)

This research project in urban marginal areas of the City of Hermosillo in Sonora, Mexico, analyzes certain socioeconomic factors taking into consideration the relationship between household income, expense and consumption of food. The study is based on information collected in a 24-hour recall survey carried out for all members of the family, frequency of consumption of foods, and a socioeconomic questionnaire. Expense and consumption of foods was divided into five different income groups and into three classes of foods: basic foods, fresh meat and high-protein foods. The results of the research study show a direct relationship between household income level and expense and consumption of the foods. The income-elasticity of the demand of basic foods was lower than that estimated for high-protein foods, and there was a marked tendency to increase consumption of high-protein foods as family income increased. More than 50% of the calories and proteins were obtained from the basic foods group, even though a large percentage of family food expense was destined for high-protein foods. The study concludes that more research is needed at a macroeconomic level to understand the basic underlying causes of the nutritional problems.

Serial cultures were obtained from 12 patients being treated with a form of penicillin for group B streptococcal disease or asymptomatic colonization. These cultures were collected prior to initiation of therapy, during therapy, and at the end of the treatment period. Pencillin eradicated GBS from the involved system (blood, CSF, bone, and joint). Cultures remained positive from at least one of three carrier sites (throat, umbilicus, and rectum) in 70% of patients cultured between days one and eight of treatment and 66% between days 10 and 21. These data demonstrate the failure of penicillin to eradicate GBS from mucous surfaces of young infants.
