ABSTRACT Purpose: This study aims to evaluate the safety and efficacy of ultrasound-guided balloon dilation compared to non-balloon dilation for percutaneous nephrolithotomy (PCNL). Materials and methods: A systematic review and meta-analysis were conducted by searching PubMed, EMBASE, and the Cochrane Library. Results were filtered using predefined inclusion and exclusion criteria as described and meta-analysis was performed using Review Manager 5.4 software. Results: A total of six studies involving 1189 patients who underwent PCNL were included. The meta-analysis results demonstrated that compared to non-balloon dilation, balloon dilation was associated with reduced haemoglobin drop [mean difference (MD) = -0.26, 95% CI = -0.40 ~ -0.12, P = 0.0002], decreased transfusion rate [odds ratio (OR) = 0.47, 95% CI = 0.24 ~ 0.92, P = 0.03], shorter tract establishment time (MD = -1.30, 95% CI = -1.87 ~ -0.72, P < 0.0001) and shorter operation time (MD = -5.23, 95% CI = -10.19 ~ -0.27, P = 0.04). Conclusions: Overall, ultrasound-guided balloon dilatation offered several advantages in PCNL procedures. It facilitated faster access establishment, as evidenced by shorter access creation time. Additionally, it reduced the risk of kidney injury by minimizing postoperative haemoglobin drop and decreasing the need for transfusions. Moreover, it enhanced the efficiency of surgery by reducing the operation time. However, it is important to note that the quality of some included studies was subpar, as they did not adequately control for confounding factors that may affect the outcomes. Therefore, further research is necessary to validate and strengthen these findings.

BACKGROUND: Outbreaks of monkeypox have been ongoing in non-endemic countries since May 2022. A thorough assessment of its global zoonotic niche and potential transmission risk is lacking. METHODS: We established an integrated database on global monkeypox virus (MPXV) occurrence during 1958 - 2022. Phylogenetic analysis was performed to examine the evolution of MPXV and effective reproductive number (Rt) was estimated over time to examine the dynamic of MPXV transmissibility. The potential ecological drivers of zoonotic transmission and inter-regional transmission risks of MPXV were examined. RESULTS: As of 24 July 2022, a total of 49 432 human patients with MPXV infections have been reported in 78 countries. Based on 525 whole genome sequences, two main clades of MPXV were formed, of which Congo Basin clade has a higher transmissibility than West African clade before the 2022-monkeypox, estimated by the overall Rt (0.81 vs. 0.56), and the latter significantly increased in the recent decade. Rt of 2022-monkeypox varied from 1.14 to 4.24 among the 15 continuously epidemic countries outside Africa, with the top three as Peru (4.24, 95% CI: 2.89-6.71), Brazil (3.45, 95% CI: 1.62-7.00) and the United States (2.44, 95% CI: 1.62-3.60). The zoonotic niche of MPXV was associated with the distributions of Graphiurus lorraineus and Graphiurus crassicaudatus, the richness of Rodentia, and four ecoclimatic indicators. Besides endemic areas in Africa, more areas of South America, the Caribbean States, and Southeast and South Asia are ecologically suitable for the occurrence of MPXV once the virus has invaded. Most of Western Europe has a high-imported risk of monkeypox from Western Africa, whereas France and the United Kingdom have a potential imported risk of Congo Basin clade MPXV from Central Africa. Eleven of the top 15 countries with a high risk of MPXV importation from the main countries of 2022-monkeypox outbreaks are located at Europe with the highest risk in Italy, Ireland and Poland. CONCLUSIONS: The suitable ecological niche for MPXV is not limited to Africa, and the transmissibility of MPXV was significantly increased during the 2022-monkeypox outbreaks. The imported risk is higher in Europe, both from endemic areas and currently epidemic countries. Future surveillance and targeted intervention programs are needed in its high-risk areas informed by updated prediction.

Our world needs to develop clean energy to reach the target of carbon peak and carbon neutralization. As one of clean energy, wind energy should contribute to energy conservation and emission reduction. Wind power generation is an important field of wind energy application. However, the fluctuation and intermittency of wind can affect the safety of power system. Therefore, prediction of wind power accurately for wind power safety, dispatching, and power grid development is significant. This paper proposes a prediction model of wind power, and predicts the wind power of two wind farms. For the complex wind speed series, the variational modal decomposition (VMD) method is used to reduce its volatility before prediction. And this paper presents an improved method to improve the prediction efficiency when least square support vector machine (LSSVM) predicts stationary series. The prediction result shows that the proposed model improves the prediction of wind power effectively, provides an effective method for wind farm to predict the wind power, and makes contributions to reducing carbon emissions and environmental protection.

BACKGROUND: Developmental cardiac tissue holds remarkable capacity to regenerate after injury and consists of regenerative mononuclear diploid cardiomyocytes. On maturation, mononuclear diploid cardiomyocytes become binucleated or polyploid and exit the cell cycle. Cardiomyocyte metabolism undergoes a profound shift that coincides with cessation of regeneration in the postnatal heart. However, whether reprogramming metabolism promotes persistence of regenerative mononuclear diploid cardiomyocytes enhancing cardiac function and repair after injury is unknown. Here, we identify a novel role for RNA-binding protein LIN28a, a master regulator of cellular metabolism in cardiac repair after injury. METHODS: LIN28a overexpression was tested using mouse transgenesis on postnatal cardiomyocyte numbers, cell cycle, and response to apical resection injury. With the use of neonatal and adult cell culture systems and adult and Mosaic Analysis with Double Markers myocardial injury models in mice, the effect of LIN28a overexpression on cardiomyocyte cell cycle and metabolism was tested. Last, isolated adult cardiomyocytes from LIN28a and wild-type mice 4 days after myocardial injury were used for RNA-immunoprecipitation sequencing. RESULTS: LIN28a was found to be active primarily during cardiac development and rapidly decreases after birth. LIN28a reintroduction at postnatal day (P) 1, P3, P5, and P7 decreased maturation-associated polyploidization, nucleation, and cell size, enhancing cardiomyocyte cell cycle activity in LIN28a transgenic pups compared with wild-type littermates. Moreover, LIN28a overexpression extended cardiomyocyte cell cycle activity beyond P7 concurrent with increased cardiac function 30 days after apical resection. In the adult heart, LIN28a overexpression attenuated cardiomyocyte apoptosis, enhanced cell cycle activity, cardiac function, and survival in mice 12 weeks after myocardial infarction compared with wild-type littermate controls. Instead, LIN28a small molecule inhibitor attenuated the proreparative effects of LIN28a on the heart. Neonatal rat ventricular myocytes overexpressing LIN28a mechanistically showed increased glycolysis, ATP production, and levels of metabolic enzymes compared with control. LIN28a immunoprecipitation followed by RNA-immunoprecipitation sequencing in cardiomyocytes isolated from LIN28a-overexpressing hearts after injury identified long noncoding RNA-H19 as its most significantly altered target. Ablation of long noncoding RNA-H19 blunted LIN28a-induced enhancement on cardiomyocyte metabolism and cell cycle activity. CONCLUSIONS: Collectively, LIN28a reprograms cardiomyocyte metabolism and promotes persistence of mononuclear diploid cardiomyocytes in the injured heart, enhancing proreparative processes, thereby linking cardiomyocyte metabolism to regulation of ploidy/nucleation and repair in the heart.

ABSTRACT Introduction: Youngsters are an important source of reserve strength among short-speed skaters, so it is necessary to perform special strength training for young lower limb muscles while improving athletes' endurance and physical function. Objective: Study the systematic development of physical training of speed skating athletes. Methods: The author uses the method of experimentation and mathematical statistics, the first level male athletes were selected as experimental objects, and the double-blind test method was used to conduct a comparative study through experiments. Results: Statistically, the T-test considerations of the control and experimental groups in the unipodal take-off stand and bilateral sliding jump tests showed statistical differences (P>0.05). The comparison of the experimental group pre and post-intervention indicates that the experimental group's performance in this test item increased significantly after training(P<0.05). Conclusion: The adaptation of the entry-level trainers to the scientific training concept played an auxiliary role in the results by increasing the emphasis on small muscle group training. RESUMO Introdução: Os jovens são uma importante fonte de força de reserva entre os patinadores de velocidade curta, portanto é necessário realizar um treinamento de força especial para os jovens músculos dos membros inferiores, melhorando ao mesmo tempo a resistência e a função física dos atletas. Objetivo: Estudar o desenvolvimento sistemático do treinamento físico dos atletas de patinação de velocidade. Métodos: O autor utiliza o método de experimentação e estatística matemática, foram selecionados como objetos experimentais os atletas do primeiro nível masculino, e o método de teste duplo-cego foi utilizado para conduzir um estudo comparativo através de experimentos. Resultados: Estatisticamente, as considerações do teste T do grupo de controle e do grupo experimental no suporte de decolagem unipodal e teste bilateral de salto deslizante apresentaram diferença estatística (P>0,05). A comparação do grupo experimental pré e pós intervenção indica que o desempenho do grupo experimental neste item de teste aumentou significativamente após o treinamento(P<0,05). Conclusão: A adaptação dos treinadores de nível básico para o conceito de treinamento científico desempenhou um papel auxiliar nos resultados, aumentando a ênfase no treinamento dos pequenos grupos musculares. Nível de evidência II; Estudos terapêuticos - investigação dos resultados do tratamento.


RESUMEN Introducción: Los jóvenes son una importante fuente de reserva de fuerza entre los patinadores de velocidad corta, por lo que es necesario realizar un entrenamiento de fuerza especial para los músculos de las extremidades inferiores de los jóvenes, mejorando al mismo tiempo la resistencia y la función física de los atletas. Objetivo: Estudiar el desarrollo sistemático de la preparación física de los atletas de patinaje de velocidad. Métodos: El autor utiliza el método de la experimentación y la estadística matemática, se seleccionaron atletas masculinos de primer nivel como objetos experimentales, y se utilizó el método de prueba de doble ciego para realizar un estudio comparativo a través de experimentos. Resultados: Estadísticamente, las consideraciones de la prueba T del grupo de control y del grupo experimental en la prueba de despegue unipodal y de salto deslizante bilateral mostraron diferencias estadísticas (P>0,05). La comparación del grupo experimental antes y después de la intervención indica que el rendimiento del grupo experimental en este ítem de la prueba aumentó significativamente después del entrenamiento (P<0,05). Conclusión: La adaptación de los entrenadores de nivel inicial al concepto de entrenamiento científico desempeñó un papel auxiliar en los resultados al aumentar el énfasis en el entrenamiento de los grupos musculares pequeños. Abstract Background Osteoarthritis (OA) is one of the most frequent chronic diseases with high morbidity worldwide, marked by degradation of the cartilage and bone, joint instability, stiffness, joint space stenosis and subchondral sclerosis. Due to the elusive mechanism of osteoarthritis (OA), we aimed to identify potential markers for OA and explore the molecular mechanisms underlying OA. Methods Expression profiles data of OA were collected from the Gene Expression Omnibus database to identify differentially expressed mRNAs (DEmRNAs) and differentially expressed lncRNAs (DElncRNAs) in OA. Functional annotation and protein-protein interaction (PPI) networks were performed. Then, nearby DEmRNAs of DElncRNAs was obtained. Moreover, GO and KEGG pathway enrichment analysis of nearby DEmRNAs of DElncRNAs was performed. Finally, expression validation of selected mRNAs and lncRNAs was performed by quantitative reverse transcriptase-polymerase chain reaction. Results In total, 2080 DEmRNAs and 664 DElncRNAs were determined in OA. PI3K-Akt signaling pathway, Endocytosis and Rap1 signaling pathway were significantly enriched KEGG pathways in OA. YWHAB, HSPA8, NEDD4L and SH3KBP1 were four hub proteins in PPI network. The AC093484.4/TRPV2 interact pair may be involved in the occurrence and development of OA. Conclusion Our study identified several DEmRNAs and DElncRNAs associated with OA. The molecular characters could provide more information for further study on OA.

Triptolide is a natural active compound that has significant neuroprotective properties and shows promising effects in the treatment of Alzheimer's disease (AD). Recent studies have shown that autophagy occurs in AD. In this study, we determined whether autophagy regulated by triptolide ameliorates neuronal death caused by amyloid-Beta1-42 (Aß1-42). We examined the effects of triptolide on cell viability, autophagy, apoptosis, and the protein kinase B/mammalian target of the rapamysin/70 kDa ribosomal protein S6 kinase (Akt/mTOR/p70S6K) signaling pathway in PC12 cells. The results indicated that triptolide treatment exhibited a cytoprotective effect against cell injury induced by Aß1-42. Triptolide also reduced apoptosis and enhanced cell survival by decreasing autophagosome accumulation and inducing autophagic degradation. Furthermore, our results also showed that activating the Akt/mTOR/p70S6K mechanism was one reason for the protection of triptolide. Triptolide treatment protected against Aß1-42-induced cytotoxicity by decreasing autophagosome accumulation, and inducing autophagic degradation in PC12 cells. These findings also suggest that the reduction of autophagosome accumulation observed in triptolide-treated cells was Akt/mTOR/p70S6K pathway dependent. Overall, triptolide exhibits a neuron protective effect and this study provides new insight into AD prevention and treatment.

The expansion of rubber (Hevea brasiliensis) cultivation plantation over the past few decades has been significantly explosive in Xishuangbanna, southwest China. More and more evidences concerning the expansion of rubber plantations lead to the negative influence to local regional hydrology. It is vital to explore the impact of climate change and rubber (Hevea brasiliensis) plantation expansion on reference evapotranspiration (ET0) for the sustainable and efficient use of regional water resources. In this study, the spatiotemporal variation of ET0 as well as its relationship in rubber plantations area in Xishuangbanna from 1970-2017 were analyzed by using trend, correlation and contribution analysis. The results showed that the rubber plantation was 12,768 ha yr-1 from 1990 to 2017 in Xishuangbanna, and nearly 40.8% of new rubber plantations expanded above 900 m in altitude from 2000 to 2017. Sunshine duration and average relative humidity were the key meteorological factors that affect ET0 in Xishuangbanna, with the sensitivity coefficient of 0.51 and 0.35, respectively. The multiyear relative change of ET0 in Xishuangbanna was 9.18%, and the total contribution of major climate factors was 7.87% during 1970 and 2017. The average relative humidity in the plantation area decreases, which directly leads to the increase of ET0. The amount of ET0 change from 2000 to 2017 affected by climate change increased at 3.13 mm/10a, whereas it was 2.17 mm/10a affected by the expansion of rubber plantations by quantitative separation. ET0 was significantly affected by climate change but intensified by the expansion of rubber plantation.

Objective: This study evaluated the effect of leucocyte and platelet-rich fibrin (L-PRF) techniques used in alveolar ridge preservation on dimensional hard tissue changes of the alveolar ridge assessed using cone-beam computed tomography (CBCT) scans. Material and Methods: Elective alveolar ridge preservation surgeries were performed on seven systemically healthy patients who underwent single posterior tooth extraction. Patients were randomly treated with PRF mixed with a commercial bone xenograft (n = 3) or PRF plug (n = 4). CBCT scans were taken immediately after tooth extraction and then four months later, just before implant surgery. Dimensional alterations in socket height (SH), socket area (SA) and socket volume (SV) were evaluated on CBCT scans by percentage differences before and after treatments. Results: Sockets treated with PRF plus bone xenograft had a tendency to maintain alveolar bone dimensions over time (SH= 11.22% to 82.74%, SA= 1.84% to 48.91% and SV= 9.11% to 203.62%), while sockets treated only with PRF plug technique presented only a modest increase in height (SH= 1.47% to 11.11%) but greater alveolar ridge resorption confirmed by a decrease in socket area and volume dimensions (SA= 0.21% to -24.09% SV= 8.53% to -54.12%). Conclusion: Within the limitations of this study, the maintenance of alveolar socket dimensions was observed when PRF was associated with a xenograft. However, the loss of alveolar ridge dimensions was not entirely prevented by PRF treatment alone.(AU)


Objetivo: Este estudo avaliou o efeito das técnicas de fibrina rica em leucócitos e plaquetas (L-PRF) usadas na preservação da crista alveolar em alterações dimensionais de tecido duro da crista alveolar avaliadas por tomografia computadorizada de feixe cônico (CBCT). Material e Métodos: Foram realizadas cirurgias eletivas de preservação da crista alveolar em sete pacientes sistemicamente saudáveis que foram submetidos a uma única extração dentária posterior. Os doentes foram tratados aleatoriamente com PRF misturado com um xenoenxerto ósseo comercial (n = 3) ou com PRF plug (n = 4). CBCT foi realizada imediatamente após a extração dentária e quatro meses depois, imediatamente antes da cirurgia de implante. Alterações dimensionais na altura do alvéolo (SH), área do alvéolo (SA) e volume do alvéolo (SV) foram avaliadas em exames CBCT por diferenças percentuais antes e depois dos tratamentos. Resultados: Os alvéolos tratados com PRF mais xenoenxerto ósseo apresentaram tendência a manter as dimensões ósseas alveolares ao longo do tempo (SH= 11.22% a 82.74%, SA= 1.84% a 48.91% e SV= 9.11% a 203.62%), enquanto alvéolos tratados apenas com a técnica de PRF plug apresentaram apenas um aumento modesto na altura (SH= 1.47% a 11.11%), mas uma maior reabsorção alveolar confirmada pela redução das dimensões de área e volume do alvéolo (SA= 0.21% a -24.09% SV= 8.53% a -54.12%). Conclusão: Dentro das limitações deste estudo, a manutenção das dimensões alveolares foi observada quando o PRF foi associado ao xenoenxerto. No entanto, a perda das dimensões alveolares não foi totalmente evitada pelo tratamento apenas com PRF. (AU)

Variation in the gastrointestinal (GI) microbiota after hematopoietic cell transplantation (HCT) has been associated with acute graft-versus-host disease (aGVHD). Because antibiotics induce dysbiosis, we examined the association of broad-spectrum antibiotics with subsequent aGVHD risk in pediatric patients undergoing HCT for acute leukemia. We performed a retrospective analysis in a dataset merged from 2 sources: (1) the Center for International Blood and Marrow Transplant Research, an observational transplantation registry, and (2) the Pediatric Health Information Services, an administrative database from freestanding children's hospitals. We captured exposure to 3 classes of antibiotics used for empiric treatment of febrile neutropenia: (1) broad-spectrum cephalosporins, (2) antipseudomonal penicillins, and (3) carbapenems. The primary outcome was grade II-IV aGVHD; secondary outcomes were grade III-IV aGVHD and lower GI GVHD. The adjusted logistic regression model (full cohort) and time-to-event analysis (subcohort) included transplantation characteristics, GVHD risk factors, and adjunctive antibiotic exposures as covariates. The full cohort included 2550 patients at 36 centers; the subcohort included 1174 patients. In adjusted models, carbapenems were associated with an increased risk of grade II-IV aGVHD in the full cohort (adjusted odds ratio [aOR], 1.24; 95% confidence interval [CI], 1.02 to 1.51) and subcohort (sub hazard ratio [HR], 1.31; 95% CI, 0.99 to 1.72), as well as with an increased risk of grade III-IV aGVHD (subHR, 1.77; 95% CI, 1.25 to 2.52). Early carbapenem exposure (before day 0) especially impacted aGVHD risk. For antipseudomonal penicillins, the associations with aGVHD were in the direction of increased risk but were not statistically significant. There was no identified association between broad-spectrum cephalosporins and aGVHD. Carbapenems, more than other broad-spectrum antibiotics, should be used judiciously in pediatric HCT recipients to minimize aGVHD risk. Further research is needed to clarify the mechanism underlying this association.

The prediction of the potential distribution of invasive alien species is key for the control of their proliferation. This study developed ensemble niche models to explore the distribution patterns of Cecropia peltata and Ulex europaeus under baseline and future conditions, as well as the factors that regulate them. The models were based on occurrence records as well as climate, land-use and topography datasets. Climatic factors played a stronger role than land-use and topographical factors in their distribution patterns. Additionally, temperature seasonality and temperature annual range were the optimal predictor for the global distributions of C. peltata and U. europaeus, respectively. Under the baseline-RCP 8.5 scenario in 2070, significant increases in habitat suitability for C. peltata were generally detected in tropical regions, while for U. europaeus under the same condition, significant increases in habitat suitability were generally observed in west coast of South America and Europe, suggesting the impacts of climate changes on species distribution may be species specific. The contrast changes of suitable habitat areas for U. europaeus under the baseline-2.6 and 8.5 scenarios may suggest that the scenarios of climate changes may modify its distribution patterns and variations in suitable habitats. The double-edged effects of global warming on plant invasions may be a result of the scenario specific climate change and the species-specific responses to changes in climate. Our findings highlight the importance of climate change scenario specific and species-specific research on the impact of climate change on plant invasions.

Circulating miRNAs presenting in plasma in a stable manner have been demonstrated their potential role as a promising biomarkers in many human diseases, such as Alzheimer's disease, melanoma and ovarian carcinoma. However, few circulating miRNAs could be used for breast ductal cancer diagnosis. Here, we identified miR-1273g-3p as a biomarker for detecting breast ductal cancer. We detected miR-1273g-3p levels in the plasma of 39 sporadic breast ductal cancer patients and 40 healthy donors by Stem-loop Quantitative Real-time PCR (qRT-PCR). The results showed the plasma miR-1273g-3p level were significantly up-regulated in breast ductal cancer patients compared with healthy donors (p=0.0139). Receiver operating characteristic (ROC) curve also revealed the significantly diagnostic ability of miR-1273g-3p in patients (p=0.0414). In addition, the plasma level of miR-1273g-3p was closely related to IIIB-IIIC TNM stage. We also confirmed the higher expression level of miR-1273g-3p in breast cancer cell lines MCF-7 (4.872±0.537) than normal breast cells (Hs 578Bst). Taken together, miR-1273g-3p could represent as a potential biomarker for early breast ductal cancer diagnosis.

Multiple models of donor killer immunoglobulin receptor (KIR) alloreactivity or KIR genotype have been reported to be protective against leukemia relapse after allogeneic transplantation. However, few studies have addressed this topic in the pediatric population. Here, we assessed the outcomes of allogeneic transplantation in children with acute lymphoblastic leukemia (ALL; n = 372) or acute myeloid leukemia (AML; n = 344) who received unrelated donor (URD) transplantation and were reported to the Center for International Blood and Marrow Transplant Research (CIBMTR) from 2005 to 2016. As expected in this pediatric population, most patients underwent myeloablative conditioning while in remission and with bone marrow as a stem cell source. We tested KIR ligand mismatch, KIR gene content (centromeric [Cen] B), KIR2DS1 mismatching, and Cen B/telomeric A using Cox regression models and found that none were significantly associated with either relapse or disease-free survival when considering the entire cohort of patients (ALL and AML), AML, or ALL separately. Moreover, there was no significant association with outcomes in the in vivo T-cell-depleted (ie, serotherapy) cohort. This study, which is the largest analysis of donor KIR in the pediatric acute leukemia population, does not support the use of KIR in the selection of URDs for children undergoing T-replete transplantation.

Relative to the rich library of small-molecule organics, few examples of ordered extended (i.e., nonmolecular) hydrocarbon networks are known. In particular, sp3 bonded, diamond-like materials represent appealing targets because of their desirable mechanical, thermal, and optical properties. While many covalent organic frameworks (COFs)-extended, covalently bonded, and porous structures-have been realized through molecular architecture with exceptional control, the design and synthesis of dense, covalent extended solids has been a longstanding challenge. Here we report the preparation of a sp3-bonded, low-dimensional hydrocarbon synthesized via high-pressure, solid-state diradical polymerization of cubane (C8H8), which is a saturated, but immensely strained, cage-like molecule. Experimental measurements show that the obtained product is crystalline with three-dimensional order that appears to largely preserve the basic structural topology of the cubane molecular precursor and exhibits high hardness (comparable to fused quartz) and thermal stability up to 300 °C. Among the plausible theoretical candidate structures, one-dimensional carbon scaffolds comprising six- and four-membered rings that pack within a pseudosquare lattice provide the best agreement with experimental data. These diamond-like molecular rods with extraordinarily small thickness are among the smallest members in the carbon nanothread family, and calculations indicate one of the stiffest one-dimensional systems known. These results present opportunities for the synthesis of purely sp3-bonded extended solids formed through the strain release of saturated molecules, as opposed to only unsaturated precursors.

RATIONALE: Possible beneficial effects of GDF11 (growth differentiation factor 11) on the normal, diseased, and aging heart have been reported, including reversing aging-induced hypertrophy. These effects have not been well validated. High levels of GDF11 have also been shown to cause cardiac and skeletal muscle wasting. These controversies could be resolved if dose-dependent effects of GDF11 were defined in normal and aged animals as well as in pressure overload-induced pathological hypertrophy. OBJECTIVE: To determine dose-dependent effects of GDF11 on normal hearts and those with pressure overload-induced cardiac hypertrophy. METHODS AND RESULTS: Twelve- to 13-week-old C57BL/6 mice underwent transverse aortic constriction (TAC) surgery. One-week post-TAC, these mice received rGDF11 (recombinant GDF11) at 1 of 3 doses: 0.5, 1.0, or 5.0 mg/kg for up to 14 days. Treatment with GDF11 increased plasma concentrations of GDF11 and p-SMAD2 in the heart. There were no significant differences in the peak pressure gradients across the aortic constriction between treatment groups at 1 week post-TAC. Two weeks of GDF11 treatment caused dose-dependent decreases in cardiac hypertrophy as measured by heart weight/tibia length ratio, myocyte cross-sectional area, and left ventricular mass. GDF11 improved cardiac pump function while preventing TAC-induced ventricular dilation and caused a dose-dependent decrease in interstitial fibrosis (in vivo), despite increasing markers of fibroblast activation and myofibroblast transdifferentiation (in vitro). Treatment with the highest dose (5.0 mg/kg) of GDF11 caused severe body weight loss, with significant decreases in both muscle and organ weights and death in both sham and TAC mice. CONCLUSIONS: Although GDF11 treatment can reduce pathological cardiac hypertrophy and associated fibrosis while improving cardiac pump function in pressure overload, high doses of GDF11 cause severe cachexia and death. Use of GDF11 as a therapy could have potentially devastating actions on the heart and other tissues.

ABSTRACT Objective: To report our experience of retroperitoneoscopic technique in semi-lateral decubitus position for the retroperitoneal nonadrenal ganglioneuromas in 18 patients, and to evaluate its clinical outcomes. Materials and Methods: From January 2012 to May 2016, 18 patients with retroperitoneal nonadrenal ganglioneuromas underwent retroperitoneoscopic resection. With the patients in semi-lateral decubitus position, a 4-port retroperitoneal approach was used. Data were collected on the tumor size, tumor location, perioperative outcomes, pathology, and last-known disease status. We reviewed the operative videos to identify surgical tips and tricks. Results: All procedures were carried out successfully without converting to open surgery. The tumors had an average size of 5.2cm. The mean operative time was 86.5 min, with a mean estimated blood loss of 85.4mL. There were three patients suffering from intraoperative complications. Postoperatively, all patients achieved an uneventful recovery; the mean postoperative hospital stay was 5.5 days. The postoperative pathology revealed to be retroperitoneal ganglioneuromas. With a mean follow-up of 39.5 months, all patients were recurrence free. The review of the operative videos revealed several tips and tricks, including keeping peritoneum and posterior Gerota fascia intact to provide a favorable operative exposure of tumors, and placing the harmonic scalpel through different ports during tumor dissection. Conclusions: With the patient in semi-lateral decubitus position and a 4-port retroperitoneal approach, retroperitoneoscopic resection of retroperitoneal nonadrenal ganglioneuroma is a feasible, effective, and safe procedure. This approach has distinct advantages including direct access to the tumor, optimal exposure of tumor and less intraperitoneal interference.
