INTRODUCTION: Nocardiosis is a rare bacterial infection caused by Nocardia spp. However, an increasing incidence has been described whereby data about epidemiology and prognosis are essential. METHODS: A retrospective descriptive study was conducted among patients with positive Nocardia spp. culture, from January 2019 to January 2023, at a Terciary Hospital in Portugal. RESULTS: Nocardiosis was considered in 18 cases with a median age of 63.8-years-old. At least one immunosuppressive cause was identified in 70% of patients. Five patients had Disseminated Nocardiosis (DN). The lung was the most common site of clinical disease (77.8%) and Nocardia was most commonly identified in respiratory tract samples. The most frequently isolated species were Nocardia nova/africana (n = 7) followed by Nocardia cyriacigeorgica (n = 3) and Nocardia pseudobrasiliensis (n = 3). The majority of the patients (94.4%) received antibiotic therapy, of whom as many as 55.6% were treated with monotherapy. The most frequently prescribed antibiotic was trimethoprim-sulfamethoxazole. Selected antimicrobial agents were generally effective, with linezolid and cotrimoxazole (100% Susceptibility [S]) and amikacin (94% S) having the most activity against Nocardia species. The median (IQR) duration of treatment was 24.2 (1‒51.4) weeks for DN; The overall one-year case fatality was 33.3% (n = 6) and was higher in the DN (66.7%). No recurrence was observed. CONCLUSION: Nocardiosis is an emerging infectious disease with a poor prognosis, particularly in DN. This review offers essential epidemiological insights and underscores the importance of gaining a better understanding of the microbiology of nocardiosis. Such knowledge can lead to the optimization of antimicrobial therapy and, when necessary, guide appropriate surgical interventions to prevent unfavorable outcomes.

Reserva Ecológica de Guapiaçu (REGUA) is a private, protected area inside a remnant of the Atlantic Forest with high biodiversity. Although the bats of the reserve have been sampled for more than two decades, few studies have been published about them. Based on a compilation of data from several surveys, we present an updated list of the bat species there and compare it with surveys from other locations in the Atlantic Forest. From August 1998 to September 2021, at least 194 sampling nights were carried out at REGUA by different research groups from various institutions, totaling 448,092 m2.h of sampling. A total of 4,069 individuals were captured, belonging to 47 species and six families. Additionally, our results indicate that it is possible that some species that occur at REGUA have not been recorded yet. REGUA has the greatest number of bat species known for the Atlantic Forest. This most likely results from the fact that the reserve includes large areas of mature, continuous forest connected with other protected areas in the State of Rio de Janeiro. Another factor contributing to the high diversity of bat species at REGUA is that the area has been intensely sampled for many years. Given that the bat assemblage there appears to be a good proxy to ascertain the ecological patterns of biodiversity in well-preserved forests, we consider REGUA to be an important area for long-term ecological research. The basic knowledge about the ecological interactions of bats with different food resources and zoonotic microorganisms offers a unique opportunity to carry out research in several areas of knowledge, making it possible to address questions about bat assemblage structure, bat-parasite ecology, competition, niche partitioning, and other related studies.

OBJECTIVE: To report the experience of the multidisciplinary health team in the construction and implementation of the assistance protocol for pets visiting patients admitted to a palliative care unit. METHOD: This is an experience report about the construction and implementation of a protocol in a university hospital in the South of Brazil by a working group from the multidisciplinary team of the Palliative Care Program and composed of three nurses, a physician, a psychologist, a social worker, a manager, and a nutritionist. The period of construction and implementation of the protocol was from September 2017 to April 2019. RESULTS: The construction of the protocol by the multidisciplinary team followed three stages: planning, execution, and implementation, which enabled the release of visits of patients' pets in palliative care. CONCLUSION: The construction of the protocol allowed for the institutionalization of the visit of patients' pets in palliative care in a university hospital, bringing benefits to patients and their families.

RESUMEN Objetivo Informar la experiencia del equipo de salud multiprofesional en la construcción e implementación de un protocolo de atención para visita de mascotas, para los pacientes internados en una unidad de cuidados paliativos. Método Se trata de un informe de experiencia sobre la construcción e implementación de un protocolo en un hospital universitario en el sur de Brasil, realizado por un grupo de trabajo originado a partir del equipo multidisciplinario del Programa de Cuidados Paliativos y compuesto por tres enfermeras, un médico, un psicólogo, un trabajador social, un administrador y un nutricionista. El período de construcción e implementación del protocolo abarca desde septiembre de 2017 hasta abril de 2019. Resultados La construcción del protocolo por la experiencia del equipo multiprofesional siguió tres etapas: planificación, ejecución e implementación, que permitieron la habilitación de las visitas de mascotas de pacientes en cuidados paliativos. Conclusión La construcción del protocolo permitió institucionalizar la visita de mascotas de pacientes en cuidados paliativos en un hospital universitario, brindando beneficios tanto a los pacientes como a sus familias.


RESUMO Objetivo Relatar a experiência da equipe multiprofissional em saúde na construção e implantação do protocolo assistencial para visita dos animais de estimação dos pacientes internados em uma Unidade de Cuidados Paliativos. Método Trata-se de um relato de experiência de uma ação realizada em um hospital universitário do sul do Brasil por um grupo de trabalho originado da equipe multiprofissional do Programa de Cuidados Paliativos, composto por três enfermeiros, um médico, um psicólogo, um assistente social, um administrador e uma nutricionista. O período de construção e implementação do protocolo foi de setembro de 2017 a abril de 2019. Resultados: A construção do protocolo pela equipe multiprofissional seguiu três etapas, planejamento, execução e implementação, e resultou na liberação para visita dos animais de estimação dos pacientes em cuidados paliativos. Conclusão A construção do protocolo permitiu a institucionalização da visita dos animais de estimação dos pacientes em cuidados paliativos em um hospital universitário, trazendo benefícios para os pacientes e seus familiares.

RESUMO

OBJETIVO: Avaliar o efeito de um programa de treinamento de força muscular progressiva e sensório-motor, no equilíbrio e na redução do risco de quedas em mulheres com osteoporose. MÉTODO: Foram selecionadas 82 mulheres sedentárias com idade entre 60 e 75 anos com osteoporose, randomizadas em dois grupos: grupo intervenção, composto por 41 pacientes submetidas a 18 semanas de treinamento de força muscular progressiva do quadríceps (a 50%, 60%, 70% até 80% de 1-RM - uma repetição máxima), e de propriocepção, associado ao tratamento clínico medicamentoso para osteoporose; e grupo controle, também composto por 41 pacientes que receberam apenas o tratamento clínico medicamentoso. O equilíbrio e o risco de quedas foram avaliados por meio da Escala de Equilíbrio de Berg (BBS) e do cálculo do Índice de Quedas (IQ). O número de quedas foi avaliado seis meses antes do início da pesquisa e nos seis meses seguintes. RESULTADOS: 69 pacientes concluíram a pesquisa. O programa promoveu melhora no equilíbrio funcional (p<0,001) demonstrado pela BBS, redução do risco de quedas calculado pelo escore geral do IQ (p<0,004) e ainda uma redução do número total de quedas, que no grupo experimental foram significativamente menores que no grupo controle (p<0,001). CONCLUSÃO: A associação do treinamento de força progressiva para quadríceps e propriocepção é eficaz na prevenção de quedas e redução de seus fatores de risco, e o modelo para previsão quantitativa do risco de quedas pode ser útil na identificação do risco de quedas em pessoas idosas. OBJECTIVE: This study aims to evaluate the effect of progressive muscular strength and proprioception training program on the balance and decrease in the risk of falls in women with osteoporosis. METHOD: 82 sedentary women aged 60-75 years with osteoporosis were selected and randomized in two groups: the intervention group comprised 41 patients submitted to an 18-week progressive muscular strength and proprioception training of quadriceps associated to drug therapy for osteoporosis; and control group, formed by 41 patients submitted to drug therapy only. The balance and fall risk were evaluated using the Balance Berg Scale (BBS) and calculating the Fall Index (FI). The number of fall was assessed six months before the research and the following six months. RESULTS: 69 patients completed the research. The program improved the functional balance (p<0.001) shown by BBS, decreased the risk of falls evidenced by the general score of FI (p<0.004) and also a decrease in the total number of falls. The experimental group showed significant lower number of falls when compared to the control group (p<0.001). CONCLUSION: The association of progressive strength of quadriceps and proprioceptive training is effective to prevent falls and reduce risk factors. The quantitative model to predict risk of falls might be useful to identify the risk of falls in the elderly. Trial registration: 082213-ANZCTR.
