RESUMO

We describe 14 cases of pilomatrixoma which were diagnosed preoperatively on FNA cytology. In contrast to the literature, our series showed male preponderance and some of the tumours in unusual locations such as thigh and breast. In three patients the tumours were larger than 5 cm. The clinical diagnosis varied from tumour not otherwise defined to sarcoma. The cytologic presentation had features which allowed a correct diagnosis in all cases and included basaloid cells surrounded by delicate pink fibres, shadow cells, giant cells, naked nuclei and calcium deposits. It is concluded that the FNA cytology of pilomatrixoma is characteristic and will allow a conclusive diagnosis even in cases with an aberrant clinical presentation.

RESUMO

This study presents eight cases of fibromatosis colli accurately diagnosed by FNA cytology. Fibromatosis colli is a rare fibroblastic lesion that manifests in the sternocleidomastoid muscle during the first to second months after birth. The tumours varied in size from 1 to 3 cm and engaged the lower, middle as well as the upper part of the sternocleidomastoid muscle. The FNA cytology smears showed mostly mature fibroblasts in a background of pink granular material and fragments of striated muscle fibres with degeneration. The cytological presentation together with the typical clinical setting will allow a definitive and accurate diagnosis, avoiding surgery.

Leishmania (Viannia) panamensis infected Balb/c mice developed a progressive swelling in the injected footpad that grew to a tumour-like lesion from day 80 onwards. We did not observe any typical ulcera, necrosis or metastasis to other parts of the skin. Neither did we observe any histopathological changes in liver or spleen during the experiment. At the site of injection, we observed progressive changes ranging from a moderate, mixed inflammatory infiltrate with few leishmania amastigotes in the macrophages to an extensive inflammation composed of monomorphic vacuolated macrophages containing large numbers of parasites. A granulomatous pattern with presence of epithelioid cells and a few multinucleated giant cells was observed at the initial phase of the infection. During later stages, focal necrosis with polymorphonuclear neutrophils was seen. Lymph nodes presented granulomatous lesions in the subcapsular area, numerous plasma cells in the medullary cords and macrophages with leishmania organisms in dilated cortical sinuses at the 4th and the 6th months of infection. This Leishmania (Viannia) panamensis infected Balb/c mice seems to be a good model for continued studies of the pathogenesis of cutaneous leishmaniasis and also for drug trials in the development of new therapeutic tools.

RESUMO

The nurse teachers in charge of the Auxiliary Nursing Course at the Heart Institute (InCor-FMUSP) propose simulation as an innovative teaching strategy and describe its application to the Cardiorespiratory System. By doing this, they make their students participate actively in their classes, thus improving the teaching-learning process.

RESUMO

The cytologic findings in fine needle aspirates of a previously treated prostatic cancer metastatic to both breasts in a 65-year-old man are described. The prostatic origin of the poorly differentiated adenocarcinoma cells was demonstrated by identification of cytoplasmic prostate-specific antigen. The clinical importance of a conclusive diagnosis differentiating a primary from a metastatic lesion is discussed; this case illustrates the value of immunocytochemical analysis as an aid to cytomorphologic diagnosis in making such determinations.

A model for pulmonary paracoccidioidomycosis in the hamster is described. The disease was induced by intratracheal inoculation of 1.7 x 10(5) viable yeast forms of P. brasiliensis. Lung histopathology, dissemination lesions and humoral and cellular immune responses were investigated at intervals up to 24 weeks after infection. Humoral immunity was studied by immunodiffusion and complement fixation tests. Cell-mediated immunity was evaluated in vitro by the macrophage migration inhibition test in the presence of phytohaemagglutinin and P. brasiliensis soluble antigen, and in vivo by the paracoccidioidin test. Thirty out of 35 infected animals (85.7%) developed pulmonary paracoccidioidomycosis. Dissemination lesions were observed in regional lymph nodes (82.8%), liver (8.5%) and spleen (5.7%). Lung involvement was mainly around bronchi and vessels. Regional lymph nodes were severely involved from the fourth week on, acquiring a pseudotumoral aspect at later stages. Specific antibodies were detected from the fourth week on, with titres increasing progressively. The cellular immune response to phytohaemagglutinin was intact throughout the experiment and the response to P. brasiliensis antigen was already detectable by the second week and remained positive to the end of the experiment. The skin test became positive from the fourth week on. Inoculation by the intratracheal route represents a highly effective way of infecting hamsters with P. brasiliensis, with the induction of localized disease, good antibody production and intact cell immunity.

As bexigas de 91 autópsias foram estudadas histologicamente para se avaliar a freqüência e a distribuiçäo anatômica de alteraçöes uroteliais proliferativas. Sessenta e sete (74%) dos casos apresentaram pelo menos uma das lesöes estudadas, sendo os ninhos de Brünn a lesäo mais freqüente (60%), seguida de "cistite" glandular (45%) e metaplasia escamosa (24%). Todas as lesöes foram mais freqüentes no trígono vesical. Ninhos de Brünn e "cistite" glandular ocorreram em todas as faixas etárias e indiferentemente nos dois sexos, sendo mais freqüentes acima dos 15 anos de idade, quando ocorreram no sexo masculino. A metaplasia escamosa foi alteraçäo observada só acima dos 15 anos de idade, em 70 a 100% das mulheres e somente em 3 a 12% dos homens. frente à baixa freqüência do carcinoma de bexiga em nosso meio, näo há razäo para considerar estas alteraçöes epiteliais täo ubíquas como lesöes pré-neoplásicas

A elevada taxa de recorrência do câncer de bexiga é parcialmente devida a processo neoplasico multifocal do urotélio, concomitante ou posterior ao diagnóstico da primeira lesäo tumoral. Esta observaçäo justifica o seguimento rigoroso dos pacientes após ressecçäo parcial destas neoplasias. Os autores relatam umc caso de carcinoma "in situ" näo-papilífero de bexiga, detectado pelo exame citológico da urina três anos após ressecçäo de carcinoma papilífero. Depois de discutir este encontro de lesäo urotelial pré-invasiva näo papilífera em paciente de alto rico e apontar a concomitância com hiperplasia nodular depróstata, os autores salientam a eficiência do escrutínio citológico na populaçäo de alto risco para neoplasia vesical
