RESUMO

The aim of this study was to analyze the healing action of low-intensity laser therapy associated with Himatanthus drasticus in wound healing in mice. The study was experimental, analytical, controlled, randomized, and prospective. Twenty Wistar rats were divided into four groups: control with injury, injury + laser, wound + Himatanthus drasticus, and wound + laser + Himatanthus drasticus. The animals underwent surgical skin wounds on their backs, and different treatments were applied based on the group. The healing process was evaluated histologically through the analysis of collagen fibers, neovascularization, and inflammatory reaction. The results showed that low-intensity laser therapy and Himatanthus drasticus treatment improved the healing process in terms of collagen synthesis, decreased inflammatory cell migration, fibroblast proliferation, neovascularization, wound size reduction, and edema reduction. However, the combination of laser and Himatanthus drasticus did not show significant improvement compared to individual treatments. There were no statistical differences in polymorphonuclear cells between the treatment groups. Low-intensity laser therapy and Himatanthus drasticus have demonstrated positive properties in improving the healing process. Further research is needed to better understand their individual and combined effects. The study contributes to the exploration of alternative wound healing methods and encourages further investigation in this field.

B7-H4 (VTCN1), a member of the B7 family, is overexpressed in several types of cancer. Here we investigated the pattern of expression of B7-H4 in salivary gland carcinomas (SGC) and assessed its potential as a prognostic marker and therapeutic target. Immunohistochemistry (IHC) analyses were performed in a cohort of 340 patient tumors, composed of 124 adenoid cystic carcinomas (ACC), 107 salivary duct carcinomas (SDC), 64 acinic cell carcinomas, 36 mucoepidermoid carcinomas (MEC), 9 secretory carcinomas (SC), as well as 20 normal salivary glands (controls). B7-H4 expression was scored and categorized into negative (<5% expression of any intensity), low (5%-70% expression of any intensity or >70% with weak intensity), or high (>70% moderate or strong diffuse intensity). The associations between B7-H4 expression and clinicopathologic characteristics, as well as overall survival, were assessed. Among all tumors, B7-H4 expression was more prevalent in ACC (94%) compared with those of SC (67%), MEC (44%), SDC (32%), and acinic cell carcinomas (0%). Normal salivary gland tissue did not express B7-H4. High expression of B7-H4 was found exclusively in ACC (27%), SDC (11%), and MEC (8%). In SDC, B7-H4 expression was associated with female gender (P = .002) and lack of androgen receptor expression (P = .012). In ACC, B7-H4 expression was significantly associated with solid histology (P < .0001) and minor salivary gland primary (P = .02). High B7-H4 expression was associated with a poorer prognosis in ACC, regardless of clinical stage and histologic subtype. B7-H4 expression was not prognostic in the non-ACC SGC evaluated. Our comparative study revealed distinct patterns of B7-H4 expression according to SGC histology, which has potential therapeutic implications. B7-H4 expression was particularly high in solid ACC and was an independent prognostic marker in this disease but not in the other SGC assessed.

OBJECTIVE: To evaluate and synthesise the factors determining patient safety culture in hospitals. METHODS: The scoping review protocol was based on the criteria of the Joanna Briggs Institute. Eligibility criteria were as follows: (1) empirical study published in a peer-reviewed journal; (2) used methods or tools to assess, study or measure safety culture or climate; (3) data collected in the hospital setting and (4) studies published in English. Relevant literature was located using PubMed, CINAHL, Web of Science and PsycINFO databases. Quantitative and qualitative analyses were performed using RStudio and the R interface for multidimensional analysis of texts and questionnaires (IRaMuTeQ). RESULTS: A total of 248 primary studies were included. The most used instruments for assessing safety culture were the Hospital Survey on Patient Safety Culture (n=104) and the Safety Attitudes Questionnaire (n=63). The Maslach Burnout Inventory (n=13) and Culture Assessment Scales based on patient perception (n=9) were used in association with cultural instruments. Sixty-six articles were included in the qualitative analysis. In word cloud and similarity analyses, the words 'communication' and 'leadership' were most prominent. Regarding the descending hierarchical classification analysis, the content was categorised into two main classes, one of which was subdivided into five subclasses: class 1a: job satisfaction and leadership (15.56%), class 1b: error response (22.22%), class 1c: psychological and empowerment nurses (20.00%), class 1d: trust culture (22.22%) and class 2: innovation worker (20.00%). CONCLUSION: The instruments presented elements that remained indispensable for assessing the safety culture, such as leadership commitment, open communication and learning from mistakes. There was also a tendency for research to assess patient and family engagement, psychological safety, nurses' engagement in decision-making and innovation.

The aim of the present study was to carry out an epidemiological survey on gastrointestinal infections by nematodes and coccidia in cattle herds in the semiarid region of Paraíba, northeastern Brazil. A total of 832 cattle were sampled, of both sexes and different ages, on 21 farms in 21 municipalities in the state of Paraíba, Brazil. Fecal samples were collected individually to perform eggs per gram (EPG) and oocysts per gram (OoPG) counts in feces, and to perform fecal cultures. To assess factors associated with nematode and coccidia infections, epidemiological survey forms were applied to the farmers. The total frequency of infections by nematodes was 59.6% (496/832) and it was 36.9% (307/832) for coccidia. The levels of infection by nematodes and coccidia were most frequently low (EPG < 300; OoPG < 1,000), in 72.4% (359/496) and 75.2% (231/307), respectively. The most frequent nematode genera was Haemonchus spp. (78.8%). The risk factors for nematode infections in cattle were beef production trait (odds ratio = 2.99) and extensive rearing system (odds ratio = 3.8); the factors for coccidia were age ≤ 12 months (odds ratio = 2.9) and body score between 1 and 2 (odds ratio = 5.2). These results prove that there are high frequencies of gastrointestinal infections by nematodes and coccidia in cattle herds in the semiarid region of Paraíba, with predominantly low levels of infection. However, given the importance of subclinical infections, adjustments to sanitary management measures can contribute to reduction of parasite indices and better performance among the animals, thus generating greater profitability for farmers.


O objetivo do presente estudo foi realizar uma análise epidemiológica sobre as infecções gastrintestinais por nematóides e coccídios em rebanhos bovinos na região semiárida da Paraíba, Nordeste do Brasil. Um total de 832 bovinos foram utilizados, de ambos os sexos e diferentes idades, oriundos de 21 fazendas em 21 municípios do estado da Paraíba, Brasil. Amostras fecais individuais foram coletadas para a realização de contagens de ovos por grama (OPG) e oocistos por grama (OoPG) de fezes e para realizar coproculturas. Para obter os fatores associados às infecções por nematódeos e coccídios, questionários epidemiológicos foram aplicados aos produtores. A frequência total de infecções por nematóides foi 59,6% (496/832) e 36,9% (307/832) para coccídios. O nível de infecção para nematóides e coccídios mais frequente foi o baixo (OPG < 300; OoPG < 1.000), em 72,4% (359/496) e 75,2% (231/307), respectivamente. O gênero de namatóide mais frequente foi Haemonchus spp. (78,8%). Os fatores de risco para infecções por nematóides em bovinos foram aptidão para corte (odds ratio = 2.99) e sistema de criação extensivo (odds ratio = 3,8); os fatores para infecções por coccídios foram idade ≤ 12 meses (odds ratio = 2,9) e escore corporal entre 1 e 2 (odds ratio = 5,2). Estes resultados demonstraram que são elevadas as frequências de infecções gastrintestinais por nematóides e coccídios em rebanhos bovinos no Semiárido da Paraíba, com predominância de infecções em nível baixo. Entretanto, devido a importância das infecções subclínicas, ajustes em medidas de manejo sanitário podem contribuir para a redução dos índices de infecções por parasitos e melhorar a performance entre os animais, gerando maiores benefícios econômicos aos produtores.

The aim of this study was to evaluate the prevalence and diversity of endoparasitic fauna and the risk factors associated with parasite infections in free-range chickens in the state of Paraíba, northeastern Brazil. Ten municipalities were visited and, in each of them, ten farms, to collect animal feces and apply epidemiological questionnaires. Feces from 417 poultry were used to perform EPG (eggs per gram) and OoPG (oocysts per gram) tests. Prevalences of 40.52% (169/417) and 39.08% (163/417) were observed for nematodes and coccidia, respectively. In 17% (71/417), mixed infections by nematodes and coccidia were observed. Nematodes of Heterakoidea superfamily were present in 100% of the positive samples (169/169), followed by Trichuris spp. (57.3%; 97/169). All the protozoan oocysts observed belonged to the genus Eimeria (100%; 163/163). The variable of presence of drooping wings was considered to be a factor associated with infection by coccidia (odds ratio = 5.412; confidence interval: 1.179-24.848; p = 0.030). It was concluded that there is high prevalence of nematodes and coccidia in free-range chickens in the state of Paraíba, Brazil. Better sanitary management measures, with greater hygiene of facilities, together with chemical control of parasites, can improve productivity by reducing the rate of gastrointestinal parasites.

Rhipicephalus microplus, also known as the cattle tick, is the parasite with the greatest impact on cattle in Brazil. The most common method for controlling this tick is the application of synthetic chemical acaricides, especially ivermectin, which belongs to the group of macrocyclic lactones (MLs). However, because ivermectin is widely used, there is concern about the development of cross-resistance within this chemical class. Thus, engorged females were collected from farms with a history of resistance to ivermectin, which was the only one among the MLs that was used as an endectocide drug. Using larval immersion tests (LIT), bioassays were performed with ivermectin, moxidectin and eprinomectin on populations of R. microplus from the semiarid region of the states of Paraíba and Ceará, Brazil. Epidemiological questionnaires were applied to collect information about tick control management. All the evaluated populations showed cross-resistance between ivermectin and moxidectin, but only one population showed cross-resistance between ivermectin and eprinomectin. Weekly or monthly administration of injectable 1% ivermectin on farms was reported. It was concluded that the frequent use of ivermectin may lead to the development of cross-resistance to moxidectin. For eprinomectin, despite the structural similarity, cross-resistance was not observed in three tick populations.

The aim of the present study was to find out the diversity of Eimeria species in cattle herds in the semiarid region of Brazil. Forty cattle fecal samples were collected from 20 farms in the Paraíba State, Northeast Brazil, and examined by centrifugation-floatation technique in sucrose solution. From each positive animal, 20 oocysts were photographed and measured. Infection by Eimeria spp. was detected in 17.12% (137/800) of the samples analyzed. All the farms had at least one animal that was positive for Eimeria spp. (100%; 20/20). In total, 2740 coccidia were photographed and measured. The species detected were: Eimeria bovis (35.1%); Eimeria canadensis (17.48%); Eimeria auburnensis (14.7%); Eimeria ellipsoidalis (9.7%); Eimeria zuernii (7.22%); Eimeria brasiliensis (4.56%); Eimeria bukidnonensis (3.97%); Eimeria illinoisensis (2.91%); Eimeria wyomingensis (1.42%); Eimeria alabamensis (1.27%); Eimeria cylindrica (0.76%); Eimeria pellita (0.54%); Eimeria ildefonsoi (0.21%); and Eimeria subspherica (0.07%). It was concluded that cattle in the semiarid region of Brazil were parasitized by 14 species of Eimeria. It is thinked that the sanitary management employed, as well as the system used for raising these animals, is the crucial point that leads to high rates of infection in this region.

We aimed to evaluate a Brazilian commercial formulation of the fungus Duddingtonia flagrans (Bioverm®) for controlling gastrointestinal nematodes in sheep kept on native pasture in the Caatinga biome, in the semiarid region of Brazil. Twenty ewes, aged between 12 and 18 months, were divided into two groups. In the treated group, each animal received 1 g of the Bioverm® product for each 10 kg of live weight, daily, together with commercial feed, for 6 months. In the control group, the animals received feed without Bioverm®. Each group remained throughout the experiment in a 1.2-ha paddock. Monthly counts of eggs per gram (EPG) of feces, fecal cultures, packed cell volume (PCV), weight measurements, and collection of leaf mass from the pasture were performed. There was greater reduction in EPG, greater weight gain, and less infestation by infective larvae in the paddock of the Bioverm® group, compared with the control group (p < 0.05). There was no significant difference (p ≥ 0.05) in the mean PCV percentage between the Bioverm® and control groups. In coprocultures, Haemonchus sp. was the most prevalent helminth. Bioverm® (D. flagrans) was efficient for biological control of sheep gastrointestinal nematodes in the semiarid region of Brazil.

Dez municípios foram visitados e, em cada um, dez fazendas, para coletar fezes e aplicar um questionário epidemiológico. Fezes de 417 aves foram utilizadas para a realização de testes de OPG (ovos por grama) e OoPG (oocistos por grama). Prevalências de 40,52% (169/417) e 39,08% (163/417) foram observadas para nematoides e coccídios, respectivamente. Em 17% (71/417), infecções mistas por nematoides e coccídios foram observadas. Nematoides da superfamília Heterakoidea estavam presentes em 100% das amostras positivas (169/169), seguidos por Trichuris spp. (57,3%; 97/169). Todos os oocistos de protozoários observados pertenceram ao gênero Eimeria (100%; 163/163). A variável de presença de asas caídas foi considerada como fator associado à infecção por coccídios (odds ratio= 5,412; interval de confiança: 1,179-24,848; p = 0,030). Concluiu-se que é alta a prevalência de nematoides e coccídios em galinhas de fundo de quintal no Estado da Paraíba. Melhorias nas medidas de manejo sanitário, com maior higiene das instalações, associados a um controle químico de parasitos, podem melhorar a produtividade pela redução das taxas de parasitismo gastrointestinal.(AU)

Objetivou-se descrever a diversidade de Eimeria em rebanhos bovinos do Semiárido do Brasil. Amostras fecais de 40 bovinos foram coletadas, provenientes de 20 fazendas no Estado da Paraíba, Nordeste do Brasil, e examinadas pela técnica de centrífugo-flutuação em solução de sacarose. De cada animal positivo, foram fotografados e mensurados 20 oocistos. A presença de Eimeria spp. foi detectada em 17,12% (137/800) das amostras analisadas. Todas as propriedades tiveram ao menos um animal positivo para Eimeria spp. (100%; 20/20). No total, foram fotografados e mensurados 2.740 coccídios, cujas ocorrências foram: Eimeria bovis (35,1%), seguida de Eimeria canadensis (17,5%), Eimeria auburnensis (14,7%), Eimeria ellipsoidallis (9,7%), Eimeria zuernii (7,22%), Eimeria brasiliensis (4,56%), Eimeria bukidnonensis (3,97%), Eimeria illinoisensis (2,91%), Eimeria wyomingensis (1,42%), Eimeria alabamensis (1,27%), Eimeria cylindrica (0,76%), Eimeria pellita (0,54%), Eimeria ildefonsoi (0,21%) e Eimeria subspherica (0,07%). The thin line between efficacy and toxicity has challenged cancer therapy. As copper is an essential micronutrient and is important to tumor biology, CuII complexes emerged as an alternative to chemotherapy; however, its biological properties need to be better understood. Thus, we report in vitro the antitumor effects of two CuII complexes named [Cu(4-fh)(phen)(ClO4)2] (complex 1) and [Cu(4-nh)(phen)(ClO4)2]·H2O (complex 2), in which 4-fh = 4-fluorophenoxyacetic acid hydrazide; 4-nh = 4-nitrobenzoic hydrazide and phen = 1,10-phenanthroline. Both complexes presented cytotoxic activity against tumor cells, but only complex 1 showed significant selectivity. Complex 1 also induced DNA-damage, led to G0/G1 arrest and triggered apoptosis, which was initiated by an autophagy dysfunction. The significant in vitro selectivity and the action mechanism of complex 1 are noteworthy and reveal this prodrug as promising for anticancer therapy.

The effectiveness of four anthelmintic classes on cattle gastrointestinal nematodes in the semi-arid region of Paraiba State, Brazil, was evaluated. Twenty farms were used, testing 40 animals in each one, totaling 800 animals. Cattle were divided into four groups composed with ten animals: I, treated with albendazole sulfoxide 15%; II, treated with ivermectin 1%; III, treated with closantel 25%; IV, treated with levamisole hydrochloride 7.5%. All treatments were administered subcutaneously. For the Fecal Egg Count Reduction Test (FECRT), individual fecal samples were collected on days 0 and 14, and sent for analysis of egg count per gram of feces (EPG) and larval cultures. It was observed that multiresistance was present in 95% (19/20) of the farms. Resistance to ivermectin and albendazole was observed in 95% (19/20), to closantel in 75% (15/20) and to levamisole in 20% (4/20). The most used management system was semi-intensive (75%; 15/20) and the ivermectin was the most reported drug for controlling helminths (65%; 13/20). Haemonchus spp. was the most prevalent helminth genus. It was concluded that the anthelmintic resistance of bovine gastrointestinal nematodes is high in the semi-arid of Paraíba State, Brazil, with multiresistance observed mainly to ivermectin, albendazole and closantel.

OBJECTIVES: to analyze the predictors of moral harassment in nursing work in critical care units. METHODS: a cross-sectional study conducted in a public hospital in Fortaleza, Ceará, with 167 nursing professionals in 2016. Sociodemographic/occupational questionnaire and Negative Acts Questionnaire Revised were applied. The analysis included descriptive statistics, measures of central tendency and dispersion, as well as Mann-Whitney, Kruskal-Wallis and Conover Inman U-tests for multiple comparisons. RESULTS: there was a 33% prevalence of self-perception of moral harassment, highlighting personal/professional disqualification and work-related harassment. The predictors of moral harassment included age, time working in the job and time in the unit, employment relationship and sector. CONCLUSIONS: young professionals (< 30 years), cooperative, crowded in intensive care or emergency units, with less time working in the job (< 5 years) or greater time in the unit (above 10 years) are the biggest victims of moral harassment in the work of nursing in critical environments.

RESUMO

OBJECTIVE: To elaborate and validate an instrument on barriers and enablers to deprescribing benzodiazepines in the patient's perspective. METHODS: This study was conducted in 3 stages: (1) a methodological stage, (2) a semi-structured pilot interview with 25 older adults undergoing clonazepam deprescribing, and (3) content validation with the Delphi technique. Content validation was performed by 50 specialists with degrees and/or experience with primary health care and/or health care of older adults, such as physicians, pharmacists, and nurses. For evaluating the obtained results, we analyzed the concordance of evaluations with the coefficient of content validity (CCV). We considered values equal to or higher than 0.8 as acceptable levels of concordance. RESULTS: The instrument was considered validated in the first round of evaluation, where all items obtained a CCV of more than 0.8 in the specialists' assessment. Nevertheless, they proposed improvements that were incorporated to the final version of the questionnaire. CONCLUSION: The instrument represents an important tool to be used by health care professionals for optimizing benzodiazepine deprescribing, with suitable levels of clarity and validity.


OBJETIVO: Elaborar e validar um instrumento sobre facilitadores e dificultadores do processo de desprescrição de benzodiazepínicos na perspectiva do paciente. METODOLOGIA: Estudo desenvolvido em três etapas, quais sejam: (1) metodológica, (2) entrevista piloto semiestruturada com 25 idosos em desprescrição de clonazepam e (3) validação de conteúdo por meio da técnica de Delphi. A validação deu-se por 50 especialistas com formação e/ou experiência na área da Atenção Primária à Saúde e/ou Saúde do Idoso, entre eles médicos, farmacêuticos e enfermeiros. Como medidas para avaliar os resultados obtidos, foi analisada a concordância da avaliação por meio do coeficiente de validade de conteúdo (CVC). Considerou-se como nível aceitável de concordância o valor maior ou igual a 0,8. RESULTADOS: O instrumento foi considerado validado na primeira rodada de avaliação, em que todos os itens avaliados obtiveram CVC superior a 0,8 na avaliação dos especialistas. Entretanto, estes propuseram sugestões de melhorias que foram incorporadas na versão final do questionário. CONCLUSÃO: O instrumento apresenta-se como uma importante ferramenta a ser utilizada pelos profissionais de saúde para a otimização do processo de desprescrição de benzodiazepínicos, possuindo índice de clareza e de validade adequados.

The apparent digestibility coefficients for crude protein (ADCCP), dry matter (ADCDM), and gross energy (ADCGE) of ingredients were determined for piapara (Megaleporinus obtusidens). Test diets were formulated to contain 69.5% of reference diet, 0.5% of chromium oxide, and 30% of test ingredients. The protein ingredients evaluated were tilapia processing residue meal (TPRM), feather and poultry blood meal (FPBM), poultry by-product meal (PBM), meat and bone meal (MBM), cottonseed meal (CM), corn gluten meal (CGM), and soybean meal (SM); the energy ingredients tested were corn (C), corn germ meal (CGRM), rice meal (RM), wheat bran (WB), and sorghum (S). Groups of 30 piaparas were fed twice daily during five days with test diets. Intestinal morphometry of fish were also evaluated. Digestibility coefficients of protein and energy ingredients were highest for soybean meal (ADCDM = 85.8%; ADCCP = 95.2%; and ADCGE = 87.2%) and corn (ADCDM = 94.5%; ADCCP = 76.2%; and ADCGE = 89.3%), respectively. Of the energy test ingredients analyzed, corn had the highest digestibility coefficients and induced beneficial changes on intestinal morphology compared to sorghum and corn germ meal. All protein ingredients showed potential for use in piapara diets, except meat and bone meal and feather and blood meal.(AU)

Dietas testes foram formuladas contendo 69,5% da dieta referência, 0,5% de óxido de cromo e 30% do ingrediente teste. Os alimentos proteicos avaliados foram: farinha do resíduo de processamento da tilapia (TPRM), farinha de penas e sangue de aves (FPBM), farinha de vísceras de aves (PBM), farinha de carne e ossos (MBM), farelo de algodão (CM), glúten de milho (CGM) e farelo de soja (SM); os alimentos energéticos testados foram: milho (C), gérmen de milho (CGRM), farelo de arroz (RM), farelo de trigo (WB) e sorgo (S). Grupos de 30 piaparas foram alimentados duas vezes ao dia durante cinco dias com as dietas testes. Também foi avaliada a morfometria intestinal dos peixes. O sistema de manejo mais utilizado foi o semi-intensivo (75%; 15/20) e a ivermectina foi o fármaco mais relatado para controle de verminose (65%; 13/20). O gênero de helminto mais prevalente foi Haemonchus spp. (76,7%). Conclui-se que é alta a resistência anti-helmíntica por nematódeos gastrintestinais de bovinos no Semiárido da Paraíba, Brasil, com multirressistência observada principalmente à ivermectina, ao albendazol e ao closantel.(AU)
