RESUMO

The aim of the study was to investigate how phase angle (PhA) is associated with subtotal and lumbar spine bone mineral density [BMD], lean soft tissue mass [LSTM], total body fat mass, android and gynoid in children and adolescents with HIV according to sex. A cross-sectional study was conducted in Florianópolis, Brazil, involving 64 children and adolescents vertically transmitted with HIV. Resistance and reactance values were obtained using bioelectrical impedance analysis, and PhA was subsequently calculated. Dual emission X-ray absorptiometry was used to assess body composition. Antiretroviral medication, physical activity (accelerometers) and skeletal maturation (wrist-carpal radiography) were used in the adjusted model. In males, PhA was directly associated with subtotal BMD (ßadj: 0.65; R²: 0.38, p < 0.01) and lumbar spine BMD (ßadj: 0.53; R²: 0.22, p = 0.01), directly associated with LSTM (ßadj: 0.76; R²: 0.46, p < 0.01), and inversely associated with gynoid fat (ßadj: -0.47; R²: 0.2, p = 0.01), in adjusted models. In females, PhA was directly associated with subtotal BMD (ßadj: 0.46; R²: 0.17, p < 0.01) and lumbar spine BMD (ßadj: 0.48; R²: 0.19, p < 0.01). It is concluded that PhA was directly associated with subtotal and regional BMD, LSTM, and inversely with gynoid fat in boys with HIV. In girls, PA was directly associated only with subtotal and regional BMD.

RESUMO

OBJECTIVE: To qualify a 3C approach model of dual-energy X-ray absorptiometry (DXA) to estimate multicomponent resting energy expenditure (REE) referenced by indirect calorimetry (IC). METHODS: A sample of 155 college students, of both sexes (18-30 years old) was evaluated. Anthropometric measures, REE by IC, and whole-body DXA-scans were performed. The REE for each body component was determined after transforming the components from the molecular (DXA) to the organ tissue level. Bland-Altman and proportional bias analyses were used to verify agreement between REE measured (REEIC ) and estimated (REEDXA ). RESULTS: Statistically significant differences were found for all sex comparisons (P < .001), except for age (P = .950). Differences from the final sex-specific models' were not found between REEIC and REEDXA (P > .05). Men also presented greater expenditure (P < .001) in each component, except for adipose tissue. The plots confirmed the validity of the model for both sexes, with low difference values between the measured and estimated REE. The mean of the differences of REEIC and REEDXA showed heteroscedasticity of the data for men (P = .004). The same error tendency was not evident for women (P = .333). CONCLUSIONS: This 3C model, estimating REE from a multicomponent approach, allows a new application of DXA as tool for understanding intraindividual differences in terms of the mass of metabolically active tissue. Sex and populational differences should be taken in account. Consequently, we present qualified sex-specific DXA models that can be applied in different contexts such as health and sports, besides considering interpersonal differences in terms of energy expenditure.

RESUMO

INTRODUCTION: Introduction: appropriate appendicular lean soft tissue (ALST) parameters to identify sarcopenia in Brazil are scarce. The use of international references may lead to a false positive diagnosis. The objective was to propose cut-off points to identify sarcopenia in older men and women using DXA-derived ALST values from a young adult population. Methods: this was an observational study with a cross-sectional analysis. University students of both sexes (n = 125), aged 20 to 30 years, underwent anthropometric measurements and DXA scanning to obtain their ALST (kg). Cut-off points for sarcopenia were set at -2 standard deviations (-2SDs) away from the mean ALST of a young sample. Absolute values and indices (ALST/height2, ALST/weight, and ALST/body mass index [BMI]) were considered as recommended by international consensus. Results: compared to women, men presented higher values of weight, height, BMI, bone mineral content, lean soft tissue, ALST, and ALST index (kg/m²). Only fat mass was higher in women, and age was not different between genders. The -2SD ALST indices obtained were ≤ 6.56 kg/m² for men and ≤ 4.67 kg/m² for women. They were below international and national values, which tended to classify false positives. Conclusions: the -2SD ALST values proposed here are validated cut-offs for identifying low muscle in older adults and to prevent misdiagnosis with sarcopenia. In addition, they contribute to efficient monitoring and control of this disease in geriatric populations.

Body composition assessment at the molecular level is relevant for the athletic population and its association with high performance is well recognized. The four-compartment molecular model (4C) is the reference method for fat mass (FM) and fat-free mass (FFM) estimation. However, its implementation in a real context is not feasible. Coaches and athletes need practical body composition methods for body composition assessment, and the bioelectrical impedance analysis method (BIA) is usually seen as a useful alternative. The aim of this study was to test the validity of BIA (Tanita, TBF-310) to determine the FM and FFM of elite judo athletes. A total of 29 males were evaluated in a period of weight stability using the reference method (4C) and the alternative method (Tanita, TBF-310). Regarding the 4C method, total-body water was assessed by deuterium dilution, bone mineral by DXA, and body volume by air displacement plethysmography. The slops and intercepts differed from 1 (0.39 and 1.11) and 0 (4.24 and -6.41) for FM and FFM, respectively. FM from Tanita TBF-310 overestimated the 4C method by 0.2 kg although no differences were found for FFM. Tanita TBF-310 explained 21% and 72% respectively in the estimation of absolute values of FM and FFM from the 4C method. Limits of agreement were significant, varying from -6.7 kg to 7.0 kg for FM and from -8.9 kg to 7.5 kg for FFM. In conclusion, TBF-310 Tanita is not a valid alternative method for estimating body composition in highly trained judo athletes.

The objective of this study was to identify the association of phase angle with physical fitness in children and adolescents infected with HIV. Sixty-four children and adolescents infected with HIV (8-15 years old) were tested for vertical transmission. The electrical bioimpedance was used to obtain the phase angle. Body fat (BF) and lean soft tissue mass (LSTM) were measured by x-ray absorptiometry, and the manual grip strength was evaluated by dynamometry. The peak oxygen consumption (VO2 peak) was measured by respiratory change in a submaximal incremental test. Moderate-vigorous physical activity (MVP) and sedentary behavior were measured using accelerometers and were used as covariates. Multiple linear regression was used. The linear correlation analyses demonstrated that the phase angle was inversely proportional to relative BF (r = -0.26), but was not associated with absolute body fat (r = -0.02). The phase angle was directly associated with LSTM (r = 0.57), handgrip strength, and (r = 0.43) peak VO2 (r = 0.55). The phase angle was not associated with absolute BF (ß = -0.017, P = 0.413) and relative (ß = -0.014, P = 0.175). The phase angle presented a direct association with LSTM (ß = 0.041, P = 0.019) and peak VO2 (ß = 0.005; P = 0.019), regardless of age, sex, sexual maturation, MVPA, sedentary behavior, antiretroviral drugs, and viral load. No association was found between phase angle and handgrip strength (ß = 0.153; P = 0.199). It was concluded that phase angle was associated with LSTM and the aerobic fitness in HIV-infected children and adolescents.

RESUMO

Abstract Magnetic resonance imaging and Computer tomography are gold standards in the measurement of muscle tissue (MT), but are expensive. Dual Energy X-Ray Absorptiometry (DXA) is also costly but safer and allows for the measurement of Appendicular Lean Soft Tissue (ALST), a strong predictor of MT. Alternatively, there are anthropometric models that predict the ALST of Portuguese athletes with low cost/risk that have not been validated in other populations. The aim of this study was to validate anthropometric Portuguese models that predict ALST in young athletes or, if the validation fails, to propose new models. The ALSTDXA of 174 young athletes was determined by DXA. Two anthropometric models (ALSTmod1 and ALSTmod2) measuring ALST among Portuguese athletes were tested. To validate the coefficient of determination, the difference (bias) and concordance correlation coefficient between predicted and actual values were computed. Finally, association between mean and difference of methods was verified. Validation failed and, for this reason, new multiple regression models were proposed and validated using PRESS statistics. The Portuguese models explained ~96% of the ALSTDXA variability. The difference between ALST (-0.7kg) was less than that found for the ALSTmod2 and ALSTDXA limits of agreement from 3.6 to -2.1 and from 6.1 to -1.5kg, respectively. The new models included three predictive equations for ALST. Only ASLTmod1was valid; however, it was prone to bias, depending on the magnitude of ALST values. The newly proposed models present validity with greater concordance (r2PRESS=0.98), lower standard error of estimate (SEEPRESS [kg]=0.91) and more homogeneous predicted extreme values.

This study evaluated the prevalence of physical activity through the practice of sports in adolescents from schools in two Brazilian cities and a Portuguese school, and its association with independent variables, such as gender and age. A cross-sectional study was conducted of schoolchildren from two cities in Brazil and one in Portugal. The total study sample was 3694 subjects (1622 males and 1872 females). Physical activity levels were assessed using Baecke's questionnaire. Body weight was measured on electronic scales and stature was measured with a portable wooden stadiometer. Numerical variables were expressed as mean, categorical variables were expressed as percentages and the chi-square test analyzed associations. The prevalence of no sport was high (39.7%), being higher in the Portuguese school than in the Brazilian schools (p < 0.001). Irrespective of being an adolescent in a Brazilian or Portuguese school, boys showed higher engagement in sports practice than girls (p < 0.001). In both, differences were identified between adolescents aged 13 to 15 (P = 0.001) and 16 to 17 (P = 0.001). The prevalence of physical inactivity among schoolchildren from two cities in Brazil and a school in Portugal was high, with the girls practicing less sport than the boys and with this imbalance likely to be higher in adolescents.

Due to the lack of valid and accessible tests to measure the body fat percentage (BF%) of adolescent taekwondo athletes (TKDA), this study aimed to analyze six anthropometric equations in the prediction of BF% with the Dual-Energy-x-Ray (DEXA) as referential method for five TKDA (12.23 years ± 1.60). The BF% estimated by the equations were compared with DEXA values using the t-student , linear regression and Bland & Altman (B&A) tests. Only the equation of Slaughter et al. (1988) was adequated by the sample size calculation, and although it subestimated the BF% (in 4.85% ± 0.98), it showed a high correlation (R = 0.935; R² = 0.874 EPE = 1.01) and low amplitude in the limits of agreement at 95% (3.84%) by B&A in comparison to DEXA. However, this equation is adequated to predict the BF% in TKDA, if it's corrected by the adjustement equation [%G (DEXA) = 1,64 + 1,24•%G (Eq 4)] generated by linear regression.

Background: A better physical fitness may have survival advantages in adults. Aim: To analyze the relationship between cardiorespiratory fitness and cardiovascular risk factors among obese subjects aged 58 years and older. Material and Methods: Cardiorespiratory fitness using the six-minute walk test, body composition by dual-energy x-ray absorptiometry and blood pressure were measured in a non-representative sample of 76 obese Portuguese subjects aged 58 to 87 years (55 women). Participants were stratified in tertiles of walking capacity according to the six-minute walk test. Results: Six minutes walk test results were negatively correlated with percentage body fat (r = -0.28; p = 0.012) and systolic blood pressure (r = -0.23; p = 0.045). Participants located in the lowest tertile for the six minutes walk test had an odds ratio of4.34 (95% confidence intervals: 1.02-18.43) for elevated blood pressure. Conclusions: A lower six minutes walk test result is associated with a higher risk for high blood pressure.

Participants located in the lowest tertile for the six minutes walk test had an odds ratio of 4.34 (95% confidence intervals: 1.02-18.43) for elevated blood pressure. CONCLUSIONS: A lower six minutes walk test result is associated with a higher risk for high blood pressure.

RESUMO

Cardiorespiratory fitness is an important health indicator in young people. The aim of this study was to investigate the effects of age, gender, body adiposity, and ethnicity on cardiorespiratory fitness in a sample of Portuguese adolescents. The sample consisted of 266 adolescents aged 12-18 years [112 boys (80 Caucasians and 32 African-Portuguese, AP) and 154 girls (109 Caucasians and 45 AP)]. Percent body fat was estimated with a hand-to-hand bioelectrical impedance device (BF300, OMROM). Cardiorespiratory fitness was assessed by a shuttle run test (Fitnessgram battery). Multiple regression models were used for statistical analysis. The results showed that girls presented lower maximal oxygen consumption and higher percent body fat than boys. Cardiorespiratory fitness was lower in Caucasian than in AP girls. Multiple regression analysis showed that percent body fat, age and the interaction of age with being Caucasian and age with female gender were significant determinants that were negatively associated with cardiorespiratory fitness. The results suggest that maximal oxygen consumption is lower in adolescents with higher adiposity and in older adolescents. The findings highlight the importance of promoting physical fitness in schools across ages, especially in older adolescents, adjusting for determinant factors such as gender and ethnicity.
