BACKGROUND: There are few randomized, controlled clinical trials about the effect of non-surgical periodontal treatment on oral health-related quality of life (OHRQL). This study aims to compare the effects of two different forms of non-surgical periodontal therapy, scaling and root planing (SRP) per quadrant and one-stage full-mouth disinfection (FMD), on periodontal clinical parameters and OHRQL of patients with chronic periodontitis. METHODS: In this randomized, controlled clinical trial, the questionnaires Oral Impacts on Daily Performance (OIDP) and Oral Health and Quality of Life (OHQoL) were given to 90 patients divided into two groups: SRP (n = 45) and FMD (n = 45). Periodontal clinical parameters recorded included probing depth, clinical attachment level, plaque index, and gingival index. For statistical analysis, χ(2) test, Fisher exact test, Mann-Whitney U test, and Wilcoxon test were used. Intention-to-treat analyses were performed at T0 (baseline) for periodontal clinical parameters, T1 (30 days after treatment) for questionnaires, and T2 (180 days after treatment) for both. RESULTS: No significant differences were identified between the SRP and FMD groups in regard to OHQoL and OIDP scores when comparing the data of T1 and T2. CONCLUSION: Patients treated by both SRP and FMD showed improvement in all periodontal clinical parameters and OHRQL, with no significant differences between treatment groups.

AIM: To relate the clinical effects of two different forms of non-surgical periodontal therapy - scaling and root planing per quadrant (SRP-Q) and one-stage full-mouth disinfection (FMD) - to patient-based outcomes such as fear, anxiety, and pain of moderate chronic periodontitis patients. METHODS: Dental Fear Survey (DFS) and Dental Anxiety Scale (DAS) questionnaires and Visual Analogue Scale (VAS) were applied to 78 patients randomized into two groups: SRP-Q (n = 37) and FMD (n = 41). Periodontal clinical parameters: probing pocket depth (PD), clinical attachment level (CAL), plaque index (PI), and gingival index (GI) were monitored at baseline and 6 months after treatment. Data were statistically analysed by chi-square, Fisher's exact, Mann-Whitney, Wilcoxon tests, Pearson's correlation, and Cluster analysis. RESULTS: All periodontal clinical parameters improved from baseline to 6 months. Patients with higher fear and anxiety showed a worse clinical periodontal status before and after treatment (mean CAL, PI, and GI). After both types of treatment, fear and anxiety decreased (FMD: p = 0.019; SRP-Q: p = 0.043) with no differences between the groups. Pain did not differ between groups (FMD: 20.6 ± 19.0 and SRP: 20.7 ± 20.0; p = 0.930). CONCLUSIONS: In moderate chronic periodontitis patients, SRP-Q and FMD provided periodontal clinical improvements and similar experiences of fear, anxiety, and pain.

BACKGROUND: This prospective study aims to evaluate and compare the periodontal status, periodontitis progression, tooth loss, and influence of predictable risk variables of two periodontal maintenance therapy programs over a 12-month period. METHODS: A total of 288 individuals diagnosed with chronic moderate-to-advanced periodontitis, who had finished active periodontal treatment, were evaluated in a public academic environment (AG) (n = 138), as well as in a private clinic (PG) (n = 150). A full-mouth periodontal examination was performed at baseline and at quadrimestral recalls, evaluating plaque index, probing depth, clinical attachment level, furcation involvement, bleeding on probing (BOP), and suppuration. Individuals' social, demographic, and biologic data, as well as compliance with recalls, were recorded. The effect of variables of interest and confounders were tested by univariate and multivariate analysis. RESULTS: The PG demonstrated lower rates of periodontitis progression and tooth loss than did the AG. After adjusting for confounders, the risk variables of BOP (P = 0.047), smoking (P = 0.003), and diabetes (P = 0.028) for the PG and smoking (P = 0.047) for the AG showed a negative influence on periodontal status. CONCLUSIONS: In both groups, the periodontal maintenance therapy minimized the negative effect of the risk variables. However, PG showed significantly less progression of periodontitis and tooth loss compared to AG.

A doença periodontal pode ser tratada com sucesso por meio de terapia mecânica não cirúrgica e/ou cirúrgica, entretanto, é necessária uma reavaliação periódica da condição periodontal do indivíduo. Tal conduta é conhecida como terapia de manutenção periodontal (TMP) e representa um fator crítico para o sucesso do tratamento periodontal. Estudos epidemiológicos demonstram a efetividade da terapia de manutenção em paralisar a progressão da doença, manter a saúde periodontal e prevenir a perda dentária. Por meio da análise dos parâmetros periodontais e de outros fatores, como estado sistêmico do paciente e influência de fatores ambientais, comportamentais e genéticos, pode ser feito um planejamento individualizado dos intervalos entre as consultas de manutenção. Assim, o objetivo dessa revisão de literatura foi pontuar a influência de fatores que podem alterar a suscetibilidade dos indivíduos e provocar falhas na terapia de manutenção periodontal.


Periodontal disease can be successfully treated by nonsurgical mechanical therapy and/or surgical protocols, however, is necessary a periodic reassessment of the individual’s periodontal condition. Such behavior is known as periodontal maintenance therapy (PMT) and represents a critical factor for the success of the periodontal treatment. Epidemiological studies show the effectiveness of maintenance therapy to control the disease progression, maintain periodontal health and prevent tooth loss. Through the analysis of periodontal parameters and other factors, such as patient’s systemic status, and influence of environmental factors, behavior and genetics, an individualized plan of intervals between maintenance visits can be made. Thus, the purpose of this literature review is to point out the influence of factors that can alter the susceptibility of individuals and cause failures in the periodontal maintenance therapy.
