AIM: This three-dimensional (3D) finite element analysis observed the stress distribution on the prosthetic screws on external hexagon implant and morse taper implant with different tightening loads. MATERIALS AND METHODS: For this, two different 3D models assembly were obtained from the manufacturer and transferred to finite element analysis software: External hex implant model (EHM), and Morse taper implant model (MTM), both compounds by 3.75 × 7 mm implant, abutment and abutment screw. Bolt pretension force was applied on the shaft next to the threads of the prosthetic abutment. Preload was calculated using the torque on the prosthetic screw as recommended by the manufacturer (EHM30 and MTM20) and 10 Ncm torque above the manufacturer recommendations (EHM40 and MTM30). Maximum von mises equivalent stresses were obtained on the screws. RESULTS: Preload values results were 243.18N (EHM30), 229.71N (MTM20), 324.24N (EHM40) and 344.57N (MTM30). In EHM30, EHM40 and MTM20 models the maximum stresses were below the yield strength of the abutment screw material. However, the maximum stress in MTM30 model was higher than the reference value. CONCLUSION: The torque loads above the manufacturer recommendations can cause plastic deformation in the MT abutment screw threads. The screws of morse taper implant can be more sensitive to higher loads than external hexagon implant. CLINICAL SIGNIFICANCE: The adequate torque can result in screw loosening, while fracture may occur if torque is excessive. Abutment screw suffers many screwing cycles in its lifetime in a way that some tightening forces are above manufacturer recommendations.

RESUMO

ABSTRACT: Purpose Zygomatic implants (ZI) constitute a suitable alternative for treating severe maxillary atrophy. However, a large number of complications associated with ZI have been reported in the literature. This paper presents the late complications associated with ZI during 12 years of experience in the same institution. Materials and methods All cases of ZI from 2000 to 2013 were retrospectively evaluated and the major complications relating to this type of rehabilitation were selected to report. Results The major complications found were: loss of implant, loss of osseointegration, bucco-sinusal communication, fenestration of alveolar mucosa, sinus pathology, and emergency palate fixations. Conclusion The clinical experience of the dental surgeon is critical in the success of zygomatic fixation. Furthermore, there should be careful planning of rehabilitation to reduce the rate of complications.

RESUMO

The procedure of maxillary sinus lifting using autogenous bone was considered the reference standard choice for oral rehabilitation in cases of severe atrophic maxilla. However, it is not always a viable option, due to the limitations or morbidity caused by grafting techniques. This has led to the development of bone substitutes, which have been elaborated and improved. Choosing the best biomaterial becomes difficult due to the wide variety of bone substitutes. The aim of this article is to present some of these materials that are reported in the current scientific literature for maxillary sinus lifting.


El procedimiento de elevación del seno maxilar utilizando hueso autógeno se consideraba la opción estándar de oro para la rehabilitación oral en casos de maxilar atrófico grave. Sin embargo, no siempre es una opción viable, debido a las limitaciones o a la morbilidad causada por técnicas de injerto, lo que justifica la existencia de sustitutos óseos que han sido elaborados y mejorados. En cuanto a la amplia variedad de sustitutos óseos, se hace difícil la mejor elección de biomaterial. El objetivo de este informe es presentar una variedad de sustitutos óseos respetados en la literatura científica actual, usados en la elevación del seno maxilar.

RESUMO

Computed tomographic scanning is a precise, noinvasive surveying technique that enables the professionals to improve the precision of implant placement by building a prototype that allows the confection of surgical guides. The authors present a clinical case of anterior tooth rehabilitation with frozen homogenous bone graft and immediately loaded titanium implant using computer-guided surgery. A multislice computed tomography was realized, and a prototype was built. All the procedures were previously realized in the prototype before started in the patient. This technique allows a better surgical planning, makes the procedures more accurate, and reduces surgery time.
