RESUMO

Thermotolerant Campylobacter is an important zoonotic pathogen known for causing gastroenteritis in humans, with poultry as its primary reservoir. A total of 468 samples were collected, of which 335 were chicken carcass samples (representing the food component), and 133 were chicken caeca samples (representing the animal component). These samples underwent culture, with colonies examined under a microscope. Species identification was achieved through multiplex PCR. Additionally, antimicrobial susceptibility profiles were determined using the Kirby-Bauer method, testing for sensitivity to gentamicin, ciprofloxacin, tetracycline, and erythromycin. Additionally, 55 C. jejuni (62.5%) and 33 C. coli (37.5%) isolates were selected for whole genome sequencing (WGS). A High prevalence of Campylobacter was observed, with rates of 95.5% (n = 127, CI95%: 92.5% - 98.5%) in the animal component and 72.5% (n = 243, CI95%: 69.9% - 75.1%) in the food component. Specifically, C. jejuni was detected in 33.1% (n = 42) of poultry farms and 38.3% (n = 93) of chicken carcasses, while C. coli was found in 64.6% (n = 82) of poultry farms and 60.5% (n = 147) of chicken carcasses. Antimicrobials with the highest rates of resistance (67%-100%) were ciprofloxacin and tetracycline, in both animal and food component isolates. Erythromycin resistance was notable, ranging from 22% to 33%, with only two C. jejuni isolates from retail were resistant to gentamicin. Furthermore, multidrug resistance was identified in 23% (20 isolates) of the Campylobacter isolates. Genetic analysis revealed the presence of fourteen resistance genes in both C. jejuni and C. coli isolates, including tet(O), blaOXA-460, blaOXA-184, blaOXA-489, blaOXA-193, blaOXA-784, blaOXA-603, aph(3')-IIIa, aad9, aph(2'')-If, aadE-Cc, sat4, and ant(6)-Ia. Additionally, twenty-five plasmids were detected in the 88 Campylobacter isolates examined. Interestingly, most isolates also harbored genes encoding putative virulence factors associated with pathogenicity, invasion, adherence, and production of cytolethal distending toxin (cdt): cheV, cheA, cheW, cheY, flaA, flgR, flaC, flaD, flgB, flgC, ciaB, ciaC. The WGS analysis showed the presence of several cgSTs in both animal and food components, with nine of them widely disseminated between components. Moreover, C. coli and C. jejuni isolates from different sources presented less than 11 single nucleotide polymorphisms (SNPs), suggesting clonality (16 isolates). Further analysis using SNP tree demonstrated widespread distribution of certain C. jejuni and C. coli clones across multiple farms and retail stores. This study presents, for the first-time, insights into the clonality, plasmid diversity, virulence, and antimicrobial resistance (AMR) of thermotolerant Campylobacter strains originating from the Ecuadorian poultry industry. The identification of AMR genes associated with the main antibiotics used in the treatment of campylobacteriosis in humans, highlights the importance of the prudent use of antimicrobials in the poultry industry. Additionally, this research remarks the need for regional studies to understand the epidemiology of this pathogen.

RESUMO

This article discusses questions concerning the future of humanity in the face of threats to the health of populations, whose impact has been exacerbated in the course of inequalities in all parts of the world, pari passu with global development in the hegemonized model since last century. The COVID-19 pandemic is a good example that illustrates this dissonance between development and inequalities. Questions were formulated to be debated about the construction of the future of world society, based on the understanding of the evolutionary character of life on the planet vis-à-vis the evils that affect large contingents of the population and represent powerful risks for this evolutionary process. These questions call attention to the discussion around social participation in the definition and control of public policies, as opposed to the hegemony of private interests in the formulation and execution of these policies, both in the scenarios of each country and in the international context.


O artigo discute questões sobre o futuro da humanidade ante as ameaças que rondam a saúde das populações, cujo impacto vem se exacerbando no curso das desigualdades em todas as partes do mundo, pari passu o desenvolvimento global no modelo hegemonizado a partir do século passado. A pandemia de COVID-19 foi tomada como um caso que bem ilustra essa dessintonia entre desenvolvimento e desigualdades. Objective To evaluate the stability of osteotomies created in the subtrochanteric and trochanteric regions in a pediatric femur model fixed by flexible intramedullary rods. Method Tomographic sections were obtained from a pediatric femur model with two elastic titanium rods and converted to a three-dimensional model. This model created a mesh with tetrahedral elements according to the finite element method. Three virtual models were obtained, and osteotomies were performed in different regions: mediodiaphyseal, subtrochanteric, and trochanteric. A vertical load of 85N was applied to the top of the femoral head, obtaining the displacements, the maximum and minimum main stress, and the equivalent Von Mises stress on the implant. Results With the applied load, displacements were observed at the osteotomy site of 0.04 mm in the diaphyseal group, 0.5 mm in the subtrochanteric group, and 0.06 mm in the trochanteric group. The maximum stress in the diaphyseal, subtrochanteric, and trochanteric groups was 10.4 Pa, 7.52 Pa, and 26.4 Pa, respectively. That is around 40% higher in the trochanteric group in regards to the diaphyseal (control). The minimum stress of the bone was located in the inner cortical of the femur. The equivalent Von Mises stress on the implants occurred at osteotomy, with a maximum value of 27.6 Pa in the trochanteric group. Conclusion In both trochanteric and subtrochanteric osteotomies, fixation stability was often lower than in the diaphyseal model, suggesting that flexible intramedullary nails are not suitable implants for proximal femoral fixations.

Os casos suspeitos de intoxicações exógenas são frequentes na clínica de cães e gatos. O presente trabalho objetivou realizar um estudo retrospectivo dos casos suspeitos de intoxicação em cães e gatos atendidos no Hospital Veterinário Universitário da UFPI, localizado em Teresina PI, durante o período de 2017 a 2021. Os casos foram classificados por agente tóxico suspeito, espécie, idade e ano de ocorrência. No total foram registrados 324 casos, sendo 58 em 2017, 88 em 2018, 94 em 2019, 50 em 2020 e 34 em 2021. A maioria dos casos ocorreu em 2019 (29,01%) tendo como causa principal a intoxicação medicamentosa (16,04%), seguida da intoxicação por agrotóxicos (10,80%). A maior prevalência foi em animais com idade inferior a cinco anos (87,03%); machos (56,79%); cães (68,20%). Foram observados: elevado número de casos sem agente tóxico identificado (28,08%) e de fichas de atendimento não preenchidas (8,33%) pelos médicos veterinários em atendimento. A prevenção, a educação continuada dos tutores e a capacitação dos médicos veterinários são destacadas como essenciais para a redução dos casos de intoxicações exógenas em cães e gatos.(AU)


Suspected cases of exogenous poisoning are frequent in the clinic of dogs and cats. The present study aimed to perform a retrospective study of suspected cases of intoxication in dogs and cats treated at the University Veterinary Hospital of UFPI, located in Teresina PI, during the period from 2017 to 2021. The cases were classified by suspected toxic agent, species, age and year of occurrence. A total of 324 cases were recorded, being 58 in 2017, 88 in 2018, 94 in 2019, 50 in 2020 and 34 in 2021. Most cases occurred in 2019 (29.01%) with drug intoxication (16.04%) as the main cause, followed by pesticide intoxication (10.80%). The highest prevalence was in animals less than five years old (87.03%); males (56.79%); dogs (68.20%). It was observed: high number of cases without identified toxic agent (28.08%) and unfilled attendance forms (8.33%) by the attending veterinarians. Prevention, continued education of guardians and training of veterinarians are highlighted as essential for the reduction of cases of exogenous poisoning in dogs and cats.(AU)


Los casos sospechosos de intoxicaciones exógenas son frecuentes en la clínica de perros y gatos. El presente estudio tuvo como objetivo realizar un estudio retrospectivo de los casos sospechosos de intoxicación en perros y gatos tratados en el Hospital Veterinario Universitario de la UFPI, ubicado en Teresina PI, durante el período de 2017 a 2021. Los casos se clasificaron por agente tóxico sospechoso, especie, edad y año de ocurrencia. Se registraron un total de 324 casos, 58 en 2017, 88 en 2018, 94 en 2019, 50 en 2020 y 34 en 2021. La mayoría de los casos se produjeron en 2019 (29,01%) siendo la intoxicación por fármacos la principal causa (16,04%), seguida de la intoxicación por plaguicidas (10,80%). La mayor prevalencia se dio en animales menores de cinco años (87,03%); machos (56,79%); perros (68,20%). Se observó un elevado número de casos sin agente tóxico identificado (28,08%) y de formularios asistenciales no cumplimentados (8,33%) por los veterinarios asistentes. Se insiste en que la prevención, la educación continuada de los propietarios de animales de compañía y la formación de los veterinarios son esenciales para reducir los casos de intoxicación exógena en perros y gatos.(AU)

Resumo Objetivo Avaliar a estabilidade de osteotomias criadas nas regiões subtrocantérica e trocantérica em modelo de fêmur pediátrico, fixadas por hastes intramedulares flexíveis. Método A partir de um modelo de fêmur pediátrico com duas hastes elásticas de titânio, foram obtidos cortes tomográficos que foram convertidos para um modelo tridimensional. Neste modelo foi criado uma malha com elementos tetraédricos, de acordo com o método dos elementos finitos. Foram obtidos três modelos virtuais, e realizadas osteotomias em regiões diferentes: mediodiafisária, subtrocantérica e trocantérica. Foi aplicado um carregamento vertical de 85N no topo da cabeça do fêmur, obtidos os deslocamentos, a tensão máxima e mínima principal e tensão equivalente de Von Mises no implante. Resultados Com o carregamento aplicado foram observados deslocamentos no local da osteotomia de 0,04mm no grupo diafisário, 0,5mm no subtrocantérico e 0,06mm no trocantérico. A tensão máxima principal foi 10,4Pa, 7,52Pa e 26,4Pa nos grupos diafisário, subtrocantérico e trocantérico, respectivamente. Ou seja, a tensão máxima foi em torno de 40% maior no grupo trocantérico, em relação ao diafisário (controle). A face de tensão mínima do osso localizou-se na cortical interna do fêmur. A tensão equivalente de Von Mises nos implantes ocorreu na osteotomia, com valor máximo de 27,6Pa no grupo trocantérico. Conclusão Tanto nas osteotomias no nível trocantérico, quanto subtrocantérico, a estabilidade da fixação foi muitas vezes menor que no modelo diafisário, sugerindo que as hastes intramedulares flexíveis não são implantes adequados para as fixações proximais do fêmur.

Salmonella Infantis and Enteritidis serovars have been reported as important causes of salmonellosis in humans worldwide. However, the virulence of these two serovars has yet to be compared. To evaluate the virulence of Salmonella Infantis (n = 23) and Salmonella Enteritidis (n = 7), we used two models: the Caco2 cells model (in vitro) and the Galleria mellonella model (in vivo). Additionally, the virulence genes of all tested strains were contrasted with phenotypic outcomes. Results showed that adhesion means were 18.2% for Salmonella Enteritidis and 38.2% for Salmonella Infantis strains. Invasion means were 77.1% for Salmonella Enteritidis and 56.2% for Salmonella Infantis strains. Significant differences were found between serovars in adherence and invasion assays. Mortality rates (58% for Salmonella Enteritidis and 62.6% for Salmonella Infantis) were not significantly different between serotypes. The distribution of virulence genes showed that genes fae (fimbrial adherence determinants) and shdA (nonfimbrial adherence determinants) were only found in Salmonella Infantis strains. On the other hand, the rck gene (invasion) and Plasmid-encoded fimbriae genes (pef A, B, C, D) were present in Salmonella Enteritidis exclusively. In conclusion, this study shows that Salmonella Enteritidis has a higher virulence potential under experimental conditions than Salmonella Infantis. However, more studies are needed to determine the risk that Salmonella Infantis could represent compared with Salmonella Enteritidis. Moreover, other in vivo models should be considered to assess the virulence of these serovars.

RESUMO

Este artigo analisa o processo de formulação de uma plataforma eletrônica para a saúde, derivada de uma encomenda tecnológica, considerando os fatores que influenciaram a trajetória dessa formulação e o formato com o qual ela foi implementada. A análise documental, bibliográfica e a realização das entrevistas semiestruturadas com informantes-chave foram trabalhadas na perspectiva metodológica de um estudo de caso com uso do process tracing e de recursos da teoria da mudança institucional gradual. Destacam-se como resultados a identificação dos principais fatores relacionados à atuação da Fundação Estatal Saúde da Família na área de ciência, tecnologia e inovação e à realização da encomenda tecnológica. Bem como os fatores relacionados à formulação de cada uma das três inovações que caracterizam a experiência em análise: o Registro Eletrônico de Saúde, a plataforma que serviu de base para o ecossistema de inovação e o modelo jurídico-administrativo da organização criada para implementar e desenvolver as soluções necessárias


This article analyzes the formulation process of an electronic platform for health, derived from a technological order, considering the factors that influenced the trajectory of this formulation and the format in which it was implemented. A documentary and bibliographic analysis and semi-structured interviews with key informants were carried out from the methodological perspective of a case study using process tracing and resources from theories of gradual institutional change. The identification of the main factors related to the integration of the Fundação Estatal Saúde da Família (State Family Health Foundation) into the science, technology and innovation area and the achievement of the technological order stand out from other results. As well as the factors related to the formulation of each of the three innovations that characterize the experience under analysis: the Electronic Health Record, the basic platform of the innovation ecosystem and the legal-administrative model of the organization created to implement and develop the necessary solutions


Este artículo analiza el proceso de formulación de una plataforma electrónica para la salud, derivada de una encomienda tecnológica, considerando los factores que influyeron en la trayectoria de esa formulación y el formato en que fue implementada. Se realizó análisis documental, bibliográfico y entrevistas semiestructuradas a informantes-clave desde la perspectiva metodológica de un estudio de caso utilizando process tracing y recursos de las teorías del cambio institucional gradual. Se destacan como resultados la identificación de los principales factores relacionados con la integración de la Fundação Estatal Saúde da Família (Fundación Estatal Salud de la Familia) en el área de ciencia, tecnología y innovación y la realización de la encomienda tecnológica. Así como los factores relacionados con la formulación de cada una de las tres innovaciones que caracterizan la experiencia en análisis: el registro electrónico de salud; la plataforma base del ecosistema de innovación y el modelo jurídico-administrativo de la organización creada para implementar y desarrollar las soluciones necesarias.

OBJECTIVE: To evaluate the quality of information on gestational syphilis (GS) and congenital syphilis (CS) on the Sistema de Informação de Agravos de Notificação (SINAN-Syphilis Brazil - Notifiable Diseases Information System) by compiling and validating completeness indicators between 2007 and 2018. METHODS: Overall, care, and socioeconomic completeness scores were compiled based on selected variables, by using ad hoc weights assigned by experts. The completeness scores were analysed, considering the region and area of residence, the pregnant woman's race/colour, and the year of case notification. Pearson's correlation coefficients were used to validate the scores obtained by the weighted average method, compared with the values obtained by principal component analysis (PCA). RESULTS: Most selected variables presented a good or excellent degree of completeness for GS and CS, except for clinical classification, pregnant woman's level of education, partner's treatment, and child's race/colour, which were classified as poor or very poor. The overall (89.93% versus 89.69%) and socioeconomic (88.71% versus 88.24%) completeness scores for GS and CS, respectively, were classified as regular, whereas the care score (GS-90.88%, and CS-90.72%) was good, despite improvements over time. Differences in the overall, care and socioeconomic completeness scores according to region, area of residence, and ethnic-racial groups were reported for syphilis notifications. The completeness scores estimated by the weighted average method and PCA showed a strong linear correlation (> 0.90). CONCLUSION: The completeness of GS and CS notifications has been improving in recent years, highlighting the variables that form the care score, compared with the socioeconomic scores, despite differences between regions, area of residence, and ethnic-racial groups. The weighted average was a viable methodological alternative easily operationalised to estimate data completeness scores, allowing routine monitoring of the completeness of gestational and congenital syphilis records.

We explored ancestry-related differences in the genetic architecture of whole-blood gene expression using whole-genome and RNA sequencing data from 2,733 African Americans, Puerto Ricans and Mexican Americans. We found that heritability of gene expression significantly increased with greater proportions of African genetic ancestry and decreased with higher proportions of Indigenous American ancestry, reflecting the relationship between heterozygosity and genetic variance. Among heritable protein-coding genes, the prevalence of ancestry-specific expression quantitative trait loci (anc-eQTLs) was 30% in African ancestry and 8% for Indigenous American ancestry segments. Most anc-eQTLs (89%) were driven by population differences in allele frequency. Transcriptome-wide association analyses of multi-ancestry summary statistics for 28 traits identified 79% more gene-trait associations using transcriptome prediction models trained in our admixed population than models trained using data from the Genotype-Tissue Expression project. Our study highlights the importance of measuring gene expression across large and ancestrally diverse populations for enabling new discoveries and reducing disparities.

Objective: To evaluate the positive predictive value (PPV) of amorphous calcifications and to analyze the imaging variables that could alter the risk of malignancy associated with this finding. Materials and Methods: This was a retrospective study of 138 stereotactically guided percutaneous vacuum-assisted biopsies of amorphous calcifications, performed between January 2012 and December 2017. All of the patients included were referred for radiological follow-up for a minimum of one year (if the histopathology showed a benign lesion) or for surgical treatment (if the histopathology showed malignancy or a lesion of uncertain malignant potential). Results: We found that the PPV of amorphous calcifications was 9.42%. However, most of the malignant amorphous calcifications were in cases of invasive carcinoma or high-grade ductal carcinoma in situ, indicating clinically relevant disease. The relative risk of malignancy associated with amorphous calcifications was 6.15 times higher in patients with a family or personal history of breast or ovarian cancer. Neither being postmenopausal nor having dense breasts was found to be predictive of malignancy in patients with amorphous calcifications. Conclusion: Amorphous calcifications in the breast had a PPV for malignancy of 9.42%, indicating the possibility of placing the finding in subcategory 4a, which requires histopathological analysis. Our finding that the risk of malignancy associated with this subtype of calcifications is up to 6.15 times higher in patients with a family or personal history of breast cancer warrants greater concern regarding the clinical, radiologic, and histopathologic correlations after biopsy.

Materiais e Métodos: Foram revisados, retrospectivamente, 138 resultados de biópsias percutâneas estereotáxicas a vácuo de calcificações amorfas, entre janeiro de 2012 e dezembro de 2017. Todas as pacientes incluídas apresentavam seguimento radiológico mínimo de um ano (histopatológico benigno) ou tratamento cirúrgico (histopatológico maligno). Resultados: O VPP das calcificações amorfas foi de 9,42%. As lesões malignas corresponderam predominantemente a carcinomas invasivos, indicando doença clinicamente relevante. O risco relativo de malignidade das calcificações amorfas foi 6,15 vezes maior em pacientes com história familiar ou pessoal de neoplasia de mama ou ovário. Status pós-menopausa e mamas densas não foram preditores de malignidade nessas pacientes. Conclusão: As calcificações amorfas na mama apresentaram VPP de malignidade de 9,42%, sugerindo possibilidade de classificação do achado na subcategoria 4a, com necessidade de investigação histopatológica. Em pacientes com história familiar ou pessoal de câncer de mama, o risco de malignidade deste subtipo de calcificações pode ser até 6,15 vezes maior, justificando maior preocupação na correlação clínica, radiológica e histopatológica após biópsia.

BACKGROUND: Congenital syphilis (CS) is a major and avoidable cause of neonatal death worldwide. In this study, we aimed to estimate excess all-cause mortality in children under 5 years with CS compared to those without CS. METHODS AND FINDINGS: In this population-based cohort study, we used linked, routinely collected data from Brazil from January 2011 to December 2017. Cox survival models were adjusted for maternal region of residence, maternal age, education, material status, self-declared race and newborn sex, and year of birth and stratified according to maternal treatment status, non-treponemal titers and presence of signs and symptoms at birth. Over 7 years, a total of 20 057 013 live-born children followed up (through linkage) to 5 years of age, 93 525 were registered with CS, and 2 476 died. The all-cause mortality rate in the CS group was 7·84/1 000 person-years compared with 2·92/1 000 person-years in children without CS, crude hazard ratio (HR) = 2·41 (95% CI 2·31 to 2·50). In the fully adjusted model, the highest under-five mortality risk was observed among children with CS from untreated mothers HR = 2·82 (95% CI 2·63 to 3·02), infants with non-treponemal titer higher than 1:64 HR = 8·87 (95% CI 7·70 to 10·22), and children with signs and symptoms at birth HR = 7·10 (95% CI 6·60 to 7·63). Among children registered with CS, CS was recorded as the underlying cause of death in 33% (495/1 496) of neonatal, 11% (85/770) of postneonatal, and 2·9% (6/210) of children 1 year of age. The main limitations of this study were the use of a secondary database without additional clinical information and the potential misclassification of exposure status. CONCLUSIONS: This study showed an increased mortality risk among children with CS that goes beyond the first year of life. It also reinforces the importance of maternal treatment that infant non-treponemal titers and the presence of signs and symptoms of CS at birth are strongly associated with subsequent mortality. OBJECTIVE: We aimed to investigate estradiol (E2) as a therapeutic drug for spinal cord injury (SCI) and elucidate the disagreement in the field about the use of this hormone after an injury. METHODS: Eleven animals underwent surgery (laminectomy at the T9-T10 levels) followed by an intravenous injection (100 µg) of an E2 bolus and the implantation of 0.5cm of Silastic tubing containing 3 mg of E2 (sham E2 + E2 bolus) immediately after the laminectomy. The SCI control animals received a moderate contusion using the Multicenter Animal SCI Study impactor device over the exposed spinal cord followed by an intravenous bolus injection of sesame oil and were implanted with empty Silastic tubing (injury SE + vehicle); treated rats received a bolus of E2 and a Silastic implant with 3 mg of E2 (injury E2 + E2 bolus). Functional locomotor recovery and fine motor coordination were assessed by the Basso, Beattie, and Bresnahan (BBB) open field test and grid-walking tests, respectively, from the acute (7 days post-injury [DPI]) to the chronic stages (35 DPI). Anatomical studies of the cord were performed using Luxol fast blue staining followed by densitometric analysis. RESULTS: As observed in the BBB open field and the grid-walking tests, E2 post-SCI did not improve locomotor function but instead increased spared white matter tissue, in the rostral region. CONCLUSION: Estradiol post-SCI, at the dose and route of administration used in this study, failed to promote locomotor recovery but partially restored spared white matter tissue.

Introducción: Durante el hallazgo arqueológico o forense de un esqueleto incompleto, con vértebras sin características identificativas, no es posible determinar el nivel regional de la columna vertebral porque existen pocas determinantes que distingan los niveles a los que pertenece una vértebra. Objetivo: Proponer una fórmula matemática para la identificación de las vértebras lumbares que no son particulares. Métodos: Se realizó un estudio observacional descriptivo en la osteoteca de la Universidad de Ciencias Médicas de Matanzas. Se analizaron todas las vértebras lumbares entre lii y liv de la colección para un total de 87 huesos. Se midieron y calcularon los índices para el cuerpo y el agujero, y se utilizó un cociente matemático para asociar ambas partes. Resultados: El diámetro transversal predominó sobre el anteroposterior en el cuerpo y en el agujero. La media del cociente entre ambos fue de 0,60 cm y permitió dividir las vértebras en 2 grupos: superior e inferior; morfológicamente, deben coincidir con las vértebras lumbares lii y liv. Conclusiones: La relación morfométrica del cociente del cuerpo-agujero puede diferenciar las vértebras lumbares que no son particulares.


Introduction: During the archaeological or forensic discovery of an incomplete skeleton, with vertebrae without identifying characteristics, it is not possible to determine the regional level of the spine because there are few determinants that distinguish the levels to which a vertebra belongs. Objective: To propose a mathematical formula for the identification of lumbar vertebrae that are not particular. Methods: A descriptive observational study was conducted in the osteotheque of the University of Medical Sciences of Matanzas. All lumbar vertebrae between LII and LIV in the collection were analyzed for a total of 87 bones. The indices for the body and hole were measured and calculated, and a mathematical quotient was used to associate both parts. Results: The transverse diameter predominated over the antero-posterior in the body and in the hole. The mean ratio between the two was 0.60 cm and allowed the vertebrae to be divided into 2 groups: upper and lower; morphologically, they must coincide with the lumbar vertebrae LII and LIV. Conclusions: The morphometric relationship of the body-hole ratio can differentiate lumbar vertebrae that are not particular.

Background: Salmonella enterica are bacteria that include more than 2,500 serovars. Most of these serovars have been linked to human foodborne illnesses, mainly related to poultry and pigs. Thus, these animals are considered the reservoirs of many Salmonella serovars and strains related to antibiotic resistance. This study aimed to determine the prevalence, serovars, ß-lactam resistance genes, and the risk factors associated with Salmonella enterica in pork commercialized in open markets of Quito city. Methods: For this, 165 pork meat samples were taken from municipal markets in three areas in the city. These samples were microbiologically processed following the ISO 6579-2014 standardized method. The polymerase chain reaction (PCR) test was used to identify Salmonella serotyping and resistance genes. Strains not identified by PCR were typed by the Kauffman White Le Minor scheme. A multivariate analysis was performed to identify risk factors associated with the presence of the microorganism. Results: Salmonella prevalence in pork was 9.1%. Identified serovars were 4, [5], 12: i:- (53.3%), Infantis (33.3%), and Derby (13.4%). Furthermore, the ß-lactam resistance genes bla CTX-M-65 could be identified in three S. infantis isolates. Multivariate analysis showed that temperature (above 8°C) and cutting surfaces (wood) presented significant association values. Conclusions: In conclusion, pork in traditional markets of Quito is contaminated with Salmonella enterica, whose main serovars pose a public health concern, and shows beta-lactam resistance.

ABSTRACT OBJECTIVE To evaluate the quality of information on gestational syphilis (GS) and congenital syphilis (CS) on the Sistema de Informação de Agravos de Notificação (SINAN-Syphilis Brazil - Notifiable Diseases Information System) by compiling and validating completeness indicators between 2007 and 2018. METHODS Overall, care, and socioeconomic completeness scores were compiled based on selected variables, by using ad hoc weights assigned by experts. The completeness scores were analysed, considering the region and area of residence, the pregnant woman's race/colour, and the year of case notification. Pearson's correlation coefficients were used to validate the scores obtained by the weighted average method, compared with the values obtained by principal component analysis (PCA). RESULTS Most selected variables presented a good or excellent degree of completeness for GS and CS, except for clinical classification, pregnant woman's level of education, partner's treatment, and child's race/colour, which were classified as poor or very poor. The overall (89.93% versus 89.69%) and socioeconomic (88.71% versus 88.24%) completeness scores for GS and CS, respectively, were classified as regular, whereas the care score (GS-90.88%, and CS-90.72%) was good, despite improvements over time. Differences in the overall, care and socioeconomic completeness scores according to region, area of residence, and ethnic-racial groups were reported for syphilis notifications. The completeness scores estimated by the weighted average method and PCA showed a strong linear correlation (> 0.90). CONCLUSION The completeness of GS and CS notifications has been improving in recent years, highlighting the variables that form the care score, compared with the socioeconomic scores, despite differences between regions, area of residence, and ethnic-racial groups. The weighted average was a viable methodological alternative easily operationalised to estimate data completeness scores, allowing routine monitoring of the completeness of gestational and congenital syphilis records.

Introducción: la calidad académica constituye una prioridad en el contexto universitario global contemporáneo. De forma gradual, se aprecia la necesidad de que las universidades evidencien su responsabilidad social por medio de instrumentos que validen su quehacer, donde se exige a dichas instituciones certificar los niveles de calidad en el desarrollo de sus procesos sustantivos como forma de rendir cuentas a la sociedad. Objetivo: diseñar un procedimiento para la medición del impacto de los programas de maestría en la Universidad de Ciencias Médica de Matanzas. Materiales y métodos: se realizó una investigación de desarrollo tecnológico, que clasifica como un estudio analítico, longitudinal prospectivo, en la Universidad de Ciencias Médicas de Matanzas, entre septiembre de 2019 y mayo de 2021. Se emplearon métodos teóricos, empíricos y estadísticos. El procedimiento metodológico empleado fue la triangulación. Resultados: se diseñó un procedimiento para medir, de modo integral, el impacto de la formación de los programas de maestrías de la Universidad de Ciencias Médicas de Matanzas, lo que permitió obtener una vía para la medición de los impactos esperados, conducentes al crecimiento intelectual y profesional de los egresados, que responde al encargo social de la universidad, sus diferentes escenarios laborales y las demandas del sector de la salud en el territorio matancero. Conclusiones: el procedimiento diseñado evidenció la necesidad y pertinencia del mismo, en virtud de la mejora continua del programa y su calidad.


Introduction: academic quality is a priority in the contemporary global university context. Gradually, it is appreciated the need for the universities to show their social responsibility by the way of instruments that validate their work, where these institutions are required to certify the quality levels in the development of their substantive processes as a way of rendering accounts to the society. Objective: to design a procedure for measuring the impact of the master's degree programs at Matanzas University of Medical Sciences. Materials and methods: a research of technological development, classified as an analytical, longitudinal prospective study, was conducted at Matanzas University of Medical Sciences, between September 2019 and May 2021. Theoretical, empirical and statistical methods were used. The methodological procedure used was triangulation. Results: a procedure was designed to measure, in a comprehensive way, the impact of the training of master's degree programs at Matanzas University of Medical Sciences. It allowed obtaining a way to measure the expected impacts, leading to the intellectual and professional growth of graduates, which responds to the social responsibility of the university, their different work scenarios and to the demands of the health sector in the territory of Matanzas. Conclusions: the designed procedure evidenced its need and relevance, for the sake of the continuous improvement of the program and its quality.

Salmonella enterica serovars cause infections in humans. S. enterica subsp. enterica serovar Infantis is considered relevant and is commonly reported in poultry products. Evaluating innovative approaches for resisting colonization in animals could contribute to the goal of reducing potential human infections. Microalgae represent a source of molecules associated with performance and health improvement in chickens. Tetraselmis chuii synthesizes fermentable polysaccharides as part of their cell wall content; these sugars are known for influencing caecal bacterial diversity. We hypothesized if its dietary administration could exert a positive effect on caecal microbiota in favor of a reduced S. Infantis load. A total of 72 one-day-old broiler chickens (COBB 500) were randomly allocated into three groups: a control, a group infected with bacteria (day 4), and a group challenged with S. Infantis but fed a microalgae-based diet. Caecal samples (n = 8) were collected two days post-infection. A PMAxxTM-based qPCR approach was developed to assess differences regarding bacterial viable load between groups. The inclusion of the microalga did not modify S. Infantis content, although the assay proved to be efficient, sensitive, and repeatable. The utilized scheme could serve as a foundation for developing novel PCR-based methodologies for estimating Salmonella colonization.

RESUMO

Neste artigo são considerada as mudanças em curso decorrentes do processo de transformação digital, em especial no contexto da Pandemia da Covid19, para discutir sobre a necessidade de garantir tanto uma transformação digital na saúde que avance na perspectiva da ampliação do acesso e da qualidade do sistema de saúde, quanto uma governança pública e a proteção dos dados pessoais das cidadãs e cidadãos. Nas seções que seguem à introdução, tratamos de cada um dos quatro aspectos do processo de transformação digital na saúde que tanto o setor público quanto o privado têm focado: a informatização dos serviços de saúde e dos processos de trabalho; a integração e troca de informações entre profissionais e serviços; o atendimento remoto; e o uso de inteligência artificial. Em cada uma dessas seções aponta-se justificativas para a implementação desses aspectos enfatizados nos sistemas de saúde, destaca-se avanços evidenciados e reflete-se sobre potenciais riscos à privacidade dos dados e ao uso comercial inadequado e lesivos às pessoas. Ao fim de cada seção são apontadas algumas sugestões que podem mitigar os riscos e aperfeiçoar a legislação vigente, em especial a Lei Geral de Proteção de Dados. Defende-se, por fim, que se construa no Brasil políticas e estratégias que combinem proteção e segurança dos dados pessoais, melhoria do sistema de saúde e desenvolvimento econômico-social sustentável e includente.
