Cleft lip and palate are anomalies of craniofacial development that require reparative surgery to provide considerable improvement in the individual's quality of life, but which can also have a negative impact on the maxilla development. The aim of this study was to understand the influence of surgical repair of cleft lip and palate on the volume of the maxillary sinus of these individuals. A narrative review was carried out using the scientific databases Pubmed and Scielo, where the articles selected by the abstract were fully read, categorized and critically analyzed. There are reports of high frequency of chronic sinusitis in patients with cleft lip and palate, especially children, that may be related to hypoplasia of the maxilla and lower volume of the maxillary sinus related to the cleft. In addition, these individuals are submitted to several surgical corrections that can also negatively influence the maxilla development due to tissue fibrosis, with suggestions for surgical techniques to reduce this interference. Thus, the patient with cleft lip and palate may present maxillary underdevelopment, which may result in hypoplasia of the maxillary sinus, and reparative surgeries can aggravate this complication. This review suggests some surgical techniques to reduce this interference, although there are few articles with clinical trials found in the literature that can explore these alternatives and correlate the results with current surgical protocols. As fissuras labiopalatinas são anomalias de desenvolvimento craniofacial que necessitam de cirurgias reparadoras para proporcionar melhora considerável à qualidade de vida do indivíduo, mas que também podem repercutir negativamente no desenvolvimento da maxila. O objetivo deste estudo foi compreender a influência das cirurgias reparadoras de fissuras labiopalatais no volume do seio maxilar desses indivíduos. Foi realizada uma revisão narrativa pelas bases de dados científicos Pubmed e Scielo, de onde os artigos selecionados pelo abstract foram lidos na íntegra, categorizados e analisados criticamente. Há relatos de alta frequência de sinusite crônica em pacientes com fissuras labiopalatais, principalmente crianças, que podem estar relacionada a hipoplasia de maxila e menor volume do seio maxilar relacionado à fissura. Além disso, esses indivíduos são submetidos a diversas correções cirúrgicas que também podem influenciar negativamente o desenvolvimento da maxila devido a fibrose tecidual, havendo sugestões de técnicas cirúrgicas para diminuir essa interferência. Então, o paciente com fissuras labiopalatais pode apresentar hipodesenvolvimento maxilar podendo ter como consequência a hipoplasia do seio maxilar e as cirurgias reparadoras podem agravar essa complicação. Essa revisão sugere algumas técnicas cirúrgicas para diminuir essa interferência, embora existam poucos artigos com ensaios clínicos encontrados na literatura que possam explorar essas alternativas e correlacionar os resultados com os protocolos cirúrgicos atuais. Oral halitosis is characterized by a foul, unpleasant breath that emanates from the oral cavity due to local or systemic conditions. Approximately 90% of offensive odors are caused by volatile sulfur compounds (VSCs). L-cysteine, used as a test solution to control bad breath, induces the formation of VSCs and serves as a preliminary rinse. The study aim was to investigate the effectiveness of L-cysteine solution in differentiating the origin of oral halitosis using a gas chromatography apparatus. Methods: In total, 37 patients with an average age of 49.56 years were evaluated and divided into two groups: halimetry before the use of L-cysteine (n= 37) and halimetry after the use of L-cysteine (n= 37). Patients over 18 years of age, without severe systemic health impairment or infectious/contagious diseases, and who did not use medicines that influenced their breath were included. Halimetry was performed using the OralCroma™ device. In the halimetry before the use of L-cysteine group, 5.40%, 5.40%, and 64.86% of the patients had high levels of sulfide, methyl mercaptan, and dimethyl sulfide, respectively. After the use of L-cysteine, 48.64%, 8.10%, and 37.84% of the patients had high levels of sulfide, methyl mercaptan, and dimethyl sulfide. In this study, L-cysteine proved to be important for the assessment of oral halitosis and effective in differentiating the origin of oral halitosis; therefore, this compound could be used for the differential diagnosis of oral halitosis origin using the OralChroma™ device.

Endodontic treatment is common in dentistry and for a correct diagnosis it is necessary to observe clinical and radiographic signs, taking into account pulp and periapical changes in the element to be treated. Studying this frequency can contribute to the literature, elucidating whether the greatest number of cases is occurring in young or elderly patients, women or men, anterior or posterior teeth. The objective of this study was to analyze the prevalence of endodontically treated teeth, through 900 panoramic radiographs. A cross-sectional study was carried out evaluating the prevalence of teeth treated endodontically from a sample of 900 panoramic radiographs, corresponding to exams performed in 2020, in a radiological clinic located in central west Brazil. The sample consisted of 540 (60%) female subjects and 360 (40%) male subjects, aged between 18 and 88 years old, being more prevalent for Individuals <31 years old with (34.9%) Sample. The prevalence of endodontically treated teeth was analyzed, the results showed 1,236 teeth in this condition, individually analyzing the upper right central incisor was the most frequent 9.22%, while for group analysis the upper teeth were 7.06%. Therefore, there was a higher prevalence of teeth treated endodontically in the maxilla, the tooth with the highest treatment index, the right upper central incisor. Women were more affected and it was observed that with increasing age, the number of teeth treated endodontically also increases. (AU)


O tratamento endodôntico é frequente na odontologia e para um correto diagnóstico é necessário observar sinais clínicos e radiográficos, levando em consideração as alterações pulpares e periapicais no elemento à ser tratado. Estudar essa frequência pode colaborar com a literatura, elucidando se o maior número de casos está ocorrendo em pacientes jovens ou idosos,mulheres ou homens, dentes anteriores ou posteriores O objetivo desse presente estudo foi analisar a prevalência de dentes tratados endodônticamente, por meio de 900 radiografias panorâmicas. Foi realizado um estudo transversal avaliando prevalência de dentes tratados endodônticamente a partir de uma amostra de 900 radiografias panorâmicas, correspondentes a exames realizados em 2020, em uma clínica radiológica situada no centro oeste do Brasil. A amostra foi constituída por 540 (60%) Indivíduos do gênero feminino e 360 (40%) do masculino, com idade entre 18 e 88 anos de idade, sendo mais prevalente para Indivíduos <31 anos de idade com (34,9%) da amostra. Foi analisada a prevalência de dentes tratados endodônticamente, os resultados mostraram 1.236 dentes nesta condição, analisando individualmente o incisivo central superiores direito foi o mais frequente 9,22%, enquanto para análise de dentes por grupos foram os dentes anteriores superiores 7,06%. Portanto, houve uma maior prevalência de dentes tratados endodônticamente em maxila, o dente com maior índice de tratamento o incisivo central superior direito. Mulheres foram mais acometidas e observou-se que com o aumento da idade, aumenta-se também o número de dentes tratados endodônticamente. (AU)

The objectives are to identify volatile sulphur compounds (VSCs) in individuals with Chronic Kidney Disease (CKD) and to relate quality of life and oral health. A case-control study with 32 individuals with CKD in haemodialysis in the study group (SG) and 32 healthy individuals in the control group (CG) was performed. The VSCs were identified by gas chromatograph before (BC) and after cysteine (AC) mouthwash and an organoleptic test. For oral health assessment, oral health index was used. For quality of life, the Oral Health Impact Profile (OHIP-14) and Medical Outcomes Study 36-Item Short Form Health Survery (SF-36) were used. The hydrogen sulphide AC, methyl mercaptan AC, tongue coating, dry mouth, plaque index, and DMFT were significantly higher in the SG. VSCs did not negatively affected the impact of oral health on the quality of life in the SG but did in the CG. As for the impact of general health on quality of life, methyl mercaptan BC and AC worsened overall health in vitality and mental health, respectively. Individuals with CKD have more halitosis than healthy individuals, and it is more related to methyl mercaptan. The halitosis worsened the general quality of life of individuals with CKD. The diagnosis and identification of the origin of halitosis is important to prevent one more factor that it worsened the global quality of life of patients with CKD.

BACKGROUND: Halitosis is a condition that affects 50% of adults and one third of patients with chronic kidney disease. The aim of this study was to evaluate oral manifestations and volatile sulfur compounds (VSCs) associated with halitosis and quality of life in pre-kidney transplantation candidates. MATERIAL AND METHODS: The organoleptic test (OT) and halimetry (HA) (before and after cysteine mouthwash) were performed in patients with the Oralchroma® device, stimulated (SE) and non-stimulated sialometry (SN), Tongue Coating Index (TCI). The OHIP-14 questionnaire was administered to assess the impact of oral health on quality of life. RESULTS: Fourteen individuals with a mean age of 49.64 ± 13.35 years were evaluated. In the organoleptic test, 57.14% of the individuals presented halitosis. Oralchroma results showed that dimethyl sulfide was above the threshold in 85.71% of the individuals, while hydrogen sulfide and methyl mercaptan were above threshold in 28.57%; after the use of cysteine, hydrogen sulfide was present in 100% of the cases, dimethyl sulfide in 57.14% and methyl mercaptan in 50%. In the non-stimulated sialometry, 57.14% of the individuals presented hyposalivation and 21.42% in the stimulated. Regarding the tongue coating index, 100% of the individuals presented tongue coating, with a mean of 7.64. The assessment of impact of oral health on quality of life showed a negative impact in all dimensions. CONCLUSIONS: Tongue coating, in association with hydrogen sulfide, was the main cause of halitosis in the study subjects, and hyposalivation may contribute to higher tongue-coating indices. These oral changes negatively affect the quality of life for pre-kidney transplantation patients. Key words:Halitosis, chronic kidney disease, quality of life, kidney transplantation.

Pessoascom necessidades especiais (PNE) representam 10% da população mundialesuas particularidades,somadasàfalta de conhecimentosteóricose experiência clínica,fazemcom que os cirurgiões-dentistas se sintam inaptos para atendê-los. Sendo assim, o objetivo desse estudo foi identificar a importância percebida pelos estudantes de Odontologia sobre a disciplina Odontologia para Pacientes com Necessidades Especiais(OPNE). Trata-se de um estudo exploratório, descritivo e qualitativo, que realizou a coleta de dados por meio de questionáriosaplicados a29estudantes ao final da disciplinaoptativade OPNE da Faculdade de Odontologia de Bauru (FOB). Os estudantes reconhecem que a ausência de contato com PNE durante o cursoacarretará insegurança profissional para realizar o atendimento odontológico desses pacientes, dificultando assim o acesso a saúde bucal desta população. A disciplina de OPNE, neste estudo, possibilitouao estudantede aprender, conviver e realizar atendimento odontológico adequado e humanizado ao PNE (AU).


People with special needs (PSN) represent 10% of the world population and their particularities, added with the lack of theoretical knowledge and clinical experience, make dentists feel unable to attend to them.Therefore, the objective of this study was to identify the importance of dentistry students in the discipline of dentistry for patients with special needs (DPSN).This is an exploratory, descriptive, and qualitative study, for which data were collectedthrough a questionnaire administeredto 29students at the end of the DPSNdiscipline atthe Bauru School of Dentistry(BSD).Students recognizedthat the lack of contact with PSN during the course may cause professional insecurity in providingdental care to these patients, thus making access to oral healthcaredifficult for this population. The DPSN discipline in this study enabled studentsto learn, live with,and provide adequate and humanized dental care to the PSNpopulation (AU).

BACKGROUND: The use of monoethanolamine oleate 5% is effective for the treatment of vascular malformations with low blood flow. OBJECTIVES: To report a case series of vascular malformations in the mouth and oral cavity treated with monoethanolamine oleate 5%. METHODS: A retrospective descriptive study was performed in electronic patient charts covering seven years. Patient demographics, diagnostic resources, lesion site, size, and number of applications of monoethanolamine oleate 5% were collected. RESULTS: A total of 21 vascular malformations were recorded, located mostly on the lower lip (52.3%) and resolved in a single application in 14 patients. The authors found 19 patients treated with sclerotherapy. Thirteen were women and six were men, with a mean age of 61 years. STUDY LIMITATION: Small sample size. CONCLUSIONS: Sclerotherapy is an effective treatment for vascular malformations of the lips and oral cavity, with resolution after only one or two applications (n=16).

Objectives: Osteoradionecrosis (ORN) is one of the most severe complications resulting from radiotherapy (RT) in patients with head and neck cancer (HNC). It is characterized by persistent exposed and devitalized bone without proper healing for greater than 6 months after a high dose of radiation in the area. To describe the profile and dental management of ORN in HNC patients undergoing RT in an oncological clinical research center. Materials and Methods: A retrospective descriptive study was performed to analyze dental records from HNC patients with ORN treated at an oncological clinical research center from 2013 to 2017. A total of 158 dental records for HNC patients were selected from a total of 583 records. Afterwards, this number was distributed to three examiners for manual assessments. Each examiner was responsible for selecting dental records that contained an ORN description, resulting in 20 dental records. Results: Mean patient age was 60.3 years with males being the most affected sex (80.0%). The most affected area was the posterior region of the mandible (60.0%) followed by the anterior region of the mandible (20.0%) and the posterior region of the maxilla (10.0%). The factors most associated with ORN were dental conditions (70.0%) followed by isolated systemic factors (10.0%) and tumor resection (5.0%). There was total exposed bone closure in 50.0% of cases. The predominant treatment was curettage associated with chlorhexidine 0.12% irrigation (36.0%). Conclusion: Poor dental conditions were related to ORN occurrence. ORN management through less invasive therapies was effective for the closure of exposed bone areas and avoidance of infection.

Os indivíduos sistemicamente comprometidos e com múltiplas comorbidades são um desafio para a abordagem cirúrgica. Pacientes com flutter atrial fazem uso de anticoagulantes para diminuir o risco de eventos tromboembólicos, porém há maior risco de hemorragia durante procedimentos cirúrgicos. As infecções de origem odontogênica podem ter disseminação hematogênica e agravar ainda mais o quadro clínico geral do indivíduo. A abordagem cirúrgica deve sempre visar à segurança e à qualidade de vida do paciente, levando em consideração os riscos e benefícios do procedimento e o preparo do profissional frente a possíveis acidentes e complicações, haja vista o risco de mortalidade perioperatória. O presente caso relata o desafio de estabelecer plano de tratamento e conduta diante de infecção odontogênica em indivíduo cardiopata grave polimedicado ASA IV com histórico de doença renal crônica sob hemodiálise, hipertensão arterial sistêmica, diabetes mellitus, anemia, insuficiência coronariana, flutter atrial, aterosclerose e pé diabético. Para o procedimento sob anestesia geral, foi instituída a terapia de ponte prévia, utilização de medidas hemostáticas locais e antibioticoterapia pré e pós-operatória. A atuação interdisciplinar mostrou-se essencial para estabelecer critérios de segurança e decidir o melhor momento para a atuação cirúrgica


Systemically compromised individuals with multiple comorbidities represent a challenge in terms of the surgical approach. Patients with atrial flutter take anticoagulants to reduce the risk of thromboembolic events, but there is a greater risk of hemorrhaging during surgical procedures. Infections of odontogenic origin may involve hematogenous dissemination and further aggravate the patient's overall clinical condition. The surgical approach should always target the patient's safety and quality of life, considering the risks and benefits of the procedure and the professional's preparedness to cope with possible accidents and complications, given the risk of perioperative mortality. This is a case report of the challenge of establishing a treatment plan and course of action for dental infection in an ASA IV polymedicated patient with a history of chronic kidney disease undergoing hemodialysis, systemic arterial hypertension, diabetes mellitus, anemia, coronary insufficiency, atrial flutter, atherosclerosis and diabetic foot. Prior bridge therapy, use of local hemostatic measures, and pre- and postoperative antibiotic therapy were instituted for the procedure to be carried out under general anesthesia. Interdisciplinary action has proved to be essential for establishing safety criteria and deciding on the best time for surgical intervention
