OBJECTIVE: To determine the factors associated with impostor syndrome in medical students from six regions of Peru. MATERIAL AND METHODS: A multicentre, cross-sectional study was conduced on students from first to the sixth year in six Peruvian regions. Sociodemographic, academic, and psychological characteristics were included through the Depression, Anxiety, and Stress Scale-21, the Rosenberg Self-Esteem Scale and the Clance Imposter Phenomenon Scale. Generalised linear models were performed using crude and adjusted estimated prevalence ratios. RESULTS: Of 2,231 medical students, 54.3% were female and 30.6% had the impostor phenomenon. An association was found between the PI and those who suffered from depression (aPR=1.51; 95%CI, 1.27-1.79), anxiety (aPR=2.25; 95%CI, 1.75-2.90), stress (aPR=1.37; 95%CI, 1.19-1.57), and being female (aPR=1.12; 95%CI, 1.01-1.26). CONCLUSIONS: Three out of 10 medical students suffer from PI; having some level of depression, anxiety, stress, being a woman, and/or attending the fourth academic year were predisposing factors for their development.

Objetivo: Determinar los factores asociados con el síndrome del impostor (IP) en estudiantes de Medicina de 6 facultades peruanas. Material y métodos: Estudio multicéntrico transversal analítico realizado en estudiantes del primer al sexto año en 6 regiones peruanas. Se incluyeron características sociodemográficas, académicas y psicológicas mediante la escala de depresión, ansiedad y estrés, la escala de autoestima de Rosenberg y la escala del Fenómeno del Impostor de Clance. Los modelos lineales generalizados se construyeron mediante razones de prevalencia estimada brutas y ajustadas. Resultados: De 2.231 estudiantes de Medicina, el 54,3% eran mujeres y el 30,6% padecía IP. Se encontró asociación entre el IP y la depresión (RPa = 1,51; IC95%, 1,27-1,79), la ansiedad (RPa = 2,25; IC95%, 1,75-2,90), el estrés (RPa = 1,37; IC95%, 1,19-1,57) y el sexo mujer (RPa = 1,12; IC95%, 1,01-1,26). Conclusiones: De cada 10 estudiantes de Medicina, 3 sufren IP; tener depresión, ansiedad o estrés, ser mujer y/o cursar el cuarto anno fueron los factores predisponentes.

Aquaculture parasite biodiversity dependents on multiple environmental characteristics, including water quality. The analysis of this relationship aims to support improvements in the production management of tilapia. For this purpose, a total of 100 juvenile fishes (Oreochromis spp.) and 30 water samples were collected at Valle del Mezquital in the Central-Eastern socioeconomical region of Mexico. A study of parasite biodiversity was carried out and water quality parameters were determined. Biodiversity in the habitat was measured using the Simpson diversity index, which considers the number of species present and the abundance of each one; we also calculate the Berger-Parker index to estimate the proportional importance of the most abundant species. In general, it was found that 86% of the examined specimens were parasitized. Parasite biodiversity was 11 genera (Simpson index = 0.55). Trichodina spp. (Ciliophora) was the dominant genus (Berger-Parker index = 0.51). The protozoa Apiosoma spp. was associated with the water hardness (Berger-Parker index = 0.57). Furthermore, the presence of monogeneans showed a positive correlation with the levels of nitrites and ammonium in the water (Berger-Parker index = 0.06-0.55). This characterization may represent a useful tool in the comprehensive management of parasites that affect the farmed tilapia. However, new data is necessary to expand the knowledge about the environment-host-parasite relationship.

Tilapia has a high socioeconomic value in many countries worldwide. However, it has been identified as a zoonotic parasite reservoir. A systematic literature search and meta-analysis were carried out in order to estimate the global prevalence of zoonotic parasites that affect tilapia. The search was performed by three field experts to avoid reviewer bias. Polled prevalence was estimated using a logistic-normal random-effect regression model in the R software. We dealt with the heterogeneity among studies through subgroup analysis, taking into account the continent, country, genus of the host, parasite taxonomic group, sample origin, and type of diagnostic test as moderator variables. Fifty-two eligible articles were identified covering five tilapia genera with a pooled prevalence of 0.14 (95% CI: 0.10−0.20) showed significant heterogeneity (I2 = 98.4; p < 0.001). The subgroup analysis revealed that the most affected host was Sarotherodon, with a prevalence of 0.42 (95% CI: 0.22−0.65). Cestode was the taxonomic group with the largest prevalence (0.40; 95% CI:0.32−0.48), followed by amoeba (0.24; 95% CI: 0.16−0.35) and nematode (0.22; 95% CI: 0.11−0.38), among which, Schyzocotyle spp., Opistorchis spp., Gnathostoma spp. and Vermamoeba spp. have an impact on public health. Significant differences (p < 0.004) were found among continents and countries, with the highest value of prevalence detected in the African continent (0.28; 95% CI: 0.20−0.37), specifically in Tanzania (0.56; 95% CI: 0.22−0.87) and Egypt (0.43; 95% CI: 0.20−0.55). The origin of samples had a significant effect (p < 0.0001) on the detected prevalence, especially from those that showed the highest prevalence (0.24; 95% CI: 0.17−0.33). Finally, there were no differences in prevalence according to the diagnostic test (p = 0.97). Our results provide useful information on the development of epidemiological programs for the control of zoonoses associated with parasites in tilapia and in the design, planning, and implementation of future research.

RESUMO

SARS-CoV-2 is an emerging infectious disease of zoonotic origin that caused the coronavirus disease in late 2019 and triggered a pandemic that has severely affected human health and caused millions of deaths. Early and massive diagnosis of SARS-CoV-2 infected patients is the key to preventing the spread of the virus and controlling the outbreak. Lateral flow immunoassays (LFIA) are the simplest biosensors. These devices are clinical diagnostic tools that can detect various analytes, including viruses and antibodies, with high sensitivity and specificity. This review summarizes the advantages, limitations, and evolution of LFIA during the SARS-CoV-2 pandemic and the challenges of improving these diagnostic devices.

BACKGROUND: Metabolic Syndrome (MetS) is a clinical entity that has been linked to several non-communicable diseases. There are various consensuses to determine its presence, such as the IDF, ALAD, Harmonized, AHA/NHLBI, NCEP-ATP III or AACE criteria. However, there is currently no standardization to properly identify it. OBJECTIVE: To assess the diagnostic concordance between different criteria for MetS in Peruvian adults undergoing bariatric surgery. METHODS: We conducted a secondary analysis of the institutional database of a bariatric clinic located in Lima, Peru. We obtained data from adults between 18-59 years who underwent bariatric surgery (Roux-en-Y Gastric Bypass or Sleeve Gastrectomy). According to the Kappa coefficient, a heatplot was designed to analyze the concordance of the criteria. RESULTS: An almost perfect concordance was found between all criteria except AACE. The highest kappa coefficient (κ = 0.980) was recorded between the IDF and ALAD criteria using all the sample. Similar results were obtained when we stratified by sex. CONCLUSIONS: This study shows that, excluding the AACE, different criteria for metabolic syndrome could be used in Latino adults undergoing bariatric surgery with similar results. Given the postoperative implications, we believe that IDF and ALAD would be the best options in our population.

Aquaculture is an important source of food and livelihood for hundreds of millions of people around the world, however, aquaculture systems are affected by different factors, among them the appearance of resistant or multiresistant bacteria to antimicrobials. The secondary metabolites of plants have been proposed as alternatives for the treatment of these bacteria. The aim of the present study was to determine the antibacterial activity of Caesalpinia coriaria fruit hydroalcoholic extract and gallic acid over Aeromonas hydrophila, Aeromonas veronii,  and Aeromonas dhakensis to identify new molecules for the treatment of diseases caused by Aeromonas spp. The C. coriaria fruit hydroalcoholic extract (HECc) was obtained by hydroalcoholic maceration and subjected to bipartition with ethyl acetate and water to obtain an aqueous fraction (Ac-FrCc) and an organic fraction (Ac-FrEtCc); gallic acid was purchased commercially. The Minimum Inhibitory Concentration (MIC), Minimum Bactericidal Concentration (MBC), MBC/MIC ratio, and cytotoxicity of HECc, its fractions, and gallic acid were determined. The results indicate that HECc fractions (Ac-FrCc and Ac-FrEtCc) and gallic acid have bactericidal activity against A. hydrophila and A. dhakensis, but only gallic acid showed bactericidal activity against A. veronii. The HECc and Ac-FrCc showed no toxicity, Ac-FrEtCc showed low toxicity, and gallic acid showed medium toxicity. The HECc, Ac-FrCc, and Ac-FrEtCc may be alternatives for the treatment of diseases caused by the genus Aeromonas, however, in vivo assays are necessary to corroborate these results.

We study the kinetics of eosinophils during the development of the cellular infiltrate surrounding the nurse cell of Trichinella spiralis (T. spiralis) in experimentally infected mice. Male CD1 mice were experimentally infected with 50 viable muscle larvae of the MSUS/MEX/91/CM-91 T. spiralis strain. Tongues and diaphragms were obtained daily from days 13 to 39 post infection. Diaphragms were compressed and subjected to Giemsa stain. Tongues were histologically sectioned and stained with erythrosine B or hematoxylin and eosin. The cellular infiltrate and the nurse cell-larva complex were detected by optical microscopy since day 16 post infection. The size of the larva increased exponentially during the course of the infection. The kinetics of eosinophils showed a multimodal trend, with a bimodal predominance. The maximum peaks were reached on days 21 and 27 post infection. The results of this study demonstrate that eosinophils occur abundantly in two transcendent moments of the T. spiralis life cycle: first, when the stage 1 larva invades the myocyte and second when the nurse cell-larva complex has been fully developed. These results help one to understand the immunobiology of T. spiralis, highlighting the importance of eosinophils in the survival of the larva in skeletal muscle. Further studies are needed to characterize the cell populations that comprise the cellular infiltrate during the development of the mother cell.

Wild boars (Sus scrofa) were introduced in Mexico for sport hunting and meat trading for human consumption, but the available data regarding their role in pathogen transmission are limited. This research and field work aimed to identify the helminths of the wild boar produced in three units of conservation management and sustainable use of wildlife placed in the eastern economic region of Mexico. Samples of feces and serum were collected from 90 animals that came from three different ranches. Stool examination and antibody determination to Fasciola hepatica, Taenia crassciceps, Ascaris suum, Toxocara canis (ELISA), and Trichinella spiralis (Western blot) were performed. In addition, 30 diaphragm samples from one ranch were obtained for artificial digestion. Eggs of Strongyloides sp. (72.2%), Metastrongylus sp. (57.7%), Oesophagostomum sp. (53.3%), and Trichuris sp. (37.7%) were found in addition to oocysts of Eimeria sp. (75.6%). Antibodies to Fasciola (8.9%), Taenia (4.4%), Ascaris (32.2%), Toxocara (20%), and Trichinella (5.5%) were found. The eggs of Strongyloides and Oesophagostomum were associated to female hosts. One nematode larva was found by artificial digestion. This is the first report to identify helminths from wild boars in Mexico. In addition, this study identifies the potential risk of the wild boar as a transmission channel of parasites that can have an impact on public health.

Nematodes of the Anisakidae family have the ability to infest a wide variety of aquatic hosts during the development of their larval stages, mainly marine mammals, aquatic birds, such as pelicans, and freshwater fish, such crucian carp, these being the hosts where the life cycle is completed. The participation of intermediate hosts such as cephalopods, shrimp, crustaceans and marine fish, is an important part of this cycle. Due to morphological changes and updates to the genetic information of the different members of the family, the purpose of this review was to carry out a bibliographic search of the genus and species of the Anisakidae family identified by molecular tests, as well as the geographical area in which they were collected. The Anisakidae family is made up of eight different parasitic genera and 46 different species. Those of clinical importance to human health are highlighted: Anisakis pegreffi, A. simplexsensu stricto, Contracaecumosculatum, Pseudoterranova azarazi, P. cattani, P. decipiens and P. krabbei. The geographical distribution of these genera and species is located mainly in the European continent, Asia and South America, as well as in North and Central America and Australia. Based on the information collected from the Anisakidae family, it was determined that the geographical distribution is affected by different environmental factors, the host and the ability of the parasite itself to adapt. Its ability to adapt to the human organism has led to it being considered as a zoonotic agent. The disease in humans manifests nonspecifically, however the consumption of raw or semi-raw seafood is crucial information to link the presentation of the parasite with the disease. The use of morphological and molecular tests is of utmost importance for the correct diagnosis of the genus and species of the Anisakidae family.

Abstract This work aimed to investigate the prevalence of Trichinella infection in horses (Equus ferus caballus) handled by rural slaughterhouses across five distinctive socioeconomic regions in Mexico. Serum samples were obtained by non-probabilistic convenience sampling in the Eastern, Southern Central and Western regions (100 samples of each). Additionally, muscle tissue samples were collected from the East (n = 45), Southeastern (n = 88), Southern Central (n = 39) and Southwestern (n = 11) regions. Antibodies were determined by Western blot and the muscle tissue was examined by artificial digestion. A global antibody prevalence of 2% was obtained. Regionally, a prevalence of 5% was observed in the East and 1% in the Southern Central region. No antibodies were detected in the West region and no larvae were found in the muscle tissue samples. These findings support the low presence of Trichinella in Mexican horses, which can positively impact the Mexican horse meat trade.


Resumen Se investigó la prevalencia de Trichinella en caballos (Equus ferus caballus) demataderos rurales en cinco regiones socioeconómicas de México. Los sueros se obtuvieron por Trichinella muestreo de conveniencia no probabilístico en las regiones Este, Centrosur y Oeste (100 muestras de cada una). Además se colectaron muestras de tejido muscular de animales de las regionesEste (n = 45), Centro Sur (n = 39), Sudeste (n = 88) y Sudoeste (n = 11). Los anticuerpos se determinaron por Western blot y el tejido muscular se examinó por digestión artificial. Se obtuvo una prevalencia global de anticuerpos del 2%. A nivel regional se observó una prevalencia del 5% enel Este y del 1% en el Centrosur. No se detectaron anticuerpos en el Oeste y no se encontraron larvas en el tejido muscular. Los datos respaldan la baja presencia de Trichinella en caballos mexicanos, lo que puede impactar positivamente en su comercio.

ABSTRACT Objective: To estimate the prevalence of depression and abuse and identify the associated factors in medical interns in Peruvian hospitals. Methods: We conducted a multicentre, cross-sectional and analytical study during the months of March to May 2016. We included medical interns from 18 Peruvian hospitals, in Lima and provinces. We used a survey that included sociodemographic data and a scale to measure the perception of abuse (psychological, physical and sexual). In addition, we used the Spanish version of the PHQ-9 to evaluate depression. We used Poisson regression with robust variances to calculate prevalence ratios (PRs). Results: A total of 402 medical interns participated in the study, the median age was 25 [IQR: 21-33], and 52.7% were male. Moreover, 25.4% of them suffered from depression. In the adjusted model, the variables associated with depression were age (PRa = 1.15), the number of hours they sleep per day (PRa = 1.23), being a woman (PRa = 3.33), performing the internship in a province (PRa = 0.25), studying at a public university (PRa = 0.64), living with parents (PRa = 0.65) or alone (PRa = 0.33), and having perceived some type of abuse (PRa = 1.07). Conclusions: A quarter of surveyed medical interns had depression. Early screening is necessary to identify perceived abuse and other triggering depression factors that may adversely affect work performance and overall mental health during medical internships.


RESUMEN Objetivo: Estimar la prevalencia de depresión y maltrato e identificar sus factores asociados en los internos de Medicina de los hospitales peruanos. Métodos: Estudio multicéntrico, transversal y analítico, llevado a cabo durante los meses de marzo a mayo de 2016. Conformaron la población los internos de Medicina en 18 sedes hospitalarias de Lima y provincias. Se aplicó una encuesta que incluía características sociodemográficas y una escala para medir la percepción de maltrato (psicológico, físico y sexual). Asimismo, para evaluar la depresión, se usó el cuestionario PHQ-9 en su versión en español. Se usó regresión de Poisson con varianzas robustas para calcular las razones de prevalencia (RP). Resultados: Participaron 402 internos de Medicina; la mediana de edad fue 25 [intervalo intercuartílico, 21-33] años y el 52,7% eran varones. Además, el 25,4% padecía depresión. En el modelo ajustado, las variables asociadas con padecer depresión son edad (RPa = 1,15), cuántas horas duermen durante el día (RPa = 1,23), ser mujer (RPa = 3,33), realizar el internado en provincia (RPa = 0,25), estudiar en una universidad pública (RPa = 0,64), vivir con sus padres (RPa = 0,65) o solo (RPa = 0,33) y percibir algún tipo de maltrato (RPa = 1,07). Conclusiones: La cuarta parte de los internos encuestados tenían depresión. Es necesario realizar un cribado temprano para identificar posibles maltratos percibidos y otros factores desencadenantes de depresión, y así evitar repercusiones negativas en el desempeño laboral y la salud mental general durante el internado médico.

Anisakisspp. nematodes are potentially zoonotic parasites; that infects a wide variety of aquatic species worldwide, with marine fish being the paratenic hosts. The aim of study was identify the presence of Anisakidae nematodes, and other parasites in Mugil curema . A total of 96 M . curema obtained from local markets in Tulancingo, Hidalgo, Mexico, were analyzed by necropsy. Only five M . curema present nematode collection in epaxial muscle. The tissues with the highest prevalence of parasites were identified, and samples of epaxial muscle with larval migration analyzed by histopathology. Visible parasites in necropsy tissues were classified according to their morphology. In one fish, Clinostomum spp. was detected in epaxial caudal muscle. The present work reports for the first time the presence of nematodes of the family Anisakidae and Clinostonum spp. metacercariae, with zoonotic potential, in M . curema intended for human consumption in Tulancingo, Hidalgo, Mexico.

Abstract Anisakisspp. nematodes are potentially zoonotic parasites; that infects a wide variety of aquatic species worldwide, with marine fish being the paratenic hosts. The aim of study was identify the presence of Anisakidae nematodes, and other parasites in Mugil curema . A total of 96 M . curema obtained from local markets in Tulancingo, Hidalgo, Mexico, were analyzed by necropsy. Only five M . curema present nematode collection in epaxial muscle. The tissues with the highest prevalence of parasites were identified, and samples of epaxial muscle with larval migration analyzed by histopathology. Visible parasites in necropsy tissues were classified according to their morphology. O objetivo deste estudo foi identificar a presença de nematóides da família Anisakidae e de outros parasitas em peixes Mugil curema . Um total de 96 M . curema , obtidos em mercados locais em Tulancingo, Hidalgo, México, foram submetidos a necropsia. Apenas cinco M. curema apresentaram coleção de nematóides no músculo epaxial. Coccidiosis in rabbit production is responsible for high morbidity, mortality, and economic losses. The use of natural antimicrobial substances in rabbits represents a promising way to improve their health and production. The aim of the present study was to assess the activity of Salix babylonica hydroalcoholic extract (SBHE) on the elimination of Eimeria spp. in rabbits. The phytochemical compounds and chemical composition of SBHE were determined. The cytotoxicity of SBHE was determined by a microwell assay using Artemia salina. Twenty-five New Zealand rabbits, 28 days old and 872 ± 171 g body weight (BW), were used in a completely randomized design. The rabbits were assigned to five groups of five rabbits, control group (A) received only basal diet (BD), B group received BD + 25 mg/kg BW of SBHE, C group received BD + 50 mg/kg BW of SBHE, D group received BD + 100 mg/kg BW of SBHE, and E group received BD + coccidiostat Baycox® (75 mg/kg body weight) for 28 days. Feces samples were collected at days 0, 7, 14, 21, and 28; morphological and morphometric identifications of Eimeria were carried out by the flotation technique and counting of oocysts by the McMaster technique. The rabbits were found naturally infected with Eimeria spp. The SBHE present phytochemicals with anticoccidial activity, and the cytotoxicity test indicate that this extract is nontoxic. This study demonstrates that oral administration of SBHE at 25 and 50 mg/kg BW reduced the release of oocysts per gram of feces. This effect was observed at day 14 and had the most significant effect at day 28 for both concentrations. The results indicate that SBHE could be a natural alternative for the control of coccidiosis in rabbit production.

The members of the Aeromonas genus are important foodborne pathogens, with a worldwide distribution. Wild rainbow trout, from the national protected area Santuario del Agua State Park, Corral de Piedra, were analyzed. Species of Aeromonas were isolated from the trout, and their pathogenic potential was analyzed based on different pathogenicity and virulence factors. The isolates were identified as A. allosaccharophila (n = 15), A. sobria (n = 8), A. veronii (n = 3), A. rivipollensis (n = 2), A. piscicola (n = 2), and A. popoffii (n = 1), by RNA polymerase sigma factor (rpoD) gene sequencing. Sequence similarity with the type strain was 92.2 to 99.6% for A. sobria isolates, 97.8 to 98.0% for A. allosaccharophila isolates, 99.2% for the A. popoffii isolate, 99.2 to 100% for A. piscicola isolates, and 98.2 to 99.2% for A. veronii isolates. Notably, isolates A30T2-gills and A30T2-spleen showed sequence similarity of 98.0% with strain A. media CECT 4232T and 99.0% with strain A. rivipollensis P2G1T. Virulence genes were detected by PCR at the following frequencies: fla and serine protease, 96.77%; aerA, 93.54%; aexT, 87.09%; lipases, 74.19%; ascV and ahyB, 67.74%; exu, 61.29%; act, 41.93%; ascF-G, 38.70%; lafA, 32.26%; alt, 6.46%; aopP, 9.67%; and ast, 3.23%. These results indicate that several Aeromonas species had the potential pathogenicity to infect wild rainbow trout in the waterway created by the Corral de Piedra dam, suggesting they could be an emerging zoonotic pathogen.

The bacterium Ornithobacterium rhinotracheale is associated with respiratory disease in wild birds and poultry. In this study, the phylogenetic analysis of nine reference strains of O. rhinotracheale belonging to serovars A to I, and eight Mexican isolates belonging to serovar A, was performed. The analysis was extended to include sequences from another 23 strains available in the public domain. The analysis showed that the 40 sequences formed six clusters, I to VI. All eight Mexican field isolates were placed in cluster I. One of the reference strains appears to present genetic diversity not previously recognized and was placed in a new genetic cluster. In conclusion, the phylogenetic analysis of O. rhinotracheale strains, based on the 16S rRNA gene, is a suitable tool for epidemiologic studies.
