RESUMO

ADHD is a neurodevelopmental disorder appearing in childhood but remaining in many cases in adults. There are both pharmacological and non-pharmacological approaches to treating ADHD, but they do not have the same efficacy in all subjects. Better knowledge of the neurophysiological basis of this disorder will allow for the design of more effective treatments. Studies performing qEEG analysis in children suggest the existence of subgroups of ADHD patients with different neurophysiological traits. There are fewer studies in adults, who might have undergone plastic changes allowing them to cope with ADHD symptoms along with brain maturation. Herein, we study cognitive performance and the theta/beta ratio in young adults with ADHD symptoms. We found that subjects with ADHD symptoms and low working memory performance (n = 30) present higher theta/beta ratios than controls (n = 40) at O2 and T6 in the eyes-closed condition, as well as a tendency toward a higher theta/beta ratio at O1 and Cz. Subjects with ADHD and high working memory performance (n = 50) do not differ from the controls in their theta/beta ratios at any derivation. Our results suggest that neuropsychological profiling could be useful for patient subgrouping. Further research will allow for the distinction of neuropsychological profiles and their neurophysiological correlates, leading to a better classification of ADHD subtypes, thus improving treatment selection.

INTRODUCTION: The purpose of this study is to report a case of venous-air embolism during a vitrectomy for endoresection of choroidal melanoma. CASE DESCRIPTION: A 31-year-old man went to the clinic because of photopsias and vision loss in his right eye. On fundoscopy of the right eye, a choroidal mass with an associated retinal detachment was found near the inferotemporal vascular arcade. Multimodal imaging was performed and diagnosis of choroidal melanoma was made. Metastatic workup ruled out systemic extension. The patient underwent pars plana vitrectomy for endoresection of the lesion. During the application of laser under air, he started complaining of chest pain and dyspnea. He presented signs of supraventricular tachycardia, tachypnea, hypotension and oxygen desaturation. He was managed with orotracheal intubation, bronchodilators and vasopressor support, and stabilization was achieved. He was discharged 2 days after with no sequalae. After 1-year of follow-up, the patient has a visual acuity of counting fingers and no signs of tumor recurrence or systemic extension. CONCLUSIONS: Although rare, vitreoretinal surgeons should be aware of this potentially fatal complication and take steps to prevent it.

Intensive research on biodegradable films based on natural raw materials such as carboxymethyl cellulose (CMC) has been performed because it enables the production of transparent films with suitable barrier properties against oxygen and fats. Considering the importance of the production of this type of film at the industrial level, a scalable and continuous drying method is required. Refractance window-conductive hydro drying (RW-CHD) is a sustainable and energy-efficient method with high potential in drying this kind of compound. The objective of this study was to evaluate the factors (CMC thickness, heating water temperature, and film type) and radiation penetration depth that affect drying time and energy consumption. It was found that drying time decreased with increasing temperature and decreasing thickness. Similarly, energy consumption decreased with decreasing temperature and thickness. However, the drying time and energy consumed per unit weight of product obtained were equivalent when drying at any of the thicknesses evaluated. Film type had little effect on time and energy consumption compared to the effects of temperature and CMC thickness. The radiation penetration depth into the CMC was determined to be 1.20 ± 0.19 mm. When the thickness was close to this value, the radiation energy was better utilized, which was reflected in a higher heating rate at the beginning of drying.

Estimating nitrogen (N) concentration in situ is fundamental for managing the fertilization of the sugarcane crop. The purpose of this work was to develop estimation models that explain how N varies over time as a function of three spectral data transformations in two stages (plant cane and first ratoon) under variable rates of N application. A randomized complete-block experimental design was applied, with four levels of N fertilization: 0, 80, 160, and 240 kg N ha-1. Six sampling events were carried out during the rapid growth stage, where the canopy reflectance spectra with a hyperspectral sensor were measured, and tissue samples for N determination in plant cane and first ratoon were taken, from 60 days after emergence (DAE) and 60 days after harvest (DAH), respectively, until days 210 DAE and 210 DAH. To build the models, partial least squares regression analysis was used and was trained by three transformations of the spectral data: (i) average reflectance spectrum (R), (ii) multiple scatter correction and Savitzky-Golay filter MSC-SG) reflectance spectrum, and (iii) calculated vegetation indices (VIs).

Juvenile myoclonic epilepsy (JME) appears in adolescence with myoclonic, absence, and generalized tonic clonic (GTC) seizures with paroxysmal activity of polyspike and slow wave (PSW), or spike and wave (SW) complexes in EEG. Our aim was to analyze the clinical characteristics, background EEG activity, and paroxysmal events in 41 patients with JME. Background EEG activity was analyzed with visual, quantitative (QEEG), and neurometric parameters. Our JME patients started with absence seizures at 11.4 ± 1.5 years old, myoclonic seizures at 13.6 ± 2.5 years, and GTC seizures at 15.1 ± 0.8 years. The seizures presented in awakening at 7:39 h with sleep deprivation, alcoholic beverage intake, and stress as the most frequent precipitant factors. Paroxysmal activity was of PSW and fast SW complexes with 40.5 ± 62.6 events/hour and a duration of 1.7 s. Right asymmetric paroxysmal activity was present in 68.3% of patients. Background EEG activity was abnormal in 31.7% of patients with visual analysis. With QEEG beta AP (absolute power) increase and AP delta decrease were the most frequent abnormalities found. Spectral analysis showed that 48.7% of patients had normal results, and 26.83% and 24.4% had higher and lower frequencies than 10.156 Hz, respectively. We concluded that, with visual analysis, background EEG activity was abnormal in a few patients and the abnormalities increased when QEEG was used.

BACKGROUND: Cervical carcinoma (CC) is one of the most frequent neoplasms, especially in developing countries. The most common histopathological type is squamous cell carcinoma (SCC), followed by adenocarcinoma (AC) and adenosquamous carcinoma (ASC). Prognosis according to histological type is controversial. OBJECTIVE: The objective of this study is to describe and compare the prognoses of the most common histologies of CC in the early stages. MATERIALS AND METHODS: We reviewed records of patients attended at the Instituto Nacional de Cancerología of Mexico with CC surgically treated Stages IA2-IB1 and IIA1, including the histological types SCC, AC, and ASC. Patients who had another malignant neoplasm, cervical cancer in situ, locally advanced neoplasm, and metastatic neoplasm were excluded from the study. A descriptive and comparative analysis was conducted. Overall survival (OS) and disease-free period were calculated for each histological type with the Kaplan-Meier method and were compared with the log-rank test. RESULTS: A total of 202 records were obtained, of which 131 (64.9%) had SCC, 57 (28.2%) AC, and 14 (6.9%) ASC. The 5-year DFS was 94.4% for SCC, 98.1% for AC, and 92.3% for ASC, without a statistically significant difference (p = 0.55). The 5-year OS for SCC was 97.9%, for AC was 97.8%, and for ASC was 100%, without a statistically significant difference (p = 0.702). CONCLUSIONS: DFS and OS did not differ between the most common histological types of CC at the early stages.

The encephalitic alphaviruses encode nsP2 protease (nsP2pro), which because of its vital role in virus replication, represents an attractive target for therapeutic intervention. To facilitate the discovery of nsP2 inhibitors we have developed a novel assay for quantitative measurement of nsP2pro activity in a cell-based format. The assay is based on a substrate fusion protein consisting of eGFP and Gaussia luciferase (Gluc) linked together by a small peptide containing a VEEV nsp2pro cleavage sequence. The expression of the substrate protein in cells along with recombinant nsP2pro results in cleavage of the substrate protein resulting in extracellular release of free Gluc. The Gluc activity in supernatants corresponds to intracellular nsP2pro-mediated substrate cleavage; thus, providing a simple and convenient way to quantify nsP2pro activity. Here, we demonstrate potential utility of the assay in identification of nsP2pro inhibitors, as well as in investigations related to molecular characterization of nsP2pro.

La habilitación y rehabilitación son procesos a través de los cuales las personas con discapacidad tienen la oportunidad de contar y mantener óptimos niveles en los planos físico, sensorial, intelectual, psicológico y social; a pesar de la existencia de diferentes campos de habilitación y rehabilitación en la fisioterapia con todo un abanico de técnicas y procedimientos (Bobath, TFNP, Perfetti, rehabilitación basada en tareas y otras) para la intervención de estos pacientes, la hipoterapia (terapia con caballos) se perfila como una opción prometedora en el restablecimiento de las deficiencias de pacientes con afecciones neurológicas centrales y periféricas, donde el caballo juega un papel fundamental. El caballo es un animal por característica muy perceptivo, lo cual le permite graduar su comportamiento y conducta según la persona que lo monta, que en primera instancia lo muestra como una vía para su utilización con personas. Montar a caballo proporciona un trabajo físico, resistencia muscular, controles posturales y enderezamientos corporales para mantener equilibrio, balance, mantenimiento de posiciones y coordinación, también favorece la adquisición de habilidades cognitivas que permiten la interacción del jinete/amazona con su medio y por darse en espacios ambientales al aire libre facilitan el desarrollo de la persona en torno a todas sus esferas (personal, familiar y social). Es la hipoterapia una modalidad asistida con animales, que utiliza al caballo como medio facilitador en la rehabilitación o habilitación de deficiencias a nivel motor, senso-perceptivo, cognitivo, comunicativo y social, se aprovecha el paso, el carácter, la voluntad, la anatomía del animal y la planeación de ejercicios terapéuticos sobre este para que el fisioterapeuta los convierta en elementos fundamentales para el proceso de restablecimiento de la funcionalidad y funcionamiento del paciente (jinete/amazona).


The habilitation and rehabilitation are processes through which people with disabilities go through to maintain optimal results at physical, sensory, intellectual, psychological and social levels. Despite the existence of different fields of habilitation and rehabilitation in physical therapy, with a wide range of techniques and procedures (Bobath, TFNP, Perfetti, rehabilitation based on assignments and others) for the intervention of these patients, the hippotherapy (therapy with horses) is emerging as a promising option in the restoration of the deficiencies of patients suffering from central and peripheral neurological disorders, where the horse plays a fundamental role. Horses are very perceptive animals which allow them to graduate their behavior and conduct according to the person who rides them, which first shows them as a means to use with people. Horse riding provides physical work, muscle endurance, postural control and straightening body to maintain balance, positions and coordination. It also favors the acquisition of cognitive skills that allow the interaction of the rider/Amazon with its environment as well as the development of the person around all his or her areas (personal, family and social) as it is practiced outdoors. Hippotherapy is a modality assisted with animals, which uses horses as a means of facilitating the rehabilitation or habilitation of people's deficiencies in their motor, senso- perceptual, cognitive, communicative and social levels by using step, character, the will, the anatomy of the animal and the planning of therapeutic exercises while riding, so that the physical therapist makes them become fundamental elements for the process of restoring the functionality and operation of the patient (rider/Amazon).

Periventricular leukomalacia (PVL) is characterized by necrosis of the cerebral white matter in the dorsolateral portions of the lateral ventricles. PVL causes motor, sensory, and cognitive deficits. The aim of this study was to analyze the conduction characteristics of the visual pathway in infants with diffuse PVL using visual evoked potentials (VEPs). We studied 11 healthy infants (mean age 3.3 ± 1.3 months) and 17 with diffuse PVL (mean age 2.9 ± 0.8 months and mean gestational age 31.9 ± 3.1 weeks). The N75, P100, and N135 wave latencies; the interwave N75-P100 and P100-N135 latencies; and the N75-P100 and P100-N135 amplitudes were measured in the occipital leads. VEPs were recorded during binocular stimulation at an angle of 120' from the Fz-Oz lead. Healthy children had mean N75, P100, and N135 wave latencies of 84.4 ± 5.8, 143.4 ± 30.6 and 222.9 ± 40.4 ms, respectively. The mean interwave N75-P100 and P100-N135 latencies were 59.0 ± 28.6 and 79.5 ± 13.6 ms, respectively. Compared with the healthy group, infants with PVL had longer N75 and N135 latencies at 92.3 ± 15.3 (P = .05) and 265.0 ms ± 60.3 (P = .05), respectively. The interwave latency P100-N135 (105.5 ± 29.1 ms; P = .017) was longer in children with PVL than in healthy infants. Infants with diffuse PVL had mild alterations in their N75, P100 and, particularly, their N135 latencies. These increases in P100-N135 interwave latencies could be because of damage to the geniculocortical pathways and V2-V3 networks.

BACKGROUND AND AIMS: Many patients with complete spinal cord injury (SCI) exhibit demyelinated and poorly myelinated nerve fibers traversing the lesion site. Conventional doses of 4-aminopyridine (4-AP, 30 mg/day) have shown to provide no or minor functional improvement in these patients. We undertook this study to test the functional effect of high doses of 4-AP on patients with chronic complete SCI with cord continuity at the site of injury demonstrated by magnetic resonance imaging. METHODS: Fourteen patients were included in a double-blind, randomized, placebo-controlled trial followed by an open label long-term follow-up. Initially, patients received 4-AP or placebo orally, with 4-AP being increased gradually (5 mg/week) to reach 30 mg/day. For long-term treatment, 4-AP was increased 10 mg periodically according to negative electroencephalogram and blood test abnormalities and minor adverse reactions. Pre-treatment, 12 and 24 weeks of the controlled trial, and 6 and 12 months of open trial evaluations, or with the highest doses reached were obtained. RESULTS: Three of 12 patients were able to walk with the assistance of orthopedic devices, 1/12 became incomplete (AIS B), 7/12 improved their somatosensory evoked potentials, 5/12 had sensation and control of bladder and anal sphincters, and 4/9 male patients had psychogenic erection. CONCLUSIONS: Positive changes were seen mainly in patients with cyst (4/5) or atrophy (3/5) of the injury site. Two patients withdrew from the study: one had seizures and one had intolerant adverse reactions. We conclude that high doses of 4-AP in the studied population produced several functional benefits not observed using lower doses.

Periventricular leukomalacia is characterized by damage to the brain's white matter and impairments in motor function. Motor-evoked potentials by transcranial magnetic stimulation evaluate corticospinal tract function. We analyzed alterations in motor-evoked potentials in newborns with periventricular leukomalacia. Thirty infants (aged 4.37 +/- 1.1 months mean +/- S.D.) were divided into three groups: 10 healthy, and 10 with focal and 10 with diffuse periventricular leukomalacia. Potentials recorded in the right abductor pollicis brevis of healthy infants indicated a total motor conduction time of 26.3 +/- 2.4 ms, central motor conduction time of 17.0 +/- 2.6 ms, and central motor conduction velocity of 12.3 +/- 2.2 m/s. In the tibialis anterior, total motor conduction time was 27.4 +/- 2.6 ms; central motor conduction time was 16.7 +/- 2.8 ms, and central motor conduction velocity was 25.2 +/- 3.4 m/s. In the focal periventricular leukomalacia and diffuse periventricular leukomalacia groups, an increase in central motor conduction time and a decrease in central motor conduction velocity (P < 0.05) were evident, without differences between the two groups. Motor-evoked potentials in periventricular leukomalacia revealed an increase in central motor conduction time and a decrease in central motor conduction velocity, without differences between diffuse and focal types.

PURPOSE: To evaluate the number of seizures and interictal epileptiform discharges (IEDs) in patients with focal neocortical epilepsy before, during and after rTMS. METHODS: Twelve patients (seven men and five women, mean age 29.3+/-15.8 years) were studied. An open-label study with baseline (4 weeks), intervention (2 weeks) and follow-up (8 weeks) periods was carried out. Repetitive transcranial magnetic stimulation (rTMS) with 900 pulses, intensity of 120% motor resting threshold and 0.5Hz frequency was used. A 120 channel EEG was recorded; an electrical source analysis of IEDs with Variable Resolution Electromagnetic Tomography (VARETA) was performed. The number of seizures per week and IEDs per minute were measured and compared in the three periods. RESULTS: During the basal period the mean seizure frequency was 2.25 per week; in the intervention period it decreased to 0.66 per week (F=2.825; p=0.0036) which corresponds to a 71% reduction. In the follow-up period the mean frequency was 1.14 seizures per week, that is, a 50% reduction in the number of seizures. In the visual EEG analysis, the baseline IED frequency was 11.9+/-8.3events/min; it decreased to 9.3+/-7.9 during 2 weeks of rTMS with a further reduction to 8.2+/-6.6 in the follow-up period. These differences however were not significant (p=0.190). CONCLUSION: We conclude that 2 weeks of rTMS at 0.5Hz with a figure-of-eight coil placed over the epileptic focus, determined with VARETA, decreases the number of seizures in patients with focal epilepsy, without reduction in IEDs.

PURPOSE: To analyze background EEG activity of patients with juvenile myoclonic epilepsy (JME) with and without antiepileptic drugs. METHODS: We studied the background EEG activity in 18 patients with JME. The qEEG analysis included absolute power (AP), relative power (RP) and mean frequency (MF) of delta, theta, alpha and beta bands. The Z scores were calculated by comparison with population parameters based on the age-dependent regression function. Seven patients were unmedicated (UM) and eleven medicated (M). RESULTS: The UM group presented 69 (4.32%) abnormal Z scores and 227 (9.05%) in the M group (P<0.001). In the UM group, AP delta abnormal Z scores were identified in frontotemporal and occipital leads. In AP alpha and beta bands an increase in Z scores was encountered in frontoparietal leads in three patients. In addition, in three patients, the AP theta Z scores were below -1.96 and distributed in all regions. In the M group, AP beta Z scores were above 1.96 in frontoparietal leads in 7 of 11 patients. The AP delta increased above 1.96 in frontotemporal and occipital leads in 6 patients of 11. The AP alpha showed an abnormal decrease in Z scores in 5 of 11 patients, whereas other 5 patients presented normal scores. The AP theta presented 7 normal Z scores out of 11; this band exhibited the lowest number of abnormalities of the 4. CONCLUSION: Patients with JME have an increase in AP delta, alpha and beta bands, which is more evident in frontoparietal regions.

The objective of this work was to explore Neurofeedback (NFB) effects on EEG current sources in Learning Disabled (LD) children, and to corroborate its beneficial consequences on behavioral and cognitive performance. NFB was given in twenty 30-min sessions to 11 LD children to reduce their abnormally high theta/alpha ratios (Experimental Group). Another five LD children with the same characteristics received a placebo treatment (Control Group). In the Control Group no changes in behavior or EEG current source were observed. In the Experimental Group, immediately after treatment children showed behavioral and cognitive improvements, but current source analysis showed few modifications; however, 2 months after treatment many changes occurred: a decrease in current of frequencies within the theta band, mainly in left frontal and cingulate regions, and enhancement in current of frequencies within the alpha band, principally in the right temporal lobe and right frontal regions, and of frequencies within the beta band, mainly in left temporal, right frontal and cingulate cortex regions. In conclusion, NFB is a possibly efficacious treatment for LD children with an abnormally high theta/alpha ratio in any lead. The changes observed in EEG current sources may reflect the neurophysiological bases of the improvement that children experienced in their behavioral and cognitive activities.

Inadequate compliance is a major contributor to unsuccessful treatment in epilepsies. To establish risk factors associated with therapeutic non-compliance in patients with epilepsy, we carried out a case-control study, nested into a cohort, with thirteen factors possibly implicated in therapeutic non-compliance. The patient's general characteristics, the illness, and patient-practitioner relationship were studied. Patients were followed during 6 months; during this time, serum levels and pill counts were registered. Of 150 patients, 66 were non-compliers and 84 were compliers. Seven of thirteen factors were statistically different with an odds ratio greater than 3 (p < 0.05). However, after log-lineal regression analysis, only the total number of pills per day and the subject's intellectual level were significant. These two factors increase 3.66 times the risk of non-compliance. We conclude that epileptic patients with a low intellectual level and more than three prescribed pills per day have a 3.66 times greater risk of non-compliance to anti epileptic treatment.
