Background: Is fundamental to determine the prevalence of human papiloma virus (HVP) high-risk serotypes in local and regional population in order for health providers to offer patients, vaccines and treatments against specific population-based serotypes. Objetive: To determine the prevalence of HPV High risk serotypes detected by PCR in patients with normal cytology from the ISSSTE Adolfo Lopez Mateos Regional Hospital. Methods: An observational, descriptive, prospective study was conducted from cervical cytologies and high risk HPV test by PCR in patients from the Regional Hospital Adolfo López Mateos, ISSSTE, during the period January 2013-December 2015. Cases of patients with negative cervical cytology were included. Information about age, the result of cervical cytology and high risk HPV test by PCR was obtained. The overall prevalence of HPV infection and the most prevalent serotypes by age groups were calculated. Results: A total of 3258 cervical smears were performed, of which 2557 were negative (78.4%), from this, the global prevalence of HPV infection was 10.2% (n=262). We found that 1.8% (n = 45) of negative reports had HPV16 infection, 0.5% (n=13) had HPV18 and 8.9% (n = 227) were infected by Viral Pool of other high-risk serotypes. The prevalence of infection by viral pool of high risk serotypes was 11.5% in women <20 years, 12.9% in women between 20-29 years and 22.2% in women between 30-39 years. This prevalence was lower in patients older than 40 years (p<0.05). Conclusion: A higher prevalence of viral pool high risk serotypes was found in patients with normal cytology, than the HPV16 and HPV-8 prevalence, which was significantly higher in women younger than 40 years.

We describe a 72-year-old man with prostate enlargement, prostate-specific antigen level of 35 ng/dl, mild polyarthritis, and constitutional symptoms. Prostatic ultrasonography suggested neoplasm; however, transrectal biopsy revealed findings consistent with polyarteritis nodosa (PAN). The patient went on to develop leg paresthesia and dysesthesia, increased serum creatinine, and systemic hypertension. Steroids and intravenous cyclophosphamide were administered, followed by improvement. Our case emphasizes the protean onset of PAN, and provides a new differential diagnosis of prostatic diseases related to elevated prostate-specific antigen.

A prospective study was realized with the purpose of evaluating the efficiency of the excisional therapy of the electrosurgery in the treatment of the High Grade Squamous Intraepithelial Lesion (H.G.S.L.I.) of the cervix. 149 patients who were attended at the colposcopy service with this diagnosis were selected during September 1990 to April 1993. The procedure was realized in 37 patients 24.8% in the operating room and in 112 patients (75.2% in the outpatient room with local anaesthesia. With this last mentioned method in 93.8% not one type of trouble was presented, referring only a slightly to moderate pain in 6.2%. The transoperatory complications were bleeding on the surgical site in 3.3% which was solved at the moment through local electrocoagulation, and late bleeding present in 4.2% of the patients. The lesion was completely eliminated when the deepness of the cone corresponded to 1.81 cm in 139 cases (93.29%). Through a colpo-cytologic follow-up in 24 months, a persistence of the virus of the human papilloma was shown in 5.5% during the first nine months and appearance of one case of H.G.S.I.L. after 18 months. Therefore, these results had a therapeutic efficiency of the global follow-up of 90.08%.

OBJECTIVE: To correlate the findings of CT and ileoobturator lymphadenectomy in patients with localized adenocarcinoma of the prostate. METHODS: 94 patients with adenocarcinoma of the prostate were evaluated. Ileoobturator lymphadenectomy and brachytherapy were performed in 61.1%, radical prostatectomy in 22.5% and lymphadenectomy with prostatic labeling for subsequent external radiation therapy in 5%. Lymph node CT and pathology findings were correlated. RESULTS: Of 92 patients with a normal CT scan, 18 had positive nodes and 19.1% were understaged. Two patients with a CT scan suggestive of metastatic adenopathy had negative pathology findings. Seventy-two of the 92 patients with normal CT scans had negative nodes, accounting for a specificity of 76.6%. CONCLUSION: Pelvic lymph node involvement changes the prognosis of prostate cancer. However, the ability of CT to detect lymph node metastasis is limited. It is therefore not a reliable method and raises the costs of staging unnecessarily.

RESUMO

It is a well known fact that in the last decade the longevity of the population has grown, which in turn has resulted in a larger group of octogenary patients being taken to coronary artery bypass procedures. Nevertheless, older age has always been considered a predisposing factor for complications. In this paper we analyze our recent experience with octogenary patients submitted to coronary artery bypass graft surgery, comparing the morbi-mortality of this group with a comparative younger age group. We found no increase in serious complications. Thus surgery should be offered irrespective of age with acceptable survival.

OBJECTIVES: We evaluated the management of the regional lymph nodes to determine the appropriate treatment for carcinoma of the penis. METHODS: The records of 36 patients with carcinoma of the penis were reviewed. Lymphadenectomy was performed in 18 patients, 17 were managed conservatively (watchful waiting) and 1 patient had a biopsy and received radiotherapy. RESULTS: Positive nodes were found in 2 of 2 pT4, 2 of 3 pT3, 8 of 13 pT2 and 2 of 12 pT1 patients submitted to lymphadenectomy. Concerning the histological grade, positive nodes were found in all of the 4 G3, 5 of 12 G2 and 3 of 20 G1 patients. The survival rate was 100% for the patients with negative lymph nodes (pNO = 6) or a single positive inguinal lymph node (pN1 = 5). A correlation was found between the T and the histological grade and the likelihood of lymph node invasion. CONCLUSIONS: The T and the histological grade of the primary lesion must be considered when deciding the approach in the management of the lymph nodes as unnecessary lymphadenectomy can be avoided and those at high risk of lymph node invasion can be treated radically and timely.

An study with 52 patients, was performed at the colposcopy Service, Hospital López Mateos, IMSS. The patients presented with an histological diagnosis of cervical in situ carcinoma. They were, previously, submitted to a colposcopic and cytological protocol study. All the patients were treated with removal of a cervical cylinder with CO2 laser. The post-operative follow up reported: Altogether normal and inflammatory reports on cytology, colposcopy and histology, 78.2%, 89.1%, and 91.2%, respectively. Therefore, it is concluded that this treatment is adequate and with a satisfactory cure rate.

We studied the electrolyte composition, pH and osmolality of six solutions for oral rehydration available in drug stores in Venezuela, and also their combinations with whole milk in a dilution of 6.6%. The solutions such as Pedialyte, Hidramilac and Hidramines showed an acid pH (4.30-5.10) in direct relationship to the concentration of carbohydrates (5% or more). Also, Pedialyte and Hidramilac had greater osmolalities (360-365 mOsm/kg) than plasma. Some of the levels of sodium and potassium in the solutions were found to be under the concentrations given by the producers. The combination of whole milk with oral solutions diminishes the concentrations of sodium and potassium in the mixture, changing the sodium-glucose relationship, pH and osmolality. The therapeutic implications for the use of oral rehydration solutions with low concentrations of sodium, acid pH, large content of glucose and elevated osmolality are discussed. It is concluded that the combination of whole milk with Sueroral (WHO) is not adequate for the optimum absorption of sodium and therefore to prevent dehydration. Therefore, except for the WHO solution and Oralite, other solutions are not adequate for the correction of the electrolytic and acid-base alterations present in infants with dehydration secondary to acute diarrhea.

RESUMO

The third published case of gastric teratoma from Mexico, is reported. A premature newborn male infant was operated on at the age of 37 days. A 250 g. mass was excised from the anterior gastric wall together with a third of the stomach. After a 5 year follow-up the patient is asymptomatic. The characteristics of 48 additional cases collected from world literature are described. A general review of germ-cell tumors in the stomach, is also exposed.
