RESUMO

The extraction and burning of coal release genotoxic pollutants, and understanding the relationship between genetic damage and the spatial distribution of residences in coal-using regions is crucial. The study aimed to conduct a spatial analysis of genotoxic damage through the of micronuclei (MNs) number and their proximity to coal mining/burning in the largest coal exploration region in Brazil. In this study, the detection of genotoxic damage was performed using the MN assay in oral cells of residents exposed to coal mining activities. Spatial analysis was conducted using QGIS 3.28.10 based on information obtained from a questionnaire administered to the population. Multiple linear regression analysis was carried out to assess the influence of the distance from residential areas to polluting sources on the number of MNs found. Additionally, Spearman's correlation was performed to identify the strength and direction of the association between the frequency of MNs and each of the polluting sources. A total of 147 MNs were quantified among all participants in the coal mining region. Notably, residents living within 2 km and 10 km of pollution sources exhibited the highest prevalence of MNs. The analysis demonstrated a significant correlation between closer proximity to pollution sources and increased MN frequency, underscoring the spatial relationship between these sources and genotoxic damage. Environmental pollutants from anthropogenic sources present a major health risk, potentially leading to irreversible damage. The spatial analysis in this study highlights the importance of targeted public policies. These policies should aim for a sustainable balance between economic development and public health, promoting effective measures to mitigate environmental impacts and protect community health.

RESUMO

OBJECTIVE: This study evaluated the effect of administration of trans-resveratrol-containing orodispersible tablets on the protein composition of the AEP and on blood plasma trans-resveratrol concentrations. METHODS: Ten volunteers participated in two crossover double-blind phases. In each phase, after dental prophylaxis, they received a trans-resveratrol (15 mg) orodispersible tablet, or a placebo tablet (without actives). The AEP formed after 120 min was collected with electrode filter papers soaked in 3 % citric acid. Blood samples were collected 30, 45, 60 and 120 min after the use of the tablet. After protein extraction, AEP samples were analyzed by shotgun labelfree quantitative proteomics and plasma samples were analyzed by high-performance liquid chromatography (HPLC). RESULTS: Eight hundred and two proteins were identified in the AEP. Among them, 336 and 213 were unique to the trans-resveratrol and control groups, respectively, while 253 were common to both groups. Proteins with important functions in the AEP had increased expression in the trans-resveratroltreated group, such as neutrophil defensins, S100 protein isoforms, lysozyme C, cystatin-D, mucin-7, alphaamylase, albumin, haptoglobin and statherin. Trans-resveratrol was detected in the plasma at all the times evaluated, with the peak at 30 min. CONCLUSIONS: The administration of trans-resveratrol in sublingual orodispersible tablets was effective both to increase the bioavailability of the polyphenol and the expression of antibacterial and acid-resistant proteins in the AEP, which might benefit oral and general health.

RESUMO

OBJECTIVE: The aim of this study was to determine the effect of different concentrations of resveratrol in protecting enamel against initial dental erosion in vitro. METHODS: Ninety bovine enamel samples (4 × 4 mm) were divided into six groups: Phosphate buffered saline (negative control; PBS), Commercial solution (Elmex Erosion Protection™; positive control) and resveratrol at 4 different concentrations (1, 10, 100 or 400 µg/mL). Initially, the samples were incubated in saliva for the formation of the acquired pellicle (250 µL, 1 h, 37 °C, 250 rpm). Afterward, the samples were incubated in the respective treatments (250 µL, 1 min, 37 °C, 250 rpm) and then reincubated in saliva (250 µL, 1 h, 37 °C, 250 rpm). Finally, the samples were subjected to an erosive challenge by incubating in 1 % citric acid (1 mL, pH 3.5, 1 min, 25 °C, 250 rpm). The percentage surface microhardness change (% SMC) was assessed using a microhardness tester. Data were analyzed by Kruskal-Wallis and Dunn's tests (p < 0.05). RESULTS: The treatments with Elmex™ and resveratrol (1, 10 and 100 µg/mL) significantly protected enamel compared to the negative control, without significant differences among them. However, the group treated with the highest resveratrol concentration (400 µg/mL) did not show a significant difference from the negative control. CONCLUSIONS: Resveratrol at concentrations ranging from 1 to 100 µg/ml was effective in preventing loss of enamel surface microhardness. CLINICAL SIGNIFICANCE: This result suggests a potential new direction for the development of dental products based on resveratrol for the prevention of dental erosion.

RESUMO

This study aimed to assess the influence of the file format on the image quality parameters (image noise, brightness, and uniformity) of periapical radiographs acquired with different digital systems. Radiographic images of an acrylic phantom were acquired with two digital systems - Digora Toto and Express, and exported into five different file formats - TIFF, BMP, DICOM, PNG, and JPEG. Image noise, image brightness (mean of gray values), and image uniformity (standard deviation of gray values) were evaluated in all images. A two-way analysis of variance with Tukey's test as a post-hoc test was used to compare the results, considering the file formats and radiographic systems as the studied factors. A significance level of 5% was adopted for all analyses. The DICOM image file format presented lower image noise, higher brightness (higher mean gray values), and greater image uniformity (p<0.001) than the other file formats, which did not differ from each other for both digital radiography systems (p>0.05). The Express system revealed lower image noise and greater image uniformity than the Digora Toto system regardless of the image file format (p<0.001). Moreover, the Express showed higher brightness than the Digora Toto for all image file formats (p<0.001), except for the DICOM image file format, which did not significantly differ between the digital radiography systems tested (p>0.05). The DICOM image file format showed lower image noise, higher brightness, and greater image uniformity than the other file formats (TIFF, BMP, PNG, and JPEG) in both digital radiography systems tested.

The development of new approaches allowing for the early assessment of COVID-19 cases that are likely to become critical and the discovery of new therapeutic targets are urgently required. In this prospective cohort study, we performed proteomic and laboratory profiling of plasma from 163 COVID-19 patients admitted to Bauru State Hospital (Brazil) between 4 May 2020 and 4 July 2020. Plasma samples were collected upon admission for routine laboratory analyses and shotgun quantitative label-free proteomics. Based on the course of the disease, the patients were divided into three groups: (a) mild (n = 76) and (b) severe (n = 56) symptoms, whose patients were discharged without or with admission to an intensive care unit (ICU), respectively, and (c) critical (n = 31), a group consisting of patients who died after admission to an ICU. Based on our data, potential therapies for COVID-19 should target proteins involved in inflammation, the immune response and complement system, and blood coagulation. Other proteins that could potentially be employed in therapies against COVID-19 but that so far have not been associated with the disease are CD5L, VDBP, A1BG, C4BPA, PGLYRP2, SERPINC1, and APOH. Targeting these proteins' pathways might constitute potential new therapies or biomarkers of prognosis of the disease.

RESUMO

Hypophosphatasia (HPP) is an inherited disease caused by a low activity of tissue-nonspecific alkaline phosphatase, a hydrolase that removes phosphate groups from many molecules. Decreased alkaline phosphatase activity leads to the accumulation of three main metabolites, i.e., pyridoxal 5´-phosphate (PLP), inorganic pyrophosphate (PPi), and phosphoethanolamine. Impairment in PLP dephosphorylation induces seizures, while PPi accumulation inhibits bone mineralization. Clinically, HPP has a wide spectrum of presentations, ranging from neonatal death to an apparent lack of symptoms. This disease is classified into six subtypes according to the age at onset of first signs or symptoms. The clinical manifestations of the disease include rickets-like bone changes, bone demineralization, fragility fractures, reduced muscular strength, chest deformity, pulmonary hypoplasia, nephrolithiasis, nephrocalcinosis, and chondrocalcinosis. Treatment of HPP consists of enzyme replacement therapy. Before this therapy was approved, treatment was palliative and associated with high morbidity and mortality. Asfotase alfa has changed the prognosis of the disease by reducing bone deformity and improving bone mineralization, lung function, and muscle weakness, among other benefits. In adults, teriparatide and anti-sclerostin antibody have been used off-label in selected cases, demonstrating benefit in accelerating fracture healing and in concomitant treatment of osteoporosis. This review summarizes the main aspects of HPP and identifies the particularities of the disease in adult patients.

RESUMO

The Candiota region, located in the extreme south of Brazil, has the largest mineral coal deposit in the country, and this activity is capable of releasing pollutants in which they are associated with the contamination of different matrices (soil, water, and air). The present study aimed to carry out a risk assessment to human health of atmospheric pollutants NO2 and SO2 and PM10-bound metal(loid)s in the municipality of Candiota, in addition to evaluating the correlation of meteorological parameters for the dynamics and potential risk of these pollutants. Pollutants were sampled from stations located almost 4 km from coal exploration activities, and the trace elements As, Cd, Se, Pb, and Ni, in addition to NO2 and SO2, were evaluated. Risk assessment was conducted taking into account the risk to adults via the inhalation route. During the sampling period, all pollutants presented values lower than national legislation or internationally accepted values, and Pb was the element that presented the highest values throughout the sampled period. The risk assessment showed no carcinogenic and non-carcinogenic risks, even when considering the sum of the risk of all analyzed pollutants. It can be observed that the highest levels of Pb, As, and Se occurred in the winter season, while the levels of Ni and Cd were higher in the spring, and the meteorological parameters were correlated with the pollutants, even using a temporal lag of 5 days. Although the air pollutants evaluated did not present a risk to human health, continuous monitoring of regions with strong mineral exploration activity must be carried out with a view to maintaining the well-being of exposed populations, mainly because there are people living in areas closer to sources of coal pollution than distance to air quality monitoring stations.

RESUMO

OBJECTIVE: To qualitatively and quantitatively assess integrated segmentation of three convolutional neural network (CNN) models for the creation of a maxillary virtual patient (MVP) from cone-beam computed tomography (CBCT) images. MATERIALS AND METHODS: A dataset of 40 CBCT scans acquired with different scanning parameters was selected. Three previously validated individual CNN models were integrated to achieve a combined segmentation of maxillary complex, maxillary sinuses, and upper dentition. Two experts performed a qualitative assessment, scoring-integrated segmentations from 0 to 10 based on the number of required refinements. Furthermore, experts executed refinements, allowing performance comparison between integrated automated segmentation (AS) and refined segmentation (RS) models. Inter-observer consistency of the refinements and the time needed to create a full-resolution automatic segmentation were calculated. RESULTS: From the dataset, 85% scored 7-10, and 15% were within 3-6. The average time required for automated segmentation was 1.7 min. Performance metrics indicated an excellent overlap between automatic and refined segmentation with a dice similarity coefficient (DSC) of 99.3%. High inter-observer consistency of refinements was observed, with a 95% Hausdorff distance (HD) of 0.045 mm. CONCLUSION: The integrated CNN models proved to be fast, accurate, and consistent along with a strong interobserver consistency in creating the MVP. CLINICAL RELEVANCE: The automated segmentation of these structures simultaneously could act as a valuable tool in clinical orthodontics, implant rehabilitation, and any oral or maxillofacial surgical procedures, where visualization of MVP and its relationship with surrounding structures is a necessity for reaching an accurate diagnosis and patient-specific treatment planning.

RESUMO

Inorganic forms of As (arsenite - As(III) and arsenate - As(V)) are prevalent in soil and recognized for their high toxicity. Once in the soil, these forms of As can compromise key organisms for ecological processes, such as earthworms. The aim of the study was to evaluate the toxicity of arsenite and arsenate in the Californian earthworm Eisenia andrei exposed in natural soil and tropical artificial soil (TAS). Adverse effects were evaluated using avoidance test, acute toxicity test, and a sublethal concentration test to assess biochemical parameters. LC50 values for arsenite were 21.27 mg/kg in natural soil and 19.0 mg/kg in TAS and for arsenate were 76.18 mg/kg in natural soil and above 120 mg/kg in TAS. In the avoidance test, this behavior was shown to be significantly higher in the natural soil and for earthworms exposed to arsenite, while total antioxidant capacity, glutathione levels, lipid damage, and DNA damage were significantly higher in animals exposed to arsenite, but without differences in relation to the two types of soil tested. Animals exposed to As(V) showed increased activity of enzymes related to glutathione metabolism. The results obtained in the present study show the impact of As exposure on the health of the Californian earthworm E. andrei, especially in the form of arsenite, and alert the public authorities that legal limits should, whenever possible, consider the soil properties and also the different chemical species of the contaminants.

RESUMO

Abstract The aim of this study was to assess the use of digital dental radiology in Brazil, by focusing on the use of image receptors, imaging exams and digital image enhancement tools, also assessing the methods of professional image transfer. Questionnaires were distributed in person on dental meetings and digitally via messaging (WhatsApp®) and mailings list. The sample of this cross-sectional study consisted of 478 questionnaires. Most participants were woman (n=315, 65.9%), with average age of 33.8±9.2 years. Descriptive and frequency analysis was performed. Chi-square and Fisher's exact tests were used (α=0.05). Most dentists worked at shared dental clinics (34.7%) and use digital image receptors (51.1%), but a representative percentage (48.9%) still exclusively use radiographic films. Photostimulable phosphor plate is the most used digital image receptor. Among extraoral exams, panoramic radiography (PAN) is the most used. Regarding dental specialties, oral radiologists and oral and maxillofacial surgeons mostly use cone-beam computed tomography (p<0.001). Most dentists who use digital systems make use of digital image enhancement tools (87.8%), mainly contrast, zoom, brightness and measurements. The most common method of professional image transfer (professional-professional and professional-patiens) is by email, with few dentists using online app and social media (26%). Therefore, while most Brazilian dentists use digital imaging systems, a significant percentage still exclusively use radiographic films. The most extraoral imaging exams used is PAN. Regarding image enhancement tools, brightness and contrast adjustments, zoom and measurements are the most applied. Finally, dentists generally use email for professional image transfer.

Trichomoniasis is the most common nonviral sexually transmitted infection in the world, but its available therapies present low efficacy and high toxicity. Diphenyl diselenide (PhSe2) is a pharmacologically active organic selenium compound; however, its clinical use is hindered by its lipophilicity and toxicity. Nanocarriers are an interesting approach to overcome the limitations associated with this compound. This study designed and evaluated a vaginal hydrogel containing PhSe2-loaded Eudragit® RS100 and coconut oil nanocapsules for the treatment of trichomoniasis. Nanocapsules presented particle sizes in the nanometric range, positive zeta potential, a compound content close to the theoretical value, and high encapsulation efficiency. The nanoencapsulation maintained the anti-Trichomonas vaginalis action of the compound while improving the scavenger action in a DPPH assay. The hydrogels were prepared by thickening nanocapsule suspensions with locust bean gum (3%). The semisolids maintained the nanometric size of the particles and the PhSe2 content at around the initial concentration (1.0 mg/g). They also displayed non-Newtonian pseudo-plastic behavior and a highly mucoadhesive property. The chorioallantoic membrane method indicated the absence of hemorrhage, coagulation, or lysis. The compound, from both non-encapsulated and nano-based hydrogel delivery systems, remained on the surface of the bovine vaginal mucosa. Therefore, the formulations displayed the intended properties and could be a promising alternative for the treatment of trichomoniasis.

RESUMO

Peripheral facial paralysis (PFP) has been shown to be a neurological manifestation of COVID-19. The current study presents two cases of PFP after COVID-19, along with a rapid review of known cases in the literature. Both case reports were conducted following CARE guidelines. We also performed a systematic review of PFP cases temporally related to COVID-19 using PubMed, Embase, and Cochrane Library databases on August 30, 2021, using a rapid review methodology. The two patients experienced PFP 102 and 110 days after COVID-19 symptom onset. SARS-CoV-2 RNA was detected in nasal samples through reverse-transcription real-time polymerase chain reaction (RT-qPCR) testing. Anosmia was the only other neurological manifestation. PFP was treated with steroids in both cases, with complete subsequent recovery. In the rapid review, we identified 764 articles and included 43 studies. From those, 128 patients with PFP were analyzed, of whom 42.1% (54/128) were male, 39.06% (50/128) female, and in 23 cases the gender was not reported. The age range was 18 to 59 (54.68%). The median time between COVID-19 and PFP was three days (ranging from the first symptom of COVID-19 to 40 days after the acute phase of infection). Late PFP associated with COVID-19 presents mild symptoms and improves with time, with no identified predictors. Late PFP should be added to the spectrum of neurological manifestations associated with the long-term effects of SARS-CoV-2 infection as a post COVID-19 condition.

Work-related musculoskeletal disorders encompass a wide variety of injuries that cause pain in bones, joints, muscles, or surrounding structures, acutely or chronically, focal or diffuse. They affect 33% of adults and account for 29% of work absenteeism due to illness. The number of cases has been increasing annually due to institutional organization based on productivity and profit, disregarding the physical and psychosocial limits of workers. Hence, this study sought to determine the prevalence of musculoskeletal pain in nursing workers and its associated factors. A cross-sectional study was conducted with 99 nursing professionals using the Nordic Musculoskeletal Symptom Questionnaire. Of the 99 nursing professionals evaluated, 84.8% reported pain in the last 12 months and 54.5% in the seven days prior to the survey. Ankles/feet (48.8%) and lower back (42.9%) were the most affected regions. Factors such as time on the job, physical effort, presence of previous musculoskeletal disease, age and sitting time increased the occurrence of symptoms. Results showed a high prevalence of musculoskeletal pain in nursing professionals. Personal, work, health and behavioral factors were identified as potentially negative for the outcome.

RESUMO

OBJECTIVE: To evaluate the influence of the file format of digital periapical radiographs on the diagnosis of vertical root fracture (VRF). STUDY DESIGN: Periapical radiographic images of 34 single-rooted teeth-19 with VRF, and 15 without VRF were acquired using two digital systems-Digora Toto, and Digora Optime, and exported into four different file formats-TIFF, BMP, PNG, and JPEG, totaling 272 radiographs. The radiographs were assessed by five examiners for the detection of VRF, using a 5-point scale (1-definitely absent; 2-probably absent; 3-uncertain; 4-probably present; 5-definitely present). Diagnostic values of area under the ROC curve, specificity, and sensitivity for the diagnosis of VRF were calculated. The results were compared by two-way Analysis of Variance with post hoc Tukey's test. The intra- and inter-examiner agreements were measured by the Kappa test. The significance level was set at 5% for all analyses. RESULTS: The values of intra-examiner agreement varied from moderate (0.56) to almost perfect (0.81), while the values of inter-examiner agreement varied from fair (0.29) to moderate (0.60). The image file format did not influence the diagnostic values for VRF for any of the radiographic systems tested (p > 0.05). Digora Toto had significantly greater values of area under the ROC curve than Digora Optime for all file formats (p = 0.001). CONCLUSION: The image file format of periapical radiographs does not influence the diagnosis of VRF, regardless of the digital radiography system.

RESUMO

OBJECTIVE: To determine the prevalence of the elongated styloid process (ESP) and its characteristics, such as sex and age of the patient, unilateral and bilateral incidence, besides variations between different populations and panoramic and CBCT examinations. MATERIALS AND METHODS: A search was performed in six databases (PubMed, Web of Science, Scopus, Cochrane, Lilacs, and Embase) to identify observational studies that used imaging exams and assessed ESP prevalence among panoramic radiograph CBCT examinations, whose transversal prevalence studies were included. Furthermore, studies with a specific group of patients or symptomatic patients were excluded. Additionally, Joanna Briggs Institute checklist was used to evaluate the quality of the studies. A meta-analysis was conducted, then subgroup analyses were performed by grouping studies according to the secondary outcomes, with a significance level set at 5%. The Grading of Recommendations Assessment, Development, and Evaluation system was used to rate the certainty in the evidence. RESULTS: The initial search resulted in 1635 studies, from which 39 articles met the inclusion criteria, encompassing 50,655 participants. The sample size varied between 82 and 5,000 participants. The prevalence of the ESP ranged from 1.3 to 94.8%, with an overall prevalence of 30.2%. The bilateral occurrence was higher than the unilateral one, but no significant predilection was observed according to sex, age, or population. The type of imaging examination also showed no difference in its detection. CONCLUSION: The overall prevalence of ESP was 30.2%, with a propensity for bilaterality, but not for any sex, age, or population geographic location. The imaging examination modality did not influence the diagnosis of ESP. However, the quality level of the studies evaluated was very low, demonstrating the need for more homogeneous primary studies on the prevalence of the ESP with a more standardized methodology. CLINICAL RELEVANCE: There is no consensus in the literature regarding the prevalence of the ESP and the characteristics of the affected patients that can cause chronic and debilitating discomfort in the head and neck region. Therefore, knowledge about the prevalence and characteristics of this condition would help dental clinicians reach the correct diagnosis.
