RESUMO

El cuerno cutáneo es un concepto semiológico. Se define como una lesión tumoral hiperqueratósica de aspecto cónico y tamaño variable, que puede tener como base una lesión de origen benigno, premaligno o maligno. El tratamiento consiste en la resección completa de la lesión, seguido de un estudio histopatológico para precisar el diagnóstico. El carcinoma sebáceo, neoplasia maligna derivada del epitelioanexial de las glándulas sebáceas, se ha descrito como una etiología poco frecuente de cuerno cutáneo. Su diagnóstico es difícil, ya que carece de elementos clínicos característicos y su histología puede ser similar al carcinoma espinocelular o basocelular, pero importante ya que constituye una neoplasia agresiva, con riesgo de recurrencia local y metástasis. A continuación, presentamos el caso de un hombre de 68 años con diagnóstico clínico de cuerno cutáneo palpebral y diagnóstico histopatológico de carcinoma sebáceo.


The cutaneous horn is a semiological concept. It is defined as an hyperkeratotic tumor of conical appearance and variable size, which may be based on benign, premalignant or malignant lesion. The treatment is the complete resection followed by an histopathology study to clarify the diagnosis. Sebaceous carcinoma, malignant neoplasm derived from adnexal epithelium of sebaceous glands, has been described as a rare etiology of cutaneous horn. Diagnosis is difficult because it doesn’t have characteristic clinical features and histology may be similar to squamous or basal cell carcinoma, but it is important because is an aggressive neoplasm with risk of local recurrence and metastasis. We present a case of a 68-year-old man, with a clinical diagnosis of eyelid cutaneous horn and histopathological diagnosis of sebaceous carcinoma.

BACKGROUND: Among hypertensive patients, other risk factors for mortality and morbidity, besides blood pressure, must be considered when therapeutic decisions are done. AIM: To assess the incidence and relevance of cardiovascular risk factors in a cohort of patients with essential hypertension. MATERIAL AND METHODS: A cohort of 1,072 treated patients with essential hypertension was followed for a period up to 25 years. Four hundred eighty six were men and 586 were women, age ranged from 31 to 70 years. At entry, 56% of subjects did not have any organic complications associated to hypertension (stage I WHO criteria), 30% had mild alterations (Stage II) and 14% had major complications (myocardial infarction, stroke, heart failure or renal failure). Likewise, 43.8% had mild, 14.5%, moderate and 41.7%, severe hypertension. Patients were treated with monotherapy or combined therapy based on diuretics, beta blockers, calcium antagonists and angiotensin converting enzyme inhibitors. Goal of therapy was 140/90 mm Hg. Risk factors associated diseases and complications were registered carefully. Causes of death were obtained from hospital records and death certificates. Mortality was analyzed using life tables (intention to treat method included). RESULTS: Blood pressure dropped significantly during follow up from a mean of 182/110 to 154/92 mm Hg. During follow up, 143 patients died and 429 complications (lethal or non lethal) were recorded. Twenty four percent of patients smoked, 24% reported alcohol intake, 56% had hypercholesterolemia, 11% were obese, 13% had diabetes and 3% had gout. The proportional hazard model showed that the existence of previous complications, the presence of more than 3 risk factors, and age over 60 and mean systolic and diastolic pressure during therapy, were independent and significant risk factors for mortality. CONCLUSIONS: The incidence of risk factors among our hypertensive patients is very similar to that of other national or international cohorts.

Ajoene, an organosulfur compound originally isolated from garlic, has been shown to be effective in short-term treatment of tinea pedis. We compare the safety and effectiveness of twice-daily topical application during 1 week of 0.6% and 1% ajoene and 1% terbinafine in the treatment of tinea pedis. Seventy soldiers from the Venezuelan Armed Forces, with clinical and mycologic diagnosis of tinea pedis, were included in this study. However, only 47 were available for final evaluation. The patients were randomly distributed into 3 treatment groups: 0.6% ajoene, 1% ajoene, and 1% terbinafine. Clinical follow-up shows a rapid decline in the signs and symptoms in all groups. Efficacy of the treatments, measured as mycologic cure, 60 days after the end of the therapy was 72% for 0.6% ajoene, 100% for 1% ajoene, and 94% for 1% terbinafine. This represents the first demonstration of the therapeutic application of an inhibitor of phospholipid biosynthesis in human dermatophytosis.

The local motion of protoporphyrin IX in the heme pocket of horseradish peroxidase has been studied using fluorescence methods. The temperature dependence of the anisotropy and lifetime of a protoporphyrin IX-apo-horseradish peroxidase complex, dissolved in a solution of 80% glycerol and 20% buffer (0.1 M phosphate, pH 7.4), was determined. Anisotropy data were analyzed in terms of the thermal coefficient of the frictional resistance to fluorophore movement. The resultant 'Y' plot was characterized by three distinct slopes. The slope corresponding to the lowest temperature regime agreed with the value obtained for fluorophores not complexed with protein. The slope corresponding to an intermediate temperature was lower indicating a larger resistance to porphyrin rotation. At higher temperatures this resistance to rotation diminished as evidenced by the increased slope. These results are contrasted with those obtained with the protoporphyrin IX-apomyoglobin complex.
