BACKGROUND: Understanding the degree of facial mobility upon postural changes is of great clinical relevance especially if facial assessment, facial measurements and/or facial markings are done in an upright position, but facial procedures are performed in a supine position. OBJECTIVE: The objective of this study is to investigate regional facial skin displacement and facial volume changes in individuals between upright and supine positions. METHODS: This multi-center study analyzed a total of 175 study participants with a mean age of 35.0 (10.2) years and a mean body mass index of 24.71 (3.5) kg/m2. 3D surface scanning technology with automated registration and alignment was utilized, and multivariate analyses were performed with adjustment for age, gender, body mass index, facial skin sagging and laxity. RESULTS: The medial face displaced less than the lateral face in both cranial (0.88 mm) and in lateral (0.76 mm) directions, and the lower face displaced more than the middle face in both cranial (1.17 mm) and lateral directions (1.37 mm). Additionally, the medial face lost, on average, 3.00cc whereas the lateral face increased by 5.86cc in volume; the middle face increased by 2.95cc, whereas the lower face decreased by 0.98cc in volume. All p < 0.001. CONCLUSION: Practitioners should be mindful that there is a statistically significant change in facial soft tissues between the upright and supine positions and that the magnitude of the change does not necessarily reflect on the aging process alone but is a multi-factorial process which should be individualized for each patient's needs. OBJECTIVES: To examine characteristics of children referred for obesity management based on referral frequency, child- and referrer-related variables associated with re-referral, and determine whether re-referral increased treatment initiation. STUDY DESIGN: This population-level, retrospective analysis included all 2- to 17-year-olds referred for obesity management to 1 of 3 multidisciplinary clinics in Alberta, Canada between April 2013 and December 2017. Children were dichotomized based on referral frequency, specifically once only or more than once (re-referred). Data were retrieved from standardized referral forms and patient registries. Analyses included logistic regression and generalized estimating equations models. RESULT: We analyzed data from 2745 children (47.2% female; mean age: 11.4 years; mean body mass index z score: 3.03) and 2705 physicians (60.2% female; 65.6% pediatricians). Overall, 300 (10.2%) children were re-referred with most (n = 276; 92.0%) being referred twice. Children were less likely to be re-referred if they were referred by a family physician (vs pediatrician) (aOR 0.62; 95% CI 0.46-0.84; P = .0018) or scheduled a clinic appointment following their index referral (aOR: 0.29; 95% CI 0.21-0.4; P < .001). Treatment initiation was higher in children who were referred once only (42.1%) vs their re-referred peers (18.0%; P < .0001); however, for children who were re-referred, they were more likely to initiate treatment following their second referral (aOR 2.3; 95% CI 1.22-4.31; P = .01). This improvement was not sustained on subsequent referrals (aOR 0.44; 95% CI 0.17-1.12; P = .08). CONCLUSIONS: Few children were re-referred for pediatric obesity management; however, for those children who were re-referred, being re-referred once only increased the likelihood of treatment initiation.

OBJECTIVES: To characterize the children who were referred, determine the proportion of referred children who enrolled, and examine factors associated with enrollment in multidisciplinary clinical care for pediatric weight management. STUDY DESIGN: This cross-sectional study included the population of children (2-17 years of age; body mass index of ≥85th percentile) referred to 1 of 3 hospital-based multidisciplinary weight management clinics in Alberta, Canada, from April 2013 to April 2016. Referral and enrollment data were obtained from Alberta Health Services databases. Bivariate and multivariable logistic regression models were used to determine the independent and combined effects of predictors of enrollment. RESULTS: Of the 2014 children (51.8% male; mean body mass index z-score: 3.42 ± 0.03) referred to multidisciplinary clinical care, 757 (37.6%) enrolled in care. Most referred children had severe obesity and were referred by physicians. Several factors independently predicted enrollment; however, in our most parsimonious multivariable model, only the time gap (OR, 0.94; 95% CI, 0.88-0.99; P = .03) between the attendance date of the orientation session and the booking date of initial appointment predicted enrollment for all children. Body mass index z-score (OR, 0.81; 95% CI, 0.67-0.98; P = .03) and time gap (OR, 0.92; 95% CI, 0.85-0.99; P = .02) predicted enrollment in children with severe obesity exclusively. CONCLUSIONS: Fewer than 40% of referred children enrolled in multidisciplinary clinical care. Reducing the duration of enrollment and providing additional support for treatment initiation to children with severe obesity may enhance treatment uptake for pediatric weight management.

OBJECTIVE: To explore parents' recommendations to enhance enrollment in multidisciplinary clinical care for managing pediatric obesity. STUDY DESIGN: Data for this interpretative description study were collected through individual, semistructured interviews that were audiorecorded, transcribed verbatim, and analyzed thematically. Parents (n = 79) were recruited from 4 multidisciplinary weight management clinics in Canada located in Edmonton, Hamilton, Montreal, and Vancouver. RESULTS: Most interviewed parents had children with obesity (body mass index ≥95th percentile; 84.2%), were female (87.3%), had postsecondary education (69.6%), and were white (75.9%). Parents' recommendations referred to enrollment opportunities, information about obesity services, motivation for treatment, and accessibility to obesity services. Specifically, parents recommended to increase referral options and follow-up contacts with families during the enrollment process, inform referring physicians and families about the availability and characteristics of obesity services, enhance families' motivation for treatment, prevent families from getting discouraged, make services more appealing to families, and address accessibility issues (eg, offering multiple options for appointment times, providing support for transportation). CONCLUSIONS: Parents' recommendations support the need for family-centered approaches to enhance enrollment; however, their feasibility, acceptability, and effectiveness remain to be tested empirically.

Se presentó un paciente que nace severamente deprimido por parto natural, a las 35,4 semanas, con peso de 2 620 g. Inmediatamente se comenzaron medidas de reanimación, con bolsa, pero al no mejorar se decide intubación traqueal. Se planteó malformación de vías respiratorias por imposibilidad de lograrlo; al intentar traqueotomía, la tráquea está ausente, planteándose el diagnóstico de agenesia traqueal. A pesar de las medidas terapéuticas tomadas, el paciente fallece 8 horas después del nacimiento. En el estudio patológico se comprobó la presencia de agenesia traqueal tipo II, de acuerdo a la clasificación de Floyd...


We present the case of a patient, who was born severely depressed by natural birth, at 35,4 week, weighting 2 620 g. Immediately he was given reanimation, with bag, but when there was not an improvement, we decided the tracheal intubation. We supposed the malformation of the respiratory ways when the intubation was impossible; when we tried to make a tracheotomy, we did not find the trachea, arriving to the diagnosis of tracheal agenesis. In spite of the taken therapeutic measures, the patient died 8 hours after birth. In the pathologic study we stated type II tracheal agenesis, according to Floyd's classification...

Desde las primeras descripciones de Golden y Stout6, este grupo de lesiones mesenquimales se consideraron de origen muscular y recibieron nombres como leiomioma, leiomioma celular, leiomioma epitelioide, leiomioblastoma, leiomioma bizarro y leiomiosarcoma. No es, sin embargo, hasta la utilización de la inmunohistoquímica y la subsiguiente demostración de la ausencia de marcadores musculares y la presencia ocasional de marcadores neurales, que surge de la mano de Mazur y Clark3 el término de tumores del estroma. Actualmente se denominan GISTs a los tumores mesenquimales CD117.positivos, fusiformes o epitelioides, primarios del tracto gastrointestinal, epiplon, mesenterio y retroperitoneo10. Los GISTs aparecen en la pared del tubo digestivo: estómago (50-60 por ciento), intestino delgado (20-30 por ciento), intestino grueso (10 por ciento) y esófago (5 por ciento) y ocasionalmente en epiplon, mesenterio y retroperitoneo (5 por ciento). En nuestro trabajo se presenta a una paciente de 67 años de edad que fue ingresada en nuestro centro por presentar un cuadro de sangramiento digestivo alto, estudiada, y se comprobó una tumoración del fundus gástrico de 6 cm , se operó y los resultados definitivos de anatomía patológica arrojaron un Tumor del Estroma Gastrointestinal.


Since the first descriptions made by Golden and Stout, this group of mesenchymal lesions is considered of muscular origin and they were named as leiomyoma, cellular leiomyoma, epithelioid leiomyoma, leiomyoblastoma, bizarre leiomyoma and leiomyosarcoma. But Mazur and Clark created the term estromal tumor only after they began to use the inmunohistochemistry and subsequently showed the absence of muscular markers, and the occasional presence of neural markers. Nowadays, gastrointestinal estromal tumors are called the primary mesenchymal CD117.positive, fusiform or epithelioid tumors of the gastrointestinal tract, epiplon, mesenterio, and retroperitoneum. The gastrointestinal estromal tumors appear at the wall of the digestive tube: stomach (50-60 percent), small intestine (20-30 percent), large intestine (10 percent) and esophagus (5 percent), and occasionally in epiplon, mesenterio, and retroperineum (5 percent). In our work we present a 67 year-old patient, entered in our hospital for presenting high digestive bleeding. We studied the case, and found a 6 cm tumor of the gastric fundus. RESUMO

Se estudian 50 pacientes ingresados en el Hospital infantil Docente "Dr. Angel A. Aballí" en el período de septiembre de 1986 a febrero de 1987, los cuales contrajeron una infección nosocomial. Se analizan una serie de indicadores de carácter general. De acuerdo con los resultados obtenidos se establecen conclusiones, se resalta el grupo de edades y el sexo más afectado, los gérmenes predominantes, el comportamiento por servicios, así como también las localizaciones más frecuentes. Se significa finalmente como la infección nosocomial repercute en la estadía y en los gastos de la institución

RESUMO

Se presenta un estudio antropométrico en 480 alumnos de ambos sexos del Centro Escolar "26 de Julio" de la ciudad de Santiago de Cuba, seleccionados de forma aleatoria estratificada para evaluar su estado nutricional, se utilizaron para ello las tablas de Iowa y las cubanas, y se realizó una comparación con 2 estudios anteriores del mismo universo. En su análisis se emplearon varios estadígrafos, tales como: el chi-cuadrado, la t de Student, análisis de series cronológicas y de las tendencias seculares, se muestra una mejoría notable en el estado nutricional de los niños de 6 a 12 años, debido a una mayor disponibilidad de alimentos en la población de nuestra provincia
