RESUMEN Introducción: La estenosis aórtica es relativamente frecuente en niños. El objetivo de este trabajo fue de caracterizar los resultados y la evolución de esta patología luego del manejo quirúrgico y percutáneo. Material y Métodos: Estudio retrospectivo de pacientes de cero a 19 años, tratados de estenosis aórtica subvalvular, valvular y supravalvular, en el Hospital de Clínicas, entre 1998 y 2019. Fueron excluidos los casos asociados a otras lesiones congénitas. Resultados: Fueron tratados 20 pacientes, edad de 3 días a 17 años, predominio masculino (80%). La estenosis fue valvular en 65% de los casos, subvalvular 20%, supravalvular 5% y múltiple en 10%. El 61% de los valvulares recibió tratamiento percutáneo, y los demás valvuloplastia quirúrgica. En el seguimiento, la tasa libre de reintervención fue de 47% a 10 años; el 50% de ellos tiene insuficiencia valvular aórtica moderada a severa. De los 13 casos de estenosis valvular, tratados percutánea o quirúrgicamente, 4 están aguardando recambio valvular. De 4 pacientes con estenosis subvalvular, 3 tienen insuficiencia aórtica leve, y gradiente medio de 20 mmHg. De dos pacientes con estenosis supravalvular, uno quedó con estenosis residual importante. En cuanto a clase funcional, todos los pacientes se encuentran en grados 1 y 2 de la escala de NYHA. No se presentaron complicaciones inmediatas en los sometidos a procedimientos percutáneos o quirúrgicos. Fallecieron dos pacientes (10.5%). Conclusiones: La estenosis aórtica afecta principalmente la región valvular, y el tratamiento tanto quirúrgico como percutáneo permiten aliviar la obstrucción hasta el momento de un reemplazo valvular.


ABSTRACT Introduction: Aortic stenosis is relatively common in children. The objective of this study was to characterize the results and clinical course of this pathology after surgical and percutaneous management. Material and methods: This was a retrospective study of patients aged 0 to 19 years, treated for subvalvular, valvular, and supravalvular aortic stenosis, at Hospital de Clínicas, between 1998 and 2019. Cases associated with other congenital lesions were excluded. Results: Twenty patients were treated, age from 3 days to 17 years, male predominance (80%). The stenosis was valvular in 65% of the cases, subvalvular 20%, supravalvular 5% and multiple in 10%. 61% of the valve recipients received percutaneous treatment, and the other surgical valvuloplasty. At follow-up, the free reoperation rate was 47% at 10 years; 50% of them have moderate to severe aortic valve regurgitation. Of the 13 cases of valve stenosis, treated percutaneously or surgically, 4 are awaiting valve replacement. Of 4 patients with subvalvular stenosis, 3 have mild aortic regurgitation and a mean gradient of 20 mmHg. Of two patients with supravalvular stenosis, one was left with significant residual stenosis. Regarding functional class, all patients are in grades 1 and 2 of the NYHA scale. There were no immediate complications in those undergoing percutaneous or surgical procedures. Two patients (10.5%) died. Conclusions: Aortic stenosis mainly affects the valve region, and both surgical and percutaneous treatment allow the obstruction to be relieved until the time of valve replacement.

Introducción: La comunicación interauricular (CIA) es una cardiopatía congénita frecuente, cuyo diagnóstico es difícil en pacientes paucisintomáticos. Sin tratamiento, puede complicarse con insuficiencia cardiaca, arritmias e hipertensión pulmonar. El electrocardiograma puede proporcionar indicios de esta cardiopatía. Objetivo: Describir los hallazgos clínicos y electrocardiográficos de pacientes portadores de CIA. Materiales y Métodos: Trabajo retrospectivo, de pacientes menores de 18 años, evaluados cardiológicamente de enero 2008 a diciembre de 2013. Se describen las variables electrocardiográficas y semiológicas sugestivas de CIA. Resultados: Hubo 77 pacientes con CIA. Los motivos de consulta más frecuentes entre los pacientes con CIA fueron el soplo cardiaco, el síndrome de Down y la disnea. En el examen físico de los pacientes con CIA: soplo sistólico en 69% y R2 desdoblado en 56%. En el electrocardiograma, los pacientes tuvieron QRS de 79 ms, eje eléctrico desviado hacia la derecha, BIRD y notch. Los criterios de sobrecarga ventricular derecha (SVD) más frecuentes fueron la onda S profunda en V6 y la onda R alta en V1. La auscultación de un R2 desdoblado en el examen físico, así como la SVD y el patrón notch fueron los hallazgos más frecuentes en pacientes con CIA. Conclusión: el electrocardiograma constituye una herramienta válida en la evaluación de pacientes con sospecha de CIA pero precisa de la combinación de hallazgos clínicos para llegar al diagnóstico.


Introduction: Atrial Septal Defect (ASD) is a frequent congenital heart disease, the diagnosis of which is difficult in paucisymptomatic patients. Without treatment, it can lead to heart failure, arrhythmias and pulmonary hypertension. The electrocardiogram may provide evidence of this condition. Objective: To describe the clinical and electrocardiographic findings of patients with ASD. Materials and Methods: This was a retrospective study of patients under age 18 who underwent cardiac evaluation from January 2008 to December 2013. The electrocardiographic and clinical finding variables suggestive of ASD are described. Results: There were 77 patients with ASD. The most frequent reasons for consultation among patients with ASD were heart murmur, Down syndrome and dyspnea. Findings on physical examination of patients with ASD were: systolic murmur in 69% and S2 split in 56%. On the electrocardiogram, the patients had QRS of 79 ms, electric axis deviated to the right, Incomplete Right Bundle Branch and notch pattern. The most frequent right ventricular volume overload (RVVO) criteria were the deep S wave in V6 and the high R wave in V1. Auscultation of a split S2 on the physical examination, as well as the RVVO and the notch pattern were the most frequent findings in patients with ASD. Conclusion: the electrocardiogram is a valid tool in the evaluation of patients with suspected ASD but requires the combination of clinical findings to reach the diagnosis.

Diaphragmatic myoblasts (DMs) are precursors of type-1 muscle cells displaying high exhaustion threshold on account that they contract and relax 20 times/min over a lifespan, making them potentially useful in cardiac regeneration strategies. Besides, it has been shown that biomaterials for stem cell delivery improve cell retention and viability in the target organ. In the present study, we aimed at developing a novel approach based on the use of poly (L-lactic acid) (PLLA) scaffolds seeded with DMs overexpressing connexin-43 (cx43), a gap junction protein that promotes inter-cell connectivity. DMs isolated from ovine diaphragm biopsies were characterized by immunohistochemistry and ability to differentiate into myotubes (MTs) and transduced with a lentiviral vector encoding cx43. After confirming cx43 expression (RT-qPCR and Western blot) and its effect on inter-cell connectivity (fluorescence recovery after photobleaching), DMs were grown on fiber-aligned or random PLLA scaffolds. DMs were successfully isolated and characterized. Cx43 mRNA and protein were overexpressed and favored inter-cell connectivity. Alignment of the scaffold fibers not only aligned but also elongated the cells, increasing the contact surface between them. This novel approach is feasible and combines the advantages of bioresorbable scaffolds as delivery method and a cell type that on account of its features may be suitable for cardiac regeneration. Future studies on animal models of myocardial infarction are needed to establish its usefulness on scar reduction and cardiac function.

Las enfermedades periodontales son infecciones caracterizadas por la presencia de más de 200 especies bacterianas que, por infestación metastásica, pueden llegar a diferentes órganos anatómicos y ocasionar cambios patológicos. En este artículo se describen los datos más importantes sobre el descubrimiento de los diversos patógenos periodontales en diferentes sistemas del organismo humano, sus aspectos epidemiológicos, mecanismos de acción, así como su relación con la patogenia de las diferentes enfermedades. También se hace mención especial de la presencia de estos gérmenes como iniciadores y perpetuadores de procesos sépticos en los sistemas cardiovascular, respiratorio, gastrointestinal, renal, nervioso, oftálmico y en el embarazo.


Periodontal diseases are infections characterized by the presence of more than 200 bacterial species that, through metastasic infection, can reach different anatomical organs and can cause pathological changes. In this work the most important data are described on the discovery of the diverse periodontal pathogens in different systems of the human organism, their epidemiological aspects, action mechanisms, as well as their relationship with the pathogenesis of different diseases. Special mention is also made on the presence of these germs as begginners and perpetuators of septic events in the cardiovascular, respiratory, gastrointestinal, renal, nervous, ophthalmic systems and in pregnancy.
