RESUMO

The discovery of an unusual specimen of Psorophora in Mississippi led to a search for new diagnostic characters on the seven United States species of Psorophora (Janthinosoma): Psorophora cyanescens, Ps. ferox, Ps. horrida, Ps. johnstonii, Ps. longipalpus, Ps. mathesoni, and Ps. mexicana. New characters were found and diagnostic descriptions and a new key are provided for identifying female specimens. The new key stresses morphological differences other than color patterns on the hindtarsi, however, notes on hindtarsal color and other variations found on each species are also provided. The status of Ps. mexicana specimens recorded from Texas is reviewed and a list of collections of this species since the 1945 record in Texas is provided. Evidence is provided supporting the recognition of Ps. mexicana as a distinct species.

We report results from 2 years of therapy with the long-acting form of the gonadotropin-releasing hormone (GnRH) analog leuprolide acetate, which was previously reported in short-term trials to be efficacious in the treatment of central precocious puberty. Thirteen girls and two boys, aged 1.9 to 9.7 years, who satisfied clinical criteria including GnRH-stimulated luteinizing hormone (LH) greater than 10 IU/L (mean radioimmunoassay LH, 29.1 +/- 5.54 IU/L), received depot leuprolide, 6 to 15 mg intramuscularly every 4 weeks. Estradiol (or testosterone), insulin-like growth factor I, and GnRH-stimulated gonadotropins were obtained at baseline, at 2 months, and at 6-month intervals with bone age determinations. Pubertal progression ceased in all patients, and menses did not occur. Mean increase in height during therapy was 5.77 +/- 2.0 cm/yr. Predicted adult height increased over baseline by 5.52 +/- 1.16 cm at 18 months. Mean estradiol values in the girls declined from 3.3 +/- 0.6 to 0.60 +/- 0.03 ng/dl, with no overlap of baseline and treatment values. The mean basal LH value was unchanged by therapy; mean basal and peak LH values for all follow-up GnRH stimulation tests were 4.05 +/- 0.57 and 4.95 +/- 0.70 IU/L, respectively. Basal and peak follicle-stimulating hormone (FSH) values were suppressed from 4.10 +/- 0.62 and 10.06 +/- 1.34 IU/L, respectively, to generally undetectable levels (< 1). Comparison with untreated control patients suggested that basal LH did not completely return to prepubertal levels, whereas FSH levels were suppressed below prepubertal levels. Estradiol, FSH, and LH levels reached their nadir by 2 months; in contrast, mean serum levels of insulin-like growth factor I progressively declined from +0.57 +/- 0.19 SD score to -0.06 +/- 0.22 SD score at 24 months. Two girls were withdrawn from the study because of reactions at injection sites, with apparent sterile abscess formation in one patient. This study provides evidence that (1) long-term treatment with depot leuprolide is characterized by immediate and sustained laboratory and clinical suppression, (2) GnRH-stimulated LH and random FSH and estradiol concentrations are useful laboratory measures of efficacy, and (3) the progressive increase in predicted adult height is temporally associated with decreased serum levels of insulin-like growth factor I and striking deceleration of bone age advancement.

Se presenta un análisis de la mortalidad perinatal y maternal que ha disminuido en el año 1988 a una tasa de 4,1 por 10.000 nacidos vivos. Se revisan las situaciones presentadas por las defunciones por aborto y complicaciones del trabajo del parto y la importancia del control pre-natal. Se presenta la situación de mortalidad maternal y por aborto por ocurrencia en los distintos Servicios de Salud. Se indica una serie de medidas tendientes a optimizar su atención

Between 1979 and 1980, 104 bats representing 13 species in 4 families were collected in California and New Mexico, U.S.A., and Baja California and Sonora, Mexico, and were examined for coccidia; only 3 (3%) had oocysts in their feces. Bats examined and their infection rates were: Molossidae: 0 of 12 Tadarida brasiliensis, 1 of 18 (6%) T. femorosacca; Natalidae: 0 of 1 Natalus stramineus; Phyllostomatidae: 0 of 1 Choeronycteris mexicana, 0 of 2 Leptonycteris sanborni, 0 of 1 Macrotus californicus; Vespertilionidae: 0 of 9 Antrozous pallidus, 0 of 28 Eptesicus fuscus, 0 of 1 Lasionycteris noctivagans, 0 of 3 Lasiurus borealis, 2 of 22 (9%) L. cinereus, 0 of 1 L. ega, 0 of 5 Pipistrellus hesperus. Sporulated oocysts were only found in T. femorosacca and these represent a new species, Eimeria tadarida n. sp. They are subspheroidal to ellipsoidal, 19 x 25 (16-23 x 20-30) microns; a micropyle is absent, and fragments within the oocyst may be oocyst residuum or multiple polar bodies. The oocyst wall, approximately 1.5 microns, is composed of a mammillated outer layer and smooth inner layer. Sporocysts are ovoidal, 8 x 12 (6-9 x 10-14) microns, and have a small Stieda body and a wide substieda body. This is only the 14th eimerian to be described from bats worldwide. Only unsporulated or partially sporulated oocysts of an eimerian were seen in 2 L. cinereus. These measured 28 x 25 (27-29 x 24-26) microns and had a mammillated outer oocyst wall.

Primary health care (PHC) services are now recognized as a mainstay of development in low-income regions of the world. Yet, in many of these areas, economic conditions that include a burgeoning international debt, precipitous drops in the prices of raw materials, expanding population bases, and faltering currency rates, make the effective financing of PHC services an issue requiring the most immediate attention. A potentially valuable source of assistance in such endeavors is data on consumers' payment patterns for PHC services, and on socioeconomic conditions within their communities. The Centro de Pesquisas de Assistência Integrada à Mulher e à Criança (CPAIMC), the principal private source of PHC in Rio de Janeiro, has collected these type of data for nine low-income communities (LICs) within the Greater Rio de Janeiro Metropolitan Region. The current paper interfaces these data with previously developed solutions to address the problem of financing PHC services within the nine LICs. The solutions were recommended by a methodology developed and implemented by two of the authors. Results indicate that the consumer payment pattern and socioeconomic data support the methodology's recommendations and, more importantly, may be used to improve the effectiveness of those recommendations. Examples are provided and implications discussed.

The feces of 212 sandhill cranes (Grus canadensis) collected in central New Mexico from October 1982 to January 1983 and from October 1983 to January 1984 were examined to determine the prevalence of coccidial oocysts. One hundred forty-five granulomatous nodules from the viscera of 64 cranes and samples of lung, small intestine, and large intestine from 58 birds were examined by light and transmission electron microscopy for the presence of intestinal or extraintestinal coccidiosis. Of the 212 fecal samples, 160 (75%) were positive for oocysts of Eimeria, including E. gruis in 139 (66%) and E. reichenowi in 118 (56%) of the samples. Eimeria bosquei sp. n. was found in two (approximately 1%) of the fecal samples. Subspheroid to ovoid oocysts of this new species are 19-27 X 14-19 (23.6 X 17.1) micrograms with ovoid sporocysts 10-14 X 7-11 (12.3 X 9.3) micrograms. A rough, heavily pitted outer oocyst wall, sporocyst residuum, Stieda and substieda bodies, and multiple polar bodies are present. The polar bodies, of varying sizes, always aggregate at the apex of the sporulated oocyst. An Adelina sp. was found in one (0.5%) crane. Coccidian developmental stages were found in the epithelium and lamina propria of the small and large intestine. Disseminated granulomatous nodules were found in the oral mucosa, esophagus, heart, descending aorta, liver, small intestine, mesenteries, and parietal peritoneum. Unique cell types resembling coccidian asexual and sexual stages were observed by light and electron microscopy in some of the nodules.
