Metabolic alterations are a common problem in people living with HIV (PLHIV), as a result of a stage of chronic inflammation that affects the homeostasis of the organism. Prolonged exposure to antiretroviral therapy has been associated with developing lipodystrophies that modify lipoprotein metabolism and inflammatory markers such as tumor necrosis factor-alpha (TNF-α) and interleukin-6 (IL-6), which are mediators of the immune response. The study aimed to associate TNF-α and IL-6 levels with their polymorphisms and metabolic alterations in PLIHV. We hypothesized that TNF-α and IL-6 levels and their polymorphisms are associated with metabolic alterations. In total, 185 PLHIV and 51 HIV-negative people were included. Biochemical parameters were determined by colorimetric assay, cytokine levels by immunoassay, and allelic discrimination by quantitative polymerase chain reaction. A correlation was found between TNF-α levels and the variables cholesterol (r = -0.171, P = 0.020) and high-density lipoprotein (HDL) (r = -0.245, P = 0.001). There are associations between HDL levels (P = 0.011) and GG genotype of rs1800629. The results suggest a metabolic alteration related to the constant immune response, especially the production of proinflammatory cytokines such as TNF-α and IL-6. It was observed that genetic factors may influence metabolism alteration, mainly in lipids.

Wildlife trafficking creates favorable scenarios for intra- and inter-specific interactions that can lead to parasite spread and disease emergence. Among the fauna affected by this activity, primates are relevant due to their potential to acquire and share zoonoses - infections caused by parasites that can spread between humans and other animals. Though it is known that most primate parasites can affect multiple hosts and that many are zoonotic, comparative studies across different contexts for animal-human interactions are scarce. We conducted a multi-parasite screening targeting the detection of zoonotic infections in wild-caught monkeys in nine Peruvian cities across three contexts: captivity (zoos and rescue centers, n = 187); pet (households, n = 69); and trade (trafficked or recently confiscated, n = 132). We detected 32 parasite taxa including mycobacteria, simian foamyvirus, bacteria, helminths, and protozoa. Monkeys in the trade context had the highest prevalence of hemoparasites (including Plasmodium malariae/brasilianum, Trypanosoma cruzi, and microfilaria) and enteric helminths and protozoa were less common in pet monkeys. However, parasite communities showed overall low variation between the three contexts. Parasite richness (PR) was best explained by host genus and the city where the animal was sampled. Squirrel (genus Saimiri) and wooly (genus Lagothrix) monkeys had the highest PR, which was ~2.2 times the PR found in tufted capuchins (genus Sapajus) and tamarins (genus Saguinus/Leontocebus) in a multivariable model adjusted for context, sex, and age. Our findings illustrate that the threats of wildlife trafficking to One Health encompass exposure to multiple zoonotic parasites well-known to cause disease in humans, monkeys, and other species. We demonstrate these threats continue beyond the markets where wildlife is initially sold; monkeys trafficked for the pet market remain a reservoir for and contribute to the translocation of zoonotic parasites to households and other captive facilities where contact with humans is frequent. Our results have practical applications for the healthcare of rescued monkeys and call for urgent action against wildlife trafficking and ownership of monkeys as pets.

La apendicitis aguda se manifiesta, en ocasiones, con una variada e inespecífica presentación clínica, lo que dificulta su diagnóstico oportuno y favorece el riesgo de complicaciones. El objetivo es actualizar la información relacionada con las características clínicas de la apendicitis aguda, para lo que se realizó una revisión no sistemática de la literatura hasta abril del año 2022. Se revisaron artículos, libros especializados y citas bibliográficas de estudios elegidos, 20 de los cuales fueron seleccionados para la revisión. Mediante esta investigación se concluye que la primera y principal manifestación clínica es el dolor abdominal. El paciente debe someterse a una cuidadosa exploración física, incluyendo un examen rectal si lo precisa, y en las mujeres a un examen ginecológico si existiera duda diagnóstica.


Acute appendicitis is manifested, sometimes, with a varied and unspecific clinical presentation, which makes difficult its timely diagnosis and favors the complication risk. The objective is to update the information related to the clinical characteristics of acute appendicitis, for which a non-systematic review of the literature was carried out until April 2022. Articles, specialized books and bibliographic citations of selected studies were reviewed, 20 of which were chosen for the review. Through this research it is concluded that the first and main manifestation is abdominal pain. The patient should undergo a rigorous physical examination, including a rectal examination if necessary, and in the case of women, a gynecological examination if there is diagnostic doubt.

El Mes de la Ciencia surge como idea para socializar y divulgar las investigaciones y el nivel de actualización sobre los principales problemas de salud que afectan a la población matancera. Expone el nivel científico de las diferentes especialidades que ofrecen servicio en las instituciones de salud; permite la interdisciplinariedad entre ellas; motiva a los estudiantes del pregrado y resulta una fuente de gestión del conocimiento para el educando del posgrado; ofrece soluciones para los desafíos de la vida cotidiana y el manejo de las principales entidades que son atendidas en las instituciones, y permite el crecimiento profesional y el intercambio entre profesionales del país y del extranjero. Los autores se proponen mostrar los primeros resultados que ha tenido el Mes de la Ciencia a partir de su implementación. Estos están relacionados con el número considerable de conferencias magistrales y talleres impartidos, del incremento de participantes por mes y del nivel de satisfacción por eventos recogido por los principales actores. Aún más, se ha transformado de evento interhospitalario a uno con carácter provincial, con participación nacional e internacional y con alta aceptación por la comunidad científica, lo que le ha merecido el reconocimiento de los directivos provinciales y del ministro de Salud Pública.


Science month arises as an idea to socialize and disseminate research and the level of updating on the main health problems affecting the population of Matanzas. It exposes the scientific level of the different specialties that offer services in health institutions; it enables interdisciplinarity between them; motivates undergraduate students and is a source of knowledge management for postgraduate students; it offers solutions for the challenges of daily life and the management of the main entities that are served in the institutions, and allows the professional growth and interchange between professional in the country and abroad. The authors intend to show the first results the Science Month has had since its implementation. These are related to the considerable number of keynote conferences and workshops given, the increase in participants per month and the level of satisfaction per events collected by the main actors. Even more, it has been transformed from an inter-hospital event to one of a provincial character, with national and international participation, and with high acceptance by the scientific community, which has earned it the recognition of the provincial management staff and the minister of Public Health.

Introducción: El internado vertical en Medicina Intensiva y Emergencias forma parte de las nuevas estrategias del Ministerio de Salud Pública para la formación de profesionales en un menor tiempo, con el fin de vencer los múltiples retos que enfrenta la salud pública. Por esa razón, la educación médica superior cubana actual tiene, entre sus múltiples retos, el perfeccionamiento de los contenidos del trabajo metodológico en algunos de sus niveles y formas. Objetivo: Elaborar una alternativa metodológica basada en la semipresencialidad, para el proceso de enseñanza-aprendizaje del módulo Trauma del internado vertical de la especialidad de Medicina Intensiva y Emergencias. Métodos: Se realizó una investigación descriptiva, prospectiva y cuasiexperimental. El universo incluyó a 20 profesores relacionados de forma directa con el internado vertical. Se aplicó un post-test a los encuestados. Resultados: El diagnóstico sirvió de base para el diseño de la alternativa metodológica basada en la semipresencialidad, la cual es pertinente, oportuna y útil para elevar el nivel epistémico y el metodológico de buenas prácticas en la impartición eficaz del referido módulo en entornos virtuales de aprendizaje. Los resultados del post-test aplicado 30 días después de someter a consideración la alternativa metodológica, se informa en escalas de Likert, los cuales fueron óptimos. Conclusiones: La implementación de la alternativa metodológica basada en la semipresencialidad, para impartir los contenidos del módulo Trauma, es factible y necesaria(AU)


Introduction: The vertical internship in Intensive Care and Emergency Medicine is part of the new strategies of the Ministry of Public Health for the training of professionals in a shorter time, in order to overcome the multiple challenges that public health faces. For this reason, current Cuban higher medical education has, among its many challenges, the improvement of the contents of methodological work in some of its levels and forms. Objective: To develop a methodological alternative based on blended attendance, for the teaching-learning process of the Trauma module of the vertical internship in the specialty of Intensive Care and Emergency Medicine. Methods: A descriptive, prospective, quasi-experimental research was carried out. The universe included 20 teachers directly related to the vertical internship. A post-test was applied to the respondents. Results: The diagnosis served as the basis for the design of the methodological alternative based on blended attendance, which is relevant, timely and useful to raise the epistemic and methodological level of good practices in the effective delivery of the aforementioned module in virtual learning environments. The result of the post-test applied 30 days after submitting the methodological alternative for consideration are reported on Likert scales, which were optimal. Conclusions: The implementation of the methodological alternative based on blended attendance to teach the contents of the Trauma module, is feasible and necessary(AU)

La vacunación es una forma de contribuir a la protección de la población al reducir el riesgo de efectos graves de la enfermedad COVID-19. Para marzo de 2021, en tiempo récord, la industria biotecnológica cubana contaba con cinco candidatos vacunales. Se realizó una intervención sanitaria con un esquema heterólogo: dos dosis de SOBERANA®02 más una dosis de SOBERANA®Plus, en trabajadores durante el período de marzo a junio de 2021, en el Instituto Finlay de Vacunas, en La Habana, Cuba. Se evaluaron los efectos directos e indirectos de la vacunación con SOBERANA®02 y SOBERANA®Plus en una cohorte de riesgo de infección, enfermedad y diseminación de la COVID-19. La cohorte se estableció en marzo de 2021 en trabajadores con alta exposición al coronavirus de tipo 2 causante del síndrome respiratorio agudo severo, en el área de consulta médica de Instituto Finlay de Vacunas, establecida como sitio clínico. Entre el 22 de marzo de 2021 y el 11 de junio de 2021, se inscribieron un total de 1.776 participantes; de ellos, 1.719 cumplieron los criterios de inclusión con un porcentaje de 96,79 por ciento para la primera dosis, 1.675 recibieron la segunda dosis y 1.653 se vacunaron SOBERANA®Plus como tercera dosis para un 97,87por ciento. Mil cuatrocientos cincuenta y siete tenían entre 19 y 59 años, con predominio del sexo femenino. De los participantes, 175 tuvieron acontecimientos adversos y se observaron, predominantemente, una hora después de la administración de cada dosis. La reacción local más referida fue el dolor en el lugar de la inyección. Se registraron pocos acontecimientos adversos no solicitados. No se notificó ningún evento adverso grave o severo asociado a la vacuna. La distribución de casos de COVID-19 fue de 30, 16 y 6 posterior a cada dosis recibida. No se notificaron muertes asociadas a COVID-19. Las vacunas SOBERANA®02 y SOBERANA®Plus tuvieron un buen perfil de seguridad y fueron capaces de reducir la enfermedad grave por COVID-19 y la muerte, ayudando a revertir la situación epidemiológica causada por el coronavirus de tipo 2 causante del síndrome respiratorio agudo severo en Cuba(AU)


Vaccination is a way to help protect people by reducing the risk of serious effects from COVID-19 illness. By March of 2021, in record time, Cuba's biotech industry had five vaccine candidates. A sanitary intervention with a heterologous scheme, two doses of SOBERANA®02 and one dose of SOBERANA®Plus, was carried out in workers during the period of March to June 2021 at Finlay Vaccine Institute, in Havana, Cuba. We evaluated the direct and indirect effects of vaccination with SOBERANA®02 and SOBERANA®Plus, in a cohort at risk of infection, disease and spread of the epidemic COVID-19. The cohort was established in March 2021, among workers with high exposure to SARS-CoV-2, at the area of medical consultation at Finlay Vaccine Institute, established as clinical site. Between March 22, 2021 and June 11, 2021, were enrolled a total of 1,776 participants and, of them, 1,719 met the inclusion criteria with a percentage of 96.79percent for first dose, of which 1,675 received the second dose and 1,653 received SOBERANA®Plus as third dose for 97.87percent. The majority of participants were aged 19-59 years (1,457), being female, the predominant sex. Among the participants, 175 had adverse events, predominantly observed one hour after the administration of each dose. The most common local reaction was injection site pain. Few unsolicited adverse events were recorded. No vaccine-associated severe or serious adverse events were reported. The distribution of COVID-19 case was 30 post first dose, 16 post second dose and 6 post last dose. No deaths associated with COVID-19 were reported. SOBERANA®02 and SOBERANA®Plus vaccines had a good safety profile and were capable of a reduction of severe COVID-19 illness and death helping to reverse the epidemiological situation caused by the SARS-COV-2 in Cuba(AU)

Coronavirus disease 2019 (COVID-19) is caused by Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV2). COVID-19 can cause a cytokine release syndrome in which cytokines, including interleukin 17 (IL-17), are massively secreted in response to a specific stimulus. This can contribute to mortality and severe forms of COVID-19. The study aimed to determine the association of SARS-CoV2 infection with the IL-17A rs2275913 and IL-17F rs763780 variants, as well as with the associated comorbidities in COVID-19-positive Mexican patients. The study included 178 patients positive to COVID-19 and 177 COVID-19 negative subjects. For genotyping, the samples were amplified with a TaqMan® probe. There was no association between the AA genotype and A allele of IL-17A variant or the IL-17F C allele with the presence of COVID-19. In regard to comorbidities, a statistically significant association was found between IL-17A rs2275913 AA genotype and hypertension, as well as with the presence of obesity (P = 0.003, OR 23, 95% CI: 2.97-178.092 and P = 0.025, OR 28, 95% CI: 1.52-178.029, respectively) in patients with COVID-19. In conclusion, rs2275913 IL-17A polymorphism in COVID-19 patients seems to confer a higher susceptibility to the presence of hypertension and obesity, increasing the risk of premature cardiovascular disease in this population. However, more studies should be conducted for a better understanding of their relation.

OBJECTIVE: Genetic and antigenic polymorphism of P. vivax apical membrane antigen-1 (pvama1I-II) from Nicaragua was examined. MATERIALS AND METHODS: Infected blood samples from patients were obtained during 2012-2013. A gene fragment comprising domains I-II was amplified and sequenced, and the genetic parameters, haplotype relationships, genetic structure, and amino acid variation in predicted B cell epitopes were analyzed. RESULTS: 65 sequences of pvama1III had 19 nonsynonymous and five synonymous nucleotide changes. Nicaraguan parasites had low diversity, high linkage disequilibrium, and few recombination events. Neutrality tests indicate a positive and divergent selection, and three genetic clusters with loss of haplotypes were demonstrated. Amino acid variation predominated in predicted B cell epitopes and was closely related to that in Latin American parasites. CONCLUSIONS: Nicaraguan P. vivax is a moderately differentiated population under contraction and focalization processes, and the antigenic diversity resembles that of Latin American parasites. This information is relevant for vaccine development and epidemiological surveillance.

Introducción: El trauma implica una forma integral de manejo que requiere una significativa inversión en dos aspectos: económico y educativo. Los pacientes politraumatizados son difíciles de valorar, diagnosticar y tratar, ya que presentan un riesgo vital elevado, por lo que se requiere un diagnóstico y tratamiento rápido, complejo y multidisciplinario. Objetivo: Determinar cómo se establece la indicación de los estudios tomográficos en pacientes politraumatizados. Materiales y métodos: Se realizó una investigación descriptiva, transversal y prospectiva. El universo estuvo constituido por 43 pacientes con diagnóstico de politrauma, que ingresaron por el Servicio de Urgencias del Hospital Universitario Clínico Quirúrgico Comandante Faustino Pérez Hernández, y a los cuales se les indicaron estudios tomográficos, o no, para definir conducta. Resultados: Predominó el grupo etario de entre 31 y 60 años, y el sexo de las personas atendidas con traumas complejos estuvo representado en un 83,7 % por pacientes masculinos. La combinación de traumas fue la que predominó en el universo estudiado, con un 55,8 % de aparición, y predominaron las tomografías múltiples, para un 60,4 %. Conclusiones: La indicación de tomografías múltiples en pacientes politraumatizados no estuvo justificada en el mayor porciento de los casos. La no recogida de la cinemática del trauma, ni la aplicación de las escalas pronósticas que están protocolizadas, llevaron a cometer errores en las indicaciones de complementarios imagenológicos y en la actuación. Se detectó un uso innecesario del equipo, elemento que acorta su vida útil y ocasiona daño biológico al paciente.


Introduction: Trauma implies a comprehensive form of management that requires a significant investment in two aspects: economic and educative. Polytraumatized patients are difficult to assess, diagnose and treat, as they present a high vital risk, therefore fast, comprehensive and multidisciplinary diagnosis and treatment are required. Objective: To determine how the indication of tomographic studies in polytraumatized patients is established. Materials and methods: A descriptive, cross-sectional and prospective research was carried out. The universe consisted of 43 patients with diagnosis of polytrauma, who entered the Emergency Service of the Clinical Surgical University Hospital Comandante Faustino Perez Hernandez, and who were indicated tomographic studies or not, to define behavior. Results: The age group between 31 and 60 years predominated, and the sex of the people treated with complex trauma was represented in 83.7% by male patients. Trauma combination predominated in the studied universe, with an occurrence of 55.8%, and multiple tomographies predominated, for 60.4%. Conclusions: The indication of multiple tomographies in polytraumatized patients was not justified in the highest percentage of cases. The non-collection of the kinematics of the trauma, nor the application of the prognosis scales that are protocoled, led to mistakes in the indication of complementary imaging and in the performance. An unnecessary use of the equipment was detected, an element that shortens its useful life and causes biological damage to the patient.

Introducción: En los últimos años se han producido cambios profundos en el manejo del paciente politraumatizado. A la vez, se han desarrollado nuevos conceptos en relación con las posibles complicaciones, los esquemas de tratamiento y las escalas pronósticas, así como en la identificación de elementos relacionados con su evolución para determinar, de manera precoz, las lesiones que amenazan la vida y precisan de un control quirúrgico inmediato o de intervencionismo radiológico. La tomografía computarizada multicorte y otros estudios imagenológicos permiten la obtención de imágenes de las estructuras corporales por planos, y proporcionan información muy detallada y útil para el diagnóstico; sin embargo, no existe consenso a la hora de indicar uno u otro en el trauma. Objetivo: Elaborar un algoritmo para la indicación eficiente de los estudios imagenológicos en el paciente politraumatizado. Materiales y métodos: Se ejecutó una investigación de desarrollo tecnológico, donde el universo de trabajo estuvo conformado por 43 pacientes con criterio de politrauma -que necesitaron estudios imagenológicos-, ingresados en el Hospital Universitario Clínico Quirúrgico Comandante Faustino Pérez Hernández, en el período comprendido entre marzo de 2020 y marzo de 2021. Resultados: Se elaboró un algoritmo para estandarizar la indicación de estudios imagenológicos en el trauma. Este trabajo considera la seguridad y protección del paciente, y el cuidado de la vida útil del equipo de tomografía axial computarizada. Conclusión: El algoritmo diseñado viabiliza la toma de decisiones respecto al uso de recursos imagenológicos en la atención a los pacientes politraumatizados.


Introduction: In recent years there have been profound changes in the management of poly-traumatized patients. At the same time, new concepts have been developed in relation to possible complications, treatment schemes and prognostic scales, as well as in the identification of elements related with its evolution to early determine the life-threatening lesions and require immediate surgical control or radiological intervention. Multislice computed tomography and other imaging studies allow obtaining images of body structures by planes and provide very detailed and useful information for the diagnosis; nevertheless there is no consensus when it comes to indicating one or the other in trauma. Objective: To develop an algorithm for the efficient indication of imaging study in the poly-traumatized patient. Materials and methods: A technological development research was carried out, where the working universe was made up by 43 patients with polytrauma criteria -who needed imaging studies- admitted to the Clinical Surgical University Hospital Faustino Perez, in the period between March 2020 and March 2021. Results: An algorithm was developed for standardizing the indication of imaging studies in trauma. This work considers the safety and protection of the patient, and the care of the life of the computed axial tomography equipment. Conclusions: The developed algorithm enables decision-making regarding the use of imaging resources in the care of poly-traumatized patients.

INTRODUCCIÓN: La enfermedad renal crónica se encuentra en ascenso.Prevenir o retardar su progresión mediante la aplicación de estrategias dirigidas al diagnóstico precoz es esencial. OBJETIVO: Evaluar la utilidad de la fórmula HUGE para el diagnóstico de Enfermedad Renal Crónica en el anciano. MATERIAL Y MÉTODO: Se realizó un estudio observacional descriptivo prospectivo y de corte longitudinal en 260 adultos mayores que ingresaron en los servicios de Geriatría y Medicina Interna del Hospital Clínico Quirúrgico "Hermanos Ameijeiras" en el período enero de 2019 y junio de 2020. RESULTADOS: El 58,5% de la muestra de estudio fueron mujeres. La edad promedio fue de 77,1 ± 7,3 años. La enfermedad renal crónica estuvo presente en el 64,2% de los pacientes. Se observó mayor frecuencia de pacientes con daño renal (32,7%) al emplear la formula CKD­ EPI en comparación con los identificados al emplear la fórmula HUGE (25,0%). Al estimar la concordancia entre ambas fórmulas se observó un estadístico kappa (k) de 0,814 (IC de 95%:0,7370 - 0,8909; p < 0,001). La sensibilidad de la fórmula de HUGE fue de un 76,5% (IC de 95%: 66,9% - 86,1%) y la especificidad de un 100% (99,7% - 100%). El valor predictivo positivo fue de 100 % (IC de 95%: 99,2% - 100%) y el negativo de 89,7% (85,2% - 94,2%). CONCLUSIONES: La fórmula CKD-EPI identifica daño renal en mayor porcentaje, en estadios precoces. Por el contrario, la fórmula de HUGE, detecta el daño renal en un porcentaje mayor en estadios más avanzados.La concordancia para diagnosticar daño renal entre la fórmula CKD-EPI y HUGE fue muy buena. La fórmula HUGE es útil, sensible y específica para evaluar la enfermedad renal crónica en los adultos mayores.


INTRODUCTION: Chronic kidney disease is on the rise. Preventing or delaying its progression through the application of strategies aimed at early diagnosis is essential. OBJECTIVE: To evaluate the usefulness of the HUGE formula for the diagnosis of Chronic Kidney Disease in the elderly. MATERIAL AND METHOD: A prospective, descriptive and longitudinal observational study was carried out in 260 older adults who were admitted to the Geriatrics and Internal Medicine services of the "Hermanos Ameijeiras" Surgical Clinical Hospital between January 2019 and June 2020. RESULTS: 58.5% of the study sample was women. The mean age was 77.1 ± 7.3 years. CKD was present in 64.2% of the patients. A higher frequency of patients with kidney damage (32.7%) was observed when using the CKD ­ EPI formula compared to those identified when using the HUGE (25.0%). When estimating the concordance between both formulas, a kappa statistic (k) of 0.814 (95% CI: 0.7370 - 0.8909; p < 0.001) was observed. The sensitivity of the HUGE formula was 76.5% (95% CI: 66.9% - 86.1%) and the specificity was 100% (99.7% - 100%). The positive predictive value was 100% (95% CI: 99.2% - 100%) and the negative predictive value was 89.7% (85.2% - 94.2%). CONCLUSIONS: The CKD-EPI formula identifies kidney damage in a higher percentage, in early stages. On the contrary, the HUGE formula detects kidney damage in a higher percentage in more advanced stages. The concordance to diagnose kidney damage between the CKD-EPI and HUGE formula was very good. The HUGE formula is useful, sensitive, and specific for evaluating chronic kidney disease in older adults.

RESUMO

The burial of Individual 3 at the Newen Antug site, a young adult woman, with a pottery grave offering characteristic of the Late Pottery period and dated to 880 years BP, is an indirect burial in a wooden structure. The form and design comprise a wooden wampo or small canoe, or a symbolic representation of one, a metaphor in current and historical Mapuche society for the voyage to the final abode of the dead, located beyond a water body which must be crossed in a boat. This is the first find of a burial in a canoe structure in Argentinian Patagonia, and the most southern example on the whole continent. It is also the earliest record in Argentina of pottery of the Red on White Bichrome tradition used as a grave offering, extending the repertoire of characteristics shared between the two slopes of the Andes mountains during the pottery periods, including ritual as well as material aspects.

Metabolic alterations, resulting from factors such as obesity or infections (HIV), generate inflammation in the body, affecting the immune system and causing oxidative stress. Prolonged exposure to antiretroviral therapy (ART) conditions the appearance of alterations considered risk factors for metabolic syndrome (MetS), affecting the quality of life in people living with HIV/AIDS (PLWHA). ß-klotho is a protein that can counteract levels of oxidative stress. The aim was to determine the relation of ß-klotho and oxidative stress with metabolic alterations in PLWHA. We hypothesized that levels of ß-klotho and malondialdehyde (MDA) are related in PLWHA on ART with overweight/obesity. As a result of comparing cases versus controls, significant differences were obtained in levels of ß-klotho (p = 0.011), MDA (p < 0.0001), body mass index (p = 0.001), and weight (p < 0.0001). The presence of MetS in PLWHA was 21.2% and 10.6% according to the World Health Organization and ATP III (National Cholesterol Education Program Adult Treatment Panel III) criteria, respectively. The founded correlations were of ß-klotho (r = 0.019) and MDA (r = 0.0001), both with CD4+ cells in PLWHA. In controls, ß-klotho was correlated with very low-density lipoprotein (r = 0.035) and atherogenic index (AI; r = 0.037), MDA with AI (r = 0.039), cholesterol, and low-density lipoprotein (r = 0.002). The increase of inflammation in the organism, owing to HIV infection and/or the presence of obesity, conditions metabolic disruption or depletion of elements needed for homeostasis in the human body.

Fundamento: las herramientas tecnológicas que sustentan las redes digitales en la actualidad permiten numerosos intentos foráneos de convertir en patrones universales, ideas, gustos, emociones y corrientes ideológicas ajenas al contexto cubano. De ahí, la imperiosa necesidad de educar a niños y jóvenes en el respeto incondicional a la identidad cultural cubana. Objetivo: consolidar sentimientos identitarios de cubanía en estudiantes de segundo año de la carrera de Letras a partir de la lectura de una selección de textos en inglés, con sus correspondientes ejercicios. Métodos: se realizó una investigación cualicuantitativa. Se emplearon métodos teóricos: histórico-lógico, análisis-síntesis e inducción-deducción; empíricos: análisis documental, observación, prueba pedagógica, cuestionario y entrevista a la unidad de estudio seleccionada; y matemático-estadísticos. Resultados: en el análisis del programa se detectó que el tratamiento a la identidad cultural cubana no tenía el énfasis necesario, el libro de texto corresponde a la serie de libros face2face de autores europeos. El diagnóstico a la muestra determinó que los estudiantes no poseen toda la información que sobre la identidad cultural cubana necesita un profesional cubano, y manifestaron la necesidad de profundizar en estos temas; por lo que se conformaron textos en inglés con sus respectivos ejercicios aplicables antes, durante y después de la lectura, los que fueron valorados por especialistas. Conclusiones: la propuesta educativa fue valorada como muy adecuada por su pertinencia social, aplicabilidad, estructura lógica y originalidad, por los especialistas consultados.


Background: the technological tools that support electronic networks today allow numerous foreign attempts to convert ideas, tastes, emotions and ideological currents outside the Cuban context into universal patterns. Hence, the urgent need to educate children and young people in unconditional respect for Cuban cultural identity. Objective: to consolidate feelings of Cuban identity in second-year students of the Arts degree from the reading of a selection of texts in English, with their corresponding exercises. Methods: a qualitative-quantitative investigation was carried out. Theoretical methods were used: historical-logical, analysis-synthesis and induction-deduction; empirical ones: documentary analysis, observation, pedagogical test, questionnaire and interview with the selected study unit; and mathematical-statistical. Results: in the analysis of the program it was detected that the treatment of Cuban cultural identity did not have the necessary emphasis, the textbook corresponds to the series of face2face books by European authors. The diagnosis of the sample determined that the students do not have all the information that a Cuban professional needs about Cuban cultural identity, and they expressed the need to delve into these issues; therefore, texts in English were created with their respective applicable exercises before, during and after reading, which were assessed by specialists. Conclusions: the educational proposal was valued as very adequate due to its social relevance, applicability, logical structure and originality, by the specialists consulted.

BACKGROUND: Antiretroviral therapy has significantly improved prognosis in treatment against HIV infection, however, prolonged exposure is associated to cardiovascular diseases, lipodystrophy, type 2 diabetes, insulin resistance, metabolic alteration, as obesity which includes the accumulation of oxidative stress in adipose tissue. FGF21 is a peptide hormone that is known to regulate glucose and lipid metabolism. FGF21 is expressed and secreted primarily in the liver and adipose tissue, promoting oxidation of glucose/fatty acids and insulin sensitivity. Alterations in FGF21 may be associated with the development of insulin resistance, metabolic syndrome and cardiovascular disease. We hypothesized that FGF21 protein levels are associated with metabolic abnormalities, placing special attention to the alterations in relation to the concurrence of overweight/obesity in people living with HIV (PLWH). DESIGN: Serum FGF21 was analyzed in 241 subjects, 160 PLWH and 81 unrelated HIV-uninfected subjects as a control group. Clinical records were consulted to obtain CD4+ cell counting and number of viral RNA copies. Serum FGF21 levels were tested for correlation with anthropometric and metabolic parameters; glucose, cholesterol, HDL, LDL, VLDL, triglycerides, insulin and indexes of atherogenesis and insulin resistance (HOMA). RESULTS: The participants were classified into four groups: (i) PLWH with normal weight, (ii) PLWH with overweight/obesity, (iii) HIV-uninfected with normal weight, and (iv) HIV-uninfected with overweight/obesity. Insulin levels were higher in normal-weight PLWH than in the HIV-uninfected group but not statistically significant, however, for the overweight/obesity PLWH group, insulin levels were significantly higher in comparison with the other three groups (p<0.0001). For FGF21, serum levels were slightly higher in the overweight/obesity groups in both patients and controls. In HIV-infected subjects, FGF21 levels showed a strong positive correlation with triglycerides, insulin levels and insulin resistance with a p-value <0.0001. In the seronegative group, FGF21 was only correlated with weight and waist circumference, showing an important association of FGF21 levels with the degree of obesity of the individuals. CONCLUSION: Insulin resistance and FGF21 elevations were observed in overweight-obese PLWH. FGF21 elevation could be viewed as a compensation mechanism as, in the control group, FGF21 correlations appeared to be confined to weight and waist circumference. This can be explained based on the action of FGF21 promoting the uptake of glucose in adipose tissue. In PLWH, FGF21 was low, possibly as a result of a change in adiposity leading to a metabolic disruption.

ABSTRACT: To examine potential risk factors associated with biochemical alterations in renal function in a population diagnosed with HIV/AIDS undergoing antiretroviral treatment.This is an observational, transversal, and relational design study that included 179 HIV-seropositive subjects. Glucose serum, cholesterol, triglycerides, total proteins, albumin, creatine, urea, blood urea nitrogen (BUN), and electrolytes levels were determined for each individual. Renal function was evaluated through the glomerular filtration rate (GFR), using the CKD-EPI equation. Chronic kidney disease (CKD) was defined as estimated glomerular filtration rate  < 60 mL/min/1.73 m2. Univariate model significant variables, with a 95% confidence interval (CI), were included in a multivariate logistic regression analysis.CKD prevalence in patients was 7.3%, with comorbidities of 7.8% for type 2 diabetes mellitus, 7.3% for arterial hypertension, and 35.2% for dyslipidemia. Additionally, both hypernatremia and hypophosphatemia were detected in 57% (n = 102) of the patients. Multivariate logistic regression suggested that CD4+ T cell count < 200 (P = .02; OR 0.2; CI 95% 0.08-0.8) was associated to hyponatremia; similarly, detectable viral load was associated to hypokalemia (P = .02; OR 5.1; CI 95% 1.2-21.3), hypocalcemia (P = .01; OR 4.1; CI 95% 1.3-12.3), and hypermagnesemia (OR 3.9; CI 95% 1.1-13.6). Patient age was associated to both hypophosphatemia (P = .01; OR 2.4; CI 95% 1.1-5.0) and hypermagnesemia (P = .01; OR 2.8; IC 95% 1.1-7.0), and high creatinine levels were associated to nucleoside reverse transcriptase inhibitor treatment (P = .001; OR 42.5; CI 95% 2.2-806.9). Lastly, high BUN levels were associated to age (P = .03; OR 3.8; CI 95% 1.0-14.4), while GFR 60 to 89 mL/min/1.73 m2 was associated to dyslipidemia (P = .02; OR 2.2; CI 95% 1.1-4.5).CD4+ T cell and viral load were the main factors associated with renal biochemical alterations.

La sexualidad es un aspecto importante en la calidad de vida de la mujer, sin embargo, los estudios sobre sexualidad dedicados a las mujeres de 60 años y más son escasos. El objetivo de este estudio fue determinar la prevalencia de la disfunción sexual femenina y sus dominios en mujeres de 60 años y más, e identificar la asociación entre enfermedades asociadas y consumo de medicamentos con la disfunción sexual. Se realizó un estudio descriptivo y transversal, en mujeres de 60 años y más, de La Habana, Cuba. Participaron 112 mujeres a las cuales se les aplicaron dos encuestas: el Índice de Función Sexual Femenino (IFSF) y otra que recogía antecedentes médicos y sociales. El 66,1% de las mujeres presentó algún grado de disfunción sexual, la prevalencia aumentaba con la edad. Los dominios más afectados fueron el deseo, la excitación y la lubricación. Se identificó una fuerte asociación entre la enfermedad de Parkinson, la depresión y la artrosis con la aparición de disfunción sexual en la población estudiada, así como el uso de antidepresivos, hipoglicemiantes orales y diuréticos. La disfunción sexual estuvo presente en el 100% de las mujeres octogenarias. Todas las mujeres con enfermedad de Parkinson presentaron disfunción sexual.


Sexuality is an important aspect in women's quality of life, however, sexuality studies dedicated to women age 60 and older are scarce. The objective of this study was to determine the prevalence of female sexual dysfunction and its domains in women age 60 and older, and to identify the association between diseases and drug use with sexual dysfunction. A descriptive and cross-cutting study was conducted, in women 60 years and older, in Havana, Cuba. Two surveys participated in 112 women: The Female Sexual Function Index (IFSF) and another who collected a medical and social history. 66.1% of women had some degree of sexual dysfunction, the prevalence increased with age. The domains most affected were desire, excitement, and lubrication. A strong association between Parkinson's disease, depression and osteoarthritis was identified with the onset of sexual dysfunction in the studied population, as well as the use of antidepressants, oral hypoglycemics and diuretics. Sexual dysfunction was present in 100% of octogenary women. All women with Parkinson's disease had sexual dysfunction.

RESUMO

The structure of B-DNA, the physiological form of the DNA molecule, has been a central topic in biology, chemistry and physics. Far from uniform and rigid, the double helix was revealed as a flexible and structurally polymorphic molecule. Conformational changes that lead to local and global changes in the helix geometry are mediated by a complex choreography of base and backbone rearrangements affecting the ability of the B-DNA to recognize ligands and consequently on its functionality. In this sense, the knowledge obtained from the sequence-dependent structural properties of B-DNA has always been thought crucial to rationalize how ligands and, most notably, proteins recognize B-DNA and modulate its activity, i.e. the structural basis of gene regulation. Honouring the anniversary of the first high-resolution X-ray structure of a B-DNA molecule, in this contribution, we present the most important discoveries of the last 40 years on the sequence-dependent structural and dynamical properties of B-DNA, from the early beginnings to the current frontiers in the field.

RESUMEN Introducción: el síndrome compartimental abdominal es una entidad clínica sistémica desencadenada por incremento en la presión intraabdominal, caracterizada clínicamente por distensión abdominal y alteraciones de las funciones respiratoria, cardiovascular, neurológica y renal. Objetivo: caracterizar el comportamiento del síndrome compartimental abdominal en los pacientes críticos que ingresaron en la unidad de cuidados intensivos del Hospital Provincial "José Ramón López Tabrane". Materiales y método: se realizó un estudio prospectivo, descriptivo, y longitudinal que tuvo como universo los pacientes adultos con factores de riesgo conocidos para síndrome compartimental abdominal, tratados entre enero de 2014 a diciembre de 2015. Se le realizó medición sistemática de la presión intraabdominal transvesical y fueron sometidos a descompresión quirúrgica en caso de hipertensión intraabdominal grados III y IV. Resultados: los valores de presión intraabdominal sostenidamente elevados, son directamente proporcionales al desarrollo del síndrome compartimental abdominal y trae aparejado disfunciones en los diferentes sistemas de órganos; en estos casos apareció complicaciones, las cuales se presentaron combinadas para todos los pacientes, y fueron mayoritarias para el grupo con presión intraabdominal grado IV. La mortalidad se comportó en un número bastante elevado lo cual estuvo relacionado con los niveles de presión intraabdominal, así como la posibilidad de que fueran reintervenidos quirúrgicamente estos pacientes. Conclusiones: se debe protocolizar en todo paciente con sospecha de desarrollar un síndrome compartimental abdominal la medición periódica de la presión intraabdominal (AU).


SUMMARY Introduction: the abdominal compartment syndrome is a systemic clinical entity triggered by an increase of the intra abdominal pressure, clinically characterized by abdominal distension and the alteration of the renal, neurological, cardiovascular and respiratory functions. Objective: to characterize the behavior of the abdominal compartment syndrome in critical patients from the intensive care unit of the provincial hospital "Jose Ramon Lopez Tabrane". Materials and methods: a longitudinal, descriptive and prospective study was carried out in a universe of adult patients with risk factors known as abdominal compartment syndrome, treated in the period of time from January 2014 to December 2015. Their transvesical intraabdominal pressure was systematically measured and they underwent surgical decompression in cases of intraabdominal hypertension grades iii and iv. Results: the values of intraabdominal pressure steadily increased are directly proportional to the development of the abdominal compartment syndrome and entails dysfunctions in the different systems of organs; in these cases complications were found, that were combined for all patients and mainly in the group with intraabdominal pressure grade iv. Mortality was present in a certainly increased group, and was related to the levels of intraabdominal pressure, and also to the possibility these patients undergoing surgical treatments again. Conclusions: it should be standardized the periodical measure of the intraabdominal pressure in any patient suspected of developing an abdominal compartment syndrome (AU).

RESUMEN Introducción: la cardiopatía isquémica es tan antigua como el hombre. Constituye uno de los problemas de salud más serios a nivel mundial. Entre sus formas clínicas está el infarto agudo de miocardio. Los síndromes isquémicos agudos, representan un espectro clínico continúo sustentado por una fisiopatología común. Objetivo: identificar los factores que causaron retraso en la aplicación del tratamiento trombolítico en los pacientes, los que se traducen en el futuro, en pérdida de calidad de vida, diferentes grados de discapacidad, y hasta del aumento de la mortalidad por esta entidad. Materiales y métodos: se realizó un estudio observacional, con 62 pacientes con criterio de trombolisis atendidos en la Unidad de Cuidados Intensivos del Hospital Docente Clínico Quirúrgico "José Ramón López Tabrane", en el período comprendido entre enero del 2012 y junio del 2014. Se aplicaron métodos empíricos, estadísticos y teóricos. Resultados: de 147 pacientes recibidos con criterios de trombolisis 62 no recibieron el tratamiento trombolítico que representó un 42,2 %. Entre las causas estuvieron falta de entrenamiento de los médicos para la aplicación de dicho tratamiento y la no existencia de transporte para su traslado en el tiempo requerido. Conclusiones: los pacientes no trombolizados con más de 12 h sin diagnóstico con criterio de infacto agudo del miocardo, incidió en el diagnóstico erróneo del médico y valencia de sus propios medios, para llegar a un centro de salud. Con contraindicación absoluta, el accidente vascular isquémico menor de tres meses fue la causante más padecida. Con contraindicación relativa fue la hipertensión arterial no controlada (AU).


SUMMARY Introduction: The Ischemic Cardiopatía is as old as the man. One constitutes from the most serious problems of health to world level and enter their clinical forms the sharp heart attack of miocardio is (IMA), causing in the world population a third of the deaths. The sharp ischemic syndromes SIAs represents a continuous clinical spectrum sustained by a fisiopatología común. Objective: He/she was carried out an observational, descriptive and traverse study, in 62 patients with trombolisis approach assisted in the UCIE of the Surgical Clinical Educational Hospital José Ramón López Tabrane, in the period understood between January of 2012 and June of the 2014.Se they applied the empiric, statistical and theoretical methods. Materials and method: identify the factors that caused delay in the application of the treatment trombolítico in this patients, which are translated in the future in loss of quality of life, different discapacidad grades and until of the increase of the mortality for this entity. Among the causes that were they were: the non training of the doctors for the application of this treatment and the not existence of transport for their transfer in the required time. Conclusion: In patient non trombolizados with more than twelve hours without diagnostic of IMA (19,4%) with approach impacted the doctor's erroneous diagnosis and valency of their own means to arrive to a center of health. With absolute contraindication, the Ischemic Vascular Accident smaller than three months was the causing one more suffered. With relative contraindication it was the arterial hypertension not controlled (AU).
