Abstract Introduction: Aquatic birds (AB) are usually associated with wetlands, which provide refuge, food, and/or nesting sites for resident and migratory species. Despite their ecological importance, there is little knowledge on AB in some tropical environments, such as those found on the Colima coast. Objective: To investigate the spatial and temporal composition of the AB community in Juluapan Lagoon, Colima, Central Mexican Pacific. Methods: Monthly counts were conducted between June 2017 and May 2018 during low-tide conditions to record habitat use by AB. Species richness and bird counts were obtained to compare sampling areas; mean richness and number of individuals were compared between seasons. Results: We detected 53 species and 5 750 individuals. The highest species richness and relative abundance values were obtained in winter at the lagoon area farthest from the connection with the marine system, where anthropogenic activity is lower. Diversity was greater in zones 2 and 3 in spring, summer, and fall. Muddy flats were the most used environment, and the most frequent activity was resting. Nesting activity was only recorded in the middle of the lagoon at the mangrove during spring. "Shorebirds" and "waders" were the most dominant groups in the bird community of the Juluapan lagoon. Conclusions: This coastal wetland is a site of great biological importance for aquatic birds; thus, conservation measures should be implemented, and there should be a continuous study of the effects of anthropogenic pressure.


Resumen Introducción: Las aves acuáticas (AA) son usualmente relacionadas a los humedales debido a que éstos funcionan como sitios de refugio, alimentación y anidación de diferentes especies residentes y migratorias. Sin embargo, el conocimiento sobre las aves acuáticas en algunos humedales es nulo. Objetivo: Investigar la composición espacio-temporal de la comunidad de AA en la laguna Juluapan, Colima, en el Pacífico Central Mexicano. Métodos: Entre junio de 2017 y mayo de 2018 se llevaron a cabo conteos mensuales en condiciones de marea baja para registrar el uso de hábitat de las AA. Se obtuvieron valores de riqueza de especies y número de individuos para realizar comparaciones entre zonas de muestreo, así como el promedio del número de especies y número de individuos para comparaciones entre temporadas. Resultados: Se registraron un total de 53 especies y 5 750 individuos. Los valores de riqueza de especies y densidad de individuos fueron más altos durante invierno, en la zona más alejada al ambiente marino, donde la actividad antropogénica es menor. La diversidad tuvo valores más altos en la zona 2 y 3, durante primavera, verano y otoño. El ambiente más explotado por las aves fueron las planicies lodosas; y el descanso fue la actividad más frecuente. Asimismo, la actividad de anidación sólo se registró en el manglar de la zona media durante primavera. Las "aves playeras" y "aves zancudas" fueron los grupos más predominantes en la comunidad de aves de la laguna Juluapan. Conclusiones: Este humedal costero es un sitio de gran importancia biológica para aves acuáticas, por lo que resulta necesario la implementación de medidas de conservación, así como el estudio de los efectos por la presión antropogénica.

Introducción: La atención en salud involucra aspectos culturales que deben conocerse para beneficiar a las personas desde el cuidado de enfermería. Objetivo: Describir el uso de la etnoenfermería como diseño de investigación y la teoría del cuidado cultural de Madeleine Leininger en la literatura de enfermería. Metodología: Se realizó una revisión integrativa bajo las fases de Withmoore y Knalf. La búsqueda bibliográfica se llevó a cabo en Medline, Cuiden Plus, Lilacs, Scopus y ScienceDirect, con los descriptores "investigación cualitativa", "enfermería", "asistencia sanitaria culturalmente competente" y "enfermería transcultural" en idioma inglés, español, portugués, incluyendo artículos originales publicados entre 2013 a 2023. Resultados: Se eligieron 12 artículos que fueron codificados para el surgimiento de cuatro categorías: necesidad de formación en etnoenfermería y teoría del cuidado de la cultura; reconocimiento del contexto cultural y barreras para el cuidado culturalmente congruente; diversidad y universalidad de los cuidados culturales; y acciones para el cuidado culturalmente congruente. Conclusiones: La revisión permitió reconocer que la teoría de cuidado cultural y la etnoenfermería han sido escasamente utilizadas en la práctica científica y profesional de enfermería. Es necesario incentivar en los profesionales la importancia de valorar el respeto por las diferentes culturas en los servicios de salud.

Objetivo: Descrever a utilização da etnoenfermagem como desenho de investigação e a teoria cultural do cuidado de Madeleine Leininger na literatura de enfermagem. Métodos: Foi efetuada uma revisão integrativa segundo as fases de Withmoore e Knalf. A pesquisa bibliográfica foi realizada nas bases de dados Medline, Cuiden Plus, Lilacs, Scopus e ScienceDirect, com os descritores "pesquisa qualitativa", "enfermagem", "assistência à saúde culturalmente competente" e "enfermagem transcultural" em inglês, espanhol e português, incluindo artigos originais publicados entre 2013 a 2023. Resultados: Selecionou-se 12 artigos, codificados para o surgimento de quatro categorias: necessidade de formação em etnoenfermagem e teoria cultural do cuidado; reconhecimento do contexto cultural e barreiras para o cuidado culturalmente congruente; diversidade e universalidade do cuidado cultural; e ações para o cuidado culturalmente congruente. Conclusões: A revisão permitiu reconhecer que a teoria dos cuidados culturais e a etnoenfermagem têm sido escassamente utilizadas na prática científica e profissional da enfermagem. É necessário incentivar nos profissionais a importância de valorizar o respeito pelas diferentes culturas nos serviços de saúde..


Introduction: Health care involves cultural aspects that must be known to benefit people from the Nursing care provided. Objective: To describe the use of Ethnonursing as a research design and Madeleine Leininger's Theory of Culture Care in the Nursing literature. Methods: n integrative review was carried out following the phases proposed by Withmoore and Knalf. The bibliographic search was conducted in Medline, Cuiden Plus, Lilacs, Scopus and ScienceDirect. The "qualitative research", "nursing", "culturally competent health care" and "transcultural nursing" descriptors were used in English, Spanish and Portuguese, including original articles published between 2013 and 2023. Results: A total of 12 articles were chosen and coded to give rise to four categories, namely: need for training in Ethnonursing and in the Theory of Culture Care; recognition of the cultural context and of barriers to culturally congruent care; culture care diversity and universality; and actions for culturally congruent care. Conclusions: The review allowed recognizing that the Theory of Culture Care and Ethnonursing have been scarcely used in the scientific and professional Nursing practice. It is necessary to encourage professionals to value due respect for different cultures in health services.

ABSTRACT Introduction: Bovine cattle raising is the most important line of the economy of Caquetá department and El Doncello municipality, according to the figures given by the Departmental Committee of Cattlemen there is a herd of2,175,065 cattle heads in the region, many of the rural families depend on said economic activity to generate their income; To do this, the life analysis methodology examines the assets that communities have which are called community capital, and the approach they use to meet their fundamental human needs. Objective: To assess sustainable means and the application of community capital in cattle rancher families in the municipality of El Doncello, Caquetá (Colombia). Materials and methods: For the collection of information the producer families of the municipality of El Doncello were assessed; based on the sources of the Departmental Committee of Cattlemen, for this purposefamilies were surveyed and participatory workshops were carried out using the livelihood methodology framing the community capitals (human, natural, financial, physical and social) using 36 variables for the assessment of them, and the fundamental human needs (basic of the person, the environment and of action) that grouped 15 analysis variables; Subsequently, the interpretation and analysis of the data was made through aprincipal component analysis-PCA relating the typologies and each of the variables assessed in this study; all the data were run in the InfoStat Program. Results: It was found that 939 families base their economy on livestock production in the municipality of El Doncello, the human capital of these families was distributed in 72 men representing 15.55 %, 81 women (17.49 %), 143 boys (30.89 %)and 167 girls (36.07%), finding three types of families: Typology 1: families with <50 bovines on theirfarms; Typology 2: between 51 and 100 bovines, and Typology 3: >101 bovines, where financial capital is the one that represents the greatest contribution given its characteristics of income generation and well-beingfor families. The principal component analysis-ACP carried out for social capital divided according to the first principal component-PC1 with a contribution of 62.3 % to Family Typology 1 (families with <50 head of cattle) to the positive end of said component strongly associated with the variables greater participation in projects and organizations to which they belong, with the particularity of having a greater number of members at home; from Typology 3 to the negative end of said component associated with the variables greater number of shared spaces in which they participate and greater level of family participation. In the same way, for natural capital, a separation was observed between Family Typology 1, associated with the variables greater number of water sources, mainly due to the fact that a large part of these producers were in the mountain range zone, greater area in stubble, greater number of spaces used and more activities for consumption, such as plantations of plantain, cocoa, cassava and other subsistence crops and Family Typology 2 strongly associated with the variables greater number of trees in pastures and greater percentage of natural products. One of the most important capitals within the producers is the financial one, for which the following was observed: the first principal component-PC1, with a contribution of 43.7%, separates Typology 1 from the positive end of said component associated with the variable income from daily wages and income from subsidies, these families are the ones that generate the least amount of assets and through which they diversify their income that are not only dependent on livestockproduction as such, but also on other types of economic activities, of the Typology 2 to the negative extreme associated with the variables income from transfers, credits, income from formal employment and income from the sale ofprocessed products, such as cheese. The second main component-PC2, with a contribution of 24.4 %, groups Family Typology 3 strongly associated with the highest income from the sale of space-use products, These families, by owning a greater number of cattle heads, base a large part of their economy in said activity, but they diversify it with other types of income such as rubber crops, cocoa, pineapple, plantain, cassava, among others. Finally, the fundamental human needs-FHN can be defined as very similar for the three types of families studied, with slight adjustments in the basic needs for Typology 1, where food, health, shelter and security are classified as bad and regular. Although the situation does not vary much for Typology 2, but it does look better for Typology 3 in which satisfaction is good, except for security, everyone has the same perception, this is mainly due to the presence of certain illegal armed groups in the region. Regarding the needs of the person, the environment and action, there are no variations between the different types. Conclusions: Finally, it was possible to observe a high cultural tie that cattle ranchers have for this economic activity that is evidenced in human capital, where we see the number of people who directly depend on livestock and its derivatives. Also, in terms of financial capital, it can be observed that there is a great diversification of the income of families that not only depend on livestock, but also on income from formal employment, daily wages, loans and sales of other crops such as plantain, cassava, rubber and cocoa; In terms of social and natural capital, a good sustainable appropriation of environmental care and the participation of rural communities are observed. Finally, a low contribution to the fundamental human needs of the producing communities is denoted, this mainly due to the social and economic conditions of the region; In this sense, this study has repercussions as a first diagnosis in the management of the livelihoods of rural cattle-raising communities in the department of Caquetá.

In December 2020, Argentina approved a new abortion law following decades of feminist and social advocacy. This paper presents qualitative findings from interviews and focus group discussions with people in local communities focusing on how individuals of reproductive age access and communicate sexual and reproductive health information, particularly regarding abortion. Sixteen in-depth interviews were conducted with key informants working in the field of SRHR and four focus group discussions took place with cisgender women and girls, transmasculine people and non-binary people of reproductive age. We found that information exchange and communication about sexual and reproductive health issues, particularly abortion, took place mainly through informal social networks engaging with activists and feminist grass-root organisations. These informal social networks were built on trust as a collective affect that enabled open communication about abortion. Information sharing through word of mouth, in person and via digital means using different social media platforms, is an important means of information sharing and communication in Argentina. Monitoring the implementation of abortion policies in this country should include investigating the impact of people accessing abortion through informal social networks in terms of abortion pathways and intersections with the formal health system.

PURPOSE: To develop Mexico's first methodologically rigorous clinical practice guideline for the management of adult overweight and obesity. The target audiences are interdisciplinary healthcare professionals across healthcare systems who are the first point of contact for patients with obesity in Mexico, patients, and health system decision makers. RECENT FINDINGS: A review of recent international obesity clinical practice guidelines and an expert consensus process identified: i) common recommendations appropriate for implementation in Mexico and ii) knowledge gaps requiring the formulation of new recommendations. In all, 20 new recommendations and 20 good practice statements were developed using the GRADE Evidence-to-Decision Framework and expert consensus. Overweight and obesity negatively impact the health and well-being of individuals and populations in Mexico. This guideline aims to establish a new evidence-based, patient-centered, non-stigmatizing, and practical treatment and management framework, based on the fundamental principles of chronic disease prevention and management.

This study aimed to investigate the effects of the cell-free supernatant of Lactiplantibacillus plantarum ATCC® 10241TM on the biofilm-forming capacity of Pseudomonas aeruginosa strains isolated from cystic fibrosis (CF) patients. In addition, the study evaluated the in vivo potential of the cell-free supernatant to modulate inflammation and reduce lung damage in mice infected with P. aeruginosa strains or co-challenged with P. aeruginosa and the Streptococcus milleri group (SMG). The results showed that CF-derived P. aeruginosa strains can infect the respiratory tract of adult mice, inducing local inflammation and lung damage. The severity of these infections was exacerbated when P. aeruginosa was co-administered with SMG. Notably, nebulization with the cell-free supernatant of L. plantarum produced beneficial effects, reducing respiratory infection severity and inflammatory responses induced by P. aeruginosa, both alone or in combination with SMG. Reduced bacterial loads and lung damage were observed in supernatant-treated mice compared to controls. Although further mechanistic studies are necessary, the results show that the cell-free supernatant of L. plantarum ATCC® 10241TM is an interesting adjuvant alternative to treat P. aeruginosa respiratory infections and superinfections in CF patients.

Human immunodeficiency virus (HIV) is a global public health problem. Coinfections in HIV patients are frequent complications that increase their mortality. The aim of this study was to assess coinfections and in-hospital mortality in a group of patients infected with HIV in Colombia. A retrospective longitudinal study was carried out. Patients treated in 4 highly complex clinics in Colombia between 2015 and 2023 were included. The cases were identified from International Classification of Diseases codes related to HIV. Sociodemographic, clinical, laboratory and pharmacological variables were collected. Descriptive, bivariate, and multivariable analyses were performed. Of the 249 patients identified, 79.1% were men, and the median age was 38.0 years. Approximately 81.1% had a diagnosis of acquired immune deficiency syndrome (AIDS). Coinfections caused by Mycobacterium tuberculosis (24.1%) and Treponema pallidum (20.5%) were the most frequent. A total of 20.5% of the patients had sepsis, 12.4% had septic shock, and the fatality rate was 15.7%. Antibiotics and antifungals were used in 88.8% and 53.8%, respectively, of the patients. Patients with a diagnosis of HIV before admission, those infected with M. tuberculosis, and those who presented with sepsis were more likely to die, whereas patients who received antiretroviral agent treatment before admission presented a lower risk. In this study, most HIV patients were in an advanced stage of the disease. Coinfection with M. tuberculosis was common and was associated with an increased risk of death. Previous HIV diagnosis and sepsis also increased the risk. Approximately half of the patients with a previous HIV diagnosis were receiving antiretroviral therapy and had a better prognosis.

The high degree of chemical modification of the chitosan chains due to protonated amine groups allows them to react with many negatively charged surfaces as anionic polymers and cell membranes, resulting in an attractive material for medical and pharmaceutics applications. Incorporating ionic iodine (I- and IO3 -) on chitosan chains is a direct way to successfully obtain chitosan-iodine nanoparticles (CSNPs-I and CSNPs-IO3) through ionic gelation. The nanoparticles (NPs) present a hemispherical morphology with sizes around 30-70 nm for CSNPs-I and CSNPs-IO3, similar to chitosan NPs, in accordance with SEM and DLS techniques. The XRD characterization did not show noticeable differences in the crystallinity index (CI) for CSNPs and CSNPs-I, 48.4 and 49.3%, respectively, but for CSNPs-IO3, the CI decreased to 43.85%. The cytotoxic effects on human tumor cells of chitosan and iodine-modified chitosan nanoparticles (CSNPs-I and CSNPs-IO3) were evaluated for 24 h in a range from 0.15 mg/mL to 0.95 mg/mL concentrations, where CSNPs-IO3 presented the lower viability for lung cancer A549, followed by cervical cancer HeLa cell and finally breast cancer MDA-MB-231, with a weight content of iodate ion in a range of 8.7 to 15 µg. This work presents the possibility of exploring chitosan-iodine NPs in medical applications.

Aim: to identify whether demographic variables, the type and length of romantic relationships, and alcohol consumption were risk factors related to attitudes toward intimate partner violence (IPV). Method: 723 adults filled the IPVAS and a demographic survey. Results: women were less likely to accept control; participants with technical education were more likely to tolerate control and abuse; those with secondary education were at higher risk to accept physical violence, while participants aged 26-35 were at lower risk; people who were dating someone were at higher risk to tolerate control, but were at a lower risk to accept physical violence; and those who consume alcohol were more likely to accept abuse. Conclusion: As attitudes toward IPV may predict its future occurrence, research on variables modulating the association between attitudes and IPV execution can strengthen evidence to implement preventive actions, in which shaping attitudes towards IPV are the primary target.


Objetivo: Identificar si las variables sociodemográficas, el tipo, la duración de las relaciones y el consumo de alcohol son factores de riesgo asociados con las actitudes hacia la violencia de pareja (VP). Método: 723 adultos completaron las IPVAS y una encuesta sociodemográfica. Resultados: las mujeres tuvieron menor probabilidad de aceptar el control; los participantes con educación técnica toleraron más control y abuso; las personas con educación secundaria presentaron mayor riesgo de aceptar violencia física, mientras que las personas entre 26 y 35 años mostraron un riesgo menor; quienes tenían una relación tuvieron riesgo mayor para tolerar control, pero menor riesgo para aceptar violencia física; aquellos que reportaron consumir alcohol tuvieron mayor probabilidad de aceptar abuso. Conclusión: Las actitudes hacia la IPV pueden predecir la ejecución de esta conducta en el futuro. Por lo tanto, estudios que indaguen sobre variables que modulen esta asociación incrementarán la evidencia para implementar acciones preventivas, en las que la formación de actitudes hacia la VP sean el objetivo principal de cambio.

Progressive multiple sclerosis (PMS) represents the worsening phase of the disease by accumulative neurodegeneration and disability, mainly refractory to current treatments. The therapeutic options remain challenging based partially on the lack of understanding of the pathogenic mechanisms but also because the early dogma was centered on neuroinflammation, overshadowing the critical role of the tissue repair process. The tissue repair target should necessarily start early in disease development and PMS should combine anti-inflammatory and neuroprotective therapeutic strategies. Increasing preclinical evidence, together with the new era of omics applied on frozen human brain tissue, shed light on some ligand receptors axis, such as GAS6/TYRO3 and PROS1/AXL required to dampen inflammation, promote tissue repair and engage remyelination, at the early stages of multiple sclerosis (MS) as a critical step in preventing or stopping neurodegeneration. Here, we will discuss those receptor/ligand pairs that could be targetable for therapeutic intervention in progressive MS disease. Significance Statement The aim for PMS should be to combine anti-inflammatory and neuroprotective therapeutic strategies based on early intervention. The TYRO3, AXL, and MERTK (TAM) signaling axis, particularly GAS6/TYRO3 and PROS1/AXL, which are involved in tempering inflammation, promoting tissue repair, and engaging remyelination, could significantly benefit patients at the early PMS.

Objective: This study aims to evaluate the clinical outcomes of surgical management for placenta accreta spectrum in a Latin American reference hospital specializing in this condition. The evaluation involves a comparison between surgeries performed on an emergent and scheduled basis. Methods: A retrospective cohort study was conducted on patients with placenta accreta spectrum who underwent surgery between January 2011 and November 2021 at a hospital in Colombia, using data from the institutional PAS registry. The study included patients with intraoperative and/or histological confirmation of PAS, regardless of prenatal suspicion. Clinical outcomes were compared between patients who had emergent surgeries and those who had scheduled surgeries. Descriptive analysis involved summary measures and the Shapiro-Wilk test for quantitative variables, with comparisons made using Pearson's Chi-squared test and the Wilcoxon rank sum test, applying a significance level of 5%. Results: A total of 113 patients were included, 84 (74.3%) of them underwent scheduled surgery, and 29 (25.6%) underwent emergency surgery. The emergency surgery group required more transfusions (72.4% vs 48.8%, p=0.047). Patients with intraoperative diagnosis of placenta accreta spectrum (21 women, 19.5%) had a greater volume of blood loss than patients taken into surgery with known presence of placenta accreta spectrum (3500 ml, IQR 1700 - 4000 vs 1700 ml, IQR 1195-2135. p <0.001). Conclusion: Patients with placenta accreta spectrum undergoing emergency surgery require transfusions more frequently than those undergoing scheduled surgery.

Introduction Virologic failure due to antiretroviral drug resistance is a threat to efforts to control the human immunodeficiency virus (HIV) epidemic. Understanding the factors that influence the genetic and clinical expression of drug resistance is fundamental for infection control. Methods A nested case-control study was conducted on a cohort of adult HIV patients between 2016 and 2022. The cases were defined as patients with a confirmed diagnosis of virologic failure due to drug resistance, as indicated by a viral genotype result. The control group consisted of patients who had not experienced virologic failure or undergone any documented changes to their antiretroviral treatment. The incidence of virologic failure over a defined period was calculated. The characteristics of each group were documented in frequency tables and measures of central tendency. To identify risk factors, multiple logistic regression models were employed, and post hoc tests were conducted. All calculations were performed with 95% confidence intervals, and p-values less than 0.05 were considered significant. Results The incidence of virologic failure over the seven-year study period was 9.2% (95% CI: 7.5-11.2%). Low CD4 T-lymphocyte count (≤200 cells/mm³) at diagnosis (adjOR 14.2, 95% CI: 3.1-64.5), history of opportunistic infections (adjOR 3.5, 95% CI: 1.9-6.4), and late enrollment into an HIV program after diagnosis (>1 year) (adjOR 9.2, 95% CI: 3.8-22.2) were identified as independent predictors of virologic failure. The drugs with the highest rates of resistance were nevirapine (84.6%), efavirenz (82.4%), emtricitabine (81.3%), lamivudine (81.3%), and atazanavir (6.6%). The most prevalent major mutations identified were K103N, M184V, and M46I/M. Approximately 50% of the secondary mutations were identified in protease regions. Conclusions The incidence of virologic failure was low in the study population. The identified risk characteristics allow for the prediction of the profile of patients susceptible to failure and for the early optimization of treatment regimens.

RESUMO

In early 2024, explosive outbreaks of Oropouche virus (OROV) linked to a novel lineage were documented in the Amazon Region of Brazil. We report the introduction of this lineage into Colombia and its co-circulation with another OROV lineage. Continued surveillance is needed to prevent further spread of OROV in the Americas.

RESUMO

Dengue cases rose to record levels during 2023-2024. We investigated dengue in Valle del Cauca, Colombia, to determine if specific virus serotypes or lineages caused its large outbreak. We detected all 4 serotypes and multiple lineages, suggesting that factors such as climatic conditions were likely responsible for increased dengue in Colombia.

OBJECTIVE: To assess effects of Healthy Change intervention on maternal perception of her child's body weight (MPCW), maternal feeding style, and obesogenic home environment. METHODS: A randomized control trial was conducted, consisting of two arms: the intervention group received the Healthy Change program, and the control group received the Hygiene and Accident Prevention program. A total of 356 mother-preschool child dyads participated, 182 in the intervention group and 174 in the control group, residing in Mexico and the United States. Data were collected at baseline and after the program through self-administered questionnaires completed by mothers and child anthropometric measurements. RESULTS: Although no significant between-group difference in pre- and post-intervention change of MPCW was found, sub-analyses revealed that a higher proportion of mothers in the intervention group accurately perceived their child's body weight at the study endpoint using categorical (67% vs. 57.1%, p < 0.005) and visual scales (48.9% vs. 41.8%, p < 0.015). Additionally, more mothers of overweight children in the intervention group accurately perceived their children's overweight and obese status compared to those in the control group (29.8% vs. 10.3%, X2 = 4.26, df = 1, p < 0.039). The intervention group also displayed a higher proportion of mothers with authoritative feeding style (26.4% vs. 16.5%, p < 0.036) and significantly higher family nutrition and physical activity scores (29.1 vs. 28.0, p < 0.000) at the study endpoint. CONCLUSIONS FOR PRACTICE: Healthy Change Intervention led to improved accuracy of MPCW, a shift toward maternal authoritative feeding styles, and positive changes in obesogenic home environments.

Objetive: Congenital heart diseases (CHD) can be found in pregnant women. Although cardiac interventions in the catheterization laboratory are considered safe and effective, it is preferable to wait 3-6 months after delivery to correct simple, uncomplicated CHD; however, this may result in follow-up losses. The objective is to present our experience in correcting CHD during the early puerperium (EP). Materials and methods: All cases of pregnant women with CHD, including atrial septal defect (ASD), patent ductus arteriosus (PDA), and aortic coarctation (CoA) between 2017-2023, who underwent percutaneous defect correction during the EP were collected. Results: Fifteen pregnant women were included, diagnosed with ASD (5), PDA (6), and CoA (4). Five patients (33.3%) were classified as WHO risk class IV; the procedure was successful in 80% of the cases, and only 1 patient presented complications. Conclusions: In our experience, the closure of uncomplicated congenital defects during the EP did not present major complications and could be a treatment strategy to prevent follow-up losses after delivery in pregnant women with CHD.

Objective: Several studies have demonstrated an association between frailty and worse outcomes in patients with acute coronary syndrome (ACS); however, there is a lack of evidence from Colombia. This study aims to evaluate the association between frailty and the risk of adverse outcomes in patients over 65 years old diagnosed with ACS. Materials and methods: A prospective cohort study was conducted, including patients over 65 years old who underwent coronary angiography due to an ACS diagnosis at a hospital in Medellín, Colombia. Frailty was assessed using the FRAIL scale. The primary outcome was all-cause mortality at 30 days. Secondary outcomes included length of hospital stay and a composite outcome of in-hospital or 30-day mortality, contrast-induced nephropathy (CIN), acute heart failure, cardiogenic shock, hemorrhagic complications, and vascular complications. Results: A total of 112 patients were included. Frail patients (n=35, 31.3%) were older, had a lower socioeconomic status, higher GRACE scores, and more severely compromised coronary vessels. A significant association was observed between frailty and 30-day mortality (relative risk [RR] 19.00, 95% confidence interval [CI]: 5.04-72.61; p<0.001), the composite outcome (RR 4.57, 95% CI: 2.56-8.34; p<0.001), and longer hospital stays (9 days vs. 5 days in the non-frail group). Conclusions: A considerable number of patients over 65 years old with ACS were frail. Frailty was associated with adverse in-hospital and 30-day outcomes.

BACKGROUND: The factors influencing sedentary time (ST) of individuals with chronic obstructive pulmonary disease (COPD) have not been thoroughly explored. AIM: To identify determinant factors of ST in individuals with stable COPD. METHODS: Individuals with COPD had their ST and variables of physical activity (PA) cross-sectionally assessed during seven days with an activity monitor. Main variables were ST/day (<1.5 METs), steps/day, time/day in light PA (light PA/day, 1.5-2.9 METs) and in moderate-to-vigorous PA (MVPA/day, ≥3 METs). Additional assessments included 6-min walking test (6MWT), Medical Research Council (MRC) scale, lung function and body composition. Multiple linear regression models were built with variables correlating significantly with ST/day. RESULTS: 50 individuals were analyzed (44 % males; 66 ± 8 years; FEV1 50 ± 19%pred). ST/day was 488 ± 160 min (61 ± 15 % of the day). 6MWT, MRC scale, MVPA/day, steps/day and light PA/day correlated significantly with ST expressed as minutes/day or as % of the day. In the multiple regression analyses, variables explaining the variance of ST in minutes/day were MRC scale (1 %) and light PA/day (53 %) (model R2 = .541, p < 0.001) and of ST in % of the day were steps/day (53 %) and light PA/day (46 %) (model R2 = .994, p < 0.001). CONCLUSION: In individuals with COPD, the variance in sedentary time can be explained by dyspnea in daily life, step count and specially time/day in light PA, which reinforces the increase in light PA (rather than necessarily MVPA) as a strategy to reduce sedentary time.

INTRODUCTION AND AIM: Chronic pancreatitis is a pathologic fibroinflammatory syndrome of the pancreas. Treatment includes medical management and endoscopic and/or surgical interventions. Our aim was to describe progression in patients with chronic pancreatitis whose initial management was either endoscopic or surgical. MATERIAL AND METHODS: A retrospective, analytic, observational, and longitudinal study was conducted that included patients diagnosed with chronic pancreatitis treated at the Hospital de Especialidades of the Centro Médico Nacional Siglo XXI from 2015 to 2021. RESULTS: Twenty-two patients were included in the study; 12 underwent endoscopy and 10 underwent surgery. The mean number of interventions performed was 3 in the endoscopic management group and 1.1 in the surgical management group (p = 0.001). Regarding pain remission (partial or total), results were statistically significant in favor of surgical management (p = 0.035). Of the 12 patients that initially underwent endoscopy, 7 (58.3%) eventually required surgery during follow-up. There were no statistically significant differences with respect to opioid and pancreatin use, readmissions, weight loss, steatorrhea, newly diagnosed diabetes, or deaths during follow-up. CONCLUSION: Pain is the main indication for invasive treatment, whether endoscopic or surgical, in patients with chronic pancreatitis. Slightly more than half of the patients that were initially managed endoscopically required surgery during follow-up. Management decisions should be multidisciplinary and individualized for each patient.
