INTRODUCTION: Patient satisfaction is the degree of conformity with the healthcare they receive. It is real evidence and one of the most important factors in determining the effectiveness and quality of healthcare systems. OBJECTIVE: To identify the quality of care in the Urology outpatient department of a third-level hospital. MATERIALS AND METHODS: The NHS (National Health Service) 2018 quality of care questionnaire with 11 sections, 133 items, and duration of approximately 25min was randomly administered to 250 patients attending Urology outpatients at a third-level public hospital in Mexico. RESULTS: According to responses, 92% (n=230) knew the reason for the consultation. 64.8% (n=162) had a consultation with the same physician by whom they were initially seen. The longest reported hospital wait time before being seen was more than 2h in 29.6% (n=74). As for consultation time, 212 patients responded and the duration was 11-20min in 52.8% (n=112). Finally, 33.2% (n=83) considered the quality of service to be good. CONCLUSIONS: The use of the NHS 2018 survey in the Urology service at a third-level public hospital in Mexico is feasible, since we managed to obtain a significant and continuous improvement in all its indicators which is satisfactory for all.

Leptospirosis is a widely distributed zoonosis caused by pathogenic strains of bacteria of the genus Leptospira (Phylum Spirochaetes). Its agents are commonly classified based on their antigenic characteristics into serogroups and serovars, which are relevant for epidemiologic studies and vaccine development. Serological tests are considered laborious and require a specialized infrastructure. Some molecular methods have been proposed to accelerate these procedures, but they still can not replace the immunological tests, thus requiring a further understanding of the genetic basis underlying the serological classification. In this work, we focused on elucidating the genetic factors determinant for the serogroup Sejroe, which is one of the most prevalent serogroups in livestock. For this, we conducted a comparative analysis using >700 leptospiral genomic samples available in the public database. The analysis showed that the genes comprising the rfb locus are the main genetic factors associated with the serological classification. Samples from the serogroup Sejroe have an rfb locus with a conserved gene composition that differs from most other serogroups. Hebdomadis and Mini were the only serogroups whose samples have an rfb locus with similar gene composition to those from serogroup Sejroe, corroborating with the serological affinity shared by them. Finally, we could determine a small region in the rfb locus in which each of those three serogroups can be distinguished by its gene composition. This is the first work that uses an extensive repertoire of genomic data of leptospiral samples to elucidate the molecular basis of the serological classification and open the road to more reliable strategies based on molecular methods for serodiagnosis.

The incidence of Leishmania/HIV co-infection is growing and few studies detail the cellular processes and macromolecules participating in co-infection. Thus, the goal of this study was to partially describe the Leishmania/HIV co-infection events by measuring molecular and functional parameters associated with both pathogens in vitro. MT-4 cells (human T-lymphocytes), primary monocytes, and peripheral blood mononuclear cells were exposed to HIV and/or Leishmania donovani. The cytopathic effects generated by the pathogens were observed through microscopy. Viral replication was assessed by monitoring p24 protein levels and parasitic proliferation/infectivity was determined using Giemsa staining. Changes in molecular markers were evaluated by ELISA and fluorescence assays. Our results showed that our system reassembles the main parameters previously described for Leishmania/HIV co-infection in patients in terms of potentiation of parasitic and viral replication/infectivity, amplification of syncytia induction, and alterations of cell viability. In addition, an amplification in NF-κB activation, changes in CXCR4/CCR5 surface expression, and a Th1→Th2 variation in cytokine/chemokine secretion were demonstrated. Altogether, this study could contribute to gain a deep understanding of the molecular events associated with Leishmania/HIV co-infection.

This study was conducted to develop predictive equations for carcass characteristics and primal cut weights of native Mexican guajolotes using body measurements (BM). For this study, a total of 36 male guajolotes (Meleagris gallopavogallopavo), aged 6 to 10 months, and mean slaughter body weight (SBW) of 4543.14 ± 656.60 g, were used. The birds were kept under traditional extensive conditions. ThefollowingBMswererecorded24 h before slaughter: thoracicperimeter (TP), body circumference (BC), body length (BL), wing length (WL), keel length (KL), shank length (SL) and shank diameter (SD). After slaughter, hot carcass weight (HCW), cold carcass weight (CCW), hot dressing percentage (HDP), cold dressing percentage (CDP), organs and viscera weight (VIS) and abdominal fat weight (AFW) were recorded. The carcasses were dissected in to five primal cut (breast, thigh, drumstick, back and wing). The SBW and BMs showed moderate to high positive correlations (p<0.01; 0.34≤r<0.97) with carcass characteristics and primal cut weights. In the equations generated to predict HCW, CCW, HDP, CDP, VIS and AFW, the R2 ranged from 0.40 to 0.96, and the predictor variables were SBW, KL, BC, WL and SL. Regarding the equations developed to predict the primal cut weights, R2 ranged from 0.58 to 0.91. In these models, SBW, BC, SD, WL and KL explained most of the observed variation. The prediction equations obtained in the study had moderate to high accuracy; therefore, they can be used by researchers, technicians and poultry producers to obtain information on the carcass composition of native Mexican guajolotes.(AU)

BACKGROUND: NCBI Taxonomy is the main taxonomic source for several bioinformatics tools and databases since all organisms with sequence accessions deposited on INSDC are organized in its hierarchical structure. Despite the extensive use and application of this data source, an alternative representation of data as a table would facilitate the use of information for processing bioinformatics data. To do so, since some taxonomic-ranks are missing in some lineages, an algorithm might propose provisional names for all taxonomic-ranks. RESULTS: To address this issue, we developed an algorithm that takes the tree structure from NCBI Taxonomy and generates a hierarchically complete taxonomic table, maintaining its compatibility with the original tree. The procedures performed by the algorithm consist of attempting to assign a taxonomic-rank to an existing clade or "no rank" node when possible, using its name as part of the created taxonomic-rank name (e.g. Ord_Ornithischia) or interpolating parent nodes when needed (e.g. Cla_of_Ornithischia), both examples given for the dinosaur Brachylophosaurus lineage. The new hierarchical structure was named Taxallnomy because it contains names for all taxonomic-ranks, and it contains 41 hierarchical levels corresponding to the 41 taxonomic-ranks currently found in the NCBI Taxonomy database. From Taxallnomy, users can obtain the complete taxonomic lineage with 41 nodes of all taxa available in the NCBI Taxonomy database, without any hazard to the original tree information. In this work, we demonstrate its applicability by embedding taxonomic information of a specified rank into a phylogenetic tree and by producing metagenomics profiles. CONCLUSION: Taxallnomy applies to any bioinformatics analyses that depend on the information from NCBI Taxonomy. Taxallnomy is updated periodically but with a distributed PERL script users can generate it locally using NCBI Taxonomy as input. BACKGROUND: Sequential treatment of Panitumumab (Pb) plus Paclitaxel (Px) as induction treatment (IT) followed by concurrent bioradiotherapy (Bio-RT) with Pb may be an alternative for locally advanced squamous cell carcinoma of the head and neck (LA-SCCHN) in patients ineligible for high-dose cisplatin therapy. METHODS: Phase II, single-arm, multicentre study, with two-stage design, in patients ≥ 18 years with stage III-IVa-b LA-SCCHN unfit for platinum. Patients received Px + Pb (9 weeks) as IT followed by Bio-RT + Pb. Primary endpoint: overall response rate (ORR) after IT, defined as: more than 70% of patients achieving complete response (CR) or partial response (PR) to IT. Secondary end-points: progression-free survival, organ preservation rate, safety profile. RESULTS: Study ended prematurely (51 patients) due to slow recruitment. ORR: 66.7% (95% CI: 53.7-79.6), 8 (15.7%) CR and 26 (51.0%) PR. 39 patients (76%) completed radiotherapy (RT). Pb and/or Px-related adverse events (AEs) grade 3-4: 56.9% during IT and 63.4% during the concomitant phase, of which most common were skin toxicity (33.3%). Five deaths occurred during treatment, two of them (3.9%) were Pb and/or Px-related. CONCLUSIONS: Although underpowered, ORR was higher than the pre-specified boundary for considering the treatment active. Although Px + Pb as IT provides some benefit, the safety profile is worse than expected. To consider Pb + Px as IT as an alternative for platinum-unsuitable LA-SCCHN, further research/investigation would be needed.

The study was done to predict egg weight from the external traits of the Guinea fowl egg using the statistical methods of multiple linear regression (MLR) and regression tree analysis (RTA). A total of 110 eggs from a flock of 23-week-old Guinea fowl were evaluated. Egg weight (EW) and external traits: eggshell weight (ESW), egg polar diameter (EPD), egg equatorial diameter (EED), egg shape index (ESI), and egg surface area (ESA) were measured. Descriptive statistics, Pearson correlation coefficients, and regression equations using the MLR were obtained; additionally, a RTA was done using the CHAID algorithm with the SPSS software (IBM ver. 22). EW presented positive correlations (p<0.0001) with ESA (r = 0.72), EPD (r = 0.65), and EED (r = 0.49). EW can be predicted through MLR using ESA as a predictor variable (R2 = 72%). Predictive accuracy improves when adding EPD and EED traits to the model (R2 = 75%). The RTA built a diagram using ESA, EED, and EPD as significant independent variables; of these, the most important variable was ESA (F = 50,295, df1 = 4, and df2 = 105; Adj. p<0.000) and the variation explained for EW was 74%. Likewise, the RTA showed that the highest egg weight (41.818 g) is obtained from eggs with a surface area > 59.03 cm2 and a polar diameter > 5.10 cm. The proposed statistical methods can be used to reliably predict the egg weight of Guinea fowl.(AU)

We demonstrate that the presence of edges in a superconducting film made of a type-I/type-II bilayer stabilizes type-II/type-I hybrid (inter-type) flux patterns, as vortex clusters, chains, and gel phase. These patterns are very sensitive to primary parameters such as applied magnetic field, layer coupling, and temperature. Thus, the magnetization versus temperature curves, M(T), for many values of coupling were used to estimate the strength of the layer couplings, and also as a guide for obtaining sequentially the flux patterns. We also show that the effect of the borders on the unrestricted states is to shift them to states of higher density, since they introduce extra compression on the vortex matter. For a low layer coupling regime, we observe an unusual magnetic response where few partial vortices (partial in a sense they miss the contribution of the type I part), repelling each other and bounded to the surfaces, populate one layer leaving the other empty. We expect that the predicted flux configurations can stimulate experimentalists in trying to observe them by direct imaging techniques.

Recent reports have shown that small and big felines could be infected by SARS-CoV-2, while other animals, like swines and mice, are apparently not susceptible to this infection. These findings raise the question of the role of cell factors associated with early stages of the viral infection in host selectivity. The cellular receptor for SARS-CoV-2 is the Angiotensin Converting Enzyme (ACE2). Transmembrane protease serine 2 (TMPRSS2) has been shown to prime the viral spike for its interaction with its receptor. GRP78 has also been proposed as a possible co-receptor. In this study, we used several bioinformatics approaches to bring clues in the interaction of ACE2, TMPRSS2, and GRP78 with SARS-CoV-2. We selected several mammalian hosts that could play a key role in viral spread by acting as secondary hosts (cats, dogs, pigs, mice, and ferrets) and evaluated their predicted permissiveness by in silico analysis. Results showed that ionic pairs (salt bridges, N-O pair, and long-range interactions) produced between ACE2 and the viral spike has an essential function in the host interaction. On the other hand, TMPRSS2 and GRP78 are proteins with high homology in all the evaluated hosts. Thus, these proteins do not seem to play a role in host selectivity, suggesting that other factors may play a role in the non-permissivity in some of these hosts. These proteins represent however interesting cell targets that could be explored in order to control the virus replication in humans and in the intermediary hosts.

El presente trabajo tiene el objetivo de explorar el desarrollo de las tecnologías de la información y comunicación (componentes de eSalud) en servicios de salud públicos de México, así como indagar sobre los facilitadores y barreras para su implementación. Se realizó un estudio de caso en el Instituto de Servicios de Salud del Estado de Aguascalientes utilizando un abordaje mixto de tipo concurrente y de predominancia cualitativa que incluyó observación no participante, entrevistas semiestructuradas y un cuestionario dirigido al personal médico. A través del estudio, se identifican diversas oportunidades de mejora en cuanto al acceso y desarrollo de las tecnologías de la información y comunicación; también se identifican diversas barreras para su implementación factibles de superarse. En cuanto a las limitaciones del estudio, se reconoce la exploración de perspectivas sólo desde la oferta de servicios. En lo que respecta a la originalidad, destaca la investigación integral de diferentes componentes de eSalud. Se concluye que es necesario fortalecer las políticas de las tecnologías de información y comunicación dirigidas al sector salud con el fin de optimizar su desarrollo e implementación


The purpose of this work is to explore the development of the Information and Communication Technologies (e-Health components) in public health services of Mexico, as well as to inquire into the enablers and barriers to its implementation. A case study was conducted at the Instituto de Servicios de Salud del Estado de Aguascalientes using a mixed approach of concurrent type and qualitative predominance that included non-participant observation, semi-structured interviews and a questionnaire addressed to medical staff. Through the study, diverse improvement opportunities for the access and development of the Information and Communication Technologies were identified; diverse barriers to its implementation were also. The study has the limitation of taking only the services offer perspectives. The originality of this study is the comprehensive research of e-Health components. It is concluded that it is necessary to strengthen the Information and Communication Technologies policies focused to the health sector in order to optimize its development and implementation

OBJECTIVE: Data normalization and clustering are mandatory steps in gene expression and downstream analyses, respectively. However, user-friendly implementations of these methodologies are available exclusively under expensive licensing agreements, or in stand-alone scripts developed, reflecting on a great obstacle for users with less computational skills. RESULTS: We developed an online tool called CORAZON (Correlations Analyses Zipper Online), which implements three unsupervised learning methods to cluster gene expression datasets in a friendly environment. It allows the usage of eight gene expression normalization/transformation methodologies and the attribute's influence. The normalizations requiring the gene length only could be performed to RNA-seq, meanwhile the others can be used with microarray and/or NanoString data. Clustering methodologies performances were evaluated through five models with accuracies between 92 and 100%. We applied our tool to obtain functional insights of non-coding RNAs (ncRNAs) based on Gene Ontology enrichment of clusters in a dataset generated by the ENCODE project. The clusters where the majority of transcripts are coding genes were enriched in Cellular, Metabolic, Transports, and Systems Development categories. Meanwhile, the ncRNAs were enriched in the Detection of Stimulus, Sensory Perception, Immunological System, and Digestion categories. Vectoral and alignment-free approaches to biological sequence representation have been explored in bioinformatics to efficiently handle big data. Even so, most current methods involve sequence comparisons via alignment-based heuristics and fail when applied to the analysis of large data sets. Here, we present "Spaced Words Projection (SWeeP)", a method for representing biological sequences using relatively small vectors while preserving intersequence comparability. SWeeP uses spaced-words by scanning the sequences and generating indices to create a higher-dimensional vector that is later projected onto a smaller randomly oriented orthonormal base. We constructed phylogenetic trees for all organisms with mitochondrial and bacterial protein data in the NCBI database. SWeeP quickly built complete and accurate trees for these organisms with low computational cost. We compared SWeeP to other alignment-free methods and Sweep was 10 to 100 times quicker than the other techniques. Dealing with electromyography (EMG) signals is often not simple. The nature of these signals is nonstationary, noisy, and high dimensional. These EMG characteristics make their predictability even more challenging. Cross recurrence plots (CRPs) have demonstrated in many works their capability of detecting very subtle patterns in signals often buried in a noisy environment. In this contribution, fifty subjects performed ten different hand movements with each hand with the aid of electrodes placed in each arm. Furthermore, the nonlinear features of each subject's signals using cross recurrence quantification analysis (CRQA) have been performed. Also, a novel methodology is proposed using CRQA as the mainstream technique to detect and classify each of the movements presented in this study. Additional tools were presented to determine to which extent this proposed methodology is able to avoid false classifications, thus demonstrating that this methodology is feasible to classify surface EMG (SEMG) signals with good accuracy, sensitivity, and specificity. Lastly, the results were compared with traditional machine learning methods, and the advantages of using the proposed methodology above such methods are highlighted.

OBJECTIVE: To assess the outcome of arthroscopic release in three patients with chronical lateral epicondylitis. MATERIAL AND METHODS: Arthroscopic release in three patients with lateral epicondylitis is performed. Mayo Clinic scale for evaluation of results is used. A review and discussion of the literature is made. RESULTS: Three patients, two female and one male, the common activities was principal labors, not athletes. Patients had significant pain. It was the principal symptom that affect the score of the rating scale. These scores improved after surgery. It was achieved early return to normal daily activities. No neurological complications were reported. DISCUSSION: Arthroscopic treatment was an alternative safe and effective for treating chronical lateral epicondilitis in this three cases. It allows simultaneous joint exploration for diagnostic purposes and to treat associated pathologies. Broader Series and studies are necessary in order to establish definitive protocols in our cases.


OBJETIVO: Mostrar el resultado a corto plazo de la liberación artroscópica en pacientes que presentan epicondilitis crónica lateral. MATERIAL Y MÉTODOS: Se realiza liberación artroscópica de tres pacientes con epicondilitis lateral. Seguimiento de seis meses. Se utiliza la escala de Clínica Mayo para valoración de resultados. Se lleva a cabo revisión y discusión de la literatura. RESULTADOS: Dos pacientes del género femenino y uno del género masculino dedicados a las actividades cotidianas, no deportistas. El dolor fue el síntoma capital que afectó el puntaje de la escala de valoración. Estos puntajes mejoraron luego de la cirugía. Se logró retorno precoz a sus actividades cotidianas. No se reportaron complicaciones neurológicas. DISCUSIÓN: El tratamiento artroscópico resultó una alternativa segura y eficaz para el tratamiento de la epicondilitis lateral en estos tres pacientes, el cual permite realizar simultáneamente exploración articular con fines diagnósticos y tratar patologías asociadas. Se requieren series más amplias y estudios comparativos a fin de establecer protocolos definitivos en nuestra casuística.

We show how the inclusion of a structural defect of determined geometry controls the vortex state in a square superconducting sample in the presence of an external magnetic field and a dc current. We simulated the defects by using the deformation parameter τ ( x , y ) , solving the non-lineal time-dependent Ginzburg-Landau equations and using the link variable method, for four different geometries as possible options for the storage vortex, simulating the behavior of a capacitor. We found an exponential dependence of the current in which the first vortex penetrates the sample J → c as a function of the area of a square central defect in the sample. We also show the effect of the defects and the transport current on the magnetization, magnetic susceptibility, vorticity, and magnetic field at the first vortex entry into the sample H 1 and the density of the superconducting electrons.

Ovalbumin is considered a protein of high nutritional value because it contains essential amino acids and is highly digestible. Therefore, it has a high biological value. Currently, the high food demand requires worldwide attention because food production is insufficient. Therefore, other alternatives are necessary to satisfy food demands, such as protein engineering. In this work, a protein with a high essential amino acid content similar to ovalbumin was synthesized by protein engineering, expressed, and digested in vitro. The assembly and sequential overlap extension PCR strategy was used to synthesize a 345-bp gene that encodes a high essential amino acid content protein (HEAAP). The 345-bp product was cloned into the vector pBAD TOPO®, and expressed in Escherichia coli BL21. PCR reactions and sequencing demonstrated the presence, orientation, and correct sequence of the insert. HEAAP expression was induced by L-arabinose and then purified using Ni-NTA affinity chromatography. The expression in E. coli was low and barely detected by Western blot assay. The in vitro multienzyme digestibility of HEAAP was around 79%, which suggests that the protein is potentially nutritious. Virtual analysis classifies the protein as unstable and hydrophilic, with a half-life in E. coli of 10 h. The recombinant HEAAP was successfully synthesized, but it is necessary to improve the digestibility and to optimize expression including selecting other expression models.

Abstract: Objective: To assess the outcome of arthroscopic release in three patients with chronical lateral epicondylitis. Material and methods: Arthroscopic release in three patients with lateral epicondylitis is performed. Mayo Clinic scale for evaluation of results is used. A review and discussion of the literature is made. Results: Three patients, two female and one male, the common activities was principal labors, not athletes. Patients had significant pain. It was the principal symptom that affect the score of the rating scale. These scores improved after surgery. It was achieved early return to normal daily activities. No neurological complications were reported. Discussion: Arthroscopic treatment was an alternative safe and effective for treating chronical lateral epicondilitis in this three cases. It allows simultaneous joint exploration for diagnostic purposes and to treat associated pathologies. Broader Series and studies are necessary in order to establish definitive protocols in our cases.

RESUMO

Surface pattern formation on amorphous SiO2 substrates by implantation of 1.0 MeV Si+ ions at a current of 1.3 µA at 70° angle is reported. Surface micrometer sized ripples perpendicular to the ion beam direction are formed, observed by scanning electron microscopy and atomic force microscopy. The morphological features are more or less similar for different fluences. The formation of surface ripples at this energy is discussed in terms of ion stopping mechanisms and patterns obtained within the low- and medium-energy ranges.
