RESUMO

In rhinoplasty, the nasal dorsum represents an important aspect for form and function. Surgical management should be performed accurately to avoid complications. The goal for dorsal preservation is to preserve the keystone area intact above the nasal bones and its relationship with the upper lateral cartilages. Unlike other rhinoplasty techniques, the lowering of the nasal dorsum requires dorsal modification prior to the surgery of the tip since it modifies the tip position by rotating it. Unlike the push-down technique, that places the nasal bones inside the nasal cavity, producing obstruction of the airway, the let-down technique improves the airway and releases the valve area, making it a functional-friendly technique.

One of the first signs of aging belongs to the upper third of the face. With the aging process, mestizo facial features become more prominent. The thicker skin-soft tissue envelope (S-STE) has a tendency to sag more, hooding of the eyelids tends to be more pronounced, and there is a tendency for eyebrows to droop, specifically the tail of the brows, because of the loss of support. A "tired" or "sad" look implies that the complex eyebrow-upper eyelid is showing one or more of these signs. Different surgical as well as non-surgical techniques have been described to treat this area, every one of them aiming at making the patient look rested and natural. The objective of this study is to describe a technique for endoscopic brow lifting, consisting on minimal incisions, a biplanar dissection, and a different fixation technique designed for helping reshape the brow. This particular surgical technique has shown the advantage of being minimally invasive and effective. Careful analysis of the patient should be made to decide both the technique and the changes desired by the patient and the surgeon. The authors believe the technique described is another option for approaching and fixating the heavy eyebrows in mestizo patients.

One of the first signs of aging belongs to the upper third of the face. At the same time, the height and shape of the eyebrows are key points of the periorbital aesthetics. A "tired" or "sad" look implies that the complex eyebrow-upper eyelid are showing one or more of these signs. Different surgical techniques as well as nonsurgical have been described to treat this area, every one of them aiming at making the patient look rested and natural. The objective of this study is to describe a technique for endoscopic browlifting, consisting of minimal incisions, a biplanar dissection, and a different fixation technique designed for helping reshape the brow. Twenty-five patients who fulfilled the criteria for the study were analyzed for brow-position changes in height and shape. All the patients were treated by the senior authors using the technique described. This particular surgical technique has shown the advantage of being minimally invasive and effective. Careful analysis of the patient should be made to decide both the technique and the changes desired by the patient and the surgeon. The authors believe the technique described is another option for approaching and fixating the eyebrow.

Material y Métodos: Se estudiaron 21 casos durante un período de 2 años, todos los pacientes fueron sometidos a ventilación jet trans traqueal percutánea. En 14 pacientes el procedimiento se realizó para microcirugía laríngea, en 5 casos se utilizó para valorar la vía aérea en pacientes con quemadura por inhalación, en un caso se utilizó por trauma laríngeo y en un caso como medida de urgencia para restablecer la vía aérea en un paciente con fracturas faciales de alto impacto, epistaxis masiva y complicaciones diversas. Resultados: En ningún caso se encontraron complicaciones inherentes al procedimiento. Conclusión: La ventilación jet trans traqueal percutánea es un procedimiento útil y usualmente sin complicaciones
