Nine hundred thirty four samples of urethral secretion proceeding from male subjects without previous treatment were analyzed with Gram stain. These subjects consulted in Santiago Western area, and belong to medium and low socioeconomical status. In 82.5% of samples, intracellular gram-negative diplococci were found, that made the presumptive diagnosis of gonococcal urethritis. The importance of direct examination of urethral secretion for the diagnosis and immediate treatment of gonococcal urethritis is emphasized.

From 1979 to 1988, 3890 positive samples for dermatophytes obtained at the Dermatology Service of Hospital San Juan de Dios (Western Santiago area) were analyzed for type of fungus involved and for relative yield of direct examination compared to culture. Direct examination was performed on samples treated with 20% KOH and stained with rhodamine. Cultures were obtained on Agar Sabouraud medium with addition of gentamycin and chloramphenicol. Yield of direct examination was 97.2% compared to 85% for culture. According to anatomic location, M canis was isolated from nearly all cases of T capitis. In cases of T pedis, nail mycosis, crural and palmar infections the most frequent species were T rubrum and T mentagrophytes M canis and T rubrum were responsible for infections of the body.

Previous treatment of dermic lesions caused by brown recluse spider bite has given disappointing results. Cold, steroids and surgical measures are usually ineffective. Following encouraging results reported with the use of dapsone (4-4 diamine diphenyl sulphone) we treated 2 patients with severe dermic lesions. Both recovered satisfactorily. Thus, dapsone is an effective therapy for dermic lesions caused by brown recluse spider bite. The potential toxic effects of this sulphone derivative suggest that it should be used only in severe cases of the disease.

A 66 year old woman with a history suggesting a tick bite on the right arm developed a slowly expanding erythematosus lesion. Similar lesions appeared on the trunk and extremities in the following months. A skin biopsy showing spiral shaped agents and the clinical findings were compatible with erythema chronicum migrans. A course of penicillin resulted in complete resolution of the lesions. After a 2 year follow up period she remains free of rheumatic, neurologic or cardiac lesions of Lyme's disease.
