This study verified the occurrence of dental sensitivity in patients submitted to a 35% hydrogen peroxide based product (Whiteness HP Maxx 35% - FGM), skin cold sensation threshold (SCST) and its influence on dental sensitivity. Sixty volunteers were divided into 4 groups (n = 15), according to SCST (low: GI and GIII, and high: GII and IV) and bleaching treatment (hydrogen peroxide: GI and GII, and placebo: GIII and GIV). SCST was determined in the inner forearm for 6 different times using a neurosensory analyzer, the TSA II (Medoc Advanced Medical Systems, Ramat Yishai, Northern District, Israel). Dental sensitivity measurements were performed 10 different times using a thermal stimulus and an intraoral device attached to TSA II, positioned in the buccal surface of the upper right central incisor. Spontaneous dental sensitivity was also determined using the Visual Analogue Scale (VAS). Data were submitted to Student's t-test and Pearson's Correlation Test (α=0.05). SCST remained the same during bleaching treatment. Distinct responses of dental sensitivity were found in patients with low and high SCST during the first and third bleaching session (p≤0.05). The teeth submitted to the bleaching treatment became more sensitive to cold than those treated with placebo. Moreover, data obtained with TSA and VAS presented moderate correlation. Bleaching treatment increased dental sensitivity and skin cold sensation threshold might represent a determining factor in this occurrence, since low and high SCST patients had different responses to the thermal stimulus in the teeth.

OBJECTIVES: The aim of this study was to investigate the knowledge and attitudes of orthodontists in the diagnosis and management of migraine without aura. MATERIAL AND METHODS: Participants were dentists, recruited among members of the Brazilian Association of Orthodontics and Facial Orthopedics (ABOR). An e-mail was sent to all ABOR members, with a link to a website, especially prepared for this research. Dentists were presented to a report of a fictional patient fulfilling diagnostic criteria for a primary headache disorder, known as migraine without aura. Participants were asked to describe how they would relieve the patient's pain. Professional procedures were classified as "adequate" or "inadequate" according to the answers given. RESULTS: 161 valid answers were received (18.8 percent response rate). Of them, 36 percent of the actions were considered to be "adequate" procedures, while 64 percent were "inadequate". The results yielded 12 main procedures, based on common characteristics. Eighty-two orthodontists suggested orthodontic treatment with or without orthognathic surgery, and some suggested using stabilization appliances prior to the orthodontic treatment. CONCLUSIONS: The majority of participants proposed inadequate therapies, and 51 percent suggested orthodontic correction of occlusion, including orthognathic surgery. Educational activities on migraine should also target orthodontists.

AIM: This study aims to present both the features of 124 consecutive patients with temporomandibular disorders (TMDs) and the results of a reversible, conservative, and low-tech treatment. METHODS AND MATERIALS: One hundred fifty-eight records of consecutive patients treated in School of Dentistry at the Univag-University Center in Várzea Grande-MT, Brazil, 124 of whom were considered TMD patients, were examined. The following data were obtained: gender, age, main complaint, diagnosis, co-morbidities, type of treatment performed, and treatment results. The patients who received a successful treatment were contacted for reevaluation four to six years after the conclusion of treatment. RESULTS: Pain was the main complaint for 92.7 percent of the patients. The majority of patients were female (female:male ratio of 4.1:1), with a peak age range between 20 and 30 years. Roughly 59.7 percent of the patients were diagnosed as having a muscular TMD, 12.9 percent as an articular TMD, and 27.4 percent as a mixed TMD. The success rate for treatment was 91.7 percent, and there was a tendency toward the long-term maintenance of good results. CONCLUSION: The features of the 124 TMD patients treated were similar to those reported in the literature with regard to gender, age, and diagnostic prevalence. Most of the disorders were of a muscular origin, and there was a predominance of women between 20 and 30 years of age. The conservative, reversible, and low-tech treatment success rate for TMD can reach values above 90 percent. Therefore, there is no need for invasive, irreversible, expensive, or high-tech treatments for the majority of patients. CLINICAL SIGNIFICANCE: The majority of TMD patients can benefit from reversible, conservative, and low-tech treatments such as parafunction control and therapeutic exercises that can be performed by any clinician once an accurate diagnosis has been made.
