Commercial swine producers are responsible for implementing changes in their production systems to ensure animal welfare. The objective of this study is to evaluate the levels of animal welfare on rearing and fattening farms certified in good animal husbandry practices using the Welfare Quality® protocol. Eighty-one certified commercial farms (n = 2,150 pigs), distributed in six geographical areas of Colombia, were evaluatedby six trained veterinarians. . A multilevel analysis was performed to identify variables associated with the animal welfare/farm rating and to identify behavioral variables. Farms evaluated had an enhanced (n = 68, 83.9%), followed by excellent (n = 7, 8.7%) and acceptable (n = 6, 7.4%) level of animal welfare. Variables significantly associated with the overall protocol rating (R2 = 0.37) were temperature comfort (OR: 1.05, p = 0.008), absence of disease (OR: 1.06, p = 0.02) and absence of pain (OR: 1.06, p = 0.0001). Significant differences in behavioral measures were observed across farms (P < 0.05). The high animal welfare scores of certified farms suggest changes in producers' understanding and attitudes towards production and animal welfare.

Flowers are the complex and highly diverse reproductive structures of angiosperms. Because of their role in sexual reproduction, the evolution of flowers is tightly linked to angiosperm speciation and diversification. Accordingly, the quantification of floral morphological diversity (disparity) among angiosperm subgroups and through time may give important insights into the evolutionary history of angiosperms as a whole. Based on a comprehensive dataset focusing on 30 characters describing floral structure across angiosperms, we used 1201 extant and 121 fossil flowers to measure floral disparity and explore patterns of floral evolution through time and across lineages. We found that angiosperms reached their highest floral disparity in the Early Cretaceous. However, decreasing disparity toward the present likely has not precluded the innovation of other complex traits at other morphological levels, which likely played a key role in the outstanding angiosperm species richness. Angiosperms occupy specific regions of the theoretical morphospace, indicating that only a portion of the possible floral trait combinations is observed in nature. The ANA grade, the magnoliids, and the early-eudicot grade occupy large areas of the morphospace (higher disparity), whereas nested groups occupy narrower regions (lower disparity).

Introducción: la fibrosis quística (FQ) es una enfermedad genética, multisistémica, caracterizada por la disfunción de las glándulas de secreción exocrina secundaria a la mutación de una proteína transmembrana que actúa como canal de cloro. La inclusión de su búsqueda en el Sistema Nacional de Pesquisa Neonatal (SNPN) determinó un gran impacto sobre la morbimortalidad de esta enfermedad, permitiendo un diagnóstico y tratamiento precoz. El íleo meconial (IM) es la manifestación más precoz de la enfermedad, ocurriendo en un 10% a 18% de los pacientes. Objetivo: describir una forma de presentación precoz de FQ con resultado de pesquisa neonatal normal. Caso clínico: 18 meses. Sexo femenino; 33 semanas de edad gestacional. Retraso en la expulsión del meconio. Al tercer día de vida íleo meconial con oclusión intestinal, vólvulo de intestino medio y necrosis intestinal. Se realiza resección quirúrgica. Tripsina inmunorreactiva (TIR) normal a los 7 y 23 días de vida. No se solicita proteína asociada a la pancreatitis (PAP). Desnutrición crónica, bronquiolitis grave a los 3 meses, neumonías virales a los 7 y 11 meses. Ingreso con diagnóstico de neumonía aguda comunitaria con insuficiencia respiratoria. Test del sudor alterado en dos oportunidades. Elastasa pancreática insuficiente. Se aísla Staphylococcus aureus y Pseudomonas aeruginosa en exudado nasal y de secreciones respiratorias. Estudio genético: mutación Delta F508. Con diagnóstico de FQ se inició abordaje multidisciplinario, tratamiento y derivación al centro especializado. Conclusiones: la existencia de IM puede dar falsos negativos en el valor de la TIR, siendo necesaria la dosificación de la PAP. El pronóstico de estos pacientes va a depender de un diagnóstico precoz y el manejo terapéutico oportuno en centros especializados.


Introduction: cystic fibrosis (CF) is a genetic, multisystemic disease, characterized by dysfunction of the exocrine secretion glands secondary to the mutation of a transmembrane protein that acts as a chloride channel. Including its research by the National Neonatal Screening System (SNPN) caused a great impact on the morbidity and mortality of this disease, enabling early diagnosis and treatment. Meconium ileus (MI) is the earliest manifestation of the disease, occurring in 10% to 18% of patients. Objective: to describe a form of early presentation of CF with normal neonatal screening results. Clinical case: 18 months. Female, 33 weeks gestational age. Delay in the expulsion of meconium. On the third day of life, meconium ileus with intestinal obstruction, midgut volvulus and intestinal necrosis. Surgical resection is performed. Trypsin immunoreactive (TIR) normal at 7 and 23 days of age. Pancreatitis-associated protein (PAP) is not requested. Chronic malnutrition, severe bronchiolitis at 3 months, viral pneumonia at 7 and 11 months. Admission with a diagnosis of acute community pneumonia with respiratory failure. Sweat test altered on two occasions. Insufficient pancreatic elastase. Staphylococcus aureus and Pseudomonas aeruginosa were isolated in nasal exudate and respiratory secretions. Genetic study: Delta F508 mutation. With the CF diagnosis, a multidisciplinary approach, treatment and referral to a specialized center began. Conclusions: the existence of MI can provide false negatives in the IRR value, making PAP dosage necessary. The prognosis of these patients will depend on early diagnosis and timely therapeutic management at specialized centers.


Introdução: a fibrose cística (FC) é uma doença genética, multissistêmica, caracterizada pela disfunção das glândulas secretoras exócrinas secundária à mutação de uma proteína transmembrana que age como canal de cloreto. A inclusão da sua pesquisa no Sistema Nacional de Triagem Neonatal (SNPN) determinou grande impacto na morbimortalidade desta doença, permitindo diagnóstico e tratamento precoces. O íleo meconial (IM) é a manifestação mais precoce da doença, ocorrendo em 10 a 18% dos pacientes. Objetivo: descrever uma forma de apresentação precoce da FC com resultados normais de triagem neonatal. Caso clínico: 18 meses. Sexo feminino. 33 semanas de idade gestacional. Atraso na expulsão do mecônio. No terceiro dia de vida, íleo meconial com obstrução intestinal, vólvulo de intestino médio e necrose intestinal. A ressecção cirúrgica é realizada. Imunorreativo à tripsina (TIR) normal aos 7 e 23 dias de vida. A proteína associada à pancreatite (PAP) não é solicitada. Desnutrição crônica, bronquiolite grave aos 3 meses, pneumonia viral aos 7 e11 meses. Internação com diagnóstico de pneumonia comunitária aguda com insuficiência respiratória. Teste do suor alterado em duas ocasiões. Elastase pancreática insuficiente. Staphylococcus aureus e Pseudomonas aeruginosa foram isolados em exsudato nasal e secreções respiratórias. Estudo genético: mutação Delta F508. Com o diagnóstico de FC iniciou-se abordagem multidisciplinar, tratamento e encaminhamento para centro especializado. Conclusões: a existência de IM pode dar falsos negativos no valor da TIR, tornando necessária a dosagem de PAP. O prognóstico destes pacientes dependerá do diagnóstico precoce e do manejo terapêutico oportuno em centros especializados.

The genus Passiflora comprises more than 500 species distributed in tropical and semitropical regions. With a great diversity of species, it is estimated that one-third is found in Colombian territory. Besides the food importance, Passiflora species are important sources of biologically active compounds, such as flavonoids. The most important symbiosis between soil fungi and vascular plants related to plant nutrition and tolerance to stress conditions is mycorrhizae. Passiflora species form arbuscular mycorrhizae, with several species of Glomeromycota. This association has been reported to alter the production of secondary metabolites. Thus, the objective of this study was to determine the relation between flavonoid content, mycorrhization, and soil nutritional content of Passiflora alata, Passiflora quadrangularis, Passiflora maliformis, and Passiflora ligularis in Colombian crops. The extracts were prepared and analyzed using UPLC/PDA-MS, and total flavonoids were quantified with the method of AlCl3. Soil characteristics, including nutritional content and percentage of colonization by arbuscular mycorrhizal fungi, were also determined. All variables were analyzed using Spearman's correlation and principal component analysis. Chromatographic analysis of the extracts allowed us to visualize the different flavonoid compositions of each extract, identifying several C-glycosylflavonoids. In this paper, we report for the first time the presence of luteolin-8-C-rhamnosyl-4'-O-glucoside, apigenin-6-C-arabinosyl-7-O-glucoside, and orientin for P. maliformis. Statistical analysis showed a negative correlation between available phosphorus (ρ = -0.90, p = <0.05) and magnesium (Mg) saturation (ρ = -0.70, p = <0.05) on flavonoid content, whereas the calcium magnesium (Ca/Mg) ratio was positively correlated (ρ = 0.70, p = <0.01). There was a nonsignificant correlation between mycorrhization and flavonoid content (ρ = -0.70, p = >0.1). These results contribute to understanding the relationship between flavonoid-mycorrhization-soil nutritional content on Passiflora spp.

The antifungal activity of palindromic peptide RWQWRWQWR and its derivatives was evaluated against clinical isolates of Candida albicans and C. auris. Also, Bidens pilosa ethanolic extracts of leaves and stem were evaluated. Furthermore, combinations of peptide, extract, and/or fluconazole (FLC) were evaluated. The cytotoxicity of peptides and extracts in erythrocytes and fibroblasts was determined. The original palindromic peptide, some derivative peptides, and the ethanolic extract of leaves of B. pilosa exhibited the highest activity in some of the strains evaluated. Synergy was obtained between the peptide and the FLC against C. auris 435. The combination of the extract and the original palindromic peptide against C. albicans SC5314, C. auris 435, and C. auris 537 decreased the minimal inhibitory concentrations (MICs) by a factor of between 4 and 16. These mixtures induced changes in cell morphology, such as deformations on the cell surface. The results suggest that the combination of RWQWRWQWR and B. pilosa extract is an alternative for enhancing antifungal activity and decreasing cytotoxicity and costs and should be considered to be a promising strategy for treating diseases caused by Candida spp.

Abstract Introduction Academic performance in medical students can be influenced by several factors, including those related to mental health and family relationships. Objective To examine the factors affecting academic performance in medical students, specifically considering potential diagnoses of depression. Method A survey was conducted among 747 fourth-year medical students. The survey included questions on sociodemographic variables, mental health, and well-being. The Patient Health Questionnaire (PHQ) was utilized, encompassing sections on depression, anxiety, panic, eating habits, alcohol consumption, and the Stress Perception Scale. Academic performance was assessed based on students' Grade Point Average (GPA). Descriptive statistics, Pearson correlation coefficients, and a linear regression model were employed for data analysis. Results The study revealed several variables significantly associated with GPA. Age (r = -.388), financial situation (r = .241), relationships with cohabitants (r = .165), and relationships with peers (r = .217) were found to have a correlation with academic performance. Additionally, repeating a course was found to be significantly associated with a person's GPA (r = .518) even after controlling for depression. Discussion and conclusion The findings indicate that robust mental health, a favorable financial situation, and positive interpersonal relationships are crucial for achieving optimal academic performance in medical students. These results emphasize the need to address mental health concerns, promote a supportive social environment, and provide financial assistance to enhance the educational outcomes of medical students.


Resumen Introducción El desempeño académico de los estudiantes de medicina puede verse influenciado por varios factores, entre ellos los relacionados con la salud mental y las relaciones familiares. Objetivo Examinar los factores que afectan el desempeño académico en estudiantes de medicina, considerando específicamente los posibles diagnósticos de depresión. Método Se realizó una encuesta entre 747 estudiantes de cuarto año de la carrera de medicina. La encuesta incluyó preguntas sobre variables sociodemográficas, salud mental y bienestar. Se utilizó el Cuestionario de Salud del Paciente (PHQ), que comprende secciones sobre depresión, ansiedad, pánico, hábitos alimentarios, consumo de alcohol y la Escala de Percepción del Estrés. El desempeño académico se evaluó con base en el promedio de calificaciones (GPA) de los estudiantes. Se emplearon estadísticas descriptivas, coeficientes de correlación de Pearson y un modelo de regresión lineal para el análisis de datos. Resultados El estudio reveló varias variables significativamente asociadas con el GPA. Se encontró que la edad (r = -.388), la situación financiera (r = .241), las relaciones con los convivientes (r = .165) y las relaciones con los compañeros (r = .217) tenían correlación con el rendimiento académico. Además, se encontró que repetir un curso estaba significativamente asociado con el GPA de una persona (r = .518) incluso después de controlar la depresión. Discusión y conclusión Los hallazgos indican que una salud mental sólida, una situación financiera favorable y relaciones interpersonales positivas son cruciales para lograr un desempeño académico óptimo en los estudiantes de medicina. Estos resultados enfatizan la necesidad de abordar los problemas de salud mental, promover un entorno social de apoyo y brindar asistencia financiera para mejorar los resultados educativos de los estudiantes de medicina.

Fossils are essential to infer past evolutionary processes. The assignment of fossils to extant clades has traditionally relied on morphological similarity and on apomorphies shared with extant taxa. The use of explicit phylogenetic analyses to establish fossil affinities has so far remained limited. In this study, we built a comprehensive framework to investigate the phylogenetic placement of 24 exceptionally preserved fossil flowers. For this, we assembled a new species-level data set of 30 floral traits for 1201 extant species that were sampled to capture the stem and crown nodes of all angiosperm families. We explored multiple analytical approaches to integrate the fossils into the phylogeny, including different phylogenetic estimation methods, topological-constrained analyses, and combining molecular and morphological data of extant and fossil species. Our results were widely consistent across approaches and showed minor differences in the support of fossils at different phylogenetic positions. The placement of some fossils agrees with previously suggested relationships, but for others, a new placement is inferred. We also identified fossils that are well supported within particular extant families, whereas others showed high phylogenetic uncertainty. Finally, we present recommendations for future analyses combining molecular and morphological evidence, regarding the selection of fossils and appropriate methodologies, and provide some perspectives on how to integrate fossils into the investigation of divergence times and the temporal evolution of morphological traits. Extrapulmonary tuberculosis is defined as that case of tuberculosis clinically diagnosed and confirmed by bacteriological studies that affects tissues and organs outside the lung parenchyma. Mexico is in third place among Latin American countries in terms of the incidence of pulmonary and extrapulmonary tuberculosis. Culture methods are still the gold standard for the diagnosis of extrapulmonary tuberculosis since they identify the species and susceptibility to drugs.


La tuberculosis extrapulmonar es aquella tuberculosis diagnosticada clínicamente y confirmada por estudios bacteriológicos que afecta a tejidos y órganos fuera del parénquima pulmonar. México es el tercer lugar en América Latina en incidencia de tuberculosis pulmonar y extrapulmonar. Los métodos de cultivo siguen siendo el método de referencia para el diagnóstico de tuberculosis extrapulmonar, ya que identifican la especie y la sensibilidad a los fármacos.

Resumen Introducción Las mujeres que se han dedicado al trabajo sexual han sido una comunidad estigmatizada, marginada y alienada socialmente. Particularmente, en el caso de las mujeres trans la situación ha sido peor, enfrentando mayores porcentajes de violencia. Objetivo Analizar la historia de vida de una mujer trans que se desempeña en el trabajo sexual, a partir de un análisis interseccional y ocupacional. Método Empleamos un estudio cualitativo basado en la técnica de investigación Historia de Vida, considerando elementos de la entrevista narrativa ocupacional. Resultados Desde su historia, podemos ver que hay elementos que han interseccionado a lo largo de su vida, tales como los dominios: estructural, disciplinar, hegemónico e interpersonal. Sus elecciones ocupacionales han estado fuertemente influenciadas por su contexto dentro de un apartheid ocupacional, buscando la generación de espacios de resistencia y agencia frente a las diferentes adversidades. Conclusiones Estas situaciones precarizan sus condiciones de vida y ponen en alerta las influencias de los sistemas económicos, políticos, heteronormados, entre otros, en la determinación de la vida de las personas.


Resumo Introdução As mulheres que se envolvem em trabalho sexual têm sido uma comunidade estigmatizada, marginalizada e socialmente alienada. Particularmente no caso das mulheres trans, há o enfrentamento de maiores porcentagens de violência. Objetivo Analisar a história de vida de uma mulher trans que atua no trabalho sexual, a partir de uma análise interseccional e ocupacional. Método Utilizou-se um estudo qualitativo baseado na técnica de pesquisa História de Vida, considerando elementos da entrevista narrativa ocupacional. Resultados A partir de sua história, podemos perceber que há elementos que se cruzaram ao longo de sua vida, como os domínios: estrutural, disciplinar, hegemônico e interpessoal. E que suas escolhas ocupacionais foram fortemente influenciadas por seu contexto dentro de um apartheid ocupacional, buscando gerar espaços de resistência e agenciamento diante de diferentes adversidades. Conclusões Essas situações tornam suas condições de vida precárias e alertam para as influências de sistemas econômicos, políticos, heteronormativos, entre outros, na determinação da vida das pessoas.


Abstract Introduction Women who have engaged in sex work have been a stigmatized, marginalized, and socially alienated community. Particularly, in the case of trans women, there is a bigger percentage of violence. Objective To analyze the life story of a trans woman who works in sex work, based on an intersectional and occupational analysis. Method We used a qualitative study based on the Life History research technique, considering elements of the occupational narrative interview. Results From her history, we can see that there are elements that have intersected throughout her life, such as the domains: structural, disciplinary, hegemonic, and interpersonal. And that her occupational choices have been strongly influenced by her context within occupational apartheid, seeking to generate spaces of resistance and agency in the face of different adversities. Conclusions These situations make her living conditions precarious and alert the influences of economic, political, and heteronormative systems, among others, in determining people's lives.

Pulmonary histoplasmosis is caused by inhaling Histoplasma capsulatum. Less than 1% develops the disease. Risk factors in immunocompetent individuals are environmental exposures in endemic areas. The objective of this study is to determine the frequency, clinical, and microbiological characteristics in immunocompetent patients. A retrospective case series study of patients diagnosed with pulmonary histoplasmosis was performed in a respiratory care unit in Mexico City from 2000 to 2020. Each patient had bronchial lavage, and three patients underwent thoracoscopy for the lung tissue sample taken for the culture in Sabouraud Dextrose Agar. Twelve patients were identified, 8 males and 4 females; the predominant symptoms were fever (83%), dyspnea (75%), chest pain (66%), hemoptysis (41%), and weight loss (33%). The computed tomography of the chest showed the following findings: patchy consolidation 12 (100%), hilar adenopathy 6 (50%), pleural effusion 6 (50%), caverns 3 (25%), and solitary pulmonary nodule in one patient (8%). Histoplasma capsulatum was found in the culture of all twelve patients. The signs and symptoms of the disease are mediated by the immune status of the host. The clinical picture is often confused with systemic diseases. It is important to have a high degree of clinical suspicion to make a timely diagnosis.

BACKGROUND: It has been demonstrated that dental restorations with rough surfaces can have several disadvantages such as pigment retention or plaque accumulation, which can facilitate caries formation, color variation, loss of brightness, degradation of restoration, among others. The present study aimed to assess surface roughness in bulk fill and conventional nanohybrid resins with and without polishing, controlling the oxygen inhibited layer. METHODS: This in vitro and longitudinal experimental study consisted of 120 resin blocks of 6 mm diameter and 4 mm depth, divided into two groups: Bulk Fill (Tetric® N-Ceram Bulk-fill, Opus Bulk Fill APS, Filtek™ Bulk Fill) and conventional nanohybrid (Tetric® N-Ceram, Opallis EA2, Filtek™ Z250 XT). Each resin group was divided into two equal parts, placing glycerin only on one of them, in order to control the oxygen inhibited layer. Subsequently, the surface roughness was measured before and after the polishing procedure with Sof-Lex discs. The data were analyzed with the T-test for related measures, and for comparison between groups before and after polishing, the non-parametric Kruskal Wallis test with the Bonferroni post hoc was used, considering a significance level of p < 0.05. RESULTS: Before polishing, the resin composites with the lowest surface roughness were Opus Bulk Fill APS (0.383 ± 0.186 µm) and Opallis EA2 (0.430 ± 0. 177 µm) with and without oxygen inhibited layer control, respectively; while after polishing, those with the lowest surface roughness were Opus Bulk Fill APS (0.213 ± 0.214 µm) and Tetric N-Ceram (0.097 ± 0.099 µm), with and without oxygen inhibited layer control, respectively. Furthermore, before and after polishing, all resins significantly decreased their surface roughness (p < 0.05) except Opus Bulk Fill APS resin with oxygen inhibited layer control (p = 0.125). However, when comparing this decrease among all groups, no significant differences were observed (p < 0.05). CONCLUSION: The Opus Bulk Fill APS resin with oxygen inhibited layer control presented lower surface roughness both before and after polishing, being these values similar at both times. However, after polishing the other bulk fill and conventional nanohybrid resins with and without oxygen inhibited layer control, the surface roughness decreased significantly in all groups, being this decrease similar in all of them.

BACKGROUND: After the detection of the first cases of SARS-CoV-2 and the declaration of a health emergency in the country, community transmission of the virus remained low to moderate until November 2020, later becoming medium and high. Children under 15 years of age constitute the least affected group. The diagnosis is confirmed by RT-qPCR. The usefulness of the CT (cycle threshold) is discussed. AIM: To describe the epidemiological and clinical characteristics of children infected with SARS-CoV-2 and the relationship between the CT value and clinical presentation. METHOD: A descriptive, retrospective study was carried out in patients under 16 years of age with SARS-CoV-2 infection, assisted by a health provider in Montevideo between epidemiological week (EW) 3 of2020 and EW 9 of2021. Clinical variables, epidemiological notion, severity and evolution were described. The CT value at diagnosis was recorded and compared with the presence of symptoms. RESULTS: Patients under 16 years of age represented 16.6% of the total infected (794 in 4,792). 70% of the cases occurred between the months of January and February 2021. 67.6% presented intradomiciliary contact. The mean age was 7.5 ± 4.1 years, 51% were male. 55% presented symptoms and 45% were asymptomatic; no significant differences were observed in the mean, mode and age range of both groups. None presented comorbidities. In the group of 440 symptomatic patients, 55% presented fever: 32% associated with other symptoms, mainly upper respiratory symptoms, and 23% as the only symptom. The CT value was obtained in 536 of the 794 patients (67.5%). The mean CT was 29.7+5.8 in the symptomatic group vs. 31.5 ± 5.7 in asymptomatic patients (p > 0.05). Three patients were hospitalized, none required intensive care and none died. CONCLUSIONS: The prevalence and incidence of SARS-CoV-2 infection in children accompany that of adults and the majority had mild or asymptomatic forms, without complications. In this series, the CT value was not different in symptomatic and asymptomatic children. Close household contact was the main source of infection.

Resumen Los niños y las niñas en edad escolar pueden pasar aproximadamente un 80 % de su jornada escolar haciendo uso de la silla. Debido a esta situación, diferentes estudios establecen una relación entre postura y problemas de salud. Señalan además, que las dificultades de escritura pueden afectar la percepción de las propias capacidades. Debido a estos resultados, se ha insistido en la importancia de inculcar hábitos posturales y grafomotrices adecuados desde edades tempranas. Los objetivos principales de este estudio fueron, por un lado, diseñar instrumentos de observación y recolección de datos de los patrones posturales y de grafomotricidad; por otro, validar dichos instrumentos mediante la opinión de personas expertas. Como objetivo secundario se consideró evaluar la consistencia interna del instrumento para valorar la postura de sedestación. En relación con el primer objetivo, ambos instrumentos mostraron valores altos de concordancia inter jueces en cuanto a la pertinencia y precisión de los ítems. Con respecto al segundo, los instrumentos fueron aplicados a una muestra de 93 niños y niñas de 1º y 5º curso de Educación primaria para evaluar la consistencia interna del instrumento postura. Los resultados de confiabilidad por dimensiones mostraron unos valores superiores a 0.7. Los instrumentos diseñados se presentan como herramientas útiles para valorar los patrones relacionados con la postura y la grafomotricidad y contribuir así a la detección de problemas asociados en niños y niñas en edad escolar. En conclusión, estos instrumentos pueden ofrecer directrices adecuadas para elaborar planes de trabajo destinados a atender las necesidades que presente el alumnado.


Abstract Schoolchildren can spend around 80% of their school day sat. In this sense, different studies establish a direct relationship between posture and health problems. Moreover, writing difficulties can also affect the perception of abilities, hence the importance of instilling proper postural and graphomotor habits from an early age. The main objectives of this study were, on the one hand, to design instruments for observation and data collection of postural and graphomotricity patterns and, on the other hand, to validate these instruments through expert opinion. A secondary objective was to evaluate the internal consistency of the instrument to assess the posture of sedestation. Concerning the first objective, both instruments showed high inter-judge concordance in terms of relevance and precision of the items. In relation to the second objective, the instruments were applied to a sample of 93 children from the 1st and 5th grades of primary school to evaluate their internal consistency of the posture instrument. Dimensional reliability results showed values greater than 0.7. The instruments designed are presented as useful tools to assess the patterns related to posture and graphomotricity and thus contribute to detecting problems associated with school-age children. In conclusion, these instruments can provide appropriate guidelines for the development of work plans to meet the needs of students.


Resumo As crianças em idade escolar podem passar aproximadamente 80% do seu dia escolar numa cadeira. Devido a esta situação, diferentes estudos estabelecem uma ligação entre a postura e os problemas de saúde. Eles também apontam que as dificuldades de escrita podem afetar a percepção das próprias habilidades. Devido a estes resultados, a importância de incutir hábitos postural e de caligrafia adequados desde uma idade precoce tem sido enfatizada. Os principais objetivos deste estudo foram, por um lado, conceber instrumentos de observação e recolha de dados de padrões posturais e grafomotores; por outro lado, validar estes instrumentos através da opinião de peritos. Um objetivo secundário foi avaliar a consistência interna do instrumento para provar a postura sentada. Em relação ao primeiro objetivo, ambos os instrumentos mostraram altos valores de concordância entre juízes em termos de relevância e precisão dos itens. Em relação ao segundo objetivo, os instrumentos foram administrados a uma amostra de 93 crianças do 1º e 5º anos do ensino fundamental para avaliar a consistência interna do instrumento de postura. Os resultados da confiabilidade por dimensões mostraram valores acima de 0,7. Os instrumentos desenhados são apresentados como ferramentas úteis para avaliar a postura e os padrões grafomotores e assim contribuir para a detecção de problemas associados em crianças em idade escolar. Em conclusão, estes instrumentos podem fornecer orientações apropriadas para o desenvolvimento de planos de trabalho que respondam às necessidades estudantis.

INTRODUCTION: Pulmonary aspergilloma is commonly associated with comorbidities that cause immunodeficiency such as diabetes mellitus, tuberculosis, human immunodeficiency virus/acquired immunodeficiency syndrome and/or a pre-existing parenchymal lung disease such as chronic obstructive pulmonary disease. Predisposing factors can further increase the risk of acquiring this mycosis. Our objective was to determine the frequency, clinical and microbiological characteristics of pulmonary aspergilloma in immunocompromised patients. METHODOLOGY: Retrospective case series of patients diagnosed with pulmonary aspergilloma in a respiratory care unit in Mexico City from 2000 to 2019 was studied. Bronchoalveolar lavage cultures on Sabouraud-dextrose agar and serum galactomannan determination were performed on each patient. RESULTS: We identified twenty-four patients with pulmonary aspergilloma (sixteen male and eight female), thirteen had a history of tuberculosis (54%), seven of diabetes mellitus (29%), three of human immunodeficiency virus/acquired immunodeficiency syndrome (13%) and one of chronic obstructive pulmonary disease (4%). The most commonly reported symptoms were hemoptysis in eighteen patients (75%), dyspnea in sixteen patients (67%) and chest pain in thirteen patients (54%). Aspergillus fumigatus was identified in all cultures and galactomannan was positive in 21 serum samples (87%). CONCLUSIONS: Coexistence of diseases that could suppress the immune system predispose to pulmonary aspergilloma; clinical presentation is often confused with other systemic diseases. A high degree of clinical suspicion is important for early detection.

BACKGROUND AND AIMS: Transient global amnesia (TGA) is a clinical syndrome characterized by sudden anterograde amnesia not accompanied by other neurological symptoms. There is no consensus on the underlying pathophysiological mechanism. However, diffusion-weighted imaging (DWI) of the magnetic resonance imaging (MRI) has demonstrated hippocampal lesions in as many as 50% of cases. This paper describes a series of patients with TGA and hippocampal lesions. METHODS: This study assessed vascular risk factors in patients older than age 18 admitted to the Hospital Universitario San Ignacio, Bogota, Colombia, from May 2017 to June 2020 with a diagnosis of TGA and evidence of hippocampal ischemic lesion on 3 Tesla brain MRI. RESULTS: The authors identified 36 patients, 72.2% female, with mean age 62 years. Cardiovascular risk factors, most frequently high blood pressure, carotid disease, and dyslipidemia, were present in 75% of these patients. Hippocampal lesions were unilateral in 80% of cases, with median size 2.5 mm, most frequently located at the hippocampal body. Approximately 14% of patients also presented acute ischemic lesions in locations other than the hippocampus. CONCLUSIONS: TGA is a clinical entity previously considered to have undetermined etiology. The present study used brain MRI to identify a group of patients with hippocampal ischemic lesions, finding associated vascular risk factors in a high proportion of them.

INTRODUCCIÓN: Tras la detección de los primeros casos de SARS-CoV-2 y declararse la emergencia sanitaria en el país, la transmisión comunitaria del virus se mantuvo baja a moderada hasta noviembre de 2020 pasando luego a ser media y alta. Los niños bajo 15 años de edad constituyen el grupo menos afectado. El diagnóstico se confirma mediante RT-qPCR. La utilidad del CT (cycle threshold) es discutida. Objetivo: Describir las características epidemiológicas y clínicas de los niños infectados con SARS-CoV-2 y la relación entre el valor del CT y la presentación clínica. METODOLOGÍA: Se realizó un estudio descriptivo, retrospectivo, en pacientes bajo 16 años de edad con infección por SARS-CoV-2, asistidos en un prestador de salud de Montevideo entre la semana epidemiológica (S.E.) 3 del 2020 y la SE 9 del 2021. Se describieron variables clínicas, noción epidemiológica, gravedad y evolución. Se consignó el valor del CT al diagnóstico y se comparó con la presencia de síntomas. RESULTADOS: Los pacientes bajo 16 años representaron el 16,6% del total de infectados (794 en 4.792). El 70% de los casos ocurrió entre los meses de enero y febrero de 2021. El 67,6% presentó contacto intradomiciliario. La media de edad fue 7,5 ± 4,1 años, 51% de sexo masculino. Presentaron síntomas 55% y fueron asintomáticos 45%; no se observaron diferencias significativas en la media, moda y rango de edades de ambos grupos. Ninguno presentó comorbilidades. En el grupo de los 440 pacientes sintomáticos, 55% presentaron fiebre: 32% asociada con otros síntomas, principalmente respiratorios altos y 23% como único síntoma. El valor de CT se obtuvo en 536 de los 794 pacientes (67,5%). La media de CT fue 29,7 ± 5,8 en el grupo de sintomáticos vs. 31,5 ± 5,7 en asintomáticos (p > 0,05). Se hospitalizaron tres pacientes, ninguno requirió cuidados intensivos ni falleció. CONCLUSIONES: La prevalencia e incidencia de la infección por SARS-CoV-2 en niños acompañan a la de adultos y la mayoría cursó formas leves o asintomáticas, sin complicaciones. En esta serie, el valor de CT no fue diferente en niños sintomáticos y asintomáticos. El contacto estrecho intradomiciliario fue la principal fuente de infección.

RESUMEN: Objetivo: Determinar las características epidemiológicas, clínicas y familiares de los pacientes con trastornos mentales adscritos a una unidad de medicina familiar (UMF) mexicana. Material y métodos: Estudio prospectivo, transversal, observacional, descriptivo, sobre 164 pacientes adscritos a una UMF mexicana, elegidos por muestreo no probabilístico que cumplieron criterios de selección: mayores de 18 años, con trastorno mental, sin impedimento físico o cognitivo. Resultados: Se presentaron 51 hombres y 113 mujeres con edad promedio de 48,6 años; 57% con más de un trastorno mental; los más frecuentes fueron depresión y ansiedad. En escolaridad sobresale licenciatura; 16% reportaron intento de suicidio, 42% tiene más de 6 años diagnosticado. El 64,6% recibe psicoterapia y farmacología; el 18% fue hospitalizado a causa de sus trastornos, gran parte de ellos entre 1 y 4 semanas y 38% tiene algún familiar con algún trastorno mental, Las comorbilidades más frecuentes fueron Hipertensión, obesidad, diabetes mellitus, dislipidemia y artrosis. Los tipos de familia más frecuentes son: nuclear o nuclear simple, con núcleo integrado y núcleo no integrado; subsisten principalmente de los servicios; viven con niveles 1 y 2 de pobreza familiar; son modernas y urbanas en mayor número. Conclusiones: las características sociodemográficas, clínicas y familiares de los pacientes con trastornos mentales adscritos a una unidad de medicina familiar (UMF) mexicana son en su mayoría mujeres, en adultez media, que cursan con más de un trastorno mental (ansiedad y depresión), tratadas con fármacos y psicoterapia, además cursaban con hipertensión, y pertenecen a familias de tipo nuclear simple, de núcleo integrado y núcleo no; subsisten por servicios; viven con niveles 1 y 2 de pobreza familiar, son modernas y urbanas.


ABSTRACT: Objective: To determine the epidemiological, clinical and family characteristics of patients with mental disorders assigned to a Mexican family medicine unit (FMU). Material and methods: Prospective, cross-sectional, observational, descriptive study of 164 patients assigned to a Mexican FMU, chosen by non-probabilistic evidence that they met the selection criteria: older than 18 years, with mental disorder, without physical or cognitive impairment. Results: 51 men and 113 women with a mean age of 48.6 years were presented; 57% with more than one mental disorder; the most frequent were depression and anxiety. In schooling overselling bachelor's degree; 16% reported attempted suicide, 42% have been diagnosed for more than 6 years. 64.6% receive psychotherapy and pharmacology; 18% were hospitalized due to their disorders, most of them between 1 and 4 weeks and 38% have a family member with a mental disorder. The most frequent comorbidities were hypertension, obesity, diabetes mellitus, dyslipidemia and osteoarthritis. The most frequent types of family are: nuclear or simple nuclear, with integrated nucleus and non-integrated nucleus; they subsist mainly on services; they live with levels 1 and 2 of family poverty; are modern and urban in greater number. Conclusions: the epidemiological, clinical and family characteristics of patients with mental disorders assigned to a Mexican family medicine unit (FMU) are mostly women, in middle adulthood, who attend more than a mental disorder (anxiety and depression), treated with drugs and psychotherapy, also had hypertension, and belong to families of simple nuclear type and non-integrated nucleus; subsist on services; they live with levels 1 and 2 of family poverty, they are modern and urban.

El objetivo de este estudio es conocer la efectividad del gel de clorhexidina y pasta de hidróxido de calcio con clorhexidina como medicamento intracanal en la eliminación del Enterococcus faecalis (E. faecalis). Se realizó una revisión bibliográfica mediante una búsqueda exhaustiva en diferentes bases de datos como: PubMed, ScienceDirect, SciELO, Springer, Google Scholar. Se seleccionaron mediante los criterios de inclusión y exclusión 37 artículos publicados en el periodo 2003-2020, los cuales fueron enfocados especialmente en E. faecalis y medicación intracanal. Teniendo como resultados que la clorhexidina en mayor porcentaje resultó ser el medicamento intracanal más eficaz en un tiempo promedio de 7 días, seguida de la combinación de hidróxido de calcio más clorhexidina que actuó de igual manera eliminando el E. faecalis de los canales radiculares, mientras que el hidróxido de calcio no presentó actividad antimicrobiana frente al E. faecalis.


The aim of this study was to know the effectiveness of chlorhexidine gel and calcium hydroxide paste with chlorhexidine as intracanal medicaments in Enterococcus faecalis(E. faecalis) elimination. A bibliographic review was done through an exhaustive search in different databases such as: PubMed, ScienceDirect, Scielo, Springer, Google Scholar. Using inclusion and exclusion criteria, 37 articles published in the period 2003-2020 were selected, which were especially focused on E. faecalis and intracanal medication. The analysis showed that chlorhexidine in a higher percentage was the most effective intracanal medication in an average time of 7 days, followed by the combination of calcium hydroxide plus chlorhexidine that acted in the same way eliminating E. faecalisfrom the root canals, while calcium hydroxide did not show any antimicrobial activity against E. faecalis.
